Currently the radix TLB code includes support for CPUs that do *not*
have MMU_FTR_LOCKLESS_TLBIE. On those CPUs we are required to take a
global spinlock before issuing a tlbie.
Radix can only be built for 64-bit Book3s CPUs, and of those, only
POWER4, 970, Cell and PA6T do not have MMU_FTR_LOCKLESS_TLBIE. Although
it's possible to build a kernel with Radix support that can also boot on
those CPUs, we happen to know that in reality none of those CPUs support
the Radix MMU, so the code can never actually run on those CPUs.
So remove the native_tlbie_lock in the Radix TLB code.
Note that there is another lock of the same name in the hash code, which
is unaffected by this patch.

_tlbiel_pid() is called with a ric (Radix Invalidation Control) argument of
either RIC_FLUSH_TLB or RIC_FLUSH_ALL.
RIC_FLUSH_ALL says to invalidate the entire TLB and the Page Walk Cache (PWC).
To flush the whole TLB, we have to iterate over each set (congruence class) of
the TLB. Currently we do that and pass RIC_FLUSH_ALL each time. That is not
incorrect but it means we flush the PWC 128 times, when once would suffice.
Fix it by doing the first flush with the ric value we're passed, and then if it
was RIC_FLUSH_ALL, we downgrade it to RIC_FLUSH_TLB, because we know we have
just flushed the PWC and don't need to do it again.

Currently we implement flushing of the page walk cache (PWC) by calling
_tlbiel_pid() with a RIC (Radix Invalidation Control) value of 1 which says to
only flush the PWC.
But _tlbiel_pid() loops over each set (congruence class) of the TLB, which is
not necessary when we're just flushing the PWC.
In fact the set argument is ignored for a PWC flush, so essentially we're just
flushing the PWC 127 extra times for no benefit.
Fix it by adding tlbiel_pwc() which just does a single flush of the PWC.

For a tlbiel with pid, we need to issue tlbiel with set number encoded. We
don't need to do ptesync for each of those. Instead we need one for the entire
tlbiel pid operation.

For fullmm tlb flush, we do a flush with RIC_FLUSH_ALL which will invalidate all
related caches (radix__tlb_flush()). Hence the pwc flush is not needed.

Three tiny changes to the ERAT flushing logic: First don't make
it depend on DD1. It hasn't been decided yet but we might run
DD2 in a mode that also requires explicit flushes for performance
reasons so make it unconditional. We also add a missing isync, and
finally remove the flush from _tlbiel_va as it is only necessary
for congruence-class invalidations (PID, LPID and full TLB), not
targetted invalidations.

Now that we have page size details encoded in pte using software pte
bits, use that to find the page size needed for tlb flush.
This function should only be used on P9 DD1, so give it a horrible name
to make that clear.

Before this patch, we used tlbiel, if we ever ran only on this core.
That was mostly derived from the nohash usage of the same. But is
incorrect, the ISA 3.0 clarifies tlbiel such that:
"All TLB entries that have all of the following properties are made
invalid on the thread executing the tlbiel instruction"
ie. tlbiel only invalidates TLB entries on the current thread. So if the
mm has been used on any other thread (aka. cpu) then we must broadcast
the invalidate.
This bug could lead to invalid TLB entries if a program runs on multiple
threads of a core.
Hence use tlbiel, if we only ever ran on only the current cpu.

Even though a tlb_flush() does a flush with invalidate all cache,
we can end up doing an RCU page table free before calling tlb_flush().
That means we can have page walk cache entries even after we free the
page table pages. This can result in us doing wrong page table walk.
Avoid this by doing pwc flush on every page table free. We can't batch
the pwc flush, because the rcu call back function where we free the
page table pages doesn't have information of the mmu gather. Thus we
have to do a pwc on every page table page freed.
Note: I also removed the dummy tlb_flush_pgtable call functions for
hash 32.

Radix invalidate control (RIC) is used to control which cache to flush
using tlb instructions. When doing a PID flush, we currently flush
everything including page walk cache. For address range flush, we flush
only the TLB. In the next patch, we add support for flushing only the
page walk cache.

In some of the radix TLB flush routines, we use a local to store the
mm->context.id, AKA the PID.
Currently we use an int, but the PID is unsigned long, so large values
of PID will be truncated. In particular MMU_NO_CONTEXT is -1, which
means all our comparisons against that value can never be true.
This means we'll issue TLB flushes when we shouldn't on radix enabled
machines.
Fix it by using an unsigned long for the local. Discovered by Coverity.

Core kernel doesn't track the page size of the VA range that we are
invalidating. Hence we end up flushing TLB for the entire mm here. Later
patches will improve this.
We also don't flush page walk cache separetly instead use RIC=2 when
flushing TLB, because we do a MMU gather flush after freeing page table.
MMU_NO_CONTEXT is updated for hash.
