If the board ID lookup function fails, return the return
code rather than return -ENODEV.
The only board ID failure reason right now is -ENODEV,
so this just provides more informative prints in kdump
and adapts to future changes.
Tested with error injection while booting with
reset_devices
on the kernel command line:

In MSI and MSI-X mode, where hpsa asks for more than one interrupt,
hpsa_request_irqs forgets if the first request_irq call failed
if later ones succeed.
It needs to exit the loop on any failure rather than continue,
freeing all irqs that were requested until that point.
Also, it needs to clear out the q numbers up to MAX_REPLY_QUEUES.
The same is true for the general hpsa_free_irqs function.
Tested with error injection of -ENOSYS on the 4th call:

HP now uses RAID-6 rather than RAID-ADG (Advanced Data Guarding)
as the marketing name for our implementation of RAID-6.
The driver considers RAID-1 and RAID-1+0 to be the same level, and
considers RAID-1ADM and RAID-1+0ADM to be the same level. Parenthesis
can be used to reflect the optional +0 portion of both those RAID levels.
Rename: RAID-ADG to RAID-6
RAID-1(1+0) to RAID-1(+0)
RAID-1(ADM) to RAID-1(+0)ADM
Also, add another const after the pointer type as suggested
by checkpatch.pl so the array is:

Change how SA controllers are reset by changing PCI power levels.
The hpsa driver was finding the PCI_PM_CTRL_STATE_MASK offset
then reading/writing a bitmask to change the power state. There
are kernel functions that do the same operations. Better to use
the kernel functions.

Sometimes when the card is restarted it may cause -
"irq 16: nobody cared (try booting with the "irqpoll" option)"
that is likely caused so, that the card, after the hard reset
finishes, pulls on the irq. Disabling the ints before or after
the hpsa_kdump_hard_reset_controller fixes it.
At this point we can't know in which state the card is,
so using SA5_INTR_OFF + SA5_REPLY_INTR_MASK_OFFSET defines directly,
instead of the function the drivers provides, seems to be apropriate.

The following patch fixes an issue observed with 4k sector disks
where the max_hw_sectors attribute was getting set too large in
sd_revalidate_disk. Since sdkp->max_xfer_blocks is in units
of SCSI logical blocks and queue_max_hw_sectors is in units of
512 byte blocks, on a 4k sector disk, every time we went through
sd_revalidate_disk, we were taking the current value of
queue_max_hw_sectors and increasing it by a factor of 8. Fix
this by only shifting sdkp->max_xfer_blocks.

This fixes a regression caused by commit 1d5203 ("scsi: handle more device
handler setup/teardown in common code").
The bug is that the alua detach() callout will try to access the
sddev->scsi_dh_data, but we have already set it to NULL. This patch
moves the clearing of that field to after detach() is called.

This patch fix is to use default firmware configuration files

This is the blk-mq part to support tag allocation policy. The default
allocation policy isn't changed (though it's not a strict FIFO). The new
policy is round-robin for libata. But it's a try-best implementation. If
multiple tasks are competing, the tags returned will be mixed (which is
unavoidable even with !mq, as requests from different tasks can be
mixed in queue)

This can happen if a multipathed device uses DIX and another path is
added via an adapter that does not support it. Multipath should not
allow this path to be added, but we should not depend upon that to avoid

We found a subtle issue with FCoE on T4 very late in the game
and decided not to productize FCoE on T4 and therefore there
are no customers that will be impacted by this change. Hence
T4 FCoE support is removed. FCoE supported only on T5 cards.

Fixes a race condition in abort handling that was injected
when multiple interrupt support was added. When only a single
interrupt is present, the adapter guarantees it will send
responses for aborted commands prior to the response for the
abort command itself. With multiple interrupts, these responses
generally come back on different interrupts, so we need to
ensure the abort thread waits until the aborted command is
complete so we don't perform a double completion. This race
condition was being hit frequently in environments which
were triggering command timeouts, which was resulting in
a double completion causing a kernel oops.

Contrary to common expectations for an "int" return, these functions
return only a positive value -- if used correctly they cannot even
return 0 because the message header will necessarily be in the skb.
This makes the very common pattern of
if (genlmsg_end(...) < 0) { ... }
be a whole bunch of dead code. Many places also simply do
return nlmsg_end(...);
and the caller is expected to deal with it.
This also commonly (at least for me) causes errors, because it is very
common to write
if (my_function(...))
/* error condition */
and if my_function() does "return nlmsg_end()" this is of course wrong.
Additionally, there's not a single place in the kernel that actually
needs the message length returned, and if anyone needs it later then
it'll be very easy to just use skb->len there.
Remove this, and make the functions void. This removes a bunch of dead
code as described above. The patch adds lines because I did
- return nlmsg_end(...);
+ nlmsg_end(...);
+ return 0;
I could have preserved all the function's return values by returning
skb->len, but instead I've audited all the places calling the affected
functions and found that none cared. A few places actually compared
the return value with <= 0 in dump functionality, but that could just
be changed to < 0 with no change in behaviour, so I opted for the more
efficient version.
One instance of the error I've made numerous times now is also present
in net/phonet/pn_netlink.c in the route_dumpit() function - it didn't
check for <0 or <=0 and thus broke out of the loop every single time.
I've preserved this since it will (I think) have caused the messages to
userspace to be formatted differently with just a single message for
every SKB returned to userspace. It's possible that this isn't needed
for the tools that actually use this, but I don't even know what they
are so couldn't test that changing this behaviour would be acceptable.

Added a support to set cpu affinity mask for each MSIX vector enabled
by the HBA. So that, running the irqbalancer will balance interrupts among
the cpus.
Change_set:
1. Added affinity_hint varable of type cpumask_var_t in adapter_reply_queue
structure. And allocated a memory for this varable by calling
alloc_cpumask_var.
2. Call the API irq_set_affinity_hint for each MSIx vector to affiniate it
with calculated cpus at driver inilization time.
3. While freeing the MSIX vector, call this same API to release the cpu
affinity mask for each MSIx vector by providing the NULL value in
cpumask argument.
4. then call the free_cpumask_var API to free the memory allocated in step 2.

For any SCSI command, if the driver receives IOC status =
SCSI_IOC_TERMINATED and log info = 0x32010081 then that command will be
completed with DID_RESET host status.
The definition of this log info value is "Virtual IO has failed and has
to be retried".
Firmware will provide this log info value with IOC Status
"SCSI_IOC_TERMINATED", whenever a drive (with is a part of a volume)
is pulled and pushed back within some minimal delay. With this log info
value, firmware informs the driver to retry the failed IO command
infinite times, so to provide some time for the firmware to discover
the reinserted drive successfully instated of just retrying failed
command for five times( doesn't giving enough time for firmware to
complete the drive discovery) and failing the IO permanently even
though drive came back successfully.

SCSI_MAX_SG_CHAIN_SEGMENTS & hardware limit (Calculated using
IOCFacts's MaxChainDepth).
2. Added a print for the message log whenever the user sets the
max_sgl_entries to a value greater than SCSI_MAX_SG_SEGMENTS to
warn about the kernel definition overriding.

When a flaky disk is there in a topology then during driver load,
discovery related I/O times out; which results in SCSI error recovery
initiating host reset and then the controller won't see any disk.
In this patch, The driver would return FAILED status to the host reset
initiated due to discovery related I/O timeout if ioc->is_driver_loading
is set. This flag would be set until we exit out of scsih_scan_finished().
i.e.
During device discovery if one of the disk is flaky
(which responds to some discovery commands and doesn't respond to some)
the driver wouldn't perform host reset for discovery related I/O timeout.
Instead it would return Failure for the host reset resulting in the
flaky disk getting removed by the SCSI Mid layer,
so other disks would be added correctly.

This patch will log a message when driver receives "Temperature Threshold
exceeded" event from any temperature sensor.
The message will look similar to like:

Change_set:
1. Get the number of sensor count of this IOC by reading IO Unit page 8 at
driver initialization time.
2. Also unmask the Temperature Threshold Event at driver initialization
time
3. Whenever a MPI2_EVENT_TEMP_THRESHOLD event is received from the
firmware, then print the sensor number, the maximum threshold number it
has exceed and the current temperature of this sensor.

1) For fusion adapters, limit reserved frames for non SCSI commands to 8
(3 for parallel IOCTLs + 5 for driver's internal DCMDs).
Earlier reserved commands for non SCSI IO frames was set to 32, so with
this implementation, increased per controller "can_queue".
Behavior of MFI controllers will remain unchanged.
2) Optimize the code related to per controller's 'can_queue' setting.

Driver calls megasas_complete_cmd() to call wake_up() for each MFI frame
that was issued through the ioctl() interface prior to the kill adapter.
This ensures userspace ioctl() system calls issued just before a kill
adapter don't get stuck in wait state and IOCTLs are returned to
the application.

This patch will address the issue of SCSI device created at OS level for
non existing VD. ldTgtIdtoLd[] array has size 256 for Extended VD firmware
and 128 for legacy firmware. Accessing indices beyond array size (OS will
send TUR, INQUIRY.. commands upto device index 255), may return valid LD
value and that particular SCSI command will be SUCCESS and creating SCSI
device for non existing target(VD).
For legacy firmware (64 VD firmware), invalidates LD (by setting LD value
to 0xff) in LdTgtIdtoLd[] array for device index beyond 127, so that
invalid LD(0xff) value should be returned beyond device index beyond 127.

This patch adds support for Secure Encrypting Drives (SED) in JBOD mode:
1) If the firmware supports SED JBOD, all non read/write commands to JBODs
will be sent via firmware path, and read/write commands to JBODs will
be sent via fastpath.
2) If the firmware does not support SED JBOD, driver will fall back to the
old design, i.e. send all JBOD I/O via fastpath.

This patch addresses below issues:
1) Few endianness bug fixes.
2) Break the iteration after (MAX_LOGICAL_DRIVES_EXT - 1)),
instead of MAX_LOGICAL_DRIVES_EXT.
3) Optimization in MFI INIT frame before firing.
4) MFI IO frame should be 256bytes aligned. Code is optimized to reduce
the size of frame for fusion adapters and make the MFI frame size
calculation a bit transparent and readable.

It is not good practice to display the kernel pointer in any message logs,
and it doesn't display any additional information. And as we know have
block-layer assigned tags we can use them to differentiate the messages.
So remove any pointer references from the displayed messages.

scsi_log_send() would display buffer pointer for higher logging levels.
This is not only of questionable value but also exposes kernel pointer to
userspace, which is discouraged in some setups. So drop this message
altogether.

Use an external buffer for __scsi_print_command() and move command
logging over to use the per-cpu logging buffer. With that we can
guarantee the command always will always be formatted in one line.
So we can even print out a variable length command correctly across
several lines. Finally rename __scsi_print_command() to
__scsi_format_comment() to better reflect the functionality.

up under high load. This patch implements scmd_printk() and

On win8 and win8 r2 hosts force SPC-3 compliance for MSFT virtual disks.
Ubuntu has been carrying a similar patch outside the tree for a while now.
Starting with win10, the host will support SPC-3 compliance. Based on all
the testing that has been done on win8 and win8 r2 hosts, we are comfortable
claiming SPC-3 compliance on these hosts as well. This will enable TRIM
support on these hosts.

Commit 4cd83ecdac changed the limits to
reflect the values on the host. It turns out that WS2008R2 cannot
correctly handle these new limits. Fix this bug by setting the limits

The host asks the guest to scan when a LUN is removed or added.
The only way a guest can identify the removed LUN is when an I/O is
attempted on a removed LUN - the SRB status code indicates that the LUN
is invalid. We currently handle this SRB status and remove the device.
Rather than waiting for an I/O to remove the device, force the discovery of
LUNs that may have been removed prior to discovering LUNs that may have
been added.

STORVSC uses its own momory pool to manage device request data. However,
the SCSI layer already has a mechanisim for allocating additional memory
for each command issued to device driver. This patch removes the memory
pool in STORVSC and makes it use SCSI layer to allocate memory for device
request data.

This patch, applicable to 8G/4G/2G adapters, adds a call that
resumes transmit operations after a link bounce. Without it, targets
that tried to suspend exchanges after a link bounce (such as tape devices
using sequence level error recovery) would never resume io operation,
causing scan failures, and eventually deadlocks if a device removal
request is made.

Generate a REPORTED LUNS DATA HAS CHANGED Unit Attention if
sysfs "max_luns" is used to change the number of scsi_debug LUNs.
This is only done if scsi_debug_scsi_level is SPC-3 or above.
Additionally, implement SPC-4 behavior which only generates
this Unit Attention on the first LUN on the target to receive
a command after the change. This condition is cleared when
a REPORT LUNS command is received.

The name provided by firmware is in a vendor specific format, publish
the slot number to have a reliable mechanism for identifying slots
across firmware implementations. If the enclosure does not provide a
slot number fallback to the component number which is guaranteed unique,
and usually mirrors the slot number.
Cleaned up the unused ses_component.desc in the process.

In support of a /dev/disk/by-slot populated with data from the enclosure
and ses modules udev needs notification when the new interface
files/links are available. Otherwise, any udev rules specified for the

Update scsi_report_lun_scan to initially always report up to 511 LUs,
as the previous default max_report_luns did. Retry in a loop if not

The blk-mq ->queue_rq method is always called from process context,
but might have preemption disabled. This means we still always
have to use GFP_ATOMIC for memory allocations, and thus need to

Commit e05fe29248 (qla2xxx: Honor FCP_RSP retry delay timer field.)
causes systems to busy-wait for about 3 minutes after boot prior to
detecting SAN disks.
During this wait period one kworker is running full-time
(though /proc/<pid>/stack has no useful data). Another kworker is
waiting for IO to complete during that whole time period.
Looking at drivers/scsi/qla2xxx/qla_os.c, fcport->retry_delay_timestamp
has a special value of 0 though that 0 value forces system to wait when
jiffies is very large value (e.g. 4294952605 - "negative" value when
signed on 32bit systems).

Commit ac61d19559 (scsi: set correct completion code in
scsi_send_eh_cmnd()) introduced a bug. It changed the stored return
value from a queuecommand call, but it didn't take into account that
the return value was used again later on. This patch fixes the bug by
changing the later usage.
There is a big comment in the middle of scsi_send_eh_cmnd() which
does a good job of explaining how the routine works. But it mentions
a "rtn = FAILURE" value that doesn't exist in the code. This patch
adjusts the code to match the comment (I assume the comment is right
and the code is wrong).
This fixes Bugzilla #88341.

When I/O is aborted by mid-layer, fnic FW will complete the I/O before
completing the abort task. In some cases abort request is completed before

7985090aa0 changed the discard heuristics to give preference to the
WRITE SAME commands that (unlike UNMAP) guarantee deterministic results.
Ming Lei discovered that QEMU SCSI's WRITE SAME implementation
internally relied on limits that were only communicated for the UNMAP
case. And therefore discard commands backed by WRITE SAME would fail.
Tweak the heuristics so we still pick UNMAP in the LBPRZ=0 case and only
prefer the WRITE SAME variants if the device has the LBPRZ flag set.

After commit b2b49ccbdd (PM: Kconfig: Set PM_RUNTIME if PM_SLEEP is
selected) PM_RUNTIME is always set if PM is set, so #ifdef blocks

When ring buffer returns an error indicating retry, storvsc may not
return a proper error code to SCSI when bounce buffer is not used.
This has introduced I/O freeze on RAID running atop storvsc devices.
This patch fixes it by always returning a proper error code.

Kernel build tools pointed out a memory leak so that has been
fixed and its error paths strengthened with a goto. Testing
showed compare and write was only working for lba=0; correcting
the length of the LBA field fixed that.

When we have an rport disconnect we race during rport deletion and
re-connection resulting in a panic. When we do this, we call
fc_remote_port_del() just before we do the calls to re-establish the
session with the FC transport with fc_remote_port_add() and then
fc_remote_port_rolechg().
If we remove the call to fc_remote_port_del() before re-establishing
the connection this prevents the race. This patch has resolved this
for multiple customers via test kernels.

This fixes random memory corruption triggered when all three of the
following are true:
* scsi-mq enabled
* T10 Protection Information (DIF) enabled
* SCSI host with sg_tablesize > SCSI_MAX_SG_SEGMENTS (128)
The symptoms of this bug are unpredictable memory corruption, BUG()s,
oopses, lockups, etc., any of which may appear to be completely
unrelated to the root cause.

Remove use of calls into t4_fw_hello() with MASTER_MUST, which results in

Dropping to untagged mode when ramping down a queue due to QUEUE FULL
events has two problems:
- nothing in the midlayer or drivers ever moves back to tagged mode
during queue ramp up.
- cmd_per_lun isn't the untagged queue depth for many modern drivers
that can handle multiple untagged commands, and this is the only
place in the midlayer assuming that.

Since we got rid of ordered tag support in 2010 the prime use case of
switching on and off ordered tags has been obsolete. The other function
of enabling/disabling tagging entirely has only been correctly implemented
by the 53c700 driver and isn't generally useful.

a potential null deref in the voltage regulator code, a race on module unload,
a ref counting fix on the well known LUNs which made it impossible to remove
the ufs module and fix to correct the information in pwr_info. In addition to

The task_collector mode (or "latency_injector", (C) Dan Willians) is an
optional I/O path in libsas that queues up scsi commands instead of
directly sending it to the hardware. It generall increases latencies
to in the optiomal case slightly reduce mmio traffic to the hardware.
Only the obsolete aic94xx driver and the mvsas driver allowed to use
it without recompiling the kernel, and most drivers didn't support it
at all.
Remove the giant blob of code to allow better optimizations for scsi-mq
in the future.

If no voltage supply regulators are defined for the UFS devices (assumed
they are always-on), ufshcd_config_vreg_load() can be called on
suspend/resume paths with vreg == NULL as hba->vreg_info.vcc* equal to
NULL, and it causes NULL pointer dereference.
This fixes it by making ufshcd_config_vreg_{h,l}pm noop when no regulators
are defined.

When dynamic clk gating feature is enabled, delayed workqueue machanism
is used in order to detect certain period of inactivity. But there is no
guarantee that scheduled gating work is completed before module unloading.
So it can cause kernel crash by accessing memory after it was freed.
Fix it by cancelling clk gating and ungating works and ensure that its
execution is finished before module unloading.

The Report supported operation codes command is very closely integrated
into the table driven parser and very useful for testing it. Its cdb
masks form the basis of the 'strict' parameter's checks. The Report
supported TMFs command is a simple extension. The Compare and write
command may even be useful, as it should be atomic due to the read-write
lock that the driver uses on its backing store (ram).

The existing 'big switch' parser in queuecommand() is changed to
a table driven parser. The old and new queuecommand() were moved
in the source so diff would not shuffle them. Apart from the new
tables most other changes are refactoring existing response code
to be more easily called out of the table parser. The 'strict'
parameter is added so that cdb_s can be checked for non-zero
values in parts of the cdb that are reserved. Some other changes
include: tweak request sense response when D_SENSE differs; support
NDOB in Write Same(16); and fix crash in Get LBA Status when LBP
was inactive.

The way the existing scsi_debug command parser associated various
inject error flags to a command was difficult to replicate in the
table driven parser. This patch adds infrastructure to append those
flags to the end of a scsi_cmnd object with the cmd_size host
template option.

SPC-3 defines SERVICE ACTION IN(12), SERVICE_ACTION OUT(12),
SERVICE ACTION OUT(16), and SERVICE ACTION BIDIRECTIONAL.
And READ MEDIA SERIAL NUMBER has long since been deprecated.

There is no reason for ULDs to pass in a flag on how to allocate the S/G
lists. While we don't need GFP_ATOMIC for the blk-mq case because we
don't hold locks, that decision can be made way down the chain without
having to pass a pointless gfp_mask argument.

scsi_reset_provider already manually runs all queues for the given host,
so it doesn't need the scsi_run_queues call from it, and it doesn't need
a reference on the device because it's synchronous.
So let's just call scsi_put_command directly and avoid the device reference
dance to simplify the code.

CONFIG2_FENAB ('feature enable') changed definition between chip
revisions, from 'Latch SCSI Phase' to 'Latch SCSI Phase, display
chip ID upon reset, and enable 24 bit addresses'.
So only enable it for am53c974 where we know what it's doing.

On PCscsi, the FENAB configuration also enables 24-bit DMA
transfer lengths (and provides the chip id in TCHI after reset).
We want to be able to enable this parameter from the DMA driver.
Check if the caller of scsi_esp_register provided a value for esp->config2.
If this is the case, assume this is not an ESP100, skip the detection
phase and leave esp->config2 untouched. It will be used in esp_reset_esp.

The am53c974 has an design issue where a single byte might be
left in the SCSI FIFO after a DMA transfer.
As the handling code is currently untested add a WARN_ON()
statement here.

Using DMA for command submission has the drawback that it might
generate additional DMA completion interrupts after the command
has been submitted to the device.
Additionally the am53c974 has a design flaw causing it
to generate spurious interrupts even though DMA completion
interrupts are not enabled.
This can be avoided by using the FIFO for command submission.

We won't ever queue more commands than the host allows. Instead of
letting drivers either reject or ignore this case handle it in
common code. Note that various driver use internal constant or
variables that are assigned to both shost->can_queue and checked
in ->change_queue_depth - I did remove those checks as well.

All drivers use the implementation for ramping the queue up and down, so
instead of overloading the change_queue_depth method call the
implementation diretly if the driver opts into it by setting the
track_queue_depth flag in the host template.
Note that a few drivers validated the new queue depth in their
change_queue_depth method, but as we never go over the queue depth
set during slave_configure or the sysfs file this isn't nessecary
and can safely be removed.

MEGASAS_HBA_OPERATIONAL before getting new RAID map. There will be a small
window where IO will come from OS with old RAID map. This patch will update
adapter state to MEGASAS_HBA_OPERATIONAL, only after driver has new RAID
map to avoid any IOs getting build using old RAID map.

update adapter state to MEGASAS_HBA_OPERATIONAL, only after driver has new

After link start-up power mode will always be PWM G1. This is not
reflected in the pwr_info struct which will keep the previous values.
Since ufshcd_change_power_mode() tries to avoid unnecessary power mode
change if the requested power mode and current power mode are same,
power mode change won't execute again after driver initialization.
This patch solves the problem by setting pwr_info to PWM G1 after link
start-up.

UFS driver adds three well known LUs in the initialization, but those
reference counts are not decremented, so it makes ufshcd module
impossible to unload.
This fixes it by putting scsi_device_put() in the initalization, and in
order to protect concurrent access to hba->sdev_ufs_device (UFS Device
W-LU) from manual delete, increment the reference count while requesting
device power mode setting.
The rest of W-LUs (hba->sdev_boot and hba->sdev_rpmb) are not directly
used from driver, so these references in struct ufs_hba are removed.

If the kernel is booted with the reset_device parameter, which
is done for kdump, then the driver needs to call pci_set_master
after pci_enable_device to reenable bus mastering (since
the preceding pci_disable_device call disables bus mastering).
Also, place that after pci_request_regions both in the
kdump code and the normal pci_init code.
Remove the comment summarizing what pci_set_master
does, with the incomplete commentary on the impact of
pci_disable_device.

Using bit fields for hardware command fields isn't portable and
relies on assumptions about how the compiler lays out the bits.
We can fix this in the driver's internal command structure, but the
ioctl interface we can't change because it is part of the
userland ABI.

The hardware needs little endian scatter gather addresses and
lengths but we were not bothering to convert from cpu byte
order as we should have been. On Intel, this is all just
a bunch of no-ops macros, but it makes the code endian-clean(er).

We were allocating roughly double the amount of memory
we should be due to ReportLUNdata and ExtendedReportLUNdata
containing a non-zero sized array but adding extra memory
to allocate as if the array were zero sized.
Track the logical and physical sizes separately.
Allocate the memory based on the specific data
structure sizes.

In the case of LUN data changing, the driver will
auto rescan and so it's not even true that "action" is
"required".

Unlike NCR5380.c, the atari_NCR5380.c core driver is limited to a single
instance because co-routine state is stored globally.
Fix this by removing the static scsi host pointer. For the co-routine,
obtain this pointer from the work_struct pointer instead. For the interrupt
handler, obtain it from the dev_id argument.

The static variable setup_use_tagged_queuing is declared in mac_scsi.c,
sun3_scsi.c and atari_scsi.c and doesn't belong in the core driver.
None of the other NCR5380 drivers suffer from this layering issue which
makes merging the core drivers more difficult and will likely hinder plans
for future use of platform data to configure the driver.
Replace the static variable with a host flag. This way it can be reported
along with the other flags.

Simplify falcon_release_lock_if_possible() by making callers responsible for
disabling local IRQ's, which they must do anyway to correctly synchronize
the ST DMA "lock" with core driver data structures. Move this
synchronization logic to the core driver with which it is tightly coupled.
Other LLD's like sun3_scsi and mac_scsi that can make use of this core
driver can just stub out the NCR5380_acquire_dma_irq() and
NCR5380_release_dma_irq() calls so the compiler will eliminate the
ST DMA code.
Remove a redundant local_irq_save/restore pair (irq's are disabled for
interrupt handlers these days). Revise the locking for
atari_scsi_bus_reset(): use local_irq_save/restore() instead of
atari_turnoff/turnon_irq(). There is no guarantee that atari_scsi still
holds the ST DMA lock during EH, so atari_turnoff/turnon_irq() could
end up dropping an IDE or floppy interrupt.

Make the atari_NCR5380.c core driver usable by sun3_scsi, mac_scsi and
others by moving some of the Falcon-specific code out of the core driver:
!IS_A_TT, atari_read_overruns and falcon_dont_release. Replace these with
hostdata variables and flags. FLAG_CHECK_LAST_BYTE_SENT is unused in
atari_NCR5380.c so don't set it.

Don't disable irqs when waiting for the ST DMA "lock"; its release may
require an interrupt.
Introduce stdma_try_lock() for use in soft irq context. atari_scsi now tells
the SCSI mid-layer to defer queueing a command if the ST DMA lock is not
available, as per Michael's patch:

The falcon_got_lock variable is race prone: we can't disable IRQs while
waiting to acquire the lock, so after acquiring it there must be some
interval during which falcon_got_lock remains false. Introduce
stdma_is_locked_by() to replace falcon_got_lock.
The falcon_got_lock tests in the EH handlers are incorrect these days. It
can happen that an EH handler is called after a command completes normally.
Remove these checks along with falcon_got_lock.
Also remove the complicated and racy fairness wait queues. If fairness is an
issue (when SCSI competes with IDE for the ST DMA interrupt), the solution
is likely to be a lower value for host->can_queue.

Testing shows that the Domex 3191D card never asserts its IRQ. Hence it is
non-functional with Linux (worse, the EH bugs in the core driver are fatal
but that's a problem for another patch). Perhaps the DT-536 chip needs
special setup? I can't find documentation for it. The NetBSD driver uses
polling apparently because of this issue.
Set host->irq = NO_IRQ so the core driver will prevent targets from
disconnecting. Don't request host->irq.

Static variables from dtc.c and pas16.c should not appear in the core
NCR5380.c driver. Aside from being a layering issue this worsens the
divergence between the three core driver variants (atari_NCR5380.c and
sun3_NCR5380.c don't support PSEUDO_DMA) and it can mean multiple hosts
share the same counters.
Fix this by making the pseudo DMA spin counters in the core more generic.
This also avoids the abuse of the {DTC,PAS16}_PUBLIC_RELEASE macros, so
they can be removed.
oak.c doesn't use PDMA and hence it doesn't use the counters and hence it
needs no write_info() method. Remove it.

If the host->info() method is not set, then host->name is used by default.
For atari_scsi, that is exactly the same text. So remove the redundant
info() method. Keep sun3_scsi.c in line with atari_scsi.
Some NCR5380 drivers return an empty string from the info() method
(arm/cumana_1.c arm/oak.c mac_scsi.c) while other drivers use the default
(dmx3191d dtc.c g_NCR5380.c pas16.c t128.c).
Implement a common info() method to replace a lot of duplicated code which
the various drivers use to announce the same information.
This replaces most of the (deprecated) show_info() output and all of the
NCR5380_print_info() output. This also eliminates a bunch of code in
g_NCR5380 which just duplicates functionality in the core driver.

The NCR5380_STATS option is only enabled by g_NCR5380 yet it adds
clutter to all three core drivers. The atari_NCR5380.c and sun3_NCR5380.c
core drivers have a slightly different implementation of the
NCR5380_STATS option.
Out of all ten NCR5380 drivers, only one of them (g_NCR5380) actually
has the code to report on the collected stats. Aside from being unreadable,
that code seems to be broken because there's no initialization of timebase.
sun3_NCR5380.c and atari_NCR5380.c have the timebase initialization but
lack the code to report the stats.
Remove all of this code to improve readability and reduce divergence
between the three core drivers.
This patch and the next one completely eliminate the PRINTP and ANDP
pre-processor abuse.

Oak scsi doesn't use any IRQ, but it sets irq = IRQ_NONE rather than
SCSI_IRQ_NONE. Problem is, the core NCR5380 driver expects SCSI_IRQ_NONE
if it is to issue IDENTIFY commands that prevent target disconnection.
And, as Geert points out, IRQ_NONE is part of enum irqreturn.
Other drivers, when they can't get an IRQ or can't use one, will set
host->irq = SCSI_IRQ_NONE (that is, 255). But when they exit they will
attempt to free IRQ 255 which was never requested.
Fix these bugs by using NO_IRQ in place of SCSI_IRQ_NONE and IRQ_NONE.
That means IRQ 0 is no longer probed by ISA drivers but I don't think
this matters.
Setting IRQ = 255 for these ISA drivers is understood to mean no IRQ.
This remains supported so as to avoid breaking existing ISA setups (which
can be difficult to get working) and because existing documentation
(SANE, TLDP etc) describes this usage for the ISA NCR5380 driver options.

Both atari_NCR5380.c and sun3_NCR5380.c core drivers #undef TAG_NONE and
then redefine it. But the original definition is unused because NCR5380.c
lacks support for tagged queueing. So just define it once.
The TAG_NEXT macro only appears in the arguments to NCR5380_select() calls.
But that routine doesn't use its tag argument as the tag was already
assigned in NCR5380_main(). So remove the unused argument and the macro.

Make use of the host template static initializer instead of assigning
handlers at run-time. Move __maybe_unused qualifiers from declarations
to definitions. Move the atari_scsi_bus_reset() wrapper after the
definition of NCR5380_bus_reset(). All of the host template handler
prototypes are now redundant so remove them.
The write_info() handler is only relevant to drivers using PSEUDO_DMA so
this patch fixes the compiler warning in atari_NCR5380.c and sun3_NCR5380.c:

When issuing I/O request, if the I/O completes before returning from
fnic_queuecommand(), we may be referencing scsi_cmnd structure that may
be freed by interrupt handler. Acquring IO lock would synchronize
fnic_queuecommand and interrupt handler.

When physical link between standalone C series and switch is down,
the fip timer is not turned off and timer expiration will keep sending
vlan request.
It can be fixed by stopping the fip_timer and
it will be restarted automatically when Link is up.

IOs belonging to an rport are aborted with Internal terminate option
when rport goes offline. Any new IO issued to the rport during this
time can reuse the terminated exchange which will cause inconsistent
state of the exchange between local port and remote port.
fc_rport_priv is set to RPORT_ST_DELETE before exchanges are aborted by
libfc. Not issuing amy more I/O requests when RPORT_ST_DELETE is set,
will avoid inconsistent state of the exchange between local port and
remote port.

In case of receive path, we do not have eth header or fcoe header available
when we take a trace so we fill the fc trace buffer with 0xff for both
values. We copy only mimimum of received data or trace buffer size -
fc header - eth and fcoe header

seconds while loop before returning -ETIMEDOUT as sometime

cxgb4i was returning wrong error and not releasing module reference if remote
end abruptly closed TCP connection. This prevents the cxgb4 network module from
being unloaded, further affecting other network drivers dependent on cxgb4
Sending to net as this affects all cxgb4 based network drivers.

Refactored all macros used in cxgb4i as part of previously started cxgb4 macro

The request (and SCSI command) tag is the tag number assigned
by the generic block-tagging code, not the SCSI-II tag messages.
Those are represented by the device flags 'tagged_supported',
'simple_tags', and 'ordered_tags'.
(The SCSI midlayer doesn't use HEAD_OF_QUEUE tags).
So fixup vmw_pvscsi to assign the correct tag type.

Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth. For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.
Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway. The one other case looks
broken, but I've kept the behavior as-is for now.
Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.
Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

Allow a driver to ask for block layer tags by setting .use_blk_tags in the
host template, in which case it will always see a valid value in
request->tag, similar to the behavior when using blk-mq. This means even
SCSI "untagged" commands will now have a tag, which is especially useful
when using a host-wide tag map.

Unless we want to build a SPI tag message we should just check SCMD_TAGGED
instead of reverse engineering a tag type through the use of
scsi_populate_tag_msg.
Also rename the function to spi_populate_tag_msg, make it behave like the
other spi message helpers, and move it to the spi transport class.

Currently scsi piggy backs on the block layer to define the concept
of a tagged command. But we want to be able to have block-level host-wide
tags assigned even for untagged commands like the initial INQUIRY, so add
a new SCSI-level flag for commands that are tagged at the scsi level, so
that even commands without that set can have tags assigned to them. Note
that this alredy is the case for the blk-mq code path, and this just lets
the old path catch up with it.
We also set this flag based upon sdev->simple_tags instead of the block
queue flag, so that it is entirely independent of the block layer tagging,
and thus always correct even if a driver doesn't use block level tagging
yet.
Also remove the old blk_rq_tagged; it was only used by SCSI drivers, and
removing it forces them to look for the proper replacement.

The T10 SBC UNMAP command does not provide any hard guarantees that
blocks will return zeroes on a subsequent READ. This is due to the fact
that the device server is free to silently ignore all or parts of the
request.
The only way to ensure that a block consistently returns zeroes after
being unmapped is to use WRITE SAME with the UNMAP bit set. Should the
device be unable to unmap one or more blocks described by the command it
is required to manually write zeroes to them.
Until now we have preferred UNMAP over the WRITE SAME variants to
accommodate thinly provisioned devices that predated the final SBC-3
spec. This patch changes the heuristic so that we favor WRITE SAME(16)
or (10) over UNMAP if these commands are marked as supported in the
Logical Block Provisioning VPD page.
The patch also disables discard_zeroes_data for devices operating in
UNMAP mode.

The calling conventions for this function are bad as it could return
-ENODEV both for a device not currently online and a not recognized ioctl.
Add a new scsi_ioctl_block_when_processing_errors function that wraps
scsi_block_when_processing_errors with the a special case for the
SG_SCSI_RESET ioctl command, and handle the SG_SCSI_RESET case itself
in scsi_ioctl. All callers of scsi_ioctl now must call the above helper
to check for the EH state, so that the ioctl handler itself doesn't
have to.

print_opcode_name() was only ever called with a '0' argument
from LLDDs and ULDs which were _not_ supporting variable length
CDBs, so the 'if' clause was never triggered.
Instead we should be using the last argument to specify
the cdb length to avoid accidental overflow when reading
the cdb buffer.

And even there only for fixed format sense formats.
As this is of rather limited use in the general case we should

If scsi_normalize_sense() fails we couldn't decode the sense
buffer, and the scsi_sense_hdr fields are invalid.
For those cases we should rather dump the sense buffer
and not try to decode invalid fields.

fas216 returns DID_BAD_TARGET for an incomplete data

Further to a January 2013 thread titled: "[PATCH] SG_SCSI_RESET ioctl
should only perform requested operation" by Jeremy Linton a patch (v3)
is presented that expands the existing ioctl to include "no_escalate"
versions to the existing resets. This requires no changes to SCSI low
level drivers (LLDs); it adds several more finely tuned reset options
to the user space. For example:

As sparse correctly pointed out, scsi_partsize should use get_unaligned_le32
to read PC partition tables from disk, as they are little endian.
The result of this bug is that we returned incorrect geometries on big
endian systems when using the scsicam variant. Which probably doesn't
matter as only old x86 systems every cared about the geometry.

Various patches have ended up changing the style of the symbolic macros/register
defines to different style.
As a result, the current kernel.org files are a mix of different macro styles.
Since this macro/register defines is used by different drivers a
few patch series have ended up adding duplicate macro/register define entries
with different styles. This makes these register define/macro files a complete
mess and we want to make them clean and consistent. This patch cleans up a part
of it.

Function pci_enable_msix_range() may return negative values for error
conditions. So it's a bug by checking (pci_enable_msix_range() != 0)
for success and causes failure to megaraid driver when MSI is disabled.

This patch fixes an issue with multipath ipr SAS devices which require a
start unit command to be issued following an adapter reset. Without this
patch, paths get marked failed following an adapter reset and since the
error handler never gets invoked to issue the start unit, the paths are
never recovered. Returning FAILED for this case ensures the error
handler wakes up to issue the start unit.

Multipath devices using the TUR path checker need to see the sense
code for a failed TUR command in their device handler. Since commit
14216561e1 we always return success for mid
layer issued TUR commands before calling the device handler, which
stopped the TUR path checker from working.
Move the call to the device handler check sense method before the early
return for TUR commands to give the device handler a chance to intercept
them.

Setups that use the blk-mq I/O path can lock up if a host with a single
device that has its door locked enters EH. Make sure to only send the
command to re-lock the door to devices that actually were reset and thus
might have lost their state. Otherwise the EH code might be get blocked
on blk_get_request as all requests for non-reset devices might be in use.

Since we have the notion of a 'last' request in a chain, we can use
this to have the hardware optimize the issuing of requests. Add
a list_head parameter to queue_rq that the driver can use to
temporarily store hw commands for issue when 'last' is true. If we
are doing a chain of requests, pass in a NULL list for the first
request to force issue of that immediately, then batch the remainder
for deferred issue until the last request has been sent.
Instead of adding yet another argument to the hot ->queue_rq path,
encapsulate the passed arguments in a blk_mq_queue_data structure.
This is passed as a constant, and has been tested as faster than
passing 4 (or even 3) args through ->queue_rq. Update drivers for
the new ->queue_rq() prototype. There are no functional changes
in this patch for drivers - if they don't use the passed in list,
then they will just queue requests individually like before.

virtio spec requires drivers to set DRIVER_OK before using VQs.
This is set automatically after restore returns, virtio scsi violated
this rule on restore by kicking event vq within restore.
To fix, call virtio_device_ready before using event queue.

We currently kick event within virtscsi_init,
before host is fully initialized.
This can in theory confuse guest if device

The kernel used to contain two functions for length-delimited,
case-insensitive string comparison, strnicmp with correct semantics and
a slightly buggy strncasecmp. The latter is the POSIX name, so strnicmp
was renamed to strncasecmp, and strnicmp made into a wrapper for the new
strncasecmp to avoid breaking existing users.
To allow the compat wrapper strnicmp to be removed at some point in the
future, and to avoid the extra indirection cost, do
s/strnicmp/strncasecmp/g.

The DEFINE_XENBUS_DRIVER() macro looks a bit weird and causes sparse
errors.
Replace the uses with standard structure definitions instead. This is
similar to pci and usb device registration.

Clear QUEUE_FLAG_ADD_RANDOM in all block drivers that set
QUEUE_FLAG_NONROT.
Historically, all block devices have automatically made entropy
contributions. But as previously stated in commit e2e1a148 ("block: add
sysfs knob for turning off disk entropy contributions"):
- On SSD disks, the completion times aren't as random as they
are for rotational drives. So it's questionable whether they
should contribute to the random pool in the first place.
- Calling add_disk_randomness() has a lot of overhead.
There are more reliable sources for randomness than non-rotational block
devices. From a security perspective it is better to err on the side of
caution than to allow entropy contributions from unreliable "random"
sources.

The code is currently riddled with "drop the hardware_lock to avoid a
deadlock" bugs that expose races. One of those races seems to expose a
valid warning in tcm_qla2xxx_clear_nacl_from_fcport_map. Add some
bandaid to it.

Remove the inline attribute. Modern compilers ignore it and the
function has grown beyond where inline made sense anyway.
Remove the BUG_ON(!cmd->sg_mapped), and instead return if sg_mapped is
not set. Every caller is doing this check, so we might as well have it
in one place instead of four.

cxgbi_inet6addr_handler() can be called in interrupt context, so use rcu
protected list while finding netdev. This is observed as a scheduling in
atomic oops when running over ipv6.

Add capability to control the auto bkops during suspend.
If host explicitly enables the auto bkops (background operation) on device
then only device would perform the bkops on its own. If auto bkops is not
enabled explicitly and if the device reaches to state where it must do
background operation, device would raise the urgent bkops exception event
to host and then host will enable the auto bkops on device. This patch
adds the option to choose whether auto bkops should be enabled during
runtime suspend or not. Since we don't want to keep the device active to
perform the non critical bkops, host will enable urgent bkops only.
Keep auto-bkops enabled after resume if urgent bkops needed.
If device bkops status shows that its in critical need of executing
background operations, host should allow the device to continue doing
background operations.

The clocks for UFS device will be managed by generic DVFS (Dynamic
Voltage and Frequency Scaling) framework within kernel. This devfreq
framework works with different governors to scale the clocks. By default,
UFS devices uses simple_ondemand governor which scales the clocks up if
the load is more than upthreshold and scales down if the load is less than
downthreshold.

The UFS controller clocks can be gated after certain period of
inactivity, which is typically less than runtime suspend timeout.
In addition to clocks the link will also be put into Hibern8 mode
to save more power.
The clock gating can be turned on by enabling the capability
UFSHCD_CAP_CLK_GATING. To enable entering into Hibern8 mode as part of
clock gating, set the capability UFSHCD_CAP_HIBERN8_WITH_CLK_GATING.
The tracing events for clock gating can be enabled through debugfs as:

Sometimes, the device shall report its maximum power and speed
capabilities, but we might not wish to configure it to use those
maximum capabilities.
This change adds support for the vendor specific host driver to
implement power change notify callback.
To enable configuring different power modes (number of lanes,
gear number and fast/slow modes) it is necessary to split the
configuration stage from the stage that reads the device max power mode.
In addition, it is not required to read the configuration more than
once, thus the configuration is stored after reading it once.

This patch adds support for UFS device and UniPro link power management
during runtime/system PM.
Main idea is to define multiple UFS low power levels based on UFS device
and UFS link power states. This would allow any specific platform or pci
driver to choose the best suited low power level during runtime and
system suspend based on their power goals.
bkops handlig:
To put the UFS device in sleep state when bkops is disabled, first query
the bkops status from the device and enable bkops on device only if
device needs time to perform the bkops.
START_STOP handling:
Before sending START_STOP_UNIT to the device well-known logical unit
(w-lun) to make sure that the device w-lun unit attention condition is
cleared.
Write protection:
UFS device specification allows LUs to be write protected, either
permanently or power on write protected. If any LU is power on write
protected and if the card is power cycled (by powering off VCCQ and/or
VCC rails), LU's write protect status would be lost. So this means those
LUs can be written now. To ensures that UFS device is power cycled only
if the power on protect is not set for any of the LUs, check if power on
write protect is set and if device is in sleep/power-off state & link in
inactive state (Hibern8 or OFF state).
If none of the Logical Units on UFS device is power on write protected
then all UFS device power rails (VCC, VCCQ & VCCQ2) can be turned off if
UFS device is in power-off state and UFS link is in OFF state. But current
implementation would disable all device power rails even if UFS link is
not in OFF state.
Low power mode:
If UFS link is in OFF state then UFS host controller can be power collapsed
to avoid leakage current from it. Note that if UFS host controller is power
collapsed, full UFS reinitialization will be required on resume to
re-establish the link between host and device.

UFS device may have standard LUs and LUN id could be from 0x00 to 0x7F.
UFS device specification use "Peripheral Device Addressing Format"
(SCSI SAM-5) for standard LUs.
UFS device may also have the Well Known LUs (also referred as W-LU) which
again could be from 0x00 to 0x7F. For W-LUs, UFS device specification only
allows the "Extended Addressing Format" (SCSI SAM-5) which means the W-LUNs
would start from 0xC100 onwards.
This means max. LUN number reported from UFS device could be 0xC17F hence
this patch advertise the "max_lun" as 0xC17F which will allow SCSI mid
layer to detect the W-LUs as well.
But once the W-LUs are detected, UFSHCD driver may get the commands with
SCSI LUN id upto 0xC17F but UPIU LUN id field is only 8-bit wide so it
requires the mapping of SCSI LUN id to UPIU LUN id. This patch also add
support for this mapping.

UFS device specification requires the UFS devices to support 4 well known
logical units:
"REPORT_LUNS" (address: 01h)
"UFS Device" (address: 50h)
"RPMB" (address: 44h)
"BOOT" (address: 30h)
UFS device's power management needs to be controlled by "POWER CONDITION"
field of SSU (START STOP UNIT) command. But this "power condition" field
will take effect only when its sent to "UFS device" well known logical unit
hence we require the scsi_device instance to represent this logical unit in
order for the UFS host driver to send the SSU command for power management.
We also require the scsi_device instance for "RPMB" (Replay Protected
Memory Block) LU so user space process can control this LU. User space may
also want to have access to BOOT LU.
This patch adds the scsi device instances for each of all well known LUs
(except "REPORT LUNS" LU).

The maximum power consumption in active is determined by bActiveICCLevel.
The configuration is done by reading max current supported by the
regulators connected to VCC, VCCQ and VCCQ2 rails on the boards, and
reading the current consumption levels from the device for each rails
(vcc/vccq/vccq2) using power descriptor.
We configure the bActiveICCLevel attribute, with the max value that
correspond to the minimum-of(VCC-current-level,VCCQ-current-level,
VCCQ2-current-level).
In order to minimize resume latency, pre-fetch icc levels and reference
clock during initialization and avoid reading them each link startup
during resume.

In ->hce_enable_notify() callback the vendor specific initialization
may carry out additional DME configuration using UIC commands and
hence the UIC command completion interrupt enable bit should be set
before the post reset notification.
Add retries if the link-startup fails. This is required since due to
hardware timing issues, the Uni-Pro link-startup might fail. The UFS
HCI recovery procedure contradicts the Uni-Pro sequence. The UFS HCI
specifies to resend DME_LINKSTARTUP command after IS.ULLS (link-lost
interrupt) is received. The Uni-Pro specifies that if link-startup
fails the link is in "down" state. The link-lost is indicated to the
DME user only when the link is up. Hence, the UFS HCI recovery procedure
of waiting for IS.ULLS and retrying link-startup may not work properly.
At the end, if detection fails, power off (disable clocks, regulators,
phy) if the UFS device detection fails. This saves power while UFS device
is not embedded into the system.

UFS devices are powered by at most three external power supplies -
- VCC - The flash memory core power supply, 2.7V to 3.6V or 1.70V to 1.95V
- VCCQ - The controller and I/O power supply, 1.1V to 1.3V
- VCCQ2 - Secondary controller and/or I/O power supply, 1.65V to 1.95V
For some devices VCCQ or VCCQ2 are optional as they can be
generated using internal LDO inside the UFS device.
Add DT bindings for voltage regulators that can be controlled
from host driver.

Some vendor specific controller versions might need to configure
vendor specific - registers, clocks, voltage regulators etc. to
initialize the host controller UTP layer and Uni-Pro stack.
Provide some common initialization operations that can be used
to configure vendor specifics. The methods can be extended in
future, for example, for power mode transitions.
The operations are vendor/board specific and hence determined with
the help of compatible property in device tree.

If LLD has added scsi device (by calling scsi_add_device) before scheduling
async scsi_scan_host then scsi_finish_async_scan() will end up calling
scsi_sysfs_add_sdev for scsi device which was already added by LLD.
This patch fixes this issue by skipping the call to scsi_sysfs_add_sdev()
if it's already visible to rest of the kernel.

Some devices may respond with wrong type for well-known logical units.
This patch forces well-known type for devices which doesn't report it
correct.

A set of flags introduced in the block layer enable better control over
how protection information is handled. These flags are useful for both
error injection and data recovery purposes. Checking can be enabled and
disabled for controller and disk, and the guard tag format is now a
per-I/O property.
Update sd_protect_op to communicate the relevant information to the
low-level device driver via a set of flags in scsi_cmnd.

A deadlock has been reported when the completion
of SCSI commands (simulated by a timer) was surprised
by a module removal. This patch removes one half of
the offending locks around timer deletions. This fix
is applied both to stop_all_queued() which is were
the deadlock was discovered and stop_queued_cmnd()
which has very similar logic.
This patch should be applied both to the lk 3.17 tree
and Christoph's drivers-for-3.18 tree.

Dan Carpenter found a issue where be2iscsi would copy the ip
from userspace to the driver buffer before checking the len
of the data being copied:

In the reboot/shutdown path, workqueue was destroyed after the
adapter resource were freed. The task associated with workqueue
was getting executed after resources were freed. This lead to
kernel panic.

The uses of "rcu_assign_pointer()" are NULLing out the pointers.
According to RCU_INIT_POINTER()'s block comment:
"1. This use of RCU_INIT_POINTER() is NULLing out the pointer"
it is better to use it instead of rcu_assign_pointer() because it has a
smaller overhead.

The T10 Protection Information format is also used by some devices that
do not go through the SCSI layer (virtual block devices, NVMe). Relocate
the relevant functions to a block layer library that can be used without
involving SCSI.

Make the choice of checksum a per-I/O property by introducing a flag
that can be inspected by the SCSI layer. There are several reasons for
this:
1. It allows us to switch choice of checksum without unloading and
reloading the HBA driver.
2. During error recovery we need to be able to tell the HBA that
checksums read from disk should not be verified and converted to IP
checksums.
3. For error injection purposes we need to be able to write a bad guard
tag to storage. Since the storage device only supports T10 CRC we
need to be able to disable IP checksum conversion on the HBA.

So far we have relied on the app tag size to determine whether a disk
has been formatted with T10 protection information or not. However, not
all target devices provide application tag storage.
Add a flag to the block integrity profile that indicates whether the
disk has been formatted with protection information.

Instead of the "operate" parameter we pass in a seed value and a pointer
to a function that can be used to process the integrity metadata. The
generation function is changed to have a return value to fit into this
scheme.

The protection interval is not necessarily tied to the logical block
size of a block device. Stop using the terms "sector" and "sectors".
Going forward we will use the term "seed" to describe the initial
reference tag value for a given I/O. "Interval" will be used to describe
the portion of the data buffer that a given piece of protection
information is associated with.

None of the filesystems appear interested in using the integrity tagging
feature. Potentially because very few storage devices actually permit
using the application tag space.
Remove the tagging functions.

For commands like REQ_COPY we need a way to pass extra information along
with each bio. Like integrity metadata this information must be
available at the bottom of the stack so bi_private does not suffice.
Rename the existing bi_integrity field to bi_special and make it a union
so we can have different bio extensions for each class of command.
We previously used bi_integrity != NULL as a way to identify whether a
bio had integrity metadata or not. Introduce a REQ_INTEGRITY to be the
indicator now that bi_special can contain different things.
In addition, bio_integrity(bio) will now return a pointer to the
integrity payload (when applicable).

Through the qla target code, the qlt_send_term_exchange() routine
is used in various different places to cleanup an exchange. For the
case of IOCB request queue is full, the exchange is left unhandled/
dangling. Existing code does not have re-try logic to cleanup the
exchange. This patch add retry logic to cleanup the exchange before
letting new commands through.
For the case of FW running out of exchanges, driver need to reply
SAM_STAT_BUSY to the initiators. This patch add a pending queue
for the busy reply in case IOCB queue is unable to handle the cmd.

ABTS path takes path similar to regular task management, but
reset_count was not initialized when allocating mcmd causing
the response to get dropped in qlt_xmit_tm_rsp. Fix this by
initializing reset_count correctly.

A chip reset can occur after driver submits command to the stack. Abort
command processing if a chip reset has occurred or in progress when you
get a follow up for a command.

Sequence level error recovery (aka FC Tape) is not really required
for disk devices. On heavily loaded system, with slow turn around,
a bunch of status enquiries using REC puts additional burden to the
target, so just turn off SLER by default.
