6486cda6c6 [S390] qdio: convert global statistics to per-device stats
Revamp the qdio performance statistics and move them from procfs to
debugfs using the seq_file interface. Since the statistics are not
intended for the general user the removal of /proc/qdio_perf should
not surprise anyone.

The per device statistics are disabled by default, writing 1 to
/<debugfs mountpoint>/qdio/<device bus ID>/statistics enables the
statistics for the given device.

8bcd9b04fd [S390] qdio: add counter for input queue full condition
Add a counter to the qdio performance statistics that indicates that no
free buffers were left in the input queue. If the counter gets increased
it means that the qdio adapter filled all available buffers and possibly
had more buffers ready but could not transmit them.

7883097f16 [S390] qdio: remove superfluous log entries and WARN_ONs.
* Don't write debug feature log entries for sl, slsb and sbal since these
  elements can be located from the qdio_q pointer which is also logged.
* Convert WARN_ON for wrong alignment of sbal to BUG_ON.
* Remove WARN_ON's for wrong alignment of q / qib / slib since these
  alignments should be guaranteed by kmem_cache_alloc alignment /
  struct aligned attribute / __get_free_page.

d302e1a5db [S390] cio: fix channel path vary
Channel path vary is currently broken: channel paths which are varied
offline are still used by Linux. The reason for this is that:

 * the path mask indicating which paths of an I/O device can be used
   is reset by each internal I/O request
 * the logic that checks if a path group is already in its designated
   target state incorrectly interprets the result "is correctly set"
   as "is correctly set and available"

Fix this by resetting the path mask only for internal I/O requests
which affect the path mask and by correcting the pgid check logic.

ffa8d2a3e8 [S390] cio: fix drvdata usage for the console subchannel
Using dev_set_drvdata prior to device_register will force the driver core
to kmalloc its private data. Since we use this for the console subchannel
lets set the drvdata before taking the subchannels spinlock.

e7d2860b69 tree-wide: convert open calls to remove spaces to skip_spaces() lib function
Makes use of skip_spaces() defined in lib/string.c for removing leading
spaces from strings all over the tree.

It decreases lib.a code size by 47 bytes and reuses the function tree-wide:
   text    data     bss     dec     hex filename
  64688     584     592   65864   10148 (TOTALS-BEFORE)
  64641     584     592   65817   10119 (TOTALS-AFTER)

Also, while at it, if we see (*str && isspace(*str)), we can be sure to
remove the first condition (*str) as the second one (isspace(*str)) also
evaluates to 0 whenever *str == 0, making it redundant. In other words,
"a char equals zero is never a space".

Julia Lawall tried the semantic patch (http://coccinelle.lip6.fr) below,
and found occurrences of this pattern on 3 more files:

    drivers/leds/ledtrig-timer.c
    drivers/video/output.c

42d61b9b41 [S390] 3215/3270 console: remove wrong comment
When converting these two drivers from bootmem to slab allocations I
forgot to remove two comments which state that this wouldn't be
possible.
So remove them now since this caused confusion already.

New definitions are added for CEX3 device types. They will be set
in the according probe functions. CEX3 device types will be handled
in the same modules as CEX2 device types. In the first step they are

a6a5d73a56 [S390] zcrypt: special command support for cex3 exploitation
Support for special command is implemented in the AP Bus in the NQAP
function __ap_send. This is extended for a further parameter special.
When set, the special bit, in GR0 will be set. Therefor the ap_message
struct is extended for a further bit. Thus calling functions of
__ap_send can use the special parameter in ap_message to give to
__ap_send. Affected is in the first place ap_queue_message, which is
called by the actual card driver. The second part of this support is
that the card driver for the CEX3C needs to set this special bit, when
an according CPRB is sent to the driver.

f4ac1d0255 [S390] dasd: let device initialization wait for LCU setup
The first DASD that is set online for a specific logical control unit
has to do certain setup steps on the storage server to make full use
of it, for example it will enable PAV.
The features and characteristics reported by the storage server will
depend on this setup, so all other devices on the same LCU will need
to wait for the setup to be finished.

eb6e199bef [S390] dasd: improve error recovery for internal I/O
Most of the error conditions reported by a FICON storage server
indicate situations which can be recovered. Sometimes the host just
needs to retry an I/O request, but sometimes the recovery
is more complex and requires the device driver to wait, choose
a different path, etc.

The DASD device driver has a fully featured error recovery
for normal block layer I/O, but not for internal I/O request which
are for example used during the device bring up.
This can lead to situations where the IPL of a system fails because
DASD devices are not properly recognized.
This patch will extend the internal I/O handling to use the existing
error recovery procedures.

22825ab769 [S390] dasd: support DIAG access for read-only devices
When a DASD device is used with the DIAG discipline, the DIAG
initialization will indicate success or error with a respective
return code. So far we have interpreted a return code of 4 as error,
but it actually means that the initialization was successful, but
the device is read-only. To allow read-only devices to be used with
DIAG we need to accept a return code of 4 as success.

Re-initialization of the DIAG access is also part of the DIAG error
recovery. If we find that the access mode of a device has been
changed from writable to read-only while the device was in use,
we print an error message.

If we detect a busy subchannel after the driver's set_offline
callback returned in ccw_device_set_offline, the current behavior
is to unregister the device, which may lead to undesired
consequences. Change this to just quiesce the subchannel and go on
with the offline processing.

Note: This is no excuse for not fixing these drivers -
after the set_offline callback they should have no running IO!

We used to maintain a "registered" flag in our ccw_device_private
structure. This patch removes the "registered" flag and converts
all users of it to device_is_registered which has the exact same
meaning.

Note: The usage the atomic operation test_and_clear_bit is replaced
by the non-atomic if (device_is_registered()) device_del(). This
will not do harm, since we serialize calls to ccw_device_unregister
with a single-threaded workqueue.

Change the initiative to update subchannel-ccw device associations
to the subchannel: when there is an indication that the internal
association no longer reflects the current hardware state, mark
each affected subchannel as requiring attention. Once processing
reaches a subchannel, determine the correct association for that
subchannel at that time and perform the necessary device_move
operations.

This change fixes problems with the previous approach which would
leave devices in an inconsistent state when a new hardware change
occurred while a device_move was already scheduled.

The flag ZFCP_STATUS_FSFREQ_TMFUNCNOTSUPP is never set and hence can
be removed. This is a leftover from the time when zfcp had to decide
whether the target supports a "logical unit reset" or not. Nowadays,
the SCSI midlayer calls the eh_device_reset_handler or the
eh_target_reset_handler and zfcp simply maps this to a "logical unit
reset" or a "target reset".

af4de36d91 [SCSI] zfcp: Block scsi_eh thread for rport state BLOCKED
In case the SCSI error recovery starts because of a SCSI command
timeout, but then something else triggers the rport to be deleted, the
SCSI error recovery will run to the end and set the SCSI device
offline. To prevent this, call the FC transport function
fc_block_scsi_eh which waits until the rport leaves the BLOCKED state.
This guarantees that communication is possible if the rport is ONLINE,
or the SCSI devices will be removed if the rport state switches to
NOT_PRESENT.

4c571c659e [SCSI] zfcp: Update FSF error reporting
The SCSI midlayer retries commands based on the remote port state and
the command status reported by the driver. Returning
DID_TRANSPORT_DISRUPTED is a better approach, use this for reporting
FSF errors back to the SCSI midlayer.  See

There is also no need in special treatment of ABORTED commands, so
remove the ZFCP_STATUS_FSFREQ_ABORTED, the commands are then returned
with DID_TRANSPORT_DISRUPTED.

Also remove the ZFCP_STATUS_FSFREQ_RETRY: It is useless, no retry is
happening in the FSF layer and nobody checks the state of this flag.

The well-known-address (WKA) port handling code is part of the FC code
in zfcp. Move everything WKA related to the zfcp_fc files and use the
common zfcp_fc prefix for structs and functions. Drop the unused key
management service while renaming the struct, no request could ever
reach this service in zfcp and it is obsolete anyway.

8830271c48 [SCSI] zfcp: Dont fail SCSI commands when transitioning to blocked fc_rport
If an error occurs that triggers the call to fc_remote_port_delete,
ideally this call would happen before any I/O is passed back to the
SCSI midlayer through scsi_done. The SCSI midlayer will retry the
commands and fc_remote_port_chkready will return the correct status
code.  But with the delay between calling scsi_done in softirq context
and the call to fc_remote_port_delete from the workqueue, there is a
window where zfcp returns DID_ERROR. This leads to SCSI error recovery
which then leads to offline SCSI devices since all recovery actions
will fail with the rport now being blocked.

In this window, zfcp has to return DID_IMM_RETRY just as the FC
transport class would do in fc_remote_port_chkready for the blocked
fc_rport. As soon as the fc_rport is BLOCKED, fc_remote_port_chkready
will do the right thing.

Additionally, there are two more cases to catch in zfcp_scsi_queuecommand:
- After the port has been opened, the unit has to be opened. During
  this period I/O has to be retried. This can also be handled with
  DID_IMM_RETRY.
- If the access to the unit fails, but the port is good, then
  this single unit cannot be accessed and I/O to this unit has to fail
  without involving the FC transport class.

9eae07ef6b [SCSI] zfcp: Assign scheduled work to driver queue
The port_scan work was scheduled to the work_queue provided by the
kernel. This resulted on SMP systems to a likely situation that more
than one scan_work were processed in parallel. This is not required
and openes the possibility of race conditions between the removal of
invalid ports and the enqueue of just scanned ports.  This patch
synchronizes the scan_work tasks by scheduling them to adapter local
work_queue.

e881a172da [SCSI] modify change_queue_depth to take in reason why it is being called
This patch modifies scsi_host_template->change_queue_depth so that
it takes an argument indicating why it is being called. This will be
used so that if a LLD needs to do some extra processing when
handling queue fulls or later ramp ups, it can do so.

This is a simple port of the drivers setting a change_queue_depth
callback. In the patch I just have these LLDs adjust the queue depth
if the user was requesting it.

c3b4a740db qeth: rework TSO functions
The maximum TSO size OSA can handle is 15 * PAGE_SIZE. This
patch reduces gso_max_size to this value and adds some sanity
checks and statistics to the TSO implementation.
Since only layer 3 is able to do TSO move all TSO related functions

0ca8cc6fe7 s390: remove cu3088 layer for lcs and ctcm
The cu3088-driver used as common base for lcs- and ctcm-devices
makes it difficult to assign the appropriate driver to an lcs-device
or a ctcm-device. This patch eliminates the cu3088-driver and thus
the root device "cu3088". Path /sys/devices/cu3088 is replaced with
the pathes /sys/devices/lcs and /sys/devices/ctcm.

8b94c1ed4d [S390] sclp: undo quiesce handler override on resume
In a system where the ctrl-alt-del init action initiated by signal
quiesce suspends the machine the quiesce handler override for
_machine_restart, _machine_halt and _machine_power_off needs to be
undone, otherwise the override is still present in the resumed
system. The next shutdown would then load the quiesce state psw
instead of performing the correct shutdown action.

The monreader device driver doesn't set dev->driver_data to NULL after
freeing the corresponding data structure. This leads to a use after
free bug in the freeze/thaw suspend/resume functions after the device

Allow the architecture to request a normal jiffy tick when the system
goes idle and tick_nohz_stop_sched_tick is called . On s390 the hook is
used to prevent the system going fully idle if there has been an
interrupt other than a clock comparator interrupt since the last wakeup.

On s390 the HiperSockets response time for 1 connection ping-pong goes
down from 42 to 34 microseconds. The CPU cost decreases by 27%.

e8a79c9ec7 [S390] call home: fix string length handling
After copying uts->nodename to the static nodename array the static
version isn't necessarily zero termininated, since the size of the
array is one byte too short.
Afterwards doing strncat(data, nodename, strlen(nodename)); may copy
an arbitrary large amount of bytes.
Fix this by getting rid of the static array and using strncat with
proper length limit.

390dfd95c5 percpu: make misc percpu symbols unique
This patch updates misc percpu related symbols such that percpu
symbols are unique and don't clash with local symbols.  This serves
two purposes of decreasing the possibility of global percpu symbol
collision and allowing dropping per_cpu__ prefix from percpu symbols.

* drivers/crypto/padlock-aes.c: s/last_cword/paes_last_cword/

* drivers/lguest/x86/core.c: s/last_cpu/lg_last_cpu/

* drivers/s390/net/netiucv.c: rename the variable used in a macro to
  avoid clashing with percpu symbol

* arch/mn10300/kernel/kprobes.c: replace current_ prefix with cur_ for
  static variables.  Please note that percpu symbol current_kprobe
  can't be changed as it's used by generic code.

Partly based on Rusty Russell's "alloc_percpu: rename percpu vars
which cause name clashes" patch.

9e820afd0c [SCSI] zfcp: Flush SCSI registration work when adding unit
When configuring a LUN for use in zfcp, flush the SCSI work to ensure
the SCSI device has been created before returning. This means that a
configuration procedure can run these commands in a script and the
SCSI device is available immediately after the unit_add:

10d00f78e6 [SCSI] zfcp: Warn about storage devices with broken PLOGI data
After opening a remote port zfcp checks if the WWPN returned in the
PLOGI maches the WWPN of the port that should have been opened. On a
mismatch zfcp assumes that the DID just changed, queries the FC
nameserver and tries again. If the situation persists the erp will
give up.

With this strategy, if the remote port always returns the wrong PLOGI
data, the remote port will not be opened. Introduce a warning, so that
the system administrator knows why the remote port is not being opened
and to have a pointer to investigate the problem on the storage
system.

934aeb587b [SCSI] zfcp: Handle WWPN mismatch in PLOGI payload
For ports, zfcp gets the DID from the FC nameserver and tries to open
the port. If the open succeeds, zfcp compares the WWPN from the
nameserver with the WWPN in the PLOGI payload. In case of a mismatch,
zfcp assumes that the DID of the port just changed and we opened the
wrong port. This means that zfcp has to forget the DID, lookup the DID
again and retry.

This error case had a problem that zfcp forgets the DID, but never
looks up a new one, stalling the ERP in this case. Fix this by
triggering the DID lookup and properly exit from the ERP. The DID
lookup will trigger a new ERP action.

Also ensure when trying to open the port again with the new DID, first
close the open port, even in the NOESC case.

6d7c5afc89 [S390] cio: change misleading console logic
Use cio_is_console() in io_subchannel_probe to indicate that the
special handling is console specific. As long as there is no other
subchannel for which this might be true, it is misleading to speak
of "early devices". Should more of these devices be introduced,
a cleanup of all console special handling is in order anyway.

The reason for that is an error in the request queue handling. It can
happen that we fetch a request, but do not process it further because
the number of queued requests exceeds TAPEBLOCK_MIN_REQUEUE.
To fix this, we should call blk_peek_request() instead of
blk_fetch_request() in the while condition and fetch the request in
the loop body afterwards.

This bug was introduced with the patch "block: implement and enforce
request peek/start/fetch" (9934c8c045)

6fca97a958 [S390] dasd: fix race condition in resume code
There is a race while re-reading the device characteristics. After
cleaning the memory area a cqr is build which reads the device
characteristics. This may take a rather long time and the device
characteristics structure is zero during this. Now it could be
possible that the block tasklet starts working and a new cqr will be
build. The build_cp command refers to the device characteristics
structure and this may lead into a divide by zero exception.
Fix this by re-reading the device characteristics into a temporary
structur and copy the data to the original structure. Also take the
ccwdev_lock.

05d419b11f [S390] Fix memory leak in /proc/cio_ignore
There is a memory leak in /proc/cio_ignore. The iterator is allocated
in cio_ignore_proc_seq_start, but never freed in
cio_ignore_proc_seq_stop, because we cannot use the iterator
that was passed by seqfile. The seqfile interface passes the last
seen iterator to the stop function and not the first one. Since our
next function will return NULL at the end, the iter passed to
cio_ignore_proc_seq_stop is NULL. The original iter has leaked.
The solution is to use seq_open_private.

The problem is that the chchp --vary 0 leads to zfcp first calling
fc_remote_port_delete which blocks all scsi devices on the remote
port. Calling scsi_remove_device later lets the sd driver issue a
SYNCHRONIZE_CACHE command. This command stays on the "stopped" request
requeue because the SCSI device is blocked. Fix this by first removing
the scsi and fc hosts which removes all scsi devices and do not use
scsi_remove_device.

cancel_work_sync is called while holding the config_mutex. But the
work that is being cancelled or flushed also uses the config_mutex.
Fix the resulting deadlock possibility by calling cancel_work_sync
earlier without holding the mutex. The best place to do is is after
offlining the device.  No new port scan work will be scheduled for the
offline device, so this is a safe place to call cancel_work_sync.

1f99bd4cbf [SCSI] zfcp: Fix oops during shutdown of offline device
With the change that the zfcp_adapter struct is only allocated when
the device is set online, the shutdown handler has to check for a
non-existing zfcp_adapter struct. On the other hand, this check is not
necessary in the offline callback, since an online device has the
zfcp_adapter allocated and we go through the offline callback before
removing the ccw device.

68d1e5f08b [S390] dasd: tolerate devices that have no feature codes
The DASD device driver reads the feature codes of a device during
device initialization. These codes are later used to determine the
availability of advanced features like PAV or High Performance FICON.
Some very old devices do not support the command to read feature
codes and the initialization routine fails.
As the feature codes are not necessary for basic DASD operations, we
can support such devices by just ignoring missing feature codes.

5314af693d [S390] zcrypt: Do not add/remove devices in s/r callbacks
Devices are no longer removed or added in the suspend and resume
callbacks. Instead they are marked unregistered in suspend. In the
resume callback the ap_scan_bus method is scheduled. The bus scan
function will remove the old device and add new ones. This way all
the device handling will be done in only one function. Additionaly
the case where the domain might change during suspend/resume is
caught. In that case the devices qid needs to re-calculated in
order of having it found by the scan method.

For messages from the tape core that is shared between the 3590 and 34xx

53f8c573ea [S390] tape: Fix device online messages
Currently, when a tape device is set online and no cartridge is loaded, we
get the messages "The tape cartridge has been successfully unloaded" and
"Determining the size of the recorded area". These messages are not correct.
To fix this, we now print the "cartridge loaded/unloaded" messages only,
when the load/unload event really occurs. In addition to that, the message
"Determining the size of the recorded area" is only printed, if a cartridge
is loaded.

99b5e2d3d7 [S390] xpram: Remove checksum validation for suspend/resume
Currently in the suspend process checksums for the XPRAM partitions are
created and stored. During the resume process it is checked,
if the checksums are still the same. If this is not the case, a kernel panic
is triggered. Unfortunately this prevents XPRAM from beeing used as suspend
device, because in this case after the checksum has been created, the
memory image is written to XPRAM and therefore the contents of the suspend
partition is changed. In order to allow XPRAM to be used as suspend device,
this patch removes the checksum validation.

and it is destroyed before the CCW driver is unregistered.

We use a test_and_clear_bit to prevent a device from being

ab6aae0902 [S390] cio: fix memleak in subchannel validation
When scanning for new subchannels we have a code path where we allocate
memory for a struct subchannel, set the device name (which is dynamically
allocated now) and do a check if the underlying device is blacklisted - if
so we free the subchannel structure.
Since we have not set up refcounting at this stage, the device name's memory
is lost. Fix this by moving the dev_set_name after the blacklist test.

Note: With this patch the init_name for the console subchannel becomes
virtually obsolete.

When using s390dbf with "%s" in sprintf format strings the string itself
is not copied to the dbf buffer.
Since in this case only pointers are stored in the s390dbf, we should
not use dev_name - which is bound to the lifetime of the device.
Reading this entry from s390dbf after the device was released will cause
an use after free error.

217ee6c64a [S390] cio: failing set online/offline processing.
When unit checks trigger sensing the device state is set to W4SENSE
until sense completion; then the device state is set back to
ONLINE. If a unit check occurs while set online or set offline
requests are processed then it might happen that the device's
temporary W4SENSE state causes these functions to terminate,
leaving the device in an inconsistent state when the state is set
back to ONLINE later on so that the device cannot be set online or
offline any longer.
To solve this, set online/offline and related rollback or error
routines are processed only if the device is in a final or
DISCONNECTED state.

Path verification events occurring for offline devices are currently
ignored. As a result, offline devices are not removed, even though

143bb6bfe3 [SCSI] zfcp: Defer resource allocation to first ccw_set_online call
So far, zfcp allocated all resources required for FCP
adapters/subchannels when the device was discovered in the ccw_probe
callback. If there are lots of unused FCP subchannels attached to a
system, this is a waste of resources. To alleviate this, defer the
resource allocation to the first call to ccw_set_online. To avoid
disruptions during possible following calls to ccw_set_offline and
then ccw_set_online, keep the adapter resources until the device is
finally being removed via ccw_remove. While doing this, also manage
the zfcp erp thread together with all other adapter resources in
zfcp_adapter_enqueue/dequeue.

Switch the creation of the zfcp erp thread from the deprecated
kernel_thread API to the kthread API. This allows also the removal of
some flags in zfcp since the kthread API handles thread creation and
shutdown internally. To allow the usage of the kthread_stop function,
replace the erp ready semaphore with a waitqueue for waiting until erp
actions arrive on the ready queue.

ea945ff84c [SCSI] zfcp: resolve false usage of dd_data in fc_rport
The fc_rport structure reserves a reference where a LLD can put
information required in a situation where the fc transport class is
triggering LLD callbacks. The zfcp driver was using this variable
directly which is discouraged. This patch solves this issue by making
this reference unnecessary.  In addition the dev_loss_tmo callback is
removed, it is not required: zfcp does not access the fc_rport after
calling fc_remote_port_delete.

status "test link", zfcp will send a series of ELS ADISC commands.
This is technically no problem, but it is enough to only issue one

In certain error scenarios ports, rports are getting attached,
validated and removed from the systems environment. Depending on the
layer this occurs asynchronously. This patch fixes the few races

7c6a3ed5bd qeth: display "undefined" value of sysfs-attribute "layer2"
If nothing has been written into the qeth sysfs-attribute layer2,
its value is "-1" meaning "not yet defined". But the value is
displayed as "1" meaning "layer2 selected". The patch changes the
reading of this "-1"-value to "-1" to make clear the layer2-attribute
has not yet been defined.

0d788c7d28 qeth: display service_level info only when valid.
qeth displayed an entry in /proc/service_level even when no valid
MCL-string was available (the MCL info is blank). The change is to
create an entry in /proc/service_level only when MCL-string is
non-zero.

e806904057 qeth: scheduling while atomic during ifconfig online sequence
In case the IP address list contains entries (not removed when the device was set
offline) this entries should be registered next time the device is brought online.
In the past this was done implicitly with the device open call but since we wait
in the set IPv4 IPA and the device open common code holds various locks this
does not work any longer.

58490f1807 qeth: HiperSockets SIGA retry support on CC=2.
Qeth HiperSockets support now retries sending of packets when the
IBM System z signals a temporary resource shortage (e.g. target
buffer full). The packet is enqueued into the device queue.
After 3 times of unsuccessful send the packet is dropped.

If io_subchannel_initialize_dev fails it will release the only
reference to the ccw device therefore the caller should not
kfree this device since this is done in the release function.

27f492ccec [SCSI] zfcp: Fix wka port processing
Under certain conditions it is possible that a WKA port ist not opened
within the expected timeframe of half a second. In this situation
the WKA port remains in the state OPENING preventing any succeding
request to open the port. This led to unrecoverable remote ports.
Fixing this by always setting an appropriate WKA port status before
leaving the function and removing the timeout value here since it's
not needed here because the general timeout processing would deal
with it if required.

When calling fc_remote_port_add make sure to not call it again before
fc_remote_port_delete has been called. In other words, ensure to
create a new fc_rport, then delete it, then create a new one again.
