When calling set_{on,off}line of a ccwgroup device driver we hold
the module reference of the owner. This is pretty useless - we don't
want to prevent module unloading but driver unbinding. Use the
driver core's device_lock instead.

Subchannel looping function for_each_subchannel_staged() allocates a
subchannel-ID-bitmap to efficiently iterate over the list of known
and unknown subchannels. Since this function is also used to iterate
over known-subchannels only, optimize that case by not requiring the
ID-bitmap allocation and falling back to simple bus_for_each_dev()
looping.

The CIO layer scans for newly available I/O devices by performing a scan
of available subchannels using the Store Subchannel (STSCH) instruction.
Performing too many STSCH instructions in a tight loop can cause high
Hypervisor overhead which can negatively impact the performance of the
virtual machine as a whole.
A subchannel scan is triggered for example during a hardware event that
indicates that a channel path has become available. It is also triggered
by the DASD device driver for each device that is set online.
This patch reduces the number of STSCH instructions being performed by
delaying the start of the actual subchannel scan by 1 second. Multiple
scan requests that are scheduled during this time will be merged into a
single scan loop.
The trade-off consists of a short delay that is introduced between
the time that the event is processed and a newly available device
becoming usable. This delay should be acceptable since it only
affects devices that have not been in use before.

The CIO layer scans for newly available I/O devices by performing a scan
of available subchannels using the Store Subchannel (STSCH) instruction.
This processing can take a significant amount of time during which no
other task can run on the same CPU (unless CONFIG_PREEMPT has been
enabled). As a result, scheduling latencies for other tasks are
increased noticeably, especially on a single-CPU system.
Fix this problem by explicitly allowing other tasks to be scheduled
each time a subchannel has been processed.

The CIO layer processes hardware events that indicate that a channel
path has become available by performing a scan of available subchannels
using the Store Subchannel (STSCH) instruction. Performing too many
STSCH instructions in a tight loop can cause high Hypervisor overhead
which can negatively impact the performance of the virtual machine as
a whole.
This patch reduces the number of STSCH instructions performed while
processing a resource accessibility event and while varying a CHPID
online.
In both cases, Linux first performs a STSCH instruction on each unused
subchannel to see if the subchannel has become available. If the STSCH
instruction indicates that the subchannel is available, a full
evaluation of this subchannel is scheduled. Since the full evaluation
includes performing a STSCH instruction, the initial STSCH is
unnecessary and can be removed.

Stop hiding scm_block's dependency to the eadm subchannel driver
(by using functions provided by the eadm subchannel instead of
wrappers provided by the scm bus).
This will help userspace recognizing module dependencies (e.g. for
building a ramdisk). As a side effect we can get rid of some code
reimplementing refcounting between those modules.

find_first_bit_left() and friends have nothing to do with the normal
LSB0 bit numbering for big endian machines used in Linux (least
significant bit has bit number 0).
Instead they use MSB0 bit numbering, where the most signficant bit has
bit number 0. So rename find_first_bit_left() and friends to
find_first_bit_inv(), to avoid any confusion.
Also provide inv versions of set_bit, clear_bit and test_bit.
This also removes the confusing use of e.g. set_bit() in airq.c which
uses a "be_to_le" bit number conversion, which could imply that instead
set_bit_le() could be used. But that is entirely wrong since the _le
bitops variant uses yet another bit numbering scheme.

get_inbound_buffer_frontier() makes use of the return value of atomic_sub()
which shouldn't work, since atomic_sub() is supposed to return void.
This only works on s390 because atomic_sub() gets mapped to atomic_sub_return()
with a define without changing it's return value to void.
So use atomic_sub_return() instead of atomic_sub() in qeth code before fixing
atomic ops.

The result of the store-clock-fast (STCKF) instruction is a bit fuzzy.
It can happen that the value stored on one CPU is smaller than the value
stored on another CPU, although the order of the stores is the other
way around. This can cause deltas of get_tod_clock() values to become
negative when they should not be.
We need to be more careful with store-clock-fast, this patch partially
reverts git commit e4b7b4238e666682555461fa52eecd74652f36bb "time:
always use stckf instead of stck if available". The get_tod_clock()
function now uses the store-clock-extended (STCKE) instruction.
get_tod_clock_fast() can be used if the fuzziness of store-clock-fast
is acceptable e.g. for wait loops local to a CPU.

With the introduction of PCI it became apparent that s390 should
convert to generic hardirqs as too many drivers do not have the
correct dependency for GENERIC_HARDIRQS. On the architecture
level s390 does not have irq lines. It has external interrupts,
I/O interrupts and adapter interrupts. This patch hard-codes all
external interrupts as irq #1, all I/O interrupts as irq #2 and
all adapter interrupts as irq #3. The additional information from
the lowcore associated with the interrupt is stored in the
pt_regs of the interrupt frame, where the interrupt handler can
pick it up. For PCI/MSI interrupts the adapter interrupt handler
scans the relevant bit fields and calls generic_handle_irq with
the virtual irq number for the MSI interrupt.

There are three users of adapter interrupts: AP, QDIO and PCI. Each
registers a single adapter interrupt with independent ISCs. Define
a "struct airq" with the interrupt handler, a pointer and a mask for
the local summary indicator and the ISC for the adapter interrupt
source. Convert the indicator array with its fixed number of adapter
interrupt sources per ISE to an array of hlists. This removes the
limitation to 32 adapter interrupts per ISC and allows for arbitrary
memory locations for the local summary indicator.

Copy the interrupt parameters from the lowcore to the pt_regs structure
in entry[64].S and reduce the arguments of the low level interrupt handler
to the pt_regs pointer only. In addition move the test-pending-interrupt
loop from do_IRQ to entry[64].S to make sure that interrupt information
is always delivered via pt_regs.

When we fetch an interrupt on the CCW console using tsch (via
ccw_device_wait_idle formerly known as wait_cons_dev) we increment
the irq count for the affected interruption class but it is not
accounted as an IO interrupt.
This is broken since commit b603d258a4
"s390: remove superfluous tpi from wait_cons_dev"
Fix it so that the sum of the individual interrupts per class matches
the number of IO interrupts again.

if it's worth to further investigate a subchannel. Move the allocation

Via ccw_device_init_count we keep track of how many devices are in
asynchronous device recognition/initialization. For early devices this
variable was not only used prior to its initialization but used
incorrectly (incremented but never decremented). Fix this by using static
initialization for this variable (and friends), make them visible to
device.c only, and decrement the counter after recognition of early
devices is finished.

Split the subchannel registration in device_initialize and device_add
and move the initialization part inside the allocation function. With
this change we can use refcounting during the complete lifespan of a
subchannel which is important for devices where we do the actually
registration at a later time.

force_console is used to wake up the CCW based console device to
print a panic message in case something goes wrong in a suspend
or resume cycle. Stop using the static console_subchannel and add
a parameter to this function to specify which ccw device we have
to wake up.

wait_cons_dev is used to busy wait for an interrupt on the console
ccw device. Stop using the static console_subchannel and add a
parameter to this function to specify on which ccw device/subchannel
we have to do the polling.
While at it rename the function to ccw_device_wait_idle and
move it to device.c

When an attempt to query or modify the grouping state of a channel path
fails due to a timeout, we cannot be sure about its state. To get back to
a defined state, disband the whole path group and try again while excluding
the offending path.

Add new attribute "vpm" to the subchannel sysfs directory of I/O
subchannels. This attribute contains a path mask indicating which
channel paths were successfully verified to be usable for I/O.

Omit known to be broken paths (those set in path_noirq_mask) for the
sense/set PGID and nop IO commands.
Note: path_noirq_mask will be reset in ccw_device_verify_start (the
paths could be healthy again). However if we restart a path
verification via verify_start this mask will not be reset (there is
no need to let the wait for an interrupt time out again - plus we
do not want to loop once we deal with the paths in unknown path group
state).

Do not abort path verification when waiting for an interrupt timed out.
Use path_noirq_mask to keep track of the paths used for this (also
maintain a path_notoper_mask for debugging purposes). If the timeout
happend to be during an operation where we query or alter the state of
path groups set the pgid_unknown flag.
With this change we allow usage of devices which have such ill-behaved
paths (if at least one path is operational).

introduced a new SEI notification type as part of pci support.
The way SEI was called with nt2 and nt0 consecutive broke the nt0
stuff used for channel subsystem notifications.
The reason why this was broken with the mentioned patch is that you
cannot selectively disable type 0 notifications (so even when asked
for type 2 only, type 0 could be presented).
The way to do it is to tell SEI which types of notification you can
process and -this is the important part- look at the SEI result which
notification type you actually received.

Now that irq sum accounting for /proc/stat's "intr" line works again we
have the oddity that the sum field (first field) contains only the sum
of the second (external irqs) and third field (I/O interrupts).
The reason for that is that these two fields are already sums of all other
fields. So if we would sum up everything we would count every interrupt
twice.
This is broken since the split interrupt accounting was merged two years
ago: 052ff461c8 "[S390] irq: have detailed
statistics for interrupt types".
To fix this remove the split interrupt fields from /proc/stat's "intr"
line again and only have them in /proc/interrupts.
This restores the old behaviour, seems to be the only sane fix and mimics
a behaviour from other architectures where /proc/interrupts also contains
more than /proc/stat's "intr" line does.

For more than two years, since f2c66cd8ee
"/proc/stat: scalability of irq num per cpu" the output of /proc/stat is
broken.
The first field in the "intr" line should contain the sum of all interrupts,
however since the above mentioned change it is always zero.
The reason for that is that a per cpu irq sum variable had been introduced
which got incremented when calling kstat_incr_irqs_this_cpu(). However
on s390 we directly incremented only the per cpu per irq counter by accessing
the array element via kstat_cpu(smp_processor_id()).irqs[...].
So fix this and use the kstat_incr_irqs_this_cpu() wrapper which increments
both: the per cpu per irq counter and the per cpu irq sum counter.

For each detected qdio device a line like to following is printed:
qdio: 0.0.4102 OSA on SC 1045 using AI:1 QEBSM:0 PCI:1 TDD:1 SIGA: W AP
The PCI flag is misleading as this stands for "program controlled interrupt".
Rename it to PRI "program requested interrupt" which is more accurate and
does not interfere with another popular piece of technology.
Leave the pci string in the code since changing that would result in a huge
patch.

The check to whom a device is reserved is done by checking the path
state of the affected channel paths. If it turns out that one path is
flagged as reserved by someone else the whole device is marked as such.
However the meaning of the RESVD_ELSE bit is that the addressed device
is reserved to a different pathgroup (and not reserved to a different
LPAR). If we do this test on a path which is currently not a member of
the pathgroup we could erroneously mark the device as reserved to
someone else.
To fix this collect the reserved state for all potential members of the
pathgroup and only mark the device as reserved if all of those potential
members have the RESVD_ELSE bit set.

* the subchannel is not provided
* there are no further subchannels in this subchannel set
With this patch we abort the store subchannel loop after cc=3 (or an
exception) and clear the subsequent bits in the subchannel id set.

Calling debug_event the s390 debug feature only logs up to buf_size
bytes of the debug view. If debug_event is called with more bytes
than buf_size the additional data is ignored and not logged in the
debug view.
Use multiple calls to debug_event if the length exceeds buf_size.

Make sure that the cdev pointer for IO subchannels is set to NULL when
we deregister the device (and release its last reference). This will
fix a bug were another process operates on an already freed ccw device.

If the subchannel event function is called from IRQ context and we
observe that the subchannel in question is gone we flag the attached
device as not operational and schedule the event function to be called
again from process context where the subchannel gets deregistered.
However if the subchannel reappeared at the time the event function
gets called from process context we would do nothing and leave the
device in not operational state. Recognize this case in sch_get_action
and trigger reexamination of the subchannel/device.

Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.
Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

Add a new interface for drivers to create a group device. Via the old
interface ccwgroup_create_from_string we would create a virtual device
in a way that only the caller of this function would match and bind to.
Via the new ccwgroup_create_dev we stop playing games with the driver
core and directly set the driver of the new group device. For drivers
which have todo additional setup steps (like setting driver_data)
provide a new setup driver callback.

wait_cons_dev waits for a particular subchannel to complete an I/O.
It is not necessary to use tpi to get the subchannel id as it is
already known. This avoids changes to the interrupt subclass mask
and allows to remove the lock & unlock of the subchannel lock.

Currently the following mechanisms are available to move active
Linux on System z instances between machines:
* z/VM 6.2 SSI (Single System Image)
* Suspend/resume
For moving Linux instances in this patch the term LGR (Linux Guest
Relocation) is used. Because such an operation is critical, it
should be detectable from Linux. With this patch for both, a live
system and a kernel dump, the information about LGRs is accessible.
To identify a guest, stsi and stfle data is used. A new function
lgr_info_log() compares the current data (lgr_info_cur) with the
last recorded one (lgr_info_last). In case the two data sets differ,
lgr_info_cur is logged to the "lgr" s390dbf.
The following trigger points call lgr_info_log():
* panic
* die
* kdump
* LGR timer
* PSW restart
* QDIO recovery
* resume
This patch also changes the s390dbf hex_ascii view. Now only printable ASCII
characters are shown.

Whenever the cpu loads an enabled wait PSW it will appear as idle to the
underlying host system. The code in default_idle calls vtime_stop_cpu
which does the necessary voodoo to get the cpu time accounting right.
The udelay code just loads an enabled wait PSW. To correct this rework
the vtime_stop_cpu/vtime_start_cpu logic and move the difficult parts
to entry[64].S, vtime_stop_cpu can now be called from anywhere and
vtime_start_cpu is gone. The correction of the cpu time during wakeup
from an enabled wait PSW is done with a critical section in entry[64].S.
As vtime_start_cpu is gone, s390_idle_check can be removed as well.

introduced a regression in regard to the zfcp data router.
Revoke the incorrect simplification of the function call arguments
for the qdio handler to make the zfcp hardware data router working
again.
This is applicable to 3.2+ kernels.

For historical reasons, we allow module_param(bool) to take an int (or
an unsigned int). That's going away.
A few drivers really want an int: they set it to -1 and a parameter
will set it to 0 or 1. This sucks: reading them from sysfs will give
'Y' for both -1 and 1, but if we change it to an int, then the users
might be broken (if they did "param" instead of "param=1").
Use a new 'bint' parser for them.
(ntfs has a different problem: it needs an int for debug_msgs because
it's also exposed via sysctl.)

The queue_start_poll function pointer field in struct qdio_initialize
had to change its type and become a vector of function pointers to
support asynchronous delivery of storage blocks so rename the field to
make the type change explicit and ensure no other user of qdio tries
to use the field the old way. During setting up the qdio queues, only
dereference vector elements if the vector is actually allocated.

If a driver wants to do command mode IO while CIO is doing
online path verification we ignore this request and provide
a fake irb when we are done and the driver can do IO again.
For transport mode IO we have no such mechanism, giving the
driver no other chance then to retry the action until we are
done. This is not very reliable.
Provide a fake irb for transport mode IO as well.

When a driver requests to do IO, we will adjust the mask of
paths to be used to exclude varied offline paths.
Drivers trying to do IO solely on paths which are online but some
way defective may lack the information to do proper error handling.
There is no reason to allow the usage of known to be broken paths.
Thus restrict the paths a ccw driver can use for IO to a subset of
the paths cio found usable (this also excludes offline paths).

Using the generic css_schedule_eval to evaluate subchannels
while resuming from hibernation is very slow when used with
many devices. Provide a new evaluation trigger which exploits
css_sched_sch_todo and use this in the resume callback for
ccw devices.

A kernel panic may occur during sending or receiving network packets
on a machine without adapter interrupts since commit d36deae.
The bug is triggered by writing to the shared indicator address which
is set to 0 if the machine doesn't have adapter interrupts.
Make the reading and setting of the shared indicator dependent on the
adapter interrupt feature and while at it move the code to the
file containing the adapter interrupt related code.

Running under z/VM with QIOASSIST enabled, qdio queues could stall if EQBS
did not extract all SBAL states. Add an instant retry for EQBS and, if the
retry fails, set up a timer to ensure outstanding SBALs are processed later.
While at it, optimize qdio_do_eqbs and qdio_do_sqbs to eliminate 3 jumps on
the hot path.

Add a timestamp per queue and update the timestamp when the queue is
scanned. Add the queue timestamps and the timestamp of the last
adapter interrupt to the debugfs output. The timestamps are useful
for debugging stall conditions.

Print a message in case we do not receive an IRQ in time (for internal
I/O). Also print the ID of the last used channel path, since it is
possible that not the device itself but this specific path might have

The SIGA-W may return with the busy bit set which means the device was
blocked. The busy loop which retries the SIGA-W for 100us may not be
long enough when running under a heavily loaded hypervisor.
Extend the retry mechanism by adding a longer second stage which retries
the SIGA-W for up to 10s. In difference to the first retry loop the second
stage is using mdelay to stop the cpu between the retries and thereby
avoid additional preassure in on the hypervisor.
If the second stage retry is successfull a device reset is avoided.

Fix the race by resetting the DSCI before scheduling the inbound tasklet

scenario in step 6.

The qdio SBAL entry flag is made-up of four different values that are
independent of one another. Some of the bits are reserved by the
hardware and should not be changed by qdio. Currently all four values
are overwritten since the SBAL entry flag is defined as an u32.
Split the SBAL entry flag into four u8's as defined by the hardware
and don't touch the reserved bits.

In ccw_device_set_online we basically start path verification and
wait for the device to reach a final state. If it turns out that the
device has no useable path we schedule the deregistration of the
device (which is still in an non-final state) and wake up the waiting
process. The deregistration process will set a final state, but if
the wake up happens to be prior to this, the device will hang forever
in ccw_device_set_online.
To fix this just set the final NOT_OPER state prior to the scheduled
deregistration of the device.
