This avoids the potential for invalid memory access, if/when there are
no priority groups, in response to invalid arguments being sent by the

"enable_group").

When target 1.9.1 gets takeover/reshape requests on devices with old superblock
format not supporting such conversions and rejects them in super_init_validation(),
it logs bogus error message (e.g. Reshape when a takeover is requested).
Whilst on it, add messages for disk adding/removing and stripe sectors
reshape requests, use the newer rs_{takeover,reshape}_requested() API,
address a raid10 false positive in checking array positions and
remove rs_set_new() because device members are already set proper.

In the past, dm-crypt used per-cpu crypto context. This has been removed
in the kernel 3.15 and the crypto context is shared between all cpus. This
patch renames the function crypt_setkey_allcpus to crypt_setkey, because
there is really no activity that is done for all cpus.

In crypt_set_key(), if a failure occurs while replacing the old key
(e.g. tfm->setkey() fails) the key must not have DM_CRYPT_KEY_VALID flag
set. Otherwise, the crypto layer would have an invalid key that still
has DM_CRYPT_KEY_VALID flag set.

Use bio_add_page(), the standard interface for adding a page to a bio,
rather than open-coding the same.
It should be noted that the 'clone' bio that is allocated using
bio_alloc_bioset(), in crypt_alloc_buffer(), does _not_ set the
bio's BIO_CLONED flag. As such, bio_add_page()'s early return for true
bio clones (those with BIO_CLONED set) isn't applicable.

The CTR DRBG segments the number of random bytes to be generated into
128 byte blocks. The current code misses the advancement of the output
buffer pointer when the requestor asks for more than 128 bytes of data.
In this case, the next 128 byte block of random numbers is copied to
the beginning of the output buffer again. This implies that only the
first 128 bytes of the output buffer would ever be filled.
The patch adds the advancement of the buffer pointer to fill the entire
buffer.

If redrat3_delete() is called, ensure ep_in and udev members are set
up so we don't dereference null in the error path. Also ensure that
rc dev device exists before we enable the receiver and that the
led urb exists before we create the led device.

There are two places in the cpufreq core in which low-level driver
callbacks may be invoked for an inactive cpufreq policy, which isn't
guaranteed to work in general. Both are due to possible races with
CPU offline.
First, in cpufreq_get(), the policy may become inactive after
the check against policy->cpus in cpufreq_cpu_get() and before
policy->rwsem is acquired, in which case using it going forward may
not be correct.
Second, an analogous situation is possible in cpufreq_update_policy().
Avoid using inactive policies by adding policy_is_inactive() checks
to the code in the above places.

There may be reasons to use generic cpufreq governors (eg. schedutil)
on Intel platforms instead of the intel_pstate driver's internal
governor. However, that currently can only be done by disabling
intel_pstate altogether and using the acpi-cpufreq driver instead
of it, which is subject to limitations.
First of all, acpi-cpufreq only works on systems where the _PSS
object is present in the ACPI tables for all logical CPUs. Second,
on those systems acpi-cpufreq will only use frequencies listed by
_PSS which may be suboptimal. In particular, by convention, the
whole turbo range is represented in _PSS as a single P-state and
the frequency assigned to it is greater by 1 MHz than the greatest
non-turbo frequency listed by _PSS. That may confuse governors to
use turbo frequencies less frequently which may lead to suboptimal
performance.
For this reason, make it possible to use the intel_pstate driver
with generic cpufreq governors as a "normal" cpufreq driver. That
mode is enforced by adding intel_pstate=passive to the kernel
command line and cannot be disabled at run time. In that mode,
intel_pstate provides a cpufreq driver interface including
the ->target() and ->fast_switch() callbacks and is listed in
scaling_driver as "intel_cpufreq".

The Kconfig and file naming for the PM8xxx driver is totally
confusing:
- Kconfig options MFD_PM8XXX and MFD_PM8921_CORE, some in-kernel
users depending on or selecting either at random.
- A driver file named pm8921-core.c even if it is indeed
used by the whole PM8xxx family of chips.
- An irqchip named pm8xxx since it was (I guess) realized that
the driver was generic for all pm8xxx PMICs.
As I may want to add support for PM8901 this is starting to get
really messy. Fix this situation by:
- Remove the MFD_PM8921_CORE symbol and rely solely on MFD_PM8XXX
and convert all users, including LEDs Kconfig and ARM defconfigs
for qcom and multi_v7 to use that single symbol.
- Renaming the driver to qcom-pm8xxx.c to fit along the two
other qcom* prefixed drivers.
- Rename functions withing the driver from 8921 to 8xxx to
indicate it is generic.
- Just drop the =m config from the pxa_defconfig, I have no clue
why it is even there, it is not a Qualcomm platform. (Possibly
older Kconfig noise from saveconfig.)

If we have a bad client submitting unfavourably across different
contexts, creating new ones, the per context scoring of badness
doesn't remove the root cause, the offending client.
To counter, keep track of per client context bans. Deny access if
client is responsible for more than 3 context bans in
it's lifetime.

Now when driver has per context scoring of 'hanging badness'
and also subsequent hangs during short windows are allowed,
if there is progress made in between, it does not make sense
to expose a ban timing window as a context parameter anymore.
Let the scoring be the sole indicator for ban policy and substitute
ban period context parameter as a boolean to get/set context
bannable property.

As hangcheck score was removed, the active decay of score
was removed also. This removed feature for hangcheck to detect
if the gpu client was accidentally or maliciously causing intermittent
hangs. Reinstate the scoring as a per context property, so that if
one context starts to act unfavourably, ban it.

Hangcheck state accumulation has gained more steps
along the years, like head movement and more recently the
subunit inactivity check. As the subunit sampling is only
done if the previous state check showed inactivity, we
have added more stages (and time) to reach a hang verdict.
Asymmetric engine states led to different actual weight of
'one hangcheck unit' and it was demonstrated in some
hangs that due to difference in stages, simpler engines
were accused falsely of a hang as their scoring was much
more quicker to accumulate above the hang treshold.
To completely decouple the hangcheck guilty score
from the hangcheck period, convert hangcheck score to a
rough period of inactivity measurement. As these are
tracked as jiffies, they are meaningful also across
reset boundaries. This makes finding a guilty engine
more accurate across multi engine activity scenarios,
especially across asymmetric engines.
We lose the ability to detect cross batch malicious attempts
to hinder the progress. Plan is to move this functionality
to be part of context banning which is more natural fit,
later in the series.

In order to simplify hangcheck state keeping, split hangcheck
per engine loop in three phases: state load, action, state save.
Add few more hangcheck actions to separate between seqno, head
and subunit movements. This helps to gather all the hangcheck
actions under a single switch umbrella.

When unloading the module, it is expected that we have finished
executing all requests and so the signal threads should be idle. Add a
warning in case there are any residual requests in the signaler rbtrees
at that point.

Read urbs are submitted back only on success, causing read pipe
running out of urbs after few errors. No more characters can
be read from tty device then until it is reopened and no errors
are reported.
Fix that by always submitting urbs back and clearing stall on
-EPIPE.
