Add a memory barrier to ensure the valid bit is read before
any of the cqe payload is read. This fixes an issue seen
on Power where the cqe payload was getting loaded before
the valid bit. When this occurred, we saw an iotag out of
range error when a command completed, but since the iotag
looked invalid the command didn't get completed to scsi core.
Later we hit the command timeout, attempted to abort the command,
then waited for the aborted command to get returned. Since the
adapter already returned the command, we timeout waiting,
and end up escalating EEH all the way to host reset. This
patch fixes this issue.

On lpfc_debugfs_initialize function the dumpHostSlim member setup happens
when 'phba->sli_rev < LPFC_SLI_REV4' is true, however when it is false NULL
has been assigned to debug_dumpHBASlim instead of debug_dumpHostSlim.

The PCIE capability offset is saved during PCI bus walking. It will
remove an unnecessary search in the PCI configuration space if this
value is referenced instead of reacquiring it. Also, pci_is_pcie is a
better way of determining if the device is PCIE or not (as it uses the
same saved PCIE capability offset).

Since commit 0998d06310
(device-core: Ensure drvdata = NULL when no driver is bound),
the driver core clears the driver data to NULL after device_release
or on probe failure. Thus, it is not needed to manually clear the

On architectures where wmb() is defined as mb(), a build error
happens since there is also a variable named 'mb' in lpfc_sli.c's
lpfc_sli_issue_mbox_s3() function. Rename the variable to 'mbx'
to prevent the build error.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

Create module parameter to disable Target Reset on FCP-Tape devices
when a "bus reset" is requested. Default is to reset all devices on
bus reset.

Fixed boot from san failure when SLI4 FC device presented on the same PCI bus
The request_firmware interface can induce delays while looking
for firmware files, even if no fw file is present. In some situations
the delays exceeded scan_wait timeouts, resulting in situations in which
the boot device had not been discovered in time. Boot Device does not
need to be on a lpfc device.
Change request_firmware use to be module paramater driven. Default is to
not attempt firmware download on boot. Add sysfs parameter to invoke
firmware update.
