9a53bf54b8 b43: Fix swatch warning
Swatch reports the following warning for main.c:

  CHECK drivers/net/wireless/b43/main.c
drivers/net/wireless/b43/main.c +4115 b43_wireless_core_stop(7) warn: variable dereferenced before check 'dev'

After analysis, this is not a bug, but a false warning. Nonetheless,
a cleanup is in order to prevent some future janitor proposing
the wrong fix, as I did in my original patch.

996bc370fa b43: Relax requirement for descriptors to be in the DMA zone
When 64-bit DMA was first used, there were problems with the
BCM4311 (14e4:4311). The problem was "fixed" by using the GFP_DMA
flag in the allocation of coherent ring descriptor memory.

The original problem is now believed to have been due to bugs in
the 64-bit DMA implementation in the rest of the kernel, and that
those bugs have been fixed. Accordingly, the requirement for the
descriptors to be in the DMA zone is relaxed.

Bounce buffers are left in the DMA zone.

8b0be90c4d b43/legacy: Remove firmware IDs
This removes the "FWxx" ID strings from the b43 and b43legacy
drivers. They were once used to match a specific driver revision
to a set of firmware files. However, this is hardly useful today.
Additionally, the IDs are not updated and maintained properly, so
they might mislead users.

523b02ea23 mac80211: fix TKIP races, make API easier to use
Our current TKIP code races against itself on TX
since we can process multiple packets at the same
time on different ACs, but they all share the TX
context for TKIP. This can lead to bad IVs etc.

Also, the crypto offload helper code just obtains
the P1K/P2K from the cache, and can update it as
well, but there's no guarantee that packets are
really processed in order.

To fix these issues, first introduce a spinlock
that will protect the IV16/IV32 values in the TX
context. This first step makes sure that we don't
assign the same IV multiple times or get confused
in other ways.

Secondly, change the way the P1K cache works. I
add a field "p1k_iv32" that stores the value of
the IV32 when the P1K was last recomputed, and
if different from the last time, then a new P1K
is recomputed. This can cause the P1K computation
to flip back and forth if packets are processed
out of order. All this also happens under the new
spinlock.

Finally, because there are argument differences,
split up the ieee80211_get_tkip_key() API into
ieee80211_get_tkip_p1k() and ieee80211_get_tkip_p2k()
and give them the correct arguments.

e7c62552ec b43: HT-PHY: implement killing radio
Closed drivers kill radio right after reading radio version and MACCTL,
so it was easy to find related PHY ops:
 phy_read(0x0810) -> 0x0000
phy_write(0x0810) <- 0x0000
To find out the mask of above OP, MMIO hack was used to fake read val:
 phy_read(0x0810) -> 0xffff
phy_write(0x0810) <- 0x0000

a8e827495a b43: HT-PHY: implement switching analog
Turning it on is always done between reading PHY version and radio
version, so it was easy to find it in MMIO dumps from ndiswrapper.
Turning off is done by writing different values to the same registers.

Every PHY has some specific bit used for reading radio regs. Analyze of

FWIW, the usage of these variables is goverened by checks that match
their initializations. So, I think these are actually false warnings.
Still, I would rather avoid the warning SPAM...

3311abbbbf b43: fill PHY ctl word1 in TX header for N-PHY
This patch fixes tramissing on OFDM rates for PHYs 1 and 2. There is
still something wrong with PHYs 3+. Tests has shown decreasing of
performance on CCK rates by 1-2%, we have to live with that.
Additionaly this noticeably reduces amount of PHY errors. They were
mostly produced by auto-switching to higher rate for better
performanced, which resulted in no transmit at all and PHY errors.

7bb4568372 mac80211: make tx() operation return void
The return value of the tx operation is commonly
misused by drivers, leading to errors. All drivers
will drop frames if they fail to TX the frame, and
they must also properly manage the queues (if they
didn't, mac80211 would already warn).

Removing the ability for drivers to return a BUSY
value also allows significant cleanups of the TX
TX handling code in mac80211.

Note that this also fixes a bug in ath9k_htc, the
old "return -1" there was wrong.

42ab135fe7 b43: rename TMS defines, drop useless condition from core reset
As discussed we do not know band width at core reset time and it is not a good
idea to reset whole just to change band. So just set unconditionally 20 MHz
band width as default during core reset.

As for defines PHY clock changed to band width in specs and it makes much more
sens to call defines by band width which is self-explainable. Updated specs do
not mention 0 value, but comparing to old ones you can notice lineral relation
between PHY clock speed and band width. So it makes sense for 0x0 value to be
10 MHz band width.
