The sbitmap and the percpu_ida perform essentially the same task,
allocating tags for commands. The sbitmap outperforms the percpu_ida as
documented here: https://lkml.org/lkml/2014/4/22/553
The sbitmap interface is a little harder to use, but being able to remove
the percpu_ida code and getting better performance justifies the additional
complexity.

In commit d2ba5675d8 ("[SCSI] qla2xxx: Disable local-interrupts while
polling for RISC status.") added a local_irq_disable() before invoking the
->intr_handler callback. The function, which was used in this callback, did
not disable interrupts while acquiring the spin_lock so a deadlock was
possible and this change was one possible solution.
The function in question was qla2300_intr_handler() and is using
spin_lock_irqsave() since commit 43fac4d97a ("[SCSI] qla2xxx: Resolve a
performance issue in interrupt"). I checked all other ->intr_handler
callbacks and all of them use the irqsave variant so it is safe to remove
the local_irq_save() block now.

Some newer target uses "Status Qualifier" response in a returned "Busy
Status". This new response code of 0x4001, which is "Scope" bits,
translates to "Affects all units accessible by target". Due to this new
value returned in the Scope bits, driver was using that value as timeout
value which resulted into driver waiting for 27min timeout.
This patch masks off this Scope bits so that driver does not use this
value as retry delay time.

means the timeout function pointer and any data that the function depends on

cases qla2x00_init_timer() initialises a completion structure needed by the

The code that fixes the crashes in the following commit introduced a small
memory leak:
commit 6a2cf8d366 ("scsi: qla2xxx: Fix crashes in qla2x00_probe_one on probe failure")
Fixing this requires a bit of reworking, which I've explained. Also provide
some code cleanup.
There is a small window in qla2x00_probe_one where if qla2x00_alloc_queues
fails, we end up never freeing req and rsp and leak 0xc0 and 0xc8 bytes
respectively (the sizes of req and rsp).
I originally put in checks to test for this condition which were based on
the incorrect assumption that if ha->rsp_q_map and ha->req_q_map were
allocated, then rsp and req were allocated as well. This is incorrect.
There is a window between these allocations:
ret = qla2x00_mem_alloc(ha, req_length, rsp_length, &req, &rsp);
goto probe_hw_failed;
[if successful, both rsp and req allocated]
base_vha = qla2x00_create_host(sht, ha);
goto probe_hw_failed;
ret = qla2x00_request_irqs(ha, rsp);
goto probe_failed;
if (qla2x00_alloc_queues(ha, req, rsp)) {
goto probe_failed;
[if successful, now ha->rsp_q_map and ha->req_q_map allocated]
To simplify this, we should just set req and rsp to NULL after we free
them. Sounds simple enough? The problem is that req and rsp are pointers
defined in the qla2x00_probe_one and they are not always passed by reference
to the routines that free them.
Here are paths which can free req and rsp:
PATH 1:
qla2x00_probe_one
ret = qla2x00_mem_alloc(ha, req_length, rsp_length, &req, &rsp);
[req and rsp are passed by reference, but if this fails, we currently
do not NULL out req and rsp. Easily fixed]
PATH 2:
qla2x00_probe_one
failing in qla2x00_request_irqs or qla2x00_alloc_queues
probe_failed:
qla2x00_free_device(base_vha);
qla2x00_free_req_que(ha, req)
qla2x00_free_rsp_que(ha, rsp)
PATH 3:
qla2x00_probe_one:
failing in qla2x00_mem_alloc or qla2x00_create_host
probe_hw_failed:
qla2x00_free_req_que(ha, req)
qla2x00_free_rsp_que(ha, rsp)
PATH 1: This should currently work, but it doesn't because rsp and rsp are
not set to NULL in qla2x00_mem_alloc. Easily remedied.
PATH 2: req and rsp aren't passed in at all to qla2x00_free_device but are
derived from ha->req_q_map[0] and ha->rsp_q_map[0]. These are only set up if
qla2x00_alloc_queues succeeds.
In qla2x00_free_queues, we are protected from crashing if these don't exist
because req_qid_map and rsp_qid_map are only set on their allocation. We are
guarded in this way:
for (cnt = 0; cnt < ha->max_req_queues; cnt++) {
if (!test_bit(cnt, ha->req_qid_map))
continue;
PATH 3: This works. We haven't freed req or rsp yet (or they were never
allocated if qla2x00_mem_alloc failed), so we'll attempt to free them here.
To summarize, there are a few small changes to make this work correctly and
(and for some cleanup):
1) (For PATH 1) Set *rsp and *req to NULL in case of failure in
qla2x00_mem_alloc so these are correctly set to NULL back in
qla2x00_probe_one
2) After jumping to probe_failed: and calling qla2x00_free_device,
explicitly set rsp and req to NULL so further calls with these pointers do
not crash, i.e. the free queue calls in the probe_hw_failed section we fall
through to.
3) Fix return code check in the call to qla2x00_alloc_queues. We currently
drop the return code on the floor. The probe fails but the caller of the
probe doesn't have an error code, so it attaches to pci. This can result in
a crash on module shutdown.
4) Remove unnecessary NULL checks in qla2x00_free_req_que,
qla2x00_free_rsp_que, and the egregious NULL checks before kfrees and vfrees
in qla2x00_mem_free.
I tested this out running a scenario where the card breaks at various times
during initialization. I made sure I forced every error exit path in
qla2x00_probe_one.

Because of the shifting around of code in qla2x00_probe_one recently,
failures during adapter initialization can lead to problems, i.e. NULL

This V2 version makes the relevant memory free routines idempotent, so
repeat calls won't cause any harm. I also removed the problematic

This patch is based on Max's original patch.
When the qla2xxx firmware is unavailable, eventually
qla2x00_sp_timeout() is reached, which calls the timeout function and
frees the srb_t instance.
The timeout function always resolves to qla2x00_async_iocb_timeout(),
which invokes another callback function called "done". All of these
qla2x00_*_sp_done() callbacks also free the srb_t instance; after
returning to qla2x00_sp_timeout(), it is freed again.
The fix is to remove the "sp->free(sp)" call from qla2x00_sp_timeout()
and add it to those code paths in qla2x00_async_iocb_timeout() which
do not already free the object.
This is how it looks like with KASAN:

When connection type is N_Port to N_Port (point-to-point), there
is a possibilty where initiator will not send PLOGI request and
will directly send PRLI. In N2N connection the port has higher
port name sends the PLOGI but not allow to send PRLI if is a
target mode. Only initiator is allowed to send PRLI.
Current driver code deletes old session when it receives PLOGI
request. If we will not receive PLOGI request then we will not
delete old session and create new session. Add check for N2N
with PRLI receive only and trigger cleanup. For this case, the
cleanup requires individual cmd abort instead of using implicit

When driver is loaded in Target/Dual mode, it creates QPair to support
MQ and allocates resources for each QPair. This Qpair initialization is
delayed until the FW personality is changed to Dual/Target mode by
issuing chip reset. At the time of chip reset firmware is re-initilized
in correct personality all the QPairs are initialized by sending
MBC_INITIALIZE_MULTIQ (001Fh).
This patch fixes memory leak by adding check to issue
MBC_INITIALIZE_MULTIQ command only while deleting rsp/req queue when the
flag is set for initiator mode, and clean up QPair resources correctly
during the driver unload. This MBX does not need to be issued for
Target/Dual mode because chip reset will reset ISP.

- More year 2038 work from Arnd slowly reaching the point where we
need to think about the syscalls themself.
- A new timer function which allows to conditionally (re)arm a timer
only when it's either not running or the new expiry time is sooner
than the armed expiry time. This allows to use a single timer for
multiple timeout requirements w/o caring about the first expiry
time at the call site.
- A new NMI safe accessor to clock real time for the printk timestamp
work. Can be used by tracing, perf as well if required.
- A large number of timer setup conversions from Kees which got
collected here because either maintainers requested so or they
simply got ignored. As Kees pointed out already there are a few
trivial merge conflicts and some redundant commits which was
unavoidable due to the size of this conversion effort.
- Avoid a redundant iteration in the timer wheel softirq processing.
- Provide a mechanism to treat RTC implementations depending on their
hardware properties, i.e. don't inflict the write at the 0.5
seconds boundary which originates from the PC CMOS RTC to all RTCs.
No functional change as drivers need to be updated separately.
- The usual small updates to core code clocksource drivers. Nothing
really exciting"

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:
SPDX license identifier # files
---------------------------------------------------|-------
GPL-2.0 11139
and resulted in the first patch in this series.
If that file was a */uapi/* path one, it was "GPL-2.0 WITH
Linux-syscall-note" otherwise it was "GPL-2.0". Results of that was:
SPDX license identifier # files
---------------------------------------------------|-------
GPL-2.0 WITH Linux-syscall-note 930
and resulted in the second patch in this series.
- if a file had some form of licensing information in it, and was one
of the */uapi/* ones, it was denoted with the Linux-syscall-note if
any GPL family license was found in the file or had no licensing in
it (per prior point). Results summary:
SPDX license identifier # files
---------------------------------------------------|------
GPL-2.0 WITH Linux-syscall-note 270
GPL-2.0+ WITH Linux-syscall-note 169
((GPL-2.0 WITH Linux-syscall-note) OR BSD-2-Clause) 21

LGPL-2.1 WITH Linux-syscall-note 3
((GPL-2.0 WITH Linux-syscall-note) OR MIT) 3
((GPL-2.0 WITH Linux-syscall-note) AND MIT) 1
and that resulted in the third patch in this series.
- when the two scanners agreed on the detected license(s), that became
the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

On error, kthread_create() returns an errno-encoded pointer, not NULL.
The routine qla2x00_probe_one() detects the error case and jumps to
probe_failed, but has already assigned the return value from
kthread_create() to ha->dpc_thread. Then probe_failed checks to see if
ha->dpc_thread is not NULL before doing cleanup on it. Since in the
error case this is also not NULL, it ends up trying to access an invalid
task pointer.
Solution is to assign NULL to ha->dpc_thread in the error path to avoid
kthread cleanup in that case.

For several reasons, it is desirable to use {READ,WRITE}_ONCE() in
preference to ACCESS_ONCE(), and new code is expected to use one of the
former. So far, there's been no reason to change most existing uses of
ACCESS_ONCE(), as these aren't harmful, and changing them results in
churn.
However, for some features, the read/write distinction is critical to
correct operation. To distinguish these cases, separate read/write

it is defined.
Add linux/nvme-fc-driver.h (and scsi/fc/fc_fs.h as nvme-fc-driver.h
needs the definition of 'struct fc_ba_rjt' from scsi/fc/fc_fs.h) to the

The value of "size" comes from the user. When we add "start + size" it
could lead to an integer overflow bug.
It means we vmalloc() a lot more memory than we had intended. I believe
that on 64 bit systems vmalloc() can succeed even if we ask it to
allocate huge 4GB buffers. So we would get memory corruption and likely
a crash when we call ha->isp_ops->write_optrom() and ->read_optrom().
Only root can trigger this bug.

If there are IO's running and host reset or chip reset is triggered,
IO's can fail due to rport time out. During chip reset recovery process,
driver notifies the transport layer that remote port no longer exist, by
calling fc_remote_port_delete(). When this function is called, it
actually delays deletion by "blocking" it. It sets the remote port state
to "FC_PORTSTATE_BLOCKED" and sets FC_RPORT_DEVLOSS_PENDING. When
driver tries to abort the command by calling its abort handler function,
abort handler will wait until remote port state is blocked state or wait
for dev_loss_tmo time. Due to this blocking, rport times out and results
in an IO failure. This patch adds a check for any active reset process
before calling abort handler function.

This patch drops two incorrect usages of tcm_qla2xxx_free_cmd()
during TMR ABORT within tcm_qla2xxx_handle_data_work() and
tcm_qla2xxx_aborted_task(), which where attempting to dispatch
into workqueue context to do tcm_qla2xxx_complete_free() and
subsequently invoke transport_generic_free_cmd().
This is incorrect because during TMR ABORT target-core will
drop the outstanding se_cmd->cmd_kref references once it has
quiesced the se_cmd via transport_wait_for_tasks(), and in
the case of qla2xxx it should not attempt to do it's own
transport_generic_free_cmd() once the abort has occured.
As reported by Pascal, this was originally manifesting as a
BUG_ON(cmd->cmd_in_wq) in qlt_free_cmd() during TMR ABORT,
with a LIO backend that had sufficently high enough WRITE
latency to trigger a host side TMR ABORT_TASK.
(v2: Drop the qla_tgt_cmd->write_pending_abort_comp changes,
as they will be addressed in a seperate series)

This reverts commit 5f572526a1.
As reported by Pascal here:
http://www.spinics.net/lists/target-devel/msg15808.html
there still appears to be another issue related to this change
to drop the original bogus tcm_qla2xxx_free_cmd() usage from
tcm_qla2xxx_handle_data_work() and tcm_qla2xxx_aborted_task().
So revert this for now, until Pascal can verify with further
debug in place to understand what's going on.

This patch drops two incorrect usages of tcm_qla2xxx_free_cmd()
during TMR ABORT within tcm_qla2xxx_handle_data_work() and
tcm_qla2xxx_aborted_task(), which where attempting to dispatch
into workqueue context to do tcm_qla2xxx_complete_free() and
subsequently invoke transport_generic_free_cmd().
In addition, for the case in tcm_qla2xxx_write_pending_status()
and tcm_qla2xxx_handle_data_work() that waits for outstanding
FCP WRITE data transfer to complete before preceeding with a
TMR ABORT, avoid se_cmd->t_transport_stop_comp that is already
used by transport_wait_for_tasks() and use a qla2xxx internal
struct completion instead.

When pci_enable_device() or pci_enable_device_mem() fail in
qla2x00_probe_one() we bail out but do a call to
pci_disable_device(). This causes the dev_WARN_ON() in
pci_disable_device() to trigger, as the device wasn't enabled
previously.
So instead of taking the 'probe_out' error path we can directly return
*iff* one of the pci_enable_device() calls fails.
Additionally rename the 'probe_out' goto label's name to the more

- Add BFQ IO scheduler under the new blk-mq scheduling framework. BFQ
was initially a fork of CFQ, but subsequently changed to implement
fairness based on B-WF2Q+, a modified variant of WF2Q. BFQ is meant
to be used on desktop type single drives, providing good fairness.
From Paolo.
- Add Kyber IO scheduler. This is a full multiqueue aware scheduler,
using a scalable token based algorithm that throttles IO based on
live completion IO stats, similary to blk-wbt. From Omar.
- A series from Jan, moving users to separately allocated backing
devices. This continues the work of separating backing device life
times, solving various problems with hot removal.
- A series of updates for lightnvm, mostly from Javier. Includes a
'pblk' target that exposes an open channel SSD as a physical block
device.
- A series of fixes and improvements for nbd from Josef.
- A series from Omar, removing queue sharing between devices on mostly
legacy drivers. This helps us clean up other bits, if we know that a
queue only has a single device backing. This has been overdue for
more than a decade.
- Fixes for the blk-stats, and improvements to unify the stats and user
windows. This both improves blk-wbt, and enables other users to
register a need to receive IO stats for a device. From Omar.
- blk-throttle improvements from Shaohua. This provides a scalable
framework for implementing scalable priotization - particularly for
blk-mq, but applicable to any type of block device. The interface is
marked experimental for now.
- Bucketized IO stats for IO polling from Stephen Bates. This improves
efficiency of polled workloads in the presence of mixed block size
IO.
- A few fixes for opal, from Scott.
- A few pulls for NVMe, including a lot of fixes for NVMe-over-fabrics.
From a variety of folks, mostly Sagi and James Smart.
- A series from Bart, improving our exposed info and capabilities from
the blk-mq debugfs support.
- A series from Christoph, cleaning up how handle WRITE_ZEROES.
- A series from Christoph, cleaning up the block layer handling of how
we track errors in a request. On top of being a nice cleanup, it also
shrinks the size of struct request a bit.
- Removal of mg_disk and hd (sorry Linus) by Christoph. The former was
never used by platforms, and the latter has outlived it's usefulness.
- Various little bug fixes and cleanups from a wide variety of folks.

Merge tag 'scsi-misc' of git://git.kernel.org/pub/scm/linux/kernel/git/jejb/scsi
Pull more SCSI updates from James Bottomley:
"This is the set of stuff that didn't quite make the initial pull and a
set of fixes for stuff which did.
The new stuff is basically lpfc (nvme), qedi and aacraid. The fixes
cover a lot of previously submitted stuff, the most important of which
probably covers some of the failing irq vectors allocation and other
fallout from having the SCSI command allocated as part of the block
allocation functions"
