9d378dfac8 powerpc/booke64: Use SPRG7 for VDSO
Previously SPRG3 was marked for use by both VDSO and critical
interrupts (though critical interrupts were not fully implemented).

In commit 8b64a9dfb0 ("powerpc/booke64:
Use SPRG0/3 scratch for bolted TLB miss & crit int"), Mihai Caraman
made an attempt to resolve this conflict by restoring the VDSO value
early in the critical interrupt, but this has some issues:

 - It's incompatible with EXCEPTION_COMMON which restores r13 from the
   by-then-overwritten scratch (this cost me some debugging time).
 - It forces critical exceptions to be a special case handled
   differently from even machine check and debug level exceptions.
 - It didn't occur to me that it was possible to make this work at all
   (by doing a final "ld r13, PACA_EXCRIT+EX_R13(r13)") until after
   I made (most of) this patch. :-)

It might be worth investigating using a load rather than SPRG on return
from all exceptions (except TLB misses where the scratch never leaves
the SPRG) -- it could save a few cycles.  Until then, let's stick with
SPRG for all exceptions.

Since we cannot use SPRG4-7 for scratch without corrupting the state of
a KVM guest, move VDSO to SPRG7 on book3e.  Since neither SPRG4-7 nor
critical interrupts exist on book3s, SPRG3 is still used for VDSO
there.

b88c4767d9 powerpc: Move local setup.h declarations to arch includes
Move the few declarations from arch/powerpc/kernel/setup.h
into arch/powerpc/include/asm/setup.h.  This resolves a
sparse warning for arch/powerpc/mm/numa.c which defines
do_init_bootmem() but can't include the setup.h header
in the prior path.

061d19f279 powerpc: Delete __cpuinit usage from all users
The __cpuinit type of throwaway sections might have made sense
some time ago when RAM was more constrained, but now the savings
do not offset the cost and complications.  For example, the fix in
commit 5e427ec2d0 ("x86: Fix bit corruption at CPU resume time")
is a good example of the nasty type of bugs that can be created
with improper use of the various __init prefixes.

After a discussion on LKML[1] it was decided that cpuinit should go
the way of devinit and be phased out.  Once all the users are gone,
we can then finally remove the macros themselves from linux/init.h.

This removes all the powerpc uses of the __cpuinit macros.  There
are no __CPUINIT users in assembly files in powerpc.

18ad51dd34 powerpc: Add VDSO version of getcpu
We have a request for a fast method of getting CPU and NUMA node IDs
from userspace. This patch implements a getcpu VDSO function,
similar to x86.

Ben suggested we use SPRG3 which is userspace readable. SPRG3 can be
modified by a KVM guest, so we save the SPRG3 value in the paca and
restore it when transitioning from the guest to the host.

I have a glibc patch that implements sched_getcpu on top of this.
Testing on a POWER7:

baseline: 538 cycles
vdso:      30 cycles

Pull "Disintegrate and delete asm/system.h" from David Howells:
 "Here are a bunch of patches to disintegrate asm/system.h into a set of
  separate bits to relieve the problem of circular inclusion
  dependencies.

  I've built all the working defconfigs from all the arches that I can
  and made sure that they don't break.

  The reason for these patches is that I recently encountered a circular
  dependency problem that came about when I produced some patches to
  optimise get_order() by rewriting it to use ilog2().

  This uses bitops - and on the SH arch asm/bitops.h drags in
  asm-generic/get_order.h by a circuituous route involving asm/system.h.

  The main difficulty seems to be asm/system.h.  It holds a number of
  low level bits with no/few dependencies that are commonly used (eg.
  memory barriers) and a number of bits with more dependencies that
  aren't used in many places (eg.  switch_to()).

  These patches break asm/system.h up into the following core pieces:

    (1) asm/barrier.h

        Move memory barriers here.  This already done for MIPS and Alpha.

    (2) asm/switch_to.h

        Move switch_to() and related stuff here.

    (3) asm/exec.h

        Move arch_align_stack() here.  Other process execution related bits
        could perhaps go here from asm/processor.h.

    (4) asm/cmpxchg.h

        Move xchg() and cmpxchg() here as they're full word atomic ops and
        frequently used by atomic_xchg() and atomic_cmpxchg().

    (5) asm/bug.h

        Move die() and related bits.

    (6) asm/auxvec.h

        Move AT_VECTOR_SIZE_ARCH here.

  Other arch headers are created as needed on a per-arch basis."

Fixed up some conflicts from other header file cleanups and moving code
around that has happened in the meantime, so David's testing is somewhat
weakened by that.  We'll find out anything that got broken and fix it..

909af768e8 coredump: remove VM_ALWAYSDUMP flag
The motivation for this patchset was that I was looking at a way for a
qemu-kvm process, to exclude the guest memory from its core dump, which
can be quite large.  There are already a number of filter flags in
/proc/<pid>/coredump_filter, however, these allow one to specify 'types'
of kernel memory, not specific address ranges (which is needed in this
case).

Since there are no more vma flags available, the first patch eliminates
the need for the 'VM_ALWAYSDUMP' flag.  The flag is used internally by
the kernel to mark vdso and vsyscall pages.  However, it is simple
enough to check if a vma covers a vdso or vsyscall page without the need
for this flag.

The second patch then replaces the 'VM_ALWAYSDUMP' flag with a new
'VM_NODUMP' flag, which can be set by userspace using new madvise flags:
'MADV_DONTDUMP', and unset via 'MADV_DODUMP'.  The core dump filters
continue to work the same as before unless 'MADV_DONTDUMP' is set on the
region.

In my testing the qemu core dump shrunk from 383MB -> 13MB with this
patch.

I also believe that the 'MADV_DONTDUMP' flag might be useful for
security sensitive apps, which might want to select which areas are
dumped.

This patch:

The VM_ALWAYSDUMP flag is currently used by the coredump code to
indicate that a vma is part of a vsyscall or vdso section.  However, we
can determine if a vma is in one these sections by checking it against
the gate_vma and checking for a non-NULL return value from
arch_vma_name().  Thus, freeing a valuable vma bit.

None of the files touched here are modules, and they are not
exporting any symbols either -- so there is no need to be including
the module.h.  Builds of all the files remains successful.

Even kernel/module.c does not need to include it, since it includes
linux/moduleloader.h instead.

f2053f1a7b powerpc/perf_counter: Fix vdso detection
perf_counter uses arch_vma_name() to detect a vdso region which in turn uses
current->mm->context.vdso_base. We need to initialise this before doing
the mmap or else we fail to detect the vdso.

On a microbenchmark that bounces a token between two 64bit processes over pipes
and calls gettimeofday each iteration (to access the VDSO), our context switch
rate goes from 268k to 277k ctx switches/sec (tested on a 4GHz POWER6).

fc5243d98a [S390] arch_setup_additional_pages arguments
arch_setup_additional_pages currently gets two arguments, the binary
format descripton and an indication if the process uses an executable
stack or not. The second argument is not used by anybody, it could
be removed without replacement.

What actually does make sense is to pass an indication if the process
uses the elf interpreter or not. The glibc code will not use anything
from the vdso if the process does not use the dynamic linker, so for
statically linked binaries the architecture backend can choose not
to map the vdso.

We're soon running out of CPU features and I need to add some new

2d1b202762 powerpc: Fixup lwsync at runtime
To allow for a single kernel image on e500 v1/v2/mc we need to fixup lwsync
at runtime.  On e500v1/v2 lwsync causes an illop so we need to patch up
the code.  We default to 'sync' since that is always safe and if the cpu
is capable we will replace 'sync' with 'lwsync'.

We introduce CPU_FTR_LWSYNC as a way to determine at runtime if this is
needed.  This flag could be moved elsewhere since we dont really use it
for the normal CPU_FTR purpose.

Finally we only store the relative offset in the fixup section to keep it
as small as possible rather than using a full fixup_entry.

89f5b7da2a Reinstate ZERO_PAGE optimization in 'get_user_pages()' and fix XIP
KAMEZAWA Hiroyuki and Oleg Nesterov point out that since the commit
557ed1fa26 ("remove ZERO_PAGE") removed
the ZERO_PAGE from the VM mappings, any users of get_user_pages() will
generally now populate the VM with real empty pages needlessly.

We used to get the ZERO_PAGE when we did the "handle_mm_fault()", but
since fault handling no longer uses ZERO_PAGE for new anonymous pages,
we now need to handle that special case in follow_page() instead.

In particular, the removal of ZERO_PAGE effectively removed the core
file writing optimization where we would skip writing pages that had not
been populated at all, and increased memory pressure a lot by allocating
all those useless newly zeroed pages.

This reinstates the optimization by making the unmapped PTE case the
same as for a non-existent page table, which already did this correctly.

While at it, this also fixes the XIP case for follow_page(), where the
caller could not differentiate between the case of a page that simply
could not be used (because it had no "struct page" associated with it)
and a page that just wasn't mapped.

We do that by simply returning an error pointer for pages that could not
be turned into a "struct page *".  The error is arbitrarily picked to be
EFAULT, since that was what get_user_pages() already used for the
equivalent IO-mapped page case.

[ Also removed an impossible test for pte_offset_map_lock() failing:
  that's not how that function works ]

fbe481756d [POWERPC] vdso: Fixes for cache block sizes
The current VDSO implementation is hardcoded to 128 byte cache blocks,
which are only used on IBM's 64-bit processors.

Convert it to get the cache block sizes out of vdso_data instead,
similar to how the ppc64 in-kernel cache flush does it.

7b5acbaac3 [POWERPC] Don't expose clock vDSO functions when CPU has no timebase
We forgot to remove the clock_gettime, clock_getres and get_tbfreq vDSO
calls on CPUs that have no timebase such as 601 or 403 (old CPUs that have
different mechanisms and for which the vDSO code will not work properly).
This fixes it.

7ac9a13717 [POWERPC] Fix vDSO page count calculation
The recent vDSO consolidation patches broke powerpc due to a mistake
in the definition of MAXPAGES constants. This fixes it by moving to
a dynamically allocated array of pages instead as I don't like much
hard coded size limits. Also move the vdso initialisation to an initcall
since it doesn't really need to be done -that- early.

Applogies for not catching the breakage earlier, Roland _did_ CC me on
his patches a while ago, I got busy with other things and forgot to test
them.

0909c8c2d5 [POWERPC] Support feature fixups in vdso's
This patch reworks the feature fixup mecanism so vdso's can be fixed up.
The main issue was that the construct:

        .long   label  (or .llong on 64 bits)

will not work in the case of a shared library like the vdso. It will
generate an empty placeholder in the fixup table along with a reloc,
which is not something we can deal with in the vdso.

The idea here (thanks Alan Modra !) is to instead use something like:

1:
        .long   label - 1b

That is, the feature fixup tables no longer contain addresses of bits of
code to patch, but offsets of such code from the fixup table entry
itself. That is properly resolved by ld when building the .so's. I've
modified the fixup mecanism generically to use that method for the rest
of the kernel as well.

Another trick is that the 32 bits vDSO included in the 64 bits kernel
need to have a table in the 64 bits format. However, gas does not
support 32 bits code with a statement of the form:

        .llong  label - 1b  (Or even just .llong label)

That is, it cannot emit the right fixup/relocation for the linker to use
to assign a 32 bits address to an .llong field. Thus, in the specific
case of the 32 bits vdso built as part of the 64 bits kernel, we are
using a modified macro that generates:

        .long   0xffffffff
        .llong  label - 1b

Note that is assumes that the value is negative which is enforced by
the .lds (those offsets are always negative as the .text is always
before the fixup table and gas doesn't support emiting the reloc the
other way around).

This patch cleans up some locking & error handling in the ppc vdso and
moves the vdso base pointer from the thread struct to the mm context
where it more logically belongs. It brings the powerpc implementation
closer to Ingo's new x86 one and also adds an arch_vma_name() function
allowing to print [vsdo] in /proc/<pid>/maps if Ingo's x86 vdso patch is
also applied.

e8222502ee [PATCH] powerpc: Kill _machine and hard-coded platform numbers
This removes statically assigned platform numbers and reworks the
powerpc platform probe code to use a better mechanism.  With this,
board support files can simply declare a new machine type with a
macro, and implement a probe() function that uses the flattened
device-tree to detect if they apply for a given machine.

We now have a machine_is() macro that replaces the comparisons of
_machine with the various PLATFORM_* constants.  This commit also
changes various drivers to use the new macro instead of looking at
_machine.

This patch adds mm->task_size to keep track of the task size of a given mm
and uses that to fix the powerpc vdso so that it uses the mm task size to
decide what pages to fault in instead of the current thread flags (which
broke when ptracing).

(akpm: I expect that mm_struct.task_size will become the way in which we
finally sort out the confusion between 32-bit processes and 32-bit mm's.  It
may need tweaks, but at this stage this patch is powerpc-only.)

6aab341e0a mm: re-architect the VM_UNPAGED logic
This replaces the (in my opinion horrible) VM_UNMAPPED logic with very
explicit support for a "remapped page range" aka VM_PFNMAP.  It allows a
VM area to contain an arbitrary range of page table entries that the VM
never touches, and never considers to be normal pages.

Any user of "remap_pfn_range()" automatically gets this new
functionality, and doesn't even have to mark the pages reserved or
indeed mark them any other way.  It just works.  As a side effect, doing
mmap() on /dev/mem works for arbitrary ranges.

Sparc update from David in the next commit.

0b14c179a4 [PATCH] unpaged: VM_UNPAGED
Although we tend to associate VM_RESERVED with remap_pfn_range, quite a few
drivers set VM_RESERVED on areas which are then populated by nopage.  The
PageReserved removal in 2.6.15-rc1 changed VM_RESERVED not to free pages in
zap_pte_range, without changing those drivers not to set it: so their pages
just leak away.

Let's not change miscellaneous drivers now: introduce VM_UNPAGED at the core,
to flag the special areas where the ptes may have no struct page, or if they
have then it's not to be touched.  Replace most instances of VM_RESERVED in
core mm by VM_UNPAGED.  Force it on in remap_pfn_range, and the sparc and
sparc64 io_remap_pfn_range.

Revert addition of VM_RESERVED to powerpc vdso, it's not needed there.  Is it
needed anywhere?  It still governs the mm->reserved_vm statistic, and special
vmas not to be merged, and areas not to be core dumped; but could probably be
eliminated later (the drivers are probably specifying it because in 2.4 it
kept swapout off the vma, but in 2.6 we work from the LRU, which these pages
don't get on).

Use the VM_SHM slot for VM_UNPAGED, and define VM_SHM to 0: it serves no
purpose whatsoever, and should be removed from drivers when we clean up.

a7f290dad3 [PATCH] powerpc: Merge vdso's and add vdso support to 32 bits kernel
This patch moves the vdso's to arch/powerpc, adds support for the 32
bits vdso to the 32 bits kernel, rename systemcfg (finally !), and adds
some new (still untested) routines to both vdso's: clock_gettime() with
support for CLOCK_REALTIME and CLOCK_MONOTONIC, clock_getres() (same
clocks) and get_tbfreq() for glibc to retreive the timebase frequency.

Tom,Steve: The implementation of get_tbfreq() I've done for 32 bits
returns a long long (r3, r4) not a long. This is such that if we ever
add support for >4Ghz timebases on ppc32, the userland interface won't
have to change.

I have tested gettimeofday() using some glibc patches in both ppc32 and
ppc64 kernels using 32 bits userland (I haven't had a chance to test a
64 bits userland yet, but the implementation didn't change and was
tested earlier). I haven't tested yet the new functions.
