introduce pv_cpu_ops to paravirtualize privleged instructions
which are defined by ia64 intrinsics.
make them indirect C function calls by introducing function
tables, pv_cpu_ops.

When I submitted 0df29025fd to ad
an #ifdef __KERNEL__ to include/asm-ia64/gcc_intrin.h a few weeks
ago I neglected to move gcc_intrin.h from header-y to unifdef-y.

Make the contents of the userspace asm/setup.h header consistent on all
architectures:
- export setup.h to userspace on all architectures
- export only COMMAND_LINE_SIZE to userspace
- frv: move COMMAND_LINE_SIZE from param.h
- i386: remove duplicate COMMAND_LINE_SIZE from param.h
- arm:
- export ATAGs to userspace
- change u8/u16/u32 to __u8/__u16/__u32
