The only part of proc_dir_entry the code outside of fs/proc
really cares about is PDE(inode)->data. Provide a helper
for that; static inline for now, eventually will be moved
to fs/proc, along with the knowledge of struct proc_dir_entry
layout.

This patch (as1675) removes the CONFIG_USB_SUSPEND option, essentially
replacing it everywhere with CONFIG_PM_RUNTIME (except for one place
in hub.c, where it is replaced with CONFIG_PM because the code needs
to be used in both runtime and system PM). The net result is code
shrinkage and simplification.
There's very little point in keeping CONFIG_USB_SUSPEND because almost
everybody enables it. The few that don't will find that the usbcore
module has gotten somewhat bigger and they will have to take active
measures if they want to prevent hubs from being runtime suspended.

Commit e66eed651f ("list: remove prefetching from regular list
iterators") removed the include of prefetch.h from list.h, which
uncovered several cases that had apparently relied on that rather
obscure header file dependency.
So this fixes things up a bit, using
grep -L linux/prefetch.h $(git grep -l '[^a-z_]prefetchw*(' -- '*.[ch]')
grep -L 'prefetchw*(' $(git grep -l 'linux/prefetch.h' -- '*.[ch]')
to guide us in finding files that either need <linux/prefetch.h>
inclusion, or have it despite not needing it.
There are more of them around (mostly network drivers), but this gets
many core ones.

Some bluetooth dongles want ISO mode, and the limited support that the
sl811 offers today is sufficient. So add a Kconfig option for people
to optionally get access to the partial functionality.

Update the USB core to deal with USB 3.0 hubs. These hubs have a slightly
different hub descriptor than USB 2.0 hubs, with a fixed (rather than
variable length) size. Change the USB core's hub descriptor to have a
union for the last fields that differ. Change the host controller drivers
that access those last fields (DeviceRemovable and PortPowerCtrlMask) to
use the union.
Translate the new version of the hub port status field into the old
version that khubd understands. (Note: we need to fix it to translate the
roothub's port status once we stop converting it to USB 2.0 hub status
internally.)
Add new code to handle link state change status. Send out new control
messages that are needed for USB 3.0 hubs, like Set Hub Depth.
This patch is a modified version of the original patch submitted by John
Youn. It's updated to reflect the removal of the "bitmap" #define, and
change the hub descriptor accesses of a couple new host controller
drivers.

Using a #define to redefine a common variable name is a bad thing,
especially when the #define is in a header. include/linux/usb/hcd.h
redefined bitmap to DeviceRemovable to avoid typing a long field in the
hub descriptor. This has unintended side effects for files like
drivers/usb/core/devio.c that include that file, since another header
included after hcd.h has different variables named bitmap.
Remove the bitmap #define and replace instances of it in the host
controller code. Cleanup the spaces around function calls and square
brackets while we're at it.

In drivers/usb/host/sl811-hcd.c::sl811h_urb_enqueue(), memory is allocated
with kzalloc() and assigned to 'ep'. If we leave via the 'fail' label due
to 'if (ep->maxpacket > H_MAXPACKET)', then 'ep' will go out of scope
without having been assigned to anything, so we'll leak the memory we
allocated.
This patch fixes the leak by simply calling kfree(ep); before jumping to
the 'fail' label.

This patch (as1349b) clears up the confusion in many USB host
controller drivers between port features and port statuses. In mosty
cases it's true that the status bit is in the position given by the
corresponding feature value, but that's not always true and it's not
guaranteed in the USB spec.
There's no functional change, just replacing expressions of the form
(1 << USB_PORT_FEAT_x) with USB_PORT_STAT_x, which has the same value.

This patch (as1348) removes the bogus
USB_PORT_FEAT_{HIGHSPEED,SUPERSPEED} symbols from ch11.h. No such
features are defined by the USB spec. (There is a PORT_LOWSPEED
feature, but the spec doesn't mention it except to say that host
software should never use it.) The speed indicators are port
statuses, not port features.
As a temporary workaround for the xhci-hcd driver, a fictional
USB_PORT_STAT_SUPER_SPEED symbol is added.

The usbcore headers: hcd.h and hub.h are shared between usbcore,
HCDs and a couple of other drivers (e.g. USBIP modules).
So, it makes sense to move them into a more public location and
to cleanup dependency of those modules on kernel internal headers.
This patch moves hcd.h from drivers/usb/core into include/linux/usb/

A while ago I provided a patch that fixed device detection after device
removal (USB: sl811-hcd: Fix device disconnect).
Chris Brissette pointed out that the detection/removal counter method
to distinguish insert or remove my fail under certain conditions.
Latest SL811HS datasheet (Document 38-08008 Rev. *D) indicates that
bit 6 (SL11H_INTMASK_RD) of the Interrupt Status Register together with
bit 5 (SL11H_INTMASK_INSRMV) can be used to determine whether a device
has been inserted or removed.

SL811 Device detected after removal used to be working in linux-2.6.22
but then broke somewhere between 2.6.22 and 2.6.28. Current
hub_port_connect_change() in drivers/usb/core/hub.c won't call
usb_disconnect() in case the SL811 driver sets portstatus
USB_PORT_FEAT_CONNECTION upon removal.
AFAIK the SL811 has only a combined Device Insert/Remove
detection bit, therefore use a count to distinguish insert or remove.

Roel Kluin pointed out that transfer_buffer_lengths in struct urb was
declared as an 'int'. This patch changes this field to be 'u32' to
prevent any potential negative conversion and comparison errors.
This triggered a few compiler warning messages when these fields were
being used with the min macro, so they have also been fixed up in this
patch.

This patch let a few discrete USB host controllers drivers (isp116x-hcd,
r8a66597-hcd and sl811-hcd) obtain IRQ flags from their IORESOURCE_IRQ
resource if configured as such, much like it's been done for the smc91x
driver.
It spares people writing support for specific boards the burden to
configure the interrupt controller independantly, and keeps all IRQ
related information in a single resource.
HCD that are integrally part of a SoC have been left aside, as there
is probably no "wiring" options...
Tested on an Xscale PXA-255 based platform with isp116x-hcd.

We want to use WARN() as a variant of WARN_ON(), however a few drivers are
using WARN() internally. This patch renames these to WARNING() to avoid the
namespace clash. A few cases were defining but not using the thing, for those
cases I just deleted the definition.

power.power_state is scheduled for removal. This patch (as1053)
removes all uses of that field from drivers/usb. Almost all of them
were write-only, the most significant exceptions being sl811-hcd.c and
u132-hcd.c.

During the last step of hibernation in the "platform" mode (with the
help of ACPI) we use the suspend code, including the devices'
->suspend() methods, to prepare the system for entering the ACPI S4
system sleep state.
But at least for some devices the operations performed by the
->suspend() callback in that case must be different from its operations
during regular suspend.
For this reason, introduce the new PM event type PM_EVENT_HIBERNATE and
pass it to the device drivers' ->suspend() methods during the last phase
of hibernation, so that they can distinguish this case and handle it as
appropriate. Modify the drivers that handle PM_EVENT_SUSPEND in a
special way and need to handle PM_EVENT_HIBERNATE in the same way.
These changes are necessary to fix a hibernation regression related
to the i915 driver (ref. http://lkml.org/lkml/2008/2/22/488).

This patch (as976) reorganizes the way sl811-hcd sets urb->status. It
now keeps the information in a local variable until the last moment.
The patch also improves the handling of faults during the status stage
of a control transfer, since it no longer needs to retain the error
information from the earlier stages.

This patch (as970) adds a new urb->unlinked field, which is used to
store the status of unlinked URBs since we can't use urb->status for
that purpose any more. To help simplify the HCDs, usbcore will check
urb->unlinked before calling the completion handler; if the value is
set it will automatically override the status reported by the HCD.

This patch (as969) continues the ongoing changes to the way HCDs
report URB statuses. The programming interface has been simplified by
making usbcore responsible for clearing urb->hcpriv and for setting
-EREMOTEIO status when an URB with the URB_SHORT_NOT_OK flag ends up
as a short transfer.
By moving the work out of the HCDs, this removes a fair amount of
repeated code.

This patch (as954) implements a suggestion of David Brownell's. Now
the host controller drivers are responsible for linking and unlinking
URBs to/from their endpoint queues. This eliminates the possiblity of
strange situations where usbcore thinks an URB is linked but the HCD
thinks it isn't. It also means HCDs no longer have to check for URBs
being dequeued before they were fully enqueued.
In addition to the core changes, this requires changing every host
controller driver and the root-hub URB handler. For the most part the
required changes are fairly small; drivers have to call
usb_hcd_link_urb_to_ep() in their urb_enqueue method,
usb_hcd_check_unlink_urb() in their urb_dequeue method, and
usb_hcd_unlink_urb_from_ep() before giving URBs back. A few HCDs make
matters more complicated by the way they split up the flow of control.
In addition some method interfaces get changed. The endpoint argument
for urb_enqueue is now redundant so it is removed. The unlink status
is required by usb_hcd_check_unlink_urb(), so it has been added to
urb_dequeue.

Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.
The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around. On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).
Where appropriate, an arch may override the generic storage facility and do
something different with the variable. On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.
Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions. Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller. A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.
I've build this code with allyesconfig for x86_64 and i386. I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.
This will affect all archs. Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:
struct pt_regs *old_regs = set_irq_regs(regs);
And put the old one back at the end:
set_irq_regs(old_regs);
Don't pass regs through to generic_handle_irq() or __do_IRQ().
In timer_interrupt(), this sort of change will be necessary:
- update_process_times(user_mode(regs));
- profile_tick(CPU_PROFILING, regs);
+ update_process_times(user_mode(get_irq_regs()));
+ profile_tick(CPU_PROFILING);
I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().
Some notes on the interrupt handling in the drivers:
(*) input_dev() is now gone entirely. The regs pointer is no longer stored in
the input_dev struct.
(*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking. It does
something different depending on whether it's been supplied with a regs
pointer or not.
(*) Various IRQ handler function pointers have been moved to type
irq_handler_t.

The purpose of this patch is to split off the case when a device does
not reply on the lower level (which is reported by HC hardware), and
a case when the device accepted the request, but does not reply at
upper level. This redefinition allows to diagnose issues easier,
without asking the user if the -110 happened "immediately".
The usbmon splits such cases already thanks to its timestamp, but
it's not always available.
I adjusted all drivers which I found affected (by searching for "urb").
Out of tree drivers may suffer a little bit, but I do not expect much
breakage. At worst they may print a few messages.

This teaches several USB host controller drivers to treat PRETHAW as a chip
reset since the controller, and all devices connected to it, are no longer in
states compatible with how the snapshotted suspend() left them.

This patch (as640) removes several put_device and the corresponding
get_device calls from the USB core and HCDs. Some of the puts were done
in atomic contexts, and none of them are needed since the core now
guarantees that every endpoint will be disabled and every URB completed
before a USB device is released.
