Commit 4857c91f0d changed the way how irq affinity is setup in
setup_ioapic_dest() from using the core helper function to
unconditionally calling the irq_set_affinity() callback of the
underlying irq chip.
That results in a NULL pointer dereference for the rare case where the
underlying irq chip is lapic_chip which has no irq_set_affinity()
callback. lapic_chip is occasionally used for the timer interrupt (irq
0).
The fix is simple: Check the availability of the callback instead of
calling it unconditionally.

The issue is easily reproducible with a simple instrumentation: if
mdelay(10) is put between mp_setup_entry() and mp_register_handler() calls
in mp_irqdomain_alloc() Hyper-V guest always fails to boot when re-routing
IRQ0. The issue seems to be caused by the fact that we don't disable
interrupts while doing IOPIC programming for legacy IRQs and IRQ0 actually
happens.
Protect the setup sequence against concurrent interrupts.

When sending IPIs, first check if the non-local part of the source and
destination APIC IDs match; if so, send via the local APIC for efficiency.
Secondly, since the AMD BIOS-kernel developer guide states IPI delivery
will occur invarient of prior deliver status, avoid polling the delivery
status bit for efficiency.

The APIC LVTT register is MMIO mapped but the TSC_DEADLINE register is an
MSR. The write to the TSC_DEADLINE MSR is not serializing, so it's not
guaranteed that the write to LVTT has reached the APIC before the
TSC_DEADLINE MSR is written. In such a case the write to the MSR is
ignored and as a consequence the local timer interrupt never fires.
The SDM decribes this issue for xAPIC and x2APIC modes. The
serialization methods recommended by the SDM differ.
xAPIC:
"1. Memory-mapped write to LVT Timer Register, setting bits 18:17 to 10b.
2. WRMSR to the IA32_TSC_DEADLINE MSR a value much larger than current time-stamp counter.
3. If RDMSR of the IA32_TSC_DEADLINE MSR returns zero, go to step 2.
4. WRMSR to the IA32_TSC_DEADLINE MSR the desired deadline."
x2APIC:
"To allow for efficient access to the APIC registers in x2APIC mode,
the serializing semantics of WRMSR are relaxed when writing to the
APIC registers. Thus, system software should not use 'WRMSR to APIC
registers in x2APIC mode' as a serializing instruction. Read and write
accesses to the APIC registers will occur in program order. A WRMSR to
an APIC register may complete before all preceding stores are globally
visible; software can prevent this by inserting a serializing
instruction, an SFENCE, or an MFENCE before the WRMSR."
The xAPIC method is to just wait for the memory mapped write to hit
the LVTT by checking whether the MSR write has reached the hardware.
There is no reason why a proper MFENCE after the memory mapped write would
not do the same. Andi Kleen confirmed that MFENCE is sufficient for the
xAPIC case as well.
Issue MFENCE before writing to the TSC_DEADLINE MSR. This can be done
unconditionally as all CPUs which have TSC_DEADLINE also have MFENCE
support.

The recent ioapic cleanups changed the affinity setting in
setup_ioapic_dest() from a direct write to the hardware to the delayed
affinity setup via irq_set_affinity().
That results in a warning from chained_irq_exit():

The reason is that irq_set_affinity() does not write directly to the
hardware. It marks the affinity setting as pending and executes it
from the next interrupt. The chained handler infrastructure does not
take the irq descriptor lock for performance reasons because such a
chained interrupt is not visible to any interfaces. So the delayed
affinity setting triggers the warning in irq_move_masked_irq().
Restore the old behaviour by calling the set_affinity function of the
ioapic chip in setup_ioapic_dest(). This is safe as none of the
interrupts can be on the fly at this point.

In the recent x2apic cleanup I got two things really wrong:
1) The safety check in __disable_x2apic which allows the function to
be called unconditionally is backwards. The check is there to
prevent access to the apic MSR in case that the machine has no
apic. Though right now it returns if the machine has an apic and
therefor the disabling of x2apic is never invoked.
2) x2apic_disable() sets x2apic_mode to 0 after registering the local
apic. That's wrong, because register_lapic_address() checks x2apic
mode and therefor takes the wrong code path.
This results in boot failures on machines with x2apic preenabled by
BIOS and can also lead to an fatal MSR access on machines without
apic.
The solutions are simple:
1) Correct the sanity check for apic availability
2) Clear x2apic_mode _before_ calling register_lapic_address()

Alex Deucher, Mark Rustad and Alexander Holler reported a regression
with the latest v4.2-rc4 kernel, which breaks some SATA controllers.
With multi-MSI capable SATA controllers, only the first port works,
all other ports time out when executing SATA commands.
This happens because the first argument to assign_irq_vector_policy()
is always the base linux irq number of the multi MSI interrupt block,
so all subsequent vector assignments operate on the base linux irq
number, so all MSI irqs are handled as the first irq number. Therefor
the other MSI irqs of a device are never set up correctly and never
fire.
Add the loop iterator to the base irq number so all vectors are
assigned correctly.

Both the per-APIC flag ".wait_for_init_deassert",
and the global atomic_t "init_deasserted"
are dead code -- remove them.
For all APIC types, "wait_for_master()"

smp_cleanup_move fiddles without protection in the interrupt
descriptors and the vector array. A concurrent irq setup/teardown or

irqdomain interfaces") introduced a regression which causes
malfunction of interrupt lines.
The reason is that the conversion of mp_check_pin_attr() missed to
update the polarity selection of the interrupt pin with the caller
provided setting and instead uses a stale attribute value. That in
turn results in chosing the wrong interrupt flow handler.
Use the caller supplied setting to configure the pin correctly which
also choses the correct interrupt flow handler.
This restores the original behaviour and on the affected
machine/driver (Surface Pro 3, i2c controller) all IOAPIC IRQ
configuration are identical to v4.1.

Migrate apic driver to the new 'set-state' interface provided by
clockevents core, the earlier 'set-mode' interface is marked obsolete
now.
This also enables us to implement callbacks for new states of clockevent
devices, for example: ONESHOT_STOPPED.
We weren't doing anything while switching to resume mode and so that
callback isn't implemented.

The __ref / __refdata annotations used to be needed because of
referencing functions / variables annotated __cpuinit /
__cpuinitdata.
But those annotations vanished during the development of v3.11.
Therefore most of the __ref / __refdata annotations are not needed
anymore. As they may hide legitimate sections mismatches, we
better get rid of them.

This leaves the irq vector(s) which are torn down on CPU M stale in
the vector array of CPU N, because CPU M does not see CPU N online
yet. There is a similar issue with concurrent newly setup interrupts.
The alloc/free protection of irq descriptors does not prevent the
above race, because it merily prevents interrupt descriptors from
going away or changing concurrently.
Prevent this by moving the call to setup_vector_irq() into the
vector_lock held region which protects set_cpu_online():

On NUMA systems, an IO device may be associated with a NUMA node.
It may improve IO performance to allocate resources, such as memory
and interrupts, from device local node.
This patch introduces a mechanism to support CPU vector allocation
policies. It tries to allocate CPU vectors from CPUs on device local
node first, and then fallback to all online(global) CPUs.
This mechanism may be used to support NumaConnect systems to allocate
CPU vectors from device local node.

Since 3cb96f0c97 ('x86/hpet: Enhance HPET IRQ to support
hierarchical irqdomains') hpet_msi_capability_lookup() uses
hpet_assign_irq(). The latter initializes irq_alloc_info on stack, but
passes a NULL pointer to irq_domain_alloc_irqs(), which causes a NULL
pointer dereference later in hpet_msi_get_hwirq().
Pass the pointer to the irq_alloc_info irq_domain_alloc_irqs().

We have 3 identical copies of the ioapic domain ops for acpi, mpparse,
and sfi. Have a global one in the io_apic code and be done with it.
To avoid include hell in io_apic.h, create a private irqdomain header
and include the generic irqdomain header from there.

While looking at the printout issue, I stumbled more than once over
the various 0/1 assignments which are either commented in strange ways
or force to lookup the meaning.
Use proper constants and fix the misleading comments. While at it
remove pointless 0 assignments in native_disable_io_apic() which have
no value for understanding the code.

io_apic_ops.init() is either NULL, if IO-APIC support is disabled at
compile time or native_io_apic_init_mappings(). No point to have that
as we can achieve the same thing with an empty inline.

x86_io_apic_ops.write is always set to native_io_apic_write(),
and nobody overrides it. So get rid of the indirection by changing
native_io_apic_write() as io_apic_write() and removing
x86_io_apic_ops.write.
Do the same for x86_io_apic_ops.modify and native_io_apic_modify().

To support legacy ISA IRQs, we need to preallocate irq_cfg structures
for legacy ISA IRQs. Refine the way to allocate irq_cfg for legacy ISA

Simplify the way to deal with remapped MSI interrupts, so we can remove
irq_chip.irq_print_chip later. We simply change the name when the
setup detects that the parent domain is remapping.

We have slightly changed the architecture interfaces to support htirq
PCI driver. It's safe because currently Hypertransport interrupt is
only enabled on x86 platforms.

Refine the interfaces to create IRQ for DMAR unit. It's a preparation
for converting DMAR IRQ to hierarchical irqdomain on x86.
It also moves dmar_alloc_hwirq()/dmar_free_hwirq() from irq_remapping.h
to dmar.h. They are not irq_remapping specific.

DMAR interrupt won't be remapped by interrupt remapping hardware,
so directly call native_compose_msi_msg() for DMAR IRQ to compose MSI
message data. This will help to simplify MSI code later.

Use new irqdomain interfaces to allocate/free IRQ for DMAR and interrupt
remapping, so we can remove GENERIC_IRQ_LEGACY_ALLOC_HWIRQ later.
The private definitions of irq_alloc_hwirqs()/irq_free_hwirqs() are a
temporary solution, they will be removed once we have converted the
interrupt remapping driver to use irqdomain framework.

remove GENERIC_IRQ_LEGACY_ALLOC_HWIRQ later.
This patch changes the interfaces between arch independent PCI driver
and arch specific code. Currently HT_IRQ is only enabled on x86, so it
does not affect other architectures.

Abstract CPU local APIC as an interrupt controller and create an
irqdomain for it to manage CPU interrupt vectors. It's the base to
enable hierarchical irqdomains on x86 systems.

Cache destination CPU APIC ID into struct irq_cfg when assigning vector
for interrupt. Upper layer just needs to read the cached APIC ID instead
of calling apic->cpu_mask_to_apicid_and(), it helps to hide APIC driver
details from IOAPIC/HPET/MSI drivers..

Fix a bug in the OEM check function that determines if the
system is a UV system and the BIOS is compatible with the
kernel's UV apic driver. This prevents some possibly obscure
panics and guards the system against being started on SGI
hardware that does not have the required kernel support.

__verify_local_APIC() is detritus from the early APIC days.
Its return value isn't used anywhere and the information it
prints when debug is enabled is already part of APIC
initialization messages printed to syslog. Off with it!

It causes the suspend to fail on at least the Chromebook Pixel, possibly
other platforms too.
Joerg Roedel points out that the logic should probably have been

instead, but since the code is not in any fast-path, so we can just live

We better provide proper functions which implement the required code
flow in the apic code rather than letting the smpboot code open code
it. That allows to make more functions static and confines the APIC
functionality to apic.c where it belongs.

enable_x2apic() is a convoluted unreadable mess because it is used for
both enablement in early boot and for setup in cpu_init().
Split the code into x2apic_enable() for enablement and x2apic_setup()
for setup of (secondary cpus). Make use of the new state tracking to
simplify the logic.

There is no point in postponing the hardware disablement of x2apic. It
can be disabled right away in the nox2apic setup function.
Disable it right away and set the state to DISABLED . This allows to
remove all the nox2apic conditionals all over the place.

Having 3 different variables to track the state is just silly and
error prone. Add a proper state tracking variable which covers the
three possible states: ON/OFF/DISABLED.
We cannot use x2apic_mode for this as this would require to change all
users of x2apic_mode with explicit comparisons for a state value
instead of treating it as boolean.

When interrupt remapping hardware is not in X2APIC, CPU X2APIC mode
will be disabled if:
1) Maximum CPU APIC ID is bigger than 255
2) hypervisior doesn't support x2apic mode.
But we should only check whether hypervisor supports X2APIC mode when
hypervisor(CONFIG_HYPERVISOR_GUEST) is enabled, otherwise X2APIC will
always be disabled when CONFIG_HYPERVISOR_GUEST is disabled and IR
doesn't work in X2APIC mode.

If remapping is in XAPIC mode, the setup code just skips X2APIC
initialization without checking max CPU APIC ID in system, which may
cause problem if system has a CPU with APIC ID bigger than 255.
Handle IR in XAPIC mode the same way as if remapping is disabled.

When kernel doesn't support X2APIC but BIOS has enabled X2APIC, system
may panic or hang without useful messages. On the other hand, it's
hard to dynamically disable X2APIC when CONFIG_X86_X2APIC is disabled.
So panic with a clear message in such a case.

If x2apic got disabled on the kernel command line, then the following
issue can happen:
enable_IR_x2apic()
....
x2apic_mode = 1;
enable_x2apic();
if (x2apic_disabled) {
__disable_x2apic();
return;
}
That leaves X2APIC disabled in hardware, but x2apic_mode stays 1. So
all other code which checks x2apic_mode gets the wrong information.
Set x2apic_mode to 0 after disabling it in hardware.
This is just a hotfix. The proper solution is to rework this code so
it has seperate functions for the initial setup on the boot processor
and the secondary cpus, but that's beyond the scope of this fix.

enable_IR_x2apic() calls setup_irq_remapping_ops() which by default
installs the intel dmar remapping ops and then calls the amd iommu irq
remapping prepare callback to figure out whether we are running on an
AMD machine with irq remapping hardware.
Right after that it calls irq_remapping_prepare() which pointlessly
checks:
if (!remap_ops || !remap_ops->prepare)
return -ENODEV;
and then calls
remap_ops->prepare()
which is silly in the AMD case as it got called from
setup_irq_remapping_ops() already a few microseconds ago.
Simplify this and just collapse everything into
irq_remapping_prepare().
The irq_remapping_prepare() remains still silly as it assigns blindly
the intel ops, but that's not scope of this patch.
The scope here is to move the preperatory work, i.e. memory
allocations out of the atomic section which is required to enable irq
remapping.

Pull x86 apic updates from Thomas Gleixner:
"After stopping the full x86/apic branch, I took some time to go
through the first block of patches again, which are mostly cleanups
and preparatory work for the irqdomain conversion and ioapic hotplug
support.
Unfortunaly one of the real problematic commits was right at the
beginning, so I rebased this portion of the pending patches without
the offenders.
It would be great to get this into 3.19. That makes reworking the
problematic parts simpler. The usual tip testing did not unearth any
issues and it is fully bisectible now.
I'm pretty confident that this wont affect the calmness of the xmas
season.
Changes:
- Split the convoluted io_apic.c code into domain specific parts
(vector, ioapic, msi, htirq)
- Introduce proper helper functions to retrieve irq specific data
instead of open coded dereferencing of pointers
- Preparatory work for ioapic hotplug and irqdomain conversion
- Removal of the non functional pci-ioapic driver
- Removal of unused irq entry stubs
- Make native_smp_prepare_cpus() preemtible to avoid GFP_ATOMIC
allocations for everything which is called from there.

When X86_LOCAL_APIC (i.e. unconditionally on x86-64),
first_system_vector will never end up being higher than
LOCAL_TIMER_VECTOR (0xef), and hence building stubs for vectors
0xef...0xff is pointlessly reducing code density. Deal with this at
build time already.
Taking into consideration that X86_64 implies X86_LOCAL_APIC, also
simplify (and hence make easier to read and more consistent with the
change done here) some #if-s in arch/x86/kernel/irqinit.c.
While we could further improve the packing of the IRQ entry stubs (the
four ones now left in the last set could be fit into the four padding
bytes each of the final four sets have) this doesn't seem to provide
any real benefit: Both irq_entries_start and common_interrupt getting
cache line aligned, eliminating the 30th set would just produce 32
bytes of padding between the 29th and common_interrupt.

While f3761db164 ("x86, irq: Fix build error caused by
9eabc99a635a77cbf09") addressed the original build problem,
declaration, inline stub, and definition still seem misplaced: It isn't
really IO-APIC related, and it's being used solely in arch/x86/pci/.
This also means stubbing it out when !CONFIG_X86_IO_APIC was at least
questionable.

After commit b2b49ccbdd (PM: Kconfig: Set PM_RUNTIME if PM_SLEEP is
selected) PM_RUNTIME is always set if PM is set, so #ifdef blocks
depending on CONFIG_PM_RUNTIME may now be changed to depend on
CONFIG_PM.
Replace CONFIG_PM_RUNTIME with CONFIG_PM in x86/kernel/apic/io_apic.c.

This code solves the issue of performing stack dumps from NMI context.
The issue is that printk() is not safe from NMI context as if the NMI
were to trigger when a printk() was being performed, the NMI could
deadlock from the printk() internal locks. This has been seen in practice.
With lots of review from Petr Mladek, this code went through several
iterations, and we feel that it is now at a point of quality to be
accepted into mainline.
Here's what is contained in this patch set:
o Creates a "seq_buf" generic buffer utility that allows a descriptor
to be passed around where functions can write their own "printk()"
formatted strings into it. The generic version was pulled out of
the trace_seq() code that was made specifically for tracing.
o The seq_buf code was change to model the seq_file code. I have
a patch (not included for 3.19) that converts the seq_file.c code
over to use seq_buf.c like the trace_seq.c code does. This was done
to make sure that seq_buf.c is compatible with seq_file.c. I may
try to get that patch in for 3.20.
o The seq_buf.c file was moved to lib/ to remove it from being dependent
on CONFIG_TRACING.
o The printk() was updated to allow for a per_cpu "override" of
the internal calls. That is, instead of writing to the console, a call
to printk() may do something else. This made it easier to allow the
NMI to change what printk() does in order to call dump_stack() without
needing to update that code as well.
o Finally, the dump_stack from all CPUs via NMI code was converted to
use the seq_buf code. The caller to trigger the NMI code would wait
till all the NMIs finished, and then it would print the seq_buf
data to the console safely from a non NMI context.

For complex interrupt routing scenarios where more than one
interrupt related chip is involved we had no proper representation
in the generic interrupt infrastructure so far. That made people
implement rather ugly constructs in their nested irq chip
implementations. The main offenders are x86 and arm/gic.
To distangle that mess we have now hierarchical irqdomains which
seperate the various interrupt chips and connect them via the
hierarchical domains. That keeps the domain specific details
internal to the particular hierarchy level and removes the
criss/cross referencing of chip internals. The resulting hierarchy

When trigger_all_cpu_backtrace() is called on x86, it will trigger an
NMI on each CPU and call show_regs(). But this can lead to a hard lock
up if the NMI comes in on another printk().
In order to avoid this, when the NMI triggers, it switches the printk
routine for that CPU to call a NMI safe printk function that records the
printk in a per_cpu seq_buf descriptor. After all NMIs have finished
recording its data, the seq_bufs are printed in a safe context.

The default self-IPI path polls the ICR to delay sending the IPI until
there is no IPI in progress. This is redundant on x86-86 APICs, since
IPIs are queued. See the AMD64 Architecture Programmer's Manual, vol 2,

Prevent 16-bit APIC IDs being truncated by using correct mask. This fixes

If the TSC is unusable or disabled, then this patch fixes:
- Confusion while trying to clear old APIC interrupts.
- Division by zero and incorrect programming of the TSC deadline
timer.
This fixes boot if the CPU has a TSC deadline timer but a missing or
broken TSC. The failure to boot can be observed with qemu using
-cpu qemu64,-tsc,+tsc-deadline
This also happens to me in nested KVM for unknown reasons.
With this patch, I can boot cleanly (although without a TSC).

Pull percpu consistent-ops changes from Tejun Heo:
"Way back, before the current percpu allocator was implemented, static
and dynamic percpu memory areas were allocated and handled separately
and had their own accessors. The distinction has been gone for many
years now; however, the now duplicate two sets of accessors remained
with the pointer based ones - this_cpu_*() - evolving various other
operations over time. During the process, we also accumulated other
inconsistent operations.
This pull request contains Christoph's patches to clean up the
duplicate accessor situation. __get_cpu_var() uses are replaced with
with this_cpu_ptr() and __this_cpu_ptr() with raw_cpu_ptr().
Unfortunately, the former sometimes is tricky thanks to C being a bit
messy with the distinction between lvalues and pointers, which led to
a rather ugly solution for cpumask_var_t involving the introduction of
this_cpu_cpumask_var_ptr().
This converts most of the uses but not all. Christoph will follow up
with the remaining conversions in this merge window and hopefully
remove the obsolete accessors"

A variable cannot be both __read_mostly and const. This
is a meaningless combination.
Just make it only const.
This fixes the LTO build with numachip enabled.

A commit in linux-next was causing boot to fail and bisection
identified the patch 4ba2968420 ("percpu: Resolve ambiguities in
__get_cpu_var/cpumask_var_"). One of the changes in that patch looks
very suspicious. Reverting the full patch fixes boot as does this
fixlet.

Set the IRQCHIP_SKIP_SET_WAKE for IOAPIC IRQ chip objects so that
interrupts from them can work as wakeup interrupts for suspend-to-idle.
After this change, running enable_irq_wake() on one of the IRQs in
question will succeed and IRQD_WAKEUP_STATE will be set for it, so
all of the suspend-to-idle wakeup mechanics introduced previously
will work for it automatically.

Now IOAPIC driver dynamically allocates IRQ numbers for IOAPIC pins.
We need to keep IRQ assignment for PCI devices during runtime power
management, otherwise it may cause failure of device wakeups.
Commit 3eec595235 "x86, irq, PCI: Keep IRQ assignment for PCI
devices during suspend/hibernation" has fixed the issue for suspend/
hibernation, we also need the same fix for runtime device sleep too.

__get_cpu_var can paper over differences in the definitions of
cpumask_var_t and either use the address of the cpumask variable
directly or perform a fetch of the address of the struct cpumask
allocated elsewhere. This is important particularly when using per cpu
cpumask_var_t declarations because in one case we have an offset into
a per cpu area to handle and in the other case we need to fetch a
pointer from the offset.
This patch introduces a new macro
this_cpu_cpumask_var_ptr()
that is defined where cpumask_var_t is defined and performs the proper
actions. All use cases where __get_cpu_var is used with cpumask_var_t
are converted to the use of this_cpu_cpumask_var_ptr().

Commit 15a3c7cc91 "x86, irq: Introduce two helper functions
to support irqdomain map operation" breaks LPSS ACPI enumerated
devices.
On startup, IOAPIC driver preallocates IRQ descriptors and programs
IOAPIC pins with default level and polarity attributes for all legacy
IRQs. Later legacy IRQ users may fail to set IOAPIC pin attributes
if the requested attributes conflicts with the default IOAPIC pin
attributes. So change mp_irqdomain_map() to allow the first legacy IRQ
user to reprogram IOAPIC pin with different attributes.

__get_cpu_var() is used for multiple purposes in the kernel source. One of
them is address calculation via the form &__get_cpu_var(x). This calculates
the address for the instance of the percpu variable of the current processor
based on an offset.
Other use cases are for storing and retrieving data from the current
processors percpu area. __get_cpu_var() can be used as an lvalue when
writing data or on the right side of an assignment.
__get_cpu_var() is defined as :
#define __get_cpu_var(var) (*this_cpu_ptr(&(var)))
__get_cpu_var() always only does an address determination. However, store
and retrieve operations could use a segment prefix (or global register on
other platforms) to avoid the address calculation.
this_cpu_write() and this_cpu_read() can directly take an offset into a
percpu area and use optimized assembly code to read and write per cpu
variables.
This patch converts __get_cpu_var into either an explicit address
calculation using this_cpu_ptr() or into a use of this_cpu operations that
use the offset. Thereby address calculations are avoided and less registers
are used when code is generated.

In the file x2apic_uv_x.c, some code is compiled conditionally
depending on CONFIG_SMP. However, the file is only built, if
CONFIG_X86_UV is enabled.
CONFIG_X86_UV depends on CONFIG_NUMA, which itself depends on
CONFIG_SMP, so the #ifdef will always evaluate to true, if the
file is compiled. Thus, it is unnecessary and can be removed.

"This is a major overhaul to the x86 apic subsystem consisting of the
following parts:
- Remove obsolete APIC driver abstractions (David Rientjes)
- Use the irqdomain facilities to dynamically allocate IRQs for
IOAPICs. This is a prerequisite to enable IOAPIC hotplug support,
and it also frees up wasted vectors (Jiang Liu)
- Misc fixlets.
Despite the hickup in Ingos previous pull request - caused by the
missing fixup for the suspend/resume issue reported by Borislav - I
strongly recommend that this update finds its way into 3.17. Some
history for you:
This is preparatory work for physical IOAPIC hotplug. The first
attempt to support this was done by Yinghai and I shot it down because
it just added another layer of obscurity and complexity to the already
existing mess without tackling the underlying shortcomings of the
current implementation.
After quite some on- and offlist discussions, I requested that the
design of this functionality must use generic infrastructure, i.e.
irq domains, which provide all the mechanisms to dynamically map linux
interrupt numbers to physical interrupts.
Jiang picked up the idea and did a great job of consolidating the
existing interfaces to manage the x86 (IOAPIC) interrupt system by
utilizing irq domains.
The testing in tip, Linux-next and inside of Intel on various machines
did not unearth any oddities until Borislav exposed it to one of his
oddball machines. The issue was resolved quickly, but unfortunately
the fix fell through the cracks and did not hit the tip tree before
Ingo sent the pull request. Not entirely Ingos fault, I also assumed
that the fix was already merged when Ingo asked me whether he could
send it.
Nevertheless this work has a proper design, has undergone several
rounds of review and the final fallout after applying it to tip and
integrating it into Linux-next has been more than moderate. It's the
ground work not only for IOAPIC hotplug, it will also allow us to move
the lowlevel vector allocation into the irqdomain hierarchy, which
will benefit other architectures as well. Patches are posted already,
but they are on hold for two weeks, see below.
I really appreciate the competence and responsiveness Jiang has shown
in course of this endavour. So I'm sure that any fallout of this will
be addressed in a timely manner.

The trampoline_phys_{high,low} members of struct apic are always
initialized to DEFAULT_TRAMPOLINE_PHYS_HIGH and TRAMPOLINE_PHYS_LOW,
respectively. Hardwire the constants and remove the unneeded members.

When a vSMP Foundation box is detected, the function apic_cluster_num() counts
the number of APIC clusters found. If more than one found, a multi board
configuration is assumed, and TSC marked as unstable. This behavior is
incorrect as vSMP Foundation may use processors from single node only, attached
to memory of other nodes - and such node may have more than one APIC cluster
(typically any recent intel box has more than single APIC_CLUSTERID(x)).
To fix this, we simply remove the code which detects a vSMP Foundation box and
affects apic_is_clusted_box() return value. This can be done because later the
kernel checks by itself if the TSC is stable using the
check_tsc_sync_[source|target]() functions and marks TSC as unstable if needed.

Sometimes it is preferred not to use the trigger_all_cpu_backtrace()
routine when one wants to avoid capturing a back trace for current. For
instance if one was previously captured recently.
This patch provides a new routine namely
trigger_allbutself_cpu_backtrace() which offers the flexibility to issue
an NMI to every cpu but current and capture a back trace accordingly.
Patch x86 and sparc to support new routine.

Introduce function mp_unmap_irq() to release IOAPIC IRQ when IRQ is not
used any more, which will typically called by pcibios_disabled_irq.
And function mp_irqdomain_unmap() is a common implementation of
irq_domain_ops.unmap for IOAPIC.

On startup, setup_IO_APIC_irqs() will program all IOAPIC pins for ISA
IRQs. Later when mp_map_pin_to_irq() is called, it just returns ISA IRQ
number without programming corresponding IOAPIC pin.
This patch consolidates the way to program IOAPIC pins for both ISA and
non-ISA IRQs into mp_map_pin_to_irq() as below:
1) For ISA IRQs, mp_irqs array is used to map IOAPIC pin to IRQ and
mp_irqdomain_map() is used to actually program the pin.
2) For non-ISA IRQs, irqdomain is used to map IOAPIC pin to IRQ, and
mp_irqdomain_map() is also used to actually program the pin.

Currently there are multiple entries to program IOAPIC pins, such as
io_apic_setup_irq_pin_once(), io_apic_set_pci_routing() and
setup_IO_APIC_irq_extra() etc.
This patch introduces two functions to help consolidate the code to
program IOAPIC pins. Function mp_set_pin_attr() is used to optionally
set trigger, polarity and NUMA node property for an IOAPIC pin.
If mp_set_pin_attr() is not invoked for a pin, the default configuration
from BIOS will be used.
Function mp_irqdomain_map() is an common implementation of irqdomain map()
operation. It figures out attribures for pin and then actually programs
the IOAPIC pin. We hope this will be the only entrance for programming
IOAPIC pin.
And the flow will:
1) caller such as xxx_pci_irq_enable figures out pin attributes.
2) Invoke mp_set_pin_attr() to set attributes for a pin. If the pin has
already bin programmed, mp_set_pin_attr() will aslo detects attribute
confictions.
3) Invoke mp_map_pin_to_irq()
3.1) If IRQ has already been assigned, return irq_find_mapping()
3.2) Else irq_create_mapping()
->irq_domain_associate()
->mp_irqdomain_map()
->io_apic_setup_irq_pin()
So every pin will only programmed once by mp_irqdomain_map(), so we
could kill io_apic_setup_irq_pin_once(), io_apic_set_pci_routing() and
setup_IO_APIC_irq_extra() etc.

Enhance function mp_register_ioapic() to support irqdomain.
When registering IOAPIC, caller may provide callbacks and parameters
for creating irqdomain. The IOAPIC core will create irqdomain later
if caller has passed in corresponding parameters.

Currently x86 support identity mapping between GSI(IOAPIC pin) and IRQ
number, so continous IRQs at low end are statically allocated to IOAPICs
at boot time. This design causes trouble to support IOAPIC hotplug.
This patch implements basic mechanism to dynamically allocate IRQ on
demand for IOAPIC pins by using irqdomain framework.
It first adds several fields into struct ioapic to support irqdomain.
Then it implements an algorithm to dynamically allocate IRQ number
for IOAPIC pins on demand.
Currently it supports three types of irqdomain:
1) LEGACY: used to support IOAPIC hosting legacy IRQs and building
identity mapping for legacy IRQs. A speical case, we dynamically
allocate IRQ number for IOAPIC pin which has GSI number below
nr_legacy_irqs() but isn't legacy IRQ. This is for backward
compatibility and avoid regression.
2) STRICT: build identity mapping between GSI and IRQ nubmer.
3) DYNAMIC: dynamically allocate IRQ number for IOAPIC pin on demand.
Legacy(ISA) IRQs is not managed by irqdomain because there may be
multiple pins sharing the same IRQ number and current irqdomain only
supports 1:1 mapping between pins and IRQ.

Currently ACPI and ioapic both implement algorithms to map (ioapic, pin)
to IRQ number. So consolidate the common part into one place, which is
also preparing for irqdomain support.
It introduces mp_map_gsi_to_irq(), which will be used to allocate IRQ
number IOAPIC pins when irqdomain is enabled.
Also rename gsi_to_irq() to map_gsi_to_irq(), later we will introduce
unmap_gsi_to_irq() when enabling IOAPIC hotplug.
