synchronize_irq() to ensure that any handler routines running on other CPU
cores finish execution. Only then free the EQ buffer.
The same thing is done when we destroy a CQ which is one of the sources
generating interrupts. In the case of CQ we want to avoid completion handlers
on a CQ that was destroyed. In the case we do the same to avoid receiving
asynchronous events after the EQ has been destroyed and its buffers freed.

synchronize_irq() to ensure that any handler routines running on other CPU

rare case when we get panic on driver unload.

98226208c8 mlx4: fix race accessing page->_count
This is illegal to use atomic_set(&page->_count, ...) even if we 'own'
the page. Other entities in the kernel need to use get_page_unless_zero()
to get a reference to the page before testing page properties, so we could
loose a refcount increment.

 "Most notable changes in here:

   1) By far the biggest accomplishment, thanks to a large range of
      contributors, is the addition of multi-send for transmit.  This is
      the result of discussions back in Chicago, and the hard work of
      several individuals.

      Now, when the ->ndo_start_xmit() method of a driver sees
      skb->xmit_more as true, it can choose to defer the doorbell
      telling the driver to start processing the new TX queue entires.

      skb->xmit_more means that the generic networking is guaranteed to
      call the driver immediately with another SKB to send.

      There is logic added to the qdisc layer to dequeue multiple
      packets at a time, and the handling mis-predicted offloads in
      software is now done with no locks held.

      Finally, pktgen is extended to have a "burst" parameter that can
      be used to test a multi-send implementation.

      Several drivers have xmit_more support: i40e, igb, ixgbe, mlx4,
      virtio_net

      Adding support is almost trivial, so export more drivers to
      support this optimization soon.

      I want to thank, in no particular or implied order, Jesper
      Dangaard Brouer, Eric Dumazet, Alexander Duyck, Tom Herbert, Jamal
      Hadi Salim, John Fastabend, Florian Westphal, Daniel Borkmann,
      David Tat, Hannes Frederic Sowa, and Rusty Russell.

   2) PTP and timestamping support in bnx2x, from Michal Kalderon.

   3) Allow adjusting the rx_copybreak threshold for a driver via
      ethtool, and add rx_copybreak support to enic driver.  From
      Govindarajulu Varadarajan.

   4) Significant enhancements to the generic PHY layer and the bcm7xxx
      driver in particular (EEE support, auto power down, etc.) from
      Florian Fainelli.

   5) Allow raw buffers to be used for flow dissection, allowing drivers
      to determine the optimal "linear pull" size for devices that DMA
      into pools of pages.  The objective is to get exactly the
      necessary amount of headers into the linear SKB area pre-pulled,
      but no more.  The new interface drivers use is eth_get_headlen().
      From WANG Cong, with driver conversions (several had their own
      by-hand duplicated implementations) by Alexander Duyck and Eric
      Dumazet.

   6) Support checksumming more smoothly and efficiently for
      encapsulations, and add "foo over UDP" facility.  From Tom
      Herbert.

   7) Add Broadcom SF2 switch driver to DSA layer, from Florian
      Fainelli.

   8) eBPF now can load programs via a system call and has an extensive
      testsuite.  Alexei Starovoitov and Daniel Borkmann.

   9) Major overhaul of the packet scheduler to use RCU in several major
      areas such as the classifiers and rate estimators.  From John
      Fastabend.

  10) Add driver for Intel FM10000 Ethernet Switch, from Alexander
      Duyck.

  11) Rearrange TCP_SKB_CB() to reduce cache line misses, from Eric
      Dumazet.

  12) Add Datacenter TCP congestion control algorithm support, From
      Florian Westphal.

  13) Reorganize sk_buff so that __copy_skb_header() is significantly
      faster.  From Eric Dumazet"

fe971b95c2 net/mlx4_en: remove NETDEV_TX_BUSY
Drivers should avoid NETDEV_TX_BUSY as much as possible.

They should stop the tx queue before qdisc even tries to push another
packet, to avoid requeues.

For a driver supporting skb->xmit_more, this is likely to be a prereq
anyway, otherwise we could have a tx deadlock : We need to force a
doorbell if TX ring is full.

1556b8746e net/mlx4_en: Use the new tx_copybreak to set inline threshold
Instead of setting inline threshold using module parameter only on
driver load, use set_tunable() to set it dynamically.
No need to store the threshold per ring, using instead the netdev global
priv->prof->inline_thold
Initial value still is set using the module parameter, therefore
backward compatability is kept.

3d03641cb4 net/mlx4_en: Avoid a cache line miss in TX completion for single frag skb's
Add frag0_dma/frag0_byte_count into mlx4_en_tx_info to avoid a cache
line miss in TX completion for frames having one dma element.  (We avoid
reading back the tx descriptor)

Note this could be extended to 2/3 dma elements later, as we have free
room in mlx4_en_tx_info

Also, mlx4_en_free_tx_desc() no longer accesses skb_shinfo(). We use a
new nr_maps fields in mlx4_en_tx_info to avoid 2 or 3 cache misses.

9fab426de7 mlx4: add a new xmit_more counter
ethtool -S reports a new counter, tracking number of time doorbell
was not triggered, because skb->xmit_more was set.

$ ethtool -S eth0 | egrep "tx_packet|xmit_more"
     tx_packets: 2413288400
     xmit_more: 666121277

I merged the tso_packet false sharing avoidance in this patch as well.

5903325a64 net/mlx5_core: Identify resources by their type
This patch puts a common part as the first field of mlx5_core_qp. This field is
used to identify which resource generated an event. This is required since upcoming
new resource types such as DC targets are allocated for the same numerical space
as regular QPs and may generate the same events. By searching the resource in the
same table we can then look at the common field to identify the resource.

d29b796ada net/mlx5_core: Use hardware registers description header file
Add an auto generated header file that describes hardware registers along with
set of macros that set/get values. The macros do static checks to avoid
overflow, handle endianess, and overall provide a clean way to code commands.
Currently the header file is small and we will add structs as we make use of
the macros.
A few commands were removed from the commands enum since they are not supported
currently and will be added when support is available.

Rearrange struct mlx5_caps so it has a "gen" field to represent the current
capabilities configured for the device. Max capabilities can also be queried
from the device. Also update capabilities struct to contain more fields as per
the latest revision if firmware specification.

1daa4303b4 net/mlx4_core: Deprecate error message at ConnectX-2 cards startup to debug
ConnectX2 HCAs have max_mtu=4k and max_vl=8 vls. However, if you specify
a 4K mtu, the max_vl supported for 4K is 4 vls. The driver at startup
attempts to set a 4K mtu using the max_vl value obtained from QUERY_PORT.

Since the max_vl value is 8 vls (which is supported up to 2K mtu size),
the first attempt to set the mtl/vl port value will fail, generating
the following error message in the log:

  mlx4_core 0000:06:00.0: command 0xc failed: fw status = 0x40

The driver then tries again, using mtu=4k, vls=4, and this succeeds.

Since we do not want to have this error message always displayed at driver
start when there are ConnectX2 HCAs on the host, we deprecate the error
message for this specific command/input_modifier/opcode_modifier/fw-status
to be debug.

a7401b9cf3 net/mlx4_core: Protect QUERY_PORT wrapper from untrusted guests
The function mlx4_QUERY_PORT_wrapper implements only the
QUERY_PORT "general" case (opcode modifier = 0).

Verify that the opcode modifier is zero, and also that the
input modifier contains only the port number in bits 0..7
(all other bits should be zero).

e1c00e10e9 net/mlx4_core: New init and exit flow for mlx4_core
In the new flow, we separate the pci initialization and teardown
from the initialization and teardown of the other resources.

__mlx4_init_one handles the pci resources initialization. It then
calls mlx4_load_one to initialize the remainder of the resources.

When removing a device, mlx4_remove_one is invoked. However, now
mlx4_remove_one calls mlx4_unload_one to free all the resources except the pci

The above separation will allow us to implement 'reset flow' in the future.
It will also enable more EQs for VFs and is a pre-step to the modern API to
enable/disable SRIOV.

Also added nvfs; an integer array of size MLX4_MAX_PORTS + 1; to the mlx4_dev
struct. This new field is used to avoid parsing the num_vfs module parameter
each time the mlx4_restart_one is called.

bbb07af4cd net/mlx4_core: Don't disable SRIOV if there are active VFs
When unloading the host driver while there are VFs active on VMs,
the PF driver disabled sriov anyway, causing kernel crashes.

We now leave SRIOV enabled, to avoid that.

When the driver is reloaded, __mlx4_init_one is invoked on the PF.
It now checks to see if SRIOV is already enabled on the PF -- and
if so does not enable sriov again.

When the HCA is configured in SRIOV IB mode (that is, at least one of
the ports is IB) and the probe_vf module param isn't specified,
mlx4_init_one() failed because of the following condition:

The root cause for that is a mistake in the initialization of num_vfs_argc
and probe_vfs_argc. When num_vfs / probe_vf aren't given, their argument

f4fd40b26b mlx4: Fix mlx4 reg/unreg mac to work properly with 0-mac addresses
There is a chance that the VF mlx4 RoCE driver (mlx4_ib) may see a 0-mac
as the current default MAC address when a RoCE interface first comes up.

In this case, the RoCE driver registers the 0-mac to get its MAC index --
used in the INIT2RTR transition when it creates its proxy Q1 qp's.

If we do not allow QP1 to be created, the RoCE driver will not come up.
If we do not register the 0-mac, but simply use a random mac-index,
QP1 will attempt to send packets with an someone's else source MAC which
will get the system into more troubled.

Since a 0-mac was previously used to indicate a free slot, this leads to
errors, both when the 0-mac is registered and when it is unregistered.

The required fix is to check in addition that the slot containing the
0-mac has a reference count of zero.

Additionally, when comparing MAC addresses, need to mask out the 2 MSBs
of the u64 mac on both sides of the comparison.

Note that when the EN driver (mlx4_en) comes up, it set itself a proper
mac --> the RoCE driver gets to be notified on that and further handing
is done with the update qp command, as was added by commit 9433c18891
("IB/mlx4: Invoke UPDATE_QP for proxy QP1 on MAC changes").

b1b6b4da78 net/mlx4_en: Add mlx4_en_get_cqe helper
This function derives the base address of the CQE from the CQE size,
and calculates the real CQE context segment in it from the factor
(this is like before). Before this change the code used the factor to
calculate the base address of the CQE as well.

The factor indicates in which segment of the cqe stride the cqe information
is located. For 32-byte strides, the segment is 0, and for 64 byte strides,
the segment is 1 (bytes 32..63). Using the factor was ok as long as we had
only 32 and 64 byte strides. However, with larger strides, the factor is zero,
and so cannot be used to calculate the base of the CQE.

The helper uses the same method of CQE buffer pulling made by all other
components that reads the CQE buffer (mlx4_ib driver and libmlx4).

77507aa249 net/mlx4_core: Enable CQE/EQE stride support
This feature is intended for archs having cache line larger then 64B.

Since our CQE/EQEs are generally 64B in those systems, HW will write
twice to the same cache line consecutively, causing pipe locks due to
he hazard prevention mechanism. For elements in a cyclic buffer, writes
are consecutive, so entries smaller than a cache line should be
avoided, especially if they are written at a high rate.

Reduce consecutive writes to same cache line in CQs/EQs, by allowing the
driver to increase the distance between entries so that each will reside
in a different cache line. Until the introduction of this feature, there
were two types of CQE/EQE:

1. 32B stride and context in the [0-31] segment
2. 64B stride and context in the [32-63] segment

This feature introduces two additional types:

3. 128B stride and context in the [0-31] segment (128B cache line)
4. 256B stride and context in the [0-31] segment (256B cache line)

Modify the mlx4_core driver to query the device for the CQE/EQE cache
line stride capability and to enable that capability when the host
cache line size is larger than 64 bytes (supported cache lines are
128B and 256B).

The mlx4 IB driver and libmlx4 need not be aware of this change. The PF
context behaviour is changed to require this change in VF drivers
running on such archs.

a91c772fa0 net/mlx4: Correctly configure single ported VFs from the host
Single port VFs are seen PCI wise on both ports of the PF (we don't have
single port PFs with ConnectX). With this in mind, it's possible for
virtualization tools to try and configure a single ported VF through
the "wrong" PF port.

To handle that, we use the PF driver mapping of single port VFs to NIC
ports and adjust the port value before calling into the low level
code that does the actual VF configuration

114840c3d2 mlx4_core: Add support for secure-host and SMP firewall
Secure-host is the general term for the capability of a device
to protect itself and the subnet from malicious host software.

This is achieved by:
1. Not allowing un-trusted entities to access device configuration
   registers, directly (through pci_cr or pci_conf) and indirectly
   (through MADs).

2. Hiding M_Key from untrusted entities.

3. Preventing the modification of GUID0 by un-trusted entities

4. Not allowing drivers on untrusted hosts to receive nor to transmit
   packets over QP0 (SMP Firewall).

The secure-host capability depends on firmware handling all QP0
packets, and not passing these packets up to the driver. Any information
required by the driver for proper operation (e.g., SM lid) is passed
via events generated by the firmware while processing QP0 MADs.

Driver support mainly requires using the MAD_DEMUX FW command at startup,
where the feature is enabled/disabled through a procedure described in
the Mellanox HCA tools package.

e630664c83 mlx4_core: Add helper functions to support MR re-registration
Add few helper functions to support a mechanism of getting an MPT,
modifying it and updating the HCA with the modified object.

The code takes 2 paths, one for directly changing the MPT (and
sometimes its related MTTs) and another one which queries the MPT and
updates the HCA via fw command SW2HW_MPT. The first path is used in
native mode; the second path is slower and is used only in SRIOV.

4d2f9bbb65 mlx5: Adjust events to use unsigned long param instead of void *
In the event flow, we currently pass only a port number in the
void *data argument.  Rather than pass a pointer to the event handlers,
we should use an "unsigned long" parameter, and pass the port number
value directly.

In the future, if necessary for some events, we can use the unsigned long
parameter to pass a pointer.

9603b61de1 mlx5: Move pci device handling from mlx5_ib to mlx5_core
In preparation for a new mlx5 device which is VPI (i.e., ports can be
either IB or ETH), move the pci device functionality from mlx5_ib
to mlx5_core.

This involves the following changes:
1. Move mlx5_core_dev struct out of mlx5_ib_dev. mlx5_core_dev
   is now an independent structure maintained by mlx5_core.
   mlx5_ib_dev now has a pointer to that struct.
   This requires changing a lot of places where the core_dev
   struct was accessed via mlx5_ib_dev (now, this needs to
   be a pointer dereference).
2. All PCI initializations are now done in mlx5_core. Thus,
   it is now mlx5_core which does pci_register_device (and not
   mlx5_ib, as was previously).
3. mlx5_ib now registers itself with mlx5_core as an "interface"
   driver. This is very similar to the mechanism employed for
   the mlx4 (ConnectX) driver. Once the HCA is initialized
   (by mlx5_core), it invokes the interface drivers to do
   their initializations.
4. There is a new event handler which the core registers:
   mlx5_core_event(). This event handler invokes the
   event handlers registered by the interfaces.

b94901f3ed net/mlx4_en: current_mac isn't updated in port up
When port is down dev_addr is changed (e.g. by bonding) but current_mac
is not touched. When port is up again, hash_mac is updated to dev_addr,
but current_mac isn't. This leads to inconsistency between current_mac
and mac_hash. Because of that, mlx4_en_replace_mac() fails to find
current_mac in mac_hash.

Fix is to reset current_mac to dev_addr when port is up - as we do for
mac_hash.

858e6c3210 net/mlx4_en: cq->irq_desc wasn't set in legacy EQ's
Fix a regression introduced by commit 35f6f45 ("net/mlx4_en: Don't use
irq_affinity_notifier to track changes in IRQ affinity map").
When core is started in legacy EQ's (number of IRQ's < rx rings), cq->irq_desc
was NULL.  This caused a kernel crash under heavy traffic - when having more
than rx NAPI budget completions.
Fixed to have it set for both EQ modes.

1645a54082 net/mlx4_core: Remove MCG in case it is attached to promiscuous QPs only
In B0 steering mode if promiscuous QP asks to be detached from MCG entry,
and it is the only one in this entry then the entry will never be deleted.
This is a wrong behavior since we don't want to keep those entries after
the promiscuous QP becomes non-promiscuous. Therefore remove steering
entry containing only promiscuous QP.

In current situation host is adding the promiscuous QP to all steering
entries and the default entry as well. In this case when having PV
and SR-IOV on the same setup bridge will receive all traffic that is
targeted to the other VMs. This is bad.
Solution: In SR-IOV mode host can add promiscuous QP to default entry only.
The above problem and fix are relevant for B0 steering mode only.

0d7869acb2 net/mlx4_core: Fix leakage of SW multicast entries
When removing multicast address in B0 steering mode there is
a bug in cases where there is a single QP registered for the address,
and this QP is also promiscuous. In such cases the entry wouldn't be
deleted from the SW structure representing all Ethernet MCG entries,
but would be removed in HW. This way when driver goes to remove it
from SW and HW structures the HW deletion fails.
Moreover the same index could later be used for registering
different address, which can be Infiniband.

32b333fe99 mlx4: mark napi id for gro_skb
Napi id was not marked for gro_skb, this will lead rx busy loop won't
work correctly since they stack never try to call low latency receive
method because of a zero socket napi id. Fix this by marking napi id
for gro_skb.

The transaction rate of 1 byte netperf tcp_rr gets about 50% increased
(from 20531.68 to 30610.88).

6ef07a9f36 mlx5_core: Fix possible race between mr tree insert/delete
In mlx5_core_destroy_mkey(), we must first remove the mr from the
radix tree and then destroy it.  Otherwise we might hit a race if the
key was reallocated and we attempted to insert it to the radix tree.

Also handle radix tree insert/delete failures.

fbc6daf197 net/mlx4_en: Ignore budget on TX napi polling
It is recommended that TX work not count against the quota.
The cost of TX packet liberation is a minute percentage of what it costs to
process an RX frame. Furthermore, that SKB freeing makes memory available for
other paths in the stack.

Give the TX a larger budget and be more aggressive about cleaning up the Tx
descriptors this budget could be changed using ethtool:
$ ethtool -C eth1 tx-frames-irq <budget>

Using a local copy of dev_addr in mlx4_en_set_mac() to prevent dev_addr
from being modified during error flow or when dev_addr is modified in
another context (which is another problem that is being discussed over
the mailing list [1]).
Also fixing bad naming of priv->prev_mac into priv->current_mac.

If used, payload will be decreased accordingly. For MTU of 1500

523ece889e net/mlx4_en: Fix set port ratelimit for 40GE
In 40GE we can't use the default bw units for set ratelimit (100 Mbps)
since the max is 255*100 Mbps = 25 Gbps (not suited for 40GE), thus we need 1 Gbps units.
But for 10GE 1 Gbps units might be too bruit so we use the following solution.

For user set ratelimit <= 25 Gbps:
        use 100 Mbps units * user_ratelimit (* 10).

For user set ratelimit > 25 Gbps:
        use 1 Gbps units * user_ratelimit.

For user set unlimited ratelimit (0 Gbps):
        use 1 Gbps units * MAX_RATELIMIT_DEFAULT (57)

Note: any value > 58 will damage the FW ratelimit computation, so we allow
      a max and any higher value will be pulled down to 57.

e326f2f13b net/mlx4_en: Don't configure the HW vxlan parser when vxlan offloading isn't set
The add_vxlan_port ndo driver code was wrongly testing whether HW vxlan offloads
are supported by the device instead of checking if they are currently enabled.

This causes the driver to configure the HW parser to conduct matching for vxlan
packets but since no steering rules were set, vxlan packets are dropped on RX.

Fix that by doing the right test, as done in the del_vxlan_port ndo handler.

960b1f454e net/mlx4_core: Fix the error flow when probing with invalid VF configuration
Single ported VF are currently not supported on configurations where
one or both ports are IB. When we hit this case, the relevant flow in
the driver didn't return error and jumped to the wrong label. Fix that.
