Historically rdmavt destroy_ah() has returned an -EBUSY when the AH has a
non-zero reference count. IBTA 11.2.2 notes no such return value or error
case:
Output Modifiers:
- Verb results:
- Operation completed successfully.
- Invalid HCA handle.
- Invalid address handle.
ULPs never test for this error and this will leak memory.
The reference count exists to allow for driver independent progress
mechanisms to process UD SWQEs in parallel with post sends. The SWQE will
hold a reference count until the UD SWQE completes and then drops the
reference.
Fix by removing need to reference count the AH. Add a UD specific
allocation to each SWQE entry to cache the necessary information for
independent progress. Copy the information during the post send
processing.

Prior to commit d345691471 ("RDMA: Handle AH allocations by IB/core"),
AH destroy path is rdmavt returned -EBUSY warning to application and
caused to potential leakage of kernel memory of AH structure.
After that commit, the AH structure is always freed but such early return
in driver code can potentially cause to use-after-free error.
Add warning to catch such situation to help driver developers to fix AH
release path.

Simplify drivers by ensuring lifetime of ib_ah object. The changes
in .create_ah() go hand in hand with relevant update in .destroy_ah().
We will use this opportunity and convert .destroy_ah() to don't fail, as
it was suggested a long time ago, because there is nothing to do in case
of failure during destroy.

The uverbs_attr_bundle with the ucontext is sent down to the drivers ib_x
destroy path as ib_udata. The next patch will use the ib_udata to free the
drivers destroy path from the dependency in 'uobject->context' as we
already did for the create path.

flag that marks whether the callback is executed in an atomic context or
not.
This will allow drivers to wait for completion instead of polling for it
when it is allowed.

rdmavt uses a crazy system that looses the type checking when assinging
functions to struct ib_device function pointers. Because of this the
signature to this function was not changed when the below commit revised
things.
Fix the signature so we are not calling a function pointer with a
mismatched signature.

Introduce AH attribute copy, move and replace APIs to be used by core and
provider drivers.
In CM code flow when ah attribute might be re-initialized twice while
processing incoming request, or initialized once while from path record
while sending out CM requests. Therefore use rdma_move_ah_attr API to
handle such scenarios instead of memcpy().
Provider drivers keeps a copy ah_attr during the lifetime of the ah.
Therefore, use rdma_replace_ah_attr() which conditionally release
reference to old ah_attr and holds reference to new attribute whose
referrence is released when the AH is freed.

rvt_check_ah() delegates lid verification to underlying
driver. Underlying driver uses different conditions to
check for dlid depending on whether the device supports
extended LIDs

Add, remove, and otherwise clean up existing comments that are leftover
from the initial code postings of rdmavt. Many of the comments were added
to provide an idea on the direction we were thinking of going. Now that the
design is solidified make a pass over and clean everything up. Also add
details where lacking.

Until all functionality is moved over to rdmavt drivers still need to
access a number of fields in data structures that are predominantly
meant to be used by rdmavt. Once these rdmavt_<ibta_object>.h header
files are no longer being touched by drivers their content should be
moved to rdmavt/<ibta_object>.h. While here move a couple #defines
over to more general IB verbs header files because they fit better.
