When trying to display tgt_counters in the debugfs, a panic can result.
There is no null check for qpair after it is assigned in the for-loop.
Unless vha->hw->queue_pair_map array is completely filled with entries, the
system will panic dereferencing a null pointer.

Current code hard codes marker request to use request and response queue

This patch fixes LUN discovery when loop ID is not yet assigned by the

The null check on pointer sess and the subsequent call is redundant as sess
is null on all the the paths that lead to the out_term2 label. Hence the
null check and the call can be removed. Also remove the redundant setting
of sess to NULL as this is not required now.

When sending an srb with qla2x00_start_sp, the sp can complete and be freed
by the time we log the debug message saying we sent it. This can cause a
panic if sp gets reused quickly or when running a kernel that poisons freed
memory.

For eh_bus_reset, driver is supposed to reset the link. Current option to
reset the link is applicable to Loop only. This patch updates current FW
option with the one that is applicable to all topologies.

When loop was made down explicitly due to cable pull, then for N2N toplogy,
if FAWWPN BIT is enabled by user, then it would restore some default
(garbage) value for Physical port WWPN, so this show garbage WWPN for the
port. Fix is, to restore physical port WWPN, if it is fabric
configuration. When loop is explicitly made down, and FAWWPN feature is
enabled, then driver need to restore original flashed WWPN.

On Transmit respond in target mode, if the chip is already reset or the
session is already deleted, then advance the command to the free step.
There is no need to abort the command, because the chip has already flushed

On session cleanup, either an implicit LOGO or an implicit PRLO is used to
flush IOs. If the flush command hit Queue Full condition, then it is
dropped. This patch adds retry code to prevent command drop.

This patch provide call back functions to stop the chip and resume the chip
if the PCI lower level driver wants to perform function level reset/FLR.
Before the FLR, the chip will be stopped to turn off all DMA activity.
After the FLR, the chip is reset to resume previous operation state.

When SGE buffer containing DIF information crosses 4G boundary, it results
in DMA error. This patch fixes this issue by calculating SGE buffer size
and if it crosses 4G boundary, driver will split it into multiple SGE
buffers to avoid DMA error.

The intent of invoking configfs_depend_item in commit 7474f52a82
("tcm_qla2xxx: Perform configfs depend/undepend for base_tpg")
was to prevent a physical Fibre Channel port removal when
virtual (NPIV) ports announced through that physical port are active.
The change does not work as expected: it makes enabled physical port
dependent on target configfs subsystem (the port's parent), something
the configfs guarantees anyway.
Besides, scheduling work in a worker thread and waiting for the work's
completion is not really a valid workaround for the requirement not to call
configfs_depend_item from a configfs callback: the call occasionally
deadlocks.
Thus, removing configfs_depend_item calls does not break anything and fixes
the deadlock problem.

Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page. Remove the
ENABLE_CLUSTERING define, and require drivers to explicitly set
DISABLE_CLUSTERING to disable this feature.

iscsi_target_mod is the only LIO fabric where fabric_ops.name differs from
the fabric_ops.fabric_name string. fabric_ops.name is used when matching
target/$fabric ConfigFS create paths, so rename it .fabric_alias and
fallback to target/$fabric vs .fabric_name comparison if .fabric_alias
isn't initialised. iscsi_target_mod is the only fabric module to set
.fabric_alias . All other fabric modules rely on .fabric_name matching and
can drop the duplicate string.

All fabrics return a const string. In all cases *except* iSCSI the
get_fabric_name() string matches fabric_ops.name.
Both fabric_ops.get_fabric_name() and fabric_ops.name are user-facing, with
the former being used for PR/ALUA state and the latter for ConfigFS
(config/target/$name), so we unfortunately need to keep both strings around
for now. Replace the useless .get_fabric_name() accessor function with a
const string fabric_name member variable.

It's specifically looking for a given request, which we will not be
supporting going forward. Also kill the qla2xxx poll implementation
as that's the only user of the nvme-fc poll, and the now unused
->poll_queue() hook.

Merge tag 'v4.20-rc3' into for-4.21/block
Merge in -rc3 to resolve a few conflicts, but also to get a few
