85c1fafd72 powerpc/mm: Use read barrier when creating real_pte
On ppc64 we support 4K hash pte with 64K page size. That requires
us to track the hash pte slot information on a per 4k basis. We do that
by storing the slot details in the second half of pte page. The pte bit
_PAGE_COMBO is used to indicate whether the second half need to be
looked while building real_pte. We need to use read memory barrier while
doing that so that load of hidx is not reordered w.r.t _PAGE_COMBO
check. On the store side we already do a lwsync in __hash_page_4K

eeb03a6eaa powerpc: Fail remap_4k_pfn() if PFN doesn't fit inside PTE
remap_4k_pfn() silently truncates upper bits of input 4K PFN
if it cannot be contained in PTE. This leads invalid memory mapping and could
result in a system crash when the memory is accessed. This patch fails
remap_4k_pfn() and returns -EINVAL if the input 4K PFN cannot be contained in
PTE.

V3 : Added parentheses to protect 'pfn' and entire macro as suggested by Brian.
V2 : Rewritten to avoid helper function as suggested by Stephen Rothwell.

5524a27d39 powerpc/mm: Convert virtual address to vpn
This patch convert different functions to take virtual page number
instead of virtual address. Virtual page number is virtual address
shifted right by VPN_SHIFT (12) bits. This enable us to have an
address range of upto 76 bits.

d28513bc7f powerpc/mm: Fix pgtable cache cleanup with CONFIG_PPC_SUBPAGE_PROT
Commit a0668cdc15 cleans up the handling
of kmem_caches for allocating various levels of pagetables.
Unfortunately, it conflicts badly with CONFIG_PPC_SUBPAGE_PROT, due to
the latter's cleverly hidden technique of adding some extra allocation
space to the top level page directory to store the extra information
it needs.

Since that extra allocation really doesn't fit into the cleaned up
page directory allocating scheme, this patch alters
CONFIG_PPC_SUBPAGE_PROT to instead allocate its struct
subpage_prot_table as part of the mm_context_t.

6c16a74d42 powerpc/mm: Fix potential access to freed pages when using hugetlbfs
When using 64k page sizes, our PTE pages are split in two halves,
the second half containing the "extension" used to keep track of
individual 4k pages when not using HW 64k pages.

However, our page tables used for hugetlb have a slightly different
format and don't carry that "second half".

Our code that batched PTEs to be invalidated unconditionally reads
the "second half" (to put it into the batch), which means that when
called to invalidate hugetlb PTEs, it will access unrelated memory.

It breaks when CONFIG_DEBUG_PAGEALLOC is enabled.

This fixes it by only accessing the second half when the _PAGE_COMBO
bit is set in the first half, which indicates that we are dealing with
a "combo" page which represents 16x4k subpages. Anything else shouldn't
have this bit set and thus not require loading from the second half.

c605782b1c powerpc/mm: Split the various pgtable-* headers based on MMU type
This patch moves the definition of the PTE format for each MMU type
to separate files instead of all in one file. This improves overall
maintainability and will make it easier to add new types.

On 64-bit, additionally, I've separated the headers relative to the
format of the page table tree (3 vs. 4 levels for 64K vs 4K pages)
from the headers specific to the PTE format for hash based processors,
this will make it easier to add support for Book3 "E" 64-bit
implementations.

There are still some type-related ifdef's in the generic headers,
we might remove them in the long run, but this patch shouldn't result
in any code change, -hopefully- just definitions being moved around.
