The interrupt handling is completely broken and has in fact never been
been enabled due to an inverted test for an interrupt endpoint in
open() that prevented the interrupt URB from being submitted.
Other highlights include missing interrupt URB resubmission (had it
ever been submitted), missing locking when managing the open-port count,
and NULL-pointer dereferences that could have been triggered by a
malicious device.
Rip out this broken crap which is beyond repair instead of pretending
we support this feature.

The driver would return success and leave the port structures
half-initialised if any of the register accesses during probe fails.
This would specifically leave the port control urb unallocated,
something which could trigger a NULL pointer dereference on interrupt
events.
Fortunately the interrupt implementation is completely broken and has
never even been enabled...
Note that the zero-length-enable register write used to set the zle-flag
for all ports is moved to attach.

Document the MCS7810 detection hack which relies on having the GPO and
GPI pins connected as recommended by ASIX.
Note that GPO (pin 42) is really RTS of the third port which will be
toggled for the corresponding physical port on two- and four-port
devices.

The current device-type detection is fragile and can't really be relied
upon. Instead of sprinkling device-id conditionals throughout the
driver, let's use the device-id table to encode the number of ports and
whether the device has a driver-controlled activity LED (MCS7810).

The driver was setting the device remote-wakeup feature during probe in
violation of the USB specification (which says it should only be set
just prior to suspending the device). This could potentially waste
power during suspend as well as lead to spurious wakeups.
Note that USB core would clear the remote-wakeup feature at first
resume.

The current ch341 divisor algorithm was known to give inaccurate results
for certain higher line speeds. Jonathan Olds <jontio@i4free.co.nz>
investigated this, determined the basic equations used to derive the
divisors and confirmed them experimentally [1].
The equations Jonathan used could be generalised further to:
baudrate = 48000000 / (2^(12 - 3 * ps - fact) * div), where
0 <= ps <= 3,
0 <= fact <= 1,
2 <= div <= 256 if fact = 0, or
9 <= div <= 256 if fact = 1
which will also give better results for lower rates.
Notably the error is reduced for the following standard rates:
1152000 (4.0% instead of 15% error)
921600 (0.16% instead of -7.5% error)
576000 (-0.80% instead of -5.6% error)
200 (0.16% instead of -0.69% error)
134 (-0.05% instead of -0.63% error)
110 (0.03% instead of -0.44% error)
but also for many non-standard ones.
The current algorithm also suffered from rounding issues (e.g.
requesting 2950000 Bd resulted in a rate of 2 MBd instead of 3 MBd and
thus a -32% instead of 1.7% error).
The new algorithm was inspired by the current vendor driver even if that
one only handles two higher rates that require fact=1 by hard coding the
corresponding divisors [2].
Michael Dreher <michael@5dot1.de> also did a similar generalisation of
Jonathan's work and has published his results with a very good summary
that provides further insights into how this device works [3].

Prolific has developed a new USB to UART chip: PL2303HXN
PL2303HXN : PL2303GC/PL2303GS/PL2303GT/PL2303GL/PL2303GE/PL2303GB
The Vendor request used by the PL2303HXN (TYPE_HXN) is different from
the existing PL2303 series (TYPE_HX & TYPE_01).
Therefore, different Vendor requests are used to issue related commands.
1. Added a new TYPE_HXN type in pl2303_type_data, and then executes
new Vendor request,new flow control and other related instructions
if TYPE_HXN is recognized.
2. Because the new PL2303HXN only accept the new Vendor request,
the old Vendor request cannot be accepted (the error message
will be returned)
So first determine the TYPE_HX or TYPE_HXN through
PL2303_READ_TYPE_HX_STATUS in pl2303_startup.
2.1 If the return message is "1", then the PL2303 is the existing
TYPE_HX/ TYPE_01 series.
The other settings in pl2303_startup are to continue execution.
2.2 If the return message is "not 1", then the PL2303 is the new
TYPE_HXN series.
The other settings in pl2303_startup are ignored.
(PL2303HXN will directly use the default value in the hardware,
no need to add additional settings through the software)
3. In pl2303_open: Because TYPE_HXN is different from the instruction of reset
down/up stream used by TYPE_HX.
Therefore, we will also execute different instructions here.
4. In pl2303_set_termios: The UART flow control instructions used by
TYPE_HXN/TYPE_HX/TYPE_01 are different.
Therefore, we will also execute different instructions here.
5. In pl2303_vendor_read & pl2303_vendor_write, since TYPE_HXN is different
from the vendor request instruction used by TYPE_HX/TYPE_01,
it will also execute different instructions here.
6. In pl2303_update_reg: TYPE_HXN used different register for flow control.
Therefore, we will also execute different instructions here.

The driver was using its struct usb_interface pointer as an inverted
disconnected flag, but was setting it to NULL without making sure all
code paths that used it were done with it.
Before commit ef61eb43ad ("USB: yurex: Fix protection fault after
device removal") this included the interrupt-in completion handler, but
there are further accesses in dev_err and dev_dbg statements in
yurex_write() and the driver-data destructor (sic!).
Fix this by unconditionally stopping also the control URB at disconnect
and by using a dedicated disconnected flag.
Note that we need to take a reference to the struct usb_interface to
avoid a use-after-free in the destructor whenever the device was
disconnected while the character device was still open.

Drop the redundant disconnect mutex which was introduced after the
open-disconnect race had been addressed generally in USB core by commit
d4ead16f50 ("USB: prevent char device open/deregister race").
Specifically, the rw-semaphore in core guarantees that all calls to
open() will have completed and that no new calls to open() will occur
after usb_deregister_dev() returns. Hence there is no need use the
driver data as an inverted disconnected flag.

A recent fix addressing a deadlock on disconnect introduced a new bug
by moving the present flag out of the critical section protected by the
driver-data mutex. This could lead to a racing release() freeing the
driver data before disconnect() is done with it.
Due to insufficient locking a related use-after-free could be triggered
also before the above mentioned commit. Specifically, the driver needs
to hold the driver-data mutex also while checking the opened flag at
disconnect().

disconnect callback returns.

This patch restores the correct DMA mask after switching back to device
mode.
The issue occurred because Device part of controller use 32 bits DMA and
Host side use 64 bits DMA. During loading XHCI driver the DMA mask
used by driver is overwritten by XHCI driver so it must be restored
to 32 bits.

At startup we should trigger the HW state machine
only if it is OTG mode. Otherwise we should just
start the respective role.
Initialize idle role by default. If we don't do this then
cdns3_idle_role_stop() is not called when switching to
host/device role and so lane switch mechanism
doesn't work. This results to super-speed device not working
in one orientation if it was plugged before driver probe.

Fix this by using a dedicated disconnected flag.

PM fix, which depends on and should have been submitted together with

Patch fixes issue with Halt Endnpoint Test observed during using g_zero
driver as DUT. Bug occurred only on some testing board.
Endpoint can defer transition to Halted state if endpoint has pending
requests.
Patch add additional condition that allows to return correct endpoint
status during Get Endpoint Status request even if the halting endpoint

The dwc3 code to get the "peripheral" / "host" / "otg" IRQ first tries
platform_get_irq_byname() and then falls back to the IRQ at index 0 if
the platform_get_irq_byname().
In this case we do not want platform_get_irq_byname() to print an error
on failure, so switch to platform_get_irq_byname_optional() instead which
does not print an error.

__usbhsg_ep_set_halt_wedge() should return -EAGAIN if the IN endpoint
has any queue or data. Otherwise, this driver is possible to cause
just STALL without sending a short packet data on g_mass_storage driver,
and then a few resetting a device happens on a host side during
a usb enumaration.

The commit 97664a207b ("usb: renesas_usbhs: shrink spin lock area")
had added a usbhsg_pipe_disable() calling into
__usbhsg_ep_set_halt_wedge() accidentally. But, this driver should
not call the usbhsg_pipe_disable() because the function discards
all queues. So, this patch removes it.

udev stored in ep->hcpriv might be NULL if tt buffer is cleared
due to a halted control endpoint during device enumeration
xhci_clear_tt_buffer_complete is called by hub_tt_work() once it's
scheduled, and by then usb core might have freed and allocated a
new udev for the next enumeration attempt.

After commit f7fac17ca9 ("xhci: Convert xhci_handshake() to use
readl_poll_timeout_atomic()"), ASMedia xHCI may fail to suspend.
Although the algorithms are essentially the same, the old max timeout is
(usec + usec * time of doing readl()), and the new max timeout is just
usec, which is much less than the old one.
Increase the timeout to make ASMedia xHCI able to suspend again.

The system can hit a deadlock if an xhci adapter breaks while initializing.
The deadlock is between two threads: thread 1 is tearing down the
adapter and is stuck in usb_unlocked_disable_lpm waiting to lock the
hcd->handwidth_mutex. Thread 2 is holding this mutex (while still trying
to add a usb device), but is stuck in xhci_endpoint_reset waiting for a
stop or config command to complete. A reboot is required to resolve.
It turns out when calling xhci_queue_stop_endpoint and
xhci_queue_configure_endpoint in xhci_endpoint_reset, the return code is
not checked for errors. If the timing is right and the adapter dies just
before either of these commands get issued, we hang indefinitely waiting
for a completion on a command that didn't get issued.
This wasn't a problem before the following fix because we didn't send
commands in xhci_endpoint_reset:

With the patch I am submitting, a duration test which breaks adapters
during initialization (and which deadlocks with the standard kernel) runs
without issue.

NVIDIA 3.1 xHCI card would lose power when moving power state into D3Cold.
Thus we need to wait for CNR bit to clear in xhci resume, just as in
xhci init.

Early xHCI 1.1 spec did not mention USB 3.1 capable hosts should set
sbrn to 0x31, or that the minor revision is a two digit BCD
containing minor and sub-minor numbers.
This was later clarified in xHCI 1.2.
Some USB 3.1 capable hosts therefore have sbrn set to 0x30, or minor
revision set to 0x1 instead of 0x10.
Detect the USB 3.1 capability correctly for these hosts as well

If an endpoint is encountered that returns USB3_LPM_DEVICE_INITIATED, keep
checking further endpoints, as there might be periodic endpoints later
that return USB3_LPM_DISABLED due to shorter service intervals.
Without this, the code can set too high a maximum-exit-latency and
prevent the use of multiple USB3 cameras that should be able to work.

If host/hub initiated link pm is prevented by a driver flag we still must
ensure that periodic endpoints have longer service intervals than link pm
exit latency before allowing device initiated link pm.
Fix this by continue walking and checking endpoint service interval if
xhci_get_timeout_no_hub_lpm() returns anything else than USB3_LPM_DISABLED
While at it fix the split line error message
