f33f748c96 powerpc/pseries: Read of-config partition via pstore
This patch set exploits the pstore subsystem to read details of
of-config partition in NVRAM to a separate file in /dev/pstore.
For instance, of-config partition details will be stored in a
file named [of-nvram-5].

Introduce os_partition member in nvram_os_partition structure to identify
if the partition is an os partition or not. This will be useful to handle

d7563c94f7 powerpc/pseries: Read/Write oops nvram partition via pstore
IBM's p series machines provide persistent storage for LPARs through NVRAM.
NVRAM's lnx,oops-log partition is used to log oops messages.
Currently the kernel provides the contents of p-series NVRAM only as a
simple stream of bytes via /dev/nvram, which must be interpreted in user
space by the nvram command in the powerpc-utils package.

This patch set exploits the pstore subsystem to expose oops partition in
NVRAM as a separate file in /dev/pstore. For instance, Oops messages will be
stored in a file named [dmesg-nvram-2]. In case pstore registration fails it
will fall back to kmsg_dump mechanism.

This patch will read/write the oops messages from/to this partition via pstore.

Introduce generic read function to read nvram partitions other than rtas.
nvram_read_error_log will be retained which is used to read rtas partition
from rtasd. nvram_read_partition is the generic read function to read from
any nvram partition.

b1f70e1f72 powerpc/pseries: Add version and timestamp to oops header
Introduce version and timestamp information in the oops header.
oops_log_info (oops header) holds version (to distinguish between old
and new format oops header), length of the oops text
(compressed or uncompressed) and timestamp.

The version field will sit in the same place as the length in old
headers. version is assigned 5000 (greater than oops partition size)
so that existing tools will refuse to dump new style partitions as
the length is too large. The updated tools will work with both
old and new format headers.

5fb621698e powerpc/eeh: Fix fetching bus for single-dev-PE
While running Linux as guest on top of phyp, we possiblly have
PE that includes single PCI device. However, we didn't return
its PCI bus correctly and it leads to failure on recovery from
EEH errors for single-dev-PE. The patch fixes the issue.

b8b3de224f powerpc/eeh: Don't check RTAS token to get PE addr
RTAS token "ibm,get-config-addr-info" or ibm,get-config-addr-info2"
are used to retrieve the PE address according to PCI address, which
made up of domain/bus/slot/function. If we don't have those 2 tokens,
the domain/bus/slot/function would be used as the address for EEH
RTAS operations. Some older f/w might not have those 2 tokens and
that blocks the EEH functionality to be initialized. It was introduced
by commit e2af155c ("powerpc/eeh: pseries platform EEH initialization").

The patch skips the check on those 2 tokens so we can bring up EEH
functionality successfully. And domain/bus/slot/function will be
used as address for EEH RTAS operations.

Adam Lackorzynski reported the following build failure on

The build fails because cpu_down() is defined only under CONFIG_HOTPLUG_CPU.

Looking further, the mobility code in pseries is one of the call-sites which
uses rtas_ibm_suspend_me(), which in turn calls rtas_cpu_state_change_mask().
And the mobility code is unconditionally compiled-in (it does not fall under
any Kconfig option). And commit 120496ac (powerpc: Bring all threads online
prior to migration/hibernation) which introduced this build regression is
critical for the proper functioning of the migration code. So it appears
that the only solution to this problem is to enable CONFIG_HOTPLUG_CPU if
SMP is enabled on PPC_PSERIES platforms. So make that change in the Kconfig.

f1dd153121 powerpc/pseries: Make 32-bit MSI quirk work on systems lacking firmware support
Recent commit e61133dda4 added support
for a new firmware feature to force an adapter to use 32 bit MSIs.
However, this firmware is not available for all systems. The hack below
allows devices needing 32 bit MSIs to work on these systems as well.
It is careful to only enable this on Gen2 slots, which should limit
this to configurations where this hack is needed and tested to work.

b72c1f6514 powerpc: Make radeon 32-bit MSI quirk work on powernv
This moves the quirk itself to pci_64.c as to get built on all ppc64
platforms (the only ones with a pci_dn), factors the two implementations
of get_pdn() into a single pci_get_dn() and use the quirk to do 32-bit
MSIs on IODA based powernv platforms.

120496ac2d powerpc: Bring all threads online prior to migration/hibernation
This patch brings online all threads which are present but not online
prior to migration/hibernation.  After migration/hibernation those
threads are taken back offline.

During migration/hibernation all online CPUs must call H_JOIN, this is
required by the hypervisor.  Without this patch, threads that are offline
(H_CEDE'd) will not be woken to make the H_JOIN call and the OS will be
deadlocked (all threads either JOIN'd or CEDE'd).

d82fb31abc powerpc/pseries: Perform proper max_bus_speed detection
On pseries machines the detection for max_bus_speed should be done
through an OpenFirmware property. This patch adds a function to perform
this detection and a hook to perform dynamic adding of the function only
for pseries. This is done by overwriting the weak
pcibios_root_bridge_prepare function which is called by
pci_create_root_bus().

The following patch implements a new PAPR change which allows
the OS to force the use of 32 bit MSIs, regardless of what
the PCI capabilities indicate. This is required for some
devices that advertise support for 64 bit MSIs but don't
actually support them.

4c23782091 ppc: Clean up scanlog
Clean up the pseries scanlog driver's use of procfs:

 (1) Don't need to save the proc_dir_entry pointer as we have the filename to
     remove with.

 (2) Save the scan log buffer pointer in a static variable (there is only one
     of it) and don't save it in the PDE (which doesn't have a destructor).

b1022fbd29 powerpc: Decode the pte-lp-encoding bits correctly.
We look at both the segment base page size and actual page size and store
the pte-lp-encodings in an array per base page size.

We also update all relevant functions to take actual page size argument
so that we can use the correct PTE LP encoding in HPTE. This should also
get the basic Multiple Page Size per Segment (MPSS) support. This is needed
to enable THP on ppc64.

4edd7ceff0 mm, hotplug: avoid compiling memory hotremove functions when disabled
__remove_pages() is only necessary for CONFIG_MEMORY_HOTREMOVE.  PowerPC
pseries will return -EOPNOTSUPP if unsupported.

Adding an #ifdef causes several other functions it depends on to also
become unnecessary, which saves in .text when disabled (it's disabled in
most defconfigs besides powerpc, including x86).  remove_memory_block()
becomes static since it is not referenced outside of
drivers/base/memory.c.

Build tested on x86 and powerpc with CONFIG_MEMORY_HOTREMOVE both enabled
and disabled.

5d88aa85c0 powerpc/pseries: Update CPU maps when device tree is updated
Platform events such as partition migration or the new PRRN firmware
feature can cause the NUMA characteristics of a CPU to change, and these
changes will be reflected in the device tree nodes for the affected
CPUs.

This patch registers a handler for Open Firmware device tree updates
and reconfigures the CPU and node maps whenever the associativity
changes. Currently, this is accomplished by marking the affected CPUs in
the cpu_associativity_changes_mask and allowing
arch_update_cpu_topology() to retrieve the new associativity information
using hcall_vphn().

Protecting the NUMA cpu maps from concurrent access during an update
operation will be addressed in a subsequent patch in this series.

f0ff7eb483 powerpc/pseries: Update firmware_has_feature() to check architecture vector 5 bits
The firmware_has_feature() function makes it easy to check for supported
features of the hypervisor. This patch extends the capability of
firmware_has_feature() to include checking for specified bits
in vector 5 of the architecture vector as reported in the device tree.

As part of this the #defines used for the architecture vector are re-defined
such that each option has the index into vector 5 and the feature bit encoded
into it. This makes checking for architecture bits when initiating data
for firmware_has_feature much easier.

When iterating over the entries in firmware_features_table we only need
to go over the actual number of entries in the array instead of declaring
it to be bigger and checking to make sure there is a valid entry in every
slot.

This patch removes the FIRMWARE_MAX_FEATURES #define and replaces the
array looping with the use of ARRAY_SIZE().

762ec15707 powerpc/pseries: Expose pseries devicetree_update()
Newer firmware on Power systems can transparently reassign platform resources
(CPU and Memory) in use. For instance, if a processor or memory unit is
predicted to fail, the platform may transparently move the processing to an
equivalent unused processor or the memory state to an equivalent unused
memory unit. However, reassigning resources across NUMA boundaries may alter
the performance of the partition. When such reassignment is necessary, the
Platform Resource Reassignment Notification (PRRN) option provides a
mechanism to inform the Linux kernel of changes to the NUMA affinity of
its platform resources.

When rtasd receives a PRRN event, it needs to make a series of RTAS
calls (ibm,update-nodes and ibm,update-properties) to retrieve the
updated device tree information. These calls are already handled in the
pseries_devicetree_update() routine used in partition migration.

This patch exposes pseries_devicetree_update() to make it accessible
to other pseries routines, this patch also updates pseries_devicetree_update()
to take a 32-bit scope parameter. The scope value, which was previously hard
coded to 1 for partition migration, is used for the RTAS calls
ibm,update-nodes/properties to update the device tree.

61435690a9 powerpc/pseries: close DDW race between functions of adapter
Given a PCI device with multiple functions in a DDW capable slot, the
following situation can be encountered: When the first function sets a
64-bit DMA mask, enable_ddw() will be called and we can fail to properly
configure DDW (the most common reason being the new DMA window's size is
not large enough to map all of an LPAR's memory). With the recent
changes to DDW, we remove the base window in order to determine if the
new window is of sufficient size to cover an LPAR's memory. We correctly
replace the base window if we find that not to be the case. However,
once we go through and re-configured 32-bit DMA via the IOMMU, the next
function of the adapter will go through the same process. And since DDW
is a characteristic of the slot itself, we are most likely going to fail
again. But to determine we are going to fail the second slot, we again
remove the base window -- but that is now in-use by the first
function/driver, which might be issuing I/O already.

To close this window, keep a list of all the failed struct device_nodes
that have failed to configure DDW. If the current device_node is in that
list, just fail out immediately and fall back to 32-bit DMA without
doing any DDW manipulation.

9fb2640159 powerpc: pSeries_lpar_hpte_remove fails from Adjunct partition being performed before the ANDCOND test
Some versions of pHyp will perform the adjunct partition test before the
ANDCOND test.  The result of this is that H_RESOURCE can be returned and
cause the BUG_ON condition to occur. The HPTE is not removed.  So add a
check for H_RESOURCE, it is ok if this HPTE is not removed as
pSeries_lpar_hpte_remove is looking for an HPTE to remove and not a
specific HPTE to remove.  So it is ok to just move on to the next slot
and try again.

pseries/iommu: remove DDW on kexec

We currently insert a property in the device-tree when we successfully
configure DDW for a given slot. This was meant to be an optimization to
speed up kexec/kdump, so that we don't need to make the RTAS calls again
to re-configured DDW in the new kernel.

However, we end up tripping a plpar_tce_stuff failure on kexec/kdump
because we unconditionally parse the ibm,dma-window property for the
node at bus/dev setup time. This property contains the 32-bit DMA window
LIOBN, which is distinct from the DDW window's. We pass that LIOBN (via
iommu_table_init -> iommu_table_clear -> tce_free ->
tce_freemulti_pSeriesLP) to plpar_tce_stuff, which fails because that
32-bit window is no longer present after
25ebc45b93 ("powerpc/pseries/iommu: remove
default window before attempting DDW manipulation").

I believe the simplest, easiest-to-maintain fix is to just change our
initcall to, rather than detecting and updating the new kernel's DDW
knowledge, just remove all DDW configurations. When the drivers
re-initialize, we will set everything back up as it was before.

71cf1def1a pseries/iommu: Ensure TCEs are cleared with non-huge DDW
There are now two kinds of DMA windows that might be presented by
PowerVM DDW support -- huge windows (that can map all of system memory
regardless of the LPAR configuration) and non-huge windows (which
can't). They are implemented slightly differently in PowerVM, and thus
have different characteristics. The most obvious is that slot isolate
doesn't clear the TCEs/window for us with non-huge windows. Thus, when a
DLPAR operation occurs on a slot using a non-huge window, TCEs are still
present (the notifier chain doesn't currently remove them explicitly)
and the DLPAR fails. Fix this by calling remove_ddw() first, which will
unmap the DDW TCEs.

Note: a corresponding change to drmgr is needed to actually successfully
DLPAR, such that the device-tree update (which causes the notifier chain
to fire) occurs before slot isolate.

a given range. However, is it not advancing the dma_offset value passed

abf917cd91 cputime: Generic on-demand virtual cputime accounting
If we want to stop the tick further idle, we need to be
able to account the cputime without using the tick.

Virtual based cputime accounting solves that problem by
hooking into kernel/user boundaries.

However implementing CONFIG_VIRT_CPU_ACCOUNTING require
low level hooks and involves more overhead. But we already
have a generic context tracking subsystem that is required
for RCU needs by archs which plan to shut down the tick
outside idle.

This patch implements a generic virtual based cputime
accounting that relies on these generic kernel/user hooks.

There are some upsides of doing this:

- This requires no arch code to implement CONFIG_VIRT_CPU_ACCOUNTING
if context tracking is already built (already necessary for RCU in full
tickless mode).

- We can rely on the generic context tracking subsystem to dynamically
(de)activate the hooks, so that we can switch anytime between virtual
and tick based accounting. This way we don't have the overhead
of the virtual accounting when the tick is running periodically.

And one downside:

- There is probably more overhead than a native virtual based cputime
accounting. But this relies on hooks that are already set anyway.

6a040ce725 powerpc/eeh: Fix crash when adding a device in a slot with DDW
The DDW code uses a eeh_dev struct from the pci_dev. However, this is
not set until eeh_add_device_late is called.

Since pci_bus_add_devices is called before eeh_add_device_late, the PCI
devices are added to the bus, making drivers' probe hooks to be called.
These will call set_dma_mask, which will call the DDW code, which will
require the eeh_dev struct from pci_dev. This would result in a crash,
due to a NULL dereference.

Calling eeh_add_device_late after pci_bus_add_devices would make the
system BUG, because device files shouldn't be added to devices there
were not added to the system. So, a new function is needed to add such
files only after pci_bus_add_devices have been called.

a413f474a0 powerpc: Disable relocation on exceptions whenever PR KVM is active
For PR KVM we allow userspace to map 0xc000000000000000. Because
transitioning from userspace to the guest kernel may use the relocated
exception vectors we have to disable relocation on exceptions whenever
PR KVM is active as we cannot trust that address.

This issue does not apply to HV KVM, since changing from a guest to the
hypervisor will never use the relocated exception vectors.

Currently the hypervisor interface only allows us to toggle relocation
on exceptions on a partition wide scope, so we need to globally disable
relocation on exceptions when the first PR KVM instance is started and

It's a bit heavy handed, but until the hypervisor gives us a lightweight
way to toggle relocation on exceptions on a single thread it's only real

1fbe9cf259 powerpc: Build kernel with -mcmodel=medium
Finally remove the two level TOC and build with -mcmodel=medium.

Unfortunately we can't build modules with -mcmodel=medium due to
the tricks the kernel module loader plays with percpu data:

# -mcmodel=medium breaks modules because it uses 32bit offsets from
# the TOC pointer to create pointers where possible. Pointers into the
# percpu data area are created by this method.

# The kernel module loader relocates the percpu data section from the
# original location (starting with 0xd...) to somewhere in the base
# kernel percpu data space (starting with 0xc...). We need a full
# 64bit relocation for this to work, hence -mcmodel=large.

On older kernels we fall back to the two level TOC (-mminimal-toc)

e5e84f0a66 powerpc: Hook up doorbells on server
This patch actually hooks up doorbell interrupts on POWER8:

- Select the PPC_DOORBELL Kconfig option from PPC_PSERIES

- Add the doorbell CPU feature bit to POWER8

- We define a new pSeries_cause_ipi_mux() function that issues a
  doorbell interrupt if the recipient is another thread within the same
  core as the sender. If the recipient is in a different core it falls
  back to using XICS to deliver the IPI as before.

- During pSeries_smp_probe() at boot, we check if doorbell interrupts
  are supported. If they are we set the cause_ipi function pointer to
  the above mentioned function, otherwise we leave it as whichever XICS
  cause_ipi function was determined by xics_smp_probe().

a474a51549 cpuidle: Measure idle state durations with monotonic clock
Many cpuidle drivers measure their time spent in an idle state by
reading the wallclock time before and after idling and calculating the
difference. This leads to erroneous results when the wallclock time gets
updated by another processor in the meantime, adding that clock
adjustment to the idle state's time counter.

If the clock adjustment was negative, the result is even worse due to an
erroneous cast from int to unsigned long long of the last_residency
variable. The negative 32 bit integer will zero-extend and result in a
forward time jump of roughly four billion milliseconds or 1.3 hours on
the idle state residency counter.

This patch changes all affected cpuidle drivers to either use the
monotonic clock for their measurements or make use of the generic time
measurement wrapper in cpuidle.c, which was already working correctly.
Some superfluous CLIs/STIs in the ACPI code are removed (interrupts
should always already be disabled before entering the idle function, and
not get reenabled until the generic wrapper has performed its second
measurement). It also removes the erroneous cast, making sure that
negative residency values are applied correctly even though they should
not appear anymore.

e716e01438 powerpc/eeh: Do not invalidate PE properly
While the EEH does recovery on the specific PE that has PCI errors,
the PCI devices belonging to the PE will be removed and the PE will
be marked as invalid since we still need the information stored in
the PE. We only invalidate the PE when it doesn't have associated
EEH devices and valid child PEs. However, the code used to check
that is wrong. The patch fixes that.

kmalloc+memcpy is replaced with kstrdup() to avoid modifying the string.

Since we don't know if they new kernel we are kexecing into has been
built to support relocation on exceptions, we disable them before we
kexec.

We do NOT disable them if we are execing a kdump kernel, because we
want to change as little state as possible and it is likely that we are
execing ourselves and will be able to handle them anyway.

We currently do this synchronously at boot from setup_arch. On a large
system this could hypothetically take a little while to complete, so
currently we will give up if we are asked to wait for more than a second
in total.

If we actually start hitting that timeout in practice we can always move
this code into a kernel thread to take care of it in the background.

1cf3d8b3d2 powerpc+of: Add of node/property notification chain for adds and removes
This patch moves the notification chain for updates to the device tree
from the powerpc/pseries code to the base OF code. This makes this
functionality available to all architectures.

Additionally the notification chain is updated to allow notifications
for property add/remove/update. To make this work a pointer to a new
struct (of_prop_reconfig) is passed to the routines in the notification chain.
The of_prop_reconfig property contains a pointer to the node containing the
property and a pointer to the property itself. In the case of property
updates, the property pointer refers to the new property.

83dac59409 cpuidle/powerpc: Fix snooze state problem in the cpuidle design on pseries.
Earlier without cpuidle framework on pseries, the native arch
idle routine comprised of both snooze and nap
states.  smt_snooze_delay variable was used to delay
the idle process entry to deeper idle state like  nap.
With the coming of cpuidle, this arch specific idle was replaced
by two different idle routines, one for supporting snooze and other
for nap. This enabled addition of more
low level idle states on pseries in the future.

On adopting the generic cpuidle framework for POWER systems,
the decision of which idle state to choose from,  given a predicted
idle time is taken by the menu governor based on
target_residency and  exit_latency of the idle states.
target_residency is the minimum time to be resident in that idle state.
Exit_latency is time taken to exit out of idle state.
Deeper the idle state, both the target residency and exit latency
would be higher.

In the current design, smt_snooze_delay is used as target_residency
for the  snooze state which is incorrect, as it is not the
minimum but the maximum duration to be in snooze state.
This would  result in the governor in taking bad decision,
as presently target_residency of nap < target_residency of snooze
inspite of nap being deeper idle state.

This patch aims to fix this problem by replacing the smt_snooze_delay loop
in snooze state, with the need_resched()  as the governor is aware of
entry and exit of various idle transitions based on which
next idle time prediction.

The governor is intelligent enough to determine the idle state the needs to
be transitioned to and maintains a whole of heuristics including
io load, previous idle states predictions etc for the same, based on
which idle state entry decision is taken.

With this fix, of setting target_residency of snooze to 0
					     nap to smt_snooze_delay
if the predicted idle time is less
than smt_snooze_delay (target_residency of nap)
value governor would pick snooze state, else nap. This adhers to the
previous native idle design.

8ea959a17f cpuidle/powerpc: Fix smt_snooze_delay functionality.
smt_snooze_delay was designed to  delay idle loop's nap entry
in the native idle code before it got  ported over to use as part of
the cpuidle framework.

A -ve value  assigned to smt_snooze_delay should result in
busy looping, in other words disabling the entry to nap state.

This particular functionality can be achieved currently by
echo 1 > /sys/devices/system/cpu/cpu*/state1/disable
but it is broken when one assigns -ve value to  the smt_snooze_delay
variable either via sysfs entry or ppc64_cpu util.

This patch aims to fix this, by disabling nap state when smt_snooze_delay
variable is set to -ve value.

This is currently over-writing the residency time updated as part of the static

When our x86 box calls __remove_pages(), release_mem_region() shows many
warnings.  And x86 box cannot unregister iomem_resource.

release_mem_region() has been changed to be called in each
PAGES_PER_SECTION by commit de7f0cba96 ("memory hotplug: release
memory regions in PAGES_PER_SECTION chunks").  Because powerpc registers
iomem_resource in each PAGES_PER_SECTION chunk.  But when I hot add
memory on x86 box, iomem_resource is register in each _CRS not
PAGES_PER_SECTION chunk.  So x86 box unregisters iomem_resource.

The patch fixes the problem.

1e38b71401 powerpc/eeh: Fix crash on converting OF node to edev
The kernel crash was reported by Alexy. He was testing some feature
with private kernel, in which Alexy added some code in pci_pm_reset()
to read the CSR after writting it. The bug could be reproduced on
Fiber Channel card (Fibre Channel: Emulex Corporation Saturn-X:
LightPulse Fibre Channel Host Adapter (rev 03)) by the following
commands.

	# echo 1 > /sys/devices/pci0004:01/0004:01:00.0/reset
	# rmmod lpfc
	# modprobe lpfc

The history behind the test case is that those additional config
space reading operations in pci_pm_reset() would cause EEH error,
but we didn't detect EEH error until "modprobe lpfc". For the case,
all the PCI devices on PCI bus (0004:01) were removed and added after
PE reset. Then the EEH devices would be figured out again based on
the OF nodes. Unfortunately, there were some child OF nodes under
PCI device (0004:01:00.0), but they didn't have attached PCI_DN since
they're invisible from PCI domain. However, we were still trying to
convert OF node to EEH device without checking on the attached PCI_DN.
Eventually, it caused the kernel crash as follows:

We have missed lots of situations where the PE hierarchy tree need
protection through the EEH global mutex. The patch fixes that for
those public APIs implemented in eeh_pe.c. The only exception is

has been protected by the mutex.

Function eeh_rmv_from_parent_pe() could be called by the path of
either normal PCI hotplug, or EEH recovery. For the former case,
we need purge the corresponding PE on removal of the associated
PE bus.

The patch tries to cover that by passing more information to function
pcibios_remove_pci_devices() so that we know if the corresponding PE
needs to be purged or be marked as "invalid".

5efc3ad732 powerpc/eeh: Introduce EEH_PE_INVALID type PE
When EEH error happens on the PE whose PCI devices don't have
attached drivers. In function eeh_handle_event(), the default
value PCI_ERS_RESULT_NONE will be returned after iterating all
drivers of those PCI devices belonging to the PE. Actually, we
don't have installed drivers for the PCI devices. Under the
circumstance, we will remove the corresponding PCI bus of the PE,
including the associated EEH devices and PE instance. However,
we still need the information stored in the PE instance to do PE
reset after that. So it's unsafe to free the PE instance.

The patch introduces EEH_PE_INVALID type PE to address the issue.
When the PCI bus and the corresponding attached EEH devices are
removed, we will mark the PE as EEH_PE_INVALID. At later point,
the PE will be changed to EEH_PE_DEVICE or EEH_PE_BUS when the
corresponding EEH devices are attached again.

(hypervisor/kernel/user and then filter in software.  We can be a lot
smarter now that we can set the DABRX dynamically.

Rework set_dabr to take a DABRX value as well.

Both the pseries and PS3 hypervisors do some checks on the DABRX
values that are passed in the hcall.  This patch stops bogus values
from being passed to hypervisor.  Also, in the case where we are
clearing the breakpoint, where DABR and DABRX are zero, we modify the
DABRX value to make it valid so that the hcall won't fail.

d7bb88629d powerpc/eeh: Probe mode support
While EEH module is installed, PCI devices is checked one by one
to see if it supports eeh. On different platforms, the PCI devices
are referred through different ways when the EEH module is loaded.
For example, on pSeries platform, that is done by OF node. However,
we would do that by real PCI devices (struct pci_dev) on PowerNV
platform in future. So we needs some mechanism to differentiate
those cases by classifying them to probe modes, either from OF
nodes or real PCI devices.

The patch implements the support to eeh probe mode. Also, the
EEH on pSeries has set it into EEH_PROBE_MODE_DEVTREE. That means
the probe will be done based on OF nodes on pSeries platform.

In addition, On pSeries platform, it's done by OF nodes. The patch
moves the the probe function from EEH core to platform dependent
backend and some cleanup applied.

Once eeh error is found, eeh event will be created and put it into
the global linked list. At the mean while, kernel thread will be
started to process it. The handler for the kernel thread originally
was eeh device sensitive.

sensitive.

Originally, all the EEH operations were implemented based on OF node.

Actually, it explicitly breaks the rules that the operation target
is PE instead of device. Therefore, the patch makes all the operations

66523d9f2d powerpc/eeh: Trace error based on PE from beginning
There're 2 conditions to trigger EEH error detection: invalid value
returned from reading I/O or config space. On each case, the function
eeh_dn_check_failure will be called to initialize EEH event and put
it into the poll for further processing.

The patch changes the function for a little bit so that the EEH error
will be traced based on PE instead of EEH device any more. Also, the
function eeh_find_device_pe() has been removed since the eeh device
is tracing the PE by struct eeh_dev::pe.

Since we've introduced dedicated struct to trace individual PEs,
it's reasonable to trace its state through the dedicated struct
instead of using "eeh_dev" any more.

The patches implements the state tracing based on PE. It's notable
that the PE state will be applied to the specified PE as well as
its child PEs. That complies with the rule that problematic parent
PE will prevent those child PEs from working properly.

During PCI hotplug and EEH recovery, the PE hierarchy tree might be
changed due to the PCI topology changes. At later point when the
PCI device is added, the PE will be created dynamically again.

The patch introduces new function to remove EEH devices from the
associated PE. That also can cause that the parent PE is removed
from the PE tree if the parent PE doesn't include valid EEH devices
and child PEs.

The patch creates PEs and associated the newly created PEs with
it parent/silbing as well as EEH devices. It would become more
straight to trace EEH errors and recover them accordingly.

Once the EEH functionality on one PCI IOA has been enabled, we
tries to create PE against it. If there's existing PE, to which
the current PCI IOA should be attached, the existing PE will be
converted from "device" type to "bus" type accordingly.

For one particular PE, it's only meaningful in the ancestor PHB
domain. Therefore, each PHB should have its own PE hierarchy tree
to trace those PEs created against the PHB.

would be first level of overall PE hierarchy tree across the

are ready. So we use slab/slub to allocate the memory chunks for newly
creatd EEH devices. That would save lots of memory.

that we needn't clear the allocated memory chunk explicitly.

Currently, we have 3 phases for EEH initialization on pSeries platform.
All of them are done through builtin functions: platform initialization,
EEH device creation, and EEH subsystem enablement. All of them are done
no later than ppc_md.setup_arch. That means that the slab/slub isn't ready
yet, so we have to allocate memory chunks on basis of PAGE_SIZE for those
dynamically created EEH devices. That's pretty expensive.

In order to utilize slab/slub for memory allocation, we have to move the EEH
initialization functions around, but all of them should be called after slab
is ready.

752f5216f1 powerpc/pseries: Round up MSI-X requests
The pseries firmware currently refuses any non power of two MSI-X
request. Unfortunately most network drivers end up asking for that
because they want a power of two for RX queues and one or two extra
for everything else.

This patch rounds up the firmware request to the next power of two
if the quota allows it. If this fails we fall back to using the
original request size.

handle_eeh_events() without checking, causing a crash if NULL was
returned, which is expected in some situations.

Currently the call to pseries_notify_cpuidle_add_cpu(), that takes
action on the cpuidle front when a cpu is added/removed
is being made from smp_xics_setup_cpu().
This caused lockdep issues as

On addition of each cpu,
resources were cleared and re-allocated each time, all in critical
section as part of start_secondary() call were interrupts are disabled.
To resolve this issue, the pseries_notify_cpuidle_add_cpu() call is
is being replaced by a hotplug notifier which
would prevent cpuidle resources from being
released and allocated each time cpu is onlined in the critical code path.
It was fixed in https://lkml.org/lkml/2012/5/18/174.

Also it is essential to call cpuidle_enable/disable_device
between  cpuidle_pause_and_lock()  and
cpuidle_resume_and_unlock()  when used externally
to avoid race conditions. Add support for CPU_ONLINE_FROZEN
and CPU_DEAD_FROZEN as part of hotplug notify event for
pseries_idle  and unregister hotplug notifier
while exiting out. The above mentioned issues
are fixed as part of this patch.

c75df6f96c powerpc: Fix usage of register macros getting ready for %r0 change
Anything that uses a constructed instruction (ie. from ppc-opcode.h),
need to use the new R0 macro, as %r0 is not going to work.

Also convert usages of macros where we are just determining an offset
(usually for a load/store), like:

Can't use STK_REG(r14) as %r14 doesn't work in the STK_REG macro since
it's just calculating an offset.

Looks like we still have issues with pSeries and Cell idle code
vs. the lazy irq state. In fact, the reset fixes that went upstream
are exposing the problem more by causing BUG_ON() to trigger (which
this patch turns into a WARN_ON instead).

We need to be careful when using a variant of low power state that
has the side effect of turning interrupts back on, to properly set
all the SW & lazy state to look as if everything is enabled before
we enter the low power state with MSR:EE off as we will return with
MSR:EE on. If not, we have a discrepancy of state which can cause
things to go very wrong later on.

This patch moves the logic into a helper and uses it from the
pseries and cell idle code. The power4/970 idle code already got
things right (in assembly even !) so I'm not touching it. The power7
"bare metal" idle code is subtly different and correct. Remains PA6T
and some hypervisor based Cell platforms which have questionable
code in there, but they are mostly dead platforms so I'll fix them
when I manage to get final answers from the respective maintainers
about how the low power state actually works on them.

c1703e85a7 powerpc/pseries: Disable interrupts around IOMMU percpu data accesses
tce_buildmulti_pSeriesLP uses a per cpu page to communicate with the
hypervisor. We currently rely on the IOMMU table spinlock but
subsequent patches will be removing that so disable interrupts
around all accesses of tce_page.

The following patch is to remove the pseries_notify_add_cpu() call
and replace it by a hot plug notifier.

This would prevent cpuidle resources being released and allocated each

25ebc45b93 powerpc/pseries/iommu: remove default window before attempting DDW manipulation
An upcoming release of firmware will add DDW extensions, in particular
an API to "reset" the DMA window to the original configuration (32-bit,
2GB in size). With that API available, we can safely remove the default
window, increasing the resources available to firmware for creation of
larger windows for the slot in question -- if we encounter an error, we
can use the new API to reset the state of the slot.

Further, this same release of firmware will make it a hard requirement
for OSes to release the existing window before any other windows will be
shown as available, to avoid conflicts in addressing between the two
windows.

In anticipation of these changes, always remove the default window
before we do any DDW manipulations.

0b17ba7258 powerpc: check_and_cede_processor() never cedes
Commit f948501b36 ("Make hard_irq_disable() actually hard-disable
interrupts") caused check_and_cede_processor to stop working.
->irq_happened will never be zero right after a hard_irq_disable
so the compiler removes the call to cede_processor completely.

The bug was introduced back in the lazy interrupt handling rework
of 3.4 but was hidden until recently because hard_irq_disable did
nothing.

This issue will eventually appear in 3.4 stable since the
hard_irq_disable fix is marked stable, so mark this one for stable
too.

e2ae715d66 kmsg - kmsg_dump() use iterator to receive log buffer content
Provide an iterator to receive the log buffer content, and convert all
kmsg_dump() users to it.

The structured data in the kmsg buffer now contains binary data, which
should no longer be copied verbatim to the kmsg_dump() users.

The iterator should provide reliable access to the buffer data, and also
supports proper log line-aware chunking of data while iterating.

This adds an update notifier mechanism for changes to properties in the
device tree.  One use of this would be a device driver that needs to act
on changes to it's properties in the device tree after a live migration
or a dynamic activation that is triggered by updates to ofdt properties.

14fb1fa6e2 powerpc: Use WARN instead of dump_stack when printing EEH error backtrace
When we get an EEH error we just print a backtrace with dump_stack
which is rather cryptic. We really should print something before
spewing out the backtrace.

Also switch from dump_stack to WARN so we get more information about
the fail - what modules were loaded, what process was running etc.
This was useful information when debugging a recent EEH subsystem bug.

The standard WARN output should also get picked up by monitoring
tools like kerneloops.

The register dump is of questionable value here but I figured it was
better to use something standard and not roll my own.

Recently, Ryan Wang tried to compile PPC pSeries platform without
CONFIG_EEH and eventually run into errors. Nishanth Aravamudan
helped to narrow down the root cause. Actually, the pSeries platform
depends on CONFIG_EEH heavily and that won't work properly without
EEH support.

According to Ben's suggestion, the patch make CONFIG_EEH invisible
and keep it as always selected on pSeries platform.

2ef822c553 powerpc/eeh: Fix crash caused by null eeh_dev
The problem was reported by Anton Blanchard. While EEH error
happened to the PCI device without the corresponding device
driver, kernel crash was seen. Eventually, I successfully
reproduced the problem on Firebird-L machine with utility
"errinjct". Initially, the device driver for Emulex ethernet
MAC has been disabled from .config and force data parity on
the Emulex ethernet MAC with help of "errinjct". Eventually,
I saw the kernel crash after issueing couple of "lspci -v"
command.

The root cause behind is that the PCI device, including the
reference to the corresponding eeh device, will be removed
from the system while EEH does recovery. Afterwards, the
PCI device will be probed again and added into the system
accordingly. So it's not safe to retrieve the eeh device from
the corresponding PCI device after the PCI device has been removed
and not added again.

The patch fixes the issue and retrieve the eeh device from OF node
instead of PCI device after the PCI device has been removed.
