6fec21449a KVM: x86: use correct page table format to check nested page table reserved bits
Intel CPUID on AMD host or vice versa is a weird case, but it can
happen.  Handle it by checking the host CPU vendor instead of the
guest's in reset_tdp_shadow_zero_bits_mask.  For speed, the
check uses the fact that Intel EPT has an X (executable) bit while
AMD NPT has NX.

29ecd66019 KVM: x86: avoid uninitialized variable warning
This does not show up on all compiler versions, so it sneaked into the
first 4.3 pull request.  The fix is to mimic the logic of the "print
sptes" loop in the "fill array" loop.  Then leaf and root can be
both initialized unconditionally.

Note that "leaf" now points to the first unused element of the array,
not the last filled element.

47ab875169 KVM: MMU: fully check zero bits for sptes
The #PF with PFEC.RSV = 1 is designed to speed MMIO emulation, however,
it is possible that the RSV #PF is caused by real BUG by mis-configure
shadow page table entries

This patch enables full check for the zero bits on shadow page table
entries (which includes not only bits reserved by the hardware, but also
bits that will never be set in the SPTE), then dump the shadow page table
hierarchy.

c258b62b26 KVM: MMU: introduce the framework to check zero bits on sptes
We have abstracted the data struct and functions which are used to check
reserved bit on guest page tables, now we extend the logic to check
zero bits on shadow page tables

The zero bits on sptes include not only reserved bits on hardware but also
the bits that SPTEs willnever use.  For example, shadow pages will never
use GB pages unless the guest uses them too.

We got the bug that qemu complained with "KVM: unknown exit, hardware
reason 31" and KVM shown these info:

This is because we got a mmio #PF and the handler see the mmio spte becomes
normal (points to the ram page)

However, this is valid after introducing fast mmio spte invalidation which
increases the generation-number instead of zapping mmio sptes, a example
is as follows:
1. QEMU drops mmio region by adding a new memslot
2. invalidate all mmio sptes
3.

        VCPU 0                        VCPU 1
    access the invalid mmio spte
                            access the region originally was MMIO before
                            set the spte to the normal ram map

    mmio #PF
    check the spte and see it becomes normal ram mapping !!!

This patch fixes the bug just by dropping the check in mmio handler, it's
good for backport. Full check will be introduced in later patches

d1fe921955 KVM: x86: reintroduce kvm_is_mmio_pfn
The call to get_mt_mask was really using kvm_is_reserved_pfn to
detect an MMIO-backed page.  In this case, we want "false" to be
returned for the zero page.

Reintroduce a separate kvm_is_mmio_pfn predicate for this use
only.

6a39bbc5da KVM: MTRR: do not map huge page for non-consistent range
Based on Intel's SDM, mapping huge page which do not have consistent
memory cache for each 4k page will cause undefined behavior

In order to avoiding this kind of undefined behavior, we force to use
4k pages under this case

699023e239 KVM: x86: add SMM to the MMU role, support SMRAM address space
This is now very simple to do.  The only interesting part is a simple
trick to find the right memslot in gfn_to_rmap, retrieving the address
space from the spte role word.  The same trick is used in the auditing
code.

The comment on top of union kvm_mmu_page_role has been stale forever,
so remove it.  Speaking of stale code, remove pad_for_nice_hex_output
too: it was splitting the "access" bitfield across two bytes and thus
had effectively turned into pad_for_ugly_hex_output.

A new function x86_set_memory_region (and the "slots_lock taken"

We need to hide SMRAM from guests not running in SMM.  Therefore,
all uses of kvm_read_guest* and kvm_write_guest* must be changed to
check whether the VCPU is in system management mode and use a
different set of memslots.  Switch from kvm_* to the newly-introduced
kvm_vcpu_*, which call into kvm_arch_vcpu_memslots_id.

and with the same auditing exception the level was coming from

f36f3f2846 KVM: add "new" argument to kvm_arch_commit_memory_region
This lets the function access the new memory slot without going through
kvm_memslots and id_to_memslot.  It will simplify the code when more
than one address space will be supported.

Unfortunately, the "const"ness of the new argument must be casted
away in two places.  Fixing KVM to accept const struct kvm_memory_slot
pointers would require modifications in pretty much all architectures,
and is left for later.

3520469d65 KVM: export __gfn_to_pfn_memslot, drop gfn_to_pfn_async
gfn_to_pfn_async is used in just one place, and because of x86-specific
treatment that place will need to look at the memory slot.  Hence inline
it into try_async_pf and export __gfn_to_pfn_memslot.

The patch also switches the subsequent call to gfn_to_pfn_prot to use
__gfn_to_pfn_memslot.  This is a small optimization.  Finally, remove
the now-unused async argument of __gfn_to_pfn.

efdfe536d8 KVM: MMU: fix MTRR update
Currently, whenever guest MTRR registers are changed
kvm_mmu_reset_context is called to switch to the new root shadow page
table, however, it's useless since:
1) the cache type is not cached into shadow page's attribute so that
   the original root shadow page will be reused

2) the cache type is set on the last spte, that means we should sync
   the last sptes when MTRR is changed

This patch fixs this issue by drop all the spte in the gfn range which
is being updated by MTRR

d69afbc6b1 KVM: MMU: fix decoding cache type from MTRR
There are some bugs in current get_mtrr_type();
1: bit 1 of mtrr_state->enabled is corresponding bit 11 of
   IA32_MTRR_DEF_TYPE MSR which completely control MTRR's enablement
   that means other bits are ignored if it is cleared

2: the fixed MTRR ranges are controlled by bit 0 of
   mtrr_state->enabled (bit 10 of IA32_MTRR_DEF_TYPE)

3: if MTRR is disabled, UC is applied to all of physical memory rather
   than mtrr_state->def_type

edc90b7dc4 KVM: MMU: fix SMAP virtualization
KVM may turn a user page to a kernel page when kernel writes a readonly
user page if CR0.WP = 1. This shadow page entry will be reused after
SMAP is enabled so that kernel is allowed to access this user page

Fix it by setting SMAP && !CR0.WP into shadow page's role and reset mmu
once CR4.SMAP is updated

decf63336e KVM: MMU: fix comment in kvm_mmu_zap_collapsible_spte
Soft mmu uses direct shadow page to fill guest large mapping with small
pages if huge mapping is disallowed on host. So zapping direct shadow
page works well both for soft mmu and hard mmu, it's just less widely
applicable.

Fix the comment to reflect this.

As Andres pointed out:

| I don't understand the value of this check here. Are we looking for a
| broken memslot? Shouldn't this be a BUG_ON? Is this the place to care
| about these things? npages is capped to KVM_MEM_MAX_NR_PAGES, i.e.
| 2^31. A 64 bit overflow would be caused by a gigantic gfn_start which
| would be trouble in many other ways.

This patch drops the memslot overflow check to make the codes more simple.

3ea3b7fa9a kvm: mmu: lazy collapse small sptes into large sptes
Dirty logging tracks sptes in 4k granularity, meaning that large sptes
have to be split.  If live migration is successful, the guest in the
source machine will be destroyed and large sptes will be created in the
destination. However, the guest continues to run in the source machine
(for example if live migration fails), small sptes will remain around
and cause bad performance.

This patch introduce lazy collapsing of small sptes into large sptes.
The rmap will be scanned in ioctl context when dirty logging is stopped,
dropping those sptes which can be collapsed into a single large-page spte.
Later page faults will create the large-page sptes.

88178fd4f7 KVM: x86: Add new dirty logging kvm_x86_ops for PML
This patch adds new kvm_x86_ops dirty logging hooks to enable/disable dirty
logging for particular memory slot, and to flush potentially logged dirty GPAs
before reporting slot->dirty_bitmap to userspace.

kvm x86 common code calls these hooks when they are available so PML logic can
be hidden to VMX specific. SVM won't be impacted as these hooks remain NULL
there.

1c91cad423 KVM: x86: Change parameter of kvm_mmu_slot_remove_write_access
This patch changes the second parameter of kvm_mmu_slot_remove_write_access from
'slot id' to 'struct kvm_memory_slot *' to align with kvm_x86_ops dirty logging
hooks, which will be introduced in further patch.

Better way is to change second parameter of kvm_arch_commit_memory_region from
'struct kvm_userspace_memory_region *' to 'struct kvm_memory_slot * new', but it
requires changes on other non-x86 ARCH too, so avoid it now.

9b51a63024 KVM: MMU: Explicitly set D-bit for writable spte.
This patch avoids unnecessary dirty GPA logging to PML buffer in EPT violation
path by setting D-bit manually prior to the occurrence of the write from guest.

We only set D-bit manually in set_spte, and leave fast_page_fault path
unchanged, as fast_page_fault is very unlikely to happen in case of PML.

For the hva <-> pa change case, the spte is updated to either read-only (host
pte is read-only) or be dropped (host pte is writeable), and both cases will be
handled by above changes, therefore no change is necessary.

f4b4b18086 KVM: MMU: Add mmu help functions to support PML
This patch adds new mmu layer functions to clear/set D-bit for memory slot, and
to write protect superpages for memory slot.

In case of PML, CPU logs the dirty GPA automatically to PML buffer when CPU
updates D-bit from 0 to 1, therefore we don't have to write protect 4K pages,
instead, we only need to clear D-bit in order to log that GPA.

For superpages, we still write protect it and let page fault code to handle
dirty page logging, as we still need to split superpage to 4K pages in PML.

As PML is always enabled during guest's lifetime, to eliminate unnecessary PML
GPA logging, we set D-bit manually for the slot with dirty logging disabled.

7e71a59b25 KVM: x86: flush TLB when D bit is manually changed.
When software changes D bit (either from 1 to 0, or 0 to 1), the
corresponding TLB entity in the hardware won't be updated immediately. We
should flush it to guarantee the consistence of D bit between TLB and
MMU page table in memory.  This is especially important when clearing
the D bit, since it may cause false negatives in reporting dirtiness.

Sanity test was done on my machine with Intel processor.

bf4bea8e9a kvm: fix kvm_is_mmio_pfn() and rename to kvm_is_reserved_pfn()
This reverts commit 85c8555ff0 ("KVM: check for !is_zero_pfn() in
kvm_is_mmio_pfn()") and renames the function to kvm_is_reserved_pfn.

The problem being addressed by the patch above was that some ARM code
based the memory mapping attributes of a pfn on the return value of
kvm_is_mmio_pfn(), whose name indeed suggests that such pfns should
be mapped as device memory.

However, kvm_is_mmio_pfn() doesn't do quite what it says on the tin,
and the existing non-ARM users were already using it in a way which
suggests that its name should probably have been 'kvm_is_reserved_pfn'
from the beginning, e.g., whether or not to call get_page/put_page on
it etc. This means that returning false for the zero page is a mistake
and the patch above should be reverted.

     percpu_ref is made independent of putting the base ref and a
     percpu_ref can now optionally be initialized in single or killed
     mode.  This allows avoiding percpu shutdown latency for cases where
     the refcounted objects may be synchronously created and destroyed
     in rapid succession with only a fraction of them reaching fully
     operational status (SCSI probing does this when combined with
     blk-mq support).  It's also planned to be used to implement forced
     single mode to detect underflow more timely for debugging.

  There's a separate branch percpu/for-3.18-consistent-ops which cleans
  up the duplicate percpu accessors.  That branch causes a number of

5712846808 kvm: Fix page ageing bugs
1. We were calling clear_flush_young_notify in unmap_one, but we are
within an mmu notifier invalidate range scope. The spte exists no more
(due to range_start) and the accessed bit info has already been
propagated (due to kvm_pfn_set_accessed). Simply call
clear_flush_young.

2. We clear_flush_young on a primary MMU PMD, but this may be mapped
as a collection of PTEs by the secondary MMU (e.g. during log-dirty).
This required expanding the interface of the clear_flush_young mmu
notifier, so a lot of code has been trivially touched.

3. In the absence of shadow_accessed_mask (e.g. EPT A bit), we emulate
the access bit by blowing the spte. This requires proper synchronizing
with MMU notifier consumers, like every other removal of spte's does.

908c7f1949 percpu_counter: add @gfp to percpu_counter_init()
Percpu allocator now supports allocation mask.  Add @gfp to
percpu_counter_init() so that !GFP_KERNEL allocation masks can be used
with percpu_counters too.

We could have left percpu_counter_init() alone and added
percpu_counter_init_gfp(); however, the number of users isn't that
high and introducing _gfp variants to all percpu data structures would
be quite ugly, so let's just do the conversion.  This is the one with
the most users.  Other percpu data structures are a lot easier to
convert.

This patch doesn't make any functional difference.

Currently, if a permission error happens during the translation of
the final GPA to HPA, walk_addr_generic returns 0 but does not fill
in walker->fault.  To avoid this, add an x86_exception* argument
to the translate_gpa function, and let it fill in walker->fault.
The nested_page_fault field will be true, since the walk_mmu is the
nested_mmu and translate_gpu instead operates on the "outer" (NPT)
instance.

The following events can lead to an incorrect KVM_EXIT_MMIO bubbling
up to userspace:

(1) Guest accesses gpa X without a memory slot. The gfn is cached in
struct kvm_vcpu_arch (mmio_gfn). On Intel EPT-enabled hosts, KVM sets
the SPTE write-execute-noread so that future accesses cause
EPT_MISCONFIGs.

(2) Host userspace creates a memory slot via KVM_SET_USER_MEMORY_REGION
covering the page just accessed.

(3) Guest attempts to read or write to gpa X again. On Intel, this
generates an EPT_MISCONFIG. The memory slot generation number that
was incremented in (2) would normally take care of this but we fast
path mmio faults through quickly_check_mmio_pf(), which only checks
the per-vcpu mmio cache. Since we hit the cache, KVM passes a
KVM_EXIT_MMIO up to userspace.

This patch fixes the issue by using the memslot generation number
to validate the mmio cache.

ee3d1570b5 kvm: fix potentially corrupt mmio cache
vcpu exits and memslot mutations can run concurrently as long as the
vcpu does not aquire the slots mutex. Thus it is theoretically possible
for memslots to change underneath a vcpu that is handling an exit.

If we increment the memslot generation number again after
synchronize_srcu_expedited(), vcpus can safely cache memslot generation
without maintaining a single rcu_dereference through an entire vm exit.
And much of the x86/kvm code does not maintain a single rcu_dereference
of the current memslots during each exit.

We can prevent the following case:

By incrementing the generation after synchronizing with kvm->srcu readers,
we ensure that the generation retrieved in (6) will become invalid soon
after (8).

Keeping the existing increment is not strictly necessary, but we
do keep it and just move it for consistency from update_memslots to
install_new_memslots.  It invalidates old cached MMIOs immediately,
instead of having to wait for the end of synchronize_srcu_expedited,
which makes the code more clearly correct in case CPU 1 is preempted
right after synchronize_srcu() returns.

To avoid halving the generation space in SPTEs, always presume that the
low bit of the generation is zero when reconstructing a generation number
out of an SPTE.  This effectively disables MMIO caching in SPTEs during
the call to synchronize_srcu_expedited.  Using the low bit this way is
somewhat like a seqcount---where the protected thing is a cache, and
instead of retrying we can simply punt if we observe the low bit to be 1.

- we have marked large sptes readonly instead of dropping them that means we

  present to nonpresent will flush all the TLBs immediately), in other words,
  the only case we need to care is mmu_spte_update()

- in mmu_spte_update(), we haved checked
  SPTE_HOST_WRITEABLE | PTE_MMU_WRITEABLE instead of PT_WRITABLE_MASK, that
  means it does not depend on PT_WRITABLE_MASK anymore

lock. Note lockless write-protection only marks the writable spte to readonly

This patch is used to avoid this kind of race:

Currently, kvm zaps the large spte if write-protected is needed, the later
read can fault on that spte. Actually, we can make the large spte readonly
instead of making them un-present, the page fault caused by read access can
be avoided

The idea is from Avi:
| As I mentioned before, write-protecting a large spte is a good idea,
| since it moves some work from protect-time to fault-time, so it reduces
| jitter.  This removes the need for the return value.

This version has fixed the issue reported in 6b73a9606, the reason of that
issue is that fast_page_fault() directly sets the readonly large spte to
writable but only dirty the first page into the dirty-bitmap that means
other pages are missed. Fixed it by only the normal sptes (on the
PT_PAGE_TABLE_LEVEL level) can be fast fixed

Read-only large sptes can be created due to read-only faults as
follows:

- QEMU pagetable entry that maps guest memory is read-only
due to COW.
- Guest read faults such memory, COW is not broken, because
it is a read-only fault.
- Enable dirty logging, large spte not nuked because it is read-only.
- Write-fault on such memory causes guest to loop endlessly
(which must go down to level 1 because dirty logging is enabled).

Fix by dropping large spte when necessary.

e0ead41a6d KVM: async_pf: Provide additional direct page notification
By setting a Kconfig option, the architecture can control when
guest notifications will be presented by the apf backend.
There is the default batch mechanism, working as before, where the vcpu
thread should pull in this information.
Opposite to this, there is now the direct mechanism, that will push the
information to the guest.
This way s390 can use an already existing architecture interface.

Still the vcpu thread should call check_completion to cleanup leftovers.

Check for this possibility, let fault exception be regenerated.

The new_cr3 MMU callback has been a wrapper for mmu_free_roots since commit

The commit message mentioned that "mmu_free_roots() is somewhat of an overkill,
but fixing that is more complicated and will be done after this minimal fix".
One year has passed, and no one really felt the need to do a different fix.
Wrap the call with a kvm_mmu_new_cr3 function for clarity, but remove the
callback.

If we let L1 use EPT, we should probably also support the INVEPT instruction.

In our current nested EPT implementation, when L1 changes its EPT table
for L2 (i.e., EPT12), L0 modifies the shadow EPT table (EPT02), and in
the course of this modification already calls INVEPT. But if last level
of shadow page is unsync not all L1's changes to EPT12 are intercepted,
which means roots need to be synced when L1 calls INVEPT. Global INVEPT
should not be different since roots are synced by kvm_mmu_load() each
time EPTP02 changes.

KVM's existing shadow MMU code already supports nested TDP. To use it, we
need to set up a new "MMU context" for nested EPT, and create a few callbacks
for it (nested_ept_*()). This context should also use the EPT versions of
the page table access functions (defined in the previous patch).
Then, we need to switch back and forth between this nested context and the
regular MMU context when switching between L1 and L2 (when L1 runs this L2
with EPT).

37406aaaee nEPT: Add EPT tables support to paging_tmpl.h
This is the first patch in a series which adds nested EPT support to KVM's
nested VMX. Nested EPT means emulating EPT for an L1 guest so that L1 can use
EPT when running a nested guest L2. When L1 uses EPT, it allows the L2 guest
to set its own cr3 and take its own page faults without either of L0 or L1
getting involved. This often significanlty improves L2's performance over the
previous two alternatives (shadow page tables over EPT, and shadow page
tables over shadow page tables).

This patch adds EPT support to paging_tmpl.h.

paging_tmpl.h contains the code for reading and writing page tables. The code
for 32-bit and 64-bit tables is very similar, but not identical, so
paging_tmpl.h is #include'd twice in mmu.c, once with PTTTYPE=32 and once
with PTTYPE=64, and this generates the two sets of similar functions.

There are subtle but important differences between the format of EPT tables
and that of ordinary x86 64-bit page tables, so for nested EPT we need a
third set of functions to read the guest EPT table and to write the shadow
EPT table.

So this patch adds third PTTYPE, PTTYPE_EPT, which creates functions (prefixed
with "EPT") which correctly read and write EPT tables.

1c118b8226 KVM: MMU: avoid fast page fault fixing mmio page fault
Currently, fast page fault incorrectly tries to fix mmio page fault when
the generation number is invalid (spte.gen != kvm.gen).  It then returns
to guest to retry the fault since it sees the last spte is nonpresent.
This causes an infinite loop.

Since fast page fault only works for direct mmu, the issue exists when
1) tdp is enabled. It is only triggered only on AMD host since on Intel host
   the mmio page fault is recognized as ept-misconfig whose handler call
   fault-page path with error_code = 0

2) guest paging is disabled. Under this case, the issue is hardly discovered
   since paging disable is short-lived and the sptes will be invalid after
   memslot changed for 150 times

Fix it by filtering out MMIO page faults in page_fault_can_be_fast.

Without this information, users will just see unexpected performance
problems and there is little chance we will get good reports from them:
note that mmio generation is increased even when we just start, or stop,
dirty logging for some memory slot, in which case users cannot expect
all shadow pages to be zapped.

printk_ratelimited() is used for this taking into account the problems
that we can see the information many times when we start multiple VMs
and guests can trigger this by reading ROM in a loop for example.

This patch tries to introduce a very simple and scale way to invalidate
all mmio sptes - it need not walk any shadow pages and hold mmu-lock

KVM maintains a global mmio valid generation-number which is stored in
kvm->memslots.generation and every mmio spte stores the current global
generation-number into his available bits when it is created

When KVM need zap all mmio sptes, it just simply increase the global
generation-number. When guests do mmio access, KVM intercepts a MMIO #PF
then it walks the shadow page table and get the mmio spte. If the
generation-number on the spte does not equal the global generation-number,
it will go to the normal #PF handler to update the mmio spte

kvm_zap_obsolete_pages uses lock-break technique to zap pages,
it will flush tlb every time when it does lock-break

We can reload mmu on all vcpus after updating the generation
number so that the obsolete pages are not used on any vcpus,
after that we do not need to flush tlb when obsolete pages
are zapped

It will do kvm_mmu_prepare_zap_page many times and use one
kvm_mmu_commit_zap_page to collapse tlb flush, the side-effects
is that causes obsolete pages unlinked from active_list but leave
on hash-list, so we add the comment around the hash list walker

Note: kvm_mmu_commit_zap_page is still needed before free
the pages since other vcpus may be doing locklessly shadow
page walking

After the patch, kvm_zap_obsolete_pages can forward progress anyway,
so update the comments

[ It improves the case 0.6% ~ 1% that do kernel building meanwhile read
  PCI ROM. ]

Note: i am not sure that "10" is the best speculative value, i just
guessed that '10' can make vcpu do not spend long time on
kvm_zap_obsolete_pages and do not cause mmu-lock too hungry.

The current kvm_mmu_zap_all is really slow - it is holding mmu-lock to
walk and zap all shadow pages one by one, also it need to zap all guest
page's rmap and all shadow page's parent spte list. Particularly, things
become worse if guest uses more memory or vcpus. It is not good for
scalability

In this patch, we introduce a faster way to invalidate all shadow pages.
KVM maintains a global mmu invalid generation-number which is stored in
kvm->arch.mmu_valid_gen and every shadow page stores the current global
generation-number into sp->mmu_valid_gen when it is created

When KVM need zap all shadow pages sptes, it just simply increase the
global generation-number then reload root shadow pages on all vcpus.
Vcpu will create a new shadow page table according to current kvm's
generation-number. It ensures the old pages are not used any more.
Then the obsolete pages (sp->mmu_valid_gen != kvm->arch.mmu_valid_gen)
are zapped by using lock-break technique

This patch changes the name to reflect this meaning better; while doing
this renaming, the code in the wrapper function is inlined into the main
body since the whole function will be inlined into the only caller now.

7ddca7e43c KVM: MMU: Move kvm_mmu_free_some_pages() into kvm_mmu_alloc_page()
What this function is doing is to ensure that the number of shadow pages
does not exceed the maximum limit stored in n_max_mmu_pages: so this is
placed at every code path that can reach kvm_mmu_alloc_page().

Although it might have some sense to spread this function in each such
code path when it could be called before taking mmu_lock, the rule was
changed not to do so.

Taking this background into account, this patch moves it into
kvm_mmu_alloc_page() and simplifies the code.

Note: the unlikely hint in kvm_mmu_free_some_pages() guarantees that the
overhead of this function is almost zero except when we actually need to
allocate some shadow pages, so we do not need to care about calling it
multiple times in one path by doing kvm_mmu_get_page() a few times.

982b3394dd KVM: x86: Optimize mmio spte zapping when creating/moving memslot
When we create or move a memory slot, we need to zap mmio sptes.
Currently, zap_all() is used for this and this is causing two problems:
 - extra page faults after zapping mmu pages
 - long mmu_lock hold time during zapping mmu pages

For the latter, Marcelo reported a disastrous mmu_lock hold time during
hot-plug, which made the guest unresponsive for a long time.

This patch takes a simple way to fix these problems: do not zap mmu
pages unless they are marked mmio cached.  On our test box, this took
only 50us for the 4GB guest and we did not see ms of mmu_lock hold time
any more.

Note that we still need to do zap_all() for other cases.  So another
work is also needed: Xiao's work may be the one.

b67bfe0d42 hlist: drop the node parameter from iterators
I'm not sure why, but the hlist for each entry iterators were conceived

        list_for_each_entry(pos, head, member)

The hlist ones were greedy and wanted an extra parameter:

        hlist_for_each_entry(tpos, pos, head, member)

Why did they need an extra pos parameter? I'm not quite sure. Not only
they don't really need it, it also prevents the iterator from looking
exactly like the list iterator, which is unfortunate.

Besides the semantic patch, there was some manual work required:

 - Fix up the actual hlist iterators in linux/list.h
 - Fix up the declaration of other iterators based on the hlist ones.
 - A very small amount of places were using the 'node' parameter, this
 was modified to use 'obj->member' instead.
 - Coccinelle didn't handle the hlist_for_each_entry_safe iterator
 properly, so those had to be fixed up manually.

caf6900f2d KVM: MMU: lazily drop large spte
Currently, kvm zaps the large spte if write-protected is needed, the later
read can fault on that spte. Actually, we can make the large spte readonly
instead of making them not present, the page fault caused by read access can
be avoided

The idea is from Avi:
| As I mentioned before, write-protecting a large spte is a good idea,
| since it moves some work from protect-time to fault-time, so it reduces
| jitter.  This removes the need for the return value.

6b81b05e44 KVM: MMU: Conditionally reschedule when kvm_mmu_slot_remove_write_access() takes a long time
If the userspace starts dirty logging for a large slot, say 64GB of
memory, kvm_mmu_slot_remove_write_access() needs to hold mmu_lock for
a long time such as tens of milliseconds.  This patch controls the lock
hold time by asking the scheduler if we need to reschedule for others.

One penalty for this is that we need to flush TLBs before releasing
mmu_lock.  But since holding mmu_lock for a long time does affect not
only the guest, vCPU threads in other words, but also the host as a
whole, we should pay for that.

In practice, the cost will not be so high because we can protect a fair
amount of memory before being rescheduled: on my test environment,
cond_resched_lock() was called only once for protecting 12GB of memory
even without THP.  We can also revisit Avi's "unlocked TLB flush" work
later for completely suppressing extra TLB flushes if needed.

No reason to make callers take mmu_lock since we do not need to protect
kvm_mmu_change_mmu_pages() and kvm_mmu_slot_remove_write_access()
together by mmu_lock in kvm_arch_commit_memory_region(): the former
calls kvm_mmu_commit_zap_page() and flushes TLBs by itself.

Note: we do not need to protect kvm->arch.n_requested_mmu_pages by
mmu_lock as can be seen from the fact that it is read locklessly.

b99db1d352 KVM: MMU: Make kvm_mmu_slot_remove_write_access() rmap based
This makes it possible to release mmu_lock and reschedule conditionally
in a later patch.  Although this may increase the time needed to protect
the whole slot when we start dirty logging, the kernel should not allow
the userspace to trigger something that will hold a spinlock for such a
long time as tens of milliseconds: actually there is no limit since it
is roughly proportional to the number of guest pages.

Another point to note is that this patch removes the only user of
slot_bitmap which will cause some problems when we increase the number
of slots further.

7751babd3c KVM: MMU: fix infinite fault access retry
We have two issues in current code:
- if target gfn is used as its page table, guest will refault then kvm will use
  small page size to map it. We need two #PF to fix its shadow page table

- sometimes, say a exception is triggered during vm-exit caused by #PF
  (see handle_exception() in vmx.c), we remove all the shadow pages shadowed
  by the target gfn before go into page fault path, it will cause infinite
  loop:
  delete shadow pages shadowed by the gfn -> try to use large page size to map
  the gfn -> retry the access ->...

To fix these, we can adjust page size early if the target gfn is used as page
table

c22885050e KVM: MMU: fix Dirty bit missed if CR0.WP = 0
If the write-fault access is from supervisor and CR0.WP is not set on the
vcpu, kvm will fix it by adjusting pte access - it sets the W bit on pte
and clears U bit. This is the chance that kvm can change pte access from
readonly to writable

Unfortunately, the pte access is the access of 'direct' shadow page table,
means direct sp.role.access = pte_access, then we will create a writable
spte entry on the readonly shadow page table. It will cause Dirty bit is
not tracked when two guest ptes point to the same large page. Note, it
does not have other impact except Dirty bit since cr0.wp is encoded into
sp.role

It can be fixed by adjusting pte access before establishing shadow page
table. Also, after that, no mmu specified code exists in the common function
and drop two parameters in set_spte

c219346325 KVM: MMU: optimize for set_spte
There are two cases we need to adjust page size in set_spte:
1): the one is other vcpu creates new sp in the window between mapping_level()
    and acquiring mmu-lock.
2): the another case is the new sp is created by itself (page-fault path) when
    guest uses the target gfn as its page table.

In current code, set_spte drop the spte and emulate the access for these case,
it works not good:
- for the case 1, it may destroy the mapping established by other vcpu, and
  do expensive instruction emulation.
- for the case 2, it may emulate the access even if the guest is accessing
  the page which not used as page table. There is a example, 0~2M is used as
  huge page in guest, in this huge page, only page 3 used as page table, then
  guest read/writes on other pages can cause instruction emulation.

Both of these cases can be fixed by allowing guest to retry the access, it
will refault, then we can establish the mapping by using small page

After this patch,
- is_noslot_pfn indicates that the gfn is not in slot

  when translate the gfn to pfn
- is_error_noslot_pfn indicates that the pfn either it is error pfns or it
  is noslot pfn
And is_invalid_pfn can be removed, it makes the code more clean

97d64b7881 KVM: MMU: Optimize pte permission checks
walk_addr_generic() permission checks are a maze of branchy code, which is
performed four times per lookup.  It depends on the type of access, efer.nxe,
cr0.wp, cr4.smep, and in the near future, cr4.smap.

Optimize this away by precalculating all variants and storing them in a
bitmap.  The bitmap is recalculated when rarely-changing variables change
(cr0, cr4) and is indexed by the often-changing variables (page fault error
code, pte access permissions).

The permission check is moved to the end of the loop, otherwise an SMEP
fault could be reported as a false positive, when PDE.U=1 but PTE.U=0.
Noted by Xiao Guangrong.

The result is short, branch-free code.

8ea667f259 KVM: MMU: Push clean gpte write protection out of gpte_access()
gpte_access() computes the access permissions of a guest pte and also
write-protects clean gptes.  This is wrong when we are servicing a
write fault (since we'll be setting the dirty bit momentarily) but
correct when instantiating a speculative spte, or when servicing a
read fault (since we'll want to trap a following write in order to
set the dirty bit).

It doesn't seem to hurt in practice, but in order to make the code
readable, push the write protection out of gpte_access() and into
a new protect_clean_gpte() which is called explicitly when needed.

was correct, the real cause of that issue was a more trivial bug of
mmu_shrink() introduced by

  KVM: MMU: do not iterate over all VMs in mmu_shrink()

Here is the bug:

We skip VMs whose n_used_mmu_pages is not zero and try to shrink others:
in other words we try to shrink empty ones by mistake.

This patch reverses the logic so that mmu_shrink() can free pages from
the first VM whose n_used_mmu_pages is not zero.  Note that we also add
comments explaining the role of nr_to_scan which is not practically
important now, hoping this will be improved in the future.

 - x86 can integrate rmap and rmap_pde and remove heuristics in

This patch avoids these extra handler calls by changing the loop order
like this:

  for each level
    for each rmap in the range
      unmap using rmap

With the preceding patches in the patch series, this made THP page
invalidation more than 5 times faster on our x86 host: the host became
more responsive during swapping the guest's memory as a result.

b3ae209697 KVM: Introduce kvm_unmap_hva_range() for kvm_mmu_notifier_invalidate_range_start()
When we tested KVM under memory pressure, with THP enabled on the host,
we noticed that MMU notifier took a long time to invalidate huge pages.

Since the invalidation was done with mmu_lock held, it not only wasted
the CPU but also made the host harder to respond.

This patch mitigates this by using kvm_handle_hva_range().

84504ef386 KVM: MMU: Make kvm_handle_hva() handle range of addresses
When guest's memory is backed by THP pages, MMU notifier needs to call
kvm_unmap_hva(), which in turn leads to kvm_handle_hva(), in a loop to
invalidate a range of pages which constitute one huge page:

  for each page
    for each memslot
      if page is in memslot
        unmap using rmap

This means although every page in that range is expected to be found in
the same memslot, we are forced to check unrelated memslots many times.
If the guest has more memslots, the situation will become worse.

Furthermore, if the range does not include any pages in the guest's
memory, the loop over the pages will just consume extra time.

This patch, together with the following patches, solves this problem by
introducing kvm_handle_hva_range() which makes the loop look like this:

  for each memslot
    for each page in memslot
      unmap using rmap

In this new processing, the actual work is converted to a loop over rmap
which is much more cache friendly than before.

e676505ac9 KVM: MMU: Force cr3 reload with two dimensional paging on mov cr3 emulation
Currently the MMU's ->new_cr3() callback does nothing when guest paging
is disabled or when two-dimentional paging (e.g. EPT on Intel) is active.
This means that an emulated write to cr3 can be lost; kvm_set_cr3() will
write vcpu-arch.cr3, but the GUEST_CR3 field in the VMCS will retain its
old value and this is what the guest sees.

This bug did not have any effect until now because:
- with unrestricted guest, or with svm, we never emulate a mov cr3 instruction
- without unrestricted guest, and with paging enabled, we also never emulate a
  mov cr3 instruction
- without unrestricted guest, but with paging disabled, the guest's cr3 is
  ignored until the guest enables paging; at this point the value from arch.cr3
  is loaded correctly my the mov cr0 instruction which turns on paging

However, the patchset that enables big real mode causes us to emulate mov cr3
instructions in protected mode sometimes (when guest state is not virtualizable
by vmx); this mov cr3 is effectively ignored and will crash the guest.

The fix is to make nonpaging_new_cr3() call mmu_free_roots() to force a cr3
reload.  This is awkward because now all the new_cr3 callbacks to the same
thing, and because mmu_free_roots() is somewhat of an overkill; but fixing
that is more complicated and will be done after this minimal fix.

Observed in the Window XP 32-bit installer while bringing up secondary vcpus.

Using RCU for lockless shadow walking can increase the amount of memory
in use by the system, since RCU grace periods are unpredictable.  We also
have an unconditional write to a shared variable (reader_counter), which
isn't good for scaling.

Replace that with a scheme similar to x86's get_user_pages_fast(): disable
interrupts during lockless shadow walk to force the freer
(kvm_mmu_commit_zap_page()) to wait for the TLB flush IPI to find the
processor with interrupts enabled.

We also add a new vcpu->mode, READING_SHADOW_PAGE_TABLES, to prevent
kvm_flush_remote_tlbs() from avoiding the IPI.

1e3f42f03c KVM: MMU: Improve iteration through sptes from rmap
Iteration using rmap_next(), the actual body is pte_list_next(), is
inefficient: every time we call it we start from checking whether rmap
holds a single spte or points to a descriptor which links more sptes.

In the case of shadow paging, this quadratic total iteration cost is a
problem.  Even for two dimensional paging, with EPT/NPT on, in which we
almost always have a single mapping, the extra checks at the end of the
iteration should be eliminated.

This patch fixes this by introducing rmap_iterator which keeps the
iteration context for the next search.  Furthermore the implementation
of rmap_next() is splitted into two functions, rmap_get_first() and
rmap_get_next(), to avoid repeatedly checking whether the rmap being
iterated on has only one spte.

Although there seemed to be only a slight change for EPT/NPT, the actual
improvement was significant: we observed that GET_DIRTY_LOG for 1GB
dirty memory became 15% faster than before.  This is probably because
the new code is easy to make branch predictions.

Note: we just remove pte_list_next() because we can think of parent_ptes
as a reverse mapping.

220f773a00 KVM: MMU: Make pte_list_desc fit cache lines well
We have PTE_LIST_EXT + 1 pointers in this structure and these 40/20
bytes do not fit cache lines well.  Furthermore, some allocators may
use 64/32-byte objects for the pte_list_desc cache.

This patch solves this problem by changing PTE_LIST_EXT from 4 to 3.

For shadow paging, the new size is still large enough to hold both the
kernel and process mappings for usual anonymous pages.  For file
mappings, there may be a slight change in the cache usage.

Note: with EPT/NPT we almost always have a single spte in each reverse
mapping and we will not see any change by this.

95d4c16ce7 KVM: Optimize dirty logging by rmap_write_protect()
Currently, write protecting a slot needs to walk all the shadow pages
and checks ones which have a pte mapping a page in it.

The walk is overly heavy when dirty pages in that slot are not so many
and checking the shadow pages would result in unwanted cache pollution.

To mitigate this problem, we use rmap_write_protect() and check only
the sptes which can be reached from gfns marked in the dirty bitmap
when the number of dirty pages are less than that of shadow pages.

This criterion is reasonable in its meaning and worked well in our test:
write protection became some times faster than before when the ratio of
dirty pages are low and was not worse even when the ratio was near the
criterion.

Note that the locking for this write protection becomes fine grained.
The reason why this is safe is descripted in the comments.

9b9b149236 KVM: MMU: Split gfn_to_rmap() into two functions
rmap_write_protect() calls gfn_to_rmap() for each level with gfn fixed.
This results in calling gfn_to_memslot() repeatedly with that gfn.

This patch introduces __gfn_to_rmap() which takes the slot as an
argument to avoid this.

This is also needed for the following dirty logging optimization.

a30f47cb15 KVM: MMU: improve write flooding detected
Detecting write-flooding does not work well, when we handle page written, if
the last speculative spte is not accessed, we treat the page is
write-flooding, however, we can speculative spte on many path, such as pte
prefetch, page synced, that means the last speculative spte may be not point
to the written page and the written page can be accessed via other sptes, so
depends on the Accessed bit of the last speculative spte is not enough

Instead of detected page accessed, we can detect whether the spte is accessed
after it is written, if the spte is not accessed but it is written frequently,
we treat is not a page table or it not used for a long time

5d9ca30e96 KVM: MMU: fix detecting misaligned accessed
Sometimes, we only modify the last one byte of a pte to update status bit,
for example, clear_bit is used to clear r/w bit in linux kernel and 'andb'
instruction is used in this function, in this case, kvm_mmu_pte_write will
treat it as misaligned access, and the shadow page table is zapped

kvm_mmu_pte_write is unsafe since we need to alloc pte_list_desc in the
function when spte is prefetched, unfortunately, we can not know how many
spte need to be prefetched on this path, that means we can use out of the
free  pte_list_desc object in the cache, and BUG_ON() is triggered, also some
path does not fill the cache, such as INS instruction emulated that does not
trigger page fault

e4e517b4be KVM: MMU: Do not unconditionally read PDPTE from guest memory
Architecturally, PDPTEs are cached in the PDPTRs when CR3 is reloaded.
On SVM, it is not possible to implement this, but on VMX this is possible
and was indeed implemented until nested SVM changed this to unconditionally
read PDPTEs dynamically.  This has noticable impact when running PAE guests.

Fix by changing the MMU to read PDPTRs from the cache, falling back to
reading from memory for the nested MMU.

The idea is from Avi:

When the page fault is caused by mmio, we cache the info in the shadow page
table, and also set the reserved bits in the shadow page table, so if the mmio
is caused again, we can quickly identify it and emulate it directly

Searching mmio gfn in memslots is heavy since we need to walk all memeslots, it
can be reduced by this feature, and also avoid walking guest page table for
soft mmu.

Introduce some interfaces to modify spte as linux kernel does:
- mmu_spte_clear_track_bits, it set the spte from present to nonpresent, and
  track the stat bits(accessed/dirty) of spte
- mmu_spte_clear_no_track, the same as mmu_spte_clear_track_bits except
  tracking the stat bits
- mmu_spte_set, set spte from nonpresent to present
- mmu_spte_update, only update the stat bits

Now, it does not allowed to set spte from present to present, later, we can
drop the atomicly opration for X86_32 host, and it is the preparing work to
get spte on X86_32 host out of the mmu lock

If the page fault is caused by mmio, the gfn can not be found in memslots, and
'bad_pfn' is returned on gfn_to_hva path, so we can use 'bad_pfn' to identify
the mmio page fault.
And, to clarify the meaning of mmio pfn, we return fault page instead of bad
page when the gfn is not allowd to prefetch

411c588dfb KVM: MMU: Adjust shadow paging to work when SMEP=1 and CR0.WP=0
When CR0.WP=0, we sometimes map user pages as kernel pages (to allow
the kernel to write to them).  Unfortunately this also allows the kernel
to fetch from these pages, even if CR4.SMEP is set.

Adjust for this by also setting NX on the spte in these circumstances.
