Commit c7ab5ef9bc entitled "b43: implement
short slot and basic rate handling" reduced the transmit throughput for
my BCM4311 device from 18 Mb/s to 0.7 Mb/s. The basic rate handling
portion is OK, the problem is in the short slot handling.
Prior to this change, the short slot enable/disable routines were never
called. Experimentation showed that the critical part was changing the
value at offset 0x0010 in the shared memory. This is supposed to contain
the 802.11 Slot Time in usec, but if it is changed from its initial value
of zero, performance is destroyed. On the other hand, changing the value
in the MMIO register corresponding to the Interframe Slot Time increased
performance from 18 to 22 Mb/s. A BCM4306/3 also shows dramatic
improvement of the transmit rate from 5.3 to 19.0 Mb/s.
Other changes in the patch include removal of the magic number for the
MMIO register, and allowing the slot time to be set for any PHY operating
in the 2.4 GHz band. Previously, the routine was executed only for G PHYs.

The TKIP key update callback is called from the RX path, where the driver
mutex is already locked. This results in a circular locking bug.
Avoid this by removing the lock.
Johannes noted that there is a separate bug: The callback still breaks on SDIO
hardware, because SDIO hardware access needs to sleep, but we are not allowed
to sleep in the callback due to mac80211's RCU locking.

When a TKIP key is updated, we should pass the station
pointer instead of just the address, since drivers can
use that to store their own data. We also need to pass
the virtual interface pointer.

If userencounter the "Fatal DMA Problem" with a BCM43XX device, and
still wish to use b43 as the driver, their only option is to rebuild
the kernel with CONFIG_B43_FORCE_PIO. This patch removes this option and
allows PIO mode to be selected with a load-time parameter for the module.
Note that the configuration variable CONFIG_B43_PIO is also removed.
Once the DMA problem with the BCM4312 devices is solved, this patch will
likely be reverted.

As shown in Kernel Bugzilla #14761, doing a controller restart after a
fatal DMA error does not accomplish anything other than consume the CPU
on an affected system. Accordingly, substitute a meaningful message for
the restart.

The routine b43_is_hw_radio_enabled() has long been a problem.
For PPC architecture with PHY Revision < 3, a read of the register
B43_MMIO_HWENABLED_LO will cause a CPU fault unless b43_status()
returns a value of 2 (B43_STAT_STARTED) (BUG 14181). Fixing that
results in Bug 14538 in which the driver is unable to reassociate
after resuming from hibernation because b43_status() returns 0.
The correct fix would be to determine why the status is 0; however,
I have not yet found why that happens. The correct value is found for
my device, which has PHY revision >= 3.
Returning TRUE when the PHY revision < 3 and b43_status() returns 0 fixes
the regression for 2.6.32.
This patch fixes the problem in Red Hat Bugzilla #538523.

Most of the irq_req_t typedef'd struct can be re-worked quite
easily:
(1) IRQInfo2 was unused in any case, so drop it.
(2) IRQInfo1 was used write-only, so drop it.
(3) Instance (private data to be passed to the IRQ handler):
Most PCMCIA drivers using pcmcia_request_irq() to actually
register an IRQ handler set the "dev_id" to the same pointer
as the "priv" pointer in struct pcmcia_device. Modify the two
exceptions (ipwireless, ibmtr_cs) to also work this waym and
set the IRQ handler's "dev_id" to p_dev->priv unconditionally.
(4) Handler is to be of type irq_handler_t.
(5) Handler != NULL already tells whether an IRQ handler is present.
Therefore, we do not need the IRQ_HANDLER_PRESENT flag in
irq_req_t.Attributes.

This rewrites the error handling policies in the TX status handler.
It tries to be error-tolerant as in "try hard to not crash the machine".
It won't recover from errors (that are bugs in the firmware or driver),
because that's impossible. However, it will return a more or less useful
error message and bail out. It also tries hard to use rate-limited messages
to not flood the syslog in case of a failure.

Enforce all device constraints on the descriptor memory region.
There are several constraints on the descriptor memory, as documented
in the specification. The current code does not enforce them and/or
incorrectly enforces them.
Those constraints are:
- The address limitations on 30/32bit engines, that also apply to
the skbs.
- The 4k alignment requirement on 30/32bit engines.
- The 8k alignment requirement on 64bit engines.

ops->set_tim() must be atomic, so b43 trying to acquire a mutex leads
to a kernel crash. This patch trades an easy to trigger crash in AP
mode for an unlikely race condition. According to Michael, the real
fix would be to allow set_tim() to sleep, since b43 is not the only
driver that needs to sleep in all callbacks.

core, instead of tedious, independent CIS parsing.
xirc32ps_cs.c: manf_id

b43/pcmcia.c: ConfigBase address and "Present"
smc91c92_cs.c: By default, mhz_setup() can use VERS_1 as it is stored
in struct pcmcia_device. Only some cards require workarounds, such as
reading out VERS_1 twice.

While there may be a case for a driver adding its
own bits of radiotap information, none currently
does. Also, drivers would have to copy the code
to generate the radiotap bits that now mac80211
generates. If some driver in the future needs to
add some driver-specific information I'd expect
that to be in a radiotap vendor namespace and we
can add a different way of passing such data up
and having mac80211 include it.
Additionally, rename IEEE80211_CONF_RADIOTAP to
IEEE80211_CONF_MONITOR since it's still used by
b43(legacy) to obtain per-frame timestamps.
The purpose of this patch is to simplify the RX
code in mac80211 to make it easier to add paged
skb support.

b43 allocates a bouncebuffer, if the supplied TX skb is in an invalid
memory range for DMA.
However, this is broken in that it fails to copy over some metadata to the
new skb.
This patch fixes three problems:
* Failure to adjust the ieee80211_tx_info pointer to the new buffer.
This results in a kmemcheck warning.
* Failure to copy the skb cb, which contains ieee80211_tx_info, to the new skb.
This results in breakage of various TX-status postprocessing (Rate control).
* Failure to transfer the queue mapping.
This results in the wrong queue being stopped on saturation and can result in queue overflow.

"b43: Fix PPC crash in rfkill polling on unload" fixed the bug reported
in Bugzilla No. 14181; however, it introduced a new bug. Whenever the
radio switch was turned off, it was necessary to unload and reload
the driver for it to recognize the switch again.
This patch fixes both the original bug in #14181 and the bug introduced by
the previous patch. It must be stated, however, that if there is a BCM4306/3
with an rfkill switch (not yet proven), then the driver will need an
unload/reload cycle to turn the device back on.

The DMA-API debugging facility complains about b43 mapping memory from
stack for SDIO-based cards.
Indeed, b43 currently allocates the PIO RX/TX header and tail buffers
from stack. The solution here is to use heap-allocated buffers instead.

In Bugzilla No. 14181, a PowerMac G4 crashes on ifdown or
module unload because the rfkill polling has not been stopped.
For the x86 architectures, the attempt to reach a now unmapped
register is not fatal as it is on PPC.

On SDIO the PIO data register seems to be hardwired to LE. So
the MACCTL bit has no effect on the endianness.
So also use block-I/O for the last word of the packet. block-I/O is always LE.

Don't abuse wl->current_dev in the LED work for checking whether we're
going down. Add an explicit variable.
This fixes a crash on rmmod dereferencing the wl->current_dev NULL pointer
in various other places of the driver.

The current verison of b43 uses "b43_phyop_switch_analog_generic" for A,
G and LP phys.
According to the spec, this is the wrong behaviour for the LP PHY
(see: http://bcm-v4.sipsolutions.net/802.11/PHY/Anacore )
While no problems on the x86 plattform where seen, this leads to a crash
on the BCM5354 SoC (MIPS 32 LE plattform).
This patch implements the analog switch for LP PHYs according to the
specs. It fixes the crash

This adds support for verbose runtime statistics.
It defaults to off and must be enabled in debugfs, if desired.
The first measurement may be incorrect, because statistics are not cleared
after they got enabled through debugfs.

This removes most of the b43 suspend/resume code (it's handled by mac80211)
and moves the registration of devices to the attachment phase. This is
required, because we must not register/unregister devices on suspend/resume.

This fixes a resume failure where a signal is pending on resume
so the firmware upload fails.
This removes the interruptible sleep, because we don't really need it.
In the worst case (with broken firmware) the sleep loop will take 1 second.
In the common case (working firmware), it will only take a few milliseconds.
So we don't really need to be interruptible.

This fixes a resume failure where a signal is pending on resume
so the firmware upload fails.
This removes the interruptible sleep, because we don't really need it.
In the worst case (with broken firmware) the sleep loop will take 1 second.
In the common case (working firmware), it will only take a few milliseconds.
So we don't really need to be interruptible.

Currently, when QoS-disable is requested, we would leave QoS enabled
in firmware, but only queue frames on one queue.
Change that and also tell firmware about disabled QoS, so it
completely ignores all the QoS parameters. Also don't upload the parameters,
if QoS is disabled.

Use a threaded IRQ handler to allow locking the mutex and
sleeping while executing an interrupt.
This removes usage of the irq_lock spinlock, but introduces
a new hardirq_lock, which is _only_ used for the PCI/SSB lowlevel
hard-irq handler. Sleeping busses (SDIO) will use mutex instead.

The A/G-PHY changes are fallout fixes from the enum change,
which in turn allows the LP-PHY code to be much simpler.
The antenna_to_phyctl change is a fix for a potential
existing bug that this patch may otherwise trigger.

The most common LP-PHY device, BCM4312, is now fully functional.
So, no need to say "probably won't work for you" anymore.
It's also not "for debuggers and developers only", as it is
perfectly usable for end-users now (at least for BCM4312).

-Fix a few nasty typos (b43_phy_* operations instead of b43_radio_*)
in the channel tune routines.
-Fix some typos & spec errors found by MMIO tracing.
-Optimize b43_phy_write & b43_phy_mask/set/maskset to use
only the minimal number of MMIO accesses. (Write is possible
using a single 32-bit MMIO write, while set/mask/maskset can
be done in 3 16-bit MMIOs).
-Set the default channel back to 1, as the bug forcing us to use
channel 7 is now fixed.
With this, the device comes up, scans, associates, transmits,
receives, monitors and injects on all channels - in other words,
it's fully functional. Sensitivity and TX power are still sub-optimal,
due to the lack of calibration (that's next on my list).

The Qdiv roundup routine is essentially a fixed-point
division algorithm, using only integer math.
However, the version in the specs had a major error
that has been recently fixed (a missing quotient++).
Replace Qdiv roundup with a rewritten, simplified version.

The RC calibration code has some typos - fix them.

Larry has started re-checking all current routines against a new
version of the Broadcom MIPS driver. This patch implements the first
round of changes he documented on the specs wiki.
Also remove a few FIXMEs regarding missing initial values for variables
with dynamic initial values where reading the values has been implemented.

The specs are beginning to support rev3 LP-PHYs - implement one of
the changes needed for rev3 support.
Also, in the new MIPS driver, the "Japan TX filter" was renamed to
"analog TX filter init" - however, calling it "init" is confusing,
so name it "set analog filter", with a comment for easier future
identification.

Over time, a whole bunch of drivers have come up
with their own scheme to delay the configure_filter
operation to a workqueue. To be able to simplify
things, allow configure_filter to sleep, and add
a new prepare_multicast callback that drivers that
need the multicast address list implement. This new
callback must be atomic, but most drivers either
don't care or just calculate a hash which can be
done atomically and then uploaded to the hardware
non-atomically.
A cursory look suggests that at76c50x-usb, ar9170,
mwl8k (which is actually very broken now), rt2x00,
wl1251, wl1271 and zd1211 should make use of this
new capability.

Larry has reported success getting scan data with an LP-PHY device,
so it's probably time to release LP-PHY support for testing.
Also disable 802.11a support for now, as 802.11a currently causes
the driver to panic on startup (NULL pointer dereference).

The spec had some nasty typos, and a large part of the rev0/1 BB
init procedure was also missing. Fix these.
Also make the init-time channel switch debuggable.
(The change from -EINVAL to -EIO is simply to make it possible
to distinguish the PLL charge pump error from a channel-not-found
error.)

The tkip hw support uncovered a bug in b43_write_probe_resp_template : it is
writing at the wrong shm offset, it is in the B43_SHM_SH_TKIPTSCTTAK
zone. Remove b43_write_probe_resp_template, b43_write_probe_resp_plcp
and b43_write_probe_resp_plcp because the probe response offload is
currently not supported by mac80211.

The low and high 16 bits were swapped.
It also adds a testcase for this to the chipaccess validation.

The mac80211 workqueue exists to enable mac80211 and drivers
to queue their own work on a single threaded workqueue. mac80211
takes care to flush the workqueue during suspend but we never
really had requirements on drivers for how they should use
the workqueue in consideration for suspend.
We extend mac80211 to document how the mac80211 workqueue should
be used, how it should not be used and finally move raw access to
the workqueue to mac80211 only. Drivers and mac80211 use helpers
to queue work onto the mac80211 workqueue:
* ieee80211_queue_work()
* ieee80211_queue_delayed_work()
These helpers will now warn if mac80211 already completed its
suspend cycle and someone is trying to queue work. mac80211
flushes the mac80211 workqueue prior to suspend a few times,
but we haven't taken the care to ensure drivers won't add more
work after suspend. To help with this we add a warning when
someone tries to add work and mac80211 already completed the
suspend cycle.
Drivers should ensure they cancel any work or delayed work
in the mac80211 stop() callback.

As shown in http://thread.gmane.org/gmane.linux.kernel.wireless.general/36497,
mac80211 has a bug that allows a call to the TX routine after the queues have
been stopped. This situation will only occur under extreme stress. Although
b43 does not crash when this condition occurs, it does generate a WARN_ON and
also logs a queue overrun message. This patch recognizes b43 is not at fault
and logs a message only when the most verbose debugging mode is enabled. In
the unlikely event that the queue is not stopped when the DMA queue becomes
full, then a warning is issued.
During testing of this patch with one output stream running repeated tcpperf
writes and a second running a flood ping, this routine was entered with
the DMA ring stopped about once per hour. The condition where the DMA queue is
full but the ring has not been stopped has never been seen by me.

Within mac80211, we often need to copy the rx status into
skb->cb. This is wasteful, as drivers could be building it
in there to start with. This patch changes the API so that
drivers are expected to pass the RX status in skb->cb, now
accessible as IEEE80211_SKB_RXCB(skb). It also updates all
drivers to pass the rx status in there, but only by making
them memcpy() it into place before the call to the receive
function (ieee80211_rx(_irqsafe)). Each driver can now be
optimised on its own schedule.
