03b61ff3c3 mm/memory-failure.c: move set_migratetype_isolate() outside get_any_page()
Chen Gong pointed out that set/unset_migratetype_isolate() was done in
different functions in mm/memory-failure.c, which makes the code less
readable/maintainable.  So this patch does it in soft_offline_page().

With this patch, we get to hold lock_memory_hotplug() longer but it's
not a problem because races between memory hotplug and soft offline are
very rare.

01b0f19707 cpu/mem hotplug: add try_online_node() for cpu_up()
cpu_up() has #ifdef CONFIG_MEMORY_HOTPLUG code blocks, which call
mem_online_node() to put its node online if offlined and then call
build_all_zonelists() to initialize the zone list.

These steps are specific to memory hotplug, and should be managed in
mm/memory_hotplug.c.  lock_memory_hotplug() should also be held for the
whole steps.

For this reason, this patch replaces mem_online_node() with
try_online_node(), which performs the whole steps with
lock_memory_hotplug() held.  try_online_node() is named after
try_offline_node() as they have similar purpose.

There is no functional change in this patch.

309d0b3917 mm/nobootmem.c: have __free_pages_memory() free in larger chunks.
On large memory machines it can take a few minutes to get through
free_all_bootmem().

Currently, when free_all_bootmem() calls __free_pages_memory(), the number
of contiguous pages that __free_pages_memory() passes to the buddy
allocator is limited to BITS_PER_LONG.  BITS_PER_LONG was originally
chosen to keep things similar to mm/nobootmem.c.  But it is more efficient
to limit it to MAX_ORDER.

16TB   401s  351s   50s

That is around 1%-3% improvement on total boot time.

This patch was spun off from the boot time rfc Robin and I had been
working on.

8bfa3f9a01 mm/huge_memory.c: fix stale comments of transparent_hugepage_flags
Since commit 13ece886d9 ("thp: transparent hugepage config choice"),
transparent hugepage support is disabled by default, and
TRANSPARENT_HUGEPAGE_ALWAYS is configured when TRANSPARENT_HUGEPAGE=y.

And since commit d39d33c332 ("thp: enable direct defrag"), defrag is
enable for all transparent hugepage page faults by default, not only in
MADV_HUGEPAGE regions.

9e4be4708e mm/compaction.c: update comment about zone lock in isolate_freepages_block
Since commit f40d1e42bb ("mm: compaction: acquire the zone->lock as
late as possible"), isolate_freepages_block() takes the zone->lock
itself.  The function description however still states that the
zone->lock must be held.

This patch removes this outdated statement.

41ecc34598 ocfs2: simplify ocfs2_invalidatepage() and ocfs2_releasepage()
Ocfs2 doesn't do data journalling.  Thus its ->invalidatepage and
->releasepage functions never get called on buffers that have journal
heads attached.  So just use standard variants of functions from
buffer.c.

b1214e4757 ocfs2: fix possible double free in ocfs2_write_begin_nolock
When ocfs2_write_cluster_by_desc() failed in ocfs2_write_begin_nolock()
because of ENOSPC, it goes to out_quota, freeing data_ac(meta_ac).  Then
it calls ocfs2_try_to_free_truncate_log() to free space.  If enough
space freed, it will try to write again.  Unfortunately, some error
happenes before ocfs2_lock_allocators(), it goes to out and free
data_ac(meta_ac) again.

fae477b6f0 ocfs2: delay migration when the lockres is in migration state
We trigger a bug in __dlm_lockres_reserve_ast() when we parallel umount 4
nodes.  The situation is as follows:

1) Node A migrate all lockres it owned(eg.  lockres A) to other nodes
   say node B when it umounts.

2) Receiving MIG_LOCKRES message from A, Node B masters the lockres A
   with DLM_LOCK_RES_MIGRATING state set.

3) Then we umount ocfs2 on node B.  It also should migrate lockres A to
   another node, say node C.  But now, DLM_LOCK_RES_MIGRATING state of
   lockers A is not cleared.  Node B triggered the BUG on lockres with
   state DLM_LOCK_RES_MIGRATING.

750e3c6581 ocfs2: skip locks in the blocked list
A parallel umount on 4 nodes triggered a bug in
dlm_process_recovery_date().  Here's the situation:

Receiving MIG_LOCKRES message, A node processes the locks in migratable
lockres.  It copys lvb from migratable lockres when processing the first
valid lock.

If there is a lock in the blocked list with the EX level, it triggers the
BUG.  Since valid lvbs are set when locks are granted with EX or PR
levels, locks in the blocked list cannot have valid lvbs.  Therefore I
think we should skip the locks in the blocked list.

786235eeba kthread: make kthread_create() killable
Any user process callers of wait_for_completion() except global init
process might be chosen by the OOM killer while waiting for completion()
call by some other process which does memory allocation.  See
CVE-2012-4398 "kernel: request_module() OOM local DoS" can happen.

When such users are chosen by the OOM killer when they are waiting for
completion() in TASK_UNINTERRUPTIBLE, the system will be kept stressed
due to memory starvation because the OOM killer cannot kill such users.

kthread_create() is one of such users and this patch fixes the problem
for kthreadd by making kthread_create() killable - the same approach
used for fixing CVE-2012-4398.

04d9cd1224 ib_isert: Avoid duplicate iscsit_increment_maxcmdsn call
This patch avoids a duplicate iscsit_increment_maxcmdsn() call for
ISER_IB_RDMA_WRITE within isert_map_rdma() + isert_reg_rdma_frwr(),
which will already be occuring once during isert_put_datain() ->
iscsit_build_rsp_pdu() operation.

It also removes the local conn->stat_sn assignment + increment,
and changes the third parameter to iscsit_build_rsp_pdu() to
signal this should be done by iscsi_target_mode code.

5e8e6b4b3a iscsi-target: Fix mutex_trylock usage in iscsit_increment_maxcmdsn
This patch fixes a >= v3.10 regression bug with mutex_trylock() usage
within iscsit_increment_maxcmdsn(), that was originally added to allow
for a special case where ->cmdsn_mutex was already held from the
iscsit_execute_cmd() exception path for ib_isert.

When !mutex_trylock() was occuring under contention during normal RX/TX
process context codepaths, the bug was manifesting itself as the following
protocol error:

  Received CmdSN: 0x000fcbb7 is greater than MaxCmdSN: 0x000fcbb6, protocol error.
  Received CmdSN: 0x000fcbb8 is greater than MaxCmdSN: 0x000fcbb6, protocol error.

This patch simply avoids the direct ib_isert callback in lio_queue_status()
for the special iscsi_execute_cmd() exception cases, that allows the problematic
mutex_trylock() usage in iscsit_increment_maxcmdsn() to go away.

d07ba8422f SUNRPC: Avoid deep recursion in rpc_release_client
In cases where an rpc client has a parent hierarchy, then
rpc_free_client may end up calling rpc_release_client() on the
parent, thus recursing back into rpc_free_client. If the hierarchy
is deep enough, then we can get into situations where the stack
simply overflows.

The fix is to have rpc_release_client() loop so that it can take
care of the parent rpc client hierarchy without needing to
recurse.

2b77933c28 sparc64: Move to 64-bit PGDs and PMDs.
To make the page tables compact, we were using 32-bit PGDs and PMDs.
We only had to support <= 43 bits of physical addresses so this was
quite feasible.

In order to support larger physical addresses we have to move to
64-bit PGDs and PMDs.

Most of the changes are straight-forward:

1) {pgd,pmd}_t --> unsigned long

2) Anything that tries to use plain "unsigned int" types with pgd/pmd
   values needs to be adjusted.  In particular things like "0U" become
   "0UL".

3) {PGDIR,PMD}_BITS decrease by one.

4) In the assembler page table walkers, use "ldxa" instead of "lduwa"
   and adjust the low bit masks to clear out the low 3 bits instead of
   just the low 2 bits during pgd/pmd address formation.

Also, use PTRS_PER_PGD and PTRS_PER_PMD in the sizing of the
swapper_{pg_dir,low_pmd_dir} arrays.

This patch does not try to take advantage of having 64-bits in the
PMDs to simplify the hugepage code, that will come in a subsequent
change.

37b3a8ff3e sparc64: Move from 4MB to 8MB huge pages.
The impetus for this is that we would like to move to 64-bit PMDs and
PGDs, but that would result in only supporting a 42-bit address space
with the current page table layout.  It'd be nice to support at least
43-bits.

The reason we'd end up with only 42-bits after making PMDs and PGDs
64-bit is that we only use half-page sized PTE tables in order to make
PMDs line up to 4MB, the hardware huge page size we use.

So what we do here is we make huge pages 8MB, and fabricate them using
4MB hw TLB entries.

Facilitate this by providing a "REAL_HPAGE_SHIFT" which is used in
places that really need to operate on hardware 4MB pages.

Use full pages (512 entries) for PTE tables, and adjust PMD_SHIFT,
PGD_SHIFT, and the build time CPP test as needed.  Use a CPP test to
make sure REAL_HPAGE_SHIFT and the _PAGE_SZHUGE_* we use match up.

This makes the pgtable cache completely unused, so remove the code
managing it and the state used in mm_context_t.  Now we have less
spinlocks taken in the page table allocation path.

The technique we use to fabricate the 8MB pages is to transfer bit 22
from the missing virtual address into the PTEs physical address field.
That takes care of the transparent huge pages case.

For hugetlb, we fill things in at the PTE level and that code already
puts the sub huge page physical bits into the PTEs, based upon the
offset, so there is nothing special we need to do.  It all just works
out.

So, a small amount of complexity in the THP case, but this code is
about to get much simpler when we move the 64-bit PMDs as we can move
away from the fancy 32-bit huge PMD encoding and just put a real PTE
value in there.

b2d4383480 sparc64: Make PAGE_OFFSET variable.
Choose PAGE_OFFSET dynamically based upon cpu type.

Original UltraSPARC-I (spitfire) chips only supported a 44-bit
virtual address space.

Newer chips (T4 and later) support 52-bit virtual addresses
and up to 47-bits of physical memory space.

Therefore we have to adjust PAGE_SIZE dynamically based upon
the capabilities of the chip.

Note that this change alone does not allow us to support > 43-bit
physical memory, to do that we need to re-arrange our page table
support.  The current encodings of the pmd_t and pgd_t pointers
restricts us to "32 + 11" == 43 bits.

This change can waste quite a bit of memory for the various tables.
In particular, a future change should work to size and allocate
kern_linear_bitmap[] and sparc64_valid_addr_bitmap[] dynamically.
This isn't easy as we really cannot take a TLB miss when accessing
kern_linear_bitmap[].  We'd have to lock it into the TLB or similar.

c920745e69 sparc64: Clean up 64-bit mmap exclusion defines.
Older UltraSPARC chips had an address space hole due to the MMU only
supporting 44-bit virtual addresses.

The top end of this hole also has the same value as the current
definition of PAGE_OFFSET, so this can be confusing.

Consolidate the defines for the userspace mmap exclusion range into
page_64.h and use them in sys_sparc_64.c and hugetlbpage.c
