New architectures currently have to provide implementations of 5 different
functions: xen_arch_pre_suspend(), xen_arch_post_suspend(),
xen_arch_hvm_post_suspend(), xen_mm_pin_all(), and xen_mm_unpin_all().
Refactor the suspend code to only require xen_arch_pre_suspend() and
xen_arch_post_suspend().

FIFO event channels require bitops on 32-bit aligned values (the event
words). Linux's bitops require unsigned long alignment which may be
64-bits.
On arm64 an incorrectly unaligned access will fault.
Fix this by aligning the bitops along with an adjustment for bit
position and using an unsigned long for the local copy of the ready
word.

x86/xen: Fix 32-bit PV guests's usage of kernel_stack
The user can launch the guest in this sequence:
xl create -p /vm.cfg [launch, but pause it]
xl shutdown latest [sets control/shutdown=poweroff]
xl unpause latest
xl console latest [and see that the guest has completely
ignored the shutdown request]
In reality the guest hasn't ignored it. It registers a watch
and gets a notification that there is value. It then calls
the shutdown_handler which ends up calling orderly_shutdown.
Unfortunately that is so early in the bootup that there
are no user-space. Which means that the orderly_shutdown fails.
But since the force flag was set to false it continues on without
reporting.
What we really want to is to use the force when we are in the
SYSTEM_BOOTING state and not use the 'force' when SYSTEM_RUNNING.
However, if we are in the running state - and the shutdown command
has been given before the user-space has been setup, there is nothing
we can do. Worst yet, we stop ignoring the 'xl shutdown' requests!
As such, the other part of this patch is to only stop ignoring
the 'xl shutdown' when we are truly in the power off sequence.
That means the user can do multiple 'xl shutdown' and we will try
to act on them instead of ignoring them.

The 'read_reply' works with 'process_msg' to read of a reply in XenBus.
'process_msg' is running from within the 'xenbus' thread. Whenever
a message shows up in XenBus it is put on a xs_state.reply_list list
and 'read_reply' picks it up.
The problem is if the backend domain or the xenstored process is killed.
In which case 'xenbus' is still awaiting - and 'read_reply' if called -
stuck forever waiting for the reply_list to have some contents.
This is normally not a problem - as the backend domain can come back
or the xenstored process can be restarted. However if the domain
is in process of being powered off/restarted/halted - there is no
point of waiting on it coming back - as we are effectively being
terminated and should not impede the progress.
This patch solves this problem by checking whether the guest is the
right domain. If it is an initial domain and hurtling towards death -
there is no point of continuing the wait. All other type of guests
continue with their behavior (as Xenstore is expected to still be
running in another domain).

Patch 99c8b79d3c "xen: Add proper irq accounting for HYPERCALL vector"
added a call to inc_irq_stat(irq_hv_callback_count) in common Xen code,
however both the inc_irq_stat function and the irq_hv_callback_count
counter are architecture specific.
This makes the code build again on ARM by moving the call into the
existing #ifdef CONFIG_X86. We may want to later do the same implementation
on ARM that x86 has though.

Xen balloon driver will update ballooned out pages' P2M entries to point
to scratch page for PV guests. In 24f69373e2 ("xen/balloon: don't alloc
page while non-preemptible", kmap_flush_unused was moved after updating
P2M table. In that case for 32 bit PV guest we might end up with
P2M X -----> S (S is mfn of balloon scratch page)
M2P Y -----> X (Y is mfn in persistent kmap entry)
kmap_flush_unused() iterates through all the PTEs in the kmap address
space, using pte_to_page() to obtain the page. If the p2m and the m2p
are inconsistent the incorrect page is returned. This will clear
page->address on the wrong page which may cause subsequent oopses if
that page is currently kmap'ed.
Move the flush back between get_page and __set_phys_to_machine to fix
this.

Subsystems that want to register CPU hotplug callbacks, as well as perform
initialization for the CPUs that are already online, often do it as shown
below:
get_online_cpus();
for_each_online_cpu(cpu)
init_cpu(cpu);
register_cpu_notifier(&foobar_cpu_notifier);
put_online_cpus();
This is wrong, since it is prone to ABBA deadlocks involving the
cpu_add_remove_lock and the cpu_hotplug.lock (when running concurrently
with CPU hotplug operations).
The xen balloon driver doesn't take get/put_online_cpus() around this code,
but that is also buggy, since it can miss CPU hotplug events in between the
initialization and callback registration:

Interestingly, the balloon code in xen can simply be reorganized as shown
below, to have a race-free method to register hotplug callbacks, without even
taking get/put_online_cpus(). This is because the initialization performed for
already online CPUs is exactly the same as that performed for CPUs that come
online later. Moreover, the code has checks in place to avoid double
initialization.

A hotplug operation that occurs between registering the notifier and calling
get_online_cpus(), won't disrupt anything, because the code takes care to

hotplug callback registration.

On xen_acpi_processor_resume() we call various procedures, which are
non atomic and can enable interrupts. To prevent the issue introduce
separate resume notify called after we enable interrupts on resume
and before we call other drivers resume callbacks.

The grant mapping API does m2p_override unnecessarily: only gntdev needs it,
for blkback and future netback patches it just cause a lock contention, as
those pages never go to userspace. Therefore this series does the following:
- the bulk of the original function (everything after the mapping hypercall)
is moved to arch-dependent set/clear_foreign_p2m_mapping
- the "if (xen_feature(XENFEAT_auto_translated_physmap))" branch goes to ARM
- therefore the ARM function could be much smaller, the m2p_override stubs
could be also removed
- on x86 the set_phys_to_machine calls were moved up to this new funcion
from m2p_override functions
- and m2p_override functions are only called when there is a kmap_ops param
It also removes a stray space from arch/x86/include/asm/xen/page.h.

Add support for MSI message groups for Xen Dom0 using the
MAP_PIRQ_TYPE_MULTI_MSI pirq map type.
In order to keep track of which pirq is the first one in the group all
pirqs in the MSI group except for the first one have the newly
introduced PIRQ_MSI_GROUP flag set. This prevents calling
PHYSDEVOP_unmap_pirq on them, since the unmap must be done with the
first pirq in the group.

The user space interface does not filter out offline cpus. It merily
verifies that the mask contains at least one online cpu. So the
selector in the irq chip implementation needs to make sure to pick
only an online cpu because otherwise:

Selector will pick first set bit, i.e. core 1

Warn if any PIRQ cannot be bound to an event channel. Remove the check
for irq_desc->action. This hypercall never fails in practice so we can
emit a warning unconditionally.
Remove a check for a valid irq desc. The only caller of
xen_destroy_irq() will only do so if the irq was previously fully
setup, which means the descriptor has been allocated as well.

As result of deprecation of MSI-X/MSI enablement functions
pci_enable_msix() and pci_enable_msi_block() all drivers
using these two interfaces need to be updated to use the
new pci_enable_msi_range() or pci_enable_msi_exact()
and pci_enable_msix_range() or pci_enable_msix_exact()

When I looked at this issue https://lkml.org/lkml/2013/11/21/158, I found that
frontswap_selfshrink() doesn't work as expected sometimes.
Pages are continuously added to frontswap and gotten back soon. It's a waste of
cpu time and increases the memory pressue of Guest OS.
Take an example.
First time in frontswap_selfshrink():
1. last_frontswap_pages = cur_frontswap_pages = 0
2. cur_frontswap_pages = frontswap_curr_pages() = 100
When 'frontswap_inertia_counter' decreased to 0:
1. last_frontswap_pages = cur_frontswap_pages = 100
2. cur_frontswap_pages = frontswap_curr_pages() = 100
3. call frontswap_shrink() and let's assumption that 10 pages are gotten back
from frontswap.
4. now frontswap_curr_pages() is 90.
If then memory is not enough in Guest OS and 9 more pages(smaller than gotten
back) added to frontswap.
Now frontswap_curr_pages() is 99 and we don't expect to get back more pages from
frontswap because geust os is under memory pressure.
But next time in frontswap_selfshrink():
1. last_frontswap_pages is set to the old value of cur_frontswap_pages(still
100)
2. cur_frontswap_pages(99) is still smaller than last_frontswap_pages.
3. call frontswap_shrink() and continue to get back pages from frontswap!!

Commit fc087e1073 (xen/events: remove
unnecessary init_evtchn_cpu_bindings()) causes a regression.
The kernel-side VCPU binding was not being correctly set for newly
allocated or bound interdomain events. In ARM guests where 2-level
events were used, this would result in no interdomain events being
handled because the kernel-side VCPU masks would all be clear.
x86 guests would work because the irq affinity was set during irq
setup and this would set the correct kernel-side VCPU binding.
Fix this by properly initializing the kernel-side VCPU binding in
bind_evtchn_to_irq().

Current xen-selfballoon driver is too aggressive which may cause OOM be
triggered more often. Eg. this bug reported by James:

There are two mainly reasons:
1) The original goal_page didn't consider some pages used by kernel space, like
slab pages and pages used by device drivers.
2) The balloon driver may not give back memory to guest OS fast enough when the
workload suddenly aquries a lot of physical memory.
In both cases, the guest OS will suffer from memory pressure and OOM may
be triggered.
The fix is make xen-selfballoon driver not that aggressive by adding extra 10%
of total ram pages to goal_page.
It's more valuable to keep the guest system reliable and response faster than
balloon out these 10% pages to XEN.

The grant mapping API does m2p_override unnecessarily: only gntdev needs it,
for blkback and future netback patches it just cause a lock contention, as
those pages never go to userspace. Therefore this series does the following:
- the original functions were renamed to __gnttab_[un]map_refs, with a new
parameter m2p_override
- based on m2p_override either they follow the original behaviour, or just set
the private flag and call set_phys_to_machine
- gnttab_[un]map_refs are now a wrapper to call __gnttab_[un]map_refs with
m2p_override false
- a new function gnttab_[un]map_refs_userspace provides the old behaviour
It also removes a stray space from page.h and change ret to 0 if
XENFEAT_auto_translated_physmap, as that is the only possible return value
there.

On ARM, address size can be 32 bits or 64 bits (if CONFIG_ARCH_PHYS_ADDR_T_64BIT
is enabled).
We can't assume that the grant frame base address will always fits in an
unsigned long. Use phys_addr_t instead of unsigned long as argument for
gnttab_setup_auto_xlat_frames.

The use of phys_to_machine and machine_to_phys in the phys<=>bus conversions
causes us to lose the top bits of the DMA address if the size of a DMA address is not the same as the size of the phyiscal address.
This can happen in practice on ARM where foreign pages can be above 4GB even
though the local kernel does not have LPAE page tables enabled (which is
totally reasonable if the guest does not itself have >4GB of RAM). In this
case the kernel still maps the foreign pages at a phys addr below 4G (as it
must) but the resulting DMA address (returned by the grant map operation) is
much higher.
This is analogous to a hardware device which has its view of RAM mapped up
high for some reason.
This patch makes I/O to foreign pages (specifically blkif) work on 32-bit ARM
systems with more than 4GB of RAM.

The first is event channel scalability by David Vrabel - we switch
over from an two-level per-cpu bitmap of events (IRQs) - to an FIFO
queue with priorities. This lets us be able to handle more events,
have lower latency, and better scalability. Good stuff.
The other is PVH by Mukesh Rathor. In short, PV is a mode where the
kernel lets the hypervisor program page-tables, segments, etc. With
EPT/NPT capabilities in current processors, the overhead of doing this
in an HVM (Hardware Virtual Machine) container is much lower than the
hypervisor doing it for us.
In short we let a PV guest run without doing page-table, segment,
syscall, etc updates through the hypervisor - instead it is all done
within the guest container. It is a "hybrid" PV - hence the 'PVH'
name - a PV guest within an HVM container.
The major benefits are less code to deal with - for example we only
use one function from the the pv_mmu_ops (which has 39 function
calls); faster performance for syscall (no context switches into the
hypervisor); less traps on various operations; etc.
It is still being baked - the ABI is not yet set in stone. But it is
pretty awesome and we are excited about it.
Lastly, there are some changes to ARM code - you should get a simple
conflict which has been resolved in #linux-next.

We have the framework to use v2, but there are no backends that
actually use it. The end result is that on PV we use v2 grants
and on PVHVM v1. The v1 has a capacity of 512 grants per page while
the v2 has 256 grants per page. This means we lose about 50%
capacity - and if we want more than 16 VIFs (each VIF takes
512 grants), then we are hitting the max per guest of 32.

In PVH the shared grant frame is the PFN and not MFN,
hence its mapped via the same code path as HVM.
The allocation of the grant frame is done differently - we
do not use the early platform-pci driver and have an
ioremap area - instead we use balloon memory and stitch
all of the non-contingous pages in a virtualized area.
That means when we call the hypervisor to replace the GMFN
with a XENMAPSPACE_grant_table type, we need to lookup the
old PFN for every iteration instead of assuming a flat
contingous PFN allocation.
Lastly, we only use v1 for grants. This is because PVHVM
is not able to use v2 due to no XENMEM_add_to_physmap
calls on the error status page (see commit
69e8f430e2
xen/granttable: Disable grant v2 for HVM domains.)
Until that is implemented this workaround has to
be in place.
Also per suggestions by Stefano utilize the PVHVM paths
as they share common functionality.
v2 of this patch moves most of the PVH code out in the
arch/x86/xen/grant-table driver and touches only minimally
the generic driver.

The 'xen_hvm_resume_frames' used to be an 'unsigned long'
and contain the virtual address of the grants. That was OK
for most architectures (PVHVM, ARM) were the grants are contiguous
in memory. That however is not the case for PVH - in which case
we will have to do a lookup for each virtual address for the PFN.
Instead of doing that, lets make it a structure which will contain
the array of PFNs, the virtual address and the count of said PFNs.
Also provide a generic functions: gnttab_setup_auto_xlat_frames and
gnttab_free_auto_xlat_frames to populate said structure with
appropriate values for PVHVM and ARM.
To round it off, change the name from 'xen_hvm_resume_frames' to
a more descriptive one - 'xen_auto_xlat_grant_frames'.
For PVH, in patch "xen/pvh: Piggyback on PVHVM for grant driver"
we will populate the 'xen_auto_xlat_grant_frames' by ourselves.
v2 moves the xen_remap in the gnttab_setup_auto_xlat_frames
and also introduces xen_unmap for gnttab_free_auto_xlat_frames.

We have this odd scenario of where for PV paths we take a shortcut
but for the HVM paths we first ioremap xen_hvm_resume_frames, then
assign it to gnttab_shared.addr. This is needed because gnttab_map
uses gnttab_shared.addr.
Instead of having:

Lets move the HVM part before the gnttab_map and remove the
first call to gnttab_map.

The function gnttab_max_grant_frames() returns the maximum amount
of frames (pages) of grants we can have. Unfortunatly it was
dependent on gnttab_init() having been run before to initialize
the boot max value (boot_max_nr_grant_frames).
This meant that users of gnttab_max_grant_frames would always
get a zero value if they called before gnttab_init() - such as
'platform_pci_init' (drivers/xen/platform-pci.c).

PVH is a PV guest with a twist - there are certain things
that work in it like HVM and some like PV. There is
a similar mode - PVHVM where we run in HVM mode with
PV code enabled - and this patch explores that.
The most notable PV interfaces are the XenBus and event channels.
We will piggyback on how the event channel mechanism is
used in PVHVM - that is we want the normal native IRQ mechanism
and we will install a vector (hvm callback) for which we
will call the event channel mechanism.
This means that from a pvops perspective, we can use
native_irq_ops instead of the Xen PV specific. Albeit in the
future we could support pirq_eoi_map. But that is
a feature request that can be shared with PVHVM.

Implement all the event channel port ops for the FIFO-based ABI.
If the hypervisor supports the FIFO-based ABI, enable it by
initializing the control block for the boot VCPU and subsequent VCPUs
as they are brought up and on resume. The event array is expanded as
required when event ports are setup.
The 'xen.fifo_events=0' command line option may be used to disable use
of the FIFO-based ABI.

Add the hypercall sub-ops and the structures for the shared data used
in the FIFO-based event channel ABI.
The design document for this new ABI is available here:

In summary, events are reported using a per-domain shared event array
of event words. Each event word has PENDING, LINKED and MASKED bits
and a LINK field for pointing to the next event in the event queue.
There are 16 event queues (with different priorities) per-VCPU.
Key advantages of this new ABI include:
- Support for over 100,000 events (2^17).
- 16 different event priorities.
- Improved fairness in event latency through the use of FIFOs.
The ABI is available in Xen 4.4 and later.

Refactor static array evtchn_to_irq array to be dynamically allocated by
implementing get and set functions for accesses to the array.
Two new port ops are added: max_channels (maximum supported number of
event channels) and nr_channels (number of currently usable event
channels). For the 2-level ABI, these numbers are both the same as
the shared data structure is a fixed size. For the FIFO ABI, these
will be different as the event array is expanded dynamically.
This allows more than 65000 event channels so an unsigned short is no
longer sufficient for an event channel port number and unsigned int is
used instead.

The FIFO-based event ABI requires additional setup of newly bound
events (it may need to expand the event array) and this setup may
fail.
xen_irq_info_common_init() is a useful place to put this setup so
allow this call to fail. This call and the other similar calls are
renamed to be *_setup() to reflect that they may now fail.
This failure can only occur with new event channels not on rebind.

Because the guest-side binding of an event to a VCPU (i.e., setting
the local per-cpu masks) is always explicitly done after an event
channel is bound to a port, there is no need to initialize all
possible events as bound to VCPU 0 at start of day or after a resume.

These two function did the same thing with different parameters, put
the common bits in retrigger_evtchn().
This changes the return value of resend_irq_on_evtchn() but the only
caller (in arch/ia64/xen/irq_xen.c) ignored the return value so this
is fine.

Since c275a57f5e "xen/balloon: Set balloon's initial state to number of
existing RAM pages" the balloon driver appears to work fine on ARM as far as I
can tell. Prior to that commit it was broken because on ARM RAM doesn't
typically start at zero, effectively leaving a big MMIO hole at the start.
This would cause the balloon driver to give away all of RAM at start of day,
which is rather inconvenient.
It was already enabled (or rather not excluded) on ARM64. The

"xen/balloon: Seperate the auto-translate logic properly (v2)"
added in the proper plumbing to work with ARM and PVH type guests.

The user has the option of disabling the platform driver:
00:02.0 Unassigned class [ff80]: XenSource, Inc. Xen Platform Device (rev 01)
which is used to unplug the emulated drivers (IDE, Realtek 8169, etc)
and allow the PV drivers to take over. If the user wishes
to disable that they can set:
xen_platform_pci=0

xen_emul_unplug=never
(on the Linux command line)
except it does not work properly. The PV drivers still try to
load and since the Xen platform driver is not run - and it
has not initialized the grant tables, most of the PV drivers

native environment).
- if running in HVM, check if user wanted 'xen_emul_unplug=never',
in which case bail out and don't load any PV drivers.

The auto-xlat logic vs the non-xlat means that we don't need to for
auto-xlat guests (like PVH, HVM or ARM):
- use P2M
- use scratch page.
However the code in increase_reservation does modify the p2m for
auto_translate guests, but not in decrease_reservation.
Fix that by avoiding any p2m modifications in both increase_reservation
and decrease_reservation for auto_translated guests.
And also avoid allocating or using scratch pages for auto_translated guests.
Lastly, since !auto-xlat is really another way of saying 'xen_pv'
remove the redundant 'xen_pv_domain' check.

Replace direct inclusions of <acpi/acpi.h>, <acpi/acpi_bus.h> and
<acpi/acpi_drivers.h>, which are incorrect, with <linux/acpi.h>
inclusions and remove some inclusions of those files that aren't
necessary.
First of all, <acpi/acpi.h>, <acpi/acpi_bus.h> and <acpi/acpi_drivers.h>
should not be included directly from any files that are built for
CONFIG_ACPI unset, because that generally leads to build warnings about
undefined symbols in !CONFIG_ACPI builds. For CONFIG_ACPI set,
<linux/acpi.h> includes those files and for CONFIG_ACPI unset it
provides stub ACPI symbols to be used in that case.
Second, there are ordering dependencies between those files that always
have to be met. Namely, it is required that <acpi/acpi_bus.h> be included
prior to <acpi/acpi_drivers.h> so that the acpi_pci_root declarations the
latter depends on are always there. And <acpi/acpi.h> which provides
basic ACPICA type declarations should always be included prior to any other
ACPI headers in CONFIG_ACPI builds. That also is taken care of including
<linux/acpi.h> as appropriate.

This failure represents a hypervisor issue, but if it does occur then nothing
good can come of returning pages which still refer to a foreign owned page
into the general allocation pool.
Instead we are forced to leak them. Log that we have done so.
The potential for failure only exists for autotranslated guest (e.g. ARM and
x86 PVH).

xen_hvm_resume_frames stores the physical address of the grant table.
englighten.c was incorrectly setting it as if it was a page frame number.
This caused the table to be mapped into the guest at an unexpected physical
address.
Additionally, a warning is improved to include the grant table address which
failed in xen_remap.

Modify the ACPI namespace scanning code to register a struct
acpi_device object for every namespace node representing a device,
processor and so on, even if the device represented by that namespace
node is reported to be not present and not functional by _STA.
There are multiple reasons to do that. First of all, it avoids
quite a lot of overhead when struct acpi_device objects are
deleted every time acpi_bus_trim() is run and then added again
by a subsequent acpi_bus_scan() for the same scope, although the
namespace objects they correspond to stay in memory all the time
(which always is the case on a vast majority of systems).
Second, it will allow user space to see that there are namespace
nodes representing devices that are not present at the moment and may
be added to the system. It will also allow user space to evaluate
_SUN for those nodes to check what physical slots the "missing"
devices may be put into and it will make sense to add a sysfs
attribute for _STA evaluation after this change (that will be
useful for thermal management on some systems).
Next, it will help to consolidate the ACPI hotplug handling among
subsystems by making it possible to store hotplug-related information
in struct acpi_device objects in a standard common way.
Finally, it will help to avoid a race condition related to the
deletion of ACPI namespace nodes. Namely, namespace nodes may be
deleted as a result of a table unload triggered by _EJ0 or _DCK.
If a hotplug notification for one of those nodes is triggered
right before the deletion and it executes a hotplug callback
via acpi_hotplug_execute(), the ACPI handle passed to that
callback may be stale when the callback actually runs. One way
to work around that is to always pass struct acpi_device pointers
to hotplug callbacks after doing a get_device() on the objects in
question which eliminates the use-after-free possibility (the ACPI
handles in those objects are invalidated by acpi_scan_drop_device(),
so they will trigger ACPICA errors on attempts to use them).

Currently balloon's initial value is set to max_pfn which includes
non-RAM ranges such as MMIO hole. As result, initial memory target
(specified by guest's configuration file) will appear smaller than
what balloon driver perceives to be the current number of available
pages. Thus it will balloon down "extra" pages, decreasing amount of
available memory for no good reason.

The PCI MMCONFIG area is usually reserved via the E820 so the Xen hypervisor
is aware of these regions. But they can also be enumerated in the ACPI
DSDT which means the hypervisor won't know of them until the initial
domain informs it of via PHYSDEVOP_pci_mmcfg_reserved.
This is what this patch does for all of the MCFG regions that the
initial domain is aware of (E820 enumerated and ACPI).

Tracepoints are only created when Xen support is enabled, but they are
also referenced within lib/swiotlb.c. So unless Xen support is enabled
the tracepoints will be missing, therefore causing builds to fail. Fix
this by moving the tracepoint creation to lib/swiotlb.c, which works
nicely because the Xen swiotlb support selects the generic swiotlb
support.

Use xen_alloc_coherent_pages and xen_free_coherent_pages to allocate or
free coherent pages.
We need to be careful handling the pointer returned by
xen_alloc_coherent_pages, because on ARM the pointer is not equal to
phys_to_virt(*dma_handle). In fact virt_to_phys only works for kernel
direct mapped RAM memory.
In ARM case the pointer could be an ioremap address, therefore passing
it to virt_to_phys would give you another physical address that doesn't
correspond to it.
Make xen_create_contiguous_region take a phys_addr_t as start parameter to
avoid the virt_to_phys calls which would be incorrect.

Xen on arm and arm64 needs SWIOTLB_XEN: when running on Xen we need to
program the hardware with mfns rather than pfns for dma addresses.
Remove SWIOTLB_XEN dependency on X86 and PCI and make XEN select
SWIOTLB_XEN on arm and arm64.
At the moment always rely on swiotlb-xen, but when Xen starts supporting
hardware IOMMUs we'll be able to avoid it conditionally on the presence
of an IOMMU on the platform.
Implement xen_create_contiguous_region on arm and arm64: for the moment
we assume that dom0 has been mapped 1:1 (physical addresses == machine
addresses) therefore we don't need to call XENMEM_exchange. Simply
return the physical address as dma address.
Initialize the xen-swiotlb from xen_early_init (before the native
dma_ops are initialized), set xen_dma_ops to &xen_swiotlb_dma_ops.

Ftrace is currently not able to detect when SWIOTLB has to do double buffering.
Under Xen you can only see it indirectly in function_graph, when
xen_swiotlb_map_page() doesn't stop after range_straddles_page_boundary(), but
calls spinlock functions, memcpy() and xen_phys_to_bus() as well. This patch
introduces the swiotlb:swiotlb_bounced event, which also prints out the
following informations to help you find out why bouncing happened:

If you use Xen, and (dev_addr + size + 1) > dma_mask, the buffer is out of the
device's DMA range. If swiotlb_force == 1, you should really change the kernel

In decrease_reservation(), if the kernel is preempted between updating
the mapping and updating the p2m then they may end up using different
scratch pages.
Use get_balloon_scratch_page() and put_balloon_scratch_page() which use
get_cpu_var() and put_cpu_var() to correctly disable preemption.

In commit cd9151e2: xen/balloon: set a mapping for ballooned out pages
we have the ballooned out page's mapping set to a scratch page.
That commit also sets the P2M entry of ballooned out page to the scratch
page's MFN. This is necessary for PV guest but not for HVM guest. On the
other hand, setting the P2M entry would trigger BUG_ON in
__set_phys_to_machine.
The correct thing to do here is to avoid calling __set_phys_to_machine
for auto translated guest.

When a foreign mapper attempts to map guest frames that are paged out,
the mapper receives an ENOENT response and will have to try again
while a helper process pages the target frame back in.
Gating checks on PRIVCMD_MMAPBATCH* ioctl args were preventing retries
of mapping calls.
Permit subsequent calls to update a sub-range of the VMA, iff nothing
is yet mapped in that range.
Since it is now valid to call PRIVCMD_MMAPBATCH* multiple times, only
set vma->vm_private_data if the parameters are valid and (if
necessary) the pages for the auto_translated_physmap case have been
allocated. This prevents subsequent calls from incorrectly entering
the 'retry' path when there are no pages allocated etc.

GNTTABOP_unmap_grant_ref unmaps a grant and replaces it with a 0
mapping instead of reinstating the original mapping.
Doing so separately would be racy.
To unmap a grant and reinstate the original mapping atomically we use
GNTTABOP_unmap_and_replace.
GNTTABOP_unmap_and_replace doesn't work with GNTMAP_contains_pte, so
don't use it for kmaps. GNTTABOP_unmap_and_replace zeroes the mapping
passed in new_addr so we have to reinstate it, however that is a
per-cpu mapping only used for balloon scratch pages, so we can be sure that
it's not going to be accessed while the mapping is not valid.

The original comment on the scanning of the start word on the 2nd pass
did not reflect the actual behaviour (the code was incorrectly masking
bit_idx instead of the pending word itself).
The documented behaviour is not actually required since if event were
pending in the MSBs, they would be immediately scanned anyway as we go
through the loop again.
Update the documentation to reflect this (instead of trying to change
the behaviour).

When a event is being bound to a VCPU there is a window between the
EVTCHNOP_bind_vpcu call and the adjustment of the local per-cpu masks
where an event may be lost. The hypervisor upcalls the new VCPU but
the kernel thinks that event is still bound to the old VCPU and
ignores it.
There is even a problem when the event is being bound to the same VCPU
as there is a small window beween the clear_bit() and set_bit() calls
in bind_evtchn_to_cpu(). When scanning for pending events, the kernel
may read the bit when it is momentarily clear and ignore the event.
Avoid this by masking the event during the whole bind operation.

The sizeof() argument in init_evtchn_cpu_bindings() is incorrect
resulting in only the first 64 (or 32 in 32-bit guests) ports having
their bindings being initialized to VCPU 0.
In most cases this does not cause a problem as request_irq() will set
the irq affinity which will set the correct local per-cpu mask.
However, if the request_irq() is called on a VCPU other than 0, there
is a window between the unmasking of the event and the affinity being
set were an event may be lost because it is not locally unmasked on
any VCPU. If request_irq() is called on VCPU 0 then local irqs are
disabled during the window and the race does not occur.
Fix this by initializing all NR_EVENT_CHANNEL bits in the local
per-cpu masks.

Currently ballooned out pages are mapped to 0 and have INVALID_P2M_ENTRY
in the p2m. These ballooned out pages are used to map foreign grants
by gntdev and blkback (see alloc_xenballooned_pages).
Allocate a page per cpu and map all the ballooned out pages to the
corresponding mfn. Set the p2m accordingly. This way reading from a
ballooned out page won't cause a kernel crash (see
http://lists.xen.org/archives/html/xen-devel/2012-12/msg01154.html).

The global array of port users and the port_user_lock limits
scalability of the evtchn device. Instead of the global array lookup,
use a per-use (per-fd) tree of event channels bound by that user and
protect the tree with a per-user lock.
This is also a prerequiste for extended the number of supported event
channels, by removing the fixed size, per-event channel array.

With the current implementation, the callback in the tail of the list
can be added twice, because the check done in
gnttab_request_free_callback is bogus, callback->next can be NULL if
it is the last callback in the list. If we add the same callback twice
we end up with an infinite loop, were callback == callback->next.
Replace this check with a proper one that iterates over the list to
see if the callback has already been added.

This is based on a patch that Zhenzhong Duan had sent - which
was missing some of the remaining pieces. The kernel has the
logic to handle Xen-type-exceptions using the paravirt interface
in the assembler code (see PARAVIRT_ADJUST_EXCEPTION_FRAME -
pv_irq_ops.adjust_exception_frame and and INTERRUPT_RETURN -
pv_cpu_ops.iret).
That means the nmi handler (and other exception handlers) use
the hypervisor iret.
The other changes that would be neccessary for this would
be to translate the NMI_VECTOR to one of the entries on the
ipi_vector and make xen_send_IPI_mask_allbutself use different
events.
Fortunately for us commit 1db01b4903
(xen: Clean up apic ipi interface) implemented this and we piggyback
on the cleanup such that the apic IPI interface will pass the right
vector value for NMI.
With this patch we can trigger NMIs within a PV guest (only tested
x86_64).
For this to work with normal PV guests (not initial domain)
we need the domain to be able to use the APIC ops - they are
already implemented to use the Xen event channels. For that
to be turned on in a PV domU we need to remove the masking
of X86_FEATURE_APIC.
Incidentally that means kgdb will also now work within
a PV guest without using the 'nokgdbroundup' workaround.
Note that the 32-bit version is different and this patch
does not enable that.

Unbinding an event channel (either with the ioctl or when the evtchn
device is closed) may deadlock because disable_irq() is called with
port_user_lock held which is also locked by the interrupt handler.
Think of the IOCTL_EVTCHN_UNBIND is being serviced, the routine has
just taken the lock, and an interrupt happens. The evtchn_interrupt
is invoked, tries to take the lock and spins forever.
A quick glance at the code shows that the spinlock is a local IRQ
variant. Unfortunately that does not help as "disable_irq() waits for
the interrupt handler on all CPUs to stop running. If the irq occurs
on another VCPU, it tries to take port_user_lock and can't because
the unbind ioctl is holding it." (from David). Hence we cannot
depend on the said spinlock to protect us. We could make it a system
wide IRQ disable spinlock but there is a better way.
We can piggyback on the fact that the existence of the spinlock is
to make get_port_user() checks be up-to-date. And we can alter those
checks to not depend on the spin lock (as it's protected by u->bind_mutex
in the ioctl) and can remove the unnecessary locking (this is
IOCTL_EVTCHN_UNBIND) path.
In the interrupt handler we cannot use the mutex, but we do not
need it.
"The unbind disables the irq before making the port user stale, so when
you clear it you are guaranteed that the interrupt handler that might
use that port cannot be running." (from David).
Hence this patch removes the spinlock usage on the teardown path
and piggybacks on disable_irq happening before we muck with the
get_port_user() data. This ensures that the interrupt handler will
never run on stale data.

The __cpuinit type of throwaway sections might have made sense
some time ago when RAM was more constrained, but now the savings
do not offset the cost and complications. For example, the fix in
commit 5e427ec2d0 ("x86: Fix bit corruption at CPU resume time")
is a good example of the nasty type of bugs that can be created
with improper use of the various __init prefixes.
After a discussion on LKML[1] it was decided that cpuinit should go
the way of devinit and be phased out. Once all the users are gone,
we can then finally remove the macros themselves from linux/init.h.
This removes all the remaining one-off uses of the __cpuinit macros
from all C files in the drivers/* directory.

Enhance adjust_managed_page_count() to adjust totalhigh_pages for
highmem pages. And change code which directly adjusts totalram_pages to
use adjust_managed_page_count() because it adjusts totalram_pages,
totalhigh_pages and zone->managed_pages altogether in a safe way.
Remove inc_totalhigh_pages() and dec_totalhigh_pages() from xen/balloon
driver bacause adjust_managed_page_count() has already adjusted
totalhigh_pages.
This patch also fixes two bugs:
1) enhances virtio_balloon driver to adjust totalhigh_pages when
reserve/unreserve pages.
2) enhance memory_hotplug.c to adjust totalhigh_pages when hot-removing
memory.
We still need to deal with modifications of totalram_pages in file
arch/powerpc/platforms/pseries/cmm.c, but need help from PPC experts.

Commit 10a7a07713 ("xen: tmem: enable Xen
tmem shim to be built/loaded as a module") allows the tmem module
to be loaded any time. For this work the frontswap API had to
be able to asynchronously to call tmem_frontswap_init before
or after the swap image had been set. That was added in git
commit 905cd0e1bf
("mm: frontswap: lazy initialization to allow tmem backends to build/run as modules").
Which means we could do this (The common case):
modprobe tmem [so calls frontswap_register_ops, no ->init]
modifies tmem_frontswap_poolid = -1
swapon /dev/xvda1 [__frontswap_init, calls -> init, tmem_frontswap_poolid is
< 0 so tmem hypercall done]
Or the failing one:
swapon /dev/xvda1 [calls __frontswap_init, sets the need_init bitmap]
modprobe tmem [calls frontswap_register_ops, -->init calls, finds out
tmem_frontswap_poolid is 0, does not make a hypercall.
Later in the module_init, sets tmem_frontswap_poolid=-1]
Which meant that in the failing case we would not call the hypercall
to initialize the pool and never be able to make any frontswap
backend calls.
Moving the frontswap_register_ops after setting the tmem_frontswap_poolid
fixes it.

If the xenbus frontend is located in a domain running xenstored, the device
resume is hanging because it is happening before the process resume. This
patch adds extra logic to the resume code to check if we are the domain
running xenstored and delay the resume if needed.

There is no point. We would just squeeze the guest to put more and
more pages in the swap disk without any purpose.
The only time it makes sense to use the selfballooning and shrinking
is when frontswap is being utilized.

If tmem is built-in or a module, the user has the option on
the command line to influence it by doing: tmem.<some option>
instead of having a variety of "nocleancache", and
"nofrontswap". The others: "noselfballooning" and "selfballooning";
and "noselfshrink" are in a different driver xen-selfballoon.c
and the patches:
xen/tmem: Remove the usage of 'noselfshrink' and use 'tmem.selfshrink' bool instead.
xen/tmem: Remove the usage of 'noselfballoon','selfballoon' and use 'tmem.selfballon' bool instead.
remove them.

The variety of disable_[cleancache|frontswap|selfshrinking] are
making this a bit complex. Just remove the "disable_" part and
change the logic around for the "nofrontswap" and "nocleancache"
parameters.

Reset the IRQ_NOAUTOEN and IRQ_NOREQUEST flags that are enabled by
default on ARM. If IRQ_NOAUTOEN is set, __setup_irq doesn't call
irq_startup, that is responsible for calling irq_unmask at startup time.
As a result event channels remain masked.
The clear is already made in bind_evtchn_to_irq with commit a8636c0 but was
missing on all others bind_*_to_irq. Move the clear in xen_irq_info_common_init.
On x86, IRQ_NOAUTOEN and IRQ_NOREQUEST are cleared by default, so this commit
doesn't impact this architecture.

Allow Xen tmem shim to be built/loaded as a module. Xen self-ballooning
and frontswap-selfshrinking are now also "lazily" initialized when the
Xen tmem shim is loaded as a module, unless explicitly disabled by
module parameters.
Note runtime dependency disallows loading if cleancache/frontswap lazy
initialization patches are not present.
If built-in (not built as a module), the original mechanism of enabling
via a kernel boot parameter is retained, but this should be considered
deprecated.
Note that module unload is explicitly not yet supported.

Upon resume, it was found that ACPI C-states were missing from non-boot CPUs.
This change registers a syscore_ops handler for this case, and re-uploads the
PM information to the hypervisor to properly reset the C-state on these
processors.

We naively assume that the IRQ value passed in is correct.
If it is not, then any dereference operation for the 'info'
structure will result in crash - so might as well guard ourselves
and sprinkle copious amounts of WARN_ON.

In unmask_evtchn(), when the mask bit is cleared after testing for
pending and the event becomes pending between the test and clear, then
the upcall will not become pending and the event may be lost or
delayed.
Avoid this by always clearing the mask bit before checking for
pending. If a hypercall is needed, remask the event as
EVTCHNOP_unmask will only retrigger pending events if they were
masked.
This fixes a regression introduced in 3.7 by
b5e579232d (xen/events: fix
unmask_evtchn for PV on HVM guests) which reordered the clear mask and
check pending operations.

With the Xen ACPI stub code (CONFIG_XEN_STUB=y) enabled, the power
C and P states are no longer uploaded to the hypervisor.
The reason is that the Xen CPU hotplug code: xen-acpi-cpuhotplug.c
and the xen-acpi-stub.c register themselves as the "processor" type object.
That means the generic processor (processor_driver.c) stops
working and it does not call (acpi_processor_add) which populates the
per_cpu(processors, pr->id) = pr;
structure. The 'pr' is gathered from the acpi_processor_get_info function
which does the job of finding the C-states and figuring out PBLK address.
The 'processors->pr' is then later used by xen-acpi-processor.c (the one that
uploads C and P states to the hypervisor). Since it is NULL, we end
skip the gathering of _PSD, _PSS, _PCT, etc and never upload the power
management data.
The end result is that enabling the CONFIG_XEN_STUB in the build means that
xen-acpi-processor is not working anymore.
This temporary patch fixes it by marking the XEN_STUB driver as
BROKEN until this can be properly fixed.

For MSI-X capable devices the hypervisor wants to write protect the
MSI-X table and PBA, yet it can't assume that resources have been
assigned to their final values at device enumeration time. Thus have
pciback do that notification, as having the device controlled by it is
a prerequisite to assigning the device to guests anyway.
This is the kernel part of hypervisor side commit 4245d33 ("x86/MSI:
add mechanism to fully protect MSI-X table from PV guest accesses") on

With git commit c705c78c0d
"acpi: Export the acpi_processor_get_performance_info" we are now
using a different mechanism to access the P-states.
The acpi_processor per-cpu structure is set and filtered by the
core ACPI code which shrinks the per_cpu contents to only online CPUs.
In the past we would call acpi_processor_register_performance()
which would have not tried to dereference offline cpus.
With the new patch and the fact that the loop we take is for
for_all_possible_cpus we end up crashing on some machines.
We could modify the loop to be for online_cpus - but all the other
loops in the code use possible_cpus (for a good reason) - so lets
leave it as so and just check if per_cpu(processor) is NULL.
With this patch we will bypass the !online but possible CPUs.

The git commit d5aaffa9dd
(cpufreq: handle cpufreq being disabled for all exported function)
tightens the cpufreq API by returning errors when disable_cpufreq()
had been called.
The problem we are hitting is that the module xen-acpi-processor which
uses the ACPI's functions: acpi_processor_register_performance,
acpi_processor_preregister_performance, and acpi_processor_notify_smm
fails at acpi_processor_register_performance with -22.
Note that earlier during bootup in arch/x86/xen/setup.c there is also
an call to cpufreq's API: disable_cpufreq().
This is b/c we want the Linux kernel to parse the ACPI data, but leave
the cpufreq decisions to the hypervisor.
In v3.9 all the checks that d5aaffa9dd
added are now hit and the calls to cpufreq_register_notifier will now
fail. This means that acpi_processor_ppc_init ends up printing:
"Warning: Processor Platform Limit not supported"
and the acpi_processor_ppc_status is not set.
The repercussions of that is that the call to
acpi_processor_register_performance fails right away at:
if (!(acpi_processor_ppc_status & PPC_REGISTERED))
and we don't progress any further on parsing and extracting the _P*
objects.
The only reason the Xen code called that function was b/c it was
exported and the only way to gather the P-states. But we can also
just make acpi_processor_get_performance_info be exported and not
use acpi_processor_register_performance. This patch does so.

Modify the request_module to prefix the file system type with "fs-"
and add aliases to all of the filesystems that can be built as modules
to match.
A common practice is to build all of the kernel code and leave code
that is not commonly needed as modules, with the result that many
users are exposed to any bug anywhere in the kernel.
Looking for filesystems with a fs- prefix limits the pool of possible
modules that can be loaded by mount to just filesystems trivially
making things safer with no real cost.
Using aliases means user space can control the policy of which
filesystem modules are auto-loaded by editing /etc/modprobe.d/*.conf
with blacklist and alias directives. Allowing simple, safe,
well understood work-arounds to known problematic software.
This also addresses a rare but unfortunate problem where the filesystem
name is not the same as it's module name and module auto-loading
would not work. While writing this patch I saw a handful of such
cases. The most significant being autofs that lives in the module
autofs4.
This is relevant to user namespaces because we can reach the request
module in get_fs_type() without having any special permissions, and
people get uncomfortable when a user specified string (in this case
the filesystem type) goes all of the way to request_module.
After having looked at this issue I don't think there is any
particular reason to perform any filtering or permission checks beyond
making it clear in the module request that we want a filesystem
module. The common pattern in the kernel is to call request_module()
without regards to the users permissions. In general all a filesystem
module does once loaded is call register_filesystem() and go to sleep.
Which means there is not much attack surface exposed by loading a
filesytem module unless the filesystem is mounted. In a user
namespace filesystems are not mounted unless .fs_flags = FS_USERNS_MOUNT,
which most filesystems do not set today.

Recently at native Rafael did some cleanup for acpi, say, drop
acpi_bus_add, remove unnecessary argument of acpi_bus_scan,
and run acpi_bus_scan under acpi_scan_lock.
This patch does similar cleanup for xen cpu hotplug, removing
redundant logic, and adding lock.

Dan Carpenter found current xen memory hotplug logic
has potential issue: at func acpi_memory_get_device()
*mem_device = acpi_driver_data(device);
while the device may be NULL and then dereference.
At native side, Rafael recently updated acpi_memory_get_device(),
dropping acpi_bus_add, adding lock, and avoiding above issue.
This patch updates xen memory hotplug logic accordingly, removing
redundant logic, adding lock, and avoiding dereference.
