0508c2f3b7 [SCSI] libsas: don't mark expanders as gone when a child device is removed
Commit 56dd2c06 "[SCSI] libsas: Don't issue commands to devices that
have been hot-removed" marked the parent device of an end-device as gone
when all the phys to the end device have been deleted.

The expander device is still present until its parent is removed.  This
is a benign change until the smp_execute_task() path is taught to check
->gone.

36a3994739 [SCSI] libsas: poll for ata device readiness after reset
Use ata_wait_after_reset() to poll for link recovery after a reset.
This combined with sas_ha->eh_mutex prevents expander rediscovery from
probing phys in an intermediate state.  Local discovery does not have a
mechanism to filter link status changes during this timeout, so it
remains the responsibility of lldds to prevent premature port teardown.
Although once all lldd's support ->lldd_ata_check_ready() that could be
used as a gate to local port teardown.

The signature fis is re-transmitted when the link comes back so we
should be revalidating the ata device class, but that is left to a future
patch.

1f4fe89c9c [SCSI] libsas: Remove redundant phy state notification calls.
In the case of an explicit sas_phy_enable call to disable a phy,
the LLDD provides the calls to sas_phy_disconnected and the
PHYE_LOSS_OF_SIGNAL event.

NOTE: This assumes that the lldd(s) generate the notification, which
appears to be the case, but only verfied on isci.

81c757bc69 [SCSI] libsas: execute transport link resets with libata-eh via host workqueue
Link resets leave ata affiliations intact, so arrange for libsas to make
an effort to avoid dropping the device due to a slow-to-recover link.
Towards this end carry out reset in the host workqueue so that it can
check for ata devices and kick the reset request to libata.  Hard
resets, in contrast, bypass libata since they are meant for associating
an ata device with another initiator in the domain (tears down
affiliations).

Need to add a new transport_sas_phy_reset() since the current
sas_phy_reset() is a utility function to libsas lldds.  They are not
prepared for it to loop back into eh.

b52df4174d [SCSI] libsas: use libata-eh-reset for sata rediscovery fis transmit failures
Since sata devices can take several seconds to recover the link on reset
the 0.5 seconds that libsas currently waits may not be enough.  Instead
if we are rediscovering a phy that was previously attached to a sata
device let libata handle any resets to encourage the device to transmit
the initial fis.

Once sas_ata_hard_reset() and lldds learn how to honor 'deadline' libsas
should stop encountering phys in an intermediate state, until then this
will loop until the fis is transmitted or ->attached_sas_addr gets
cleared, but in the more likely initial discovery case we keep existing
behavior.

3a2cdf391b [SCSI] libsas: defer SAS_TASK_NEED_DEV_RESET commands to libata
lldds use the SAS_TASK_NEED_DEV_RESET interface to request that eh
perform a reset.  In the sata device case defer the commands that
triggered the reset to libata-eh context so it can perform its pre and
post reset management.

In the sas_ata_post_internal() case the reset request is falling on deaf
ears as the sas_task is immediately destroyed without any reset action.
Since it is currently a nop, and likely superfluous given the conversion
to new-style libata-eh, just drop the request.

3944f50995 [SCSI] libsas: let libata handle command timeouts
libsas-eh if it successfully aborts an ata command will hide the timeout
condition (AC_ERR_TIMEOUT) from libata.  The command likely completes
with the all-zero task->task_status it started with.  Instead, interpret
a TMF_RESP_FUNC_COMPLETE as the end of the sas_task but keep the scmd
around for libata-eh to handle.

9095a64a9a [SCSI] libsas: fix timeout vs completion race
Until we have told the lldd to forget a task a timed out operation can
return from the hardware at any time.  Since completion frees the task
we need to make sure that no tasks run their normal completion handler
once eh has decided to manage the task.  Similar to
ata_scsi_cmd_error_handler() freeze completions to let eh judge the
outcome of the race.

Task collector mode is problematic because it presents a situation where
a task can be timed out and aborted before the lldd has even seen it.
For this case we need to guarantee that a task that an lldd has been
told to forget does not get queued after the lldd says "never seen it".
With sas_scsi_timed_out we achieve this with the ->task_queue_flush
mutex, rather than adding more time.

a3a142524a [SCSI] libsas: prevent double completion of scmds from eh
We invoke task->task_done() to free the task in the eh case, but at this
point we are prepared for scsi_eh_flush_done_q() to finish off the scmd.

Introduce sas_end_task() to capture the final response status from the
lldd and free the task.

Also take the opportunity to kill this warning.

3dff5721e4 [SCSI] libsas: close error handling vs sas_ata_task_done() race
Since sas_ata does not implement ->freeze(), completions for scmds and
internal commands can still arrive concurrent with
ata_scsi_cmd_error_handler() and sas_ata_post_internal() respectively.
By the time either of those is called libata has committed to completing
the qc, and the ATA_PFLAG_FROZEN flag tells sas_ata_task_done() it has
lost the race.

In the sas_ata_post_internal() case we take on the additional
responsibility of freeing the sas_task to close the race with
sas_ata_task_done() freeing the the task while sas_ata_post_internal()
is in the process of invoking ->lldd_abort_task().

e500a34b02 [SCSI] libsas: kill invocation of scsi_eh_finish_cmd from sas_ata_task_done
Prior to the conversion to the new-style libata-eh sas_ata_task_done()
may have been the last opportunity to clean up the scmd, but now
libata-eh explicitly handles this case.  It also races against sas-eh.
If a lldd completes a task after SAS_TASK_STATE_ABORTED is set it could
trigger a spurious decrement of shost->host_failed.  Current lldds have
the band-aid of checking SAS_TASK_STATE_ABORTED before calling
->task_done(), but better to just let the scmds escalate to libata for
race free cleanup.

b91bb29618 [SCSI] libsas: use ->set_dmamode to notify lldds of NCQ parameters
sas_discover_sata() notifies lldds of sata devices twice.  Once to allow
the 'identify' to be sent, and a second time to allow aic94xx (the only
libsas driver that cares about sata_dev.identify) to setup NCQ
parameters before the device becomes known to the midlayer.  Replace
this double notification and intervening 'identify' with an explicit
->lldd_ata_set_dmamode notification.  With this change all ata internal
commands are issued by libata, so we no longer need sas_issue_ata_cmd().

The data from the identify command only needs to be cached in one
location so ata_device.id replaces domain_device.sata_dev.identify.

87c8331fcf [SCSI] libsas: prevent domain rediscovery competing with ata error handling
libata error handling provides for a timeout for link recovery.  libsas
must not rescan for previously known devices in this interval otherwise
it may remove a device that is simply waiting for its link to recover.
Let libata-eh make the determination of when the link is stable and
prevent libsas (host workqueue) from taking action while this
determination is pending.

Using a mutex (ha->disco_mutex) to flush and disable revalidation while
eh is running requires any discovery action that may block on eh be
moved to its own context outside the lock.  Probing ATA devices
explicitly waits on ata-eh and the cache-flush-io issued during device
removal may also pend awaiting eh completion.  Essentially any rphy
add/remove activity needs to run outside the lock.

This adds two new cleanup states for sas_unregister_domain_devices()
'allocated-but-not-probed', and 'flagged-for-destruction'.  In the
'allocated-but-not-probed' state  dev->rphy points to a rphy that is
known to have not been through a sas_rphy_add() event.  At domain
teardown check if this device is still pending probe and cleanup
accordingly.  Similarly if a device has already been queued for removal
then sas_unregister_domain_devices has nothing to do.

312d3e5611 [SCSI] libsas: remove ata_port.lock management duties from lldds
Each libsas driver (mvsas, pm8001, and isci) has invented a different
method for managing the ap->lock.  The lock is held by the ata
->queuecommand() path.  mvsas drops it prior to acquiring any internal
locks which allows it to hold its internal lock across calls to
task->task_done().  This capability is important as it is the only way
the driver can flush task->task_done() instances to guarantee that it no
longer has any in-flight references to a domain_device at
->lldd_dev_gone() time.

b1124cd3ec [SCSI] libsas: introduce sas_drain_work()
When an lldd invokes ->notify_port_event() it can trigger a chain of libsas
events to:

  1/ form the port and find the direct attached device

  2/ if the attached device is an expander perform domain discovery

A call to flush_workqueue() will only flush the initial port formation work.
Currently libsas users need to call scsi_flush_work() up to the max depth of
chain (which will grow from 2 to 3 when ata discovery is moved to its own
discovery event).  Instead of open coding multiple calls switch to use
drain_workqueue() to flush sas work.

drain_workqueue() does not handle new work submitted during the drain so
libsas needs a bit of infrastructure to hold off unchained work submissions
while a drain is in flight.  A lldd ->notify() event is considered 'unchained'
while a sas_discover_event() is 'chained'.  As Tejun notes:

  "For now, I think it would be best to add private wrapper in libsas to
   support deferring unchained work items while draining."

735f7d2fed [SCSI] libsas: fix domain_device leak
Arrange for the deallocation of a struct domain_device object when it no
longer has:
1/ any children
2/ references by any scsi_targets
3/ references by a lldd

The comment about domain_device lifetime in
Documentation/scsi/libsas.txt is stale as it appears mainline never had
a version of a struct domain_device that was registered as a kobject.
We now manage domain_device reference counts on behalf of external
agents.

1a34c06401 [SCSI] libsas: fix port->dev_list locking
port->dev_list maintains a list of devices attached to a given sas root port.
It needs to be mutated under a lock as contexts outside of the
single-threaded-libsas-workqueue access the list via sas_find_dev_by_rphy().
Fixup locations where the list was being mutated without a lock.

This is a follow-up to commit 5911e963 "[SCSI] libsas: remove expander
from dev list on error", where Luben noted [1]:

    > 2/ We have unlocked list manipulations in sas_ex_discover_end_dev(),
    > sas_unregister_common_dev(), and sas_ex_discover_end_dev()

    Yes, I can see that and that is very unfortunate.

ac013ed1cb [SCSI] isci: export phy events via ->lldd_control_phy()
Allow the sas-transport-class to update events for local phys via a new
PHY_FUNC_GET_EVENTS command to ->lldd_control_phy().  Fixup drivers that
are not prepared for new enum phy_func values, and unify
->lldd_control_phy() error codes.

These are the SAS defined phy events that are reported in a
smp-report-phy-error-log command:
 * /sys/class/sas_phy/<phyX>/invalid_dword_count
 * /sys/class/sas_phy/<phyX>/running_disparity_error_count
 * /sys/class/sas_phy/<phyX>/loss_of_dword_sync_count

b50102d3e9 [SCSI] isci: atapi support
Based on original implementation from Jiangbi Liu and Maciej Trela.

ATAPI transfers happen in two-to-three stages.  The two stage atapi
commands are those that include a dma data transfer.  The data transfer
portion of these operations is handled by the hardware packet-dma
acceleration.  The three-stage commands do not have a data transfer and
are handled without hardware assistance in raw frame mode.

stage1: transmit host-to-device fis to notify the device of an incoming
atapi cdb.  Upon reception of the pio-setup-fis repost the task_context
to perform the dma transfer of the cdb+data (go to stage3), or repost
the task_context to transmit the cdb as a raw frame (go to stage 2).

stage2: wait for hardware notification of the cdb transmission and then
go to stage 3.

stage3: wait for the arrival of the terminating device-to-host fis and
terminate the command.

To keep the implementation simple we only support ATAPI packet-dma
protocol (for commands with data) to avoid needing to handle the data
transfer manually (like we do for SATA-PIO).  This may affect
compatibility for a small number of devices (see
ATA_HORKAGE_ATAPI_MOD16_DMA).

If the data-transfer underruns, or encounters an error the
device-to-host fis is expected to arrive in the unsolicited frame queue
to pass to libata for disposition.  However, in the DONE_UNEXP_FIS (data
underrun) case it appears we need to craft a response.  In the
DONE_REG_ERR case we do receive the UF and propagate it to libsas.

The queue-depth for libsas-attached devices initializes to 32 and can
only be increased manually via sysfs to a max of 64, while mpt2sas
attached devices initialize to 254 and dynamically float via the
midlayer ->change_queue_depth interface.

No performance regression was observed with this change on the isci
driver.

8ec6552f4a [SCSI] libsas: sgpio write support
Add SFF-8485 v0.7 / SAS-1 smp-write-gpio register support to libsas.
Defer SAS-2 support unless/until it defines an sgpio interface.

Minimum implementation needed to get the lights blinking.
try_test_sas_gpio_gp_bit() provides a common method to parse the
incoming write data (raw bitstream), and the to_sas_gpio_gp_bit() helper
routine can be used as a basis for the set/clear operations for the
'read' implementation.  Host implementations parse as many bits
(ODx.[012]) as are locally supported and report the number of registers
successfully written.  If the submitted data overruns the internal
number of registers available report the write as a success with the
number of bytes remaining reported in ->resid_len.

Example (assuming an active backplane) set the "identify" pattern for
the first 21 devices:

smp_write_gpio --count=2 --data=92,49,24,92,24,92,49,24 -t 4 --index=1 /dev/bsg/sas_hostX

24926dadc4 [SCSI] libsas: fix failure to revalidate domain for anything but the first expander child.
In an enclosure model where there are chaining expanders to a large body
of storage, it was discovered that libsas, responding to a broadcast
event change, would only revalidate the domain of first child expander
in the list.

The issue is that the pointer value to the discovered source device was
used to break out of the loop, rather than the content of the pointer.

This still remains non-compliant as the revalidate domain code is
supposed to loop through all child expanders, and not stop at the first
one it finds that reports a change count. However, the design of this
routine does not allow multiple device discoveries and that would be a
more complicated set of patches reserved for another day. We are fixing
the glaring bug rather than refactoring the code.

ac81c6a832 [SCSI] libsas: fix sas_queuecommand return values
->queuecommand must return either 0, or one of the SCSI_MLQUEUE_* return
values.  Non-transient errors are indicated by setting cmd->result before
calling ->scsi_done and returning 0.  Fix libsas to adhere to this calling
convention.  Note that the DID_ERROR for returns from the low-level driver
might not be correct for all cases, but it's the best we can do with
the current layering in libsas.  I also suspect that the pre-existing
handling of -SAS_QUEUE_FULL should really be SCSI_MLQUEUE_HOST_BUSY, but
I'll leave that for a separate change.

Commit 56dd2c06 "libsas: Don't issue commands to devices that have been
hot-removed" edited Darrick's original patch to remove setting 'gone' in
the sas_deform_port() path because that prevented scsi sync cache
commands from being issued when the driver was unloaded.  However, this
allows true device gone notifications (as signaled port phy events) to
trigger sync cache commands to devices that are known to be unreachable.

Teach libsas which sas_deform_port() invocations are likely device gone
events.

a82058a730 [SCSI] libsas: fix ata list corruption issue
I think this stems from a misunderstanding of how the ata error handler
works.  ata_scsi_cmd_error_handler() gets called with a passed in list
of commands to handle.  However, that list may still not be empty when
it exits.  The command ata_scsi_port_error_handler() must be called
(which takes no list) before the list will be completely emptied.  This
bites the sas error handler because the two are called from different
functions and the original list has gone out of scope before
ata_scsi_port_error_handler() is called. leading to some commands
dangling on bare stack, which is a potential memory corruption issue.
Fix this by manually deleting all outstanding commands from the on-stack
list before it goes out of scope.

ATAPI check condition needs to be treated the same as a success or
protocol return.  The register returns from the PACKET command are all
correctly positioned in the device to host register FIS and so we
should collect them properly.  Right at the moment this doesn't matter
because libata sends a request sense always for ATAPI errors, but if
it ever checked the registers, we should have the correct contents
just in case.

9ee91f7fb5 [SCSI] libsas: fix runaway error handler problem
libsas makes use of scsi_schedule_eh() but forgets to clear the
host_eh_scheduled flag in its error handling routine.  Because of this,
the error handler thread never gets to sleep; it's constantly awake and
trying to run the error routine leading to console spew and inability to
run anything else (at least on a UP system).  The fix is to clear the
flag as we splice the work queue.

00f0254ed9 [SCSI] libsas: fix definition of wideport, include local sas address
To date libsas has only looked at the attached sas address when
determining the formation of wide ports.  The specification and some
hardware expects that phys with different addresses will not form a wide
port unless the local peer phys also match each other.  Introduce a flag
to select stricter behavior at sas_register_ha() time.  The flag can be
dropped once it is known that all libsas users expect the same behavior.

Current drivers just initialize this field to zero and get the
traditional behavior.

f281233d3e SCSI host lock push-down
Move the mid-layer's ->queuecommand() invocation from being locked
with the host lock to being unlocked to facilitate speeding up the
critical path for drivers who don't need this lock taken anyway.

The patch below presents a simple SCSI host lock push-down as an
equivalent transformation.  No locking or other behavior should change
with this patch.  All existing bugs and locking orders are preserved.

Additionally, add one parameter to queuecommand,
	struct Scsi_Host *
and remove one parameter from queuecommand,
	void (*done)(struct scsi_cmnd *)

Scsi_Host* is a convenient pointer that most host drivers need anyway,
and 'done' is redundant to struct scsi_cmnd->scsi_done.

Minimal code disturbance was attempted with this change.  Most drivers
needed only two one-line modifications for their host lock push-down.

3e4ec3443f libata: kill ATA_FLAG_DISABLED
ATA_FLAG_DISABLED is only used by drivers which don't use
->error_handler framework and is largely broken.  Its only meaningful
function is to make irq handlers skip processing if the flag is set,
which is largely useless and even harmful as it makes those ports more
likely to cause IRQ storms.

Kill ATA_FLAG_DISABLED and makes the callers disable attached devices
instead.  ata_port_probe() and ata_port_disable() which manipulate the
flag are also killed.

This simplifies condition check in IRQ handlers.  While updating IRQ
handlers, remove ap NULL check as libata guarantees consecutive port
allocation (unoccupied ports are initialized with dummies) and
long-obsolete ATA_QCFLAG_ACTIVE check (checked by ata_qc_from_tag()).

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

e881a172da [SCSI] modify change_queue_depth to take in reason why it is being called
This patch modifies scsi_host_template->change_queue_depth so that
it takes an argument indicating why it is being called. This will be
used so that if a LLD needs to do some extra processing when
handling queue fulls or later ramp ups, it can do so.

This is a simple port of the drivers setting a change_queue_depth
callback. In the patch I just have these LLDs adjust the queue depth
if the user was requesting it.

5381837f12 [SCSI] libsas: reuse the original port when hotplugging phys in wide ports
There's a hotplug problem in the way libsas allocates ports: it loops over the
available ports first trying to add to an existing for a wide port and
otherwise allocating the next free port.  This scheme only works if the port
array is packed from zero, which fails if a port gets hot unplugged and the
array becomes sparse.  In that case, a new port is formed even if there's a
wide port it should be part of.  Fix this by creating two loops over all the
ports:  the first to see if the phy should be part of a wide port and the
second to form a new port in an empty port slot.

5f49f63178 block: set rq->resid_len to blk_rq_bytes() on issue
In commit c3a4d78c58, while introducing
rq->resid_len, the default value of residue count was changed from
full count to zero.  The conversion was done under the assumption that
when a request fails residue count wasn't defined.  However, Boaz and
James pointed out that this wasn't true and the residue count should
be preserved for failed requests too.

This patchset restores the original behavior by setting rq->resid_len
to blk_rq_bytes(rq) on request start and restoring explicit clearing
in affected drivers.  While at it, take advantage of the fact that
rq->resid_len is set to full count where applicable.

* ide-cd: rq->resid_len cleared on pc success

* mptsas: req->resid_len cleared on success

* sas_expander: rsp/req->resid_len cleared on success

* mpt2sas_transport: req->resid_len cleared on success

* ide-cd, ide-tape, mptsas, sas_host_smp, mpt2sas_transport, ub: take
  advantage of initial full count to simplify code

c3a4d78c58 block: add rq->resid_len
rq->data_len served two purposes - the length of data buffer on issue
and the residual count on completion.  This duality creates some
headaches.

First of all, block layer and low level drivers can't really determine
what rq->data_len contains while a request is executing.  It could be
the total request length or it coulde be anything else one of the
lower layers is using to keep track of residual count.  This
complicates things because blk_rq_bytes() and thus
[__]blk_end_request_all() relies on rq->data_len for PC commands.
Drivers which want to report residual count should first cache the
total request length, update rq->data_len and then complete the
request with the cached data length.

Secondly, it makes requests default to reporting full residual count,
ie. reporting that no data transfer occurred.  The residual count is
an exception not the norm; however, the driver should clear
rq->data_len to zero to signify the normal cases while leaving it
alone means no data transfer occurred at all.  This reverse default
behavior complicates code unnecessarily and renders block PC on some
drivers (ide-tape/floppy) unuseable.

This patch adds rq->resid_len which is used only for residual count.

While at it, remove now unnecessasry blk_rq_bytes() caching in
ide_pc_intr() as rq->data_len is not changed anymore.

242f9dcb8b block: unify request timeout handling
Right now SCSI and others do their own command timeout handling.
Move those bits to the block layer.

Instead of having a timer per command, we try to be a bit more clever
and simply have one per-queue. This avoids the overhead of having to
tear down and setup a timer for each command, so it will result in a lot
less timer fiddling.

82ef04fb4c libata: make SCR access ops per-link
Logically, SCR access ops should take @link; however, there was no
compelling reason to convert all SCR access ops when adding @link
abstraction as there's one-to-one mapping between a port and a non-PMP
link.  However, that assumption won't hold anymore with the scheduled
addition of slave link.

4c9bf4e799 libata: replace tf_read with qc_fill_rtf for non-SFF drivers
Now that all SFF stuff is separated out of core layer, core layer
doesn't call ops->tf_read directly.  It gets called only via
ops->qc_fill_rtf() for non-SFF drivers.  This patch directly
implements private ops->qc_fill_rtf() for non-SFF controllers and kill
ops->tf_read().

This is much cleaner for non-SFF controllers as some of them have to
cache SFF register values in private data structure and report the
cached values via ops->tf_read().  Also, ops->tf_read() gets nasty for
controllers which don't have clear notion of TF registers when
operation is not in progress.

As this change makes default ops->qc_fill_rtf unnecessary, move
ata_sff_qc_fill_rtf() form ata_base_port_ops to ata_sff_port_ops where
it belongs.

22183bf569 libata: add qc_fill_rtf port operation
On command completion, ata_qc_complete() directly called ops->tf_read
to fill qc->result_tf.  This patch adds ops->qc_fill_rtf to replace
hardcoded ops->tf_read usage.

ata_sff_qc_fill_rtf() which uses ops->tf_read to fill result_tf is
implemented and set in ata_base_port_ops and other ops tables which
don't inherit from ata_base_port_ops, so this patch doesn't introduce
any behavior change.

ops->qc_fill_rtf() is similar to ops->sff_tf_read() but can only be
called when a command finishes.  As some non-SFF controllers don't
have TF registers defined unless they're associated with in-flight
commands, this limited operation makes life easier for those drivers
and help lifting SFF assumptions from libata core layer.

a29c051536 [SCSI] libsas: use the supplied address for SATA devices rather than changing it
Once the phy reset is plumbed in properly, SATA error handling fails
nastily because we change the port attached_sas_address using the WWN
field of the IDENTIFY message.  This is a nice thing to do in theory,
but it really destroys hotplug because any event on the port causes an
automatic mismatch between the sas_address the phy just picked up and
the one we propagate into the port.  However ugly they are, we have to
stick with the sas addresses made up by the phys and expanders.

Also does a few cosmetic changes to the way port printing is done to
make it clearer how a port is formed.

a8e14fec16 [SCSI] libsas: fix error handling
The libsas error handler has two fairly fatal bugs

1. scsi_sas_task_done calls scsi_eh_finish_cmd() too early.  This
   happens if the task completes after it has been aborted but before
   the error handler starts up.  Because scsi_eh_finish_cmd()
   decrements host_failed and adds the task to the done list, the
   error handler start check (host_failed == host_busy) never passes
   and the eh never starts.

2. The multiple task completion paths sas_scsi_clear_queue_... all
   simply delete the task from the error queue.  This causes it to
   disappear into the ether, since a command must be placed on the
   done queue to be finished off by the error handler.  This behaviour
   causes the HBA to hang on pending commands.

Fix 1. by moving the SAS_TASK_STATE_ABORTED check to an exit clause at
the top of the routine and calling ->scsi_done() unconditionally (it
is a nop if the timer has fired).  This keeps the task in the error
handling queue until the eh starts.

Fix 2. by making sure every task goes through task complete followed
by scsi_eh_finish_cmd().

Tested this by firing resets across a disk running a hammer test (now
it actually survives without hanging the system)

dde2020754 libata: eliminate the home grown dma padding in favour of
that provided by the block layer

ATA requires that all DMA transfers begin and end on word boundaries.
Because of this, a large amount of machinery grew up in ide to adjust
scatterlists on this basis.  However, as of 2.5, the block layer has a
dma_alignment variable which ensures both the beginning and length of a
DMA transfer are aligned on the dma_alignment boundary.  Although the
block layer does adjust the beginning of the transfer to ensure this
happens, it doesn't actually adjust the length, it merely makes sure
that space is allocated for transfers beyond the declared length.  The
upshot of this is that scatterlists may be padded to any size between
the actual length and the length adjusted to the dma_alignment safely
knowing that memory is allocated in this region.

Right at the moment, SCSI takes the default dma_aligment which is on a
512 byte boundary.  Note that this aligment only applies to transfers
coming in from user space.  However, since all kernel allocations are
automatically aligned on a minimum of 32 byte boundaries, it is safe to
adjust them in this manner as well.

tj: * Adjusting sg after padding is done in block layer.  Make libata
      set queue alignment correctly for ATAPI devices and drop broken
      sg mangling from ata_sg_setup().
    * Use request->raw_data_len for ATAPI transfer chunk size.
    * Killed qc->raw_nbytes.
    * Separated out killing qc->n_iter.

94aa5e5f62 [SCSI] aic7xxx_old, eata_pio, ips, libsas: don't zero out sense_buffer in queuecommand
LLDs don't need to zero out scsi_cmnd::sense_buffer in queuecommand
since scsi-ml does. This is a preparation of the future changes to
allocate the sense_buffer only when necessary.

Many LLDs zero out the sense_buffer before touching it on the error
case. This patch lets them alone for now because new APIs for them
would be added later on.

ff2aeb1eb6 libata: convert to chained sg
libata used private sg iterator to handle padding sg.  Now that sg can
be chained, padding can be handled using standard sg ops.  Convert to
chained sg.

* s/qc->__sg/qc->sg/

* s/qc->pad_sgent/qc->extra_sg[]/.  Because chaining consumes one sg
  entry.  There need to be two extra sg entries.  The renaming is also
  for future addition of other extra sg entries.

* Padding setup is moved into ata_sg_setup_extra() which is organized
  in a way that future addition of other extra sg entries is easy.

* qc->orig_n_elem is unused and removed.

* qc->n_elem now contains the number of sg entries that LLDs should
  map.  qc->mapped_n_elem is added to carry the original number of
  mapped sgs for unmapping.

* The last sg of the original sg list is used to chain to extra sg
  list.  The original last sg is pointed to by qc->last_sg and the
  content is stored in qc->saved_last_sg.  It's restored during
  ata_sg_clean().

* All sg walking code has been updated.  Unnecessary assertions and
  checks for conditions the core layer already guarantees are removed.

     1. If they're returned to outside applications, no-one knows what
        they mean.
     2. Eventually they'll clash with the ever expanding standard error
        codes.

The problem error code in question is ETASK.  I've replaced this by
ECOMM (communications error on send) a network error code that seems to
most closely relay what ETASK meant.

2d4b63e150 [SCSI] libsas: don't treat underrun as an error on SMP tasks
All SMP tasks sent through bsg generate messages like:

sas: smp_execute_task: task to dev 500605b000001450 response: 0x0 status 0x81

Three times (because the task gets retried).  Firstly, don't retry
either overrun or underrun (the data buffer isn't going to change size)
and secondly, just report the underrun but don't set an error for it.
This is necessary so bsg can report back the residual.

ba25f9dcc4 Use helpers to obtain task pid in printks
The task_struct->pid member is going to be deprecated, so start
using the helpers (task_pid_nr/task_pid_vnr/task_pid_nr_ns) in
the kernel.

The first thing to start with is the pid, printed to dmesg - in
this case we may safely use task_pid_nr(). Besides, printks produce
more (much more) than a half of all the explicit pid usage.

9af5c9c97d libata-link: introduce ata_link
Introduce ata_link.  It abstracts PHY and sits between ata_port and
ata_device.  This new level of abstraction is necessary to support
SATA Port Multiplier, which basically adds a bunch of links (PHYs) to
a ATA host port.  Fields related to command execution, spd_limit and
EH are per-link and thus moved to ata_link.

This patch only defines the host link.  Multiple link handling will be
added later.  Also, a lot of ap->link derefences are added but many of
them will be removed as each part is converted to deal directly with
ata_link instead of ata_port.

This patch introduces no behavior change.

92631fa4d0 [SCSI] libsas: Fix potential NULL dereference in sas_smp_get_phy_events()
In sas_smp_get_phy_events() we never test if the call to
alloc_smp_req(RPEL_REQ_SIZE) succeeds or fails. That means we run
the risk of dereferencing a NULL pointer if it does fail. Far
better to test if we got NULL back and in that case return -ENOMEM
just as we already do for the other memory allocation in that
function.

20c2df83d2 mm: Remove slab destructors from kmem_cache_create().
Slab destructors were no longer supported after Christoph's
c59def9f22 change. They've been
BUGs for both slab and slub, and slob never supported them
either.

This rips out support for the dtor pointer from kmem_cache_create()
completely and fixes up every single callsite in the kernel (there were
about 224, not including the slab allocator definitions themselves,
or the documentation references).

This one was noticed by Gilbert Wu of Adaptec:

The libata core actually does the DMA mapping for you, so there has to
be an exception in the device drivers that *don't* do dma mapping for
ATA commands.  However, since we've already done this, libsas must now
dma map any ATA commands that it wishes to issue ... and yes, this is a
horrible mess.

Additionally, the test in aic94xx for ATA protocols isn't quite right.

ATA devices need special handling for sas_task_abort.  If the ATA command
came from SCSI, then we merely need to tell SCSI to abort the scsi_cmnd.
However, internal commands require a bit more work--we need to fill the qc
with the appropriate error status and complete the command, and eventually
post_internal will issue the actual ABORT TASK.

The SATL should connect the scsi_cmnd to the sas_task (despite the presence
of libata) so that requests to abort scsi_cmnds headed to the ATA device
can be processed by the EH and aborted correctly.  The abort status should
still be propagated from sas -> ata -> scsi.

38691593cd [SCSI] sas_ata: Don't copy aic94xx's sactive to ata_port
Since the aic94xx sequencer assigns its own NCQ tags to ATA commands, it
no longer makes any sense to copy the sactive field in the STP response
to ata_port->sactive, as that will confuse libata.  Also, libata seems
to be capable of managing sactive on its own.

The attached patch gets rid of one of the causes of the BUG messages in
ata_qc_new, and seems to work without problems on an IBM x206m.

1c50dc83f9 [SCSI] sas_ata: ata_post_internal should abort the sas_task
This patch adds a new field, lldd_task, to ata_queued_cmd so that libata
users such as libsas can associate some data with a qc.  The particular
ambition with this patch is to associate a sas_task with a qc; that way,
if libata decides to timeout a command, we can come back (in
sas_ata_post_internal) and abort the sas task.

One question remains: Is it necessary to reset the phy on error, or will
the libata error handler take care of it?  (Assuming that one is written,
of course.)  This patch, as it is today, works well enough to clean
things up when an ATA device probe attempt fails halfway through the probe,
though I'm not sure this is always the right thing to do.

35a7f2f698 [SCSI] sas_ata: sas_ata_qc_issue should return AC_ERR_*
The sas_ata_qc_issue function was incorrectly written to return error
codes such as -ENOMEM.  Since libata OR's qc->err_mask with the
return value, It is necessary to make my code return one of the
AC_ERR_ codes instead.  For now, use AC_ERR_SYSTEM because an error
here means that the OS couldn't send the command to the controller.

If anybody has a suggestion for a better AC_ERR_ code to use, please
suggest it.

3eb7a51a3a [SCSI] sas_ata: Satisfy libata qc function locking requirements
ata_qc_complete and ata_sas_queuecmd require that the port lock be held
when they are called.  sas_ata doesn't do this, leading to BUG messages
about qc tags newly allocated qc tags already being in use.  This patch
fixes the locking, which should clean up the rest of those messages.

So far I've tested this against an IBM x206m with two SATA disks with no
BUG messages and no other signs of things going wrong, and the machine
finally passed the pounder stress test.

This is a respin of my earlier patch that migrates the ATA support code
into a separate file.  For now, the controversial linking bits have
been removed per James Bottomley's request for a patch that contains
only the migration diffs, which means that libsas continues to require
libata.  I intend to address that problem in a separate patch.

This patch is against the aic94xx-sas-2.6 git tree, and it has been
sanity tested on my x206m with Seagate SATA and SAS disks without
uncovering any new problems.

We actually had two problems: the one with the tag (which is fixed by
zeroing the tag before sending the taskfile to the sequencer) but the
other with the fact that we sent our first NCQ command to the device
before the sequencer had been informed of the NCQ tagging
capabilities.  I fixed the latter by moving the rphy_add() to the
correct point in the code after the NCQ capabilities are set up.

bdab4e8778 [SCSI] libsas: support NCQ for SATA disks
This patch adds SATAII NCQ support to libsas.  Both the use_ncq and the
dma_xfer flags in ata_task must be set for NCQ to work correctly on the
Adaptec SAS controller.  The rest of the patch adds ATA_FLAG_NCQ to
sata_port_info and sets up ap->scsi_host so that ata_setup_ncq doesn't
crash.  Please note that this patch is against the aic94xx-sas git tree,
not scsi-misc.  Thanks also to James Bottomley for providing an earlier
version of this patch from which to work.

I've tested this patch on a x206m with a ST380819AS SATA2 disk plugged
into the Adaptec SAS controller.  The drive came up with a queue depth
of 31, and I successfully ran an I/O flood test to coerce libata into
sending multiple commands simultaneously.  A kernel probe recorded the
maximum tag number that had been seen before and after the flood test;
before the test it was 2 and after it was 30, as I expected.

1acce1942a [SCSI] libsas: Add SATA support to STP piece for SATA on SAS expanders
This patch adds support for SATA over SAS expanders to the previous two
SATA support in libsas patches.

There were a couple of nasty non trivial things to sort out before this
one could be made to work.

Firstly, I'd like to thank Doug Gilbert for diagnosing a problem with
the LSI expanders where the REPORT_SATA_PHY command was returning the
D2H FIS in the wrong order (Although, here, I think I have to blame the
SAS standards which specifies the FIS "shall be returned in little
endian format" and later on "which means resp[24] shall be FIS type"
The latter, of course, implying big endian format).  Just to make sure,
I put a check for the D2H FIS type being in the wrong position and
reverse the FIS data if it is.

The second is a problem outlined in Annex G of the SAS standard (again,
a technical point with D2H FIS ... necessitating a phy reset on certain
conditions).

With the patch, I can now see my SATA-1 disk in a cascaded expander
configuration.

8314418629 Freezer: make kernel threads nonfreezable by default
Currently, the freezer treats all tasks as freezable, except for the kernel
threads that explicitly set the PF_NOFREEZE flag for themselves.  This
approach is problematic, since it requires every kernel thread to either
set PF_NOFREEZE explicitly, or call try_to_freeze(), even if it doesn't
care for the freezing of tasks at all.

It seems better to only require the kernel threads that want to or need to
be frozen to use some freezer-related code and to remove any
freezer-related code from the other (nonfreezable) kernel threads, which is
done in this patch.

The patch causes all kernel threads to be nonfreezable by default (ie.  to
have PF_NOFREEZE set by default) and introduces the set_freezable()
function that should be called by the freezable kernel threads in order to
unset PF_NOFREEZE.  It also makes all of the currently freezable kernel
threads call set_freezable(), so it shouldn't cause any (intentional)
change of behaviour to appear.  Additionally, it updates documentation to
describe the freezing of tasks more accurately.

91a6902958 sysfs: add parameter "struct bin_attribute *" in .read/.write methods for sysfs binary attributes
Well, first of all, I don't want to change so many files either.

What I do:
Adding a new parameter "struct bin_attribute *" in the
.read/.write methods for the sysfs binary attributes.

In fact, only the four lines change in fs/sysfs/bin.c and
include/linux/sysfs.h do the real work.
But I have to update all the files that use binary attributes
to make them compatible with the new .read and .write methods.
I'm not sure if I missed any. :(

Why I do this:
For a sysfs attribute, we can get a pointer pointing to the
struct attribute in the .show/.store method,
while we can't do this for the binary attributes.
I don't know why this is different, but this does make it not
so handy to use the binary attributes as the regular ones.
So I think this patch is reasonable. :)

Who benefits from it:
The patch that exposes ACPI tables in sysfs
requires such an improvement.
All the table binary attributes share the same .read method.
Parameter "struct bin_attribute *" is used to get
the table signature and instance number which are used to
distinguish different ACPI table binary attributes.

Without this parameter, we need to offer different .read methods
for different ACPI table binary attributes.
This is impossible as there are various ACPI tables on different
platforms, and we don't know what they are until they are loaded.

7b595756ec sysfs: kill unnecessary attribute->owner
sysfs is now completely out of driver/module lifetime game.  After
deletion, a sysfs node doesn't access anything outside sysfs proper,
so there's no reason to hold onto the attribute owners.  Note that
often the wrong modules were accounted for as owners leading to
accessing removed modules.

This patch kills now unnecessary attribute->owner.  Note that with
this change, userland holding a sysfs node does not prevent the
backing module from being unloaded.

6473d160b4 PCI: Cleanup the includes of <linux/pci.h>
I noticed that many source files include <linux/pci.h> while they do
not appear to need it. Here is an attempt to clean it all up.

In order to find all possibly affected files, I searched for all
files including <linux/pci.h> but without any other occurence of "pci"
or "PCI". I removed the include statement from all of these, then I
compiled an allmodconfig kernel on both i386 and x86_64 and fixed the
false positives manually.

My tests covered 66% of the affected files, so there could be false
positives remaining. Untested files are:

libsas: Don't BUG when connecting two expanders via wide port

When a device is connected to an expander, the discovery process goes through
sas_ex_discover_dev to figure out what's attached to the phy.  If it is the
case that the phy being discovered happens to be the second phy of a wide link
to an expander, that discover_dev function will incorrectly call
sas_ex_discover_expander, which creates another sas_port and tries to attach the
other sas_phys to the new port, thus triggering a BUG.  The correct thing to do is
to check the other ex_phys of the expander to see if there's a sas_port for this
sas_phy, and attach the sas_phy to the existing sas_port.

This is easily triggered if one enables the phys of a wide port between
expanders one by one.

This second version of the patch fixes a small regression in the case where
all the phys show up at once and we accidentally try to attach to a port
that hasn't been created yet.

ad689233be [SCSI] libsas: Handle SCSI commands that complete with failure codes
This patch moves the code that handles SAS failures out of the main EH
function and into a separate function.  It also detects commands that have
no sas_task (i.e. they completed, but with error data) and sends them into
scsi_error for processing.  This allows us to handle SCSI errors (and
enables auto-spinup as a side effect) instead of dropping them on the
floor and falling into an infinite loop.  It also requires the
implementation of a device reset function, which the SAS failure code has
been modified to employ for REQ_DEVICE_RESET.

6f63caae21 [SCSI] libsas: Clean up discovery failure handler code
sas_rphy_delete does two things: it removes the sas_rphy from the transport
layer and frees the sas_rphy.  This can be broken down into two functions,
sas_rphy_remove and sas_rphy_free; sas_rphy_remove is of interest to
sas_discover_root_expander because it calls functions that require
sas_rphy_add as a prerequisite and can fail (namely sas_discover_expander).
In that case, sas_discover_root_expander needs to be able to undo the effects
of sas_rphy_add yet leave the job of freeing the sas_rphy to the caller of
sas_discover_root_expander.

This patch also removes some unnecessary code from sas_discover_end_dev
to eliminate an unnecessary cycle of sas_notify_lldd_gone/found for SAS
devices, thus eliminating a sas_rphy_remove call (and fixing a race condition
where a SCSI target scan can come in between the gone and found call).
It also moves the sas_rphy_free calls into sas_discover_domain and
sas_ex_discover_end_dev to complement the sas_rphy_allocation via
sas_get_port_device.

This patch does not change the semantics of sas_rphy_delete.

3b6e9fafc4 [SCSI] libsas: Fix incorrect sas_port deformation in sas_form_port
Currently, sas_form_port checks the given asd_sas_phy's sas_phy to see if
there's already a port attached.  If so, the SAS addresses of the port and
the phy are compared to determine if we need to detach from the port
because the addresses don't match or if we can stop; the SAS address stored
in the sas_port reflects whatever device _was_ attached to the port/phy, and
the SAS address stored in the sas_port reflects whatever device we just
discovered.  As written, the code detaches from the port if the addresses
_do_ match, and prints an error if they do _not_ match.  I believe this to
be incorrect, as it seems more logical to keep the port if the addresses
match (i.e. the phy was reset but the device didn't change), and detach it
they do not (i.e. the device changed).

396819fba8 [SCSI] libsas: Delay issuing ABORT TASK TMF until the error handler
sas_task_abort() should simply abort the upper-level SCSI command and wait
until the error handler to send the actual ABORT TASK command.  By
deferring things to the EH we simplify the concurrency coordination and
eliminate some race conditions.  Note that sas_task_abort has a few hooks
to handle libsas internal commands properly too.

Also rename do_sas_task_abort to __sas_task_abort just in case we really
want to abort the task *right now* and we don't have a scsi_cmnd attached
to the command.  This is a hook for libata internal commands to abort.

port after all commands are aborted.

6d4dcd4dae [SCSI] libsas: Reset timer on taskless scsi_cmnds in sas_scsi_timed_out
Every so often, a scsi_cmnd will time out, and the libsas timeout handler
will discover that the scsi_cmnd does not have a sas_task attached to it.
This can happen in two cases: (1) the scsi_cmnd actually made it through
libsas to the HBA and is now going through scsi_done, or (2) the
scsi_cmnd has been held up (host lock, slab alloc, etc) and libsas has
not yet attached a sas_task.  In both cases, it is safe to ask SCSI for
more time to process the command via EH_RESET_TIMER; we cannot blindly
return EH_HANDLED because if (2) happens, we could end up calling
scsi_done while another CPU is heading towards sas_queuecommand, which
causes slab corruption when sas_task_done updates the freed scsi_cmnd.

On a system with many SAS targets, it appears possible that a scsi_cmnd
can time out without ever making it to the SAS LLDD or at the same time
that a completion is occurring.  In both of these cases, telling the
LLDD to abort the sas_task makes no sense because the LLDD won't know
about the sas_task; what we really want to do is to increase the timer.
Note that this involves creating another sas_task bit to indicate
whether or not the task has been sent to the LLDD; I could have
implemented this by slightly redefining SAS_TASK_STATE_PENDING, but
this way seems cleaner.

This second version amends the aic94xx portion to set the
TASK_AT_INITIATOR flag for all sas_tasks that were passed to
lldd_execute_task.

This patch implements a REQ_DEVICE_RESET handler for the aic94xx
driver.  Like the earlier REQ_TASK_ABORT patch, this patch defers the
device reset to the Scsi_Host's workqueue, which has the added benefit
of ensuring that the device reset does not happen at the same time
that the abort tmfs are being processed.  After the phy reset, the
busted drive should go away and be re-detected later, which is indeed
what I've seen on both a x260 and a x206m.
