8cf4ecc0ca KVM: PPC: Book3S HV: Enable radix guest support
This adds a few last pieces of the support for radix guests:

* Implement the backends for the KVM_PPC_CONFIGURE_V3_MMU and
  KVM_PPC_GET_RMMU_INFO ioctls for radix guests

* On POWER9, allow secondary threads to be on/off-lined while guests
  are running.

* Set up LPCR and the partition table entry for radix guests.

* Don't allocate the rmap array in the kvm_memory_slot structure
  on radix.

* Don't try to initialize the HPT for radix guests, since they don't
  have an HPT.

* Take out the code that prevents the HV KVM module from
  initializing on radix hosts.

At this stage, we only support radix guests if the host is running
in radix mode, and only support HPT guests if the host is running in
HPT mode.  Thus a guest cannot switch from one mode to the other,
which enables some simplifications.

a29ebeaf55 KVM: PPC: Book3S HV: Invalidate TLB on radix guest vcpu movement
With radix, the guest can do TLB invalidations itself using the tlbie
(global) and tlbiel (local) TLB invalidation instructions.  Linux guests
use local TLB invalidations for translations that have only ever been
accessed on one vcpu.  However, that doesn't mean that the translations
have only been accessed on one physical cpu (pcpu) since vcpus can move
around from one pcpu to another.  Thus a tlbiel might leave behind stale
TLB entries on a pcpu where the vcpu previously ran, and if that task
then moves back to that previous pcpu, it could see those stale TLB
entries and thus access memory incorrectly.  The usual symptom of this
is random segfaults in userspace programs in the guest.

To cope with this, we detect when a vcpu is about to start executing on
a thread in a core that is a different core from the last time it
executed.  If that is the case, then we mark the core as needing a
TLB flush and then send an interrupt to any thread in the core that is
currently running a vcpu from the same guest.  This will get those vcpus
out of the guest, and the first one to re-enter the guest will do the
TLB flush.  The reason for interrupting the vcpus executing on the old
core is to cope with the following scenario:

	CPU 0			CPU 1			CPU 4
	(core 0)			(core 0)			(core 1)

	VCPU 0 runs task X      VCPU 1 runs
	core 0 TLB gets
	entries from task X
	VCPU 0 moves to CPU 4
							VCPU 0 runs task X
							Unmap pages of task X
							tlbiel

				(still VCPU 1)			task X moves to VCPU 1
				task X runs
				task X sees stale TLB
				entries

That is, as soon as the VCPU starts executing on the new core, it
could unmap and tlbiel some page table entries, and then the task
could migrate to one of the VCPUs running on the old core and
potentially see stale TLB entries.

Since the TLB is shared between all the threads in a core, we only
use the bit of kvm->arch.need_tlb_flush corresponding to the first
thread in the core.  To ensure that we don't have a window where we
can miss a flush, this moves the clearing of the bit from before the
actual flush to after it.  This way, two threads might both do the
flush, but we prevent the situation where one thread can enter the
guest before the flush is finished.

8f7b79b837 KVM: PPC: Book3S HV: Implement dirty page logging for radix guests
This adds code to keep track of dirty pages when requested (that is,
when memslot->dirty_bitmap is non-NULL) for radix guests.  We use the
dirty bits in the PTEs in the second-level (partition-scoped) page
tables, together with a bitmap of pages that were dirty when their
PTE was invalidated (e.g., when the page was paged out).  This bitmap
is stored in the first half of the memslot->dirty_bitmap area, and
kvm_vm_ioctl_get_dirty_log_hv() now uses the second half for the
bitmap that gets returned to userspace.

01756099e0 KVM: PPC: Book3S HV: MMU notifier callbacks for radix guests
This adapts our implementations of the MMU notifier callbacks
(unmap_hva, unmap_hva_range, age_hva, test_age_hva, set_spte_hva)
to call radix functions when the guest is using radix.  These
implementations are much simpler than for HPT guests because we
have only one PTE to deal with, so we don't need to traverse
rmap chains.

5a319350a4 KVM: PPC: Book3S HV: Page table construction and page faults for radix guests
This adds the code to construct the second-level ("partition-scoped" in
architecturese) page tables for guests using the radix MMU.  Apart from
the PGD level, which is allocated when the guest is created, the rest
of the tree is all constructed in response to hypervisor page faults.

As well as hypervisor page faults for missing pages, we also get faults
for reference/change (RC) bits needing to be set, as well as various
other error conditions.  For now, we only set the R or C bit in the
guest page table if the same bit is set in the host PTE for the
backing page.

This code can take advantage of the guest being backed with either
transparent or ordinary 2MB huge pages, and insert 2MB page entries
into the guest page tables.  There is no support for 1GB huge pages
yet.

f4c51f841d KVM: PPC: Book3S HV: Modify guest entry/exit paths to handle radix guests
This adds code to  branch around the parts that radix guests don't
need - clearing and loading the SLB with the guest SLB contents,
saving the guest SLB contents on exit, and restoring the host SLB
contents.

Since the host is now using radix, we need to save and restore the
host value for the PID register.

On hypervisor data/instruction storage interrupts, we don't do the
guest HPT lookup on radix, but just save the guest physical address
for the fault (from the ASDR register) in the vcpu struct.

c927013227 KVM: PPC: Book3S HV: Add userspace interfaces for POWER9 MMU
This adds two capabilities and two ioctls to allow userspace to
find out about and configure the POWER9 MMU in a guest.  The two
capabilities tell userspace whether KVM can support a guest using
the radix MMU, or using the hashed page table (HPT) MMU with a
process table and segment tables.  (Note that the MMUs in the
POWER9 processor cores do not use the process and segment tables
when in HPT mode, but the nest MMU does).

The KVM_PPC_CONFIGURE_V3_MMU ioctl allows userspace to specify
whether a guest will use the radix MMU or the HPT MMU, and to
specify the size and location (in guest space) of the process
table.

The KVM_PPC_GET_RMMU_INFO ioctl gives userspace information about
the radix MMU.  It returns a list of supported radix tree geometries
(base page size and number of bits indexed at each level of the
radix tree) and the encoding used to specify the various page
sizes for the TLB invalidate entry instruction.

Initially, both capabilities return 0 and the ioctls return -EINVAL,
until the necessary infrastructure for them to operate correctly
is added.

bc3551257a powerpc/64: Allow for relocation-on interrupts from guest to host
With host and guest both using radix translation, it is feasible
for the host to take interrupts that come from the guest with
relocation on, and that is in fact what the POWER9 hardware will
do when LPCR[AIL] = 3.  All such interrupts use HSRR0/1 not SRR0/1
except for system call with LEV=1 (hcall).

Therefore this adds the KVM tests to the _HV variants of the
relocation-on interrupt handlers, and adds the KVM test to the
relocation-on system call entry point.

We also instantiate the relocation-on versions of the hypervisor
data storage and instruction interrupt handlers, since these can
occur with relocation on in radix guests.

cc3d294013 powerpc/64: Enable use of radix MMU under hypervisor on POWER9
To use radix as a guest, we first need to tell the hypervisor via
the ibm,client-architecture call first that we support POWER9 and
architecture v3.00, and that we can do either radix or hash and
that we would like to choose later using an hcall (the
H_REGISTER_PROC_TBL hcall).

Then we need to check whether the hypervisor agreed to us using
radix.  We need to do this very early on in the kernel boot process
before any of the MMU initialization is done.  If the hypervisor
doesn't agree, we can't use radix and therefore clear the radix
MMU feature bit.

Later, when we have set up our process table, which points to the
radix tree for each process, we need to install that using the
H_REGISTER_PROC_TBL hcall.

3f4ab2f83b powerpc/pseries: Fixes for the "ibm,architecture-vec-5" options
This fixes the byte index values for some of the option bits in
the "ibm,architectur-vec-5" property. The "platform facilities options"
bits are in byte 17 not byte 14, so the upper 8 bits of their
definitions need to be 0x11 not 0x0E. The "sub processor support" option
is in byte 21 not byte 15.

Note none of these options are actually looked up in
"ibm,architecture-vec-5" at this time, so there is no bug.

When checking whether option bits are set, we should check that
the offset of the byte being checked is less than the vector
length that we got from the hypervisor.

a97a65d53d KVM: PPC: Book3S: 64-bit CONFIG_RELOCATABLE support for interrupts
64-bit Book3S exception handlers must find the dynamic kernel base
to add to the target address when branching beyond __end_interrupts,
in order to support kernel running at non-0 physical address.

Support this in KVM by branching with CTR, similarly to regular
interrupt handlers. The guest CTR saved in HSTATE_SCRATCH1 and
restored after the branch.

Without this, the host kernel hangs and crashes randomly when it is
running at a non-0 address and a KVM guest is started.

4b5d62ca17 powerpc/mm: add radix__remove_section_mapping()
Tear down and free the four-level page tables of physical mappings
during memory hotremove.

Borrow the basic structure of remove_pagetable() and friends from the
identically-named x86 functions. Reduce the frequency of tlb flushes and
page_table_lock spinlocks by only doing them in the outermost function.
There was some question as to whether the locking is needed at all.
Leave it for now, but we could consider dropping it.

Memory must be offline to be removed, thus not in use. So there
shouldn't be the sort of concurrent page walking activity here that
might prompt us to use RCU.

09206b600c powernv: Pass PSSCR value and mask to power9_idle_stop
The power9_idle_stop method currently takes only the requested stop
level as a parameter and picks up the rest of the PSSCR bits from a
hand-coded macro. This is not a very flexible design, especially when
the firmware has the capability to communicate the psscr value and the
mask associated with a particular stop state via device tree.

This patch modifies the power9_idle_stop API to take as parameters the
PSSCR value and the PSSCR mask corresponding to the stop state that
needs to be set. These PSSCR value and mask are respectively obtained
by parsing the "ibm,cpu-idle-state-psscr" and
"ibm,cpu-idle-state-psscr-mask" fields from the device tree.

In addition to this, the patch adds support for handling stop states
for which ESL and EC bits in the PSSCR are zero. As per the
architecture, a wakeup from these stop states resumes execution from
the subsequent instruction as opposed to waking up at the System
Vector.

The older firmware sets only the Requested Level (RL) field in the
psscr and psscr-mask exposed in the device tree. For older firmware
where psscr-mask=0xf, this patch will set the default sane values that
the set for for remaining PSSCR fields (i.e PSLL, MTL, ESL, EC, and
TR). For the new firmware, the patch will validate that the invariants
required by the ISA for the psscr values are maintained by the
firmware.

[Optimize the number of instructions before entering STOP with
ESL=EC=0, validate the PSSCR values provided by the firimware

823b7bd515 powernv:idle: Add IDLE_STATE_ENTER_SEQ_NORET macro
Currently all the low-power idle states are expected to wake up
at reset vector 0x100. Which is why the macro IDLE_STATE_ENTER_SEQ
that puts the CPU to an idle state and never returns.

On ISA v3.0, when the ESL and EC bits in the PSSCR are zero, the CPU
is expected to wake up at the next instruction of the idle
instruction.

This patch adds a new macro named IDLE_STATE_ENTER_SEQ_NORET for the
no-return variant and reuses the name IDLE_STATE_ENTER_SEQ
for a variant that allows resuming operation at the instruction next
to the idle-instruction.

1d0761d255 powerpc/powernv: Initialise nest mmu
POWER9 contains an off core mmu called the nest mmu (NMMU). This is
used by other hardware units on the chip to translate virtual
addresses into real addresses. The unit attempting an address
translation provides the majority of the context required for the
translation request except for the base address of the partition table
(ie. the PTCR) which needs to be programmed into the NMMU.

This patch adds a call to OPAL to set the PTCR for the nest mmu in
opal_init().

79270e0a3f powerpc/mm/hash: Properly mask the ESID bits when building proto VSID
The proto VSID is built using both the MMU context id and effective
segment ID (ESID). We should not have overlapping bits between those.
That could result in us having a VSID collision. With the current code
we missed masking the top bits of the ESID. This implies for kernel
address we ended up using the top 4 bits of the ESID as part of the
proto VSID, which is wrong.

The current code use the top 4 context values (0x7fffc - 0x7ffff) for
the kernel. With those context IDs used for the kernel, we don't run
into VSID collisions because we get the same proto VSID irrespective of
whether we mask the ESID bits or not. eg:

  ea         = 0xf000000000000000
  context    = 0x7ffff

  w/out masking:
  proto_vsid = (0x7ffff << 6 | 0xf000000000000000 >> 40)
	     = (0x1ffffc0 | 0xf00000)
	     =  0x1ffffc0

  with masking:
  proto_vsid = (0x7ffff << 6 | ((0xf000000000000000 >> 40) & 0x3f))
	     = (0x1ffffc0 | (0xf00000 & 0x3f))
	     =  0x1ffffc0 | 0)
	     =  0x1ffffc0

So although there is no bug, the code is still overly subtle, so fix it
to save ourselves pain in future.

7ede531773 KVM: PPC: Book3S: Move 64-bit KVM interrupt handler out from alt section
A subsequent patch to make KVM handlers relocation-safe makes them
unusable from within alt section "else" cases (due to the way fixed
addresses are taken from within fixed section head code).

Stop open-coding the KVM handlers, and add them both as normal. A more
optimal fix may be to allow some level of alternate feature patching in
the exception macros themselves, but for now this will do.

The TRAMP_KVM handlers must be moved to the "virt" fixed section area
(name is arbitrary) in order to be closer to .text and avoid the dreaded
"relocation truncated to fit" error.

Change the calling convention to put the trap number together with
CR in two halves of r12, which frees up HSTATE_SCRATCH2 in the HV
handler.

The 64-bit PR handler entry translates the calling convention back
to match the previous call convention (i.e., shared with 32-bit), for
simplicity.

75b8247276 powerpc/8xx: Perf events on PPC 8xx
This patch has been reworked since RFC version. In the RFC, this patch
was preceded by a patch clearing MSR RI for all PPC32 at all time at
exception prologs. Now MSR RI clearing is done only when this 8xx perf
events functionality is compiled in, it is therefore limited to 8xx
and merged inside this patch.
Other main changes have been to take into account detailed review from
Peter Zijlstra. The instructions counter has been reworked to behave
as a free running counter like the three other counters.

The 8xx has no PMU, however some events can be emulated by other means.

This patch implements the following events (as reported by 'perf list'):
  cpu-cycles OR cycles				[Hardware event]
  instructions					[Hardware event]
  dTLB-load-misses				[Hardware cache event]
  iTLB-load-misses				[Hardware cache event]

'cycles' event is implemented using the timebase clock. Timebase clock
corresponds to CPU clock divided by 16, so number of cycles is
approximatly 16 times the number of TB ticks

On the 8xx, TLB misses are handled by software. It is therefore
easy to count all TLB misses each time the TLB miss exception is
called.

'instructions' is calculated by using instruction watchpoint counter.
This patch sets counter A to count instructions at address greater
than 0, hence we count all instructions executed while MSR RI bit is
set. The counter is set to the maximum which is 0xffff. Every 65535
instructions, debug instruction breakpoint exception fires. The
exception handler increments a counter in memory which then
represent the upper part of the instruction counter. We therefore
end up with a 48 bits counter. In order to avoid unnecessary overhead
while no perf event is active, this counter is started when the first
event referring to this counter is added, and the counter is stopped
when the last event referring to it is deleted. In order to properly
support breakpoint exceptions, MSR RI bit has to be unset in exception
epilogs in order to avoid breakpoint exceptions during critical
sections during changes to SRR0 and SRR1 would be problematic.

All counters are handled as free running counters.

17d48610ae KVM: PPC: Book 3S: XICS: Implement ICS P/Q states
This patch implements P(Presented)/Q(Queued) states for ICS irqs.

When the interrupt is presented, set P. Present if P was not set.
If P is already set, don't present again, set Q.
When the interrupt is EOI'ed, move Q into P (and clear Q). If it is
set, re-present.

The asserted flag used by LSI is also incorporated into the P bit.

When the irq state is saved, P/Q bits are also saved, they need some
qemu modifications to be recognized and passed around to be restored.
KVM_XICS_PENDING bit set and saved should also indicate
KVM_XICS_PRESENTED bit set and saved. But it is possible some old
code doesn't have/recognize the P bit, so when we restore, we set P
for PENDING bit, too.

The idea and much of the code come from Ben.

4ad8622dc5 powerpc/8xx: Implement hw_breakpoint
This patch implements HW breakpoint on the 8xx. The 8xx has
capability to manage HW breakpoints, which is slightly different
than BOOK3S:
1/ The breakpoint match doesn't trigger a DSI exception but a
dedicated data breakpoint exception.
2/ The breakpoint happens after the instruction has completed,
no need to single step or emulate the instruction,
3/ Matched address is not set in DAR but in BAR,
4/ DABR register doesn't exist, instead we have registers
LCTRL1, LCTRL2 and CMPx registers,
5/ The match on one comparator is not on a double word but
on a single word.

The patch does:
1/ Prepare the dedicated registers in call to __set_dabr(). In order
to emulate the double word handling of BOOK3S, comparator E is set to
DABR address value and comparator F to address + 4. Then breakpoint 1
is set to match comparator E or F,
2/ Skip the singlestepping stage when compiled for CONFIG_PPC_8xx,
3/ Implement the exception. In that exception, the matched address
is taken from SPRN_BAR and manage as if it was from SPRN_DAR.
4/ I/D TLB error exception routines perform a tlbie on bad TLBs. That
tlbie triggers the breakpoint exception when performed on the
breakpoint address. For this reason, the routine returns if the match
is from one of those two tlbie.

Anton says: In commit 4db7327194 ("powerpc: Add option to use jump
label for cpu_has_feature()") and commit c12e6f24d4 ("powerpc: Add
option to use jump label for mmu_has_feature()") we added:

  BUILD_BUG_ON(!__builtin_constant_p(feature))

to cpu_has_feature() and mmu_has_feature() in order to catch usage
issues (such as cpu_has_feature(cpu_has_feature(X), which has happened
once in the past). Unfortunately LLVM isn't smart enough to resolve
this, and it errors out.

I work around it in my clang/LLVM builds of the kernel, but I have just
discovered that it causes a lot of issues for the bcc (eBPF) trace tool
(which uses LLVM).

For now just #ifdef it away for clang builds.

d4fde568a3 powerpc/64: Use optimized checksum routines on little-endian
Currently we have optimized hand-coded assembly checksum routines for
big-endian 64-bit systems, but for little-endian we use the generic C
routines. This modifies the optimized routines to work for
little-endian. With this, we no longer need to enable
CONFIG_GENERIC_CSUM. This also fixes a couple of comments in
checksum_64.S so they accurately reflect what the associated instruction
does.

b492f7e4e0 powerpc/64: Fix checksum folding in csum_tcpudp_nofold and ip_fast_csum_nofold
These functions compute an IP checksum by computing a 64-bit sum and
folding it to 32 bits (the "nofold" in their names refers to folding
down to 16 bits).  However, doing (u32) (s + (s >> 32)) is not
sufficient to fold a 64-bit sum to 32 bits correctly.  The addition
can produce a carry out from bit 31, which needs to be added in to
the sum to produce the correct result.

To fix this, we copy the from64to32() function from lib/checksum.c
and use that.

We now support THP with both 64k and 4K page size configuration
for radix. (hash only support THP with 64K page size). Hence we
will have CONFIG_TRANSPARENT_HUGEPAGE enabled for both PPC_64K
and PPC_4K config. Since we only need large pmd page table
with hash configuration (to store the slot information
in the second half of the table) restrict the large pmd page table
to THP and 64K configs.

178f358208 powerpc: Ignore reserved field in DCSR and PVR reads and writes
IBM bit 31 (for the rest of us - bit 0) is a reserved field in the
instruction definition of mtspr and mfspr. Hardware is encouraged to
(and does) ignore it.

As a result, if userspace executes an mtspr DSCR with the reserved bit
set, we get a DSCR facility unavailable exception. The kernel fails to
match against the expected value/mask, and we silently return to
userspace to try and re-execute the same mtspr DSCR instruction. We
loop forever until the process is killed.

We should do something here, and it seems mirroring what hardware does
is the better option vs killing the process. While here, relax the
matching of mfspr PVR too.

When we switched to big endian page table, we never updated the hugepd
format such that it can work for both big endian and little endian
config. This patch series update hugepd format such that it is looked at

I did update the FSL hugepd_ok check to check for the top bit instead
of checking > 0.

467d278249 powerpc: ima: get the kexec buffer passed by the previous kernel
Patch series "ima: carry the measurement list across kexec", v8.

The TPM PCRs are only reset on a hard reboot.  In order to validate a
TPM's quote after a soft reboot (eg.  kexec -e), the IMA measurement
list of the running kernel must be saved and then restored on the
subsequent boot, possibly of a different architecture.

The existing securityfs binary_runtime_measurements file conveniently
provides a serialized format of the IMA measurement list.  This patch
set serializes the measurement list in this format and restores it.

Up to now, the binary_runtime_measurements was defined as architecture
native format.  The assumption being that userspace could and would
handle any architecture conversions.  With the ability of carrying the
measurement list across kexec, possibly from one architecture to a
different one, the per boot architecture information is lost and with it
the ability of recalculating the template digest hash.  To resolve this
problem, without breaking the existing ABI, this patch set introduces
the boot command line option "ima_canonical_fmt", which is arbitrarily
defined as little endian.

The need for this boot command line option will be limited to the
existing version 1 format of the binary_runtime_measurements.
Subsequent formats will be defined as canonical format (eg.  TPM 2.0
support for larger digests).

A simplified method of Thiago Bauermann's "kexec buffer handover" patch
series for carrying the IMA measurement list across kexec is included in
this patch set.  The simplified method requires all file measurements be
taken prior to executing the kexec load, as subsequent measurements will
not be carried across the kexec and restored.

This patch (of 10):

The IMA kexec buffer allows the currently running kernel to pass the
measurement list via a kexec segment to the kernel that will be kexec'd.
The second kernel can check whether the previous kernel sent the buffer
and retrieve it.

This is the architecture-specific part which enables IMA to receive the
measurement list passed by the previous kernel.  It will be used in the
next patch.

The change in machine_kexec_64.c is to factor out the logic of removing
an FDT memory reservation so that it can be used by remove_ima_buffer.

1dd38b6c27 mm: move vma_is_anonymous check within pmd_move_must_withdraw
Independent of whether the vma is for anonymous memory, some arches like
ppc64 would like to override pmd_move_must_withdraw().

One option is to encapsulate the vma_is_anonymous() check for general
architectures inside pmd_move_must_withdraw() so that is always called
and architectures that need unconditional overriding can override this
function.  ppc64 needs to override the function when the MMU is
configured to use hash PTE's.

07e326610e mm: add tlb_remove_check_page_size_change to track page size change
With commit e77b0852b5 ("mm/mmu_gather: track page size with mmu
gather and force flush if page size change") we added the ability to
force a tlb flush when the page size change in a mmu_gather loop.  We
did that by checking for a page size change every time we added a page
to mmu_gather for lazy flush/remove.  We can improve that by moving the
page size change check early and not doing it every time we add a page.

This also helps us to do tlb flush when invalidating a range covering
dax mapping.  Wrt dax mapping we don't have a backing struct page and
hence we don't call tlb_remove_page, which earlier forced the tlb flush
on page size change.  Moving the page size change check earlier means we
will do the same even for dax mapping.

We also avoid doing this check on architecture other than powerpc.

In a later patch we will remove page size check from tlb_remove_page().

4b91428699 powerpc/8xx: Implement support of hugepages
8xx uses a two level page table with two different linux page size
support (4k and 16k). 8xx also support two different hugepage sizes
512k and 8M. In order to support them on linux we define two different
page table layout.

The size of pages is in the PGD entry, using PS field (bits 28-29):
00 : Small pages (4k or 16k)
01 : 512k pages
10 : reserved
11 : 8M pages

For 512K hugepage size a pgd entry have the below format
[<hugepte address >0101] . The hugepte table allocated will contain 8
entries pointing to 512K huge pte in 4k pages mode and 64 entries in
16k pages mode.

For 8M in 16k mode, a pgd entry have the below format
[<hugepte address >1101] . The hugepte table allocated will contain 8
entries pointing to 8M huge pte.

For 8M in 4k mode, multiple pgd entries point to the same hugepte
address and pgd entry will have the below format
[<hugepte address>1101]. The hugepte table allocated will only have one
entry.

For the time being, we do not support CPU15 ERRATA when HUGETLB is
selected

9b081e1080 powerpc: port 64 bits pgtable_cache to 32 bits
Today powerpc64 uses a set of pgtable_caches while powerpc32 uses
standard pages when using 4k pages and a single pgtable_cache
if using other size pages.

In preparation of implementing huge pages on the 8xx, this patch
replaces the specific powerpc32 handling by the 64 bits approach.

This is done by:
* moving 64 bits pgtable_cache_add() and pgtable_cache_init()
in a new file called init-common.c
* modifying pgtable_cache_init() to also handle the case
without PMD
* removing the 32 bits version of pgtable_cache_add() and
pgtable_cache_init()
* copying related header contents from 64 bits into both the
book3s/32 and nohash/32 header files

On the 8xx, the following cache sizes will be used:
* 4k pages mode:
- PGT_CACHE(10) for PGD
- PGT_CACHE(3) for 512k hugepage tables
* 16k pages mode:
- PGT_CACHE(6) for PGD
- PGT_CACHE(7) for 512k hugepage tables
- PGT_CACHE(3) for 8M hugepage tables

Some tracepoints have a registration function that gets enabled when the
tracepoint is enabled. There may be cases that the registraction function
must fail (for example, can't allocate enough memory). In this case, the
tracepoint should also fail to register, otherwise the user would not know
why the tracepoint is not working.

d7baee6901 powerpc/iommu: Stop using @current in mm_iommu_xxx
This changes mm_iommu_xxx helpers to take mm_struct as a parameter
instead of getting it from @current which in some situations may
not have a valid reference to mm.

This changes helpers to receive @mm and moves all references to @current
to the caller, including checks for !current and !current->mm;
checks in mm_iommu_preregistered() are removed as there is no caller
yet.

This moves the mm_iommu_adjust_locked_vm() call to the caller as
it receives mm_iommu_table_group_mem_t but it needs mm.

This should cause no behavioral change.

and cache mm_struct in the VFIO container. Since mm_context_t does not
have reference counting, we will be using mm_struct which does have

This changes mm_iommu_init/mm_iommu_cleanup to receive mm_struct rather
than mm_context_t (which is embedded into mm).

This should not cause any behavioral change.

1c885808e4 tcp: SOF_TIMESTAMPING_OPT_STATS option for SO_TIMESTAMPING
This patch exports the sender chronograph stats via the socket
SO_TIMESTAMPING channel. Currently we can instrument how long a
particular application unit of data was queued in TCP by tracking
SOF_TIMESTAMPING_TX_SOFTWARE and SOF_TIMESTAMPING_TX_SCHED. Having
these sender chronograph stats exported simultaneously along with
these timestamps allow further breaking down the various sender
limitation.  For example, a video server can tell if a particular
chunk of video on a connection takes a long time to deliver because
TCP was experiencing small receive window. It is not possible to
tell before this patch without packet traces.

To prepare these stats, the user needs to set
SOF_TIMESTAMPING_OPT_STATS and SOF_TIMESTAMPING_OPT_TSONLY flags
while requesting other SOF_TIMESTAMPING TX timestamps. When the
timestamps are available in the error queue, the stats are returned
in a separate control message of type SCM_TIMESTAMPING_OPT_STATS,
in a list of TLVs (struct nlattr) of types: TCP_NLA_BUSY_TIME,
TCP_NLA_RWND_LIMITED, TCP_NLA_SNDBUF_LIMITED. Unit is microsecond.

Now that we've defined structures to describe each of the client
architecture vectors, we can use those to construct the value we pass to
firmware.

This avoids the tricks we previously played with the W() macro, allows
us to properly endian annotate fields, and should help to avoid bugs
introduced by failing to have the correct number of zero pad bytes
between fields.

It also means we can avoid hard coding IBM_ARCH_VEC_NRCORES_OFFSET in
order to update the max_cpus value and instead just set it.

    of the "ibm,ppc-interrupt-server#s" property of the CPU in the OF
    device tree.

  Semantics:
  - Invalid target: return H_Parameter.
  - Otherwise: Generate a system reset NMI on target thread(s),
    return H_Success.

This will be used by crash/debug code to get stuck CPUs into a known
state.

a0458284f0 powerpc: Add support code for kexec_file_load()
This patch adds the support code needed for implementing
kexec_file_load() on powerpc.

This consists of functions to load the ELF kernel, either big or little
endian, and setup the purgatory enviroment which switches from the first
kernel to the second kernel.

None of this code is built yet, as it depends on CONFIG_KEXEC_FILE which
we have not yet defined. Although we could define CONFIG_KEXEC_FILE in
this patch, we'd then have a window in history where the kconfig symbol
is present but the syscall is not, which would be awkward.

Commit 2965faa5e0 ("kexec: split kexec_load syscall from kexec core
code") introduced CONFIG_KEXEC_CORE so that CONFIG_KEXEC means whether
the kexec_load system call should be compiled-in and CONFIG_KEXEC_FILE
means whether the kexec_file_load system call should be compiled-in.
These options can be set independently from each other.

Since until now powerpc only supported kexec_load, CONFIG_KEXEC and
CONFIG_KEXEC_CORE were synonyms. That is not the case anymore, so we
need to make a distinction. Almost all places where CONFIG_KEXEC was
being used should be using CONFIG_KEXEC_CORE instead, since
kexec_file_load also needs that code compiled in.

This patch adds a new software defined pte bit. We use the reserved
fields of ISA 3.0 pte definition since we will only be using this on DD1
code paths. We can possibly look at removing this code later.

The software bit will be used to differentiate between 64K/4K and 2M
ptes. This helps in finding the page size mapping by a pte so that we
can do efficient tlb flush.

We don't support 1G hugetlb pages yet. So we add a DEBUG WARN_ON to
catch wrong usage.

f4944613ad KVM: PPC: Decrease the powerpc default halt poll max value
KVM_HALT_POLL_NS_DEFAULT is an arch specific constant which sets the
default value of the halt_poll_ns kvm module parameter which determines
the global maximum halt polling interval.

The current value for powerpc is 500000 (500us) which means that any
repetitive workload with a period of less than that can drive the cpu
usage to 100% where it may have been mostly idle without halt polling.
This presents the possibility of a large increase in power usage with
a comparatively small performance benefit.

Reduce the default to 10000 (10us) and a user can tune this themselves
to set their affinity for halt polling based on the trade off between power
and performance which they are willing to make.

984d7a1ec6 powerpc/mm: Fixup kernel read only mapping
With commit e58e87adc8 ("powerpc/mm: Update _PAGE_KERNEL_RO") we
started using the ppp value 0b110 to map kernel readonly. But that
facility was only added as part of ISA 2.04. For earlier ISA version
only supported ppp bit value for readonly mapping is 0b011. (This
implies both user and kernel get mapped using the same ppp bit value for
readonly mapping.).
Update the code such that for earlier architecture version we use ppp
value 0b011 for readonly mapping. We don't differentiate between power5+
and power5 here and apply the new ppp bits only from power6 (ISA 2.05).
This keep the changes minimal.

This fixes issue with PS3 spu usage reported at
https://lkml.kernel.org/r/rep.1421449714.geoff@infradead.org

This is leading to warnings such as:

84f7139c06 KVM: PPC: Book3S HV: Enable hypervisor virtualization interrupts while in guest
The new XIVE interrupt controller on POWER9 can direct external
interrupts to the hypervisor or the guest.  The interrupts directed to
the hypervisor are controlled by an LPCR bit called LPCR_HVICE, and
come in as a "hypervisor virtualization interrupt".  This sets the
LPCR bit so that hypervisor virtualization interrupts can occur while
we are in the guest.  We then also need to cope with exiting the guest
because of a hypervisor virtualization interrupt.

f725758b89 KVM: PPC: Book3S HV: Use OPAL XICS emulation on POWER9
POWER9 includes a new interrupt controller, called XIVE, which is
quite different from the XICS interrupt controller on POWER7 and
POWER8 machines.  KVM-HV accesses the XICS directly in several places
in order to send and clear IPIs and handle interrupts from PCI
devices being passed through to the guest.

In order to make the transition to XIVE easier, OPAL firmware will
include an emulation of XICS on top of XIVE.  Access to the emulated
XICS is via OPAL calls.  The one complication is that the EOI
(end-of-interrupt) function can now return a value indicating that
another interrupt is pending; in this case, the XIVE will not signal
an interrupt in hardware to the CPU, and software is supposed to
acknowledge the new interrupt without waiting for another interrupt
to be delivered in hardware.

This adapts KVM-HV to use the OPAL calls on machines where there is
no XICS hardware.  When there is no XICS, we look for a device-tree
node with "ibm,opal-intc" in its compatible property, which is how
OPAL indicates that it provides XICS emulation.

In order to handle the EOI return value, kvmppc_read_intr() has
become kvmppc_read_one_intr(), with a boolean variable passed by
reference which can be set by the EOI functions to indicate that
another interrupt is pending.  The new kvmppc_read_intr() keeps
calling kvmppc_read_one_intr() until there are no more interrupts
to process.  The return value from kvmppc_read_intr() is the
largest non-zero value of the returns from kvmppc_read_one_intr().

7c5b06cadf KVM: PPC: Book3S HV: Adapt TLB invalidations to work on POWER9
POWER9 adds new capabilities to the tlbie (TLB invalidate entry)
and tlbiel (local tlbie) instructions.  Both instructions get a
set of new parameters (RIC, PRS and R) which appear as bits in the
instruction word.  The tlbiel instruction now has a second register
operand, which contains a PID and/or LPID value if needed, and
should otherwise contain 0.

This adapts KVM-HV's usage of tlbie and tlbiel to work on POWER9
as well as older processors.  Since we only handle HPT guests so
far, we need RIC=0 PRS=0 R=0, which ends up with the same instruction
word as on previous processors, so we don't need to conditionally
execute different instructions depending on the processor.

The local flush on first entry to a guest in book3s_hv_rmhandlers.S
is a loop which depends on the number of TLB sets.  Rather than
using feature sections to set the number of iterations based on
which CPU we're on, we now work out this number at VM creation time
and store it in the kvm_arch struct.  That will make it possible to
get the number from the device tree in future, which will help with
compatibility with future processors.

Since mmu_partition_table_set_entry() does a global flush of the
whole LPID, we don't need to do the TLB flush on first entry to the
guest on each processor.  Therefore we don't set all bits in the
tlb_need_flush bitmap on VM startup on POWER9.

This adds code to handle two new guest-accessible special-purpose
registers on POWER9: TIDR (thread ID register) and PSSCR (processor
stop status and control register).  They are context-switched
between host and guest, and the guest values can be read and set
via the one_reg interface.

The PSSCR contains some fields which are guest-accessible and some
which are only accessible in hypervisor mode.  We only allow the
guest-accessible fields to be read or set by userspace.

9d66195807 powerpc/64: Provide functions for accessing POWER9 partition table
POWER9 requires the host to set up a partition table, which is a
table in memory indexed by logical partition ID (LPID) which
contains the pointers to page tables and process tables for the
host and each guest.

This factors out the initialization of the partition table into
a single function.  This code was previously duplicated between
hash_utils_64.c and pgtable-radix.c.

This provides a function for setting a partition table entry,
which is used in early MMU initialization, and will be used by
KVM whenever a guest is created.  This function includes a tlbie
instruction which will flush all TLB entries for the LPID and
all caches of the partition table entry for the LPID, across the
system.

This also moves a call to memblock_set_current_limit(), which was
in radix_init_partition_table(), but has nothing to do with the
partition table.  By analogy with the similar code for hash, the
call gets moved to near the end of radix__early_init_mmu().  It
now gets called when running as a guest, whereas previously it
would only be called if the kernel is running as the host.

6654c9368a powerpc/eeh: Refactor EEH PE reset functions
eeh_pe_reset and eeh_reset_pe are two different functions in the same
file which do mostly the same thing.  Not only is this confusing, but
potentially causes disrepancies in functionality, notably eeh_reset_pe
as it does not check return values for failure.

Refactor this into the following:

 - eeh_pe_reset(): stays as is, performs a single operation, exported
 - eeh_pe_reset_full(): new, full reset process that calls eeh_pe_reset()
 - eeh_reset_pe(): removed and replaced by eeh_pe_reset_full()
 - eeh_reset_pe_once(): removed

0d808df06a KVM: PPC: Book3S HV: Save/restore XER in checkpointed register state
When switching from/to a guest that has a transaction in progress,
we need to save/restore the checkpointed register state.  Although
XER is part of the CPU state that gets checkpointed, the code that
does this saving and restoring doesn't save/restore XER.

This fixes it by saving and restoring the XER.  To allow userspace
to read/write the checkpointed XER value, we also add a new ONE_REG
specifier.

The visible effect of this bug is that the guest may see its XER
value being corrupted when it uses transactions.

This keeps a per vcpu cache for recently page faulted MMIO entries.
On a page fault, if the entry exists in the cache, we can avoid some
time-consuming paths, for example, looking up HPT, locking HPTE twice
and searching mmio gfn from memslots, then directly call
kvmppc_hv_emulate_mmio().

In current implenment, we limit the size of cache to four. We think
it's enough to cover the high-frequency MMIO HPTEs in most case.
For example, considering the case of using virtio device, for virtio
legacy devices, one HPTE could handle notifications from up to
1024 (64K page / 64 byte Port IO register) devices, so one cache entry
is enough; for virtio modern devices, we always need one HPTE to handle
notification for each device because modern device would use a 8M MMIO
register to notify host instead of Port IO register, typically the
system's configuration should not exceed four virtio devices per
vcpu, four cache entry is also enough in this case. Of course, if needed,
we could also modify the macro to a module parameter in the future.

8f272a5dd6 powerpc/pseries: Move CMO code from plapr_wrappers.h to platforms/pseries
Currently there's some CMO (Cooperative Memory Overcommit) code, in
plpar_wrappers.h. Some of it is #ifdef CONFIG_PSERIES and some of it
isn't. The end result being if a file includes plpar_wrappers.h it won't
build with CONFIG_PSERIES=n.

Fix it by moving the CMO code into platforms/pseries. The two hcall
wrappers can just be moved into their only caller, cmm.c, and the
accessors can go in pseries.h.

Note we need the accessors because cmm.c can be built as a module, so
there needs to be a split between the built-in code vs the module, and
that's achieved by using those accessors.

79ab11cdb9 locking/core: Introduce cpu_relax_yield()
For spinning loops people do often use barrier() or cpu_relax().
For most architectures cpu_relax and barrier are the same, but on
some architectures cpu_relax can add some latency.
For example on power,sparc64 and arc, cpu_relax can shift the CPU
towards other hardware threads in an SMT environment.
On s390 cpu_relax does even more, it uses an hypercall to the
hypervisor to give up the timeslice.
In contrast to the SMT yielding this can result in larger latencies.
In some places this latency is unwanted, so another variant
"cpu_relax_lowlatency" was introduced. Before this is used in more
and more places, lets revert the logic and provide a cpu_relax_yield
that can be called in places where yielding is more important than
latency. By default this is the same as cpu_relax on all architectures.

6b243fcfb5 powerpc/64: Simplify adaptation to new ISA v3.00 HPTE format
This changes the way that we support the new ISA v3.00 HPTE format.
Instead of adapting everything that uses HPTE values to handle either
the old format or the new format, depending on which CPU we are on,
we now convert explicitly between old and new formats if necessary
in the low-level routines that actually access HPTEs in memory.
This limits the amount of code that needs to know about the new
format and makes the conversions explicit.  This is OK because the
old format contains all the information that is in the new format.

This also fixes operation under a hypervisor, because the H_ENTER
hypercall (and other hypercalls that deal with HPTEs) will continue
to require the HPTE value to be supplied in the old format.  At
present the kernel will not boot in HPT mode on POWER9 under a
hypervisor.

This fixes and partially reverts commit 50de596de8
("powerpc/mm/hash: Add support for Power9 Hash", 2016-04-29).

5246adec59 powerpc/pseries: Use H_CLEAR_HPT to clear MMU hash table during kexec
An hcall was recently added that does exactly what we need during kexec
- it clears the entire MMU hash table, ignoring any VRMA mappings.

Try it and fall back to the old method if we get a failure.

On a POWER8 box with 5TB of memory, this reduces the time it takes to
kexec a new kernel from from 4 minutes to 1 minute.

f4329f2ecb powerpc/64s: Reduce exception alignment
Exception handlers are aligned to 128 bytes (L1 cache) on 64s, which is
overkill. It can reduce the icache footprint of any individual exception
path. However taken as a whole, the expansion in icache footprint seems
likely to be counter-productive and cause more total misses.

Create IFETCH_ALIGN_SHIFT/BYTES, which should give optimal ifetch
alignment with much more reasonable alignment. This saves 1792 bytes
from head_64.o text with an allmodconfig build.

Other subarchitectures should define appropriate IFETCH_ALIGN_SHIFT
values if this becomes more widely used.

e6740ae631 powerpc: Fix exception vector build with 2.23 era binutils
The changes to use gas sections for constructing the exception vectors
causes a build break when using binutils 2.23:

  arch/powerpc/kernel/exceptions-64s.S:770: Error: operand out of range
  (0xffffffffffff8100 is not between 0x0000000000000000 and 0x000000000000ffff)

And so on.

Reported by Hugh with binutils-2.23.2-8.1.4.ppc64 from openSUSE 13.1 and
also Naveen & Denis using 2.23.52.0.1-26.el7 from RHEL 7. Strangely
binutils 2.22 (what I test with) is not affected.

This is caused by the use of @l in LOAD_HANDLER(). The @l was only
recently added in commit a24553dd02 ("powerpc/pseries: Remove
unnecessary syscall trampoline").

Luckily the gas section changes split out the LOAD_SYSCALL_HANDLER()
macro, which means we actually *don't* need to use @l in LOAD_HANDLER()
any more, only in LOAD_SYSCALL_HANDLER().

So drop the @l from LOAD_HANDLER().

f23ed166f2 powerpc/64s: Fix system reset interrupt winkle wakeups
Wakeups from winkle set the low bit of the HSPRG0 register, to
distinguish it from other sleep states. This is also the PACA pointer.
The system reset exception handler fails to mask this bit away before
using this value before using it as the PACA pointer.

Fix this by adding a new type of exception prolog macro where we already
have the PACA set in r13, and have the system reset vector mask it out.
The winkle wakeup handler will store the masked value back into HSPRG0.

fb479e44a9 powerpc/64s: relocation, register save fixes for system reset interrupt
This patch does a couple of things. First of all, powernv immediately
explodes when running a relocated kernel, because the system reset
exception for handling sleeps does not do correct relocated branches.

Secondly, the sleep handling code trashes the condition and cfar
registers, which we would like to preserve for debugging purposes (for
non-sleep case exception).

This patch changes the exception to use the standard format that saves
registers before any tests or branches are made. It adds the test for
idle-wakeup as an "extra" to break out of the normal exception path.
Then it branches to a relocated idle handler that calls the various
idle handling functions.

After this patch, POWER8 CPU simulator now boots powernv kernel that is
running at non-zero.

bd77c44986 powerpc/mm/radix: Use tlbiel only if we ever ran on the current cpu
Before this patch, we used tlbiel, if we ever ran only on this core.
That was mostly derived from the nohash usage of the same. But is
incorrect, the ISA 3.0 clarifies tlbiel such that:

"All TLB entries that have all of the following properties are made
invalid on the thread executing the tlbiel instruction"

ie. tlbiel only invalidates TLB entries on the current thread. So if the
mm has been used on any other thread (aka. cpu) then we must broadcast
the invalidate.

This bug could lead to invalid TLB entries if a program runs on multiple
threads of a core.

Hence use tlbiel, if we only ever ran on only the current cpu.

80f23935ca powerpc: Convert cmp to cmpd in idle enter sequence
PowerPC's "cmp" instruction has four operands. Normally people write
"cmpw" or "cmpd" for the second cmp operand 0 or 1. But, frequently
people forget, and write "cmp" with just three operands.

With older binutils this is silently accepted as if this was "cmpw",
while often "cmpd" is wanted. With newer binutils GAS will complain
about this for 64-bit code. For 32-bit code it still silently assumes
"cmpw" is what is meant.

In this instance the code comes directly from ISA v2.07, including the
cmp, but cmpd is correct. Backport to stable so that new toolchains can
build old kernels.

1e76609cc1 powerpc: implement arch_reserved_kernel_pages
Currently significant amount of memory is reserved only in kernel booted
to capture kernel dump using the fa_dump method.

Kernels compiled with CONFIG_DEFERRED_STRUCT_PAGE_INIT will initialize
only certain size memory per node.  The certain size takes into account
the dentry and inode cache sizes.  Currently the cache sizes are
calculated based on the total system memory including the reserved
memory.  However such a kernel when booting the same kernel as fadump
kernel will not be able to allocate the required amount of memory to
suffice for the dentry and inode caches.  This results in crashes like

Hence only implement arch_reserved_kernel_pages() for CONFIG_FA_DUMP

ce0761419f powerpc/bpf: Implement support for tail calls
Tail calls allow JIT'ed eBPF programs to call into other JIT'ed eBPF
programs. This can be achieved either by:
(1) retaining the stack setup by the first eBPF program and having all
subsequent eBPF programs re-using it, or,
(2) by unwinding/tearing down the stack and having each eBPF program
deal with its own stack as it sees fit.

To ensure that this does not create loops, there is a limit to how many
tail calls can be done (currently 32). This requires the JIT'ed code to
maintain a count of the number of tail calls done so far.

Approach (1) is simple, but requires every eBPF program to have (almost)
the same prologue/epilogue, regardless of whether they need it. This is
inefficient for small eBPF programs which may not sometimes need a
prologue at all. As such, to minimize impact of tail call
implementation, we use approach (2) here which needs each eBPF program
in the chain to use its own prologue/epilogue. This is not ideal when
many tail calls are involved and when all the eBPF programs in the chain
have similar prologue/epilogue. However, the impact is restricted to
programs that do tail calls. Individual eBPF programs are not affected.

We maintain the tail call count in a fixed location on the stack and
updated tail call count values are passed in through this. The very
first eBPF program in a chain sets this up to 0 (the first 2
instructions). Subsequent tail calls skip the first two eBPF JIT
instructions to maintain the count. For programs that don't do tail
calls themselves, the first two instructions are NOPs.

5d176f751e powerpc: tm: Enable transactional memory (TM) lazily for userspace
Currently the MSR TM bit is always set if the hardware is TM capable.
This adds extra overhead as it means the TM SPRS (TFHAR, TEXASR and
TFAIR) must be swapped for each process regardless of if they use TM.

For processes that don't use TM the TM MSR bit can be turned off
allowing the kernel to avoid the expensive swap of the TM registers.

A TM unavailable exception will occur if a thread does use TM and the
kernel will enable MSR_TM and leave it so for some time afterwards.

dc3106690b powerpc: tm: Always use fp_state and vr_state to store live registers
There is currently an inconsistency as to how the entire CPU register
state is saved and restored when a thread uses transactional memory
(TM).

Using transactional memory results in the CPU having duplicated
(almost) all of its register state. This duplication results in a set
of registers which can be considered 'live', those being currently
modified by the instructions being executed and another set that is
frozen at a point in time.

On context switch, both sets of state have to be saved and (later)
restored. These two states are often called a variety of different
things. Common terms for the state which only exists after the CPU has
entered a transaction (performed a TBEGIN instruction) in hardware are
'transactional' or 'speculative'.

Between a TBEGIN and a TEND or TABORT (or an event that causes the
hardware to abort), regardless of the use of TSUSPEND the
transactional state can be referred to as the live state.

The second state is often to referred to as the 'checkpointed' state
and is a duplication of the live state when the TBEGIN instruction is
executed. This state is kept in the hardware and will be rolled back
to on transaction failure.

Currently all the registers stored in pt_regs are ALWAYS the live
registers, that is, when a thread has transactional registers their
values are stored in pt_regs and the checkpointed state is in
ckpt_regs. A strange opposite is true for fp_state/vr_state. When a
thread is non transactional fp_state/vr_state holds the live
registers. When a thread has initiated a transaction fp_state/vr_state
holds the checkpointed state and transact_fp/transact_vr become the
structure which holds the live state (at this point it is a
transactional state).

This method creates confusion as to where the live state is, in some
circumstances it requires extra work to determine where to put the
live state and prevents the use of common functions designed (probably
before TM) to save the live state.

With this patch pt_regs, fp_state and vr_state all represent the
same thing and the other structures [pending rename] are for
checkpointed state.

d11994314b powerpc: signals: Stop using current in signal code
Much of the signal code takes a pt_regs on which it operates. Over
time the signal code has needed to know more about the thread than
what pt_regs can supply, this information is obtained as needed by
using 'current'.

This approach is not strictly incorrect however it does mean that
there is now a hard requirement that the pt_regs being passed around
does belong to current, this is never checked. A safer approach is for
the majority of the signal functions to take a task_struct from which
they can obtain pt_regs and any other information they need. The
caveat that the task_struct they are passed must be current doesn't go
away but can more easily be checked for.

Functions called from outside powerpc signal code are passed a pt_regs
and they can confirm that the pt_regs is that of current and pass
current to other functions, furthurmore, powerpc signal functions can
check that the task_struct they are passed is the same as current
avoiding possible corruption of current (or the task they are passed)
if this assertion ever fails.

bb85fb5803 powerpc: During context switch, check before setting mm_cpumask
During context switch, switch_mm() sets our current CPU in mm_cpumask.
We can avoid this atomic sequence in most cases by checking before
setting the bit.

Testing on a POWER8 using our context switch microbenchmark:

Performance improves 2%.

57f266497d powerpc: Use gas sections for arranging exception vectors
Use assembler sections of fixed size and location to arrange the 64-bit
Book3S exception vector code (64-bit Book3E also uses it in head_64.S
for 0x0..0x100).

This allows better flexibility in arranging exception code and hiding
unimportant details behind macros.

Gas sections can be a bit painful to use this way, mainly because the
assembler does not know where they will be finally linked. Taking
absolute addresses requires a bit of trickery for example, but it can
be hidden behind macros for the most part.

Generated code is mostly the same except locations, offsets, alignments.

The "+ 0x2" is only required for the trap number / kvm exit number,
which gets loaded as a constant into a register.

Previously, code also used + 0x2 for label names, but we changed to
using "H" to distinguish HV case for that. Remove the last vestiges
of that.

__after_prom_start is taking absolute address of a label in another
fixed section. Newer toolchains seemed to compile this okay, but older
ones do not. FIXED_SYMBOL_ABS_ADDR is more foolproof, it just takes an
additional line to define.

da2bc4644c powerpc/64s: Add new exception vector macros
Create arch/powerpc/include/asm/head-64.h with macros that specify
an exception vector (name, type, location), which will be used to
label and lay out exceptions into the object file.

Naming is moved out of exception-64s.h, which is used to specify the
implementation of exception handlers.

objdump of generated code in exception vectors is unchanged except for
names. Alignment directives scattered around are annoying, but done
this way so that disassembly can verify identical instruction
generation before and after patch. These get cleaned up in future
patch.

We change the way KVMTEST works, explicitly passing EXC_HV or EXC_STD
rather than overloading the trap number. This removes the need to have
SOFTEN values for the overloaded trap numbers, eg. 0x502.

2e5bbb5461 KVM: PPC: Book3S HV: Migrate pinned pages out of CMA
When PCI Device pass-through is enabled via VFIO, KVM-PPC will
pin pages using get_user_pages_fast(). One of the downsides of
the pinning is that the page could be in CMA region. The CMA
region is used for other allocations like the hash page table.
Ideally we want the pinned pages to be from non CMA region.

This patch (currently only for KVM PPC with VFIO) forcefully
migrates the pages out (huge pages are omitted for the moment).
There are more efficient ways of doing this, but that might
be elaborate and might impact a larger audience beyond just
the kvm ppc implementation.

The magic is in new_iommu_non_cma_page() which allocates the
new page from a non CMA region.

I've tested the patches lightly at my end. The full solution
requires migration of THP pages in the CMA region. That work
will be done incrementally on top of this.

This supports PCI surprise hotplug. The design is highlighted as
below:

   * The PCI slot's surprise hotplug capability is exposed through
     device node property "ibm,slot-surprise-pluggable", meaning
     PCI surprise hotplug will be disabled if skiboot doesn't support
     it yet.
   * The interrupt because of presence or link state change is raised
     on surprise hotplug event. One event is allocated and queued to
     the PCI slot for workqueue to pick it up and process in serialized
     fashion. The code flow for surprise hotplug is same to that for
     managed hotplug except: the affected PEs are put into frozen state
     to avoid unexpected EEH error reporting in surprise hot remove path.

The MMCR2 register is available twice, one time with number 785
(privileged access), and one time with number 769 (unprivileged,
but it can be disabled completely). In former times, the Linux
kernel was using the unprivileged register 769 only, but since
commit 8dd75ccb57 ("powerpc: Use privileged SPR number
for MMCR2"), it uses the privileged register 785 instead.
The KVM-PR code then of course also switched to use the SPR 785,
but this is causing older guest kernels to crash, since these
kernels still access 769 instead. So to support older kernels
with KVM-PR again, we have to support register 769 in KVM-PR, too.

88b02cf97b KVM: PPC: Book3S: Treat VTB as a per-subcore register, not per-thread
POWER8 has one virtual timebase (VTB) register per subcore, not one
per CPU thread.  The HV KVM code currently treats VTB as a per-thread
register, which can lead to spurious soft lockup messages from guests
which use the VTB as the time source for the soft lockup detector.
(CPUs before POWER8 did not have the VTB register.)

For HV KVM, this fixes the problem by making only the primary thread
in each virtual core save and restore the VTB value.  With this,
the VTB state becomes part of the kvmppc_vcore structure.  This
also means that "piggybacking" of multiple virtual cores onto one
subcore is not possible on POWER8, because then the virtual cores
would share a single VTB register.

PR KVM emulates a VTB register, which is per-vcpu because PR KVM
has no notion of CPU threads or SMT.  For PR KVM we move the VTB
state into the kvmppc_vcpu_book3s struct.

834e5a6921 powerpc/8xx: use SPRN_EIE and SPRN_EID to enable/disable interrupts
The 8xx has two special registers called EID (External Interrupt
Disable) and EIE (External Interrupt Enable) for clearing/setting
EE in MSR. It avoids the three instructions set mfmsr/ori/mtmsr or
mfmsr/rlwinm/mtmsr and it avoids using a general register.

We just have to write something in the special register to change MSR EE
bit. So we write r0 into the register, regardless of r0 value.

Writing to one of those two special registers also set the MSR RI bit,
but this bit is only unset during beginning of exception prolog and end
of exception epilog. When executing C-functions MSR RI is always set.

a24553dd02 powerpc/pseries: Remove unnecessary syscall trampoline
When we originally added the ability to split the exception vectors from
the kernel (commit 1f6a93e4c3 ("powerpc: Make it possible to move the
interrupt handlers away from the kernel" 2008-09-15)), the LOAD_HANDLER() macro
used an addi instruction to compute the offset of the common handler
from the kernel base address.

Using addi meant the handler had to be within 32K of the kernel base
address, due to the addi instruction taking a signed immediate value.
That necessitated creating a trampoline for the system call handler,
because system_call_common (in entry64.S) is not linked within 32K of
the kernel base address.

Later in commit 61e2390ede ("powerpc: Make load_hander handle upto 64k
offset" 2012-11-15) we changed LOAD_HANDLER to take a 64K offset, by
changing it to use ori.

Although system_call_common is not in head_64.S or exceptions-64s.S, it
is included in head-y, which causes it to be linked early in the kernel
text, so in practice it ends up below 64K. Additionally if it can't be
placed below 64K the linker will fail to build with a "relocation
truncated to fit" error.

So remove the trampoline.

Newer toolchains are able to work out that the ori in LOAD_HANDLER only
takes a 16 bit offset, and so they generate a 16 bit relocation. Older
toolchains (binutils 2.22 at least) are not so smart, so we have to add
the @l annotation to tell the assembler to generate a 16 bit relocation.

fe036a0605 powerpc/64/kexec: Fix MMU cleanup on radix
Just using the hash ops won't work anymore since radix will have
NULL in there. Instead create an mmu_cleanup_all() function which
will do the right thing based on the MMU mode.

For Radix, for now I clear UPRT and the PTCR, effectively switching
back to Radix with no partition table setup.

Currently set it to NULL on BookE thought it might be a good idea
to wipe the TLB there (Scott ?)

ef24ba7091 powerpc: Remove all usages of NO_IRQ
NO_IRQ has been == 0 on powerpc for just over ten years (since commit
0ebfff1491 ("[POWERPC] Add new interrupt mapping core and change
platforms to use it")). It's also 0 on most other arches.

Although it's fairly harmless, every now and then it causes confusion
when a driver is built on powerpc and another arch which doesn't define
NO_IRQ. There's at least 6 definitions of NO_IRQ in drivers/, at least
some of which are to work around that problem.

So we'd like to remove it. This is fairly trivial in the arch code, we
just convert:

    if (irq == NO_IRQ)	to	if (!irq)
    if (irq != NO_IRQ)	to	if (irq)
    irq = NO_IRQ;	to	irq = 0;
    return NO_IRQ;	to	return 0;

And a few other odd cases as well.

At least for now we keep the #define NO_IRQ, because there is driver
code that uses NO_IRQ and the fixes to remove those will go via other
trees.

Note we also change some occurrences in PPC sound drivers, drivers/ps3,
and drivers/macintosh.

bea2dccccf powerpc: Don't change the section in _GLOBAL()
Currently the _GLOBAL() macro unilaterally sets the assembler section to
".text" at the start of the macro. This is rude as the caller may be
using a different section.

So let the caller decide which section to emit the code into. On big
endian we do need to switch to the ".opd" section to emit the OPD, but
do that with pushsection/popsection, thereby leaving the original
section intact.

I verified that the order of all entries in System.map is unchanged
after this patch. The actual addresses shift around slightly so you
can't just diff the System.map.

03465f899b powerpc: Use kprobe blacklist for exception handlers
Currently we mark the C implementations of some exception handlers as
__kprobes. This has the effect of putting them in the ".kprobes.text"
section, which separates them from the rest of the text.

Instead we can use the blacklist macros to add the symbols to a
blacklist which kprobes will check. This allows the linker to move
exception handler functions close to callers and avoids trampolines in
larger kernels.

d8bced27be powerpc/fadump: Set core e_flags using kernel's ELF ABI version
Firmware Assisted Dump is a facility to dump kernel core with assistance
from firmware. As part of this process the kernel ELF ABI version is
stored in the core file.

Currently fadump.h defines this to 0 if it is not already defined. This
clashes with a define in elf.h which sets it based on the current task -
not based on the kernel's ELF ABI version.

Use the compiler-provided #define _CALL_ELF which tells us the ELF ABI
version of the kernel to set e_flags, this matches what binutils does.

Remove the definition in fadump.h, which becomes unused.

5d375199ea KVM: PPC: Book3S HV: Set server for passed-through interrupts
When a guest has a PCI pass-through device with an interrupt, it
will direct the interrupt to a particular guest VCPU.  In fact the
physical interrupt might arrive on any CPU, and then get
delivered to the target VCPU in the emulated XICS (guest interrupt
controller), and eventually delivered to the target VCPU.

Now that we have code to handle device interrupts in real mode
without exiting to the host kernel, there is an advantage to having
the device interrupt arrive on the same sub(core) as the target
VCPU is running on.  In this situation, the interrupt can be
delivered to the target VCPU without any exit to the host kernel
(using a hypervisor doorbell interrupt between threads if
necessary).

This patch aims to get passed-through device interrupts arriving
on the correct core by setting the interrupt server in the real
hardware XICS for the interrupt to the first thread in the (sub)core
where its target VCPU is running.  We do this in the real-mode H_EOI
code because the H_EOI handler already needs to look at the
emulated ICS state for the interrupt (whereas the H_XIRR handler
doesn't), and we know we are running in the target VCPU context
at that point.

We set the server CPU in hardware using an OPAL call, regardless of
what the IRQ affinity mask for the interrupt says, and without
updating the affinity mask.  This amounts to saying that when an
interrupt is passed through to a guest, as a matter of policy we
allow the guest's affinity for the interrupt to override the host's.

This is inspired by an earlier patch from Suresh Warrier, although
none of this code came from that earlier patch.

f7af5209b8 KVM: PPC: Book3S HV: Complete passthrough interrupt in host
In existing real mode ICP code, when updating the virtual ICP
state, if there is a required action that cannot be completely
handled in real mode, as for instance, a VCPU needs to be woken
up, flags are set in the ICP to indicate the required action.
This is checked when returning from hypercalls to decide whether
the call needs switch back to the host where the action can be
performed in virtual mode. Note that if h_ipi_redirect is enabled,
real mode code will first try to message a free host CPU to
complete this job instead of returning the host to do it ourselves.

Currently, the real mode PCI passthrough interrupt handling code
checks if any of these flags are set and simply returns to the host.
This is not good enough as the trap value (0x500) is treated as an
external interrupt by the host code. It is only when the trap value
is a hypercall that the host code searches for and acts on unfinished
work by calling kvmppc_xics_rm_complete.

This patch introduces a special trap BOOK3S_INTERRUPT_HV_RM_HARD
which is returned by KVM if there is unfinished business to be
completed in host virtual mode after handling a PCI passthrough
interrupt. The host checks for this special interrupt condition
and calls into the kvmppc_xics_rm_complete, which is made an
exported function for this reason.

e3c13e56a4 KVM: PPC: Book3S HV: Handle passthrough interrupts in guest
Currently, KVM switches back to the host to handle any external
interrupt (when the interrupt is received while running in the
guest). This patch updates real-mode KVM to check if an interrupt
is generated by a passthrough adapter that is owned by this guest.
If so, the real mode KVM will directly inject the corresponding
virtual interrupt to the guest VCPU's ICS and also EOI the interrupt
in hardware. In short, the interrupt is handled entirely in real
mode in the guest context without switching back to the host.

In some rare cases, the interrupt cannot be completely handled in
real mode, for instance, a VCPU that is sleeping needs to be woken
up. In this case, KVM simply switches back to the host with trap
reason set to 0x500. This works, but it is clearly not very efficient.
A following patch will distinguish this case and handle it
correctly in the host. Note that we can use the existing
check_too_hard() routine even though we are not in a hypercall to
determine if there is unfinished business that needs to be
completed in host virtual mode.

The patch assumes that the mapping between hardware interrupt IRQ
and virtual IRQ to be injected to the guest already exists for the
PCI passthrough interrupts that need to be handled in real mode.
If the mapping does not exist, KVM falls back to the default
existing behavior.

The KVM real mode code reads mappings from the mapped array in the
passthrough IRQ map without taking any lock.  We carefully order the
loads and stores of the fields in the kvmppc_irq_map data structure
using memory barriers to avoid an inconsistent mapping being seen by
the reader. Thus, although it is possible to miss a map entry, it is
not possible to read a stale value.

 array (just use the mapped array), removed the kick_all_cpus_sync()

9576730d0e KVM: PPC: select IRQ_BYPASS_MANAGER
Select IRQ_BYPASS_MANAGER for PPC when CONFIG_KVM is set.
Add the PPC producer functions for add and del producer.

[paulus@ozlabs.org - Moved new functions from book3s.c to powerpc.c
 so booke compiles; added kvm_arch_has_irq_bypass implementation.]
