6e6d6932a3 rt2800usb: handle TX status timeouts
The watchdog just triggers rt2800usb_work_txdone() when it
detects a TX status timeout, thus rt2800usb_work_txdone() needs to
handle this case.

75256f0348 rt2x00: fix queue timeout checks
Add a timestamp to each queue entry which is updated whenever
the status of the entry changes, and remove the per-queue
timestamps.  The previous check was incorrect and caused both
false positives and false negatives.

With the corrected check it comes apparent that the TX status
usually times out on rt2800usb unless there is sufficient traffic
(i.e. the next TX will complete the previous TX status).

0e0d39e5f3 rt2800usb: read TX_STA_FIFO asynchronously
Trying to fix the "TX status report missed" warnings
by reading the TX_STA_FIFO entries as quickly as possible.
The TX_STA_FIFO is too small in hardware, thus reading
it only from the workqueue is too slow and entries get lost.

Start an asynchronous read of the TX_STA_FIFO directly from
the TX URB completion callback (atomic context, thus it cannot
use the blocking rt2800_register_read()). If the async
read returns a valid FIFO entry, it is pushed into a larger
FIFO inside struct rt2x00_dev, until rt2800_txdone() picks
it up.

A .tx_dma_done callback is added to struct rt2x00lib_ops
to trigger the async read from the URB completion callback.

15a533c47f rt2x00: Use correct TBTT_SYNC config in AP mode
This seems to fix problems with some powersaving clients since a
positive value in TBTT_SYNC_CFG_TBTT_ADJUST introduces beacon skew,
which is not wanted in AP mode.

Also update the rest of the TBTT_SYNC config according to the
legacy drivers in AP mode.

10e11568ca rt2x00: Make rt2x00_queue_entry_for_each more flexible
Allow passing a void pointer to rt2x00_queue_entry_for_each which in
turn in provided to the callback function.

Furthermore, allow the callback function to stop processing by returning
true. And also notify the caller of rt2x00_queue_entry_for_each if the
loop was canceled by the callback.

No functional changes, just preparation for an upcoming patch.

7dab73b37f rt2x00: Split rt2x00dev->flags
The number of flags defined for the rt2x00dev->flags field,
has been growing over the years. Currently we are approaching
the maximum number of bits which are available in the field.

A secondary problem, is that one part of the field are initialized only
during boot, because the driver requirements are initialized or device
requirements are loaded from the EEPROM. In both cases, the flags are
fixed and will not change during device operation. The other flags are
the device state, and will change frequently. So far this resulted in the fact
that for some flags, the atomic bit accessors are used, while for the others
the non-atomic variants are used.

By splitting the flags up into a "flags" and "cap_flags" we can put all flags
which are fixed inside "cap_flags". This field can then be read non-atomically.
In the "flags" field we keep the device state, which is going to be read atomically.

This adds more room for more flags in the future, and sanitizes the field access methods.

62fe778412 rt2x00: Fix stuck queue in tx failure case
Since commit 0b7fde54f9 "rt2x00: Protect
queue control with mutex" rt2x00 used rt2x00queue_pause_queue for
stopping a tx queue in mac80211. But in case of a failure in the tx
path rt2x00 still called ieee80211_stop_queue which stopped the queue
but prevented rt2x00queue_unpause_queue to wake the queue up again
resulting in a stuck tx queue.

Fix this by also using rt2x00queue_pause_queue in case of tx failures.

44704e5d7d rt2x00: Enable WLAN LED on Ralink SoC (rt305x) devices
This patch adds WLAN LED support to the mac80211 rt2x00 driver for
Ralink SoC (rt305x) devices.  The current WLAN LED drivers in
rt2800lib.c set the LED brightness via an MCU request, but do nothing
for SoC.  This patch checks for SoC and sets the register to enable the
WLAN LED (instead of an MCU request).  This enables the WLAN LED for
RT305x devices.

6a4c499e86 rt2x00: Add an error message when trying to send on a full queue
We already tell mac80211 to stop the queue when we hit a certain
threshold. Hence, it shouldn't happen at all that a frame gets queued
for tx on a full queue. Add an error message for this case.

f16d2db704 rt2x00: Fix tx aggregation problems with some clients
Some clients seem to rely upon the reception of BlockAckReqs to flush
their rx reorder buffer. In order to fix aggregation for these clients
rt2x00 should send a BlockAckReq if the transmission of an AMPDU
subframe fails.

Introduce a new flag TXDONE_AMPDU to indicate that this is an AMPDU
subframe and pass IEEE80211_TX_STAT_AMPDU_NO_BACK to mac80211 if an
AMPDU subframe failed during transmission.

This fixes aggregation problems with Intel 5100 Windows STAs (and maybe
others as well).

351151e8ac rt2x00: Fix STBC transmissions to STAs with Rx STBC > 1
For STBC transmissions rt2x00 used the number of RxSTBC streams the
destination STA indicates in its HT capabilities as STBC value in
the TXWI.

However, the legacy drivers and our own comment in rt2800.h suggest
that the STBC field in the TXWI only allows a value of 0 or 1.
The values 2 and 3 are reserved (probably for future devices).

And indeed, STBC transmissions to STAs indicating more then 1
RxSTBC stream fail when the STBC field is set to something >1.

Fix this by only setting the STBC field to 1 when STBC should be
used.

9e33a35538 rt2x00: Implement tx power temperature compensation
rt2800 devices should adjust their tx power in accordance with the
eeproms temperature calibration values. Add a new driver callback
gain_calibration that is called every 4 seconds.

The rt2800 gain calibration routine simply runs the tx power
configuration that takes care of calculating the temperature
compensation delta.

We don't need to synchronize the calls to rt2800_config_txpower
as they should all happen from mac80211's single threaded workqueue.

2f2bb7e8bd rt2x00: Remove obsolete rt2x00queue_align_payload
Since commit d1c3a37cee ("mac80211:
clarify alignment docs, fix up alignment") removed the requirement
for a 4-byte aligned payload rt2x00queue_align_payload is obsolete
as mac80211 will align the payload when it passes the frame to the
net stack.

As a result we can remove the call to rt2x00queue_align_payload in the
rx path and since that's the last user we can remove
rt2x00queue_align_payload altogether.

One advantage is that we save some alignment operations for frames
that don't need to be aligned (for example beause they are not passed
to the net stack).

2af242e19f rt2x00: Don't recalculate HT40 compensation for each rate
Previously the HT40 tx power compensation value was calculated for each
rate. However, the calculation is independent of the tx rate and as such
can be precalculated and just passed in for each rate.

f78987cf8b rt2x00: Calculate tx status fifo size instead of hardcoding it
Instead of hardcoding the tx status fifo size as 512 calculate it based
on the number of tx queues and the number of entries per queue. Also
round the size up to a power of 2 as kfifo would otherwise round it
down.

On rt2800pci this will increase the kfifo size from 512 bytes to 1024
bytes which is then able to hold the tx status for all entries in all
tx queues.

Furthermore, if the number of tx queues or tx entries changes in the
future (use of the MGMT queue for example) the kfifo size doesn't need
to be updated.

In the spirit of optimizing the code to get the queue structure of TX queues,
also optimize the code to get beacon queues. We can simply use the bcn queue
field of the rt2x00_dev structure instead of using the rt2x00queue_get_queue
function.

61c6e4893f rt2x00: Include ATIM queue support in rt2x00queue_get_tx_queue.
The ATIM queue is considered to be a TX queue by the drivers that support
the queue. Therefore include support for the ATIM queue to the
rt2x00queue_get_tx_queue function so that the drivers that support the ATIM
queue can also use that function.

Add the support in such a way that drivers that do not support the ATIM
queue are not penalized in their efficiency.

1ed3811c33 rt2x00: Fix rt2800 key assignment in multi bssid setups
When setting up multiple BSSIDs in AP mode on an rt2800pci device we
previously used the STAs AID to select an appropriate key slot. But
since the AID is per VIF we can end up with two STAs having the same AID
and thus using the same key index. This resulted in one STA overwriting
the key information of another STA.

Fix this by simply searching for the next unused entry in the pairwise
key table.

Also bring the key table init in sync with deleting keys by initializing
the key table entries to 0 instead of 1.

55b585e290 rt2x00: Don't call ieee80211_get_tx_rate for MCS rates
ieee80211_get_tx_rate is not valid for HT rates. Hence, restructure the
TX desciptor creation to be aware of MCS rates. The generic TX desciptor
creation now cares about the rate_mode (CCK, OFDM, MCS, GF).

As a result, ieee80211_get_tx_rate gets only called for legacy rates.

HT and no-HT rt2x00 devices use a partly different TX descriptor.
Optimize the tx desciptor memory layout by putting the PLCP and HT
substructs into a union and introduce a new driver flag to decide which
TX desciptor format is used by the device.

This saves us the expensive PLCP calculation fOr HT devices and the HT
descriptor setup on no-HT devices.

Newer devices like rt2800* own a hardware sequence counter and thus
don't need to use a software sequence counter at all. Add a new driver
flag to shortcut the software sequence number generation on devices that
don't need it.

rt61pci, rt73usb and rt2800* seem to make use of a hw sequence counter
while rt2400pci and rt2500* need to do it in software.

11f818e0eb rt2x00: Optimize calls to rt2x00queue_get_queue
In some cases (tx path for example) we don't need to check for non-tx
queues in rt2x00queue_get_queue. Hence, introduce a new method
rt2x00queue_get_tx_queue that is only valid for tx queues and use it in
places where only tx queues are valid.

Furthermore, this new method is quite short and as such can be inlined
to avoid the function call overhead.

This only converts the txdone functions of drivers that don't use an ATIM
queue and the generic tx path.

7bb4568372 mac80211: make tx() operation return void
The return value of the tx operation is commonly
misused by drivers, leading to errors. All drivers
will drop frames if they fail to TX the frame, and
they must also properly manage the queues (if they
didn't, mac80211 would already warn).

Removing the ability for drivers to return a BUSY
value also allows significant cleanups of the TX
TX handling code in mac80211.

Note that this also fixes a bug in ath9k_htc, the
old "return -1" there was wrong.

The TX/RX work structures must be able to run independently
of other workqueues. This is because mac80211 might use
the flush() callback function from various context, which depends
on the TX/RX work to complete while the main thread is blocked
(until the the TX queues are empty).

This should reduce the number of  'Queue %d failed to flush' warnings.

a0aff623ca rt2x00: Fix FIXME comments in rt61pci and rt73usb on Michael MIC.
Both rt61pci and rt73usb check the Michael MIC in hardware and strip the
Michael MIC from received frames. This is perfectly allowed by mac80211 as
long as this is properly reported to mac80211.
Both these drivers reported the Michael MIC handling properly to mac80211, but
still contained a FIXME comment on this, which is not needed to be handled, since
mac80211 doesn't really need the Michael MIC in this case.

bcf3cfd047 rt2x00: Convert rt2400pci interrupt handling to use tasklets
Fix interrupt processing on slow machines by using individual tasklets
for each different device interrupt. This ensures that while a RX or TX
status tasklet is scheduled only the according device interrupt is
masked and other interrupts such as TBTT can still be processed.

Also, this allows us to use tasklet_hi_schedule for TBTT processing
which is required to not send out beacons with a wrong DTIM count (due
to delayed periodic beacon updates). Furthermore, this improves the
latency between the TBTT and sending out buffered multi- and broadcast
traffic.

As a nice bonus, the interrupt handling overhead should be much lower.

Compile-tested only.

Fix interrupt processing on slow machines by using individual tasklets
for each different device interrupt. This ensures that while a RX or TX
status tasklet is scheduled only the according device interrupt is
masked and other interrupts such as TBTT can still be processed.

Also, this allows us to use tasklet_hi_schedule for TBTT processing
which is required to not send out beacons with a wrong DTIM count (due
to delayed periodic beacon updates). Furthermore, this improves the
latency between the TBTT and sending out buffered multi- and broadcast
traffic.

As a nice bonus, the interrupt handling overhead should be much lower.

1dae8d342e rt2x00: Limit beacon updates in bss_info_changed to USB devices
Currently there are two places that trigger a beacon update on PCI
devices. The bss_info_changed callback and the periodic update
triggered by the TBTT or PRETBTT interrupt.

Since the next TBTT or PRETBTT interrupt will periodically fetch an
updated beacon remove the update_beacon call in the bss_info_changed
callback for PCI devices.

In the worst case it will take one beacon interval longer to fetch
the new beacon then before. For devices that have a PRETBTT interrupt
there should be no change at all.

69cf36a452 rt2x00: Refactor beacon code to make use of start- and stop_queue
This patch allows to dynamically remove beaconing interfaces without
shutting beaconing down on all interfaces.

The only place to start and stop beaconing are now the start- and
stop_queue callbacks. Hence, we can remove some register writes during
interface bring up (config_intf) and only write the correct sync mode
to the register there.

When multiple beaconing interfaces are present we should enable
beaconing as soon as mac80211 enables beaconing on at least one of
them. The beacon queue gets stopped when the last beaconing
interface was stopped by mac80211. Therefore, introduce another
interface counter to keep track ot the number of enabled beaconing
interfaces and start or stop the beacon queue accordingly.

To allow single interfaces to stop beaconing, add a new driver
callback clear_beacon to clear a single interface's beacon without
affecting the other interfaces. Don't overload the clear_entry callback
for clearing beacons as that would introduce additional overhead
(check for each TX queue) into the clear_entry callback which is used
on the drivers TX/RX hotpaths.

Furthermore, the write beacon callback doesn't need to enable beaconing
anymore but since beaconing should be disabled while a new beacon is
written or cleared we still disable beacon generation and enable it
afterwards again in the driver specific callbacks. However, beacon
related interrupts should not be disabled/enabled here, that's solely
done from the start- and stop queue callbacks. It would be nice to stop
the beacon queue just before the beacon update and enable it afterwards
in rt2x00queue itself instead of the current implementation that relies
on the driver doing the right thing. However, since start- and
stop_queue are mutex protected we cannot use them for atomic beacon
updates.

59eb21a650 cfg80211: Extend channel to frequency mapping for 802.11j
Extend channel to frequency mapping for 802.11j Japan 4.9GHz band, according to
IEEE802.11 section 17.3.8.3.2 and Annex J. Because there are now overlapping
channel numbers in the 2GHz and 5GHz band we can't map from channel to
frequency without knowing the band. This is no problem as in most contexts we
know the band. In places where we don't know the band (and WEXT compatibility)
we assume the 2GHz band for channels below 14.

This patch does not implement all channel to frequency mappings defined in
802.11, it's just an extension for 802.11j 20MHz channels. 5MHz and 10MHz
channels as well as 802.11y channels have been omitted.

The following drivers have been updated to reflect the API changes:
iwl-3945, iwl-agn, iwmc3200wifi, libertas, mwl8k, rt2x00, wl1251, wl12xx.
The drivers have been compile-tested only.

0b01f030d3 mac80211: track receiver's aggregation reorder buffer size
The aggregation code currently doesn't implement the
buffer size negotiation. It will always request a max
buffer size (which is fine, if a little pointless, as
the mac80211 code doesn't know and might just use 0
instead), but if the peer requests a smaller size it
isn't possible to honour this request.

In order to fix this, look at the buffer size in the
addBA response frame, keep track of it and pass it to
the driver in the ampdu_action callback when called
with the IEEE80211_AMPDU_TX_OPERATIONAL action. That
way the driver can limit the number of subframes in
aggregates appropriately.

Note that this doesn't fix any drivers apart from the
addition of the new argument -- they all need to be
updated separately to use this variable!

412b31334b rt2x00: Fix firmware loading regression on x86_64.
Commit 6175ddf06b changes the way
memcpy_toio() works for x86_64, causing firmware loading to fail for
some Ralink WLAN devices with the rt2800pci driver since linux 2.6.34.
This causes the log message: "phy0 -> rt2800pci_load_firmware: Error -
PBF system register not ready.".

Fix this by using __iowrite32_copy instead of memcpy_toio().

f615e9a38a rt2x00: Fix WMM Queue naming
The Queue names were incorrectly copied from the legacy drivers,
as a result the queue names were inversed to what was expected.

This renames the queues using this mapping:
	QID_AC_BK -> QID_AC_VO (priority 0)
	QID_AC_BE -> QID_AC_VI (priority 1)
	QID_AC_VI -> QID_AC_BE (priority 2)
	QID_AC_VO -> QID_AC_BK (priority 3)

Note that this was a naming problem only, which didn't affect
the assignment of frames to their respective queues.

Add the rt2x00_dmastart function to rt2x00lib which
marks the queue_entry as "owned by device", and increased
the Q_INDEX number.

This cleanups up the index handling by rt2x00lib which
at until so far used hackish approaches to keep the
RX queue index numbering sane.

As part of the queue refactoring, change the queue callback
function names to have 3 different actions: start, kick & stop.

We can now also remove the STATE_RADIO_RX_ON/STATE_RADIO_RX_OFF
device_state flags, and replace the usage with using the
start_queue/stop_queue callback functions.
This streamlines the RX queue handling to the
similar approach as all other queues.

5450b7e2f0 rt2x00: Introduce 3 queue commands in drivers (start, kick, stop).
As part of the queue refactoring, we now introduce
3 queue commands: start, kick, stop.

 - Start: will enable a queue, for TX this will
   not mean anything, while for beacons and RX
   this will update the registers to enable the queue.
 - Kick: This will kick all pending frames to
   the hardware. This is needed for the TX queue
   to push all frames to the HW after the queue
   has been started
 - Stop: This will stop the queue in the hardware,
   and cancel any pending work (So this doesn't
   mean the queue is empty after a stop!).

Move all code from the drivers into the appropriate
functions, and link those calls to the old rt2x00lib
callback functions (we will fix this later when we
refactor the queue control inside rt2x00lib).

89b25f60e0 rt2x00: Don't frequently reset beacon interval in AdHoc mode
Commit 0204464329 "Check for specific changed
flags when updating the erp config" changed the way in which a new beacon
interval gets handled. However, due to a bug in rt2800usb and rt2800pci the
beacon interval was reset during each scan, thus causing problems in AdHoc
mode.

Fix this by not cleaning up the beacon interval when killing the beacon queue
but just prevent the device from sending out beacons.

a061a93b6e rt2x00: Ensure TX-ed frames are returned in the original state.
Recent changes to the TX-done code of rt2x00 resulted in TX-ed frames not
being returned to mac80211 in the original state, and therefore with
insufficient headroom for re-transmissions.

Fix this by reverting the changes done and by ensuring we remove the inserted
L2pad by moving the header backwards instead of the data forwards.

At the same time also make sure that the rt2x00queue_remove_l2pad will not
move any memory when a frame has no data at all.

All rt2x00 drivers except rt2800pci call ieee80211_tx_status() from
a workqueue, which causes "NOHZ: local_softirq_pending 08" messages.

To fix it, add ieee80211_tx_status_ni() similar to ieee80211_rx_ni()
which can be called from process context, and call it from
rt2x00lib_txdone().  For the rt2800pci special case a driver
flag is introduced.

rt2800usb chips need to add 1~3 bytes zero padding after each 802.11 header & payload,
and at the end need to add 4 bytes zero padding whether doing TX bulk aggregation or not,

TXINFO_W0_USB_DMA_TX_PKT_LEN in TXINFO must include 1-3 bytes padding after 802.11 header & payload
but do not include 4 bytes end zero padding.

In rt2800usb_get_tx_data_len do not consider multiple of the USB packet size case, sometimes this will
cause USB DMA problem.

For some hardware the REGISTER_BUSY_COUNT isn't sufficient,
increase the REGISTER_BUSY_COUNT to 100 to catch most
devices which have more problems with accessing the registers.

For normal operating devices nothing would change as they will
exit the loop early anyway.

8c5765fda4 rt2x00: Add watchdog functions for HW queue
Add watchdog functions for managing the Queues inside the hardware.
Normally the driver doesn't have much to do with these queues
directly, but the Ralink drivers did implement watchdog functions
for these. These watchdog functions are not triggered that often,
compared to the other watchdog functions, but I have at least
seen them trigger once or twice during a long stresstest run.

fa8b4b22d5 rt2x00: Fix hw crypto in AP mode for some devices
The BSSID register shouldn't be set in AP mode on some older devices (like
rt73usb) as it breaks hw crypto on these. However, rt2800 devices explicitly
need the BSSID register set to the same value as our own MAC address (only
in AP mode).

Hence, don't set the BSSID from rt2x00lib but move it down into rt2800 to
avoid problems on older devices.

This fixes a regression (at least for rt73usb) and avoids a new regression
for rt2800 devices in 2.6.36.

By not scheduling the TX/RX completion worker threads
when Radio is disabled, or hardware has been unplugged,
the queues cannot be completely cleaned.

This causes crashes when the hardware has been unplugged while
the radio is still enabled.

f44df18c58 rt2x00: Implement flush callback
Implement a basic flush callback function, which simply loops
over all TX queues and waits until all frames have been transmitted
and the status reports have been gathered.

At this moment we don't support dropping any frames during the
flush, but mac80211 will only send 'false' for this argument anyway,
so this is not important at this time.

When the TX status handler failed to clear the queue
in rt2x00usb_watchdog_tx_dma() we shouldn't use a failsave
to use the rt2x00usb txdone handler.

If a driver has overriden the txdone handler it must make
sure the txdone handler is capable of cleaning up the queue itself.

2b23cdaa3b rt2x00: Reduce tx descriptor size
The tx descriptor values qid, cw_min, cw_max and aifs are directly
accessible through the tx entry struct. So there's no need to copy
them into the tx descriptor and passing them to the indiviual drivers.
Instead we can just get the correct value from the tx entry.

efd2f271e4 rt2x00: Sync Tx and RX ring sizes with legacy drivers
All rt2x00 devices used the same Tx and Rx ring size (24 entries) till
now. Newer devices (like rt2800) can however make use of a larger TX and
RX ring due to 11n capabilities (AMPDUs of size 64 for example).

Hence, bring rt2x00 in sync with the legacy drivers and use the same TX
and RX ring sizes. Also remove the global defines RX_ENTRIES, TX_ENTRIES,
BEACON_ENTRIES and ATIM_ENTRIES and use per driver values.

That is 24 entries for rt2400pci, 32 entries for rt2500pci, rt2500usb,
rt61pci and rt73usb and 128 (RX) and 64 (TX) for rt2800pci and rt2800usb.

5ada28bf76 led-class: always implement blinking
Currently, blinking LEDs can be awkward because it is not guaranteed that
all LEDs implement blinking.  The trigger that wants it to blink then
needs to implement its own timer solution.

Rather than require that, add led_blink_set() API that triggers can use.
This function will attempt to use hw blinking, but if that fails
implements a timer for it.  To stop blinking again, brightness_set() also
needs to be wrapped into API that will stop the software blink.

As a result of this, the timer trigger becomes a very trivial one, and
hopefully we can finally see triggers using blinking as well because it's
always easy to use.

6038f373a3 llseek: automatically add .llseek fop
All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.

The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.

New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time.  Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.

The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.

Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

a13c8f3133 rt2x00: Fix URB error handling
kill_urb guarentees that when the function returns, the URB has
been fully killed. This means we don't need the extra sleeping
after the call to kill_urb.

kill_urb can however also guarentee the submit_urb to fail, as
a result, we must catch the return value from submit_urb an
correctly mark the entry as owned by the driver, and the
status as broken.

cdfd2c5cff rt2x00: Move watchdog work to kernel work_queue
The watchdog function must run on a work_queue
which is independent of any other work inside rt2x00.

The main reasons, being that a broken work on the mac80211
work_queue can otherwise prevent the watchdog to run (while
in fact the watchdog could fix the issue). And on the other
hand because the watchdog relies on the completion of the
completion handlers for RX/TX which for the USB case, occur
on the mac80211 workqueue.

This fixes some "Queue %d failed to flush" errors, which were
caused by the watchdog function waiting on the completion
handler which was scheduled to run right after the watchdog.

fa69560f31 rt2x00: Simplify Queue function arguments
A lot of functions accept a struct rt2x00_dev combined with
either a struct queue_entry or struct data_queue argument.
This can be simplified by only passing on the queue/entry
argument.

In cases where rt2x00_dev and a sk_buff are send together,
we can send the queue_entry instead.

rt2x00usb_alloc_urb and rt2x00usb_free_urb have a bit
of vague naming. Instead they allocate all the data which
belongs to a rt2x00 data queue entry.

The patches "rt2x00: Improve TX status entry validation" and "rt2x00: rework tx
status handling in rt2800pci" together were causing problems with tx status
processing in rt2800pci:

phy1 -> rt2800pci_txdone: Warning - Got TX status for an empty queue 3, dropping
phy1 -> rt2800pci_txdone: Warning - Got TX status for an unavailable queue 7, dropping

Fix this by using the correct field definition for getting the QID out of the
tx status report.

b34793ee27 rt2x00: Work around hw aggregation oddity in rt2800
If a frame is not meant to be sent as AMPDU or part of it the hw might
still decide to aggregate this frame if a previous frame started an
AMPDU. However, this will limit the usefulness of the reported tx rate
since the reported rate will be the one specified in the TXWI of the
first frame and thus it is not possible to reliably caculate the
number of retrys by substracting the reported tx rate from the tx rate
in the TXWI.

To fix this issue, only report the successful rate for frames that were
not meant to be aggregated but ended up in an aggregate.

Example:
Frame A (MCS7, AMPDU=1) B (MCS7, AMPDU=1) C (MCS12, AMDPU=0, PROBE_RATE)

Although frame C shoudn't be aggregated the hw might sill put it
into an AMPDU together with A and B. If the transmission succeeds the tx
status will contain MCS7 for all three frames. In that case we should
only report MCS7 as success rate and avoid reporting MCS12-MCS8 as
failed tx attempts as this will affect the future rate control
decisions.

This oddity might strike us in other scenarious as well but the most
common "wrong" report happened for frames used to probe a different tx
rate.

This improves the rate control decisions notable.

01e946f22a rt2x00: Improve cooperation between rt2800pci and minstrel
In order to lower the impact of probe rates don't send a frame as AMPDU
if the rate control algorithm sets IEEE80211_TX_CTL_RATE_CTRL_PROBE.
Otherwise a whole aggregate would be send with a probe rate which might
lead to numerous retries.

1f0280cb35 rt2x00: Fix oops caused by error path in rt2x00lib_start
When rt2x00lib_enable_radio fails to enable the radio, rt2x00lib_start
will call rt2x00queue_uninitialize to uninitialize the queues. Since,
the queues are not initialized here but already in rt2x00lib_initialize
we shouldn't uninitialize the queues here. Otherwise, a consecutive call
to rt2x00lib_start will oops (see below) because it assumes the queues
are already initialized.

74ee3802c0 rt2x00: Update comment about the AMPDU flag in the TXWI
During testing with AMPDUs it turned out that the rt2800 hw will aggregate
consecutive frames with the same RA and TID when the first frame in a
possible aggregate has set AMPDU=1 in the TXWI. If a following frame has
set AMPDU=0 in its TXWI it might sill end up in the aggregate of the
previous frame. Update the comment accordingly.

bc8a979e2d rt2x00: Improve TX status entry validation
The TX_STA_FIFO contains some information for identifying
a outgoing frame, however matching by WCID and ACK status is
not sufficient to 100% identify the macthing queue_entry structure
(containing the SKB buffer) which belongs to the status report.

Within TX_STA_FIFO we have a 4-bit field named PACKETID, which is
currently used to encode the queue id. The queue ID is however
limited to values from 0 to 3, which means 2 bits are sufficient
to encode the value. With the remaining 2 bits we can encode a
partial queue_entry index number. The value of PACKETID is not
allowed to become 0, with the queue ID ranging from 0 to 3, at least
one of the bits for the entry identification must be 1.

That leaves us with 3 possible values we can still encode in the
bits. Altough this doesn't allow 100% accurate matching of the
TX_STA_FIFO queue to a queue_entry structure, it at least improves
the accuracy. This allows us to better detect if we have missed the
TX_STA_FIFO report, which in turn reduces the number of watchdog
warnings regarding the TX status timeout.
