ccw_io_helper neglected to reset vcdev->err after a new channel
program had been successfully started, resulting in stale errors
delivered after one I/O failed. Reset the error after a new
channel program has been successfully started with no old I/O
pending.

The current location for mapping virtio devices does not take
into consideration the standby memory. This causes the failure
of mapping standby memory since the location for the mapping is
already taken by the virtio devices. To fix the problem, we move
the location to beyond the end of standby memory.

This is the pull request for the earlier patchset[1] with the same
name. It's only three patches (the first one was committed to
workqueue tree) but the merge strategy is a bit involved due to the
dependencies.
* Because the conversion needs features from wq/for-3.10,
block/for-3.10/core is based on rc3, and wq/for-3.10 has conflicts
with rc3, I pulled mainline (rc5) into wq/for-3.10 to prevent those
workqueue conflicts from flaring up in block tree.
* Resolving the issue that Jan and Dave raised about debugging
requires arch-wide changes. The patchset is being worked on[2] but
it'll have to go through -mm after these changes show up in -next,
and not included in this pull request.

The conflict is trivial - one removes an "if ()" conditional while the
other removes "rbi->bi_next = NULL" right above it. We just need to

so that you can use it for verification. The test merge commit has
proper merge description.
While these changes are a bit of pain to route, they make code simpler
and even have, while minute, measureable performance gain[3] even on a
workload which isn't particularly favorable to showing the benefits of
this conversion.

Just a little convenience macro - main reason to add it now is preparing
for immutable bio vecs, it'll reduce the size of the patch that puts

The 3270 device nodes 227/0 and 228/0 are special. 227/0 is never used,
and 228/0 is used to redirect full screen applications from /dev/3270/tub
to the real device node of the 3270 terminal.
To keep the device names /dev/3270/tty<x> consistent with the minor number
the device driver sets minor_start to 0 and skips minor 0. That makes the
tty index equivalent to the minor number. But doing so seems to causes
problems with init scripts.
A better solution is to set minor_start to the correct value of 1 and
set name_base to 1 as well. tty_register_device will then automatically
create the correct tty name.

Give a bad return code when specifying a router setting that is either

It allows for cleaning up on a considerable amount of places. They did
port_get, wakeup, kref_put. Now the only thing needed is to call
tty_port_tty_wakeup which does exactly that.
One exception is ifx6x60 where tty_wakeup was open-coded. We now call
tty_wakeup properly there.

Standby memory that is located outside [0,OLDMEM_SIZE] is currently used
by the s390 memory detection. This leads to additional memory consumption
due to allocation of page structures. To fix this, we now do not
add standby memory if the kernel is started in kdump mode.

If a block device driver cannot fetch all requests from the blocklayer
it's in his responsibility to call the request function at a later time.
Normally this would be done after the next irq for the underlying device
is handled. However in situations where we have no outstanding request
we have to schedule the request function for a later time.
This is determined using an internal counter of requests issued to the
hardware.
In some cases where we give a request back to the block layer unhandled
the number of queued requests was not adjusted. Fix this class of
failures by adjusting queued_requests in all functions used to give
a request back to the block layer.

Lookups by channel/vq pair on host during virtio notifications might be
expensive. Interpret hypercall return value as a cookie which host can

Fix up trivial conflicts, mostly just due to the TTY config option
clashing with the EXPERIMENTAL removal.
The s390 architecture is unique in respect to dirty page detection,
it uses the change bit in the per-page storage key to track page
modifications. All other architectures track dirty bits by means
of page table entries. This property of s390 has caused numerous
problems in the past, e.g. see git commit ef5d437f71
"mm: fix XFS oops due to dirty pages without buffers on s390".
To avoid future issues in regard to per-page dirty bits convert
s390 to a fault based software dirty bit detection mechanism. All
user page table entries which are marked as clean will be hardware
read-only, even if the pte is supposed to be writable. A write by
the user process will trigger a protection fault which will cause
the user pte to be marked as dirty and the hardware read-only bit
is removed.
With this change the dirty bit in the storage key is irrelevant
for Linux as a host, but the storage key is still required for
KVM guests. The effect is that page_test_and_clear_dirty and the
related code can be removed. The referenced bit in the storage
key is still used by the page_test_and_clear_young primitive to
provide page age information.
For page cache pages of mappings with mapping_cap_account_dirty
there will not be any change in behavior as the dirty bit tracking
already uses read-only ptes to control the amount of dirty pages.
Only for swap cache pages and pages of mappings without
mapping_cap_account_dirty there can be additional protection faults.
To avoid an excessive number of additional faults the mk_pte
primitive checks for PageDirty if the pgprot value allows for writes

When the DASD devices are detached from the driver, then the
dasd_generic_remove function is called. One of the things this
function should do is to remove the DASD specific sysfs attributes,
but this is not done in all cases. This is likely to cause an oops
when at a later point sysfs stumbles over the stale pointers. In
particular this happens when when the modules are unloaded and loaded
again.

When an attempt to query or modify the grouping state of a channel path
fails due to a timeout, we cannot be sure about its state. To get back to
a defined state, disband the whole path group and try again while excluding
the offending path.

Add new attribute "vpm" to the subchannel sysfs directory of I/O
subchannels. This attribute contains a path mask indicating which

Omit known to be broken paths (those set in path_noirq_mask) for the
sense/set PGID and nop IO commands.
Note: path_noirq_mask will be reset in ccw_device_verify_start (the
paths could be healthy again). However if we restart a path
verification via verify_start this mask will not be reset (there is
no need to let the wait for an interrupt time out again - plus we
do not want to loop once we deal with the paths in unknown path group
state).

Do not abort path verification when waiting for an interrupt timed out.
Use path_noirq_mask to keep track of the paths used for this (also
maintain a path_notoper_mask for debugging purposes). If the timeout
happend to be during an operation where we query or alter the state of
path groups set the pgid_unknown flag.
With this change we allow usage of devices which have such ill-behaved
paths (if at least one path is operational).

Under LPAR the zfcpdump HSA is a shared resource. Up to now the HSA memory
is released when the zcore file is closed. Dump programs that know that
they do not need the HSA memory any more (e.g. because they already dumped it)
could release it earlier. This would allow other LPARs to use it again.
To achieve this a new debugfs file "hsa" is added that can be used to read
the HSA size and to release the HSA by writing "0" into the file.

Convert the synchronous size sense code to an interrupt driven
approach. This allows to set the device online even if the
terminal is not connected. With the new code views can be
registered without a connected terminal, the tty can be opened
as soon as the device is online. After the terminal has been
connected and the size has been determined the tty is resized
to match the device characteristics..

Add a notifier to create / destroy the device nodes for the tty view
and the fullscreen view. Only device nodes for online devices are
created and the device names will follow the convention as outlined
in Documentation/devices.txt: 3270/tty<x> for the tty nodes,
3270/tub<x> for hte fullscreen nodes and 3270/tub for the fullscreen
control node.

virtio_ccw_setup_vq() failed to unwind correctly on errors. In
particular, it failed to delete the virtqueue on errors, leading to
list corruption when virtio_ccw_del_vqs() iterated over a virtqueue
that had not been added to the vcdev's list.
Fix this with redoing the error unwinding in virtio_ccw_setup_vq(),
using a single path for all errors.

Commit 46d3ceab "tcp: TCP Small Queues" has severly degraded
performance for single connection RR workloads on HiperSockets with
MTU >=16K due to a conflict of the TCP Small Queues approach with our
buffer scan threshold which releases buffers not frequently enough yet.
This fix restores performance to the same level as before cited commit.

As of now, s390dbf entries for the cards are discarded as soon as the
device is removed. However, this will also bar us of all chances of
getting valuable debug information after a device has been removed.
This patch will keep the s390dbf entries around until the qeth module
is removed.

The existing port isolation mode 'forward' will now verify that the adjacent
switch port supports the required reflective relay (RR) mode. This patch adds
the required error handling for the cases where enabling port isolation mode
'forward' can now fail.
Furthermore, once established, we never fall back from one of the port
isolation modes to a non-isolated mode without further user-interaction.
This includes cases where the isolation mode was enabled successfully, but
ceases to work e.g. due to configuration changes at the switch port.
Finally, configuring an isolation mode with the device being offline
will make onlining the device fail permanently upon errors encountered until
either errors are resolved or the isolation mode is changed by the user to a
different mode.

The option allows you to remove TTY and compile without errors. This
saves space on systems that won't support TTY interfaces anyway.
bloat-o-meter output is below.
The bulk of this patch consists of Kconfig changes adding "depends on
TTY" to various serial devices and similar drivers that require the TTY
layer. Ideally, these dependencies would occur on a common intermediate
symbol such as SERIO, but most drivers "select SERIO" rather than
"depends on SERIO", and "select" does not respect dependencies.
bloat-o-meter output comparing our previous minimal to new minimal by
removing TTY. The list is filtered to not show removed entries with awk
'$3 != "-"' as the list was very long.

introduced a new SEI notification type as part of pci support.
The way SEI was called with nt2 and nt0 consecutive broke the nt0
stuff used for channel subsystem notifications.
The reason why this was broken with the mentioned patch is that you
cannot selectively disable type 0 notifications (so even when asked
for type 2 only, type 0 could be presented).
The way to do it is to tell SEI which types of notification you can
process and -this is the important part- look at the SEI result which
notification type you actually received.

Also we move tty_flip_buffer_push declaration from include/linux/tty.h
to include/linux/tty_flip.h to all others while we are changing it
anyway.

One point is to have less places where we actually need tty pointer.
The other is that low_latency is bound to buffer processing and
buffers are now in tty_port. So it makes sense to move low_latency to
tty_port too.

In rare cases a virtio command might try to issue a ccw before a former
ccw was answered with a tsch. This will cause CC=2 (busy). Lets just

Partially revert ae289dc1f "s390/3215: fix tty close handling", since this
leads sometimes to hanging agetty processes and therefore systems that get
stuck while starting.
This was magically fixed (bisected) by a common code patch from Alan Cox:
36b3c070 "tty: Move the handling of the tty release logic", however it was
unrelated.
Since the removed code worked for a decade, nobody knows anymore why it was
in there in the first place and debugging the observed hang is non-trivial
(at least for me :) ), let's just re-add the removed code before we see
other side effects.

Now that irq sum accounting for /proc/stat's "intr" line works again we
have the oddity that the sum field (first field) contains only the sum
of the second (external irqs) and third field (I/O interrupts).
The reason for that is that these two fields are already sums of all other
fields. So if we would sum up everything we would count every interrupt
twice.
This is broken since the split interrupt accounting was merged two years
ago: 052ff461c8 "[S390] irq: have detailed
statistics for interrupt types".
To fix this remove the split interrupt fields from /proc/stat's "intr"
line again and only have them in /proc/interrupts.
This restores the old behaviour, seems to be the only sane fix and mimics
a behaviour from other architectures where /proc/interrupts also contains
more than /proc/stat's "intr" line does.

For more than two years, since f2c66cd8ee
"/proc/stat: scalability of irq num per cpu" the output of /proc/stat is
broken.
The first field in the "intr" line should contain the sum of all interrupts,
however since the above mentioned change it is always zero.
The reason for that is that a per cpu irq sum variable had been introduced
which got incremented when calling kstat_incr_irqs_this_cpu(). However
on s390 we directly incremented only the per cpu per irq counter by accessing
the array element via kstat_cpu(smp_processor_id()).irqs[...].
So fix this and use the kstat_incr_irqs_this_cpu() wrapper which increments
both: the per cpu per irq counter and the per cpu irq sum counter.

Add a driver for kvm guests that matches virtual ccw devices provided
by the host as virtio bridge devices.
These virtio-ccw devices use a special set of channel commands in order
to perform virtio functions.

Running under a kvm host does not necessarily imply the presence of
a page mapped above the main memory with the virtio information;
however, the code includes a hard coded access to that page.
Instead, check for the presence of the page and exit gracefully
before we hit an addressing exception if it does not exist.

The PCI flag is misleading as this stands for "program controlled interrupt".
Rename it to PRI "program requested interrupt" which is more accurate and
does not interfere with another popular piece of technology.
Leave the pci string in the code since changing that would result in a huge
patch.

The check to whom a device is reserved is done by checking the path
state of the affected channel paths. If it turns out that one path is
flagged as reserved by someone else the whole device is marked as such.
However the meaning of the RESVD_ELSE bit is that the addressed device
is reserved to a different pathgroup (and not reserved to a different
LPAR). If we do this test on a path which is currently not a member of
the pathgroup we could erroneously mark the device as reserved to
someone else.
To fix this collect the reserved state for all potential members of the
pathgroup and only mark the device as reserved if all of those potential
members have the RESVD_ELSE bit set.

Add SCLP PCI configure/deconfigure and implement a PCI hotplug
controller (s390_pci_hpc). The hotplug controller creates a slot
for every PCI function in stand-by or configured state. The PCI
functions are named after the PCI function ID (fid). By writing to
the power attribute in /sys/bus/pci/slots/<fid>/power the PCI function
is moved to stand-by or configured state. If moved to the configured
state the device is automatically scanned by the s390 PCI layer.

The regular behavior of the DASD device driver when setting a device
offline is to return all outstanding I/O as failed. This behavior is
different from that of other System z operating systems and may lead
to unexpected data loss. Adding an explicit 'safe' offline function
will allow customers to use DASDs in the way they expect them to work.

If a channel path is cabled incorrectly and the device is suspended and
resumed the device may be inaccessible afterwards.
Make the path connection check not interrupt the resume callback there
could be other valid paths available.

Return -EINVAL if set_{on,off}line is called for already {on,off}line
devices. Also return -EINVAL for drivers which do not provide a callback
for set_{on,off}line. This behaviour is now consistent with the one for

After commit "TTY: move tty buffers to tty_port", the tty buffers are
not freed in some drivers. This is because tty_port_destructor is not
called whenever a tty_port is freed. This was an assumption I counted
with but was unfortunately untrue. So fix the drivers to fulfil this
assumption.
To be sure, the TTY buffers (and later some stuff) are gone along with
the tty_port, we have to call tty_port_destroy at tear-down places.
This is mostly where the structure containing a tty_port is freed.
This patch does exactly that -- put tty_port_destroy at those places.

The 3215 console always has the RAW3215_FIXED flag set, which causes
raw3215_shutdown() not to wait for outstanding I/O requests if an attached
tty gets closed.
The flag however can be simply removed, so we can guarantee that all requests
belonging to the tty have been processed when the tty is closed.
However the tasklet that belongs to the 3215 device may be scheduled even if
there is no tty attached anymore, since we have a race between console and tty
processing.
Thefore unconditional tty_wakekup() in raw3215_wakeup() can cause the following

The easiest solution is simply to check if tty is NULL in the tasklet.
If it is NULL nothing is to do (no tty attached), otherwise tty_wakeup()
can be called, since we hold a reference to the tty.
This is not nice... but it is a small patch and it works.

If the set_mac_address() function of qeth is invoked, qeth deletes
the old mac address first on OSA. Only if deletion returns
successfully the new mac address is set on OSA. Deletion may return
with a return value "MAC not found on OSA". In this case qeth
should continue setting the new mac address.
When the OSA cable is pulled, OSA forgets any set mac address. If
the OSA network interface acts as a slave to a bonding master
interface, bonding can invoke the set_mac_address function for
failover purposes and depends on successful setting of the new mac
address even though the old mac address could no longer be deleted.

Return codes of IPA_CMD_QIPASSIST are not checked, especially the ones which
indicate that the command is not supported. As a result, the device driver
would not enable all available features on older card generations.
This patch adds proper checking and sets the bare minimum in the supported
functions flags to avoid follow-on errors.

The recovery thread, when failing, tears down the respective interface. To do
so, it needs to obtain the rtnl lock first, as the interface configuration is
changed.
If another process tries to modify an interface setting at the same time, that
process can obtain the rtnl lock first, but the respective callback in the qeth
driver will block until recovery has completed - which cannot happen since the
calling process already obtained it.
In one particular case, the bonding driver acquired the rtnl lock to modify the
card's MAC address, while the recovery failed at the same time due to the card
being removed. Hence qeth_l2_set_mac_address (implicitly holding the rtnl lock)
was waiting on qeth_l2_recover, which deadlocked when waiting on the rtnl lock.
This patch uses rtnl_trylock instead of rtnl_lock in the recovery thread. If the
lock cannot be obtained, the interface will be left up, but the card state
remains in CARD_STATE_RECOVER, which will prevent any further activities on the
card.

As we skipped the merge window for 3.6-rc1 for the tty tree, everything
is now settled down and working properly, so we are ready for 3.7-rc1.
Here's the patchset, it's big, but the large changes are removing a
firmware file and adding a staging tty driver (it depended on the tty
core changes, so it's going through this tree instead of the staging
tree.)
All of these patches have been in the linux-next tree for a while.

virtio network device multiqueue support reserves
vq 3 for future use (useful both for future extensions and to make it
pretty - this way receive vqs have even and transmit - odd numbers).
Make it possible to skip initialization for
specific vq numbers by specifying NULL for name.
Document this usage as well as (existing) NULL callback.
Drivers using this not coded up yet, so I simply tested
with virtio-pci and verified that this patch does
not break existing drivers.

Instead of storing the queue index in transport-specific virtio structs,
this patch moves them to vring_virtqueue and introduces an helper to get
the value. This lets drivers simplify their management and tracing of
virtqueues.

A common way to prepare a z/VM mini disk is to format the real device
with a z/VM tool like CPFMTXA and then define a mini disk that excludes
the first cylinder, i.e. the cylinder 0 of the virtual disk is located
at cylinder 1 of the real device.
The DASD device driver will recognize such a mini disk as formatted, as
the uniform record layout on the disk matches that of an LDL formatted
device. However, the cylinder value in the 'count' field of the ECKD
records matches the geometry of the real device, and not that of the
mini disk, so I/O requests will fail with 'record not found' errors.
To make the mini disk usable, it needs to be formatted with a tool like
dasdfmt. To enable tools like distribution installation tools to
recognize this situation, the DASD device driver should report such a
mini disk as 'not formatted'.
To this end we need to extend the device recognition code to check not
just for proper record sizes, but also for proper cylinder/head/record
values.

Calling debug_event the s390 debug feature only logs up to buf_size
bytes of the debug view. If debug_event is called with more bytes
than buf_size the additional data is ignored and not logged in the
debug view.
Use multiple calls to debug_event if the length exceeds buf_size.

Changing the return value of vmlogrdr_open() to -EOPNOTSUPP if O_NONBLOCK
is specified shouldn't have any negative side effects.
Any existing user wouldn't specify that flag since it wouldn't work anyway.

Change return code handling of the stsi() function:
In case function code 0 was specified the return value is the
current configuration level (already shifted). That way all
the code that actually copied the stsi_0() function can go
away.
Otherwise the return value is 0 (success) or negative to
indicate an error (currently only -EOPNOTSUPP).
Also stsi() is no longer an inline function. The function is
not performance critical, but every caller would generate an
exception table entry for this function.

does not free before leaving from the error handling cases. The
