The __pXd_offset() macros are identical to the pXd_index() macros and
there is no point to keep both of them. All architectures define and use
pXd_index() so let's keep only those to make mips consistent with the rest
of the kernel.

As the bug report [1] pointed out, <linux/vermagic.h> must be included
after <linux/module.h>.
I believe we should not impose any include order restriction. We often
sort include directives alphabetically, but it is just coding style
convention. Technically, we can include header files in any order by
making every header self-contained.
Currently, arch-specific MODULE_ARCH_VERMAGIC is defined in
<asm/module.h>, which is not included from <linux/vermagic.h>.
Hence, the straight-forward fix-up would be as follows:

This works enough, but for further cleanups, I split MODULE_ARCH_VERMAGIC
definitions into <asm/vermagic.h>.
With this, <linux/module.h> and <linux/vermagic.h> will be orthogonal,
and the location of MODULE_ARCH_VERMAGIC definitions will be consistent.
For arc and ia64, MODULE_PROC_FAMILY is only used for defining
MODULE_ARCH_VERMAGIC. I squashed it.
For hexagon, nds32, and xtensa, I removed <asm/modules.h> entirely
because they contained nothing but MODULE_ARCH_VERMAGIC definition.
Kbuild will automatically generate <asm/modules.h> at build-time,
wrapping <asm-generic/module.h>.

There are many platforms with exact same value for VM_DATA_DEFAULT_FLAGS
This creates a default value for VM_DATA_DEFAULT_FLAGS in line with the
existing VM_STACK_DEFAULT_FLAGS. While here, also define some more
macros with standard VMA access flag combinations that are used
frequently across many platforms. Apart from simplification, this
reduces code duplication as well.

Move access_ok() in and pagefault_enable()/pagefault_disable() out.
Mechanical conversion only - some instances don't really need
a separate access_ok() at all (e.g. the ones only using
get_user()/put_user(), or architectures where access_ok()

In the x86 MM code we'd like to untangle various types of historic
header dependency spaghetti, but for this we'd need to pass to
the generic vmalloc code various vmalloc related defines that
customarily come via the <asm/page.h> low level arch header.

SuperH is the only user of the current implementation of early platform
device support. We want to introduce a more robust approach to early
probing. As the first step - move all the current early platform code
to arch/sh.
In order not to export internal drivers/base functions to arch code for
this temporary solution - copy the two needed routines for driver
matching from drivers/base/platform.c to arch/sh/drivers/platform_early.c.
Also: call early_platform_cleanup() from subsys_initcall() so that it's
called after all early devices are probed.

The naming of pgtable_page_{ctor,dtor}() seems to have confused a few
people, and until recently arm64 used these erroneously/pointlessly for
other levels of page table.
To make it incredibly clear that these only apply to the PTE level, and to
align with the naming of pgtable_pmd_page_{ctor,dtor}(), let's rename them
to pgtable_pte_page_{ctor,dtor}().
These changes were generated with the following shell script:

Both pgtable_cache_init() and pgd_cache_init() are used to initialize kmem
cache for page table allocations on several architectures that do not use
PAGE_SIZE tables for one or more levels of the page table hierarchy.
Most architectures do not implement these functions and use __weak default
NOP implementation of pgd_cache_init(). Since there is no such default
for pgtable_cache_init(), its empty stub is duplicated among most
architectures.
Rename the definitions of pgd_cache_init() to pgtable_cache_init() and
drop empty stubs of pgtable_cache_init().

long time, but have not got much traction in the last decade and are only
used on ia64 and sh architectures.
The numbers in the initial commit look interesting but probably don't

history, but it's unhelpful to have this code and divergent allocator
behaviour for minor archs.
Also it might be better to instead make more general improvements to page
allocator if this is still so slow.

The mmiowb() macro is horribly difficult to use and drivers will continue
to work most of the time if they omit a call when it is required.
Rather than rely on driver authors getting this right, push mmiowb() into
arch_spin_unlock() for sh. If this is deemed to be a performance issue,
a subsequent optimisation could make use of ARCH_HAS_MMIOWB to elide
the barrier in cases where no I/O writes were performed inside the
critical section.

As the generic rwsem-xadd code is using the appropriate acquire and
release versions of the atomic operations, the arch specific rwsem.h
files will not be that much faster than the generic code as long as the
atomic functions are properly implemented. So we can remove those arch
specific rwsem.h and stop building asm/rwsem.h to reduce maintenance
effort.
Currently, only x86, alpha and ia64 have implemented architecture
specific fast paths. I don't have access to alpha and ia64 systems for
testing, but they are legacy systems that are not likely to be updated
to the latest kernel anyway.
By using a rwsem microbenchmark, the total locking rates on a 4-socket
56-core 112-thread x86-64 system before and after the patch were as
follows (mixed means equal # of read and write locks):

There were some run-to-run variations for the multi-thread tests. For
x86-64, using the generic C code fast path seems to be a little bit
faster than the assembly version with low lock contention. Looking at
the assembly version of the fast paths, there are assembly to/from C
code wrappers that save and restore all the callee-clobbered registers
(7 registers on x86-64). The assembly generated from the generic C
code doesn't need to do that. That may explain the slight performance
gain here.
The generic asm rwsem.h can also be merged into kernel/locking/rwsem.h
with no code change as no other code other than those under
kernel/locking needs to access the internal rwsem macros and functions.

I do not see any consistency about headers_install of <linux/kvm_para.h>
and <asm/kvm_para.h>.
According to my analysis of Linux 5.1-rc1, there are 3 groups:
[1] Both <linux/kvm_para.h> and <asm/kvm_para.h> are exported
alpha, arm, hexagon, mips, powerpc, s390, sparc, x86
[2] <asm/kvm_para.h> is exported, but <linux/kvm_para.h> is not
arc, arm64, c6x, h8300, ia64, m68k, microblaze, nios2, openrisc,
parisc, sh, unicore32, xtensa
[3] Neither <linux/kvm_para.h> nor <asm/kvm_para.h> is exported
csky, nds32, riscv
This does not match to the actual KVM support. At least, [2] is
half-baked.
Nor do arch maintainers look like they care about this. For example,
commit 0add53713b ("microblaze: Add missing kvm_para.h to Kbuild")
exported <asm/kvm_para.h> to user-space in order to fix an in-kernel
build error.
We have two ways to make this consistent:
[A] export both <linux/kvm_para.h> and <asm/kvm_para.h> for all
architectures, irrespective of the KVM support
[B] Match the header export of <linux/kvm_para.h> and <asm/kvm_para.h>
to the KVM support
My first attempt was [A] because the code looks cleaner, but Paolo
suggested [B].
So, this commit goes with [B].
For most architectures, <asm/kvm_para.h> was moved to the kernel-space.
I changed include/uapi/linux/Kbuild so that it checks generated
asm/kvm_para.h as well as check-in ones.
After this commit, there will be two groups:
[1] Both <linux/kvm_para.h> and <asm/kvm_para.h> are exported
arm, arm64, mips, powerpc, s390, x86
[2] Neither <linux/kvm_para.h> nor <asm/kvm_para.h> is exported

Every in-kernel use of this function defined it to KERNEL_DS (either as
an actual define, or as an inline function). It's an entirely
historical artifact, and long long long ago used to actually read the
segment selector valueof '%ds' on x86.
Which in the kernel is always KERNEL_DS.
Inspired by a patch from Jann Horn that just did this for a very small
subset of users (the ones in fs/), along with Al who suggested a script.
I then just took it to the logical extreme and removed all the remaining
gunk.
Roughly scripted with

The time, stime, utime, utimes, and futimesat system calls are only
used on older architectures, and we do not provide y2038 safe variants
of them, as they are replaced by clock_gettime64, clock_settime64,
and utimensat_time64.
However, for consistency it seems better to have the 32-bit architectures
that still use them call the "time32" entry points (leaving the
traditional handlers for the 64-bit architectures), like we do for system
calls that now require two versions.
Note: We used to always define __ARCH_WANT_SYS_TIME and
__ARCH_WANT_SYS_UTIME and only set __ARCH_WANT_COMPAT_SYS_TIME and
__ARCH_WANT_SYS_UTIME32 for compat mode on 64-bit kernels. Now this is
reversed: only 64-bit architectures set __ARCH_WANT_SYS_TIME/UTIME, while
we need __ARCH_WANT_SYS_TIME32/UTIME32 for 32-bit architectures and compat
mode. The resulting asm/unistd.h changes look a bit counterintuitive.
This is only a cleanup patch and it should not change any behavior.

Commit 594cc251fd ("make 'user_access_begin()' do 'access_ok()'")
broke both alpha and SH booting in qemu, as noticed by Guenter Roeck.
It turns out that the bug wasn't actually in that commit itself (which
would have been surprising: it was mostly a no-op), but in how the
addition of access_ok() to the strncpy_from_user() and strnlen_user()
functions now triggered the case where those functions would test the
access of the very last byte of the user address space.
The string functions actually did that user range test before too, but
they did it manually by just comparing against user_addr_max(). But
with user_access_begin() doing the check (using "access_ok()"), it now
exposed problems in the architecture implementations of that function.
For example, on alpha, the access_ok() helper macro looked like this:

and what it basically tests is of any of the high bits get set (the
USER_DS masking value is 0xfffffc0000000000).
And that's completely wrong for the "addr+size" check. Because it's
off-by-one for the case where we check to the very end of the user
address space, which is exactly what the strn*_user() functions do.
Why? Because "addr+size" will be exactly the size of the address space,
so trying to access the last byte of the user address space will fail
the __access_ok() check, even though it shouldn't. As a result, the
user string accessor functions failed consistently - because they
literally don't know how long the string is going to be, and the max
access is going to be that last byte of the user address space.
Side note: that alpha macro is buggy for another reason too - it re-uses
the arguments twice.
And SH has another version of almost the exact same bug:

so far so good: yes, a user address must be below the limit. But then:

is wrong with the exact same off-by-one case: the case when "addr+size"
is exactly _equal_ to the limit is actually perfectly fine (think "one
byte access at the last address of the user address space")
The SH version is actually seriously buggy in another way: it doesn't
actually check for overflow, even though it did copy the _comment_ that
talks about overflow.
So it turns out that both SH and alpha actually have completely buggy
implementations of access_ok(), but they happened to work in practice
(although the SH overflow one is a serious serious security bug, not
that anybody likely cares about SH security).
This fixes the problems by using a similar macro on both alpha and SH.
It isn't trying to be clever, the end address is based on this logic:
unsigned long __ao_end = __ao_a + __ao_b - !!__ao_b;
which basically says "add start and length, and then subtract one unless
the length was zero". We can't subtract one for a zero length, or we'd
just hit an underflow instead.
For a lot of access_ok() users the length is a constant, so this isn't
actually as expensive as it initially looks.

Patch series "Add support for fast mremap".
This series speeds up the mremap(2) syscall by copying page tables at
the PMD level even for non-THP systems. There is concern that the extra
'address' argument that mremap passes to pte_alloc may do something
subtle architecture related in the future that may make the scheme not
work. Also we find that there is no point in passing the 'address' to
pte_alloc since its unused. This patch therefore removes this argument
tree-wide resulting in a nice negative diff as well. Also ensuring
along the way that the enabled architectures do not do anything funky
with the 'address' argument that goes unnoticed by the optimization.
Build and boot tested on x86-64. Build tested on arm64. The config
enablement patch for arm64 will be posted in the future after more
testing.
The changes were obtained by applying the following Coccinelle script.

Nobody has actually used the type (VERIFY_READ vs VERIFY_WRITE) argument
of the user address range verification function since we got rid of the
old racy i386-only code to walk page tables by hand.
It existed because the original 80386 would not honor the write protect
bit when in kernel mode, so you had to do COW by hand before doing any
user access. But we haven't supported that in a long time, and these
days the 'type' argument is a purely historical artifact.
A discussion about extending 'user_access_begin()' to do the range
checking resulted this patch, because there is no way we're going to
move the old VERIFY_xyz interface to that model. And it's best done at
the end of the merge window when I've done most of my merges, so let's
just get this done once and for all.
This patch was mostly done with a sed-script, with manual fix-ups for
the cases that weren't of the trivial 'access_ok(VERIFY_xyz' form.
There were a couple of notable cases:
- csky still had the old "verify_area()" name as an alias.
- the iter_iov code had magical hardcoded knowledge of the actual
values of VERIFY_{READ,WRITE} (not that they mattered, since nothing
really used it)
- microblaze used the type argument for a debug printout
but other than those oddities this should be a total no-op patch.
I tried to fix up all architectures, did fairly extensive grepping for
access_ok() uses, and the changes are trivial, but I may have missed
something. Any missed conversion should be trivially fixable, though.

This concludes the main part of the system call rework for 64-bit time_t,
which has spread over most of year 2018, the last six system calls being
- ppoll
- pselect6
- io_pgetevents
- recvmmsg
- futex
- rt_sigtimedwait
As before, nothing changes for 64-bit architectures, while 32-bit
architectures gain another entry point that differs only in the layout
of the timespec structure. Hopefully in the next release we can wire up
all 22 of those system calls on all 32-bit architectures, which gives
us a baseline version for glibc to start using them.
This does not include the clock_adjtime, getrusage/waitid, and
getitimer/setitimer system calls. I still plan to have new versions
of those as well, but they are not required for correct operation of
the C library since they can be emulated using the old 32-bit time_t
based system calls.
Aside from the system calls, there are also a few cleanups here,
removing old kernel internal interfaces that have become unused after
all references got removed. The arch/sh cleanups are part of this,
there were posted several times over the past year without a reaction
from the maintainers, while the corresponding changes made it into all
other architectures.

I worked with Firoz Khan to change all architectures to have their system
call tables (syscall.S and asm/unistd.h) generated by a script from a more
readable input file the same way that we already had on x86, s390 and arm.
I offered to take those conversions through the asm-generic tree that
did not get picked up by the architecture maintainers, and fortunately
all but one have now been accepted into arch maintainer trees, so this
branch only contains the conversion for arch/sh/, with permission from
Rich.
The conversion does not include the old 64-bit sh5 architecture, which
has never shipped and not even compiled in a long time. The table
in include/uapi/asm/unistd.h is also not included here, as Firoz is
still working on that one, it will have to wait for the next following
merge window, hopefully together with the addition of the 64-bit
time_t system calls for the y2038 work that led to the system call
table rework.

NR_syscalls macro holds the number of system call exist
in sh architecture. We have to change the value of NR-
_syscalls, if we add or delete a system call.
One of the patch in this patch series has a script which
will generate a uapi header based on syscall.tbl file.
The syscall.tbl file contains the total number of system
calls information. So we have two option to update NR_sy-
scalls value.
1. Update NR_syscalls in asm/unistd.h manually by count-
ing the no.of system calls. No need to update NR_sys-
calls until we either add a new system call or delete
existing system call.
2. We can keep this feature it above mentioned script,
that will count the number of syscalls and keep it in
a generated file. In this case we don't need to expli-
citly update NR_syscalls in asm/unistd.h file.
The 2nd option will be the recommended one. For that, I
added the __NR_syscalls macro in uapi/asm/unistd_32/64.h
along with NR_syscalls which is moved to asm/unistd.h.
The macro __NR_syscalls also added for making the name
convention same across all architecture. While __NR_sys-
calls isn't strictly part of the uapi, having it as part
of the generated header to simplifies the implementation.
We also need to enclose this macro with #ifdef __KERNEL__
to avoid side effects.

The only remaining user of board_time_init() is the of-generic
machine, and that just calls the global timer_init() function.
Calling that one has no effect on non-DT platforms, so we can
simply call it unconditionally in place of board_time_init().

All platforms are now converted to RTC drivers, so this has become
obsolete. The board_time_init() callback still has one caller, but
could otherwise also get killed.
This removes one more usage of the deprecated timespec structure,
which overflows in y2038.

Most architectures provide prototypes for the PCI I/O mapping operations
when asm/io.h is included but SH doesn't currently do that, leading to
for example warnings in sound/pci/hda/patch_ca0132.c when pci_iomap() is
used on current -next. Make SH more consistent with other architectures
by including asm-generic/pci_iomap.h in asm/io.h.

Prefer _THIS_IP_ defined in linux/kernel.h.
Most definitions of current_text_addr were the same as _THIS_IP_, but
a few archs had inline assembly instead.
This patch removes the final call site of current_text_addr, making all
of the definitions dead code.

The sys_llseek sytem call is needed on all 32-bit architectures and
none of the 64-bit ones, so we can remove the __ARCH_WANT_SYS_LLSEEK guard
and simplify the include/asm-generic/unistd.h header further.
Since 32-bit tasks can run either natively or in compat mode on 64-bit
architectures, we have to check for both !CONFIG_64BIT and CONFIG_COMPAT.
There are a few 64-bit architectures that also reference sys_llseek
in their 64-bit ABI (e.g. sparc), but I verified that those all
select CONFIG_COMPAT, so the #if check is still correct here. It's
a bit odd to include it in the syscall table though, as it's the
same as sys_lseek() on 64-bit, but with strange calling conventions.

We have four generations of stat() syscalls:
- the oldstat syscalls that are only used on the older architectures
- the newstat family that is used on all 64-bit architectures but
lacked support for large files on 32-bit architectures.
- the stat64 family that is used mostly on 32-bit architectures to
replace newstat
- statx() to replace all of the above, adding 64-bit timestamps among
other things.
We already compile stat64 only on those architectures that need it,
but newstat is always built, including on those that don't reference
it. This adds a new __ARCH_WANT_NEW_STAT symbol along the lines of
__ARCH_WANT_OLD_STAT and __ARCH_WANT_STAT64 to control compilation of
newstat. All architectures that need it use an explict define, the
others now get a little bit smaller, and future architecture (including
64-bit targets) won't ever see it.

We can't pass the breakpoint directly on arch_check_bp_in_kernelspace()
anymore because its architecture internal datas (struct arch_hw_breakpoint)
are not yet filled by the time we call the function, and most
implementation need this backend to be up to date. So arrange the
function to take the probing struct instead.

Many of the inc/dec ops are mandatory, but for most architectures inc/dec are
simply trivial wrappers around their corresponding add/sub ops.
Let's make all the inc/dec ops optional, so that we can get rid of these
boilerplate wrappers.
The instrumented atomics are updated accordingly.
There should be no functional change as a result of this patch.

Some of the atomics return the result of a test applied after the atomic
operation, and almost all architectures implement these as trivial
wrappers around the underlying atomic. Specifically:
* <atomic>_inc_and_test(v) is (<atomic>_inc_return(v) == 0)
* <atomic>_dec_and_test(v) is (<atomic>_dec_return(v) == 0)
* <atomic>_sub_and_test(i, v) is (<atomic>_sub_return(i, v) == 0)
* <atomic>_add_negative(i, v) is (<atomic>_add_return(i, v) < 0)
Rather than have these definitions duplicated in all architectures, with
minor inconsistencies in formatting and documentation, let's make these
operations optional, with default fallbacks as above. Implementations
must now provide a preprocessor symbol.
The instrumented atomics are updated accordingly.
Both x86 and m68k have custom implementations, which are left as-is,
given preprocessor symbols to avoid being overridden.
There should be no functional change as a result of this patch.

Several architectures these have a near-identical implementation based
on atomic_read() and atomic_cmpxchg() which we can instead define in
<linux/atomic.h>, so let's do so, using something close to the existing
x86 implementation with try_cmpxchg().
Where an architecture provides its own atomic_fetch_add_unless(), it
must define a preprocessor symbol for it. The instrumented atomics are
updated accordingly.
Note that arch/arc's existing atomic_fetch_add_unless() had redundant
barriers, as these are already present in its atomic_cmpxchg()
implementation.
There should be no functional change as a result of this patch.

While __atomic_add_unless() was originally intended as a building-block
for atomic_add_unless(), it's now used in a number of places around the
kernel. It's the only common atomic operation named __atomic*(), rather
than atomic_*(), and for consistency it would be better named
atomic_fetch_add_unless().
This lack of consistency is slightly confusing, and gets in the way of
scripting atomics. Given that, let's clean things up and promote it to
an official part of the atomics API, in the form of
atomic_fetch_add_unless().
This patch converts definitions and invocations over to the new name,
including the instrumented version, using the following script:

The sh implementation of <asm/cmpxchg-xchg.h> pulls in <linux/bitops.h>
so that it can refer to BITS_PER_BYTE. It also transitively relies on
this pulling in <linux/compiler.h> for READ_ONCE().
Replace the #include with <linux/bits.h> and <linux/compiler.h>.
