341acbb3aa KVM: PPC: BOOK3S: HV: Use base page size when comparing against slb value
With guests supporting Multiple page size per segment (MPSS),
hpte_page_size returns the actual page size used. Add a new function to
return base page size and use that to compare against the the page size
calculated from SLB. Without this patch a hpte lookup can fail since
we are comparing wrong page size in kvmppc_hv_find_lock_hpte.

e2500be2b8 powerpc/powernv: Remove OPAL v1 takeover
In commit 27f4488872 "Add OPAL takeover from PowerVM" we added support
for "takeover" on OPAL v1 machines.

This was a mode of operation where we would boot under pHyp, and query
for the presence of OPAL. If detected we would then do a special
sequence to take over the machine, and the kernel would end up running
in hypervisor mode.

OPAL v1 was never a supported product, and was never shipped outside
IBM. As far as we know no one is still using it.

Newer versions of OPAL do not use the takeover mechanism. Although the
query for OPAL should be harmless on machines with newer OPAL, we have
seen a machine where it causes a crash in Open Firmware.

The code in early_init_devtree() to copy boot_command_line into cmd_line
was added in commit 817c21ad9a "Get kernel command line accross OPAL
takeover", and AFAIK is only used by takeover, so should also be
removed.

d997c00c5a powerpc: Add ppc_global_function_entry()
ABIv2 has the concept of a global and local entry point to a function.
In most cases we are interested in the local entry point, and so that is
what ppc_function_entry() returns.

However we have a case in the ftrace code where we want the global entry
point, and there may be other places we need it too. Rather than special
casing each, add an accessor.

For ABIv1 and 32-bit there is only a single entry point, so we return
that. That means it's safe for the caller to use this without also
checking the ABI version.

The generic code uses gcc built-ins which work fine so there's no benefit
in implementing our own anymore.

We can't completely remove the ld/st_le* functions as some historical
cruft still uses them, but that's next on the radar

357b2f3dd9 powerpc/eeh: Dump PE location code
As Ben suggested, it's meaningful to dump PE's location code
for site engineers when hitting EEH errors. The patch introduces
function eeh_pe_loc_get() to retireve the location code from
dev-tree so that we can output it when hitting EEH errors.

If primary PE bus is root bus, the PHB's dev-node would be tried
prior to root port's dev-node. Otherwise, the upstream bridge's
dev-node of the primary PE bus will be check for the location code
directly.

5c7a35e3e2 powerpc/powernv: Fix killed EEH event
On PowerNV platform, EEH errors are reported by IO accessors or poller
driven by interrupt. After the PE is isolated, we won't produce EEH
event for the PE. The current implementation has possibility of EEH
event lost in this way:

The interrupt handler queues one "special" event, which drives the poller.
EEH thread doesn't pick the special event yet. IO accessors kicks in, the
frozen PE is marked as "isolated" and EEH event is queued to the list.
EEH thread runs because of special event and purge all existing EEH events.
However, we never produce an other EEH event for the frozen PE. Eventually,
the PE is marked as "isolated" and we don't have EEH event to recover it.

The patch fixes the issue to keep EEH events for PEs that have been
marked as "isolated" with the help of additional "force" help to
eeh_remove_event().

3df48c981d powerpc/perf: Ensure all EBB register state is cleared on fork()
In commit 330a1eb "Core EBB support for 64-bit book3s" I messed up
clear_task_ebb(). It clears some but not all of the task's Event Based
Branch (EBB) registers when we duplicate a task struct.

That allows a child task to observe the EBBHR & EBBRR of its parent,
which it should not be able to do.

Fix it by clearing EBBHR & EBBRR.

96d0161086 powerpc: Correct DSCR during TM context switch
Correct the DSCR SPR becoming temporarily corrupted if a task is
context switched during a transaction.

The problem occurs while suspending the task and is caused by saving
the DSCR to thread.dscr after it has already been set to the CPU's
default value:

__switch_to() calls __switch_to_tm()
	which calls tm_reclaim_task()
	which calls tm_reclaim_thread()
	which calls tm_reclaim()
		where the DSCR is set to the CPU's default
__switch_to() calls _switch()
		where thread.dscr is set to the DSCR

When the task is resumed, it's transaction will be doomed (as usual)
and the DSCR SPR will be corrupted, although the checkpointed value
will be correct. Therefore the DSCR will be immediately corrected by
the transaction aborting, unless it has been suspended. In that case
the incorrect value can be seen by the task until it resumes the
transaction.

The fix is to treat the DSCR similarly to the TAR and save it early
in __switch_to().

A program exposing the problem is added to the kernel self tests as:
tools/testing/selftests/powerpc/tm/tm-resched-dscr.

fb5a515704 powerpc: Remove platforms/wsp and associated pieces
__attribute__ ((unused))

WSP is the last user of CONFIG_PPC_A2, so we remove that as well.

Although CONFIG_PPC_ICSWX still exists, it's no longer selectable for
any Book3E platform, so we can remove the code in mmu-book3e.h that
depended on it.

 "Here is the bulk of the powerpc changes for this merge window.  It got
  a bit delayed in part because I wasn't paying attention, and in part
  because I discovered I had a core PCI change without a PCI maintainer
  ack in it.  Bjorn eventually agreed it was ok to merge it though we'll
  probably improve it later and I didn't want to rebase to add his ack.

  There is going to be a bit more next week, essentially fixes that I
  still want to sort through and test.

  The biggest item this time is the support to build the ppc64 LE kernel
  with our new v2 ABI.  We previously supported v2 userspace but the
  kernel itself was a tougher nut to crack.  This is now sorted mostly
  thanks to Anton and Rusty.

  We also have a fairly big series from Cedric that add support for
  64-bit LE zImage boot wrapper.  This was made harder by the fact that
  traditionally our zImage wrapper was always 32-bit, but our new LE
  toolchains don't really support 32-bit anymore (it's somewhat there
  but not really "supported") so we didn't want to rely on it.  This
  meant more churn that just endian fixes.

  This brings some more LE bits as well, such as the ability to run in
  LE mode without a hypervisor (ie. under OPAL firmware) by doing the
  right OPAL call to reinitialize the CPU to take HV interrupts in the
  right mode and the usual pile of endian fixes.

  There's another series from Gavin adding EEH improvements (one day we
  *will* have a release with less than 20 EEH patches, I promise!).

  Another highlight is the support for the "Split core" functionality on
  P8 by Michael.  This allows a P8 core to be split into "sub cores" of
  4 threads which allows the subcores to run different guests under KVM
  (the HW still doesn't support a partition per thread).

  And then the usual misc bits and fixes ..."

a5d862576a powerpc: Allow ppc_md platform hook to override memory_block_size_bytes
The pseries platform code unconditionally overrides
memory_block_size_bytes regardless of the running platform.

Create a ppc_md hook that so each platform can choose to
do what it wants.

5d73320a96 powerpc: 64bit sendfile is capped at 2GB
commit 8f9c0119d7 (compat: fs: Generic compat_sys_sendfile
implementation) changed the PowerPC 64bit sendfile call from
sys_sendile64 to sys_sendfile.

Unfortunately this broke sendfile of lengths greater than 2G because
sys_sendfile caps at MAX_NON_LFS. Restore what we had previously which
fixes the bug.

4926616c77 powerpc/powernv: Add calls to support little endian host
When running as a powernv "host" system on P8, we need to switch
the endianness of interrupt handlers. This does it via the appropriate
call to the OPAL firmware which may result in just switching HID0:HILE
but depending on the processor version might need to do a few more
things. This call must be done early before any other processor has
been brought out of firmware.

f6187769da sys_sgetmask/sys_ssetmask: add CONFIG_SGETMASK_SYSCALL
sys_sgetmask and sys_ssetmask are obsolete system calls no longer
supported in libc.

This patch replaces architecture related __ARCH_WANT_SYS_SGETMAX by expert
mode configuration.That option is enabled by default for those
architectures.

4f9b16a647 mm: disable zone_reclaim_mode by default
When it was introduced, zone_reclaim_mode made sense as NUMA distances
punished and workloads were generally partitioned to fit into a NUMA
node.  NUMA machines are now common but few of the workloads are
NUMA-aware and it's routine to see major performance degradation due to
zone_reclaim_mode being enabled but relatively few can identify the
problem.

Those that require zone_reclaim_mode are likely to be able to detect
when it needs to be enabled and tune appropriately so lets have a
sensible default for the bulk of users.

This patch (of 2):

zone_reclaim_mode causes processes to prefer reclaiming memory from
local node instead of spilling over to other nodes.  This made sense
initially when NUMA machines were almost exclusively HPC and the
workload was partitioned into nodes.  The NUMA penalties were
sufficiently high to justify reclaiming the memory.  On current machines
and workloads it is often the case that zone_reclaim_mode destroys
performance but not all users know how to detect this.  Favour the
common case and disable it by default.  Users that are sophisticated
enough to know they need zone_reclaim_mode will detect it.

c46a7c817e x86: define _PAGE_NUMA by reusing software bits on the PMD and PTE levels
_PAGE_NUMA is currently an alias of _PROT_PROTNONE to trap NUMA hinting
faults on x86.  Care is taken such that _PAGE_NUMA is used only in
situations where the VMA flags distinguish between NUMA hinting faults
and prot_none faults.  This decision was x86-specific and conceptually
it is difficult requiring special casing to distinguish between PROTNONE
and NUMA ptes based on context.

Fundamentally, we only need the _PAGE_NUMA bit to tell the difference
between an entry that is really unmapped and a page that is protected
for NUMA hinting faults as if the PTE is not present then a fault will
be trapped.

Swap PTEs on x86-64 use the bits after _PAGE_GLOBAL for the offset.
This patch shrinks the maximum possible swap size and uses the bit to
uniquely distinguish between NUMA hinting ptes and swap ptes.

was a pretty active cycle for KVM.  Changes include:
 
 - a lot of s390 changes: optimizations, support for migration,

 - ARM changes are pretty small: support for the PSCI 0.2 hypercall
   interface on both the guest and the host (the latter acked by Catalin)

 - support for running u-boot on embedded POWER targets
 
 - pretty large changes to MIPS too, completing the userspace interface
   and improving the handling of virtualized timer hardware
 
 - for x86, a larger set of changes is scheduled for 3.17.  Still,
   we have a few emulator bugfixes and support for running nested
   fully-virtualized Xen guests (para-virtualized Xen guests have
   always worked).  And some optimizations too.

9bc01a9bc7 KVM: PPC: Book3S HV: Work around POWER8 performance monitor bugs
This adds workarounds for two hardware bugs in the POWER8 performance
monitor unit (PMU), both related to interrupt generation.  The effect
of these bugs is that PMU interrupts can get lost, leading to tools
such as perf reporting fewer counts and samples than they should.

The first bug relates to the PMAO (perf. mon. alert occurred) bit in
MMCR0; setting it should cause an interrupt, but doesn't.  The other
bug relates to the PMAE (perf. mon. alert enable) bit in MMCR0.
Setting PMAE when a counter is negative and counter negative
conditions are enabled to cause alerts should cause an alert, but
doesn't.

The workaround for the first bug is to create conditions where a
counter will overflow, whenever we are about to restore a MMCR0
value that has PMAO set (and PMAO_SYNC clear).  The workaround for
the second bug is to freeze all counters using MMCR2 before reading
MMCR0.

Old guests try to use the magic page, but map their trampoline code inside
of an NX region.

Since we can't fix those old kernels, try to detect whether the guest is sane
or not. If not, just disable NX functionality in KVM so that old guests at
least work at all. For newer guests, add a bit that we can set to keep NX
functionality available.

On recent IBM Power CPUs, while the hashed page table is looked up using
the page size from the segmentation hardware (i.e. the SLB), it is
possible to have the HPT entry indicate a larger page size.  Thus for
example it is possible to put a 16MB page in a 64kB segment, but since
the hash lookup is done using a 64kB page size, it may be necessary to
put multiple entries in the HPT for a single 16MB page.  This
capability is called mixed page-size segment (MPSS).  With MPSS,
there are two relevant page sizes: the base page size, which is the
size used in searching the HPT, and the actual page size, which is the
size indicated in the HPT entry. [ Note that the actual page size is
always >= base page size ].

We use "ibm,segment-page-sizes" device tree node to advertise
the MPSS support to PAPR guest. The penc encoding indicates whether
we support a specific combination of base page size and actual
page size in the same segment. We also use the penc value in the
LP encoding of HPTE entry.

This patch exposes MPSS support to KVM guest by advertising the
feature via "ibm,segment-page-sizes". It also adds the necessary changes
to decode the base page size and the actual page size correctly from the
HPTE entry.

POWER8 introduced a new interrupt type called "Facility unavailable interrupt"
which contains its status message in a new register called FSCR.

Handle these exits and try to emulate instructions for unhandled facilities.

5deb8e7ad8 KVM: PPC: Make shared struct aka magic page guest endian
The shared (magic) page is a data structure that contains often used
supervisor privileged SPRs accessible via memory to the user to reduce
the number of exits we have to take to read/write them.

When we actually share this structure with the guest we have to maintain
it in guest endianness, because some of the patch tricks only work with
native endian load/store operations.

Since we only share the structure with either host or guest in little
endian on book3s_64 pr mode, we don't have to worry about booke or book3s hv.

For booke, the shared struct stays big endian. For book3s_64 hv we maintain
the struct in host native endian, since it never gets shared with the guest.

For book3s_64 pr we introduce a variable that tells us which endianness the
shared struct is in and route every access to it through helper inline
functions that evaluate this variable.

8f20a3ab27 KVM: PPC: E500: Add dcbtls emulation
The dcbtls instruction is able to lock data inside the L1 cache.

We don't want to give the guest actual access to hardware cache locks,
as that could influence other VMs on the same system. But we can tell
the guest that its locking attempt failed.

By implementing the instruction we at least don't give the guest a
program exception which it definitely does not expect.

1739ea9e13 powerpc: Fix regression of per-CPU DSCR setting
Since commit "efcac65 powerpc: Per process DSCR + some fixes (try#4)"
it is no longer possible to set the DSCR on a per-CPU basis.

The old behaviour was to minipulate the DSCR SPR directly but this is no
longer sufficient: the value is quickly overwritten by context switching.

This patch stores the per-CPU DSCR value in a kernel variable rather than
directly in the SPR and it is used whenever a process has not set the DSCR
itself. The sysfs interface (/sys/devices/system/cpu/cpuN/dscr) is unchanged.

Writes to the old global default (/sys/devices/system/cpu/dscr_default)
now set all of the per-CPU values and reads return the last written value.

The new per-CPU default is added to the paca_struct and is used everywhere
outside of sysfs.c instead of the old global default.

e2186023f2 powerpc/powernv: Add support for POWER8 split core on powernv
Upcoming POWER8 chips support a concept called split core. This is where the
core can be split into subcores that although not full cores, are able to
appear as full cores to a guest.

The splitting & unsplitting procedure is mildly complicated, and explained at
length in the comments within the patch.

One notable detail is that when splitting or unsplitting we need to pull
offline cpus out of their offline state to do work as part of the procedure.

The interface for changing the split mode is via a sysfs file, eg:

 $ echo 2 > /sys/devices/system/cpu/subcores_per_core

Currently supported values are '1', '2' and '4'. And indicate respectively that
the core should be unsplit, split in half, and split in quarters. These modes
correspond to threads_per_subcore of 8, 4 and 2.

We do not allow changing the split mode while KVM VMs are active. This is to
prevent the value changing while userspace is configuring the VM, and also to
prevent the mode being changed in such a way that existing guests are unable to
be run.

CPU hotplug fixes by Srivatsa.  max_cpus fixes by Mahesh.  cpuset fixes by
benh.  Fix for irq race by paulus.  The rest by mikey and mpe.

5853aef1ac powerpc: Add threads_per_subcore
On POWER8 we have a new concept of a subcore. This is what happens when
you take a regular core and split it. A subcore is a grouping of two or
four SMT threads, as well as a handfull of SPRs which allows the subcore
to appear as if it were a core from the point of view of a guest.

Unlike threads_per_core which is fixed at boot, threads_per_subcore can
change while the system is running. Most code will not want to use
threads_per_subcore.

To support split core we need to be able to force all secondaries into
nap, so the core can detect they are idle and do an unsplit.

Currently power7_nap() will return without napping if there is an irq
pending. We want to ignore the pending irq and nap anyway, we will deal
with the interrupt later.

441c19c8a2 powerpc/kvm/book3s_hv: Rework the secondary inhibit code
As part of the support for split core on POWER8, we want to be able to
block splitting of the core while KVM VMs are active.

The logic to do that would be exactly the same as the code we currently
have for inhibiting onlining of secondaries.

Instead of adding an identical mechanism to block split core, rework the
secondary inhibit code to be a "HV KVM is active" check. We can then use
that in both the cpu hotplug code and the upcoming split core code.

 should use @high and @higha in place of @h and @ha.  @h and @ha
 (and their associated relocs R_PPC64_ADDR16_HI and R_PPC64_ADDR16_HA)
 now report overflow if the value is out of 32-bit signed range.
 ie. @h and @ha assume you're building a 32-bit value. This is needed
 to report out-of-range -mcmodel=medium toc pointer offsets in @toc@h
 and @toc@ha expressions, and for consistency I did the same for all
 other @h and @ha relocs.

Replacing @h with @high in one strategic location fixes the relocation
errors. This has to be done conditionally since the assembler either
supports @h or @high but not both.

ba52464a62 of: Stop naming platform_device using dcr address
There is now a way to ensure all platform devices get a unique name when
populated from the device tree, and the DCR_NATIVE code path is broken
anyway. PowerPC Cell (PS3) is the only platform that actually uses this
path.  Most likely nobody will notice if it is killed. Remove the code
and associated ugly #ifdef.

The user-visible impact of this patch is that any DCR device on Cell
will get a new name in the /sys/devices hierarchy.

00f554fade powerpc: memcpy optimization for 64bit LE
Unaligned stores take alignment exceptions on POWER7 running in little-endian.
This is a dumb little-endian base memcpy that prevents unaligned stores.
Once booted the feature fixup code switches over to the VMX copy loops
(which are already endian safe).

The question is what we do before that switch over. The base 64bit
memcpy takes alignment exceptions on POWER7 so we can't use it as is.
Fixing the causes of alignment exception would slow it down, because
we'd need to ensure all loads and stores are aligned either through
rotate tricks or bytewise loads and stores. Either would be bad for
all other 64bit platforms.

Our PV guest patching code assembles chunks of instructions on the fly when it
encounters more complicated instructions to hijack. These instructions need
to live in a section that we don't mark as non-executable, as otherwise we
fault when jumping there.

Right now we put it into the .bss section where it automatically gets marked
as non-executable. Add a check to the NX setting function to ensure that we
leave these particular pages executable.

7f06f21d40 powerpc/tm: Add checking to treclaim/trechkpt
If we do a treclaim and we are not in TM suspend mode, it results in a TM bad
thing (ie. a 0x700 program check).  Similarly if we do a trechkpt and we have
an active transaction or TEXASR Failure Summary (FS) is not set, we also take a
TM bad thing.

This should never happen, but if it does (ie. a kernel bug), the cause is
almost impossible to debug as the GPR state is mostly userspace and hence we
don't get a call chain.

This adds some checks in these cases case a BUG_ON() (in asm) in case we ever
hit these cases.  It moves the register saving around to preserve r1 till later
also.

2299d03a63 powerpc: powernv: Framework to show the correct clock in /proc/cpuinfo
Currently, the code in setup-common.c for powerpc assumes that all
clock rates are same in a smp system. This value is cached in the
variable named ppc_proc_freq and is the value that is reported in
/proc/cpuinfo.

However on the PowerNV platform, the clock rate is same only across
the threads of the same core. Hence the value that is reported in
/proc/cpuinfo is incorrect on PowerNV platforms. We need a better way
to query and report the correct value of the processor clock in
/proc/cpuinfo.

The patch achieves this by creating a machdep_call named
get_proc_freq() which is expected to returns the frequency in Hz. The
code in show_cpuinfo() can invoke this method to display the correct
clock rate on platforms that have implemented this method. On the
other powerpc platforms it can use the value cached in ppc_proc_freq.

2196c6f1ed powerpc/powernv: Return secondary CPUs to firmware before FW update
Firmware update on PowerNV platform takes several minutes. During
this time one CPU is stuck in FW and the kernel complains about "soft
lockups".

This patch returns all secondary CPUs to firmware before starting
firmware update process.

b2b5efcf20 powerpc/powernv: Fundamental reset on PLX ports
The patch intends to support fundamental reset on PLX downstream
ports. If the PCI device matches any one of the internal table,
which includes PLX vendor ID, bridge device ID, register offset
for fundamental reset and bit, fundamental reset will be done
accordingly. Otherwise, it will fail back to hot reset.

Additional flag (EEH_DEV_FRESET) is introduced to record the last
reset type on the PCI bridge.

d92a208d08 powerpc/pci: Mask linkDown on resetting PCI bus
The problem was initially reported by Wendy who tried pass through
IPR adapter, which was connected to PHB root port directly, to KVM
based guest. When doing that, pci_reset_bridge_secondary_bus() was
called by VFIO driver and linkDown was detected by the root port.
That caused all PEs to be frozen.

The patch fixes the issue by routing the reset for the secondary bus
of root port to underly firmware. For that, one more weak function
pci_reset_secondary_bus() is introduced so that the individual platforms
can override that and do specific reset for bridge's secondary bus.

26833a5029 powerpc/eeh: Make the delay for PE reset unified
Basically, we have 3 types of resets to fulfil PE reset: fundamental,
hot and PHB reset. For the later 2 cases, we need PCI bus reset hold
and settlement delay as specified by PCI spec. PowerNV and pSeries
platforms are running on top of different firmware and some of the
delays have been covered by underly firmware (PowerNV).

The patch makes the delays unified to be done in backend, instead of
EEH core.

d2b0f6f77e powerpc/eeh: No hotplug on permanently removed dev
The issue was detected in a bit complicated test case where
we have multiple hierarchical PEs shown as following figure:

PE#4 (have 2 PCI devices) is the child of PE#3, which has 2 p2p
bridges. We accidentally had less-known scenario: PE#4 was removed
permanently from the system because of permanent failure (e.g.
exceeding the max allowd failure times in last hour), then we detects
EEH errors on PE#3 and tried to recover it. However, eeh_dev instances
for pdev#0/1 were not detached from PE#4, which was still connected to
PE#3. All of that was because of the fact that we rely on count-based
pcibios_release_device(), which isn't reliable enough. When doing
recovery for PE#3, we still apply hotplug on PE#4 and pdev#0/1, which
are not valid any more. Eventually, we run into kernel crash.

The patch fixes above issue from two aspects. For unplug, we simply
skip those permanently removed PE, whose state is (EEH_PE_STATE_ISOLATED
&& !EEH_PE_STATE_RECOVERING) and its frozen count should be greater
than EEH_MAX_ALLOWED_FREEZES. For plug, we marked all permanently
removed EEH devices with EEH_DEV_REMOVED and return 0xFF's on read
its PCI config so that PCI core will omit them.

8a5ad35686 powerpc/eeh: Cleanup EEH subsystem variables
There're 2 EEH subsystem variables: eeh_subsystem_enabled and
eeh_probe_mode. We needn't maintain 2 variables and we can just
have one variable and introduce different flags. The patch also
introduces additional flag EEH_FORCE_DISABLE, which will be used
to disable EEH subsystem via boot parameter ("eeh=off") in future.
Besides, the patch also introduces flag EEH_ENABLED, which is
changed to disable or enable EEH functionality on the fly through
debugfs entry in future.

With the patch applied, the creteria to check the enabled EEH
functionality is changed to:

!EEH_FORCE_DISABLED && EEH_ENABLED : Enabled
                       Other cases : Disabled

When calling into eeh_gather_pci_data() on pSeries platform, we
possiblly don't have pci_dev instance yet, but eeh_dev is always
ready. So we use cached capability from eeh_dev instead of pci_dev
for log dump there. In order to keep things unified, we also cache
PCI capability positions to eeh_dev for PowerNV as well.

d0914f503f powerpc/eeh: Block PCI-CFG access during PE reset
We've observed multiple PE reset failures because of PCI-CFG
access during that period. Potentially, some device drivers
can't support EEH very well and they can't put the device to
motionless state before PE reset. So those device drivers might
produce PCI-CFG accesses during PE reset. Also, we could have
PCI-CFG access from user space (e.g. "lspci"). Since access to
frozen PE should return 0xFF's, we can block PCI-CFG access
during the period of PE reset so that we won't get recrusive EEH
errors.

The patch adds flag EEH_PE_RESET, which is kept during PE reset.
The PowerNV/pSeries PCI-CFG accessors reuse the flag to block
PCI-CFG accordingly.

9e04937560 powerpc/eeh: Remove EEH_PE_PHB_DEAD
The PE state (for eeh_pe instance) EEH_PE_PHB_DEAD is duplicate to
EEH_PE_ISOLATED. Originally, those PHBs (PHB PE) with EEH_PE_PHB_DEAD
would be removed from the system. However, it's safe to replace
that with EEH_PE_ISOLATED.

The patch also clear EEH_PE_RECOVERING after fenced PHB has been handled,
either failure or success. It makes the PHB PE state consistent with:

	PHB functions normally		  NONE
	PHB has been removed		  EEH_PE_ISOLATED
	PHB fenced, recovery in progress  EEH_PE_ISOLATED | RECOVERING

3441f04b4b powerpc/powernv: Create OPAL sglist helper functions and fix endian issues
We have two copies of code that creates an OPAL sg list. Consolidate
these into a common set of helpers and fix the endian issues.

The flash interface embedded a version number in the num_entries
field, whereas the dump interface did did not. Since versioning
wasn't added to the flash interface and it is impossible to add
this in a backwards compatible way, just remove it.

b3d627a5f2 cpufreq: powernv: cpufreq driver for powernv platform
Backend driver to dynamically set voltage and frequency on
IBM POWER non-virtualized platforms.  Power management SPRs
are used to set the required PState.

This driver works in conjunction with cpufreq governors
like 'ondemand' to provide a demand based frequency and
voltage setting on IBM POWER non-virtualized platforms.

PState table is obtained from OPAL v3 firmware through device
tree.

powernv_cpufreq back-end driver would parse the relevant device-tree
nodes and initialise the cpufreq subsystem on powernv platform.

The code was originally written by svaidy@linux.vnet.ibm.com. Over
time it was modified to accomodate bug-fixes as well as updates to the
the cpu-freq core. Relevant portions of the change logs corresponding
to those modifications are noted below:

 * The policy->cpus needs to be populated in a hotplug-invariant
   manner instead of using cpu_sibling_mask() which varies with
   cpu-hotplug. This is because the cpufreq core code copies this
   content into policy->related_cpus mask which should not vary on
   cpu-hotplug. [Authored by srivatsa.bhat@linux.vnet.ibm.com]

 * Create a helper routine that can return the cpu-frequency for the
   corresponding pstate_id. Also, cache the values of the pstate_max,
   pstate_min and pstate_nominal and nr_pstates in a static structure
   so that they can be reused in the future to perform any

 * Create a driver attribute named cpuinfo_nominal_freq which creates
   a sysfs read-only file named cpuinfo_nominal_freq. Export the
   frequency corresponding to the nominal_pstate through this
   interface.

     Nominal frequency is the highest non-turbo frequency for the
   platform.  This is generally used for setting governor policies
   from user space for optimal energy efficiency. [Authored by

a08a53ea4c powerpc/le: Enable RTAS events support
The current kernel code assumes big endian and parses RTAS events all
wrong. The most visible effect is that we cannot honor EPOW events,
meaning, for example, we cannot shut down a guest properly from the
hypervisor.

This new patch is largely inspired by Nathan's work: we get rid of all
the bit fields in the RTAS event structures (even the unused ones, for
consistency). We also introduce endian safe accessors for the fields used
by the kernel (trivial rtas_error_type() accessor added for consistency).

72cde5a88d KVM: PPC: Book3S HV: Save/restore host PMU registers that are new in POWER8
Currently we save the host PMU configuration, counter values, etc.,
when entering a guest, and restore it on return from the guest.
(We have to do this because the guest has control of the PMU while
it is executing.)  However, we missed saving/restoring the SIAR and
SDAR registers, as well as the registers which are new on POWER8,
namely SIER and MMCR2.

This adds code to save the values of these registers when entering
the guest and restore them on exit.  This also works around the bug
in POWER8 where setting PMAE with a counter already negative doesn't
generate an interrupt.

797f9c07eb KVM: PPC: Book3S HV: Don't use kvm_memslots() in real mode
With HV KVM, some high-frequency hypercalls such as H_ENTER are handled
in real mode, and need to access the memslots array for the guest.
Accessing the memslots array is safe, because we hold the SRCU read
lock for the whole time that a guest vcpu is running.  However, the
checks that kvm_memslots() does when lockdep is enabled are potentially
unsafe in real mode, when only the linear mapping is available.
Furthermore, kvm_memslots() can be called from a secondary CPU thread,
which is an offline CPU from the point of view of the host kernel,
and is not running the task which holds the SRCU read lock.

To avoid false positives in the checks in kvm_memslots(), and to avoid
possible side effects from doing the checks in real mode, this replaces
kvm_memslots() with kvm_memslots_raw() in all the places that execute
in real mode.  kvm_memslots_raw() is a new function that is like
kvm_memslots() but uses rcu_dereference_raw_notrace() instead of
kvm_dereference_check().

e4e3812150 KVM: PPC: Book3S HV: Add transactional memory support
This adds saving of the transactional memory (TM) checkpointed state
on guest entry and exit.  We only do this if we see that the guest has
an active transaction.

It also adds emulation of the TM state changes when delivering IRQs
into the guest.  According to the architecture, if we are
transactional when an IRQ occurs, the TM state is changed to
suspended, otherwise it's left unchanged.

e59d24e612 KVM: PPC: Book3S HV: Fix incorrect userspace exit on ioeventfd write
When the guest does an MMIO write which is handled successfully by an
ioeventfd, ioeventfd_write() returns 0 (success) and
kvmppc_handle_store() returns EMULATE_DONE.  Then
kvmppc_emulate_mmio() converts EMULATE_DONE to RESUME_GUEST_NV and
this causes an exit from the loop in kvmppc_vcpu_run_hv(), causing an
exit back to userspace with a bogus exit reason code, typically
causing userspace (e.g. qemu) to crash with a message about an unknown
exit code.

This adds handling of RESUME_GUEST_NV in kvmppc_vcpu_run_hv() in order
to fix that.  For generality, we define a helper to check for either
of the return-to-guest codes we use, RESUME_GUEST and RESUME_GUEST_NV,
to make it easy to check for either and provide one place to update if
any other return-to-guest code gets defined in future.

Since it only affects Book3S HV for now, the helper is added to
the kvm_book3s.h header file.

We use the helper in two places in kvmppc_run_core() as well for
future-proofing, though we don't see RESUME_GUEST_NV in either place
at present.

The previous commit added constraint and register handling to allow
processes using EBB (Event Based Branches) to request access to the BHRB
(Branch History Rolling Buffer).

With that in place we can allow processes using EBB to access the BHRB.
This is achieved by setting BHRBA in MMCR0 when we enable EBB access. We
must also clear BHRBA when we are disabling.

Some power8 revisions have a hardware bug where we can lose a
Performance Monitor (PMU) exception under certain circumstances.

We will be adding a workaround for this case, see the next commit for
details. The observed behaviour is that writing PMAO doesn't cause an
exception as we would expect, hence the name of the feature.

d20f78d252 x86, random: Enable the RDSEED instruction
Upcoming Intel silicon adds a new RDSEED instruction, which is similar
to RDRAND but provides a stronger guarantee: unlike RDRAND, RDSEED
will always reseed the PRNG from the true random number source between
each read.  Thus, the output of RDSEED is guaranteed to be 100%
entropic, unlike RDRAND which is only architecturally guaranteed to be
1/512 entropic (although in practice is much more.)

The RDSEED instruction takes the same time to execute as RDRAND, but
RDSEED unlike RDRAND can legitimately return failure (CF=0) due to
entropy exhaustion if too many threads on too many cores are hammering
the RDSEED instruction at the same time.  Therefore, we have to be
more conservative and only use it in places where we can tolerate
failures.

This patch introduces the primitives arch_get_random_seed_{int,long}()
but does not use it yet.

While bolted handlers (including e6500) do not need to deal with a TLB
miss recursively causing another TLB miss, nested TLB misses can still
happen with crit/mc/debug exceptions -- so we still need to honor
SPRG_TLB_EXFRAME.

We don't need to spend time modifying it in the TLB miss fastpath,
though -- the special level exception will handle that.

9d378dfac8 powerpc/booke64: Use SPRG7 for VDSO
Previously SPRG3 was marked for use by both VDSO and critical
interrupts (though critical interrupts were not fully implemented).

In commit 8b64a9dfb0 ("powerpc/booke64:
Use SPRG0/3 scratch for bolted TLB miss & crit int"), Mihai Caraman
made an attempt to resolve this conflict by restoring the VDSO value
early in the critical interrupt, but this has some issues:

 - It's incompatible with EXCEPTION_COMMON which restores r13 from the
   by-then-overwritten scratch (this cost me some debugging time).
 - It forces critical exceptions to be a special case handled
   differently from even machine check and debug level exceptions.
 - It didn't occur to me that it was possible to make this work at all
   (by doing a final "ld r13, PACA_EXCRIT+EX_R13(r13)") until after
   I made (most of) this patch. :-)

It might be worth investigating using a load rather than SPRG on return
from all exceptions (except TLB misses where the scratch never leaves
the SPRG) -- it could save a few cycles.  Until then, let's stick with
SPRG for all exceptions.

Since we cannot use SPRG4-7 for scratch without corrupting the state of
a KVM guest, move VDSO to SPRG7 on book3e.  Since neither SPRG4-7 nor
critical interrupts exist on book3s, SPRG3 is still used for VDSO
there.

Once special level interrupts are supported, we may take nested TLB
misses -- so allow the same thread to acquire the lock recursively.

The lock will not be effective against the nested TLB miss handler
trying to write the same entry as the interrupted TLB miss handler, but
that's also a problem on non-threaded CPUs that lack TLB write
conditional.  This will be addressed in the patch that enables crit/mc
support by invalidating the TLB on return from level exceptions.

This enables support for userspace to fetch and initiate FSP and
Platform dumps from the service processor (via firmware) through sysfs.

Based on original patch from Vasant Hegde <hegdevasant@linux.vnet.ibm.com>

Flow:
  - We register for OPAL notification events.
  - OPAL sends new dump available notification.
  - We make information on dump available via sysfs
  - Userspace requests dump contents
  - We retrieve the dump via OPAL interface
  - User copies the dump data
  - userspace sends ack for dump
  - We send ACK to OPAL.

sysfs files:
  - We add the /sys/firmware/opal/dump directory
  - echoing 1 (well, anything, but in future we may support
    different dump types) to /sys/firmware/opal/dump/initiate_dump
    will initiate a dump.
  - Each dump that we've been notified of gets a directory
    in /sys/firmware/opal/dump/ with a name of the dump type and ID (in hex,
    as this is what's used elsewhere to identify the dump).
  - Each dump has files: id, type, dump and acknowledge
    dump is binary and is the dump itself.
    echoing 'ack' to acknowledge (currently any string will do) will
    acknowledge the dump and it will soon after disappear from sysfs.

OPAL APIs:
  - opal_dump_init()
  - opal_dump_info()
  - opal_dump_read()
  - opal_dump_ack()
  - opal_dump_resend_notification()

Currently we are only ever notified for one dump at a time (until
the user explicitly acks the current dump, then we get a notification
of the next dump), but this kernel code should "just work" when OPAL
starts notifying us of all the dumps present.

774fea1a38 powerpc/powernv: Read OPAL error log and export it through sysfs
Based on a patch by: Mahesh Salgaonkar <mahesh@linux.vnet.ibm.com>

This patch adds support to read error logs from OPAL and export
them to userspace through a sysfs interface.

We export each log entry as a directory in /sys/firmware/opal/elog/

Currently, OPAL will buffer up to 128 error log records, we don't
need to have any knowledge of this limit on the Linux side as that
is actually largely transparent to us.

Each error log entry has the following files: id, type, acknowledge, raw.
Currently we just export the raw binary error log in the 'raw' attribute.
In a future patch, we may parse more of the error log to make it a bit
easier for userspace (e.g. to be able to display a brief summary in
petitboot without having to have a full parser).

If we have >128 logs from OPAL, we'll only be notified of 128 until
userspace starts acknowledging them. This limitation may be lifted in
the future and with this patch, that should "just work" from the linux side.

A userspace daemon should:
- wait for error log entries using normal mechanisms (we announce creation)
- read error log entry
- save error log entry safely to disk
- acknowledge the error log entry
- rinse, repeat.

On the Linux side, we read the error log when we're notified of it. This
possibly isn't ideal as it would be better to only read them on-demand.
However, this doesn't really work with current OPAL interface, so we
read the error log immediately when notified at the moment.

I've tested this pretty extensively and am rather confident that the
linux side of things works rather well. There is currently an issue with
the service processor side of things for >128 error logs though.

6b36ba8492 powerpc/pseries: Update dynamic cache nodes for suspend/resume operation
pHyp can change cache nodes for suspend/resume operation. Currently the
device tree is updated by drmgr in userspace after all non boot CPUs are
enabled. Hence, we do not modify the cache list based on the latest cache
nodes. Also we do not remove cache entries for the primary CPU.

This patch removes the cache list for the boot CPU, updates the device tree
before enabling nonboot CPUs and adds cache list for the boot cpu.

This patch also has the side effect that older versions of drmgr will
perform a second device tree update from userspace. While this is a
redundant waste of a couple cycles it is harmless since firmware returns the
same data for the subsequent update-nodes/properties rtas calls.

9ac8cde938 powerpc/pseries: Use remove_memory() to remove memory
The memory remove code for powerpc/pseries should call remove_memory()
so that we are holding the hotplug_memory lock during memory remove
operations.

This patch updates the memory node remove handler to call remove_memory()
and adds a ppc_md.remove_memory() entry to handle pseries specific work
that is called from arch_remove_memory().

During memory remove in pseries_remove_memblock() we have to stay with
removing memory one section at a time. This is needed because of how memory
resources are handled. During memory add for pseries (via the probe file in
sysfs) we add memory one section at a time which gives us a memory resource
for each section. Future patches will aim to address this so will not have
to remove memory one section at a time.

Detect and recover from machine check when inside opal on a special
scom load instructions. On specific SCOM read via MMIO we may get a machine
check exception with SRR0 pointing inside opal. To recover from MC
in this scenario, get a recovery instruction address and return to it from
MC.

Each recoverable address range entry is (start address, len,
recovery address), 2 cells each for start and recovery address, 1 cell for
len, totalling 5 cells per entry. During kernel boot time, build up the
recovery table with the list of recovery ranges from device-tree node which
will be used during machine check exception to recover from MMIO SCOM UE.

During "Fast-sleep" and deeper power savings state, decrementer and
timebase could be stopped making it out of sync with rest
of the cores in the system.

Add a firmware call to request platform to resync timebase
using low level platform methods.

aca79d2b6e powerpc/powernv: Add context management for Fast Sleep
Before adding Fast-Sleep into the cpuidle framework, some low level
support needs to be added to enable it. This includes saving and
restoring of certain registers at entry and exit time of this state
respectively just like we do in the NAP idle state.

1b67bee129 powerpc: Implement tick broadcast IPI as a fixed IPI message
For scalability and performance reasons, we want the tick broadcast IPIs
to be handled as efficiently as possible. Fixed IPI messages
are one of the most efficient mechanisms available - they are faster than
the smp_call_function mechanism because the IPI handlers are fixed and hence
they don't involve costly operations such as adding IPI handlers to the target
CPU's function queue, acquiring locks for synchronization etc.

Luckily we have an unused IPI message slot, so use that to implement
tick broadcast IPIs efficiently.

402d9a1e02 powerpc: Free up the slot of PPC_MSG_CALL_FUNC_SINGLE IPI message
The IPI handlers for both PPC_MSG_CALL_FUNC and PPC_MSG_CALL_FUNC_SINGLE map
to a common implementation - generic_smp_call_function_single_interrupt(). So,
we can consolidate them and save one of the IPI message slots, (which are
precious on powerpc, since only 4 of those slots are available).

So, implement the functionality of PPC_MSG_CALL_FUNC_SINGLE using
PPC_MSG_CALL_FUNC itself and release its IPI message slot, so that it can be
used for something else in the future, if desired.

573ebfa660 powerpc: Increase stack redzone for 64-bit userspace to 512 bytes
The new ELFv2 little-endian ABI increases the stack redzone -- the
area below the stack pointer that can be used for storing data --
from 288 bytes to 512 bytes.  This means that we need to allow more
space on the user stack when delivering a signal to a 64-bit process.

To make the code a bit clearer, we define new USER_REDZONE_SIZE and
KERNEL_REDZONE_SIZE symbols in ptrace.h.  For now, we leave the
kernel redzone size at 288 bytes, since increasing it to 512 bytes
would increase the size of interrupt stack frames correspondingly.

Gcc currently only makes use of 288 bytes of redzone even when
compiling for the new little-endian ABI, and the kernel cannot
currently be compiled with the new ABI anyway.

In the future, hopefully gcc will provide an option to control the
amount of redzone used, and then we could reduce it even more.

This also changes the code in arch_compat_alloc_user_space() to
preserve the expanded redzone.  It is not clear why this function would
ever be used on a 64-bit process, though.

d2ae2e20fb driver/memory:Move Freescale IFC driver to a common driver
Freescale IFC controller has been used for mpc8xxx. It will be used
 for ARM-based SoC as well. This patch moves the driver to driver/memory
 and fix the header file includes.

  Also remove module_platform_driver() and  instead call
  platform_driver_register() from subsys_initcall() to make sure this module
  has been loaded before MTD partition parsing starts.

a0a4419e30 powerpc: Link VDSOs at 0x0
perf is failing to resolve symbols in the VDSO. A while (1)
gettimeofday() loop shows:

The reason for this is that we are linking our VDSO shared libraries
at 1MB, which is a little weird. Even though this is uncommon, Alan
points out that it is valid and we should probably fix perf userspace.

Regardless, I can't see a reason why we are doing this. The code
is all position independent and we never rely on the VDSO ending
up at 1M (and we never place it there on 64bit tasks).

Changing our link address to 0x0 fixes perf VDSO symbol resolution:

56eecdb912 mm: Use ptep/pmdp_set_numa() for updating _PAGE_NUMA bit
Archs like ppc64 doesn't do tlb flush in set_pte/pmd functions when using
a hash table MMU for various reasons (the flush is handled as part of
the PTE modification when necessary).

ppc64 thus doesn't implement flush_tlb_range for hash based MMUs.

Additionally ppc64 require the tlb flushing to be batched within ptl locks.

The reason to do that is to ensure that the hash page table is in sync with
linux page table.

We track the hpte index in linux pte and if we clear them without flushing
hash and drop the ptl lock, we can have another cpu update the pte and can
end up with duplicate entry in the hash table, which is fatal.

We also want to keep set_pte_at simpler by not requiring them to do hash
flush for performance reason. We do that by assuming that set_pte_at() is
never *ever* called on a PTE that is already valid.

This was the case until the NUMA code went in which broke that assumption.

Fix that by introducing a new pair of helpers to set _PAGE_NUMA in a
way similar to ptep/pmdp_set_wrprotect(), with a generic implementation
using set_pte_at() and a powerpc specific one using the appropriate
mechanism needed to keep the hash table in sync.
