On some architectures (like ARM), virt_to_gfn cannot be used for
vmalloc'd memory because of its reliance on virt_to_phys. This patch
introduces a check for vmalloc'd addresses and obtains the PFN using
vmalloc_to_pfn in that case.

XEN data pointer which contains XEN specific information.
handler data is meant for interrupt handlers and not for storing irq chip
specific information as some devices require handler data to store internal
per interrupt information, e.g. pinctrl/GPIO chained interrupt handlers.
This obviously creates a conflict of interests and crashes the machine
because the XEN pointer is overwritten by the driver pointer.
As the XEN data is not handler specific it should be stored in
irqdesc::irq_data::chip_data instead.
A simple sed s/irq_[sg]et_handler_data/irq_[sg]et_chip_data/ cures that.

It is possible that the scatter-gather table during dmabuf import has
non-zero offset of the data, but user-space doesn't expect that.
Fix this by failing the import, so user-space doesn't access wrong data.

Xen is requiring 64-bit machines today and since Xen 4.14 it can be
built without 32-bit PV guest support. There is no need to carry the
burden of 32-bit PV guest support in the kernel any longer, as new
guests can be either HVM or PVH, or they can use a 64 bit kernel.
Remove the 32-bit Xen PV support from the kernel.

Patch series "mm: cleanup usage of <asm/pgalloc.h>"
Most architectures have very similar versions of pXd_alloc_one() and
pXd_free_one() for intermediate levels of page table. These patches add
generic versions of these functions in <asm-generic/pgalloc.h> and enable
use of the generic functions where appropriate.
In addition, functions declared and defined in <asm/pgalloc.h> headers are
used mostly by core mm and early mm initialization in arch and there is no
actual reason to have the <asm/pgalloc.h> included all over the place.
The first patch in this series removes unneeded includes of
<asm/pgalloc.h>
In the end it didn't work out as neatly as I hoped and moving
pXd_alloc_track() definitions to <asm-generic/pgalloc.h> would require
unnecessary changes to arches that have custom page table allocations, so
I've decided to move lib/ioremap.c to mm/ and make pgalloc-track.h local
to mm/.
This patch (of 8):
In most cases <asm/pgalloc.h> header is required only for allocations of
page table memory. Most of the .c files that include that header do not
use symbols declared in <asm/pgalloc.h> and do not require that header.
As for the other header files that used to include <asm/pgalloc.h>, it is
possible to move that include into the .c file that actually uses symbols
from <asm/pgalloc.h> and drop the include from the header file.
The process was somewhat automated using

xen_dma_sync_for_cpu, xen_dma_sync_for_device, xen_arch_need_swiotlb are
getting called passing dma addresses. On some platforms dma addresses
could be different from physical addresses. Before doing any operations
on these addresses we need to convert them back to physical addresses
using dma_to_phys.
Move the arch_sync_dma_for_cpu and arch_sync_dma_for_device calls from
xen_dma_sync_for_cpu/device to swiotlb-xen.c, and add a call dma_to_phys
to do address translations there.
dma_cache_maint is fixed by the next patch.

With some devices physical addresses are different than dma addresses.
To be able to deal with these cases, we need to call phys_to_dma on
physical addresses (including machine addresses in Xen terminology)
before returning them from xen_swiotlb_alloc_coherent and
xen_swiotlb_map_page.
We also need to convert dma addresses back to physical addresses using
dma_to_phys in xen_swiotlb_free_coherent and xen_swiotlb_unmap_page if
we want to do any operations on them.
Call dma_to_phys in is_xen_swiotlb_buffer.
Introduce xen_phys_to_dma and call phys_to_dma in its implementation.
Introduce xen_dma_to_phys and call dma_to_phys in its implementation.
Call xen_phys_to_dma/xen_dma_to_phys instead of
xen_phys_to_bus/xen_bus_to_phys through swiotlb-xen.c.
Everything is taken care of by these changes except for
xen_swiotlb_alloc_coherent and xen_swiotlb_free_coherent, which need a
few explicit phys_to_dma/dma_to_phys calls.

It is not strictly needed. Call virt_to_phys on xen_io_tlb_start
instead. It will be useful not to have a start_dma_addr around with the
next patches.
Note that virt_to_phys is not the same as xen_virt_to_bus but actually
it is used to compared again __pa(xen_io_tlb_start) as passed to
swiotlb_init_with_tbl, so virt_to_phys is actually what we want.

xen_alloc_coherent_pages might return pages for which virt_to_phys and
virt_to_page don't work, e.g. ioremap'ed pages.
So in xen_swiotlb_free_coherent we can't assume that virt_to_page works.
Instead add a is_vmalloc_addr check and use vmalloc_to_page on vmalloc
virt addresses.
This patch fixes the following crash at boot on RPi4 (the underlying
issue is not RPi4 specific):

This has been fixed by commit dca4436d1c which added the out
of bounds check to __add_memory, so that trying to add blocks past
MAX_PHYSMEM_BITS will fail.
Note the check in the Xen balloon driver was bogus anyway, as it
checked the start address of the resource, but it should instead test
the end address to assert the whole resource falls below
MAX_PHYSMEM_BITS.

target_unpopulated is incremented with nr_pages at the start of the
function, but the call to free_xenballooned_pages will only subtract
pgno number of pages, and thus the rest need to be subtracted before
returning or else accounting will be skewed.

Previously, if lock_pages() end up partially mapping pages, it used
to return -ERRNO due to which unlock_pages() have to go through
each pages[i] till *nr_pages* to validate them. This can be avoided
by passing correct number of partially mapped pages & -ERRNO separately,
while returning from lock_pages() due to error.
With this fix unlock_pages() doesn't need to validate pages[i] till
*nr_pages* for error scenario and few condition checks can be ignored.

Some Makefiles already pass -fno-stack-protector unconditionally.
For example, arch/arm64/kernel/vdso/Makefile, arch/x86/xen/Makefile.
No problem report so far about hard-coding this option. So, we can
assume all supported compilers know -fno-stack-protector.
GCC 4.8 and Clang support this option (https://godbolt.org/z/_HDGzN)
Get rid of cc-option from -fno-stack-protector.
Remove CONFIG_CC_HAS_STACKPROTECTOR_NONE, which is always 'y'.
Note:
arch/mips/vdso/Makefile adds -fno-stack-protector twice, first
unconditionally, and second conditionally. I removed the second one.

no problem as all callers of xenbus_map_ring_valloc() only use the
return value to print an error message, and in case of mapping errors
the grant status value has already been printed by __xenbus_map_ring()
before.

xenbus_map_ring_valloc() and its sub-functions are putting quite large
structs and arrays on the stack. This is problematic at runtime, but
might also result in build failures (e.g. with clang due to the option
-Werror,-Wframe-larger-than=... used).
Fix that by moving most of the data from the stack into a dynamically
allocated struct. Performance is no issue here, as
xenbus_map_ring_valloc() is used only when adding a new PV device to
a backend driver.
While at it move some duplicated code from pv/hvm specific mapping
functions to the single caller.

This all started about 6 month ago with the attempt to move the Posix CPU
timer heavy lifting out of the timer interrupt code and just have lockless
quick checks in that code path. Trivial 5 patches.
This unearthed an inconsistency in the KVM handling of task work and the
review requested to move all of this into generic code so other
architectures can share.
Valid request and solved with another 25 patches but those unearthed
inconsistencies vs. RCU and instrumentation.
Digging into this made it obvious that there are quite some inconsistencies
vs. instrumentation in general. The int3 text poke handling in particular
was completely unprotected and with the batched update of trace events even
more likely to expose to endless int3 recursion.
In parallel the RCU implications of instrumenting fragile entry code came
up in several discussions.
The conclusion of the X86 maintainer team was to go all the way and make
the protection against any form of instrumentation of fragile and dangerous
code pathes enforcable and verifiable by tooling.
A first batch of preparatory work hit mainline with commit d5f744f9a2.
The (almost) full solution introduced a new code section '.noinstr.text'
into which all code which needs to be protected from instrumentation of all
sorts goes into. Any call into instrumentable code out of this section has
to be annotated. objtool has support to validate this. Kprobes now excludes
this section fully which also prevents BPF from fiddling with it and all
'noinstr' annotated functions also keep ftrace off. The section, kprobes
and objtool changes are already merged.
The major changes coming with this are:
- Preparatory cleanups
- Annotating of relevant functions to move them into the noinstr.text
section or enforcing inlining by marking them __always_inline so the
compiler cannot misplace or instrument them.
- Splitting and simplifying the idtentry macro maze so that it is now
clearly separated into simple exception entries and the more
interesting ones which use interrupt stacks and have the paranoid
handling vs. CR3 and GS.
- Move quite some of the low level ASM functionality into C code:
- enter_from and exit to user space handling. The ASM code now calls
into C after doing the really necessary ASM handling and the return
path goes back out without bells and whistels in ASM.
- exception entry/exit got the equivivalent treatment
- move all IRQ tracepoints from ASM to C so they can be placed as
appropriate which is especially important for the int3 recursion
issue.
- Consolidate the declaration and definition of entry points between 32
and 64 bit. They share a common header and macros now.
- Remove the extra device interrupt entry maze and just use the regular
exception entry code.
- All ASM entry points except NMI are now generated from the shared header
file and the corresponding macros in the 32 and 64 bit entry ASM.
- The C code entry points are consolidated as well with the help of
DEFINE_IDTENTRY*() macros. This allows to ensure at one central point
that all corresponding entry points share the same semantics. The
actual function body for most entry points is in an instrumentable
and sane state.
There are special macros for the more sensitive entry points,
e.g. INT3 and of course the nasty paranoid #NMI, #MCE, #DB and #DF.
They allow to put the whole entry instrumentation and RCU handling
into safe places instead of the previous pray that it is correct
approach.
- The INT3 text poke handling is now completely isolated and the
recursion issue banned. Aside of the entry rework this required other
isolation work, e.g. the ability to force inline bsearch.
- Prevent #DB on fragile entry code, entry relevant memory and disable
it on NMI, #MC entry, which allowed to get rid of the nested #DB IST
stack shifting hackery.
- A few other cleanups and enhancements which have been made possible
through this and already merged changes, e.g. consolidating and
further restricting the IDT code so the IDT table becomes RO after
init which removes yet another popular attack vector
- About 680 lines of ASM maze are gone.
There are a few open issues:
- An escape out of the noinstr section in the MCE handler which needs
some more thought but under the aspect that MCE is a complete
trainwreck by design and the propability to survive it is low, this was
not high on the priority list.
- Paravirtualization
When PV is enabled then objtool complains about a bunch of indirect
calls out of the noinstr section. There are a few straight forward
ways to fix this, but the other issues vs. general correctness were
more pressing than parawitz.
- KVM
KVM is inconsistent as well. Patches have been posted, but they have
not yet been commented on or picked up by the KVM folks.
- IDLE
Pretty much the same problems can be found in the low level idle code
especially the parts where RCU stopped watching. This was beyond the
scope of the more obvious and exposable problems and is on the todo
list.
The lesson learned from this brain melting exercise to morph the evolved
code base into something which can be validated and understood is that once
again the violation of the most important engineering principle
"correctness first" has caused quite a few people to spend valuable time on
problems which could have been avoided in the first place. The "features
first" tinkering mindset really has to stop.

Convert the XEN/PV hypercall to IDTENTRY:
- Emit the ASM stub with DECLARE_IDTENTRY
- Remove the ASM idtentry in 64-bit
- Remove the open coded ASM entry code in 32-bit
- Remove the old prototypes
The handler stubs need to stay in ASM code as they need corner case handling
and adjustment of the stack pointer.
Provide a new C function which invokes the entry/exit handling and calls
into the XEN handler on the interrupt stack if required.
The exit code is slightly different from the regular idtentry_exit() on
non-preemptible kernels. If the hypercall is preemptible and need_resched()
is set then XEN provides a preempt hypercall scheduling function.
Move this functionality into the entry code so it can use the existing
idtentry functionality.

As a preparatory change for making alloc_intr_gate() __init split
xen_callback_vector() into callback vector setup via hypercall
(xen_setup_callback_vector()) and interrupt gate allocation
(xen_alloc_callback_vector()).
xen_setup_callback_vector() is being called twice: on init and upon
system resume from xen_hvm_post_suspend(). alloc_intr_gate() only
needs to be called once.

Patch series "mm: consolidate definitions of page table accessors", v2.
The low level page table accessors (pXY_index(), pXY_offset()) are
duplicated across all architectures and sometimes more than once. For
instance, we have 31 definition of pgd_offset() for 25 supported
architectures.
Most of these definitions are actually identical and typically it boils
down to, e.g.

These definitions can be shared among 90% of the arches provided
XYZ_SHIFT, PTRS_PER_XYZ and xyz_page_vaddr() are defined.
For architectures that really need a custom version there is always
possibility to override the generic version with the usual ifdefs magic.
These patches introduce include/linux/pgtable.h that replaces
include/asm-generic/pgtable.h and add the definitions of the page table
accessors to the new header.
This patch (of 12):
The linux/mm.h header includes <asm/pgtable.h> to allow inlining of the
functions involving page table manipulations, e.g. pte_alloc() and
pmd_alloc(). So, there is no point to explicitly include <asm/pgtable.h>
in the files that include <linux/mm.h>.
The include statements in such cases are remove with a simple loop:

Without it a PVH dom0 is mostly useless, as it would balloon down huge
amounts of RAM in order get physical address space to map foreign
memory and grants, ultimately leading to an out of memory situation.
Such option is also needed for HVM or PVH driver domains, since they
also require mapping grants into physical memory regions.

Commit a926f81d2f ("xen/cpuhotplug: Replace cpu_up/down() with
device_online/offline()") replaced cpu_down() with device_offline()
call which requires that the CPU has been registered before. This
registration, however, happens later from topology_init() which
is called as subsys_initcall(). setup_vcpu_hotplug_event(), on the
other hand, is invoked earlier, during arch_initcall().
As result, booting a PV(H) guest with vcpus < maxvcpus causes a crash.
Move setup_vcpu_hotplug_event() (and therefore setup_cpu_watcher()) to
late_initcall(). In addition, instead of performing all offlining steps
in setup_cpu_watcher() simply call disable_hotplug_cpu().

This can be surprising to callers that might incorrectly be expecting
atomicity for accesses to aggregate structures, although there are other
callers where tearing is actually permissable (e.g. if they are using
something akin to sequence locking to protect the access).

architectures, as these appear to be widespread and tend to work out ok
if either the architecture supports atomic 64-bit accesses (x86, armv7)
or if the variable being accesses represents a virtual address and
therefore only requires 32-bit atomicity in practice.
Take a step in that direction by introducing a variant of
'compiletime_assert_atomic_type()' and use it to check the pointer
argument to {READ,WRITE}_ONCE(). Expose __{READ,WRITE}_ONCE() variants
which are allowed to tear and convert the one broken caller over to the
new macros.

xenbus_map_ring_valloc() maps a ring page and returns the status of the
used grant (0 meaning success).
There are Xen hypervisors which might return the value 1 for the status
of a failed grant mapping due to a bug. Some callers of
xenbus_map_ring_valloc() test for errors by testing the returned status
to be less than zero, resulting in no error detected and crashing later
due to a not available ring page.
Set the return value of xenbus_map_ring_valloc() to GNTST_general_error
in case the grant status reported by Xen is greater than zero.
This is part of XSA-316.

xen_pcibk_get_interrupt_type() assumes INTERRUPT_TYPE_NONE being 0
(initialize ret to 0 and return as INTERRUPT_TYPE_NONE).
Fix the definition to make INTERRUPT_TYPE_NONE really 0, and also shift
other values to not leave holes.
But also, do not assume INTERRUPT_TYPE_NONE being 0 anymore to avoid
similar confusions in the future.

The core device API performs extra housekeeping bits that are missing
from directly calling cpu_up/down().
See commit a6717c01dd ("powerpc/rtas: use device model APIs and
serialization during LPM") for an example description of what might go
wrong.
This also prepares to make cpu_up/down() a private interface of the cpu
subsystem.

The current codebase makes use of the zero-length array language
extension to the C90 standard, but the preferred mechanism to declare
variable-length types such as these ones is a flexible array member[1][2],
introduced in C99:

By making use of the mechanism above, we will get a compiler warning
in case the flexible array does not occur last in the structure, which
will help us prevent some kind of undefined behavior bugs from being
inadvertently introduced[3] to the codebase from now on.
Also, notice that, dynamic memory allocations won't be affected by
this change:
"Flexible array members have incomplete type, and so the sizeof operator
may not be applied. As a quirk of the original implementation of
zero-length arrays, sizeof evaluates to zero."[1]
This issue was found with the help of Coccinelle.

A driver's 'reclaim_memory' callback can race with 'probe' or 'remove'
because it will be called whenever memory pressure is detected. To
avoid such race, this commit embeds a spinlock in each 'xenbus_device'
and make 'xenbus' to hold the lock while the corresponded callbacks are
running.

Granting pages consumes backend system memory. In systems configured
with insufficient spare memory for those pages, it can cause a memory
pressure situation. However, finding the optimal amount of the spare
memory is challenging for large systems having dynamic resource
utilization patterns. Also, such a static configuration might lack
flexibility.
To mitigate such problems, this commit adds a memory reclaim callback to
'xenbus_driver'. If a memory pressure is detected, 'xenbus' requests
every backend driver to volunarily release its memory.
Note that it would be able to improve the callback facility for more
sophisticated handlings of general pressures. For example, it would be
possible to monitor the memory consumption of each device and issue the
release requests to only devices which causing the pressure. Also, the
callback could be extended to handle not only memory, but general
resources. Nevertheless, this version of the implementation defers such
sophisticated goals as a future work.

Commit 3aa6c19d2f ("xen/balloon: Support xend-based toolstack")
tried to fix a regression with running on rather ancient Xen versions.
Unfortunately the fix was based on the assumption that xend would
just use another Xenstore node, but in reality only some downstream
versions of xend are doing that. The upstream xend does not write
that Xenstore node at all, so the problem must be fixed in another
way.
The easiest way to achieve that is to fall back to the behavior
before commit 96edd61dcf ("xen/balloon: don't online new memory
initially") in case the static memory maximum can't be read.
This is achieved by setting static_max to the current number of
memory pages known by the system resulting in target_diff becoming
zero.

QEMU running in a stubdom needs to be able to set INTX_DISABLE, and the
MSI(-X) enable flags in the PCI config space. This adds an attribute
'allow_interrupt_control' which when set for a PCI device allows writes
to this flag(s). The toolstack will need to set this for stubdoms.
When enabled, guest (stubdomain) will be allowed to set relevant enable
flags, but only one at a time - i.e. it refuses to enable more than one
of INTx, MSI, MSI-X at a time.
This functionality is needed only for config space access done by device
model (stubdomain) serving a HVM with the actual PCI device. It is not
necessary and unsafe to enable direct access to those bits for PV domain
with the device attached. For PV domains, there are separate protocol
messages (XEN_PCI_OP_{enable,disable}_{msi,msix}) for this purpose.
Those ops in addition to setting enable bits, also configure MSI(-X) in
dom0 kernel - which is undesirable for PCI passthrough to HVM guests.
This should not introduce any new security issues since a malicious
guest (or stubdom) can already generate MSIs through other ways, see
[1] page 8. Additionally, when qemu runs in dom0, it already have direct
access to those bits.
This is the second iteration of this feature. First was proposed as a
direct Xen interface through a new hypercall, but ultimately it was
rejected by the maintainer, because of mixing pciback and hypercalls for
PCI config space access isn't a good design. Full discussion at [2].

If a driver probe() fails then leave the xenstore state alone. There is no
reason to modify it as the failure may be due to transient resource
allocation issues and hence a subsequent probe() may succeed.
If the driver supports re-binding then only force state to closed during
remove() only in the case when the toolstack may need to clean up. This can
be detected by checking whether the state in xenstore has been set to
closing prior to device removal.
NOTE: Re-bind support is indicated by new boolean in struct xenbus_driver,
which defaults to false. Subsequent patches will add support to
some backend drivers.

xenbus_dev_shutdown() is seemingly intended to cause clean shutdown of PV
frontends when a guest is rebooted. Indeed the function waits for a
conpletion which is only set by a call to xenbus_frontend_closed().
This patch removes the shutdown() method from backends and moves
xenbus_dev_shutdown() from xenbus_probe.c into xenbus_probe_frontend.c,
renaming it appropriately and making it static.
NOTE: In the case where the backend is running in a driver domain, the
toolstack should have already terminated any frontends that may be
using it (since Xen does not support re-startable PV driver domains)
so xenbus_dev_shutdown() should never be called.

CONFIG_PREEMPTION is selected by CONFIG_PREEMPT and by CONFIG_PREEMPT_RT.
Both PREEMPT and PREEMPT_RT require the same functionality which today
depends on CONFIG_PREEMPT.
Switch the preempt anand xen-ops code over to use CONFIG_PREEMPTION.

With sufficient many pages to map gntdev can reach order 9 allocation
sizes. As there is no need to have physically contiguous buffers switch
to kvcalloc() in order to avoid failing allocations.

Today there is a global limit of pages mapped via /dev/xen/gntdev set
to 1 million pages per default. There is no reason why that limit is
existing, as total number of grant mappings is limited by the
hypervisor anyway and preferring kernel mappings over userspace ones
doesn't make sense. It should be noted that the gntdev device is
usable by root only.
Additionally checking of that limit is fragile, as the number of pages
to map via one call is specified in a 32-bit unsigned variable which
isn't tested to stay within reasonable limits (the only test is the
value to be <= zero, which basically excludes only calls without any
mapping requested). So trying to map e.g. 0xffff0000 pages while
already nearly 1000000 pages are mapped will effectively lower the
global number of mapped pages such that a parallel call mapping a
reasonable amount of pages can succeed in spite of the global limit
being violated.
So drop the global limit and introduce per call limit instead. This
per call limit (default: 65536 grant mappings) protects against
allocating insane large arrays in the kernel for doing a hypercall
which will fail anyway in case a user is e.g. trying to map billions
of pages.

The non-zero check on ret is always going to be false because
ret was initialized as zero and the only place it is set to
non-zero contains a return path before the non-zero check. Hence
the check is redundant and can be removed.

__xen_evtchn_do_upcall() contains guards against being called
recursively. This mechanism was introduced in the early pvops times
(kernel 2.6.26) when there were all the Xen backend drivers missing
from the upstream kernel, and some of those out-of-tree drivers were
enabling interrupts in their event handlers (which was explicitly
allowed in the initial XenoLinux).
Nowadays we don't need to support those old drivers any more and the
capability to allow recursive calls of __xen_evtchn_do_upcall() can
be removed.

This is another round of bug fixing and cleanup. This time the focus is on
the driver pattern to use mmu notifiers to monitor a VA range. This code
is lifted out of many drivers and hmm_mirror directly into the
mmu_notifier core and written using the best ideas from all the driver
implementations.
This removes many bugs from the drivers and has a very pleasing
diffstat. More drivers can still be converted, but that is for another
cycle.
- A shared branch with RDMA reworking the RDMA ODP implementation
- New mmu_interval_notifier API. This is focused on the use case of
monitoring a VA and simplifies the process for drivers
- A common seq-count locking scheme built into the mmu_interval_notifier
API usable by drivers that call get_user_pages() or hmm_range_fault()
with the VA range
- Conversion of mlx5 ODP, hfi1, radeon, nouveau, AMD GPU, and Xen GntDev
drivers to the new API. This deletes a lot of wonky driver code.

There's no in-kernel users for the k(un)map stuff. And the mmap one is
actively harmful - return 0 and then _not_ actually mmaping can't end

gntdev simply wants to monitor a specific VMA for any notifier events,
this can be done straightforwardly using mmu_interval_notifier_insert()
over the VMA's VA range.
The notifier should be attached until the original VMA is destroyed.
It is unclear if any of this is even sane, but at least a lot of duplicate
code is removed.

Convert xen-platform from the legacy PCI power management callbacks to the
generic operations. This is one step towards removing support for the
legacy PCI callbacks.
The generic .resume_noirq() operation is called by pci_pm_resume_noirq() at
the same point the legacy PCI .resume_early() callback was, so this patch
should not change the xen-platform behavior.

The valid memory address check in dma_capable only makes sense when mapping
normal memory, not when using dma_map_resource to map a device resource.
Add a new boolean argument to dma_capable to exclude that check for the
dma_map_resource case.
