During prereset, -ENODEV return from ata_wait_ready() is not an error.

Some SATA controllers (sata_sil) use 0xff to indicate port not ready
status, not port empty. As libata interprets 0xff as port empty, this
causes unnecessary reset failure and retry. Don't consider 0xff as
port empty if SStatus is available and indicates that port is online.

With STANDBYDOWN tracking added, libata.spindown_compat isn't
necessary anymore. If userspace shutdown(8) issues STANDBYNOW, libata
warns. If userspace shutdown(8) doesn't issue STANDBYNOW, libata does
the right thing. Userspace can tell whether kernel supports spindown
by testing whether sysfs node manage_start_stop exists as before.

As with all other drivers, sata_nv's hpriv is allocated with
devm_kzalloc() and there's no need to free it explicitly. Kill
nv_remove_one() which incorrectly used kfree() instead of devm_kfree()
and use ata_pci_remove_one() directly.

Because nvidia SATA controllers onward base on AHCI, so wildcard in sata_nv
driver is unnecessary. Also the wildcard sometimes cause sata_nv driver to
be loaded for AHCI controllers,which is not as expected.

Our assumption that most distros issue STANDBYNOW seems wrong. The
upstream sysvinit and thus many distros including gentoo and opensuse
don't take any action for libata disks on spindown. We can skip
compat handling for these distros so that they don't need to update
anything to take advantage of kernel-side shutdown.

Unlocking ap->lock and ssleeping don't work because SCSI commands can
be issued from completion path without context. Reimplement delayed
completion by allowing translation functions to override
qc->scsidone(), storing the original completion function to
scmd->scsi_done() and overriding qc->scsidone() with a function which
schedules delayed invocation of scmd->scsi_done().
This isn't pretty at all but all the ugly parts are thankfully
contained in the stop translation path where the compat feature is
implemented.

Whether a controller needs IDE or SATA ACPI hierarchy is determined by
the programming interface of the controller not by whether the
controller is SATA or PATA, or it supports slave device or not. This
patch adds ATA_FLAG_ACPI_SATA port flags which tells libata-acpi that
the port needs SATA ACPI nodes, and sets the flag for ahci and
sata_sil24.

Device might be resized during ata_dev_configure() due to HPA or
(later) ACPI _GTF. Currently it's worked around by caching n_sectors
before turning off HPA. The cached original size is overwritten if
the device is reconfigured without being hardreset - which always
happens after configuring trasnfer mode. If the device gets hardreset
for some reason after that, revalidation fails with -ENODEV.
This patch makes size checking more robust by moving n_sectors check
from ata_dev_reread_id() to ata_dev_revalidate() after the device is
fully configured. No matter what happens during configuration, a
device must have the same n_sectors after fully configured to be
treated as the same device.

Separate out ata_dev_reread_id() from ata_dev_revalidate().
ata_dev_reread_id() reads IDENTIFY page and determines whether the
same device is still there. ata_dev_revalidate() reconfigures after
reread completes. This will be used by ACPI update.

I have a system where I have a simple IDE controller that sits on a
local bus without bus master dma capability, and thus no dma_mapping
ops defined for the device/bus.
pata_platform works great for me, with the exception of using the generic
ata_port_start which tries to do a dmam_alloc_coherent.
Looks like it doesn't need to allocate a prd table at all, so replace it
with a dummy function instead.

After certain errors, some devices report complete garbage on
IDENTIFY. This can cause ata_dev_read_id() to fail with -EINVAL
resulting in immediate disabling of the device. Give the device one
last chance after -EINVAL to allow recovery from such situations. As
-EINVAL is triggered very rarely, this shouldn't cause any noticeable
affect on more common error paths.

It seems the world isn't as frank as we thought and some devices lie
about who they are. Fallback to the other IDENTIFY if IDENTIFY is
aborted by the device. As this is the strategy used by IDE for a long
time, it shouldn't cause too much problem.

libata enables SCSI host during ATA host activation which happens
after IRQ handler is registered and IRQ is enabled. All ATA ports are
in frozen state when IRQ is enabled but frozen ports may raise limited
number of IRQs after being frozen - IOW, ->freeze() is not responsible
for clearing pending IRQs. During normal operation, the IRQ handler
is responsible for clearing spurious IRQs on frozen ports and it
usually doesn't require any extra code.
Unfortunately, during host initialization, the IRQ handler can end up
scheduling EH for a port whose SCSI host isn't initialized yet. This
results in OOPS in the SCSI midlayer. This is relatively short window
and scheduling EH for probing is the first thing libata does after
initialization, so ignoring EH scheduling until initialization is
complete solves the problem nicely.
This problem was spotted by Berck E. Nash in the following thread.

The intention of using port_mask in SFF init helpers was to eventually
support exoctic configurations such as combination of legacy and
native port on the same controller. This never became actually
necessary and the related code always has been subtly broken one way
or the other. Now that new init model is in place, there is no reason
to make common helpers capable of handling all corner cases. Exotic
cases can simply dealt within LLDs as necessary.
This patch removes port_mask handling in SFF init helpers. SFF init
helpers don't take n_ports argument and interpret it into port_mask
anymore. All information is carried via port_info. n_ports argument
is dropped and always two ports are allocated. LLD can tell SFF to
skip certain port by marking it dummy. Note that SFF code has been
treating unuvailable ports this way for a long time until recent
breakage fix from Linus and is consistent with how other drivers
handle with unavailable ports.
This fixes 1-port legacy host handling still broken after the recent
native mode fix and simplifies SFF init logic. The following changes
are made...
* ata_pci_init_native_host() and ata_init_legacy_host() both now try
to initialized whatever they can and mark failed ports dummy. They
return 0 if any port is successfully initialized.
* ata_pci_prepare_native_host() and ata_pci_init_one() now doesn't
take n_ports argument. All info should be specified via port_info
array. Always two ports are allocated.
* ata_pci_init_bmdma() exported to be used by LLDs in exotic cases.
* port_info handling in all LLDs are standardized - all port_info
arrays are const stack variable named ppi. Unless the second port
is different from the first, its port_info is specified as NULL
(tells libata that it's identical to the last non-NULL port_info).
* pata_hpt37x/hpt3x2n: don't modify static variable directly. Make an
on-stack copy instead as ata_piix does.
* pata_uli: It has 4 ports instead of 2. Don't use
ata_pci_prepare_native_host(). Allocate the host explicitly and use
init helpers. It's simple enough.

Now that libata uses sd->manage_start_stop, libata spins down disk on
shutdown. In an attempt to compensate libata's previous shortcoming,
some distros sync and spin down disks attached via libata in their
shutdown(8). Some disks spin back up just to spin down again on
STANDBYNOW1 if the command is issued when the disk is spun down, so
this double spinning down causes problem.
This patch implements module parameter libata.spindown_compat which,
when set to one (default value), prevents libata from spinning down
disks on shutdown thus avoiding double spinning down. Note that
libata spins down disks for suspend to mem and disk, so with
libata.spindown_compat set to one, disks should be properly spun down
in all cases without modifying shutdown(8).
shutdown(8) should be fixed eventually. Some drive do spin up on
SYNCHRONZE_CACHE even when their cache is clean. Those disks
currently spin up briefly when sd tries to shutdown the device and
then the machine powers off immediately, which can't be good for the
head. We can't skip SYNCHRONIZE_CACHE during shudown as it can be
dangerous data integrity-wise.
So, this spindown_compat parameter is already scheduled for removal by
the end of the next year and here's what shutdown(8) should do.
* Check whether /sys/modules/libata/parameters/spindown_compat
exists. If it does, write 0 to it.
* For each libata harddisk {
* Check whether /sys/class/scsi_disk/h:c:i:l/manage_start_stop
exists. Iff it doesn't, synchronize cache and spin the disk
down as before.
}
The above procedure will make shutdown(8) work properly with kernels
before this change, ones with this workaround and later ones without
it.
To accelerate shutdown(8) updates, if the compat mode is in use, this
patch prints BIG FAT warning for five seconds during shutdown (the
optimal interval to annoy the user just the right amount discovered by
hours of tireless usability testing).

Reimplement suspend/resume support using sdev->manage_start_stop.
* Device suspend/resume is now SCSI layer's responsibility and the
code is simplified a lot.
* DPM is dropped. This also simplifies code a lot. Suspend/resume
status is port-wide now.
* ata_scsi_device_suspend/resume() and ata_dev_ready() removed.
* Resume now has to wait for disk to spin up before proceeding. I
couldn't find easy way out as libata is in EH waiting for the
disk to be ready and sd is waiting for EH to complete to issue
START_STOP.
* sdev->manage_start_stop is set to 1 in ata_scsi_slave_config().
This fixes spindown on shutdown and suspend-to-disk.

Rather than having every driver fiddle about setting its private
IRQ operations and data, provide a helper function to contain
this functionality in one place.
Arrange to remove the driver-private IRQ operations and data when
the device is removed from the driver, and remove the driver
private code to do this.
This fixes potential problems caused by drivers forgetting to
remove these hooks.

This patch fixes some problems with ADMA-capable controllers with
regard to freeze, thaw and irq_clear libata callbacks. Freeze and
thaw didn't switch the ADMA-specific interrupts on or off, and more
critically the irq_clear function didn't respect the restriction that
the notifier clear registers for both ports have to be written at
the same time even when only one port is being cleared. This could
result in timeouts on one port when error handling (i.e. as a result
of hotplug) occurred on the other port.
As well, this fixes some issues in the interrupt handler: we shouldn't
check any ADMA status if the port has ADMA switched off because of
an ATAPI device, and it also checks to see if any ADMA interrupt has
been raised even when we are in port-register mode.

There is a known problem with sata_promise on SATAII-150/300 TX4
controller cards: it enumerates drives in an order that differs
from the port numbers printed on the controller cards. However,
Promise's BIOS and Linux driver both get the order right.
I investigated Promise's Linux driver (v1.01.0.23), and found
that it explicitly changes the mapping from logical port number
to ATA engine MMIO address on the SATAII TX4 cards. It does this
on all SATAII TX4 cards, without inspecting revision etc. The
SATAII TX2plus cards continue to use the same mapping that was
used for the first-generation chips.
This patch updates sata_promise to use the new port number to
ATA engine mapping on SATAII TX4 cards, which fixes the drive
enumeration order problem on those cards. Tested on several
1st and 2nd generation TX2plus and TX4 chips.

The sata_promise error decode update changed pdc_host_intr()
to return and not complete the qc after detecting an error.
Unfortunately not completing the qc:s causes them to always
time out on error, which is wrong and has nasty side-effects.
This patch updates pdc_error_intr() to call ata_port_abort(),
similar to ahci and sata_sil24. Doing this is important as it
makes EH see the original error and not a bogus timeout.

libata previously depended upon waits in prereset to get resets after
hotplug right for both spin up and device ready wait. This was
necessary both for reliablity and speed as reset was likely to fail if
initiated too early and each try usually took more than 30secs to
fail. Previous patches fixed the reliability part by fixing status
and SCR handling in resets. This patch remedies the speed part by
improving reset sequencing.
Prereset waiting timeout is adjusted to 10s because spinup wait is
replaced by reset sequencing and !BSY wait is not as important as
before. During boot or module loading where the drive is already
fully spun up, !BSY wait succeeds immediately, so 10s should be enough
in most cases. It matters after hotplugging or other error
conditions, but in those cases, !BSY wait in prereset simply can't be
relied upon due to the varied and weird behaviors ATA controllers and
devices show.
Reset is now driven by ata_eh_reset_timeouts[] table which contains
timeouts for each reset try. The first reset can be softreset but the
following ones are always hardreset if available. Each timeout
defines deadline for the reset try. If a reset try fails, reset is
retried with the next timeout till the end of the timeout table is
reached. If a reset try fails before the timeout with error, libata
waits till the deadline of the failed try before retrying.
IOW, the timeout table defines timetable of reset tries such that the
n'th try always begins at least after the sum of all previous timeouts
has passed. The current timetable defines 4 tries and takes around 1
minute.
@0 : First try. This should succeed most of the time during boot.
@10 : 10s is enough to spin up most consumer harddrives. Give it
another shot.
@20 : 20s should spin up > 99% of working drives. This has 30s
timeout for retarded devices needing long idleness post reset.
@55 : Final try with 5s timeout just in case.
The above timetable is trade off between not annoying the device too
much with frequent resets and taking reasonable amount of time in most
cases. Some controllers may do better with shorter timeouts while
others may fare better with longer but we just can't rely upon LLD
writers to test each controller with wide variety of devices using
various scenarios. We need default behavior which reasonably fits
most cases.
I've tested the above timetable on a dozen SATA controllers and a few
PATA controllers with about a dozen different drives from all major
vendors and 4 different ODDs from three different vendors for both
boot and hotplug (if available) cases.
Boot probing is not affected unless the device is broken in which
cases new code gives up on the port after a minute rather than five or
nine minutes. When hotplugging, most devices get detected on the
first or second try. Multi-platter drives with long spin up time
which sometimes took > 40 secs with the original code, now usually
comes up during the second try and at least right after the third try
@20.

This patch updates ata_std_prereset() as follows.
* Don't fail on phy resume failure. Just whine and continue. Failure
from prereset makes libata abort whole reset sequence and give up
the port, so prereset() should be best effort. This is more
important with the coming EH updates as prereset() will be called
with shorter timeout.
* If ata_wait_ready() fails, whine and request hardreset instead.

For PATA, 0xff status indicates empty port. For SATA, it depends on
how the controller emulates status register. On some controllers,
0xff is used to represent broken link or certain stage during reset.
libata currently deals SATA the same. This hasn't caused any problem
because problematic situations usually only occur after hotplug or
other link disruption events and libata blindly waited for the device
to spin up and settle after hotplug giving the link and device
whatever time to go through those stages.
libata is going to replace unconditional spinup wait with generic
timed sequence of resets, so not only getting 0xff handling right for
SATA is, well, the right thing to do, it's much more important now.
This patch makes the following changes.
* Make ata_bus_softreset() return -ENODEV if any of its wait fails
due to 0xff status.
* Fail soft/hardreset if status wait returns -ENODEV indicating 0xff
status while SStatus says the link is online. e.g. Reset fails if
status is 0xff after reset when SStatus reports the linke is online.
If SCR registers are not available, everything is the same as
before.

Add @deadline to prereset and reset methods and make them honor it.
ata_wait_ready() which directly takes @deadline is implemented to be
used as the wait function. This patch is in preparation for EH timing
improvements.
* ata_wait_ready() never does busy sleep. It's only used from EH and
no wait in EH is that urgent. This function also prints 'be
patient' message automatically after 5 secs of waiting if more than
3 secs is remaining till deadline.
* ata_bus_post_reset() now fails with error code if any of its wait
fails. This is important because earlier reset tries will have
shorter timeout than the spec requires. If a device fails to
respond before the short timeout, reset should be retried with
longer timeout rather than silently ignoring the device.
There are three behavior differences.
1. Timeout is applied to both devices at once, not separately. This
is more consistent with what the spec says.
2. When a device passes devchk but fails to become ready before
deadline. Previouly, post_reset would just succeed and let
device classification remove the device. New code fails the
reset thus causing reset retry. After a few times, EH will give
up disabling the port.
3. When slave device passes devchk but fails to become accessible
(TF-wise) after reset. Original code disables dev1 after 30s
timeout and continues as if the device doesn't exist, while the
patched code fails reset. When this happens, new code fails
reset on whole port rather than proceeding with only the primary
device.
If the failing device is suffering transient problems, new code
retries reset which is a better behavior. If the failing device is
actually broken, the net effect is identical to it, but not to the
other device sharing the channel. In the previous code, reset would
have succeeded after 30s thus detecting the working one. In the new
code, reset fails and whole port gets disabled. IMO, it's a
pathological case anyway (broken device sharing bus with working
one) and doesn't really matter.
* ata_bus_softreset() is changed to return error code from
ata_bus_post_reset(). It used to return 0 unconditionally.
* Spin up waiting is to be removed and not converted to honor
deadline.
* To be on the safe side, deadline is set to 40s for the time being.

The Marvell IDE interface on my machine would hit a BUG_ON() in
lib/iomem.c because it was calling ata_pci_init_one() specifying just a
single port on the host, but that would actually end up trying to
initialize two ports, the second one with bogus information.
This fixes "ata_pci_init_one()" so that it actually passes down the
n_ports variable that it got from the low-level driver to the host
allocation routine ("ata_host_alloc_pinfo()"), which results in the ATA
layer actually having the correct port number information.
And in order to make it all work, I also needed to fix a few places that
had incorrectly hard-coded the fact that a host always had exactly two
ports (both ata_pci_init_bmdma() and ata_request_legacy_irqs() would
just always iterate over both ports).

Russell King hit a case where quantisation errors accumulated such that
the cycle time was shorter than rather than equal to the active/recovery

(S)ATA drives can be configured for "power-up in standby",
a mode whereby a specific "spin up now!" command is required
before the first media access.
Currently, a drive with this feature enabled can not be used at all
with libata, and once in this mode, the drive becomes a doorstop.
The older drivers/ide subsystem at least enumerates the drive,
so that it can be woken up after the fact from a userspace HDIO_*
command, but not libata.
This patch adds support to libata for the "power-up in standby"
mode where a "spin up now!" command (SET_FEATURES) is needed.
With this, libata will recognize such drives, spin them up,
and then re-IDENTIFY them if necessary to get a full/complete
set of drive features data.
Drives in this state are determined by looking for
special values in id[2], as documented in the current ATA specs.

Add support for ignoring the BIOS HPA result (off by default) and setting
the disk to the full available size unless already frozen.
Tested with various platforms/disks and confirmed to work with the
Macintosh (which broke earlier) and ata_piix (breakage due to the LBA48
readback that Tejun fixed).
For normal users this brings us, I believe, to feature parity with old IDE
(and of course more featured in some areas too).

Convert ahci, sata_sil, sata_sil24, sata_svw, sata_qstor, sata_mv,
sata_sx4, sata_vsc and sata_inic162x to new init model.
Now that host and ap are available during intialization, functions are
converted to take either host or ap instead of low level parameters
which were inevitable for functions shared between init and other
paths. This simplifies code quite a bit.
* init_one()'s now follow more consistent init order
* ahci_setup_port() and ahci_host_init() collapsed into
ahci_init_one() for init order consistency
* sata_vsc uses port_info instead of setting fields manually
* in sata_svw, k2_board_info converted to port_info (info is now in
port flags). port number is honored now.
Tested on ICH7/8 AHCI, jmb360, sil3112, 3114, 3124 and 3132.

Convert sata_via and sata_promise to new init model. Both controllers
can have combined configuration (SATA + PATA) and used twisted
initialization method (modifying port in ->port_start) to overcome
probe_ent limitations.
This patch converts both drivers to new init model in which such
configuration is natively supported.
* promise: Combined pata port now uses separate port_info entry right
after the sata counterpart entry.
* promise: Controller configuration is discerned using ap->flags.
This simplifies init path and makes it look more like other LLDs.
* via: Both SATA and PATA ports in vt6421 are represented in their
own port_info structure.
Tested on PDC20375 (SATA150 TX2plus) [105a:3375] and PDC40775 (SATA
300 TX2plus) [105a:3d73]. Couldn't test via cuz my c3 won't boot the
current kernel.

These will be used to convert LLDs to new init model.
* Add irq_handler field to port_info. In new init model, requesting
IRQ is LLD's responsibility and libata doesn't need to know about
irq_handler. Most LLDs can simply register their irq_handler but
some need different irq_handler depending on specific chip. The
added port_info->irq_handler field can be used by LLDs to select
the matching IRQ handler in such cases.
* Add ata_dummy_port_info.
* Implement ata_pci_prepare_native_host(), a helper to alloc ATA host,
acquire all resources and init the host in one go.

replaces ata_pci_init_native_mode(). As there are remaining LLD
users, the old function isn't removed yet.

Convert legacy PCI host handling to alloc-init-register model.
ata_init_legacy_host(), ata_request_legacy_irqs() and
ata_pci_init_bmdma() are separated out and follow the new init model.
The two legacy handling functions use separate ata_legacy_devres
instead of generic devm_* resources. This reduces devres overhead for
legacy hosts which was a bit high because it didn't use PCI/iomap
merged resoruces.
ata_pci_init_one() is rewritten in terms of the aboved functions but
native mode handling is still using the old method. Conversion will
be completed when native mode handling is updated.

Reorganize ata_host_alloc() and its subroutines into the following
three functions.
* ata_host_alloc() : allocates host and its ports. shost is not
registered automatically.
* ata_scsi_add_hosts() : allocates and adds shosts associated with an
ATA host. Used by ata_host_register().
* ata_host_register() : takes a fully initialized ata_host structure
and registers it to libata layer and probes it.
Only ata_host_alloc() and ata_host_register() are exported.
ata_device_add() is rewritten using the above functions. This patch
does not introduce any observable behavior change. Things worth
mentioning.
* print_id is assigned at registration time and LLDs are allowed to
overallocate ports and reduce host->n_ports during initialization.
ata_host_register() will throw away unused ports automatically.
* All SCSI host initialization stuff now resides in
ata_scsi_add_hosts() in libata-scsi.c, where it should be.
* ipr is now the only user of ata_host_init(). Either kill it by
converting ipr to use ata_host_alloc() and friends or rename and
move it to libata-scsi.c

->post_internal_cmd is simplified EH for internal commands. Its
primary mission is to stop the controller such that no rogue memory
access or other activities occur after the internal command is
released. It may provide error diagnostics by setting qc->err_mask
but this hasn't been a requirement.
To ignore SETXFER failure for CFA devices, libata needs to know
whether a command was failed by the device or for any other reason.
ie. internal command needs to get AC_ERR_DEV right.
This patch makes the following changes to AC_ERR_DEV handling and
->post_internal_cmd semantics to accomodate this need and simplify
callback implementation.
1. As long as the correct bits in the result TF registers are set,
there is no need to set AC_ERR_DEV explicitly. libata EH core
takes care of that for both normal and internal commands.
2. The only requirement for ->post_internal_cmd() is to put the
controller into quiescent state. It needs not to set any err_mask.
3. ata_exec_internal_sg() performs minimal error analysis such that
AC_ERR_DEV is automatically set as long as result_tf is filled
correctly.

There are several registers which describe how the controller is
configured. These registers are sometimes implemented as r/w
registers which are configured by firmware and get cleared on
controller reset or after suspend/resume cycle. ahci saved and
restored those values inside ahci_reset_controller() which is a bit
messy and doesn't work over suspend/resume cycle.
This patch implements ahci_save/restore_initial_config(). The save
function is called during driver initialization and saves cap and
port_map to hpriv. The restore function is called after the
controller is reset to restore the initial values.
Sometimes the initial firmware values are inconsistent and need to be
fixed up. This is handled by ahci_save_initial_config(). For this,
there are two versions of saved registers. One to write back to the
hardware register, the other to use during driver operation. This is
necessary to keep ahci's behavior unchanged (write back fixed up
port_map while keeping cap as-is).
This patch makes ahci save the register values once before the first
controller reset, not after it's been reset. Also, the same stored
values are used written back after each reset, so the register values
are properly recovered after suspend/resume cycle.

Promise ATA ports should always be reset by pdc_reset_port()
when errors are detected, but the recent error reason decoding
update to sata_promise replaced that reset with a freeze.
This patch changes the error detection to do a reset again.
This makes the error decoding update safer, as it now only
adds error decoding without changing any other behaviour.

The READ/WRITE LONG commands are theoretically obsolete,
but the majority of drives in existance still implement them.
The WRITE_LONG and WRITE_LONG_ONCE commands are of particular
interest for fault injection testing -- eg. creating "media errors"
at specific locations on a disk.
The fussy bit is that these commands require a non-standard
sector size, usually 520 bytes instead of 512.
This patch adds support to libata for READ/WRITE LONG commands
issued via SG_IO/ATA_16.

The Asus W5F laptop uses a short cable instead of the 80-wire style, and thus
needs to be in the ich_laptop special cases for correct detection and support
of UDMA/100 for the hard drive. I noticed this because I have the W5F laptop,
and was tracing apparent slowness.

Reading from the ATA shadow registers while we are in ADMA mode may cause
undefined behavior. Don't read the ATA status register when completing
commands for this reason, it shouldn't be needed as the controller will
notify us if the command failed. Also, don't allow commands with result
taskfile requested to execute in ADMA mode, since that requires accessing
the shadow registers. We also still need to override tf_read since libata
will read the result taskfile on a command failure, and we need to go into
port register mode before allowing this.

It tries to spot when the slave is a mirror of the master and to fix up
problems that causes.
I've got two confirmations so far that this plus the "can fail set_xfer" patch
work for folks who had problems before. Also if you are unfortunate enough to
be running something like HAL then it'll automount the same disk twice for you
and corrupt it without the fix (aint that nice...)

There was a rare report where SB600 reported SERR_INTERNAL and SRST
couldn't get it out of the failure mode. Hardreset on SERR_INTERNAL.
As the problem is intermittent, whether this fixes the problem or not
hasn't been verified yet, but hardresetting the channel on internal
error is a good idea anyway.

This patch changes sata_promise so that the PATA ports
on TX2plus chips are bound to the pdc_pata_ops structure.
This means that operations called from the SATA ops
structures don't need any SATA-vs-PATA tests any more.
Instead, operations that depend on a port being SATA or
PATA are separated into different procedures.
* pdc_cable_type() is split into a PATA version and a
SATA version
* pdc_error_handler() is split into a PATA version and a
SATA version, that both call a common version after
setting up the `hardreset' function pointer
* pdc_old_check_atapi_dma() is now only used for SATAI
ports, so is renamed to pdc_old_sata_check_atapi_dma()
and simplified
* pdc_sata_scr_{read,write}() are now only used for SATA
ports, so their is-not-SATA tests are removed
* pdc_port_start() is split into three procedures: a wrapper
which performs the ->ops adjustment on TX2plus PATA ports,
a procedure with the common code, and a procedure with
the SATA-specific code (this bit might be cleaned up by
Tejun's new init model)

Both old-IDE and libata should be able handle all controllers and
devices found using normal resource reservation methods.
This eliminates the awful, low-performing split-driver configuration
where old-IDE drove the PATA portion of a PCI device, in PIO-only mode,
and libata drove the SATA portion of the /same/ PCI device, in DMA mode.
Typically vendors would ship SATA hard drive / PATA optical
configuration, which would lend itself to slow (PIO-only) CD-ROM
performance.
For Intel users running in combined mode, it is now wholly dependent on
your driver choice (potentially link order, if you compile both drivers
in) whether old-IDE or libata will drive your hardware.
In either case, you will get full performance from both SATA and PATA
ports now, without having to pass a kernel command line parameter.

Now that we have ata_do_set_mode() available for drivers to use we don't
actually need ->post_set_mode() as the driver can wrap set_mode nicely
and do stuff before or after (eg PCMCIA needs before), so we can kill off
a method in all the structs

This changeset revolves around the fact that all the SiS controllers have
the same enable bits, but differing cable detection methods. Previously
that meant each type had its own error_handler methods. Instead we can
now implement different ->cable_detect methods and share a single
error_handler which does the filtering by enable bits.
In addition we had some auto const arrays that should be static const. I'm
not sure if gcc already treats them intelligently but adding the static
will make sure.

2.6.21-rc has horrible problems with libata and PATA cable types (and
thus speeds). This occurs because Tejun fixed a pile of other bugs and
we now do cable detect enforcement for drive side detection properly.
Unfortunately we don't do the process around cable detection right. Tejun
identified the problem and pointed to the right Annex in the spec, this patch
implements the rest of the needed changes.
We add a ->cable_detect() method called after the identify
sequence which allows a host to do host side detection at this point
should it wish, or to modify the results of the drive side identify.
This separate ->cable_detect method also cleans up a lot of code because
many drivers have their own error_handler methods which really just set
the cable type.
If there is no ->cable_detect method the cable type is left alone so a
driver setting it earlier (eg because it has the SATA flags set or
because it uses the old error_handler approach) will still do the right
thing (or at least the same thing) as before.
This patch simply adds the cable_detect method and helpers it doesn't use
them but other follow up patches will (ie Adrian please don't submit
patches to unexport them ;))

The chips covered by sata_mv have a 32-bit DMA boundary they must not
cross, not a 64K boundary. We are merely limited to a 64K maximum
segment size. Therefore, the DMA scatter/gather table fill code can be
greatly simplified, and we need not cut in half the S/G table size as
reported to the SCSI layer.
Also, the driver forget to turn on 64-bit DMA at the PCI layer. All
other data structures (both hardware and software) have been prepped for
64-bit PCI DMA. It was simply never turned on. <fingers crossed> let's
see if it still works...

A small number of SiS setups require special handling (not many judging
by how long this dumb bug survived). A couple of Fedora 7 devel testers
hit an Oops on pata_sis loading which is caused by terminal confusion
between chipset as 'the chipset we have found' and chipset as 'array
iterator'

2.6.21 fix lba48 bug in libata fill_result_tf()
This adds some NCQ blacklist entries taken from the Silicon Image 3124/3132
Windows driver .inf files. There are some confirming reports of problems
with these drives under Linux (for example http://lkml.org/lkml/2007/3/4/178)
so let's disable NCQ on these drives.
[ I'm personally starting to wonder whether we shouldn't disable NCQ by
default, and perhaps have a white-list. There seems to be a *lot* of
drives that do this wrong.. - Linus ]

Based on this, those last two statements fill_result_tf()
appear to me to be in the wrong order, in that the tf->flags
are uninitialized at the point where tf_read() is invoked.
So for lba48 commands, tf_read() won't be reading back the

get copied to result_tf before they are used by tf_read().

I've seen this several times on this drive, completely reproducible.
Once it has hung, power needs to be cut from the drive to recover it, a
simple reboot is not enough. So I'd suggest disabling NCQ on this
drive.

In LBA48 mode we have to help the controller to get anything to work. The
chip provides a register giving word counts meant for ATAPI use which we
can use. However we need to load the count in words not bytes..

For drive side cable detection to work correctly, drives need to be
identified backwards such that the slave device releases PDIAG- before
the mater drive tries to detect cable type. ata_bus_probe() was fixed
by commit f31f0cc2f0 but the new EH path
wasn't fixed. This patch makes new EH path do IDENTIFY backwards.
ata_dev_configure() for new devices are still performed master first.
This is to keep the detection messages in forward order.

There is a HW issue in ATI SB600 SATA that PxSERR.E should not be
set on some conditions, for example, when there is no media in SATA
CD/DVD drive or media is not ready, AHCI controller fails to execute
ATAPI commands and reports PORT_IRQ_TF_ERR, but ATI SB600 SATA
controller sets PxSERR.E at the
same time, which is not necessary.
This patch is just to ignore the INTERNAL ERROR in such case.

->prereset() returns -ENOENT to tell libata that the port is empty and
reset sequencing should be stopped. This is not an error condition.
Update ata_eh_reset() such that it sets device classes to ATA_DEV_NONE
and return success in on -ENOENT. This makes spurious error message
go away.

The ACPI specification states, and BIOS implementations depend on,
_STM being called before _GTF.
SATA does this, but PATA does not. So for now, simply
prevent execution of _GTF on PATA devices. Longer term we
should implement ACPI support for PATA devices in libata.

Commit 721449bf0d added support for using the
ADMA notifier bits to determine which commands to check for completion.
However there have been reports that this causes command timeouts in certain
cases. This is still being investigated. In addition, apparently the notifiers
won't work if ADMA is disabled on the other port as a result of an ATAPI device
being connected, and we don't handle this case properly.
For now, just restore the previous behavior of checking all active commands
to see if they are complete, without relying on the notifiers.

Recently I got my hands on nVidia's MCP61 PM-AM board, and
it contains IDE chip configured by BIOS with only primary
channel enabled. This confuses code which probes for
device DMA capabilities - it gets 0x60 (happy duplex
device) from primary channel BMDMA, but 0xFF (nobody here)
from secondary channel BMDMA. Due to this code then believes
that chip is simplex. I do not address this problem in
my patch, as I'm not sure how to handle this. Probably
ata_pci_init_one should have bitmap of enabled/possible
interfaces instead of their count, but it looks like
quite intrusive change, and maybe we do not care - for device
with only one channel simplex and regular DMA engines are
same.
But making device simplex pointed out that support for
DMA on simplex devices is currently broken - ata_dev_xfermask
tests whether device is simplex and if it is whether DMA
engine was assigned to this port. If not then it strips
out DMA bits from device. Problem is that code which assigns
DMA engine to port in ata_set_mode first detect device
mode and assigns DMA engine to channel only if some DMA
capable device was found.
And as xfermask stripped out DMA bits, host->simplex_claimed
is always NULL with current implementation.
By allowing DMA either if simplex_claimed is NULL or if it
points to current port DMA can be finally used - it gets
assigned to first port which contains any DMA capable
device.

For 2.6.20 it mostly used to just not work, for 2.6.21-rc it crashes, this
seems to be down to luck (bad or good). The libata-acpi code needs to
avoid doing PCI work on non-PCI devices. This is one hack although it's
not pretty and perhaps there is a "right" way to check if a struct device
* is PCI ?

Fix wrong "port" calculations in pdc202xx_{configure_piomode,set_dmamode}()
They were broken for all configurations except one (master device on primary
channel, no other devices) and as a result device settings + PIO/DMA timings
were being programmed into the wrong PCI registers. This could result in
a large variety of problems including data corruption, hangs etc. (depending
on devices used and your luck :-).

resulted in overclocked PIO timing setting (iff PIO0 was used)
- when programming PIO mode bits 0x18 weren't cleared so suboptimal
timings were used for PIO1-4 if PIO0 was previously set (bit 0x10)
and for PIO0/3/4 if PIO1/2 was previously set (bit 0x08)
and finally bump driver version.

pata_legacy fails to detect the disk on my old ISA/VLB 486:
it starts to probe io=0x1f0 ctr=0x3f6 irq=15, complains
loudly about IDENTIFYs timing out, and finally fails.
(Sorry I couldn't capture the kernel's boot messages.)
It turns out that the driver's mapping from io to irq in
legacy_irq[] is wrong: index 0 for io=0x1f0 has irq=15 but
should have irq=14, and index 1 for io=0x170 has irq=14 but
should have irq=15. This is confirmed by a comparison with
include/asm-i386/ide.h:ide_default_irq().
This patch swaps the first two elements in legacy_irq[],
which makes pata_legacy work on my 486.

Alan Cox noticed several hooks in pata_* drivers were missing, when
he authored his ->cable_detect hook patches. This patch extracts
just those fixes from Alan's patches, adding the necessary hooks
(usually ->freeze, ->thaw, and ->post_internal_cmd) to the drivers.

2.6.21-rc has horrible problems with libata and PATA cable types (and
thus speeds). This occurs because Tejun fixed a pile of other bugs and
we now do cable detect enforcement for drive side detection properly.
Unfortunately we don't do the process around cable detection right. Tejun
identified the problem and pointed to the right Annex in the spec, this patch
implements the needed changes.
The basic requirement is that we have to identify the slave before the
master.
The patch switches the identify order so that we can do the drive side
detection correctly.

The initial simplex handling code is fooled if you suspend and resume.
This also causes problems with some single channel controllers which
claim to be simplex.
The fix is fairly simple, instead of keeping a flag to remember if we
gave away the simplex channel we remember the actual owner. As the owner
is always part of the host_set we don't even need a refcount.
Knowing the owner also means we can reassign simplex DMA channels in
future hotplug code etc if we need to

Spurious SDB FIS during NCQ might not contain spurious completions.
It could be spurious TF update or invalid async notification. Treat
as HSM violation iff a spurious SDB FIS contains spurious completions;
otherwise, just whine once about it.

Make jmiron_ata quirk update pdev->class after programming the device
and update ahci and pata_jmicron such that they match class code
instead of checking function number manually. For ahci, it matches
for vendor and class. For pata_jmicron, it matches vendor, device and
class as IDE class isn't as well defined as AHCI class.
This makes jmicron device matching more conventional and script
friendly.

IRQ mask bits assumed a 60xx or newer generation chip, which is very
wrong for the 50xx series. Luckily both generations shared the per-port
interrupt mask bits, leaving only the "misc chip features" bits to be
completely mismatched.
Fix 50xx by ensuring we only program bits that exist.

The code in mv_edma_cfg() reflected its 60xx origins, by doing things
[slightly] incorrectly on the older 50xx and newer 6042/7042 chips.
Clean up the EDMA configuration setup such that, each chip family
carefully initializes its own EDMA setup.

The change to the devres layer re-orders the execution of cleanup
functions and in turn causes the pcmcia layer to oops as it zaps a
pointer now needed later on. We simply leave the pointer alone.

Each place in pdc_ata_init_one() that initialises a SATA
port first calls pdc_ata_setup_port(), and then manually
assigns the port's ->scr_addr. Simplify the code by extending
pdc_ata_setup_port() to also handle scr_addr initialisation;
for PATA ports we pass NULL as scr_addr.
The initialisation of the PATA-only 20619 redundantly set
up scr_addr for the ports. Remove this.
Tested on 20619, 20575, and 20775 chips.

Separate sata_vsc interrupt handling into a normal (per-port) path and an
error path with the addition of vsc_port_intr and vsc_error_intr
respectively. The error path handles interrupt based
hotplug events which requires the definition of vsc_freeze and vsc_thaw.
Note: vsc_port_intr has a workaround for unexpected interrupts that occur
during polled commands. This fixes a regression between 2.6.19 and 2.6.20.

sata_sil used to trigger HSM error if IRQ occurs during polling
command. This didn't matter because polling wasn't used in sata_sil.
However, as of 2.6.20, all IDENTIFYs are performed by polling and
device detection sometimes fails due to spurious IRQ. This patch
makes sata_sil ignore and clear spurious IRQ while executing commands
by polling.

Recently Tejun wrote a patch to ahci.c to make it raise a HSM violation
if the drive attempted to complete a tag that wasn't outstanding. We could
run into the same problem with sata_nv ADMA. This adds code to raise a HSM
violation error if the controller gives us a notifier tag that isn't
outstanding, since the drive may be issuing spurious completions.

sata_nv implemented its own copies of the BMDMA helper functions for ADMA,
since the ADMA BMDMA status registers are PIO while the other registers
are MMIO, and this was the only way to handle this previously. Now that
we have iomap support, the standard routines should just work, so use them.
The only thing we need to override as far as ADMA and BMDMA is the
post_internal_cmd callback, where we should only call ata_post_internal_cmd
if we are in port-register mode.

SDB FIS containing spurious NCQ completions is a clear protocol
violation. Currently, only some Maxtors with early firmware revisions
are showing this problem. Those firmwares have other NCQ related
problems including buggy NCQ error reporting and occasional lock up
after NCQ errors.
Consider spurious NCQ completions HSM violation and freeze the port
after it. EH will turn off NCQ after this happens several times.
Eventually drives which show this behavior should be blacklisted for
NCQ.

Remove the DMA setup function. As pointed out by Sergey we set the actual
DMA clock timing in set_dmamode so we don't actually need to do anything
with it at set up time, but just leave the PIO timings loaded.

ata_port has two different id fields - id and port_no. id is
system-wide 1-based unique id for the port while port_no is 0-based
host-wide port number. The former is primarily used to identify the
ATA port to the user in printk messages while the latter is used in
various places in libata core and LLDs to index the port inside the
host.
The two fields feel quite similar and sometimes ap->id is used in
place of ap->port_no, which is very difficult to spot. This patch
renames ap->id to ap->print_id to reduce the possibility of such bugs.
Some printk messages are adjusted such that id string (ata%u[.%u])
isn't printed twice and/or to use ata_*_printk() instead of hardcoded
id format.

We already have code that handles hotplug interrupt indications in ADMA
mode, this turns on the control flag that actually enables these interrupts.
Also fixes some cases in the same functions where a 16-bit register was read
using a readl instead of a readw.

The hardware provides us a notifier register that indicates what command
tags have completed. Use this to determine which CPBs to check, rather
than blindly checking all active CPBs. This should provide a minor
performance win, since if the controller has touched some of these
incomplete CPBs, accessing them will likely result in a cache miss.

This edits the taskfile setup to more closely match the way that libata
sends the taskfile for other controllers. This avoids putting taskfile writes
into the CPB buffer that are not needed according to the taskfile flags.

on both vt6420 and 6421 - BAR5 wasn't mapped for both controllers

Fix ata_scsi_change_queue_depth() such that...
* NCQ on/off is exactly determined using the same logic as the issue path.
* queue depth is adjusted to 1 if NCQ is not enabled.
* -EINVAL is returned if requested action is ignored due to limitations.
This fixes the bug which allows queue depth to be increased on
blacklisted NCQ hosts/devices.

libata used disable pdev only on PM_EVENT_SUSPEND while re-enable pdev
unconditionally. This was okay before ref-counted pdev enable update
but it now makes the pdev pinned after swsusp cycle (enabled twice but
disabled only once) and devres sanity check whines about it.
Fix it by unconditionally disabling pdev on all suspend events.

This patch appears to solve some problems with commands timing out in
cases where an NCQ command is immediately followed by a non-NCQ command
(or possibly vice versa). This is a rather ugly solution, but until we
know more about why this is needed, this is about all we can do.

ata_probe_ent_alloc() had a temporary hack such that devm_kzalloc()
was used for allocation if devres had been previously initialized on
the device; otherwise, plain kzalloc() was used. This was to make the
code useable from both the old and devres-aware libata drivers during
transition. This hack made ata_sas_port_alloc() unable to determine
how the probe_ent is allocated, causing double free in some cases.
Remove the now-unneeded hack and make ata_sas_port_alloc() use
devm_kfree().

Some debug output in the ADMA error_handler function was removed recently,
but it may be useful in certain cases, like NCQ commands timing out. Add it
back in, but make it a bit more intelligent so that it only prints if
command(s) are active and only prints the CPBs for those commands.
That way it won't spew at inappropriate times like suspend/resume.

Somehow the sis_info133 external definition ended up in libata.h and that
was included by both drivers. However libata.h contains libata-* specific
internals and clashing defines like DRV_NAME so this makes a mess. Move
the extern into the C file and remove the warnings

I was trying to use HDIO_DRIVE_TASK for something today,
and discovered that the libata implementation does not copy
over the upper four LBA bits from args[6].
This is serious, as any tools using this ioctl would have their
commands applied to the wrong sectors on the drive, possibly resulting
in disk corruption.
Ideally, newer apps should use SG_IO/ATA_16 directly,
avoiding this bug. But with libata poised to displace drivers/ide,
better compatibility here is a must.
This patch fixes libata to use the upper four LBA bits passed
in from the ioctl.
The original drivers/ide implementation copies over all bits
except for the master/slave select bit. With this patch,
libata will copy only the four high-order LBA bits,
just in case there are assumptions elsewhere in libata (?).

The current EH speed down code is more of a proof that the EH
framework is capable of adjusting transfer speed in response to error.
This patch puts some intelligence into EH speed down sequence. The
rules are..
* If there have been more than three timeout, HSM violation or
unclassified DEV errors for known supported commands during last 10
mins, NCQ is turned off.
* If there have been more than three timeout or HSM violation for known
supported command, transfer mode is slowed down. If DMA is active,
it is first slowered by one grade (e.g. UDMA133->100). If that
doesn't help, it's slowered to 40c limit (UDMA33). If PIO is
active, it's slowered by one grade first. If that doesn't help,
PIO0 is forced. Note that this rule does not change transfer mode.
DMA is never degraded into PIO by this rule.
* If there have been more than ten ATA bus, timeout, HSM violation or
unclassified device errors for known supported commands && speeding
down DMA mode didn't help, the device is forced into PIO mode. Note
that this rule is considered only for PATA devices and is pretty
difficult to trigger.
One error can only trigger one rule at a time. After a rule is
triggered, error history is cleared such that the next speed down
happens only after some number of errors are accumulated. This makes
sense because now speed down is done in bigger stride.

* Move forcing device to PIO0 on device disable into
ata_dev_disable(). This makes both old and new EHs act the same
way.
* Speed down only PIO mode on probe failure. All commands used during
probing are PIO commands. There's no point in speeding down DMA.
* Retry at least once after -ENODEV. Some devices report garbled
IDENTIFY data after certain events. This shouldn't cause device
detach and re-attach.
* Rearrange EH failure path for simplicity.

Make ata_down_xfermask_limit() accept @sel instead of @force_pio0.
@sel selects how the xfermask limit will be adjusted. The following
selectors are defined.
* ATA_DNXFER_PIO : only speed down PIO
* ATA_DNXFER_DMA : only speed down DMA, don't cause transfer mode change
* ATA_DNXFER_40C : apply 40c cable limit
* ATA_DNXFER_FORCE_PIO : force PIO
* ATA_DNXFER_FORCE_PIO0 : force PIO0 (same as original with @force_pio0 == 1)
* ATA_DNXFER_ANY : same as original with @force_pio0 == 0
Currently, only ANY and FORCE_PIO0 are used to maintain the original
behavior. Other selectors will be used later to improve EH speed down
sequence.

This is the patch for PATA controller of Celleb.
This driver uses the managed iomap (devres).
Because this driver needs special taskfile accesses, there is
a copy of ata_std_softreset(). ata_dev_try_classify() is exported
so that it can be used in this function.
