43bfc24ec1 i40iw: Add missing memory barriers
Remove duplicate set_64bit_val call to offset 24.
Replace some instances of set_64bit_val with
i40iw_insert_wqe_hdr as valid bit needs a write
barrier and should be the last write operation for the WQE.

f16dc0aa5e i40iw: Add support for port reuse on active side connections
During OpenMPI scale up testing, we observe rdma_connect
failures if ports are reused on multiple connections.
This is because the Control Queue-Pair (CQP) command to add
the reused port to Accelerated Port Bit VectorTable (APBVT)
fails as there already exists an entry.

Check for duplicate port before invoking the CQP command
to add APBVT entry and delete the entry only if the port
is not in use.

471b370d52 i40iw: Call i40iw_cm_disconn on modify QP to disconnect
If QP modify to closing/terminate/error fails, connection is
not torn down as there is no corresponding asynchronous
event that will initiate the teardown.

Add explicit call to i40iw_cm_disconn if not waiting in
modify QP, otherwise schedule it in CM timer.

47fb3c1610 i40iw: Prevent multiple netdev event notifier registrations
Netdev event notifier registration/de-registration is not
synchronized with a lock and there is a possibility of a
duplicate registration of notifier before the unregister
completes.

Register netdev event notifiers during module init and
de-register them at module exit.

This avoids the need to tie the registration to first netdev
client interface open and de-registration to last client
interface close and the synchronization to achieve it.

This also fixes a crash due to duplicate registration.

d26875b43d i40iw: Improve CQP timeout logic
The current timeout logic for Control Queue-Pair (CQP) OPs
does not take into account whether CQP makes progress but
rather blindly waits for a large timeout value, 100000 jiffies
for the completion event. Improve this by setting the timeout
based on whether the CQP is making progress or not. If the CQP
is hung, the timeout will happen sooner, in 5000 jiffies. Each
time the CQP progress is detetcted, the timeout extends by 5000
jiffies.

Remove NULL check for cm_node->listener in i40iw_accept
as listener is always present at this point.

Remove the check for cm_node->accept_pend and related code
in i40iw_cm_event_connected as the cm_node in this context
is only pertinent to active node and cm_node->accept_pend
is always 0.

This fixes the following smatch warnings,

9abb0d1bbd RDMA: Simplify get firmware interface
There is a need to forward FW version to user space
application through RDMA netlink. In order to make it safe, there
is need to declare nla_policy and limit the size of FW string.

The new define IB_FW_VERSION_NAME_MAX will limit the size of
FW version string. That define was chosen to be equal to
ETHTOOL_FWVERS_LEN, because many drivers anyway are limited
by that value indirectly.

The introduction of this define allows us to remove the string size
from get_fw_str function signature.

af56e53ccd i40iw: Free QP PBLEs when the QP is destroyed
If the physical buffer list entries (PBLEs) of a QP are freed
up at i40iw_dereg_mr, they can be assigned to a newly
created QP before the previous QP is destroyed. Fix this
by freeing PBLEs only when the QP is destroyed.

44b99f88cd i40iw: Avoid memory leak of CQP request objects
Control Queue Pair (CQP) request objects, which have
not received a completion upon interface close, remain
in memory.

To fix this, identify and free all pending CQP request
objects during destroy CQP OP.

6327cb09df i40iw: Release cm_id ref on PCI function reset
On PCI function reset, cm_id reference is not released
which causes an application hang, as it waits on the
cm_id to be released on rdma_destroy.

To fix this, call i40iw_cm_disconn during a PCI function
reset to clean-up resources and release cm_id reference.

ac6424b981 sched/wait: Rename wait_queue_t => wait_queue_entry_t
Rename:

	wait_queue_t		=>	wait_queue_entry_t

'wait_queue_t' was always a slight misnomer: its name implies that it's a "queue",
but in reality it's a queue *entry*. The 'real' queue is the wait queue head,
which had to carry the name.

Start sorting this out by renaming it to 'wait_queue_entry_t'.

This also allows the real structure name 'struct __wait_queue' to
lose its double underscore and become 'struct wait_queue_entry',
which is the more canonical nomenclature for such data types.

3e7e1193e2 IB: Replace ib_umem page_size by page_shift
Size of pages are held by struct ib_umem in page_size field.

It is better to store it as an exponent, because page size by nature
is always power-of-two and used as a factor, divisor or ilog2's argument.

The conversion of page_size to be page_shift allows to have portable
code and avoid following error while compiling on ARM:

871a8623d3 i40iw: Receive netdev events post INET_NOTIFIER state
Netdev notification events are de-registered only when all
client iwdev instances are removed. If a single client is closed
and re-opened, netdev events could arrive even before the Control
Queue-Pair (CQP) is created, causing a NULL pointer dereference crash
in i40iw_get_cqp_request. Fix this by allowing netdev event
notification only after we have reached the INET_NOTIFIER state with
respect to device initialization.

c4550c63b3 IB: Query ports via the core instead of direct into the driver
Change the drivers to call ib_query_port in their get port
immutable handler instead of their own query port handler.

Doing this required to set the core cap flags of this device
before the ib_query_port call is made, since the IB core might
need these caps to serve the port query.

Drivers are ensured by the IB core that the port attributes passed
to the port query verb implementation are zero, and hence we
removed the zeroing from the drivers.

This patch doesn't add any new functionality.

fd90d4d4c2 i40iw: Fix memory leak in CQP destroy when in reset
On a device close, the control QP (CQP) is destroyed by calling
cqp_destroy which destroys the CQP and frees its SD buffer memory.
However, if the reset flag is true, cqp_destroy is never called and
leads to a memory leak on SD buffer memory. Fix this by always calling
cqp_destroy, on device close, regardless of reset. The exception to this
when CQP create fails. In this case, the SD buffer memory is already
freed on an error check and there is no need to call cqp_destroy.

When flush QP and there are no pending work requests, signal completion
to unblock i40iw_drain_sq and i40iw_drain_rq which are waiting on

Also, signal completion if flush QP fails to prevent the drain SQ or RQ
from being blocked indefintely.

The macros I40IW_STAG_KEY_FROM_STAG and I40IW_STAG_INDEX_FROM_STAG are
apparently bad - they are using the logical "&&" operation which
does not make sense here. It should have been a bitwise "&" instead.
Since the macros seem to be completely unused, let's simply remove
them so that nobody accidentially uses them in the future. And while
we're at it, also remove the unused macro I40IW_CREATE_STAG.

d4165e3abd i40iw: Fix incorrect assignment of SQ head
The SQ head is incorrectly incremented when the number
of WQEs required is greater than the number available.
The fix is to use the I40IW_RING_MOV_HEAD_BY_COUNT
macro. This checks for the SQ full condition first and
only if SQ has room for the request, then we move the
head appropriately.

Device notifications are not received after the first interface is
closed; since there is an unregister for notifications on every
interface close. Correct this by unregistering for device
notifications only when the last interface is closed. Also, make
all operations on the i40iw_notifiers_registered atomic as it
can be read/modified concurrently.
