net: get rid of SET_ETHTOOL_OPS

Dave Miller mentioned he'd like to see SET_ETHTOOL_OPS gone.
This does that.

Mostly done via coccinelle script:

8b6da5fb96 staging/octeon-ethernet: Call dev_kfree/consume_skb_any instead of dev_kfree_skb.
Replace dev_kfree_skb with dev_kfree_skb_any in cvm_oct_xmit_pow which
can be called in hard irq and other contexts, on the code paths that
drop packets.

Replace dev_kfree_skb with dev_consume_skb_any in cvm_oct_xmit_pow which
can be called in hard irq and other contexts, on the code path where
the packet is transmitted successfully.

5ff8bebbd2 staging: octeon-ethernet: drop CONFIG_CAVIUM_OCTEON_NUM_PACKET_BUFFERS
We don't have such Kconfig option, so the current code is dead and
the documentation is wrong. Users can adjust this setting by using
module parameter or kernel command line, so we can delete this code.

885a947e5b staging: delete non-required instances of include <linux/init.h>
None of these files are actually using any __init type directives
and hence don't need to include <linux/init.h>.  Most are just a
left over from __devinit and __cpuinit removal, or simply due to
code getting copied from one driver to the next.

cd39f7374d staging: octeon-ethernet: allow to set IRQ smp_affinity freely
Currently the driver assumes that CPU 0 is handling all the hard IRQs.
This is wrong in Linux SMP systems where user is allowed to assign to
hardware IRQs to any CPU. The driver will stop working if user sets
smp_affinity so that interrupts end up being handled by other than CPU
0. The patch fixes that.

Enable only those interrupts that we can handle & acknowledge in the
interrupt handler.

At least on EdgeRouter Lite, the hardware may occasionally interrupt with
some error condition when the physical link status changes frequently.
Since the interrupt condition is not acked properly, this leads to the

Remove skb allocation failure warnings. They will trigger a page
allocation warning already. Also, one of the warnings was not ratelimited,
causing the box to lock up under heavy traffic & low memory.

9ddebc46e7 MIPS: OCTEON: Rename Kconfig CAVIUM_OCTEON_REFERENCE_BOARD to CAVIUM_OCTEON_SOC
CAVIUM_OCTEON_SOC most place we used to use CPU_CAVIUM_OCTEON.  This
allows us to CPU_CAVIUM_OCTEON in places where we have no OCTEON SOC.

Remove CAVIUM_OCTEON_SIMULATOR as it doesn't really do anything, we can
get the same configuration with CAVIUM_OCTEON_SOC.

Use strlcpy where possible to ensure the string is \0 terminated.

fb09bafda6 Staging tree pull request for 3.5-rc1
Here is the big staging tree pull request for the 3.5-rc1 merge window.
 
 Loads of changes here, and we just narrowly added more lines than we
 added:
  622 files changed, 28356 insertions(+), 26059 deletions(-)
 
 But, good news is that there is a number of subsystems that moved out of
 the staging tree, to their respective "real" portions of the kernel.
 
 Code that moved out was:
 	- iio core code
 	- mei driver
 	- vme core and bridge drivers
 
 There was one broken network driver that moved into staging as a step
 before it is removed from the tree (pc300), and there was a few new
 drivers added to the tree:
 	- new iio drivers
 	- gdm72xx wimax USB driver
 	- ipack subsystem and 2 drivers
 
 All of the movements around have acks from the various subsystem
 maintainers, and all of this has been in the linux-next tree for a
 while.

Resolved the iwlwifi conflict with mainline using 3-way diff posted
by John Linville and Stephen Rothwell.  In 'net' we added a bug
fix to make iwlwifi report a more accurate skb->truesize but this
conflicted with RX path changes that happened meanwhile in net-next.

In e1000e a conflict arose in the validation code for settings of
adapter->itr.  'net-next' had more sophisticated logic so that
logic was used.

ec47ea8247 skb: Add inline helper for getting the skb end offset from head
With the recent changes for how we compute the skb truesize it occurs to me
we are probably going to have a lot of calls to skb_end_pointer -
skb->head.  Instead of running all over the place doing that it would make
more sense to just make it a separate inline skb_end_offset(skb) that way
we can return the correct value without having gcc having to do all the
optimization to cancel out skb->head - skb->head.

b5c19ca801 staging/octeon: Fix PHY binding in octeon-ethernet driver.
Commit d6c25be (mdio-octeon: use an unique MDIO bus name.) changed the
names used to refer to MDIO buses.  The ethernet driver must be
changed to match, so that the PHY drivers can be attached.

a4be637b34 staging/octeon: Software should check the checksum of no tcp/udp packets
Icmp packets with wrong checksum are never dropped since skb->ip_summed is
set to CHECKSUM_UNNECESSARY.

When icmp packets with wrong checksum pass through the octeon net driver,
the not_IP, IP_exc, L4_error hardware indicators show no error. so the
driver sets CHECKSUM_UNNECESSARY on skb->ip_summed.

L4_error only works for TCP/UDP, not for ICMP.
