8cc7f5338e ARM: msm: Migrate to common clock framework
Move the existing clock code in mach-msm to the common clock
framework. We lose our capability to set the rate of and enable a
clock through debugfs. This is ok though because the debugfs
features are mainly used for testing and development of new clock
code.

To maintain compatibility with the original MSM clock code we
make a wrapper for clk_reset() that calls the struct msm_clk
specific reset function. This is necessary for the usb and sdcc
devices on MSM until a better suited API is made available.

421faca0b5 ARM: msm: Make proc_comm clock control into a platform driver
To move closer to the generic struct clock framework move the
proc_comm based clock code to a platform driver. The data
describing the struct clks still live in the devices-$ARCH file,
but the clock initialization is done at driver binding time.

2f8b6fe4a9 ARM: msm: Remove custom clk_set_{max,min}_rate() API
There are no users of this API anymore so let's just remove it.
If a need arises in the future we can extend the common clock API
to handle it.

85a7df1f85 ARM: msm: Remove custom clk_set_flags() API
Nobody is using this API upstream and it's just contributing
cruft. Remove it so the MSM clock API is closer to the generic
struct clock API.

e8db0be124 PM QoS: Move and rename the implementation files
The PM QoS implementation files are better named
kernel/power/qos.c and include/linux/pm_qos.h.

The PM QoS support is compiled under the CONFIG_PM option.

bd32344a6b msm: clock: Migrate to clkdev
Migrating to clkdev has several advantages:

 * Less code in mach-msm/clock.c

 * A more robust clk_get() implementation

 * clk_add_alias() support

 * clk_get_sys() support

In general, this will help board authors setup clock aliases and
break the dependency on device pointers in the clock tables.

2a52220c89 msm: clock: Remove references to clk_ops_pcom
Not all devices use proc_comm and determining if a clock is local
vs. remote is fragile when done by comparing clk_ops pointers.
Instead, implement an is_local() function for all clk_ops to
determine if the clock is local. Doing this allows us to remove
the last references to clk_ops_pcom from clock.c and compile it
for targets with CONFIG_MSM_PROC_COMM=n.

We don't need to set the clk_ops at runtime until 7x30 local
clock detection comes in. Right now it's just complicating things
so just set the ops pointer statically.

d64560fe3e msm: clock: Move debugfs code from clock.c to clock-debug.c
The clock debugfs code is large enough, and easy enough to separate,
that it deserves its own file which is compiled only when
CONFIG_DEBUG_FS is enabled.

Also, cleanup header file #includes that are no longer required.

3a790bbe79 msm: add handling for clocks tagged as CLK_MINMAX
CLK_MINMAX is used to denote clocks that have a wide variation
in possible frequencies. This handling just sets the min and
max values to the same value.

5e96da5d50 msm: generalize clock support.
The 'PCOM' method of clock control (commands issued to the radio
CPU) is shared across several (but not all) Qualcomm SOCs.
Generalize this clock mechanism so these other SOCs can be added.
