When the driver reads state values from the hw it might happen that
different values are read in subsequent reads and this can cause problems,

There is an error with the medium access timeout feature of the sd driver. The
sdkp->medium_access_timed_out value is reset to zero in sd_done() in the wrong
place. Currently it is reset to zero only when a command returns sense data.
This can result in cases where the medium access check falsely triggers from
timed out commands which are hours or days apart.
For example, an I/O command times out and is aborted. It then retries and
succeeds. But with no sense data generated and returned, the
medium_access_timed_out value is not reset. If no sd command returns sense
data, then the next command to time out (however far in time from the first
failure) will trigger the medium access timeout and put the device offline.
The resetting of sdkp->medium_access_timed_out should occur before the check
for sense data.
To reproduce using scsi_debug, use SCSI_DEBUG_OPT_TIMEOUT or
SCSI_DEBUG_OPT_MAC_TIMEOUT to force an I/O command to timeout. Then, remove
the opt value so the I/O will succeed on retry. Perform more I/O as desired.
Finally, repeat the process to make a new I/O command time out. Without the
patch, the device will be marked offline even though many I/O commands have
succeeded between the 2 instances of timed out commands.

Instead of letting the ULD play games with the prep_fn move back to
the model of a central prep_fn with a callback to the ULD. This
already cleans up and shortens the code by itself, and will be required
to properly support blk-mq in the SCSI midlayer.

By folding scsi_end_request into its only caller we can significantly clean
up the completion logic. We can use simple goto labels now to only have
a single place to finish or requeue command there instead of the previous
convoluted logic.

Instead of trying to guess when we have a BIDI buffer in scsi_release_buffers
add a function to explicitly free the BIDI ressoures in the one place that
handles them. This avoids needing a special __scsi_release_buffers for the
case where we already have freed the request as well.

On suspend, _scsih_suspend calls mpt2sas_base_free_resources, which
in turn calls pci_disable_device if the device is enabled prior to
suspending. However, _scsih_suspend also calls pci_disable_device
itself.
Thus, in the event that the device is enabled prior to suspending,
pci_disable_device will be called twice. This patch removes the
duplicate call to pci_disable_device in _scsi_suspend as it is both
unnecessary and results in a kernel oops.

It is possible by passing a netlink socket to a more privileged
executable and then to fool that executable into writing to the socket
data that happens to be valid netlink message to do something that
privileged executable did not intend to do.
To keep this from happening replace bare capable and ns_capable calls
with netlink_capable, netlink_net_calls and netlink_ns_capable calls.
Which act the same as the previous calls except they verify that the
opener of the socket had the desired permissions as well.

This patch fixes a corner case in the previous USB Deadlock fix patch (12023e7
[SCSI] Fix USB deadlock caused by SCSI error handling).
The scenario is abort command, set flag, abort completes, send TUR, TUR
doesn't return, so we now try to abort the TUR, but scsi_abort_eh_cmnd()
will skip the abort because the flag is set and move straight to reset.

USB requires that every command be aborted first before we escalate to reset.
In particular, USB will deadlock if we try to reset first before aborting the
command.
Unfortunately, the flag we use to tell if a command has already been aborted:
SCSI_EH_ABORT_SCHEDULED is not cleared properly leading to cases where we can
requeue a command with the flag set and proceed immediately to reset if it
fails (thus causing USB to deadlock).
Fix by clearing the SCSI_EH_ABORT_SCHEDULED flag if it has been set. Which
means this will be the second time scsi_abort_command() has been called for
the same command. IE the first abort went out, did its thing, but now the
same command has timed out again.
So this flag gets cleared, and scsi_abort_command() returns FAILED, and _no_
asynchronous abort is being scheduled. scsi_times_out() will then proceed to
call scsi_eh_scmd_add(). But as we've cleared the SCSI_EH_ABORT_SCHEDULED
flag the SCSI_EH_CANCEL_CMD flag will continue to be set, and the command will
be aborted with the main SCSI EH routine.

We're seeing a case where the contents of scmd->result isn't being reset after
a SCSI command encounters an error, is resubmitted, times out and then gets
handled. The error handler acts on the stale result of the previous error
instead of the timeout. Fix this by properly zeroing the scmd->status before
the command is resubmitted.

We unconditionally execute scsi_eh_get_sense() to make sure all failed
commands that should have sense attached, do. However, the routine forgets
that some commands, because of the way they fail, will not have any sense code
... we should not bother them with a REQUEST_SENSE command. Fix this by
testing to see if we actually got a CHECK_CONDITION return and skip asking for
sense if we don't.

Initialize local variable trans_support before it is used rather
than after. It is supposed to contain the value of a register on the
controller containing bits that describe which transport modes the
controller supports (e.g. "performant", "ioaccel1", "ioaccel2"). A
NULL pointer dereference will almost certainly occur if trans_support
is not initialized at the right point. If for example the uninitialized
trans_support value does not have the bit set for ioaccel2 support when it
should be, then ioaccel2_alloc_cmds_and_bft() will not get called as it
should be and the h->ioaccel2_blockFetchTable array will remain NULL
instead of being allocated. Too late, trans_support finally gets
initialized with the correct value with ioaccel2 mode bit set,
which later causes calc_bucket_map() to be called to fill in
h->ioaccel2_blockFetchTable[]. However h->ioaccel2_blockFetchTable
is NULL because it didn't get allocated because earlier trans_support
wasn't initialized at the right point.

This was used in the olden days, back when onions were proper
yellow. Basically it mapped to the current buffer to be
transferred. With highmem being added more than a decade ago,
most drivers map pages out of a bio, and rq->buffer isn't
pointing at anything valid.
Convert old style drivers to just use bio_data().
For the discard payload use case, just reference the page
in the bio.

We don't like this, but things have diverged with the blk-mq fixes
in 3.15-rc1. So merge it in.
This patch fixes I/O errors with the sym53c8xx_2 driver when the disk
returns QUEUE FULL status.
When the controller encounters an error (including QUEUE FULL or BUSY
status), it aborts all not yet submitted requests in the function
sym_dequeue_from_squeue.
This function aborts them with DID_SOFT_ERROR.
If the disk has full tag queue, the request that caused the overflow is
aborted with QUEUE FULL status (and the scsi midlayer properly retries
it until it is accepted by the disk), but the sym53c8xx_2 driver aborts
the following requests with DID_SOFT_ERROR --- for them, the midlayer
does just a few retries and then signals the error up to sd.
The result is that disk returning QUEUE FULL causes request failures.
The error was reproduced on 53c895 with COMPAQ BD03685A24 disk
(rebranded ST336607LC) with command queue 48 or 64 tags. The disk has
64 tags, but under some access patterns it return QUEUE FULL when there
are less than 64 pending tags. The SCSI specification allows returning
QUEUE FULL anytime and it is up to the host to retry.

Several spots in the kernel perform a sequence like:
skb_queue_tail(&sk->s_receive_queue, skb);
sk->sk_data_ready(sk, skb->len);
But at the moment we place the SKB onto the socket receive queue it
can be consumed and freed up. So this skb->len access is potentially
to freed up memory.
Furthermore, the skb->len can be modified by the consumer so it is
possible that the value isn't accurate.
And finally, no actual implementation of this callback actually uses
the length argument. And since nobody actually cared about it's
value, lots of call sites pass arbitrary values in such as '0' and
even '1'.
So just remove the length argument from the callback, that way there
is no confusion whatsoever and all of these use-after-free cases get
fixed as a side effect.
Based upon a patch by Eric Dumazet and his suggestion to audit this
issue tree-wide.

async_schedule() sd resume work to allow disks and other devices to
resume in parallel.
This moves the entirety of scsi_device resume to an async context to
ensure that scsi_device_resume() remains ordered with respect to the
completion of the start/stop command. For the duration of the resume,
new command submissions (that do not originate from the scsi-core) will
be deferred (BLKPREP_DEFER).
It adds a new ASYNC_DOMAIN_EXCLUSIVE(scsi_sd_pm_domain) as a container
of these operations. Like scsi_sd_probe_domain it is flushed at
sd_remove() time to ensure async ops do not continue past the
end-of-life of the sdev. The implementation explicitly refrains from
reusing scsi_sd_probe_domain directly for this purpose as it is flushed
at the end of dpm_resume(), potentially defeating some of the benefit.
Given sdevs are quiesced it is permissible for these resume operations
to bleed past the async_synchronize_full() calls made by the driver
core.
We defer the resolution of which pm callback to call until
scsi_dev_type_{suspend|resume} time and guarantee that the callback
parameter is never NULL. With this in place the type of resume
operation is encoded in the async function identifier.
There is a concern that async resume could trigger PSU overload. In the
enterprise, storage enclosures enforce staggered spin-up regardless of
what the kernel does making async scanning safe by default. Outside of
that context a user can disable asynchronous scanning via a kernel
command line or CONFIG_SCSI_SCAN_ASYNC. Honor that setting when
deciding whether to do resume asynchronously.

"A small collection of fixes that should go in before -rc1. The pull

- A two patch fix for a regression with block enabled tagging caused
by a commit in the initial pull request. One patch is from Martin
and ensures that SCSI doesn't truncate 64-bit block flags, the
other one is from me and prevents us from double using struct
request queuelist for both completion and busy tags. This caused
anything from a boot crash for some, to crashes under load.
- A blk-mq fix for a potential soft stall when hot unplugging CPUs
with busy IO.

In order to support local WRITE_INSERT + READ_STRIP operations for
non PI enabled fabrics, the fabric driver needs to be able signal
what protection offload operations are supported.
This is done at session initialization time so the modes can be
signaled by individual se_wwn + se_portal_group endpoints, as well
as optionally across different transports on the same endpoint.
For iser-target, set TARGET_PROT_ALL if the underlying ib_device
has already signaled PI offload support, and allow this to be
exposed via a new iscsit_transport->iscsit_get_sup_prot_ops()
callback.
For loopback, set TARGET_PROT_ALL to signal SCSI initiator mode
operation.
For all other drivers, set TARGET_PROT_NORMAL to disable fabric
level PI.

Now that TASK_ABORTED status is not generated for all cases by
TMR ABORT_TASK + LUN_RESET, a new TFO->abort_task() caller is
necessary in order to give fabric drivers a chance to unmap
hardware / software resources before the se_cmd descriptor is
released via the normal TFO->release_cmd() codepath.
This patch adds TFO->aborted_task() in core_tmr_abort_task()
in place of the original transport_send_task_abort(), and
also updates all fabric drivers to implement this caller.
The fabric drivers that include changes to perform cleanup
via ->aborted_task() are:
- iscsi-target
- iser-target
- srpt
- tcm_qla2xxx
The fabric drivers that currently set ->aborted_task() to
NOPs are:
- loopback
- tcm_fc
- usb-gadget
- sbp-target
- vhost-scsi
For the latter five, there appears to be no additional cleanup
required before invoking TFO->release_cmd() to release the
se_cmd descriptor.

Messages about discovered disk properties are only printed once unless
they are found to have changed. Errors encountered during mode sense,
however, are printed every time we revalidate.
Quiesce mode sense errors so they are only printed during the first
scan.

This allows drivers to specify the size of their per-command private
data in the host template and then get extra memory allocated for
each command instead of needing another allocation in ->queuecommand.
With the current SCSI code that already does multiple allocations for
each command this probably doesn't make a big performance impact, but
it allows to clean up the drivers, and prepare them for using the
blk-mq infrastructure where the common allocation will make a difference.

We don't use the passed in scsi command for anything, so just add a adapter-
wide internal status to go along with the internal scb that is used unter
int_mtx to pass back the return value and get rid of all the complexities
and abuse of the scsi_cmnd structure.
This gets rid of the only user of scsi_allocate_command/scsi_free_command,
which can now be removed.

EVPD page 0x83 is used to uniquely identify the device.
So instead of having each and every program issue a separate
SG_IO call to retrieve this information it does make far more
sense to display it in sysfs.
Some older devices (most notably tapes) will only report reliable
information in page 0x80 (Unit Serial Number). So export this
in the sysfs attribute 'vpd_pg80'.

This change is about the following:
(1) If the number of targets is 16+ then default ring_pages to 32.
(2) Change default queue depth (per device) to 254.
(3) Implement change_queue_depth function so that queue_depth per device can
be changed at run time. Honors the request only if coming from sysfs.
(4) Clean up the info returned by modinfo.

This change ensures that pvscsi_abort() function returns SUCCESS
only when the command in question was actually completed, otherwise
returns FAILURE. The code before change, was causing a bug where
driver tries to complete a command to the mid-layer while the mid-layer
has already requested the driver to abort that command, in response
to which the driver has responded with SUCCESS causing mid-layer
to free the command struct.

As result deprecation of MSI-X/MSI enablement functions

If, when the ipr driver loads, the adapter is in an EEH error state,
it will currently oops and not be able to recover, as it attempts
to access memory that has not yet been allocated. We've seen this
occur in some kexec scenarios. The following patch fixes the oops
and also allows the driver to recover from these probe time EEH errors.

This adds a module parameter to enable clustering.
Without enabling clustering support, the transfer length for read and
write scsi commands is limited upto 8MB when page size is 4KB and
sg_tablesize is 2048 (= SCSI_MAX_SG_CHAIN_SEGMENTS). I would like to
test commands with more than that transfer length.

Currently, clustering support for scsi_debug is disabled. This is
because there are for_each_sg() loops which assume that each sg list
element is consisted with a single page. But enabling clustering
support, each sg list element for scsi commands can be consisted with
multiple pages.
This replaces these for_each_sg() loops with sg mapping iterator which
is capable of handling each sg list element is consisted with multiple
pages.

When resp_xdwriteread() can't allocate temporary buffer, it returns -1.
But the return value is used as scsi status code and -1 is not
interpreted as correct code.
target_core_mod has similar xdwriteread emulation code. So this mimics
what target_core_mod does for xdwriteread when running out of memory.

Reading partially unwritten sectors generates a false positive logical
block reference tag check failure when DIF is enabled.
This bug is caused by missing ei_lba increment in loop of dif_verify()
when unwritten sector is skipped.

The only libata user that actually cares about the result from a suspend
operation is libsas. However, it only cares about whether queuing a new
operation collides with an in-flight one. All libsas does with the
error is retry, but we can just let libata wait for the previous
operation before continuing.

1/ Unifying all ata port pm operations on an ata_port_pm_ prefix
2/ Marking all ata port pm helper routines as returning void, only
ata_port_pm_ entry points need to fake a 0 return value.
3/ Killing ata_port_{suspend|resume}_common() in favor of calling

4/ Killing the wrappers that just do a to_ata_port() conversion
5/ Clearly marking the entry points that do async operations with an
_async suffix.

The bnx2/bnx2x rings are made up of linked pages. However there is an
upper limit on the page size as some the page size settings are 16-bit
in the hardware/firmware interface. In the current code, some parts
use BNX2_PAGE_SIZE which has a 16K upper limit and some parts use
PAGE_SIZE. On archs with >= 64K PAGE_SIZE, it generates some compile
warnings. Define a new CNIC_PAGE_SZIE which has an upper limit of
16K and use it consistently in all relevant parts.

iSCSI needs to be at least aware that a task involves protection
information. In case it does, after the transaction completed libiscsi
will ask the transport to check the protection status of the
transaction.
Unlike transport errors, DIF errors should not prevent successful
completion of the transaction from the transport point of view, but
should be escelated to scsi mid-layer when constructing the scsi
result and sense data.
check_protection routine will return the ascq corresponding to the DIF
error that occured (or 0 if no error happened).

SCSI devices may only be removed by calling scsi_remove_device().
That function must invoke blk_cleanup_queue() before the final put
of sdev->sdev_gendev. Since blk_cleanup_queue() waits for the
block queue to drain and then tears it down, scsi_request_fn cannot
be active anymore after blk_cleanup_queue() has returned and hence
the get_device()/put_device() pair in scsi_request_fn is unnecessary.

Do not expose drives that are undergoing a format immediately
to the OS, instead wait until they are ready before bringing
them online. This is so that logical drives created with
"rapid parity initialization" do not get immediately kicked
off the system for being unresponsive.

Evidently some wacky USB-ATA bridges don't recognize the SYNCHRONIZE
CACHE command, as shown in this email thread:

The fact that we can't tell them to drain their caches shouldn't
prevent the system from going into suspend. Therefore sd_sync_cache()
shouldn't return an error if the device replies with an Invalid
Command ASC.

If a phy device is removed, the device can get error of I/O and HBA maybe
receieve IO_OPEN_CNX_ERROR_IT_NEXUS_LOSS of event which causes pm8001_work_fn
to reset the phy device but in pm8001_task_exec we don't assign a value for
the device field of the ccb and in other case a ccb used have device field
set, when ccb is freed the field device of the ccb don't be set NULL.So there
is possibility of getting another device reset in function mpi_ssp_completion.

In function pm8001_dev_gone_notify we consume a tag from bitmap resource and
later don't free that tag resource, so the tag resource will never be
freed. In the function we don't need to use any tag to issue command for
unregistering the device .

Host lock is added back around queuecommand. Host lock removal can create race
conditon between ISR path(when RAID map update interrupt is raised) and IO
build path of driver, since IO build path is making use of RAID map, and in
case of RAID map update interrupt, old RAID map copy is memset to zero, which
some IOs may be referencing in build IO path. Changes done for performance
boost- 1) Added code to set SMP IRQ affinity per CPU. 2) Pass MSI-x index,
while issuing sysPD IO.

If consistent DMA mask is set to 64 bit, fall back to 32bit DMA mask and 32bit consistent DMA mask.
64bit consistent DMA mask may be set on some 64bit DMA slot, which causes DMA offset "10000000000000" and
MFI_INIT and IOCTL frames will have high memory addresses, leads to firmware FAULT.

Don't wait forever for firmware response for internal DCMDs sent from driver
firmware. Such DCMDs will be posted to firmware with timeout. Timeout is also
introduced for DCMD sent to abort the commands. DCMD sent via IOCTL path will
still be always blocking to keep the IOCTL design intact.

Currently, scsi error handling in scsi_io_completion() tries to
unconditionally requeue scsi command when device keeps some error state.
For example, UNIT_ATTENTION causes infinite retry with
action == ACTION_RETRY.
This is because retryable errors are thought to be temporary and the scsi
device will soon recover from those errors. Normally, such retry policy is
appropriate because the device will soon recover from temporary error state.
But there is no guarantee that device is able to recover from error state
immediately. Some hardware error can prevent device from recovering.
This patch adds timeout in scsi_io_completion() to avoid infinite command
retry in scsi_io_completion(). Once scsi command retry time is longer than
this timeout, the command is treated as failure.

iscsi_queuecommand will only take in commands that can fit in the
current window. So, if a command is on the cmdqueue then it can
fit in the current window. If a command is on the mgmtqueue, then
we are setting the immediate bit so they will also fit in the
window. As a result, we never need to to do a iscsi_conn_queue_work
when the maxCmdSn is increased.
What should happen is that a command will complete the window will
be increased, then the scsi layer will send us more commands by
running the scsi_device queues.

Replace the session lock with two locks, a forward lock and
a backwards lock named frwd_lock and back_lock respectively.
The forward lock protects resources that change while sending a
request to the target, such as cmdsn, queued_cmdsn, and allocating
task from the commands' pool with kfifo_out.
The backward lock protects resources that change while processing
a response or in error path, such as cmdsn_exp, cmdsn_max, and
returning tasks to the commands' pool with kfifo_in.
Under a steady state fast-path situation, that is when one
or more processes/threads submit IO to an iscsi device and
a single kernel upcall (e.g softirq) is dealing with processing
of responses without errors, this patch eliminates the contention
between the queuecommand()/request response/scsi_done() flows
associated with iscsi sessions.
Between the forward and the backward locks exists a strict locking
hierarchy. The mutual exclusion zone protected by the forward lock can
enclose the mutual exclusion zone protected by the backward lock but not
vice versa.
For example, in iscsi_conn_teardown or in iscsi_xmit_data when there is
a failure and __iscsi_put_task is called, the backward lock is taken while
the forward lock is still taken. On the other hand, if in the RX path a nop
is to be sent, for example in iscsi_handle_reject or __iscsi_complete_pdu
than the forward lock is released and the backward lock is taken for the
duration of iscsi_send_nopout, later the backward lock is released and the
forward lock is retaken.
libiscsi_tcp uses two kernel fifos the r2t pool and the r2t queue.
The insertion and deletion from these queues didn't corespond to the
assumption taken by the new forward/backwards session locking paradigm.
That is, in iscsi_tcp_clenup_task which belongs to the RX (backwards)
path, r2t is taken out from r2t queue and inserted to the r2t pool.
In iscsi_tcp_get_curr_r2t which belong to the TX (forward) path, r2t
is also inserted to the r2t pool and another r2t is pulled from r2t
queue.
Only in iscsi_tcp_r2t_rsp which is called in the RX path but can requeue
to the TX path, r2t is taken from the r2t pool and inserted to the r2t
queue.
In order to cope with this situation, two spin locks were added,
pool2queue and queue2pool. The former protects extracting from the
r2t pool and inserting to the r2t queue, and the later protects the
extracing from the r2t queue and inserting to the r2t pool.

Restructure the iscsi_tcp_r2t_rsp routine in order to avoid allocating
r2t from r2tpool.queue and returning it back in case the parameters
rhdr->data_length and or rhdr->data_offset prohibit the requing.
Since the values of these parameters are known prior to the allocation,
we can pre-check and thus avoid futile allocations.

For various error conditions the bfa driver just returns
'DID_ERROR', which carries no information at all about the
actual source of error.

Issue:
Driver holds rom-lock for too long during reset recovery.
During adapter reset testing, it was found that the driver
holds the rom-lock for too long, because of which other
drivers fail to acquire the rom-lock, leading to reset
failures.
The primary cause is, in the bootstrap code, while
holding the rom-lock, the driver checks if the peg is
halted, causing a 2 second contention.
Fix:
When a reset recovery starts, the driver deduces the cause, and
sets appropriate flags in watchdog & recover_adapter routines.
This flag should be used to determine if bootstrap is invoked
from probe or reset context, reducing the rom-lock footprint of
the drivers.

it should just skip the entry and adjust the total buffer size by
subtracting the skipped bytes from it. This is to ensure that there

Pending IO wait does not complete after triggering Graceful reset,
causing ack timeout and call traces.
Fix:
1. Reducing the IO command wait timeout before triggering reset,
as logically also timeout should be less than reset timeout (10sec).
2. Moving the abort IO after chip reset, because only after
chip reset, driver owns the IO otherwise it is with firmware and can
still revert back with response.

For ISP8xxx, adapter initialization is not retried if
qla4xxx_initialize_adapter fails.
Fix:
If qla4xxx_initialize_adapter fails, first check if failure is due to IRQs not
attached in order to skip retrial, then free the IRQs and then retry

In iSCSI Boot scenario, when machine is reboot/shutdown phase
the active sessions are not closed. Driver queue cleanup is
done as part of unload and device is disabled.
Sessions are still active, iSCSI commands are issued from
session which comes to driver, as driver cleanup and device
disabled there is kernel stack dump with errors.
Fix is invoking iscsi_session_failure with ISCSI_ERR_INVALID_HOST
on all the active sessions when shutdown routine is called.

When an MBX command timeout happens,the resources associated with
the MBX command were freed. If FW were to give the response to
host after the timeout value set by driver then driver crashes as the MBX Cmd
resources were already freed.
This patch fixes this issue by maintaing a state flag for each of
the MBX command posted/timedout/completed.

Add controller-based data-at-rest encryption compatibility
to ioaccel2 path (HP SSD Smart Path).
Encryption feature requires driver to supply additional fields
for encryption enable, tweak index, and data encryption key index
in the ioaccel2 request structure.
Encryption enable flag and data encryption key index come from
raid_map data structure from raid offload command.
During ioaccel2 submission, check device structure's raid map to see if
encryption is enabled for the device. If so, call new function below.
Add function set_encrypt_ioaccel2 to set encryption flag, data encryption key
index, and calculate tweak value from request's logical block address.

Allow driver to schedule a rescan whenever a request fails on the ioaccel2 path.
This eliminates the possibility of driver getting stuck in non-ioaccel mode.
IOaccel mode (HP SSD Smart Path) is disabled by driver upon error detection.
Driver relied on idea that request would be retried through normal path, and a
subsequent error would occur on that path, and be processed by controller
firmware. As part of that process, controller disables ioaccel mode and later
reinstates it, signalling driver to change modes.
In some error cases, the error will not duplicate on the standard path,
so the driver could get stuck in non-ioaccel mode.
To avoid that, we allow driver to request a rescan during the next run of the
rescan thread.

Allow SSD Smart Path for a controller to be disabled by
the user, regardless of settings in controller firmware
or array configuration.

For shared SAS configurations, hosts need to poll Smart Arrays
periodically in order to be able to detect configuration changes
such as logical drives being added or removed from remote hosts.
A register on the controller indicates when such events have
occurred, and the driver polls the register via a workqueue
and kicks off a rescan of devices if such an event is detected.
Additionally, changes to logical drive raid offload eligibility
are autodetected in this way.

This enables sending i/o's destined for RAID logical drives
which can be serviced by a single physical disk down a different,
faster i/o path directly to physical drives for certain logical
volumes on SSDs bypassing the Smart Array RAID stack for a
performance improvement.

For "mode 1" io accelerated commands, the command tag is in
a different location than for commands that go down the normal

When commands sent down the "fast path" fail, they must be re-tried down the
normal RAID path. We do this by kicking i/o's back to the scsi mid layer with
a DID_SOFT_ERROR status, which causes them to be retried. This won't work for
SG_IO's and other non REQ_TYPE_FS i/o's which could get kicked all the way back
to the application, which may have no idea that the command needs resubmitting
and likely no way to resubmit it in such a way the that driver can recognize it
as a resubmit and send it down the normal RAID path. So we just always send
non REQ_TYPE_FS i/o's down the normal RAID path, never down the "fast path".

For certain i/o's to certain devices (unmasked physical disks) we
can bypass the RAID stack firmware and do the i/o to the device
directly and it will be faster.

There is an extended report luns command which contains
additional information about physical devices. In particular
we need to get the physical device handle so we can use an
alternate i/o path for fast physical devices like SSDs so
we can speed up certain i/o's by bypassing the RAID stack
code in the controller firmware.

spin_lock_irqsave for the HBA lock is called in one function where flag
is local to that function. Another function is called from the first
function where lock has to be released using spin_unlock_irqrestore for
calling task_done of libsas. In the second function also flag is declared
and used. For calling task_done there is no need to enable the irq. So
instead of using spin_lock_irqsave and spin_unlock_irqrestore, spin_lock
and spin_unlock is used now. This also avoids passing the flags across all
the functions where HBA lock is being used. Also removed redundant code.

the driver to use 16 MSI-X vectors, despite the fact that
older controllers would provide only 4.
This was causing MSI-X registration to drop down to INTx
mode. But as the controller support performant mode, the
initialisation will become confused and cause the machine
to stall during boot.
This patch fixes up the MSI-X registration to re-issue
the pci_enable_msix() call with the correct number of
MSI-X vectors. With that the hpsa driver continues to
works on older controllers like the P200.

When the host template doesn't declare an eh_host_reset_handler
the eh_deadline mechanism is pointless and will set the

In the highly unusual case where two threads are running concurrently through
the scanning code scanning the same target, we run into the situation where
one may allocate the target while the other is still using it. In this case,
because the reap checks for STARGET_CREATED and kills the target without
reference counting, the second thread will do the wrong thing on reap.
Fix this by reference counting even creates and doing the STARGET_CREATED
check in the final put.

This patch eliminates the reap_ref and replaces it with a proper kref.
On last put of this kref, the target is removed from visibility in
sysfs. The final call to scsi_target_reap() for the device is done from
__scsi_remove_device() and only if the device was made visible. This
ensures that the target disappears as soon as the last device is gone
rather than waiting until final release of the device (which is often
too long).

qla2x00_mem_alloc() returns 1 on success and -ENOMEM on failure. On the
one hand the caller assumes non-zero is success but on the other hand
the caller also assumes that it returns an error code.
I've fixed it to return zero on success and a negative error code on
failure. This matches the documentation as well.

isci is needlessly tying libata's hands by returning
SAM_STAT_CHECK_CONDITION to some ata errors. Instead, prefer
SAS_PROTO_RESPONSE to let libata (via sas_ata_task_done()) disposition
the device-to-host fis.
For example isci is triggering an HSM Violation where AHCI is showing a
simple media error for the same bus condition:

libsas sometimes short circuits timeouts to force commands into error
recovery. It is misleading to log that the command timed-out in
sas_scsi_timed_out() when in fact it was just queued for error handling.
It's also redundant in the case of a true timeout as libata eh will
detect and report timeouts via it's AC_ERR_TIMEOUT facility.
Given that some environments consider "timeout" errors to be indicative
of impending device failure demote the sas_scsi_timed_out() timeout
message to be disabled by default. This parallels ata_scsi_timed_out().

SCSI retry delay upon SAM_STAT_BUSY/_SET_FULL was not being handled
in bnx2fc. This patch adds such handling by returning TARGET_BUSY
to the SCSI ML for the corresponding LUN until the retry timer expires.

The problem has been identified to be a change in the scsi_remove_device
path where a call to the pm_runtime_set_memalloc_noio was added when
del_gendisk is called in this path. Note that the new pm routine
attempts to cycle through all parent devices from the FC target device
to set the memalloc_noio flag. Because of this new change, a dependency
was created between the FC target device and the parent netdev device
in the destroy path.
In order to synchronized the destroy paths, bnx2fc has been modified
to flush all destroy workqueues in the NETDEV_UNREGISTER return path.

This was a debugging measure to toggle enabled/disabled
when testing. But for real production setups, it's not
safe to toggle this setting without either reloading
drivers of quiescing IO first. Neither of which the toggle
enforces.
Additionally, it makes drivers deal with the conditional
state.
Remove it completely. It's up to the driver whether iopoll
is enabled or not.

sleep_on is known broken and going away. The atari_scsi driver is one of
two remaining users in the falcon_get_lock() function, which is a rather
crazy piece of code. This does not attempt to fix the driver's locking
scheme in general, but at least prevents falcon_get_lock from going to
sleep when no other thread holds the same lock or tries to get it,
and we no longer schedule with irqs disabled.

In the first place, the loop 'for' in the macro 'for_each_isci_host'
(drivers/scsi/isci/host.h:314) is incorrect, because it accesses
the 3rd element of 2 element array. After the 2nd iteration it executes
the instruction:
ihost = to_pci_info(pdev)->hosts[2]
(while the size of the 'hosts' array equals 2) and reads an
out of range element.
In the second place, this loop is incorrectly optimized by GCC v4.8
(see http://marc.info/?l=linux-kernel&m=138998871911336&w=2).
As a result, on platforms with two SCU controllers,
the loop is executed more times than it can be (for i=0,1 and 2).
It causes kernel panic during entering the S3 state
and the following oops after 'rmmod isci':

Remove an erroneous BUG_ON() in the case of a hard reset timeout. The
reset timeout handler puts the port into the "awaiting link-up" state.
The timeout causes the device to be disconnected and we need to be in
the awaiting link-up state to re-connect the port. The BUG_ON() made
the incorrect assumption that resets never timeout and we always
complete the reset in the "resetting" state.
Testing this patch also uncovered that libata continues to attempt to
reset the port long after the driver has torn down the context. Once
the driver has committed to abandoning the link it must indicate to
libata that recovery ends by returning -ENODEV from
->lldd_I_T_nexus_reset().

This fixes requesting of the MSI-X vectors for the base response queue.
The iteration in the for loop in qla24xx_enable_msix() was incorrect.
We should only iterate of the first two MSI-X vectors and not the total
number of MSI-X vectors that have given to the driver for this device
from pci_enable_msix() in this function.

A selective retransmission request (SRR) is a fibre-channel
protocol control request which provides support for requesting
retransmission of a data sequence in response to an issue such as
frame loss or corruption. These events are experienced
infrequently in fibre-channel based networks which makes
it difficult to test and assess codepaths which handle these
events.
We were fortunate enough, for some definition of fortunate, to
have a metro-area single-mode SAN link which, at 10 GBPS
sustained load levels, would consistently generate SRR's in
a SCST based target implementation using our SCST/in-kernel
Qlogic target interface driver. In response to an SRR the
in-kernel Qlogic target driver immediately panics resulting
in a catastrophic storage failure for serviced initiators.
The culprit was a debug statement in the qla_target.c file which
does not verify that a pointer to the SCSI CDB is not null.
The unchecked pointer dereference results in the kernel panic
and resultant system failure.
The other two references to the SCSI CDB by the SRR handling code
use a ternary operator to verify a non-null pointer is being
acted on. This patch simply adds a similar test to the implicated
debug statement.
This patch is a candidate for any stable kernel being maintained
since it addresses a potentially catastrophic event with
minimal downside.

This patch performs configfs_depend_item() during TPG enable for
base_tpg (eg: non-NPIV) ports, and configfs_undepend_item() during
TPG disable for base_tpg.
This is done to ensure that any attempt to configfs rmdir a base_tpg
with active NPIV ports will fail with -EBUSY, until all associated
NPIV ports have been explicitly shutdown and base_tpg disabled.
Note that the actual configfs_[un]depend_item() is done from seperate
process context, as these are not intended to be called directly
from configfs callbacks.

This patch adds an check to qlt_stop_phase1() to avoid shutdown
when the base_vha contains a non-zero fc_host->npiv_vports_inuse
count.
This includes holding qla_tgt_mutex in qlt_stop_phase1() between
the fc_host->npiv_vports_inuse check + setting of tgt->tgt_stop to
avoid a possible race between qlt_lport_register() -> tcm_qla2xxx
-> tcm_qla2xxx_lport_register_npiv_cb() calling fc_vport_create().

This patch closes a race between qlt_lport_register() and
tcm_qla2xxx callback logic by holding qla_tgt_mutex before
making the callback.
In order for this to work, the qlt_add_target() and
qlt_remove_target() code has been changed to avoid the
accessing qla_tgt_mutex + list_[add,del] for NPIV enabled
ports.
This bug introduced in v3.14-rc1 code with commit 49a47f2.

driver-core now supports synchrnous self-deletion of attributes and
the asynchrnous removal mechanism is scheduled for removal. Use it
instead of device_schedule_callback(). This makes "delete" behave
synchronously.

The rport timers must be stopped before the SRP initiator destroys the
resources associated with the SCSI host. This is necessary because
otherwise the callback functions invoked from the SRP transport layer
could trigger a use-after-free. Stopping the rport timers before
invoking scsi_remove_host() can trigger long delays in the SCSI error
handler if a transport layer failure occurs while scsi_remove_host()
is in progress. Hence move the code for stopping the rport timers from
srp_rport_release() into a new function and invoke that function after
scsi_remove_host() has finished. This patch fixes the following
sporadic kernel crash:

The current behavior of the SRP transport layer when a transport layer
error is encountered is to block SCSI command processing only if
fast_io_fail_tmo != off. The current behavior of the FC transport
layer when a transport layer error is encountered is to block SCSI
command processing no matter which value fast_io_fail_tmo has been set
to. Make the behavior of the SRP transport layer consistent with that
of the FC transport layer to avoid confusion.

This patch changes qla2xxx qlt_lport_register() code to accept
target_lport_ptr + npiv_wwpn + npiv_wwnn parameters, and updates
tcm_qla2xxx to use the new tcm_qla2xxx_lport_register_npiv_cb()
callback for invoking fc_vport_create() from configfs context
via tcm_qla2xxx_npiv_make_lport() code.
In order for this to work, the qlt_lport_register() callback is
now called without holding qla_tgt_mutex, as the fc_vport creation
process will call qlt_vport_create() -> qlt_add_target(), which
already expects to acquire it.
It enforces /sys/kernel/config/target/qla2xxx_npiv/$NPIV_WWPN
naming in the following format:
$PHYSICAL_WWPN@$NPIV_WWPN:$NPIV_WWNN
and assumes the $PHYSICAL_WWPN in question has already had been
configured for target mode in non NPIV mode.
Finally, it updates existing tcm_qla2xxx_lport_register_cb() logic
to setup the non NPIV assignments that have now been moved out of
qlt_lport_register() code.

when a non zero return from the passed (*callback) does not call drop the
local reference via scsi_host_put() before returning.
This currently does not effect existing tcm_qla2xxx code as the passed callback
will never fail, but fix this up regardless for future code.

vqs are freed in virtscsi_freeze but the hotcpu_notifier is not

Race conditions are theoretically possible between the MPT PCI device
removal and the generic PCI bus rescan and device removal that can be
triggered via sysfs.
To avoid those race conditions make the MPT PCI code use
pci_stop_and_remove_bus_device_locked().

This patch removes a bug in enlarge_buffer() that can make a
read or write fail under special conditions.
After changing TRY_DIRECT_IO to 0 and ST_MAX_SG to 32 in
st_options.h, a program that writes a first block of 128k and
than a second bigger block (e.g. 256k) fails. The second write
returns errno EOVERFLOW, as enlarge_buffer() checks the sg list
and detects that it already is full.
As enlarge_buffer uses different page allocation orders
depending on the size of the buffer needed, the check does not
make sense.

In direct attach mode, if port features like QoS/Trunk/FEC/CR are
enabled, driver can receive PLOGI from the target before firmware
notifies lport online to the driver. In case the lport is not yet online
and a PLOGI is received, send a reject to the peer port, instead of
dropping it on the floor.

This patch includes change to enable firmware patch simplication
feature. This feature is targeted to address the requirement to have
independent patch release for firmware. Prior to 3.2.3, releasing a
patch fix for firmware requires changes to bfa, to use new firmware
images. But with these changes, if the new firmware is flashed on to the
HBA with brocade adapter management utilites, driver uses the new
firmware after checking the patch release byte in the firmware version.

do_div() is meant for divisions of 64-bit number by 32-bit numbers.
Passing 64-bit divisor types caused issues in the past on 32-bit platforms,
cfr. commit ea077b1b96 ("m68k: Truncate base

The former minimum valid value of 'eh_deadline' is 1s, which means
the earliest occasion to shorten EH is 1 second later since a
command is failed or timed out. But if we want to skip EH steps
ASAP, we have to wait until the first EH step is finished. If the
duration of the first EH step is long, this waiting time is
excruciating. So, it is necessary to accept 0 as the minimum valid
value for 'eh_deadline'.
According to my test, with Hannes' patchset 'New EH command timeout
handler' as well, the minimum IO time is improved from 73s
(eh_deadline = 1) to 43s(eh_deadline = 0) when commands are timed
out by disabling RSCN and target port.

32bit accesses are guaranteed to be atomic, so we can remove
the spinlock when checking for eh_deadline. We only need to
make sure to catch any updates which might happened during
the call to time_before(); if so we just recheck with the
correct value.

When a command runs into a timeout we need to send an 'ABORT TASK'
TMF. This is typically done by the 'eh_abort_handler' LLDD callback.
Conceptually, however, this function is a normal SCSI command, so
there is no need to enter the error handler.
This patch implements a new scsi_abort_command() function which
invokes an asynchronous function scsi_eh_abort_handler() to
abort the commands via the usual 'eh_abort_handler'.
If abort succeeds the command is either retried or terminated,
depending on the number of allowed retries. However, 'eh_eflags'
records the abort, so if the retry would fail again the
command is pushed onto the error handler without trying to
abort it (again); it'll be cleared up from SCSI EH.

(Handle disk devices which can not process medium access commands)
was introduced to offline any device which cannot process medium
access commands.
However, commit 3eef6257de
(Reduce error recovery time by reducing use of TURs) reduced
the number of TURs by sending it only on the first failing
command, which might or might not be a medium access command.
So in combination this results in an erratic device offlining
during EH; if the command where the TUR was sent upon happens
to be a medium access command the device will be set offline,
if not everything proceeds as normal.
This patch moves the check to the final test, eliminating
this problem.

After getting warnings in an allyesconfig build[1] from this
driver, I decided to remind myself just how old it was, and
whether it warranted fixing. In the Kconfig help text, I found:
"This driver will eventually be phased out entirely"
Going back to the history archive, I see the line was added[2]
in Feb 2002, when we moved from v2.4.2.1 ---> v2.4.2.2
So, with over a decade of notification, and multiple major releases
since then, I think we can justify removing this. Currently we have
people wasting time building it during routine testing, and then
wasting more time re-researching the known reported warnings, only to
find that nobody really is willing to integrate the fixes[3] for it.
A quick search didn't seem to indicate any active user base for it.
If someone happens to have a quirky _old_ card that the eleven year
old "new" driver doesn't work with, then it is entirely reasonable
that they stick with a kernel version that predates this removal.
