1c41a9570a usb: patches for v4.1 merge window
As usual, a big pile of commits. This time a total
 of 111 non-merge commits.
 
 Other than the usual set of cleanups and non-critical
 fixes, we have some interesting work for AM335x's MUSB
 babble recovery. Now that takes a lot less time and we
 don't have to Reset MUSB all the time.
 
 The printer gadget has been converted to configfs interface
 and the atmel udc has learned suspend/resume with wakeup.

This patch adds runtime power management support for otg fsm mode, since
A-device in a_idle state cannot detect data pulse irq after suspended, here
enable wakeup by connection before suspend to make it can be resumed by DP;

6adb9b7b5f usb: chipidea: add a flag for turn on vbus early for host
Some usb PHYs need power supply from vbus to make it work, eg mxs-phy, if
there is no vbus, USB PHY will not in correct state when the controller starts
to work, for host, this requires vbus should be turned on before setting port
power(PP) of ehci, to work with this kind of USB PHY design, this patch adds
a flag CI_HDRC_TURN_VBUS_EARLY_ON, can be checked by host driver to turn on
vbus while start host.

78f0357ec8 usb: chipidea: host: add .bus_suspend quirk
For chipidea, its resume sequence is not-EHCI compatible, see
below description for FPR at portsc. So in order to send SoF in
time for remote wakeup sequence(within 3ms), the RUN/STOP bit must
be set before the resume signal is ended, but the usb resume
code may run after resume signal is ended, so we had to set it
at suspend path.

Force Port Resume - RW. Default = 0b.
1= Resume detected/driven on port.
0=No resume (K-state) detected/driven on port.
Host mode:
Software sets this bit to one to drive resume signaling. The Controller sets this bit to '1' if
a J-to-K transition is detected while the port is in the Suspend state. When this bit
transitions to a '1' because a J-to-K transition is detected, the Port Change Detect bit in
the USBSTS register is also set to '1'. This bit will automatically change to '0' after the
resume sequence is complete. This behavior is different from EHCI where the controller
driver is required to set this bit to a '0' after the resume duration is timed in the driver.
Note that when the controller owns the port, the resume sequence follows the defined

sequence documented in the USB Specification Revision 2.0. The resume signaling
(Full-speed 'K') is driven on the port as long as this bit remains a '1'. This bit will remain
a '1' until the port has switched to idle. Writing a '0' has no affect because the port
controller will time the resume operation, clear the bit and the port control state switches
to HS or FS idle.
This field is '0' if Port Power(PP) is '0' in host mode.

This bit is not-EHCI compatible.

905276c431 Chipidea: Set connect-at-fullspeed bit when entering host mode if CI_HDRC_FORCE_FULLSPEED is set in the platform data
PORTSC_PFSC is not set on entering host mode which means the USB OTG
controller will attempt to enumerate USB devices at high speed even when the
CI_HDRC_FORCE_FULLSPEED flag is set in the platform data.

This patch ensures it is set right before host mode operations begin if needed.

Title: Adding a dTD to a Primed Endpoint May Not Get Recognized

Impacted Configuration: All device mode configurations.

Description:
There is an issue with the add dTD tripwire semaphore (ATDTW bit in USBCMD register)
that can cause the controller to ignore a dTD that is added to a primed endpoint.
When this happens, the software can read the tripwire bit and the status bit at '1'
even though the endpoint is unprimed.

After executing a dTD, the device controller endpoint state machine executes a final
read of the dTD terminate bit to check if the application added a dTD to the linked
list at the last moment. This read is done in the finpkt_read_latest_next_td (44) state.
After the read is performed, if the terminate bit is still set, the state machine moves
to unprime the endpoint. The decision to unprime the endpoint is done in the
checkqh_decision (59) state, based on the value of the terminate bit.
Before reaching the checkqh_decision state, the state machine traverses the
writeqhtd_status (57), writeqh_status (56), and release_prime_mask (42) states.
As shown in the waveform, the ep_addtd_tripwire_clr signal is not set to clear
the tripwire bit in these states.

Workaround:
The software must implement a periodic poll cycle, and check for each dTD
pending on execution (Active = 1), if the enpoint is primed. It can do this by reading
the corresponding bits in the ENDPTPRIME and ENDPTSTAT registers. If these bits are
read at 0, the software needs to re-prime the endpoint by writing 1 to the corresponding
bit in the ENDPTPRIME register. This can be done for every microframe, every frame or
with a larger interval, depending on the urgency of transfer execution for the application.

8721a752cc usb: chipidea: usbmisc_imx: add imx6sx initialization routine
Except the same process with earlier imx6, it has below two features:

- Choose which vbus voltage as vbus wakeup source
We choose B_SESSION_VALID as vbus wakeup source since when the system
goes to suspend, the vbus comparator can't compare the vbus voltage
for VBUS_VALID.

- Disable dp/dm (linestate) change as wakeup source at device mode
when the vbus is not there, we don't expect dp/dm change waking up
usb controller at this situation.

All imx usb controller's non core registers uses the same clock gate with
core registers, the usbmisc_imx is the library for imx glue driver, the
glue keeps clock on when it calls usbmisc_imx API to change non-core register.

Besides, we will support runtime pm in the future, it also needs to
close this clock when the usb is not in use.

d5d1e1bed4 usb: chipidea: udc: return immediately if re-enable non-empty endpoint
Some gadget driver (like uac1) will try to enable endpoint again even
the ep is not empty, it will cause the ep reset again and may affect
the dTD list which has already queued.

It returns -EBUSY immediately, and indicate the endpoint is in use.
In this way, the ep's behavior will not be affected, and the gadget
driver is also notified.

This patch adds response to a_alt_hnp_support set feature request from legacy
A device, that is, B-device can provide a message to the user indicating that
the user needs to connect the B-device to an alternate port on the A-device.
A device sets this feature indicates to the B-device that it is connected
to an A-device port that is not capable of HNP, but that the A-device does have
an alternate port that is capable of HNP.

24c498df1c Revert "usb: chipidea: remove duplicate dev_set_drvdata for host_start"
This reverts commit 14b4099c07

It moved platform_set_drvdata(pdev, ci) before hcd is created,
and the hcd will assign itself as ci controller's drvdata during
the hcd creation function (in usb_create_shared_hcd), so it
overwrites the real ci's drvdata which we want to use.

So, if the controller is at host mode, the system suspend
API will get the wrong struct ci_hdrc pointer, and cause the
oops.

 They are dominated by the removal of the .owner field in platform
 drivers.  They touch a lot of files, but they are "simple" changes, just
 removing a line in a structure.
 
 Other than that, a few minor driver core and debugfs changes.  There are
 some ath9k patches coming in through this tree that have been acked by
 the wireless maintainers as they relied on the debugfs changes.
 
 Everything has been in linux-next for a while.

21a5b579cb usb: chipidea: fix phy handling
The generic plaftorm device for ChipIdea drivers is probed by calling
ci_hdrc_probe. The device structure used is not the one of the specific
ChipIdea driver but the one of the generic ChipIdea platform device.

This results in not being able to probe the PHYs as we're not using the
right device structure. Since all ChipIdea drivers are retrieving their
PHYs in their specific driver code, this didn't impact any of them yet.

Fixes it using the right device structure (dev->parent).

The phy needs some delay to output the stable status from low
power mode. And for OTGSC, the status inputs are debounced
using a 1 ms time constant, so, delay 2ms for controller to get
the stable status(like vbus and id) when the phy leaves low power.

'driver' argument to ->udc_stop(), we can
safely remove it.

This commit is based on previous work by
Robert Baldyga <r.baldyga@samsung.com> which
can be found at [1]; however that patch turned
out to have a high probability of regressing
many UDC drivers because of a blind search & replace
s/driver/$udc->driver/ which caused the 'driver'
argument to stop_activity() to be a valid non-NULL
pointer when it should be NULL, thus causing UDCs
to mistakenly call gadget driver's ->disconnect()
callback.

16853d7bcb usb: ci_hdrc_imx: Return -EINVAL for missing USB PHY
-ENODEV is interpreted by the generic driver probing function as a
non-matching driver. This leads to a missing probe failure message.

Also a missing USB PHY is more of an invalid configuration of the usb
driver because it is necessary.

This patch returns -EINVAL if devm_usb_get_phy_by_phandle() returned -ENODEV.

953c664697 usb: chipidea: udc: Disable auto ZLP generation on ep0
There are 2 methods for ZLP (zero-length packet) generation:
1) In software
2) Automatic generation by device controller

1) is implemented in UDC driver and it attaches ZLP to IN packet if
   descriptor->size < wLength
2) can be enabled/disabled by setting ZLT bit in the QH

When gadget ffs is connected to ubuntu host, the host sends
get descriptor request and wLength in setup packet is 255 while the
size of descriptor which will be sent by gadget in IN packet is
64 byte. So the composite driver sets req->zero = 1.
In UDC driver following code will be executed then

        if (hwreq->req.zero && hwreq->req.length
            && (hwreq->req.length % hwep->ep.maxpacket == 0))
                add_td_to_list(hwep, hwreq, 0);

Case-A:
So in case of ubuntu host, UDC driver will attach a ZLP to the IN packet.
ubuntu host will request 255 byte in IN request, gadget will send 64 byte
with ZLP and host will come to know that there is no more data.
But hold on, by default ZLT=0 for endpoint 0 so hardware also tries to
automatically generate the ZLP which blocks enumeration for ~6 seconds due
to endpoint 0 STALL, NAKs are sent to host for any requests (OUT/PING)

Case-B:
In case when gadget ffs is connected to Apple device, Apple device sends
setup packet with wLength=64. So descriptor->size = 64 and wLength=64
therefore req->zero = 0 and UDC driver will not attach any ZLP to the
IN packet. Apple device requests 64 bytes, gets 64 bytes and doesn't
further request for IN data. But ZLT=0 by default for endpoint 0 so
hardware tries to automatically generate the ZLP which blocks enumeration
for ~6 seconds due to endpoint 0 STALL, NAKs are sent to host for any
requests (OUT/PING)

According to USB2.0 specs:

    8.5.3.2 Variable-length Data Stage
    A control pipe may have a variable-length data phase in which the
    host requests more data than is contained in the specified data
    structure. When all of the data structure is returned to the host,
    the function should indicate that the Data stage is ended by
    returning a packet that is shorter than the MaxPacketSize for the
    pipe. If the data structure is an exact multiple of wMaxPacketSize
    for the pipe, the function will return a zero-length packet to indicate
    the end of the Data stage.

In Case-A mentioned above:
If we disable software ZLP generation & ZLT=0 for endpoint 0 OR if software
ZLP generation is not disabled but we set ZLT=1 for endpoint 0 then
enumeration doesn't block for 6 seconds.

In Case-B mentioned above:
If we disable software ZLP generation & ZLT=0 for endpoint then enumeration
still blocks due to ZLP automatically generated by hardware and host not needing
it. But if we keep software ZLP generation enabled but we set ZLT=1 for
endpoint 0 then enumeration doesn't block for 6 seconds.

So the proper solution for this issue seems to disable automatic ZLP generation
by hardware (i.e by setting ZLT=1 for endpoint 0) and let software (UDC driver)
handle the ZLP generation based on req->zero field.

e4adcff09c usb: chipidea: udc: delete td from req's td list at ep_dequeue
We need to delete un-finished td from current request's td list
at ep_dequeue API, otherwise, this non-user td will be remained
at td list before this request is freed. So if we do ep_queue->
ep_dequeue->ep_queue sequence, when the complete interrupt for
the second ep_queue comes, we search td list for this request,
the first td (added by the first ep_queue) will be handled, and
its status is still active, so we will consider the this transfer
still not be completed, but in fact, it has completed. It causes
the peripheral side considers it never receives current data for
this transfer.

We met this problem when do "Error Recovery Test - Device Configured"
test item for USBCV2 MSC test, the host has never received ACK for
the IN token for CSW due to peripheral considers it does not get this
CBW, the USBCV test log like belows:

33f92a8a92 chipidea: usbmisc_imx: Allow USB OTG to work on mx51
The field PLLDIVVALUE of register PHY_CTRL_1 selects the reference clock source
for the PHY:
00 = sysclock uses 19.2 MHz
01 = sysclock uses 24 MHz
10 = sysclock uses 26 MHz
11 = sysclock uses 27 MHz

The reset value for this field is 10 according to the reference manual, and
even though this reset value works for mx53, it does not work for mx51.

So instead of relying on the reset value for the PLLDIVVALUE field, explicitly
set it to 01 so that a 24MHz clock can be selected for the PHY and allowing both
mx51 and mx53 to have USB OTG port functional.

Succesfully tested 'g_ether' on a imx51-babbage and on a imx53-qsb boards.

90893b90d3 usb: chipidea: add proper delay for waiting correct PHY status
After the PHY has powered and initialized, it needs some delay
for controller to reflect PHY's status. Some status and values
for id, vbus, dp/dm are only stable after this delay. The current
code tries to clear id/vbus status without enough delay, it
causes the status are not cleared properly.

This patch add 2ms delay after phy has initialized, and clear the
unexpected status after that.

d03cccff9c usb: chipidea: coordinate usb phy initialization for different phy type
For internal PHY (like UTMI), the phy clock may from internal pll,
it is on/off on the fly, the access PORTSC.PTS will hang without
phy clock. So, the usb_phy_init which will open phy clock needs to
be called before hw_phymode_configure.

For external PHY (like ulpi), it needs to configure portsc.pts before
visit viewport, or the viewport can't be visited. so phy_phymode_configure
needs to be called before usb_phy_init.

ENDPTFLUSH and ENDPTPRIME registers are set by software and clear
by hardware. There is a bit for each endpoint. When we are setting
a bit for an endpoint we should make sure we do not touch other
endpoint bit. There is a race condition if the hardware clear the
bit between the read and the write in hw_write.

2fc5a7dace usb: chipidea: udc: using MultO at TD as real mult value for ISO-TX
We have met a bug that the high bandwidth ISO-TX transfer has failed
at the last packet if it is less than 1024, the TD status shows it
is "Transaction Error".

The root cause of this problem is: the mult value at qh is not correct
for current TD's transfer length. We use TD list to queue un-transfer
TDs, and change mult for new adding TDs. If new adding TDs transfer length
less than 1024, but the queued un-transfer TDs transfer length is larger
than 1024, the transfer error will occur, and vice versa.
Usually, this problem occurs at the last packet, and the first packet for
new frame.

We fixed this problem by setting Mult at QH as the largest value (3), and
set MultO (Multiplier Override) at TD according to every transfer length.
It can cover both hardware version less than 2.3 (the real mult is MultO
if it is not 0) and 2.3+ (the real mult is min(qh.mult, td.multo)).

Since the MultO bits are only existed at TX TD, we keep the ISO-RX behavior
unchanged.

For stable tree: 3.11+.

5332ff1fb6 usb: chipidea: need to mask INT_STATUS when write otgsc
For otgsc, both enable bits and status bits are in it. So we need
to make sure the status bits are not be cleared when write enable
bits. It can fix one bug that we plug in/out Micro AB cable fast,
and sometimes, the IDIS will be cleared wrongly when handle last
ID interrupt (ID 0->1), so the current interrupt will not occur.

hw_phymode_configure configures the PORTSC registers and allow the
following phy_inits to operate on the right parameters. This fix a problem

e117e742d3 usb: gadget: add "maxpacket_limit" field to struct usb_ep
This patch adds "maxpacket_limit" to struct usb_ep. This field contains
maximum value of maxpacket supported by driver, and is set in driver probe.
This value should be used by autoconfig() function, because value of field
"maxpacket" is set to value from endpoint descriptor when endpoint becomes
enabled. So when autoconfig() function will be called again for this endpoint,
"maxpacket" value will contain wMaxPacketSize from descriptior instead of
maximum packet size for this endpoint.

For this reason this patch adds new field "maxpacket_limit" which contains
value of maximum packet size (which defines maximum endpoint capabilities).
This value is used in ep_matches() function used by autoconfig().

Value of "maxpacket_limit" should be set in UDC driver probe function, using
usb_ep_set_maxpacket_limit() function, defined in gadget.h. This function
set choosen value to both "maxpacket_limit" and "maxpacket" fields.

This patch modifies UDC drivers by adding support for maxpacket_limit.

5a1e1456fc usb: chipidea: fix nobody cared IRQ when booting with host role
If we connect Male-A-To-Male-A cable between otg-host and host pc,
the ci->vbus_active is set wrongly, and cause the controller run
at peripheral mode when we load gadget module (ci_udc_start will be run),
but the software runs at host mode due to id = 0. The ehci_irq
can't handle suspend (USBi_SLI) interrupt which is enabled for
peripheral mode, it causes no one will handle irq error.

This patch is needed for 3.12 stable

     drivers forcefully setting DMA masks.  This is more a marker for
     future work to further clean these locations up - the code which
     creates the devices really should be initialising these, but to fix
     that in one go along with this change could potentially be very
     disruptive.

  The last thing this series does is prise away some of Linux's addition
  to "DMA addresses are physical addresses and RAM always starts at
  zero".  We have ARM LPAE systems where all system memory is above 4GB
  physical, hence having DMA masks interpreted by (eg) the block layers
  as describing physical addresses in the range 0..DMAMASK fails on
  these platforms.  Santosh Shilimkar addresses this in this series; the
  patches were copied to the appropriate people multiple times but were
  ignored.

  Fixing this also gets rid of some ARM weirdness in the setup of the
  max*pfn variables, and brings ARM into line with every other Linux
  architecture as far as those go"

22d9d8e831 DMA-API: usb: use dma_set_coherent_mask()
The correct way for a driver to specify the coherent DMA mask is
not to directly access the field in the struct device, but to use
dma_set_coherent_mask().  Only arch and bus code should access this
member directly.

Convert all direct write accesses to using the correct API.

e7ef5265b1 usb: chipidea: udc: free pending TD at removal procedure
There is a pending TD which is not freed after request finishes,
we do this due to a controller bug. This TD needs to be freed when
the driver is removed. It prints below error message when unload
chipidea driver at current code:
"ci_hdrc ci_hdrc.0: dma_pool_destroy ci_hw_td, b0001000 busy"
It indicates the buffer at dma pool are still in use.

This commit will free the pending TD at driver's removal procedure,
it can fix the problem described above.

We find this issue due to the PHY's clock refcount is not correct
due to -EPROBE_DEFER return after phy's init.

f84839daa7 usb: chipidea: udc: fix the oops after rmmod gadget
When we rmmod gadget, the ci->driver needs to be cleared.
Otherwise, when we plug in usb cable again, the driver will
consider gadget is there, and go to enumeration procedure,

92b336d7f6 usb: chipidea: udc: Consolidate the call of disconnect
The udc-core will call gadget's driver->disconnect, so we should avoid
calling gadget's disconnect again at ci_udc_stop in case the gadget's
unbind free some structs which is still used at gadget's disconnect.

4a29567b77 chipidea: udc: free pending TD at removal procedure
There is a pending TD which is not freed after request finishes,
we do this due to a controller bug. This TD needs to be freed when
the driver is removed. It prints below error message when unload
chipidea driver at current code:
"ci_hdrc ci_hdrc.0: dma_pool_destroy ci_hw_td, b0001000 busy"
It indicates the buffer at dma pool are still in use.

This commit will free the pending TD at driver's removal procedure,
it can fix the problem described above.
