Introduce XHCI_SUSPEND_RESUME_CLKS quirk as a means to suspend and resume
clocks if the hardware is capable of doing so. We assume that clocks will
be needed if the device may wake.

usb_maxpacket() looks up the endpoint number in the pipe which can fail
if the interface or configuration changes before the routine is called.
This is unexpected and may even cause a modulo by zero afterwards.
So use usb_endpoint_maxp() routine which uses the endpoint stored in URB
to get the maxpacket.

Turning off port power in shutdown did cause issues such as a laptop not
proprly powering off, and some specific usb devies failing to enumerate the
subsequent boot after a warm reset.
So revert this.

After xHC controller is started, either in probe or resume, it can take
a while before any of the connected usb devices are visible to the roothub
due to link training.
It's possible xhci driver loads, sees no acivity and suspends the host
before the USB device is visible.
In one testcase with a hotplugged xHC controller the host finally detected
the connected USB device and generated a wake 500ms after host initial
start.
If hosts didn't suspend the device duringe training it probablty wouldn't
take up to 500ms to detect it, but looking at specs reveal USB3 link
training has a couple long timeout values, such as 120ms
RxDetectQuietTimeout, and 360ms PollingLFPSTimeout.
So Add a 500ms grace period that keeps polling the roothub for 500ms after
start, preventing runtime suspend until USB devices are detected.

The remove path in xhci platform driver tries to remove and put both main
and shared hcds even if only a main hcd exists (one roothub)
This causes a null pointer dereference in reboot for those controllers.
Check that the shared_hcd exists before trying to remove it.

This happens when @udev->reset_resume is set to true, when usb resume,
the flow as below:
- hub_resume
- usb_disable_interface
- usb_disable_endpoint
- usb_hcd_disable_endpoint
- xhci_endpoint_disable // it set @ep->hcpriv to NULL
Then when reset usb device, it will drop allocated endpoints,
the flow as below:
- usb_reset_and_verify_device
- usb_hcd_alloc_bandwidth
- xhci_mtk_drop_ep
but @ep->hcpriv is already set to NULL, the bandwidth will be not
released anymore.
Due to the added endponts are stored in hash table, we can drop the check
of @ep->hcpriv.

Currently uses the worst case byte budgets on FS/LS bus bandwidth,
for example, for an isochronos IN endpoint with 192 bytes budget, it
will consume the whole 5 uframes(188 * 5) while the actual FS bus
budget should be just 192 bytes. It cause that many usb audio headsets
with 3 interfaces (audio input, audio output, and HID) cannot be
configured.
To improve it, changes to use "approximate" best case budget for FS/LS
bandwidth management. For the same endpoint from the above example,
the approximate best case budget is now reduced to (188 * 2) bytes.

In patch a1a2b7125e (Drop static setup of IRQ resource from DT

drivers were supposed to have switched to platform_get_irq()

The sa1111 platform is one of the two remaining users of the old Arm
specific "dmabounce" code, which is an earlier implementation of the
generic swiotlb.
Linus Walleij submitted a patch that removes dmabounce support from
the ixp4xx, and I had a look at the other user, which is the sa1111
companion chip.
Looking at how dmabounce is used, I could narrow it down to one driver
one three machines:
- dmabounce is only initialized on assabet/neponset, jornada720 and
badge4, which are the platforms that have an sa1111 and support
DMA on it.
- All three of these suffer from "erratum #7" that requires only
doing DMA to half the memory sections based on one of the address
lines, in addition, the neponset also can't DMA to the RAM that
is connected to sa1111 itself.
- the pxa lubbock machine also has sa1111, but does not support DMA
on it and does not set dmabounce.
- only the OHCI and audio devices on sa1111 support DMA, but as
there is no audio driver for this hardware, only OHCI remains.
In the OHCI code, I noticed that two other platforms already have
a local bounce buffer support in the form of the "local_mem"
allocator. Specifically, TMIO and SM501 use this on a few other ARM
boards with 16KB or 128KB of local SRAM that can be accessed from the
OHCI and from the CPU.
While this is not the same problem as on sa1111, I could not find a
reason why we can't re-use the existing implementation but replace the
physical SRAM address mapping with a locally allocated DMA buffer.
There are two main downsides:
- rather than using a dynamically sized pool, this buffer needs
to be allocated at probe time using a fixed size. Without
having any idea of what it should be, I picked a size of
64KB, which is between what the other two OHCI front-ends use
in their SRAM. If anyone has a better idea what that size
is reasonable, this can be trivially changed.
- Previously, only USB transfers to unaddressable memory needed
to go through the bounce buffer, now all of them do, which may
impact runtime performance for USB endpoints that do a lot of
transfers.
On the upside, the local_mem support uses write-combining buffers,
which should be a bit faster for transfers to the device compared to
normal uncached coherent memory as used in dmabounce.

If ports are not turned off in shutdown then runtime suspended
self-powered USB devices may survive in U3 link state over S5.
During subsequent boot, if firmware sends an IPC command to program
the port in DISCONNECT state, it will time out, causing significant
delay in the boot time.
Turning off roothub port power is also recommended in xhci
specification 4.19.4 "Port Power" in the additional note.

irq is disabled in xhci_quiesce(called by xhci_halt, with bit:2 cleared
in USBCMD register), but xhci_run(called by usb_add_hcd) re-enable it.
It's possible that you will receive thousands of interrupt requests
after initialization for 2.0 roothub. And you will get a lot of
warning like, "xHCI dying, ignoring interrupt. Shouldn't IRQs be
disabled?". This amount of interrupt requests will cause the entire
system to freeze.
This problem was first found on a device with ASM2142 host controller
on it.

The Octeon usb driver has been in staging for a long time and used in
Ubiquiti routers for a while now.
It's been built and then tested on real hardware with several usb devices
and it is proven to be stable and ready to be moved to its proper place
in the kernel tree.
Move it to drivers/usb/host and adjust its Makefile, Kconfig and defconfig
dependencies.
Many thanks to the developers who made it happen.

In the re-init path xhci_resume() passes 'hcd->primary_hcd' to hci_init(),
however this field isn't initialized by __usb_create_hcd() for a HCD
without secondary controller.
xhci_resume() is called once per xHC device, not per hcd, so the extra
checking for primary hcd can be removed.

Add support controller reset via a reset-controller usually in infracfg,
it's different with the software reset by IPPC which only used to reset MAC,
and it will also reset IPPC meanwhile.

When Linux is running under OP-TEE, the SFR is set as secured and thus
the AT91_OHCIICR_USB_SUSPEND register isn't accessible. Add a SMC to
do the appropriate call to suspend the controller.
The SMC id is fetched from the device-tree property
"microchip,suspend-smc-id". if present, then the syscon regmap is not
used to enter suspend and a SMC is issued.

5.18-rc1. For the most part it's been a quiet development cycle for the
USB core, but there are the usual "hot spots" of development activity.

- usb_maxpacket() api changes to remove an unneeded and unused
parameter.
- usb-serial driver device id updates and small cleanups
- typec cleanups and fixes based on device testing
- device tree updates for usb properties

All of these have been in linux-next for weeks with no reported
problems.

This series has been 12 years in the making, it mostly finishes the
work that was started with the founding of Linaro to clean up platform
support in the kernel.
The largest change here is a cleanup of the omap1 platform, which
is the final ARM machine type to get converted to the common-clk
subsystem. All the omap1 specific drivers are now made independent of the
mach/*.h headers to allow the platform to be part of a generic ARMv4/v5
multiplatform kernel. The last bit that enables this support is still
missing here while we wait for some last dependencies to make it into
the mainline kernel through other subsystems.
The s3c24xx, ixp4xx, iop32x, ep93xx and dove platforms were all almost
at the point of allowing multiplatform kernels, this work gets completed
here along with a few additional cleanup. At the same time, the s3c24xx
and s3c64xx are now deprecated and expected to get removed in the future.

initcall_debug shows that OHCI controllers take ~60ms to probe on
Rockchip RK3399 systems:
probe of fe3a0000.usb returned 1 after 58941 usecs
A few of these can add up to waste non-trivial amounts of time at boot.
These host controllers don't provide resources to other drivers, so
this shouldn't contribute to exposing race conditions.
Chrome OS kernels have carried this patch on some systems for a while
without issues. Similar patches have been merged for a variety of (e)MMC
host controllers for similar reasons.

The support for xHCI controllers with only one roothub, and the code
to defer primary roothub registation until second roothub got merged
to usb-next for 5.19 at the same time.

These got merged in such a way that the flag to defer primary roothub
registration is set even for xHC controllers with just one roothub.

The bandwidth budget table is introduced to trace ideal bandwidth used
by each INT/ISOC endpoint, but in fact the endpoint may consume more
bandwidth and cause data transfer error, so it's better to leave some
margin. Obviously it's difficult to find the best margin for all cases,
instead take use of the worst-case scenario.

Due to the scheduler allocates the optimal bandwidth for FS ISOC endpoints,
this may be not enough actually and causes data transfer error, so come up
with an estimate that is no less than the worst case bandwidth used for
any one mframe, but may be an over-estimate.

The XHCI_RESET_EP_QUIRK was added in 2009 to support prototype xHC
hardware from Fresco Logic that needed an additional configure endpoint
command after a reset endpoint.
That hardware should not have made it to the market.
Now, 13 years later its about time we get rid of it.
quirk was added in commit ac9d8fe7c6 ("USB: xhci: Add quirk for Fresco
Logic xHCI hardware.")
Print a debug message about the removed quirk if against all odds we run
into this controller.

Don't enable U1 or U2 Link powermanagenet (LPM) states for USB3
devices connected to tier 2 or further hubs.
For unknown reasons we previously only prevented U1.
Be consistent, and prevent both U1/U2 states if tier policy doesn't
allow LPM.
Also check the tier policy a bit earlier, and return if U1/U2 is
not allowed. This avoids unnecessary xhci MEL commands.

The 'stop endpoint' command timer was started when a 'stop endpoint'
command was added to the command queue.
This can trigger unwanted timeouts if there are several pending commands
in the queue that xHC needs to handle first.
The generic command timer, which was added later than the 'stop endpoint'
timeout timer, times each command currently being handled by xHC hardware.
A timed out stop endpoint command was treated as a more severe issue than
other failed commands, so the separate stop endpoint timer was left
unchanged.
Use the generic command timer for stop endpoint commands. Identify if
the timed out command was a stop endpoint command in the generic handler,
and treat it with the same severity as earlier.

This patch is in preparation of an extension where in case of a
root hub with no ports no shared hcd will be created.
Whether one of the root hubs has no ports we figure our in
usb_add_hcd() for the primary hcd. Therefore create the shared hcd
only after this call.

Factoring out parts of xhci_gen_setup() has two motivations:
- When adding functionaliy to omit shared hcd if not needed in a
subsequent patch, we'll have to call xhci_hcd_init_usb3_data()
from two places.
- It reduces size of xhci_gen_setup() and makes it better readable.

Set "HCD_FLAG_DEFER_RH_REGISTER" to hcd->flags in xhci_run() to defer
registering primary roothub in usb_add_hcd() if xhci has two roothubs.
This will make sure both primary roothub and secondary roothub will be
registered along with the second HCD.
This is required for cold plugged USB devices to be detected in certain
PCIe USB cards (like Inateck USB card connected to AM64 EVM or J7200 EVM).
This patch has been added and reverted earier as it triggered a race
in usb device enumeration.
That race is now fixed in 5.16-rc3, and in stable back to 5.4

The third argument of usb_maxpacket(): in_out has been deprecated
because it could be derived from the second argument (e.g. using
usb_pipeout(pipe)).
N.B. function usb_maxpacket() was made variadic to accommodate the
transition from the old prototype with three arguments to the new one
with only two arguments (so that no renaming is needed). The variadic
argument is to be removed once all users of usb_maxpacket() get
migrated.

There is a deadlock in oxu_bus_suspend(), which is shown below:

We hold oxu->lock in position (1) of thread 1, and use
del_timer_sync() to wait timer to stop, but timer handler
also need oxu->lock in position (2) of thread 2. As a result,
oxu_bus_suspend() will block forever.
This patch extracts del_timer_sync() from the protection of
spin_lock_irq(), which could let timer handler to obtain
the needed lock.

The driver, OMAP1 specific, now omits clk_prepare/unprepare() steps, not
supported by OMAP1 custom implementation of clock API. However, non-CCF
stubs of those functions exist for use on such platforms until converted
to CCF.
Update the driver to be compatible with CCF implementation of clock API.

It is not recommened to use platform_get_resource(pdev, IORESOURCE_IRQ)
for requesting IRQ's resources any more, as they can be not ready yet in
case of DT-booting.
platform_get_irq() instead is a recommended way for getting IRQ even if
it was not retrieved earlier.
It also makes code simpler because we're getting "int" value right away
and no conversion from resource to int is required.

powerpc's asm/prom.h brings some headers that it doesn't
need itself.
In order to clean it up in a further step, first clean all
files that include asm/prom.h
Some files don't need asm/prom.h at all. For those ones,
just remove inclusion of asm/prom.h
Some files don't need any of the items provided by asm/prom.h,
but need some of the headers included by asm/prom.h. For those
ones, add the needed headers that are brought by asm/prom.h at
the moment, then remove asm/prom.h
Some files really need asm/prom.h but also need some of the
headers included by asm/prom.h. For those one, leave asm/prom.h
but also add the needed headers so that they can be removed
from asm/prom.h in a later step.

In zhaoxin platform, some ehci projects will latch a wakeup signal
internal when plug in a device on port during system S0. This wakeup
signal will turn on when ehci runtime suspend, which will trigger a
system control interrupt that will resume ehci back to D0. As no
device connect, ehci will be set to runtime suspend and turn on the
internal latched wakeup signal again. It will cause a suspend-resume
loop and generate system control interrupt continuously.
Fixed this issue by clear wakeup signal latched in ehci internal when
ehci resume callback is called.

pm_runtime_get_sync will increment pm usage counter
even it failed. Forgetting to putting operation will
result in reference leak here. We fix it by replacing
it with pm_runtime_resume_and_get to keep usage counter
balanced.

different places, with little hope of completely cleaning that up.
Split up the file into a portion that becomes a linux-wide header

The mach/hardware.h is included in lots of places, and it provides
three different things on pxa:
- the cpu_is_pxa* macros
- an indirect inclusion of mach/addr-map.h
- the __REG() and io_pv2() helper macros
Split it up into separate <linux/soc/pxa/cpu.h> and mach/pxa-regs.h
headers, then change all the files that use mach/hardware.h to
include the exact set of those three headers that they actually
need, allowing for further more targeted cleanup.
linux/soc/pxa/cpu.h can remain permanently exported and is now in
a global location along with similar headers. pxa-regs.h and
addr-map.h are only used in a very small number of drivers now
and can be moved to arch/arm/mach-pxa/ directly when those drivers
are to pass the necessary data as resources.

The first U3 wake signal by the host may be lost if the USB 3 connection is
tunneled over USB4, with a runtime suspended USB4 host, and firmware
implemented connection manager.
Specs state the host must wait 100ms (tU3WakeupRetryDelay) before
resending a U3 wake signal if device doesn't respond, leading to U3 -> U0
link transition times around 270ms in the tunneled case.

While rebooting, XHCI controller and its bus device will be shut down
in order by .shutdown callback. Stopping roothubs polling in
xhci_shutdown() can prevent XHCI driver from accessing port status
after its bus device shutdown.
Take PCIe XHCI controller as example, if XHCI driver doesn't stop roothubs
polling, XHCI driver may access PCIe BAR register for port status after
parent PCIe root port driver is shutdown and cause PCIe bus error.

In some situations software handles TRB events slower than adding TRBs.
If the number of TRB events to be processed in a given interrupt is exactly
the same as the event ring size 256, then the local variable
"event_ring_deq" that holds the initial dequeue position is equal to
software_dequeue after handling all 256 interrupts.
It will cause driver to not update ERDP to hardware,
Software dequeue pointer is out of sync with ERDP on interrupt exit.
On the next interrupt, the event ring may full but driver will not
update ERDP as software_dequeue is equal to ERDP.

Hardware ERDP is updated mid event handling if there are more than 128
events in an interrupt (half of ring size).
Fix this by updating the software local variable at the same time as

The overwhelming bulk of this pull request is a change from Uwe
Kleine-König which changes the return type of the remove() function to
void as part of some wider work he's doing to do this for all bus types,
causing updates to most SPI device drivers. The branch with that on has
been cross merged with a couple of other trees which added new SPI
drivers this cycle, I'm not expecting any build issues resulting from
the change.
Otherwise it's been a relatively quiet release with some new device
support, a few minor features and the welcome completion of the
conversion of the subsystem to use GPIO descriptors rather than numbers:

Make sure a malicious backend can't cause any harm other than wrong
I/O data.
Missing are verification of the request id in a response, sanitizing

The function usb_endpoint_maxp() (called by usb_maxpacket()) already
does the sanitazation of the USB endpoint max packet size. The call to
max_packet() does the same thing and is thus removed.
However, the macro max_packet() is kept because it is used elsewhere
in the file.

The usage of gnttab_end_foreign_access() in xenhcd_gnttab_done() is
not safe against a malicious backend, as the backend could keep the
I/O page mapped and modify it even after the granted memory page is
being used for completely other purposes in the local system.
So replace that use case with gnttab_try_end_foreign_access() and
disable the PV host adapter in case the backend didn't stop using the
granted page.
In xenhcd_urb_request_done() immediately return in case of setting
the device state to "error" instead of looking into further backend
responses.

A race between system resume and device-initiated resume may result in
runtime PM imbalance on USB2 root hub. If a device-initiated resume
starts and system resume xhci_bus_resume() directs U0 before hub driver
sees the resuming device in RESUME state, device-initiated resume will
not be finished in xhci_handle_usb2_port_link_resume(). In this case,
usb_hcd_end_port_resume() call is missing.
This changes calls usb_hcd_end_port_resume() if resuming device reaches
U0 to keep runtime PM balance.

xhci_decode_ctrl_ctx() returns the untouched buffer as-is if both "drop"
and "add" parameters are zero.
Fix the function to return an empty string in that case.
It was not immediately clear from the possible call chains whether this
issue is currently actually triggerable or not.
Note that before commit 4843b4b5ec ("xhci: fix even more unsafe memory
usage in xhci tracing") the result effect in the failure case was different
as a static buffer was used here, but the code still worked incorrectly.

xhci_decode_usbsts() is expected to return a zero-terminated string by
its only caller, xhci_stop_endpoint_command_watchdog(), which directly
logs the return value:
xhci_warn(xhci, "USBSTS:%s\n", xhci_decode_usbsts(str, usbsts));
However, if no recognized bits are set in usbsts, the function will
return without having called any sprintf() and therefore return an
untouched non-zero-terminated caller-provided buffer, causing garbage
to be output to log.
Fix that by always including the raw value in the output.
Note that before commit 4843b4b5ec ("xhci: fix even more unsafe memory
usage in xhci tracing") the result effect in the failure case was different
as a static buffer was used here, but the code still worked incorrectly.

xhci_reset() timeout was increased from 250ms to 10 seconds in order to
give Renesas 720201 xHC enough time to get ready in probe.
xhci_reset() is called with interrupts disabled in other places, and
waiting for 10 seconds there is not acceptable.
Add a timeout parameter to xhci_reset(), and adjust it back to 250ms
when called from xhci_stop() or xhci_shutdown() where interrupts are
disabled, and successful reset isn't that critical.
This solves issues when deactivating host mode on platforms like SM8450.
For now don't change the timeout if xHC is reset in xhci_resume().
No issues are reported for it, and we need the reset to succeed.
Locking around that reset needs to be revisited later.
Additionally change the signed integer timeout parameter in
xhci_handshake() to a u64 to match the timeout value we pass to
readl_poll_timeout_atomic()

This reverts commit 3241929b67.
Armada 3720 phy driver (phy-mvebu-a3700-comphy.c) does not return
-EOPNOTSUPP from phy_power_on() callback anymore.
So remove XHCI_SKIP_PHY_INIT flag from xhci_mvebu_a3700_plat_setup() and
then also whole xhci_mvebu_a3700_plat_setup() function which is there just
to handle -EOPNOTSUPP for XHCI_SKIP_PHY_INIT.
xhci plat_setup callback is not used by any other xhci plat driver, so
remove this callback completely.

At the end of qtd_fill(), we assign the 'int count' variable to the 'size_t
length' field of 'struct ehci_qtd' -- which implies a problematic type cast.
Let's make that variable and the function's result *unsigned int* instead...

Every lpm commmand, both for USB 2 and USB 3 devies used the same
xhci->lpm_command structure to change max exit latency.
xhci->lpm_command is only protected by a hcd->bandwidth mutex, which is
not enoungh as USB 2 and USB 3 devices are behind separate HCDs.
Simplify code and avoid unnecessary locking risks by allocating
separate command structures for each lpm command, just like with
all other commands.

To support systems with several xhci controllers with active
dbc on each xhci we need to use IDR to identify and give
an index to each port.
Avoid using global struct tty_driver.driver_state for storing
dbc port pointer as it won't work with several dbc ports

The current workaround to call the dbc_tty_init() in probe is
not working in case we have several xhci devices with dbc enabled.
dbc_tty_init() should be called only once by a module init call when
module is loaded.
until dbgtty is its own module call dbc_tty_init() from xhci
module init call.
Same is true for unloading and dbc_tty_exit()

These names give the impression the functions are related to
module init calls, but are in fact creating and removing the dbc
fake device
Rename them to xhci_create_dbc_dev() and xhci_remove_dbc_dev().
We will need the _init and _exit names for actual dbc module init
and exit calls.

Refactor xhci_dbc_init(), splitting it into logical
parts closer to the Linux device model.
- Create the fake dbc device, depends on xhci strucure
- Allocate a dbc structure, xhci agnostic
- Call xhci_dbc_tty_probe(), similar to actual probe.
Adjustments to xhci_dbc_exit and xhci_dbc_remove are also needed
as a result to the xhci_dbc_init() changes
Mostly non-functional changes, except for creating the dbc sysfs
entry earlier, together with the dbc structure.

The -ENODEV return value from xhci_check_args() is incorrectly changed
to -EINVAL in a couple places before propagated further.
xhci_check_args() returns 4 types of value, -ENODEV, -EINVAL, 1 and 0.
xhci_urb_enqueue and xhci_check_streams_endpoint return -EINVAL if
the return value of xhci_check_args <= 0.
This causes problems for example r8152_submit_rx, calling usb_submit_urb
in drivers/net/usb/r8152.c.
r8152_submit_rx will never get -ENODEV after submiting an urb when xHC
is halted because xhci_urb_enqueue returns -EINVAL in the very beginning.

When HCE(Host Controller Error) is set, it means an internal
error condition has been detected. Software needs to re-initialize
the HC, so add this check in xhci resume.

The value returned by an spi driver's remove function is mostly ignored.
(Only an error message is printed if the value is non-zero that the
error is ignored.)
So change the prototype of the remove function to return no value. This
way driver authors are not tempted to assume that passing an error to
the upper layer is a good idea. All drivers are adapted accordingly.
There is no intended change of behaviour, all callbacks were prepared to
return 0 before.

The Fresco Logic FL1100 controller needs the TRUST_TX_LENGTH quirk like
other Fresco controllers, but should not have the BROKEN_MSI quirks set.
BROKEN_MSI quirk causes issues in detecting usb drives connected to docks
with this FL1100 controller.
The BROKEN_MSI flag was apparently accidentally set together with the
TRUST_TX_LENGTH quirk
Original patch went to stable so this should go there as well.

platform_get_resource(pdev, IORESOURCE_IRQ, ..) relies on static
allocation of IRQ resources in DT core code, this causes an issue
when using hierarchical interrupt domains using "interrupts" property
in the node as this bypasses the hierarchical setup and messes up the
irq chaining.
In preparation for removal of static setup of IRQ resource from DT core
code use platform_get_irq().

Introduces the Xen pvUSB frontend. With pvUSB it is possible for a Xen
domU to communicate with a USB device assigned to that domU. The
communication is all done via the pvUSB backend in a driver domain
(usually Dom0) which is owner of the physical device.
The pvUSB frontend is a USB hcd for a virtual USB host connector.
The code is taken from the pvUSB implementation in Xen done by Fujitsu
based on Linux kernel 2.6.18.
Changes from the original version are:
- port to upstream kernel
- put all code in just one source file
- move module to appropriate location in kernel tree
- adapt to Linux style guide
- minor code modifications to increase readability

Make xhci_disable_slot() synchronous, thus ensuring it, and
xhci_free_dev() calling it return after xHC controller completes
the disable slot command.
Otherwise the roothub and xHC host may runtime suspend, and clear the
command ring while the disable slot command is being processed.
This causes a command completion mismatch as the completion event can't
be mapped to the correct command.
Command ring gets out of sync and commands time out.
Driver finally assumes host is unresponsive and bails out.

When the xHCI is quirked with XHCI_RESET_ON_RESUME, runtime resume
routine also resets the controller.
This is bad for USB drivers without reset_resume callback, because
there's no subsequent call of usb_dev_complete() ->
usb_resume_complete() to force rebinding the driver to the device. For
instance, btusb device stops working after xHCI controller is runtime
resumed, if the controlled is quirked with XHCI_RESET_ON_RESUME.
So always take XHCI_RESET_ON_RESUME into account to solve the issue.

Turns out some xHC controllers require all 64 bits in the CRCR register
to be written to execute a command abort.
The lower 32 bits containing the command abort bit is written first.
In case the command ring stops before we write the upper 32 bits then
hardware may use these upper bits to set the commnd ring dequeue pointer.
Solve this by making sure the upper 32 bits contain a valid command
ring dequeue pointer.
The original patch that only wrote the first 32 to stop the ring went
to stable, so this fix should go there as well.

Some platforms, e.g. 8183/8192, use low level latch way to keep
wakeup signal, it may latch a wrong signal if debounce more time,
and enable wakeup function earlier.

latch wakeup_signal mistakenly at (1), should latch it at (2);
Workaround: delay about 100us to enable wakeup, meanwhile decrease
debounce time.
