bf81973a5d [SCSI] scsi_dh_alua: Add one more check-condition for alua handler
This patch adds one more check-condition for scsi_dh_alua handler. Without
this, the handler attach fails sometimes during the discovery. I have noticed
this with NetApp E-Series storage with alua mode. Also removed some
unnecessary brackets {} for consistency.

14ed9cc7e7 [SCSI] ipr: Add support to flash FPGA and flash back DRAM images
The write buffer command is used to download and burn new IOA FW images.
The same interface can now be used to flash FPGA and flash back DRAM images.
To download and flash the new images takes more than 15 minutes, so increase
the write buffer command timeout to 30 minutes.

The FPGA and flash back DRAM images don't have the same card_type as the IOA FW
image. So, remove the sanity checking from the driver.  The adapter has sanity
checking and will only accept a valid image.

1a34c06401 [SCSI] libsas: fix port->dev_list locking
port->dev_list maintains a list of devices attached to a given sas root port.
It needs to be mutated under a lock as contexts outside of the
single-threaded-libsas-workqueue access the list via sas_find_dev_by_rphy().
Fixup locations where the list was being mutated without a lock.

This is a follow-up to commit 5911e963 "[SCSI] libsas: remove expander
from dev list on error", where Luben noted [1]:

    > 2/ We have unlocked list manipulations in sas_ex_discover_end_dev(),
    > sas_unregister_common_dev(), and sas_ex_discover_end_dev()

    Yes, I can see that and that is very unfortunate.

fd8fa9071e [SCSI] bnx2fc: call ctlr_link_up only when the interface is enabled
Link may not be up when the driver receives ulp_start event, and hence
fcoe_ctlr_link_up is not called. Call fcoe_ctlr_link_up during
indicate_netevent only when the interface is enabled. (It has to be called when
enabled because that is an indication that the vlan discovery is completed).

a90b412cb8 e1000e: locking bug introduced by commit 67fd4fcb
Commit 67fd4fcb (e1000e: convert to stats64) added the ability to update
statistics more accurately and on-demand through the net_device_ops
.ndo_get_stats64 hook, but introduced a locking bug on 82577/8/9 when
linked at half-duplex (seen on kernels with CONFIG_DEBUG_ATOMIC_SLEEP=y and
CONFIG_PROVE_LOCKING=y).  The commit introduced code paths that caused a
mutex to be locked in atomic contexts, e.g. an rcu_read_lock is held when
irqbalance reads the stats from /sys/class/net/ethX/statistics causing the
mutex to be locked to read the Phy half-duplex statistics registers.

The mutex was originally introduced to prevent concurrent accesses of
resources (the NVM and Phy) shared by the driver, firmware and hardware
a few years back when there was an issue with the NVM getting corrupted.
It was later split into two mutexes - one for the NVM and one for the Phy
when it was determined the NVM, unlike the Phy, should not be protected by
the software/firmware/hardware semaphore (arbitration of which is done in
part with the SWFLAG bit in the EXTCNF_CTRL register).  This latter
semaphore should be sufficient to prevent resource contention of the Phy in
the driver (i.e. the mutex for Phy accesses is not needed), but to be sure
the mutex is replaced with an atomic bit flag which will warn if any
contention is possible.

Also add additional debug output to help determine when the sw/fw/hw
semaphore is owned by the firmware or hardware.

23b1a99b87 mtd: nand: initialize ops.mode
Our `ops' information was converted to a local variable recently, and
apparently, old code relied on the fact that the global version was
often left in a valid mode. We can't make this assumption on local
structs, and we shouldn't be relying on a previous state anyway.

Instead, we initialize mode to 0 for don't-care situations (i.e., the
operation does not use OOB anyway) and MTD_OPS_PLACE_OOB when we want to
place OOB data.

This fixes a bug with nand_default_block_markbad(), where we catch on
the BUG() call in nand_fill_oob():

d5de1907d0 mtd: provide an alias for the redboot module name
parse_mtd_partitions takes a list of partition types; if the driver
isn't loaded, it attempts to load it, and then it grabs the partition
parser.  For redboot, the module name is "redboot.ko", while the parser
name is "RedBoot".  Since modprobe is case-sensitive, attempting to
modprobe "RedBoot" will never work.  I suspect the embedded systems that
make use of redboot just always manually loaded redboot prior to loading
their specific nand chip drivers (or statically compiled it in).

5f94913795 mtd: m25p80: don't probe device which has status of 'disabled'
On some platforms such as P3060QDS, has multiple spi flashes, but they are

4598fc2c94 dmaengine: mid_dma: mask_peripheral_interrupt only when dmac is idle
The mask_peripheral_interrupt is called when channel wants to mask the
interrupt. Move this to suspend as this masking affects other channels
as well. Not touching unmask here and unmask would still be done of
first channel use

Per the text in  Documentation/SubmitChecklist as below, we should
explicitly have header linux/errno.h in localtimer.h for ENXIO
reference.

1: If you use a facility then #include the file that defines/declares
   that facility.  Don't depend on other header files pulling in ones
   that you use.

Otherwise, we may run into some compiling error like the following one,
if any file includes localtimer.h without CONFIG_LOCAL_TIMERS defined.

wrapper will insert either the function_name or NULL, depending on the config
options."

necessary place.

e14d62c05c RDMA/cxgb4: Use correct QID in insert_recv_cqe()
When creating flushed receive CQEs, set the QPID field in the t4_cqe
to the SQ QID and not the RQ QID.  Otherwise the poll code will not

01e7da6ba5 RDMA/cxgb4: Make sure flush CQ entries are collected on connection close
At the time when a peer closes the connection, iw_cxgb4 will not send
a cq event if ibqp.uobject exists.  In that case, its possible for a
user application to get blocked in ibv_get_cq_event().

To resolve this, call the cq's comp_handler to unblock any read from
ibv_get_cq_event().  This will trigger userspace to poll the cq and
collect flush status completions for any pending work requests.

19696f09da [media] cx23885: Avoid incorrect error handling and reporting
Previous driver over-reported errors and stopped the video fifo
(causing video to stop) if 1) A risc error occured 2) The risc processor
detected that it has missing lines in the video frame 3) The risc fifo
is about to overflow.

The previous driver reported all three of these cases as risc errors
when this is technically not correct. So, the function was cleaned up.

1. Ensure that risc opcode related errors are correctly shown as such,
   and not overly reported for non-risc-opcode cases.
2. Ensure that line sync and overflow errors are not reported as opcode errors.
3. Ensure that only the risc-op-code case can stop the fifo and dump channel
   / risc processor information.

The net result is that if video becomes unstable, cable disconnect, this will
not trigger a stop of the video firmware (due to missing lines errors).

1. Removed the verbose 'dump channel state to console', very noisy in
   weak signal conditions.
2. No need for the video buffer timeout to stop the risc engine here.
   Clearer and easier to maintain if start_video_dma() is the single
   place that this is done.

2ccdd9a59b [media] cx25840: Enable support for non-tuner LR1/LR2 audio inputs
The change effects cx23885 boards only and preserves support for existing
boards. Essentially, if we're using baseband audio into the cx23885 AV
core then we have to patch registers. The cx23885 driver will call
with either CX25840_AUDIO8 to signify tuner audio or AUDIO7 to
signify baseband audio. If/When we become more comfortable with this change
across a series of products then we may decide to relax the cx23885 only
restriction.
