b105798fa5 sfc: Get rid of per-NIC-type phys_addr_channels and mem_map_size
EF10 functions don't have a fixed BAR size, and the minimum is not
large enough for all the queues we might want to allocate.  We have to
find out the BAR size at run-time, and therefore phys_addr_channels
and mem_map_size cannot be defined per-NIC-type.

Change efx_nic_type::mem_map_size to a function pointer which is
called to find the wanted memory map size (before probe).

Replace efx_nic_type::phys_addr_channels with efx_nic::max_channels,
to be initialised by the probe function.

df2cd8af09 sfc: Add support for MCDI v2
MCDI v2 adds a second header dword with wider command and length
fields.  It also defines extra error codes.

Change the fallback error number for unknown MCDI error codes from EIO
to EPROTO.  EIO is treated as indicating the MCDI transport has failed
and we need to reset the function, which is rather drastic.

v2 error codes and lengths don't fit into completion events, so for a
v2-capable transport, always read the response header rather then
using the event fields.

f2b0befd1d sfc: Update MCDI protocol definitions for EF10
EF10 controllers do not have shared memory for communication with the
MC; instead it reads requests and writes responses in host memory,
which allows for longer messages.  It is also responsible for all
datapath control operations and hardware resource allocation, which
requires a large number of new commands and adds more possible error
cases.  MCDI v2 extends the message header to support this.

Update the MCDI protocol definition header to include v2 lengths,
errors and messages, and a few definitions specific to the
SFC9100 family (codenames Farmingdale and Huntington) which is
the first generation of EF10.

Some messages have been extended, so adjust the code accordingly:
- The request for MC_CMD_DRV_ATTACH now includes a datapath firmware
  ID.  This is ignored by Siena but we should fill it in anyway,
  initially always specifying low-latency datapath.
- The response for MC_CMD_GET_LOOPBACK_MODES now includes a 40G
  field.  Accept shorter responses that don't include it.

5bc283e513 sfc: Translate MCDI error numbers received in events
Currently we only translate error codes in efx_mcdi_poll(), but we
also need to do so in efx_mcdi_ev_cpl().

The reason we didn't notice before is that the MC firmware error codes
are mostly taken from Unix/Linux and no translation is necessary on
most architectures.  Make sure we notice any future failure by
changing the sign of resprc (matching the kernel convention) and BUG
if it's ever positive at command completion.

237a3a3b15 net/mlx4_en: Fix handling of dma_map failure
Result of skb_frag_dma_map() and dma_map_single() wasn't checked.
Added a check and proper handling in case of failure.
Moved the mapping to the beginning of mlx4_en_xmit(), before updating
the ring data structure to make error handling easier.

When hardware gets into error state, must notify user about it.
When QP in error state no traffic will be tx'ed from the attached
tx_ring.

Driver should know how to recover from this unexpected state. I will send later
on the recovery flow, but having the print shouldn't be delayed.

0e316f1b9b net/mlx4_en: Disable global flow control when PFC enabled
Fix a bug when FC and PFC are enabled/disabled at the same time.
According to ConnectX-3 Programmer Manual these two features are mutial
exclusive.  So make sure when enabling PFC to turn off global FC and
vise versa.  Otherwise it hurts the performance.

86094f7f38 sfc: Move and rename Falcon/Siena common NIC operations
Add efx_nic_type operations for the many efx_nic functions that need
to be implemented different on EF10.  For now, change most of the
existing efx_nic_*() functions into inline wrappers.  As a later step,
we may be able to improve branch prediction for operations used on the
fast path by copying the pointers into each queue/channel structure.

Move the Falcon/Siena implementations to new file farch.c and rename
the functions and static data to use a prefix of 'efx_farch_'.

Move efx_may_push_tx_desc() to nic.h, as the EF10 TX code will also
use it.

07a251c80c qlcnic: Implement ndo_get_phys_port_id for 82xx adapter
Each function driver instance uses the MAC address of the
lowest function belonging to that physical port as a unique
port identifier. This port identifier is read and cached in
driver during probe and provided to user space through
ndo_get_phys_port_id()

e42c3d85af sfc: Refactor queue teardown sequence to allow for EF10 flush behaviour
Currently efx_stop_datapath() will try to flush our DMA queues (if DMA
is enabled), then finalise software and hardware state for each queue.
However, for EF10 we must ask the MC to finalise each queue, which
implicitly starts flushing it, and then wait for the flush events.
We therefore need to delegate more of this to the NIC type.

Combine all the hardware operations into a new NIC-type operation
efx_nic_type::fini_dmaq, and call this before tearing down the
software state and buffers for all the DMA queues.

efx_unregister_netdev() should not call efx_release_tx_buffers()
directly, as it is already done when closing the device:
efx_net_stop() -> efx_stop_all() -> efx_stop_datapath() ->
efx_fini_tx_queue() -> efx_release_tx_buffers().

(This was presumably a workaround for a race between efx_stop_all()
and the data path that has since been properly fixed.)

1840667a85 sfc: Limit scope of a Falcon A1 IRQ workaround
We unconditionally acknowledge legacy interrupts just before disabling
them.  This workaround is needed on Falcon A1 but probably not on
later chips where the legacy interrupt mechanism is different.  It was
also originally done after the IRQ handler was removed, not before.
Restore the original behaviour for Falcon A1 only by doing this
acknowledgement in the efx_nic_type::fini operation.

d829118705 sfc: Rework IRQ enable/disable
There are many problems with the current efx_stop_interrupts() and
efx_start_interrupts():

1. On Siena, it is unsafe to disable the master IRQ enable bit
(DRV_INT_EN_KER) while any IRQ sources are enabled.

2. On EF10 there is no master IRQ enable bit, so we cannot expect to
defer IRQs without tearing down event queues.  (Though I don't think
we will need to keep any event queues around while the device is down,
as we do for VFDI on Siena.)

3. synchronize_irq() only waits for a running IRQ handler to finish,
not for any propagation through IRQ controllers.  Therefore an IRQ may
still be received and handled after efx_stop_interrupts() returns.
IRQ handlers can then race with channel reallocation.

To fix this:

a. Introduce a software IRQ enable flag.  So long as this is clear,
IRQ handlers will only acknowledge IRQs and not touch the channel
structures.

b. Define a new struct efx_msi_context as the context for MSIs.  This
is never reallocated and is sufficient to find the software enable
flag and the channel structure.  It also includes the channel/IRQ
name, which was previously separated out as it must also not be
reallocated.

c. Split efx_{start,stop}_interrupts() into
efx_{,soft_}_{enable,disable}_interrupts().  The 'soft' functions
don't touch the hardware master enable flag (if it exists) and don't
reinitialise or tear down channels with the keep_eventq flag set.

0d19a540be sfc: Add GFP flags to efx_nic_alloc_buffer() and make most callers allow blocking
Most call sites for efx_nic_alloc_buffer() are part of the probe or
reconfiguration paths and can allocate with GFP_KERNEL.  A few others
should use GFP_NOIO (I think).  Only one is in atomic context and
must use the current GFP_ATOMIC.

9528b92193 sfc: Ensure MCDI buffers, but not lengths, are dword aligned
We currently require that MCDI request and response lengths are
multiples of 4 bytes, because we will copy dwords in and out of shared
memory and we want to be sure we won't read or write out of bounds.
But all we really need to know is that there is sufficient padding for
that.  Also, we should ensure that buffers are dword-aligned, as on
some architectures misaligned access will result in data corruption or
a crash.

Change the buffer type to array-of-efx_dword_t and remove the
requirement that the lengths are multiples of 4.

c5bb0e9891 sfc: Use proper macros to declare and access MCDI arrays
A few functions are using heap buffers; change them to use stack
buffers as we really don't need to resort to the heap for a 252
byte buffer in process context.

MC_CMD_MEMCPY is quite weird in that it can use inline data placed in
the request buffer after the array of records.  Thus there are two
variable-length arrays and we can't use the normal accessors for
the second.  So we have to use _MCDI_PTR() in efx_sriov_memcpy().

a649dfcb48 sfc: Fill out the set of MCDI accessors
We need to access arrays of 16-bit words and 32-bit dwords in MCDI
buffers based on the MCDI protocol definitions.

We should also be able to read and write fields within structures,
without specifying an array index each time.  So add MCDI_FIELD()
and make MCDI_ARRAY_FIELD() use it.  Also add MCDI_SET_FIELD().

Split MCDI_ARRAY_PTR() into MCDI_ARRAY_STRUCT_PTR() and
_MCDI_ARRAY_PTR(), which are currently identical but will diverge in
later changes.

Add _MCDI_DWORD() which yields an lvalue for the given dword field
and change MCDI_DWORD(), MCDI_SET_DWORD() and MCDI_QWORD() to use it.

Fold the rather trivial MCDI_PTR2() into MCDI_PTR() and _MCDI_DWORD().

Remove MCDI_SET_DWORD2() and MCDI_QWORD2().  MCDI_DWORD2() should also
go, but it still has one user which we'll get rid of later.

f3851b0acc sfc: Fix lookup of default RX MAC filters when steered using ethtool
commit 385904f819 ('sfc: Don't use
efx_filter_{build,hash,increment}() for default MAC filters') used the
wrong name to find the index of default RX MAC filters at insertion/
update time.  This could result in memory corruption and would in any
case silently fail to update the filter.

d8a64420eb net: cpsw: Add support for wake-on-lan for cpsw
Some phy's can be configured to enable wake on lan (e.g. at803x or marvell 88E1318S).
There is no way how to enable wol on CPSW with such connected phys. This patch
adds this support. It is provided by calling the phy's related code.

Tested on board with at8030x connected phy. Wol interrupt line is
connected to GPIO0 on am335x.

Use devm_ioremap_resource instead of devm_request_and_ioremap.

This was done using the semantic patch
scripts/coccinelle/api/devm_ioremap_resource.cocci

The relevant call to platform_get_resource was manually moved down to the
call to devm_ioremap_resource.

The register layout is a SoC characteristic, so it's wrong that it's stored
in the otherwise board specific platform data. Add 'register_type' field to

8f9d513803 r8169: remember WOL preferences on driver load
Do not clear Broadcast/Multicast/Unicast Wake Flag or LanWake in
Config5. This is necessary to preserve WOL state when the driver is
loaded. Although the r8168 vendor driver does not write Config5 (it has
been commented out), Hayes Wang from Realtek said that masking bits like
this is more sensible.

Add the ndo_busy_poll handler. Use skb_mark_napi_id to mark receive
packets with the napi id. For each slice, use per-slice spinlock and
state variable to ensure that only one handler processes receive
pacekts.

Locking, statistics counters, and the ndo_busy_poll handler follow
those in the ixgbe driver. So, credit goes to that driver's authors.

c196ce05cf pch_gbe: ethtool cannot change parameters when link is down
When attempting to change e.g. the advertising mask when the link is down
ecmd->speed is -1 causing mii_ethtool_sset() to bail out.

This bug bit when connecting to a gigabit switch through a 4-pin (industrial)
cable, since link negotiation would not complete (both endpoints claimed to
be gigabit-capable, but this is not possible with only 4 pins).
Any attempt to fix this by setting autonegation to not offer 1000Mbps
failed as the setting would not be accepted while the link was still down...

Set ecmd->speed to SPEED_1000 to satisfy mii_ethtool_sset()
(the actual value of ecmd->speed doesn't matter as long as it is valid,
since a re-negotation is forced afterwards).

d765bb41fd drivers/net: enic: Generate notification of hardware crash
This patch generates a hardware crash notification (NETDEV_REBOOT)
during reset. After a hardware crash, ENIC resets all its resources
including queue pair filters programmed by USNIC. USNIC registers for
this notification, and on receiving it, reprograms the queue pair
filters.

49baea8816 bnx2x: set VF DMAE when first function has 0 supported VFs
There are possible HW configurations in which PFs will have SR-IOV capability
but will have Max VFs set to 0 - this happens when there are Multi-Function
devices where the VFs are allocated to only some of the PFs.

DMAE is configured to support VFs only if the configuring PF has supported VFs.
In case the first PF to be loaded will be one without supported VFs, it will
not configure DMAE to the VF-supporting mode. When VFs of other PFs will be
loaded later on, they will not be able to communicate with their PF.

This changes the requirement for configuring DMAE for VF-supporting mode;
If the device has SR-IOV capabilities there must be some PF that has
max supported VFs > 0, thus it will configure the DMAE for supporting VFs.

Since SR-IOV can be activated dynamically and iproute2 can be called
asynchronously, the various callbacks need a robust sanity check before
attempting to access the SR-IOV database and members since there are numerous
states in which it can find the driver (e.g., PF is down, sriov was not enabled
yet, VF is down, etc.).

In many of the states the callback result will be null pointer dereference.

07b4eb3b53 bnx2x: Change DCB context handling
After notification that DCBX configuration has ended arrived to the driver,
the driver configured the FW/HW in sleepless context.
As a result, it was possible to reach a race (mostly with CNIC registration)
in which the configuration will return a timeout, failing to set the DCBX
results correctly.

This patch moves the configuration following the DCBX end into the slowpath
RTNL task (i.e., sleepless context protected by the RTNL lock), allowing the
configuration to cope with such races.

Since commit 3deb816 "bnx2x: Add a periodic task for link PHY events"
link state changes can be detected not only via the attention flow but also
from the periodic task.
If the link state will change in such a manner (i.e., via the periodic task),
dropless flow-control will not be configured.

This patch remedies the issue, adding the missing configuration to all required
flows.

0a324f3189 net/mlx5_core: Support MANAGE_PAGES and QUERY_PAGES firmware command changes
In the previous QUERY_PAGES command version we used one command to get the
required amount of boot, init and post init pages.  The new version uses the
op_mod field to specify whether the query is for the required amount of boot,
init or post init pages. In addition the output field size for the required
amount of pages increased from 16 to 32 bits.

In MANAGE_PAGES command the input_num_entries and output_num_entries fields
sizes changed from 16 to 32 bits and the PAS tables offset changed to 0x10.

In the pages request event the num_pages field also changed to 32 bits.

In the HCA-capabilities-layout the size and location of max_qp_mcg field has
been changed to support 24 bits.

This patch isn't compatible with firmware versions < 5; however, it  turns out that the
first GA firmware we will publish will not support previous versions so this should be OK.

Commit d8af4dfd8 ("net/tg3: Fix kernel crash") introduced a possible
NULL pointer dereference in tg3 driver when !netdev || !netif_running(netdev)
condition is met and netdev is NULL. Then, the jump to the 'done' label
calls dev_close() with a netdevice that is NULL. Therefore, only call
dev_close() when we have a netdevice, but one that is not running.

2fdac010bd drivers/net/ethernet/via/via-velocity.c: update napi implementation
Drivers supporting NAPI should use a NAPI-specific function for receiving
packets.  Hence netif_rx is changed to netif_receive_skb.

Furthermore netif_napi_del should be used in the probe and remove function
to clean up the NAPI resource information.

23402bddf9 gianfar: Add flow control support
eTSEC has Rx and Tx flow control capabilities that may be enabled
through MACCFG1[Rx_Flow, Tx_Flow] bits.  These bits must not be set
however when eTSEC is operated in Half-Duplex mode.  Unfortunately,
the driver currently sets these bits unconditionally.
This patch adds the proper handling of the PAUSE frame capability
register bits by implementing the ethtool -A interface.  When pause
autoneg is enabled, the controller uses the phy's capability to
negotiate PAUSE frame settings with the link partner and reconfigures
its Rx_Flow and Tx_Flow settings to match the capabilities of the
link partner.  If pause autoneg is off, the PAUSE frame generation
may be forced manually (ethtool -A).  Flow control is disabled by
default now.
This implementation is inspired by the tg3 driver.

The MOXA UC-711X hardware(s) has an ethernet controller that seem
to be developed internally. The IC used is "RTL8201CP".

Since there is no public documentation, this driver is mostly the

Cleanup gfar_start_xmit()'s fast path by factoring out "redundant"
FCB insertion code (repeated gfar_add_fcb() calls and related)
and by reducing the number of if() clauses (i.e. if(fcb) checks).
Improve maintainability (e.g. there's less code and easier to read)
also by introducing do_csum and do_vlan to mark the other 2 Tx TOE
functionalities, following the same model as do_tstamp.
fcb_len may also be 0 now, to mark that Tx FCB insertion conditions
(do_csum, do_vlan, do_tstamp) have not been met.

02d88fb4fb gianfar: Fix Tx csum generation errata handling
Both [eTSEC76] and [eTSEC12] errata relate to Tx checksum generation
(for some MPC83xx and MCP8548 older revisions). They require the same
workaround: manual checksum computation and insertion, and disabling
the H/W Tx csum acceleration feature (per frame) through Tx FCB
(Frame Control Block) csum offload settings.

The workaround for [eTSEC76] needs to be fixed because it currently
fails to disable H/W Tx csum insertion via FCB. This patch fixes it
and provides a common workaround implementation for both Tx csum errata.

9e9402eb45 cnic, bnx2i: Fix bug on some bnx2x devices that don't support iSCSI
On some bnx2x devices, iSCSI is determined to be unsupported only after
firmware is downloaded.  We need to check max_iscsi_conn again after
NETDEV_UP and block iSCSI init operations.  Without this fix, iscsiadm
can hang as the firmware will not respond to the iSCSI init message.

8b09be5f17 bnx2x: Revising locking scheme for MAC configuration
On very rare occasions, repeated load/unload stress test in the presence of
our storage driver (bnx2i/bnx2fc) causes a kernel panic in bnx2x code
(NULL pointer dereference). Stack traces indicate the issue happens during MAC
configuration; thorough code review showed that indeed several races exist
in which one thread can iterate over the list of configured MACs while another
deletes entries from the same list.

This patch adds a varient on the single-writer/Multiple-reader lock mechanism -
It utilizes an already exsiting bottom-half lock, using it so that Whenever
a writer is unable to continue due to the existence of another writer/reader,
it pends its request for future deliverance.
The writer / last readers will check for the existence of such requests and
perform them instead of the original initiator.
This prevents the writer from having to sleep while waiting for the lock
to be accessible, which might cause deadlocks given the locks already
held by the writer.

Another result of this patch is that setting of Rx Mode is now made in
sleepable context - Setting of Rx Mode is made under a bottom-half lock, which
was always nontrivial for the bnx2x driver, as the HW/FW configuration requires
wait for completions.
Since sleep was impossible (due to the sleepless-context), various mechanisms
were utilized to prevent the calling thread from sleep, but the truth was that
when the caller thread (i.e, the one calling ndo_set_rx_mode()) returned, the
Rx mode was still not set in HW/FW.

bnx2x_set_rx_mode() will now overtly schedule for the Rx changes to be
configured by the sp_rtnl_task which hold the RTNL lock and is sleepable
context.

0508ad6468 net/mlx4_core: Don't give VFs MAC addresses which are derived from the PF MAC
If the user has not assigned a MAC address to a VM, then don't give it MAC which
is based on the PF one. The current derivation scheme is wrong and leads to VM
MAC collisions when the number of cards/hypervisors becomes big enough.

Instead, just give it zeros and let them figure out what to do with that.

f3286a3af8 tile: support multiple mPIPE shims in tilegx network driver
The initial driver support was for a single mPIPE shim on the chip
(as is the case for the Gx36 hardware).  The Gx72 chip has two mPIPE
shims, so we extend the driver to handle that case.

5e7a54a2a7 tile: fix panic bug in napi support for tilegx network driver
The code used to call napi_disable() in an interrupt handler
(from smp_call_function), which in turn could call msleep().
Unfortunately you can't sleep in an interrupt context.

Luckily it turns out all the NAPI support functions are
just operating on data structures and not on any deeply
per-cpu data, so we can arrange to set up and tear down all
the NAPI state on the core driving the process, and just
do the IRQ enable/disable as a smp_call_function thing.

a8eaed55f8 tile: set hw_features and vlan_features in setup
This change allows the user to configure various features of the tile
networking drivers on and off.  There is no change to the default
initialization state of either the tilegx or tilepro drivers.

Neither driver needs the ndo_fix_features or ndo_set_features callbacks,
since the generic code already handles the dependencies for
fix_features, and there is no hardware state to tweak in set_features.

cf3c4c0306 8139cp: Add dma_mapping_error checking
Self explanitory dma_mapping_error addition to the 8139 driver, based on this:

It showed several backtraces arising for dma_map_* usage without checking the
return code on the mapping.  Add the check and abort the rx/tx operation if its
failed.  Untested as I have no hardware and the reporter has wandered off, but
seems pretty straightforward.

ba361cb3d4 sh_eth: r8a7790: Handle the RFE (Receive FIFO overflow Error) interrupt
The RFE interrupt is enabled for the r8a7790 but isn't handled,
resulting in the interrupts core noticing unhandled interrupts, and
eventually disabling the ethernet IRQ.

Fix it by adding RFE to the bitmask of error interrupts to be handled
for r8a7790.

cd23b14b65 mlx5_core: Implement new initialization sequence
Introduce enbale_hca and disable_hca commands to signify when the
driver starts or ceases to operate on the device.

In addition the driver will use boot and init pages count; boot pages
is required to allow firmware to complete boot commands and the other
to complete init hca.  Command interface revision is bumped to 4 to
enforce using supported firmware.

This patch breaks compatibility with old versions of firmware (< 4);
however, the first GA firmware we will publish will support version 4
so this should not be a problem.

first submission of "ixgbe: Retain VLAN filtering in promiscuous + VT
mode" had a typo, which Joe Perches pointed out and is fixed in this

Alex updates the ixgbe driver to use the generic helper pci_vfs_assigned
instead of the driver specific function ixgbe_vfs_are_assigned.

Don Skidmore provides 4 patches for ixgbe, the first being a fix for
flow control ethtool reporting.  Originally ixgbe_device_supports_autoneg_fc()
was expected to be called by only copper devices, which lead to false
information being displayed via ethtool.  Two other patches add support
for fixed fiber for SFP+ devices and the addition of a quad-port x520
adapter.  The last patch simply bumps the driver version.

Emil Tantilov provides 3 fixes for ixgbe, two of which resolve
semaphore lock issues.  The third fix resolves several issues in the
previous implementation of the SFF data dumps of SFP+ modules.

The remaining ixgbe and pci patches are from Jacob Keller.  The pci
patches exposes bus speed, link speed and bus width so that drivers
can take advantage of this information.  In addition, adds a pci function
which obtains minimum link width and speed.  Jacob also provides the
ixgbe patch to incorporate the pci function. He provides a patch that
fixes a lockdep issue created due to ixgbe_ptp_stop always running
cancel_work_sync even if the work item had not been created properly with
INIT_WORK. This issue was found and reported by Stephen Hemminger.

674c18b2ed ixgbe: clear semaphore bits on timeouts
This patch changes the error code path in ixgbe_acquire_swfw_sync() to deal
with cases where acquiring SW semaphore times out.

In cases where the SW/FW semaphore bits were set (i.e. due to a crash) the
driver will hang on load. With this patch the driver will clear
the stuck bits if the semaphore was not acquired in the allotted time.

8fecf67c7e ixgbe: fix lockdep annotation issue for ptp's work item
This patch fixes a lockdep issue created due to ixgbe_ptp_stop always running
cancel_work_sync even if the work item had not been created properly with
INIT_WORK. This is caused because ixgbe_ptp_stop did not check to actually
ensure PTP was running first. The new implementation introduces a state in the
&adapter->state field which is used to indicate that PTP is running. (This
replaces the IXGBE_FLAG2_PTP_ENABLED field). This state will use the atomic
set_bit, test_bit, and test_and_clear_bit functions. ixgbe_ptp_stop will check
to ensure that PTP was enabled, (and if not, it will not attempt to do any
cleanup work from ixgbe_ptp_init). This resolves the lockdep annotation warning
found by Stephen Hemminger

e027d1aec4 ixgbe: call pcie_get_mimimum_link to check if device has enough bandwidth
This patch uses the new pcie_get_minimum_link function to perform a check to
ensure that the adapter is hooked into a slot which is capable of providing the
necessary bandwidth. This check supersedes the original method which only
checked the current pci device. The new method is capable of determining the
minimum speed and link of an entire PCI chain.

When the mvneta driver is compiled as module, the clock is disabled before
it's loading. This will reset the registers values and all configuration
made by the bootloader.

This patch sets the "sgmii serdes configuration" register to a magical value

[PATCH] atl1c: use custom skb allocator

We had reports ( https://bugzilla.kernel.org/show_bug.cgi?id=54021 )
that using high order pages for skb allocations is problematic for atl1c

We do not know exactly what the problem is, but we suspect that crossing
4K pages is not well supported by this hardware.

Use a custom allocator, using page allocator and 2K fragments for
optimal stack behavior. We might make this allocator generic
in future kernels.

This patch fixes several issues with the previous implementation of the
SFF data dump of SFP+ modules:

- removed the __IXGBE_READ_I2C flag - I2C access locking is handled in the
  HW specific routines

- fixed the read loop to read data from ee->offset to ee->len

- the reads fail if __IXGBE_IN_SFP_INIT is set in the process - this is
  needed because on some HW I2C operations can take long time and disrupt
  the SFP and link detection process

3dcc2f4142 ixgbe: fix semaphore lock for I2C read/writes on 82598
ixgbe_read/write_i2c_phy_82598() does not hold the SWFW_SYNC
semaphore for the entire function. Instead the lock is held only
during the phy.ops.read/write_reg operations. As result when the
function is being called simultaneously the I2C read/writes can
be corrupted.

The following patch introduces the SWFW_SYNC semaphore for the
entire ixgbe_read/write_i2c_phy_82598() function. To accomplish
this I had to create 2 separate functions:

ixgbe_read_phy_reg_mdi()
ixgbe_write_phy_reg_mdi()

Those functions are identical to ixgbe_read/write_phy_reg_generic()
sans the locking, and can be used in ixgbe_read/write_i2c_phy_82598()
with the SWFW_SYNC semaphore being held.
