Some digging showed that a merge at some previous point partially
added broken mmap() support, causing this trace. Remove the broken
code completely.

On top of that, the calculation in place for "buffer full" depended on
UYUV instead of HM12, while our GStreamer code was picking HM12 in
some circumstances.

Finally, the V4L2_CAP_STREAMING capability was never exposed. Patch it
into the YUV encoder node only.

1bf5842fe3 [media] cx18: Clean up mmap() support for raw YUV
The initial version of this patch (commit
d5976931639176bb6777755d96b9f8d959f79e9e) had some issues:

 * It didn't correctly calculate the size of the YUV buffer for 4:2:2,
   resulting in capture sometimes being offset by 1/3rd of a picture.

 * There were a lot of variables duplicating information the driver
   already knew, which have been removed.

 * There was an in-kernel format conversion - libv4l can do this one,
   and is the right place to do format conversions anyway.

 * Some magic numbers weren't properly explained.

Fix all these issues, leaving just the move from videobuf to videobuf2
to do.

e3bfeabbf5 [media] cx18: Add support for Hauppauge HVR-1600 models with s5h1411
The newest variants of the HVR-1600 have an s5h1411/tda18271 for the digital
frontend.  Add support for these boards.

a3bc5e3304 [media] v4l/cx18: update workqueue usage
With cmwq, there's no reason to use separate out_work_queue.  Drop it
and use system_wq instead.  The in_work_queue needs to be ordered so
can't use one of the system wqs; however, as it isn't used to reclaim
memory, allocate the workqueue with alloc_ordered_workqueue() without
WQ_MEM_RECLAIM.

a363436396 [media] Patch for cx18 module with added support of GoTView PCI DVD3 Hybrid tuner
I've got code which adds support of GoTView PCI DVD3 Hybrid tuner in
cx18 module and Andy Walls in ivtv mailing-list gave me some advice on
making a patch and sending it here. So here's the patch against
staging/2.6.37-rc1 branch (the tutorial recommends to include it as
plain text but if it's the case I can surely send as an attachment):

Several comments on the patch:

1. Both users on the official Gotview forum and support said that PCI
DVD3 is very similar to Yuan MPC718 card so the main part of code is
taken from Yuan configuration. Some users reported it to work properly.

2. Everything is being initialized correctly including analog, dvb,
radio and alsa parts. Analogue part and alsa virtual card is tested by
myself using original Gotview card.

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

d68b687b1e V4L/DVB: cx18: rework cx18-alsa module loading to support automatic loading
Restructure the way the module gets loaded so that it gets loaded automatically
when cx18 is loaded, and make it work properly if there are multiple cards
present (since the old code would only take one opportunity to connect to cx18
instances when the module first loaded).

82acdc84cc V4L/DVB (13908): cx18: Add initial working VIDIOC_G_ENC_INDEX ioctl() support
VIDIOC_G_ENC_INDEX support see the light of day.
Some notes:
1. With default capture parameters, the CX23418 seems to transfer 192 index
entries (4.5 kB worth) at 10 second intervals.

2. Index streams don't seem to be supported for MPEG 2 TS streams

3. The index entries seem to claim every frame is a B-Frame.  Possible
firmware bug.

4. The cx18 driver does not try to capture an index stream when inserting
sliced VBI into the MPEg stream as the offsets would need fixup.

efc0b127b2 V4L/DVB (13902): cx18: Update MPEG Index stream buffers module option processing
Update the module options related to INDEX stream buffer allocation.  A single
CX2341[5678] index entry is only 24 bytes.  Large buffers for the IDX stream
will prevent the CX23418 from transferring index data over at all.  Buffers
of around 1.5 kB or 64 index entries seem to be just fine.  We'll default to
63 buffers/MDLs as that is the firmware limit per stream and IDX stream buffers
are not high rate.  There is no reason on earth to allocate the previous 1 MB
default of buffer space for the IDX stream.

This is in anticipation of implementing the G_ENC_INDEX ioctl() in the cx18
driver.

22dce188ef V4L/DVB (13430): cx18: Fix YUV capture so that encoder passes a single frame per transfer
Fix YUV capture such that the encoder will pass one frame per transfer.  This
will allow the application to maintain frame alignment when a transfer from
the encoder is missed due to high system latency in service the CX23418 IRQ.

Also force YUV buffer sizes to be specified in multiples of 33.75 kB, the
smalled amount of buffer sizes need to store a complete set of HM12 4:2:0
macroblocks specifying 32 lines of the frame.  A full 60Hz/525 line
screen requires 15 * 33.75 kB per frame and a full 50Hz/625 line screen
requires 18 * 33.75 kB per frame so the default buffer size is 3 * 33.75 kB,
requiring exactly 5 or 6 buffers per MDL respectively.  The bytes needed per
frame and hence MDL need not be the bytes in an integer number of buffers.
However, if frame artifacts are seen with scaled screen sizes, the YUV buffer
size can be set 34 kB (33.75 kB) to get rid of the artifacts at the cost of more
copies between the kernel and userspace.

40c5520f55 V4L/DVB (11618): cx18: Convert per stream mutex locks to per queue spin locks
To avoid sleeps in providing buffers to user space and in handling incoming
buffers from the capture unit, converted the per stream mutex for locking
queues to 3 spin locks.  There is now a spin lock per queue
to increase concurrency when moving buffers around.

Also simplified queue manipulations and buffer handling of incoming buffers
of data from the capture unit.

This change defers sending all CX18_CPU_DE_SET_MDL commands, for a stream with
an ongoing capture, by adding a work queue to handle sending such commands when
needed.  This prevents any sleeps, caused by notifying the firmware of new
usable buffers, when a V4L2 application read() is being satisfied or when
an incoming buffer is processed by the cx18-NN-in work queue thread.

Convert GPIO line functions, such a audio routing and device resets, to
v4l2_subdevices.  This essentially completes the conversion of cx18 to the
v4l2_device/v4l2_subdevice framework.  No regression testing has taken place as
of yet.  Also an ivtv legacy bug with GPIO mux routing and going to/from radio
mode was commented, but not fixed.

decoder core internal to the CX23418.  Completed separation and encapsulation
of the A/V decoder core interface as a v4l2_subdevice.  The cx18 driver now
compiles and links again.

776fa86988 V4L/DVB (10440): cx18: Fix presentation timestamp (PTS) for VBI buffers
The old code from ivtv used a CX23415/6 PTS, which was simply left at 0 in the
cx18 driver.  Since the CX23418 gives us what I think is a PTS (or some other
90 kHz clock count) with each VBI buffer, this change has the cx18 driver use
that as a PTS.

dd073434b5 V4L/DVB (9805): cx18: Port fix for raw/sliced VBI mixup from ivtv and cx25840
This is a port of the fixes Hans Verkuil made for ivtv/cx25840:
The service_set field was used to determine whether raw or sliced VBI was
desired. This is incorrect since it is perfectly valid to select sliced VBI
with a service_set of 0.

Instead the driver should check on VIDIOC_S_FMT whether the type
field matches the raw or sliced VBI type.

Updated the cx18 driver accordingly, including an additional check in
cx18_start_v4l2_encode_stream() that didn't exist in ivtv.

cx18: Allow more than 63 capture buffers in rotation per stream.  Implement
q_busy to hold buffers the firmware has for use.  q_free holds truly unused
buffers in a pool.  New buffers are given to the firmware as soon as the
firmware returns one, if there are any to give to the firmware.
