c1d036c4d1 [IA64] mca.c: Fix cast from integer to pointer warning
ia64_mca_cpu_init has a void *data local variable that is assigned
the value from either __get_free_pages() or mca_bootmem(). The problem
is that __get_free_pages returns an unsigned long and mca_bootmem, via
alloc_bootmem(), returns a void *. format_mca_init_stack takes the void *,
and it's also used with __pa(), but that casts it to long anyway.

This results in the following build warning:

arch/ia64/kernel/mca.c:1898: warning: assignment makes pointer from
integer without a cast

Cast the return of __get_free_pages to a void * to avoid
the warning.

a396768574 [IA64] disable interrupts at end of ia64_mca_cpe_int_handler()
SAL requires that interrupts be enabled when making some calls
to it to pick up error records, so we enable interrupts inside
this handler.  We should disable them again at the end.

Found by a new WARN_ONCE that tglx added to handle_irq_event_percpu()

f1a2003e22 ACPI / PM: Merge do_suspend_lowlevel() into acpi_save_state_mem()
The function do_suspend_lowlevel() is specific to x86 and defined in
assembly code, so it should be called from the x86 low-level suspend
code rather than from acpi_suspend_enter().

Merge do_suspend_lowlevel() into the x86's acpi_save_state_mem() and
change the name of the latter to acpi_suspend_lowlevel(), so that the
function's purpose is better reflected by its name.

19df0c2fef percpu: align percpu readmostly subsection to cacheline
Currently percpu readmostly subsection may share cachelines with other
percpu subsections which may result in unnecessary cacheline bounce
and performance degradation.

This patch adds @cacheline parameter to PERCPU() and PERCPU_VADDR()
linker macros, makes each arch linker scripts specify its cacheline
size and use it to align percpu subsections.

This is based on Shaohua's x86 only patch.

d18960494f ACPI, intel_idle: Cleanup idle= internal variables
Having four variables for the same thing:
  idle_halt, idle_nomwait, force_mwait and boot_option_idle_overrides
is rather confusing and unnecessary complex.

if idle= boot param is passed, only set up one variable:
boot_option_idle_overrides

Introduces following functional changes/fixes:
  - intel_idle driver does not register if any idle=xy
    boot param is passed.
  - processor_idle.c will also not register a cpuidle driver
    and get active if idle=halt is passed.
    Before a cpuidle driver with one (C1, halt) state got registered
    Now the default_idle function will be used which finally uses
    the same idle call to enter sleep state (safe_halt()), but
    without registering a whole cpuidle driver.

That means idle= param will always avoid cpuidle drivers to register
with one exception (same behavior as before):
idle=nomwait
may still register acpi_idle cpuidle driver, but C1 will not use
mwait, but hlt. This can be a workaround for IO based deeper sleep
states where C1 mwait causes problems.

05f2f274c8 [IA64] Avoid array overflow if there are too many cpus in SRAT table
acpi_numa_init() has to parse the whole SRAT table, even if the
kernel wants to limit the number of cpus it will use (because the
ones it is going to use may be described by entries at the end of
the SRAT table).  Avoid overflowing the node_cpuid array.

b3e19d924b fs: scale mntget/mntput
The problem that this patch aims to fix is vfsmount refcounting scalability.
We need to take a reference on the vfsmount for every successful path lookup,
which often go to the same mount point.

The fundamental difficulty is that a "simple" reference count can never be made
scalable, because any time a reference is dropped, we must check whether that
was the last reference. To do that requires communication with all other CPUs
that may have taken a reference count.

We can make refcounts more scalable in a couple of ways, involving keeping
distributed counters, and checking for the global-zero condition less
frequently.

- check the global sum once every interval (this will delay zero detection
  for some interval, so it's probably a showstopper for vfsmounts).

- keep a local count and only taking the global sum when local reaches 0 (this
  is difficult for vfsmounts, because we can't hold preempt off for the life of
  a reference, so a counter would need to be per-thread or tied strongly to a
  particular CPU which requires more locking).

- keep a local difference of increments and decrements, which allows us to sum
  the total difference and hence find the refcount when summing all CPUs. Then,
  keep a single integer "long" refcount for slow and long lasting references,
  and only take the global sum of local counters when the long refcount is 0.

This last scheme is what I implemented here. Attached mounts and process root
and working directory references are "long" references, and everything else is
a short reference.

This allows scalable vfsmount references during path walking over mounted
subtrees and unattached (lazy umounted) mounts with processes still running
in them.

This results in one fewer atomic op in the fastpath: mntget is now just a
per-CPU inc, rather than an atomic inc; and mntput just requires a spinlock
and non-atomic decrement in the common case. However code is otherwise bigger
and heavier, so single threaded performance is basically a wash.

fb045adb99 fs: dcache reduce branches in lookup path
Reduce some branches and memory accesses in dcache lookup by adding dentry
flags to indicate common d_ops are set, rather than having to check them.
This saves a pointer memory access (dentry->d_op) in common path lookup
situations, and saves another pointer load and branch in cases where we
have d_op but not the particular operation.

fe15ce446b fs: change d_delete semantics
Change d_delete from a dentry deletion notification to a dentry caching
advise, more like ->drop_inode. Require it to be constant and idempotent,
and not take d_lock. This is how all existing filesystems use the callback
anyway.

This makes fine grained dentry locking of dput and dentry lru scanning
much simpler.

e21763dbce [IA64] perfmon: Change vmalloc to vzalloc and drop memset.
vzalloc() nicely zeroes memory for us, so we don't have to do a vmalloc()
and then manually memset() the returned memory when all we want is for it
to be zero. Patch changes this for pfm_rvmalloc().

75c1c91cb9 [IA64] eliminate race condition in smp_flush_tlb_mm
A race condition exists within smp_call_function_many() when called from
smp_flush_tlb_mm().  On rare occasions the cpu_vm_mask can be cleared
while smp_call_function_many is executing, occasionally resulting in a
hung process.

Make a copy of the mask prior to calling smp_call_function_many().

6038f373a3 llseek: automatically add .llseek fop
All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.

The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.

New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time.  Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.

The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.

Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

4de0a75948 [IA64] Initialize interrupts later (from init_IRQ())
Thomas Gleixner is cleaning up the generic irq code, and ia64 ran
into problems because it calls register_intr() before early_irq_init()
is called.  Move the call to acpi_boot_init() from setup_arch() to
init_IRQ().

As a bonus - moving the call later means we no longer need the
hacks in iosapic.c to switch between the bootmem and regular
allocator - we can just used kzalloc() for allocation.

Mostly trivial stuff for this. The only tricky part was realizing
that the new handler_*_irq() paths do not use desc->chip->end(irq).
Not a problem for the edge case as the ia64 iosapic routine for
that was nop(). But the "level" case handled interrupt migration
there.  Just use a slightly modified version of the "end" routine
as "unmask" for the level triggered case.

f574c84319 [IA64] fix siglock
When ia64 converted to using ticket locks, an inline implementation
of trylock/unlock in fsys.S was missed.  This was not noticed because
in most circumstances it simply resulted in using the slow path because
the siglock was apparently not available (under old spinlock rules).

Problems occur when the ticket spinlock has value 0x0 (when first
initialised, or when it wraps around). At this point the fsys.S
code acquires the lock (changing the 0x0 to 0x1. If another process
attempts to get the lock at this point, it will change the value from
0x1 to 0x2 (using new ticket lock rules). Then the fsys.S code will
free the lock using old spinlock rules by writing 0x0 to it. From
here a variety of bad things can happen.

Make do_execve() take a const filename pointer so that kernel_execve() compiles
correctly on ARM:

arch/arm/kernel/sys_arm.c:88: warning: passing argument 1 of 'do_execve' discards qualifiers from pointer target type

This also requires the argv and envp arguments to be consted twice, once for
the pointer array and once for the strings the array points to.  This is
because do_execve() passes a pointer to the filename (now const) to
copy_strings_kernel().  A simpler alternative would be to cast the filename
pointer in do_execve() when it's passed to copy_strings_kernel().

do_execve() may not change any of the strings it is passed as part of the argv
or envp lists as they are some of them in .rodata, so marking these strings as
const should be fine.

Further kernel_execve() and sys_execve() need to be changed to match.

This has been test built on x86_64, frv, arm and mips.

4565f0170d dma-mapping: unify dma_get_cache_alignment implementations
dma_get_cache_alignment returns the minimum DMA alignment.  Architectures
defines it as ARCH_DMA_MINALIGN (formally ARCH_KMALLOC_MINALIGN).  So we
can unify dma_get_cache_alignment implementations.

Note that some architectures implement dma_get_cache_alignment wrongly.
dma_get_cache_alignment() should return the minimum DMA alignment.  So
fully-coherent architectures should return 1.  This patch also fixes this
issue.

30da552428 PCI: MSI: Restore read_msi_msg_desc(); add get_cached_msi_msg_desc()
commit 2ca1af9aa3285c6a5f103ed31ad09f7399fc65d7 "PCI: MSI: Remove
unsafe and unnecessary hardware access" changed read_msi_msg_desc() to
return the last MSI message written instead of reading it from the
device, since it may be called while the device is in a reduced
power state.

However, the pSeries platform code really does need to read messages
from the device, since they are initially written by firmware.
Therefore:
- Restore the previous behaviour of read_msi_msg_desc()
- Add new functions get_cached_msi_msg{,_desc}() which return the
  last MSI message written
- Use the new functions where appropriate

7615856ebf timkeeping: Fix update_vsyscall to provide wall_to_monotonic offset
update_vsyscall() did not provide the wall_to_monotoinc offset,
so arch specific implementations tend to reference wall_to_monotonic
directly. This limits future cleanups in the timekeeping core, so
this patch fixes the update_vsyscall interface to provide
wall_to_monotonic, allowing wall_to_monotonic to be made static
as planned in Documentation/feature-removal-schedule.txt

b71ab8c202 workqueue: increase max_active of keventd and kill current_is_keventd()
Define WQ_MAX_ACTIVE and create keventd with max_active set to half of
it which means that keventd now can process upto WQ_MAX_ACTIVE / 2 - 1
works concurrently.  Unless some combination can result in dependency
loop longer than max_active, deadlock won't happen and thus it's
unnecessary to check whether current_is_keventd() before trying to
schedule a work.  Kill current_is_keventd().

(Lockdep annotations are broken.  We need lock_map_acquire_read_norecurse())

Enable 'HAVE_MEMORYLESS_NODES' by default when NUMA configured on ia64.
Initialize percpu 'numa_mem' variable when starting secondary cpus.
Generic initialization will handle the boot cpu.

Nothing uses 'numa_mem_id()' yet.  Subsequent patch with modify slab to
use this.

Preparation to make task->signal immutable, no functional changes.

It doesn't matter which pointer we check under tasklist to ensure the task
was not released, ->signal or ->sighand.  But we are going to make
->signal refcountable, change the code to use ->sighand.

Note: this code doesn't need this check and tasklist_lock at all, it
should be converted to use lock_task_sighand().  And, the code under
SIGNAL_STOP_STOPPED check looks wrong.

2c2df8418a x86, acpi/irq: Introduce apci_isa_irq_to_gsi
There are a number of cases where the current code makes the assumption
that isa irqs identity map to the first 16 acpi global system intereupts.
In most instances that assumption is correct as that is the required
behaviour in dual i8259 mode and the default behavior in ioapic mode.

However there are some systems out there that take advantage of acpis
interrupt remapping  for the isa irqs to have a completely different
mapping of isa_irq to gsi.

Introduce acpi_isa_irq_to_gsi to perform this mapping explicitly in the
code that needs it.  Initially this will be just the current assumed
identity mapping to ensure it's introduction does not cause regressions.

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

d8191fa4a3 ACPI: processor: driver doesn't need to evaluate _PDC
Now that the early _PDC evaluation path knows how to correctly
evaluate _PDC on only physically present processors, there's no
need for the processor driver to evaluate it later when it loads.

To cover the hotplug case, push _PDC evaluation down into the
hotplug paths.

6ffdc5774a ia64: Convert ia64 to use read/update_persistent_clock

the amount of arch specific code we have to maintain, and allowing for

52cf25d0ab Driver core: Constify struct sysfs_ops in struct kobj_type
Constify struct sysfs_ops.

This is part of the ops structure constification
effort started by Arjan van de Ven et al.

Benefits of this constification:

 * prevents modification of data that is shared
   (referenced) by many other structure instances
   at runtime

 * detects/prevents accidental (but not intentional)
   modification attempts on archs that enforce
   read-only kernel data at runtime

 * potentially better optimized code as the compiler
   can assume that the const data cannot be changed

 * the compiler/linker move const data into .rodata
   and therefore exclude them from false sharing

8d9032bbe4 elf coredump: add extended numbering support
The current ELF dumper implementation can produce broken corefiles if
program headers exceed 65535.  This number is determined by the number of
vmas which the process have.  In particular, some extreme programs may use
more than 65535 vmas.  (If you google max_map_count, you can find some
users facing this problem.) This kind of program never be able to generate
correct coredumps.

This patch implements ``extended numbering'' that uses sh_info field of
the first section header instead of e_phnum field in order to represent
upto 4294967295 vmas.

This is supported by
AMD64-ABI(http://www.x86-64.org/documentation.html) and
Solaris(http://docs.sun.com/app/docs/doc/817-1984/).
Of course, we are preparing patches for gdb and binutils.

1fcccbac89 elf coredump: replace ELF_CORE_EXTRA_* macros by functions
elf_core_dump() and elf_fdpic_core_dump() use #ifdef and the corresponding
macro for hiding _multiline_ logics in functions.  This patch removes
#ifdef and replaces ELF_CORE_EXTRA_* by corresponding functions.  For
architectures not implemeonting ELF_CORE_EXTRA_*, we use weak functions in
order to reduce a range of modification.

This cleanup is for my next patches, but I think this cleanup itself is
worth doing regardless of my firnal purpose.

5beb493052 mm: change anon_vma linking to fix multi-process server scalability issue
The old anon_vma code can lead to scalability issues with heavily forking
workloads.  Specifically, each anon_vma will be shared between the parent
process and all its child processes.

In a workload with 1000 child processes and a VMA with 1000 anonymous
pages per process that get COWed, this leads to a system with a million
anonymous pages in the same anon_vma, each of which is mapped in just one
of the 1000 processes.  However, the current rmap code needs to walk them
all, leading to O(N) scanning complexity for each page.

This can result in systems where one CPU is walking the page tables of
1000 processes in page_referenced_one, while all other CPUs are stuck on
the anon_vma lock.  This leads to catastrophic failure for a benchmark
like AIM7, where the total number of processes can reach in the tens of
thousands.  Real workloads are still a factor 10 less process intensive
than AIM7, but they are catching up.

This patch changes the way anon_vmas and VMAs are linked, which allows us
to associate multiple anon_vmas with a VMA.  At fork time, each child
process gets its own anon_vmas, in which its COWed pages will be
instantiated.  The parents' anon_vma is also linked to the VMA, because
non-COWed pages could be present in any of the children.

This reduces rmap scanning complexity to O(1) for the pages of the 1000
child processes, with O(N) complexity for at most 1/N pages in the system.
 This reduces the average scanning cost in heavily forking workloads from
O(N) to 2.

The only real complexity in this patch stems from the fact that linking a
VMA to anon_vmas now involves memory allocations.  This means vma_adjust
can fail, if it needs to attach a VMA to anon_vma structures.  This in
turn means error handling needs to be added to the calling functions.

A second source of complexity is that, because there can be multiple
anon_vmas, the anon_vma linking in vma_adjust can no longer be done under
"the" anon_vma lock.  To prevent the rmap code from walking up an
incomplete VMA, this patch introduces the VM_LOCK_RMAP VMA flag.  This bit
flag uses the same slot as the NOMMU VM_MAPPED_COPY, with an ifdef in mm.h
to make sure it is impossible to compile a kernel that needs both symbolic
values for the same bitflag.

Some test results:

Without the anon_vma changes, when AIM7 hits around 9.7k users (on a test
box with 16GB RAM and not quite enough IO), the system ends up running
>99% in system time, with every CPU on the same anon_vma lock in the
pageout code.

With these changes, AIM7 hits the cross-over point around 29.7k users.
This happens with ~99% IO wait time, there never seems to be any spike in
system time.  The anon_vma lock contention appears to be resolved.

d868080d2a [IA64] Only build arch/ia64/kernel/acpi.o when CONFIG_ACPI
The following commit broke the ia64 sim_defconfig build:
	3b2b84c0b81108a9a869a88bf2beeb5a95d81dd1
	ACPI: processor: driver doesn't need to evaluate _PDC

This is because it added:
	+#include <acpi/processor.h>

To arch/ia64/kernel/acpi.c. Unfortunately, the ia64_simdefconfig does
not turn on CONFIG_ACPI, and we get build errors.

The fix described in $subject seems to be the most sensible way to
untangle the mess.

The other issue is that acpi_get_sysname() is required for all configs,
most of which define CONFIG_ACPI, but are not CONFIG_IA64_GENERIC. Turn
it into an inline to cover the "non generic" ia64 configs; to prevent
a duplicate definition build error, we need to wrap the definition in
acpi.o inside an #ifdef.

Finally, move the pm_idle and pm_power_off exports into process.c (which
is always built), similar to other architectures, and allow the sim
defconfig to link.

2b633e3fac smp: Use nr_cpus= to set nr_cpu_ids early
On x86, before prefill_possible_map(), nr_cpu_ids will be NR_CPUS aka
CONFIG_NR_CPUS.

Add nr_cpus= to set nr_cpu_ids. so we can simulate cpus <=8 are installed on
normal config.

-v2: accordging to Christoph, acpi_numa_init should use nr_cpu_ids in stead of
     NR_CPUS.

Disable kprobe booster when CONFIG_PREEMPT=y at this time,
because it can't ensure that all kernel threads preempted on
kprobe's boosted slot run out from the slot even using
freeze_processes().

The booster on preemptive kernel will be resumed if
synchronize_tasks() or something like that is introduced.

6c57a33290 [IA64] __per_cpu_idtrs[] is a memory hog
__per_cpu_idtrs is statically allocated ... on CONFIG_NR_CPUS=4096
systems it hogs 16MB of memory. This is way too much for a quite
probably unused facility (only KVM uses dynamic TR registers).

Change to an array of pointers, and allocate entries as needed on
a per cpu basis.  Change the name too as the __per_cpu_ prefix is
confusing (this isn't a classic <linux/percpu.h> type object).

6c5807d7bc ACPI: processor: finish unifying arch_acpi_processor_init_pdc()
The only thing arch-specific about calling _PDC is what bits get
set in the input obj_list buffer.

There's no need for several levels of indirection to twiddle those
bits. Additionally, since we're just messing around with a buffer,
we can simplify the interface; no need to pass around the entire
struct acpi_processor * just to get at the buffer.

When INIT is sent, ip/psr/pfs register is stored to the I-resources
(iip/ipsr/ifs registers), and they are copied in the min-state save
area(pmsa_{iip,ipsr,ifs}).

Therefore, in creating pt_regs at ia64_mca_modify_original_stack(),
cr_{iip,ipsr,ifs} should be derived from pmsa_{iip,ipsr,ifs}. But
current code copies pmsa_{xip,xpsr,xfs} to cr_{iip,ipsr,ifs}
when PSR.ic is 0.

It's ok when PSR.ic is not 0. But when PSR.ic is 0, this could be
a problem when we investigate kernel as the value of regs->cr_iip does
not point to where INIT really interrupted.

At first I tried to change finish_pt_regs() so that it uses always
pmsa_{iip,ipsr,ifs} for cr_{iip,ipsr,ifs}, but Keith Owens pointed out
it could cause another problem if I change it.

09b366b78c [IA64] preallocate IA64_IRQ_MOVE_VECTOR
Previously, we tried to use IA64_DEF_FIRST_DEVICE_VECTOR (0x30) as the
IA64_IRQ_MOVE_VECTOR.  However, we allocate other IRQs from the device
vector range, so there's no guarantee that IA64_DEF_FIRST_DEVICE_VECTOR
will still be available when we register IA64_IRQ_MOVE_VECTOR.

This patch statically allocates 0x30 for IA64_IRQ_MOVE_VECTOR and
removes it from the device vector range.

Without this patch, we crash on machines like the HP rx3600 that use
vector 48 (0x30) as the ACPI SCI interrupt:

bb52d66940 Get rid of open-coding in ia64_brk()
The comment in there used to be true, but these days do_brk() does
the arch-specific check that covers what we open-coded here.  So we
can use sys_brk() just fine, only need to do force_successful_syscall_return()
after it.

See commit 3a45975681 - that's when the
checks in do_brk() had been originally added.

0696b711e4 timekeeping: Fix clock_gettime vsyscall time warp
Since commit 0a544198 "timekeeping: Move NTP adjusted clock multiplier
to struct timekeeper" the clock multiplier of vsyscall is updated with
the unmodified clock multiplier of the clock source and not with the
NTP adjusted multiplier of the timekeeper.

This causes user space observerable time warps:
new CLOCK-warp maximum: 120 nsecs,  00000025c337c537 -> 00000025c337c4bf

Add a new argument "mult" to update_vsyscall() and hand in the
timekeeping internal NTP adjusted multiplier.

ad32e8cb86 swiotlb: Defer swiotlb init printing, export swiotlb_print_info()
This enables us to avoid printing swiotlb memory info when we
initialize swiotlb. After swiotlb initialization, we could find
that we don't need swiotlb.

This patch removes the code to print swiotlb memory info in
swiotlb_init() and exports the function to do that.

dd17c8f729 percpu: remove per_cpu__ prefix.
Now that the return from alloc_percpu is compatible with the address
of per-cpu vars, it makes sense to hand around the address of per-cpu
variables.  To make this sane, we remove the per_cpu__ prefix we used
created to stop people accidentally using these vars directly.

Now we have sparse, we can use that (next patch).

This patch updates percpu related symbols in ia64 such that percpu
symbols are unique and don't clash with local symbols.  This serves
two purposes of decreasing the possibility of global percpu symbol
collision and allowing dropping per_cpu__ prefix from percpu symbols.

a2e2725541 net: Introduce recvmmsg socket syscall
Meaning receive multiple messages, reducing the number of syscalls and
net stack entry/exit operations.

Next patches will introduce mechanisms where protocols that want to
optimize this operation will provide an unlocked_recvmsg operation.

This takes into account comments made by:

. Paul Moore: sock_recvmsg is called only for the first datagram,
  sock_recvmsg_nosec is used for the rest.

. Caitlin Bestler: recvmmsg now has a struct timespec timeout, that
  works in the same fashion as the ppoll one.

  If the underlying protocol returns a datagram with MSG_OOB set, this
  will make recvmmsg return right away with as many datagrams (+ the OOB
  one) it has received so far.

. Rémi Denis-Courmont & Steven Whitehouse: If we receive N < vlen
  datagrams and then recvmsg returns an error, recvmmsg will return
  the successfully received datagrams, store the error and return it
  in the next call.

This paves the way for a subsequent optimization, sk_prot->unlocked_recvmsg,
where we will be able to acquire the lock only at batch start and end, not at
every underlying recvmsg call.

52594762a3 ia64: convert to dynamic percpu allocator
Unlike other archs, ia64 reserves space for percpu areas during early
memory initialization.  These areas occupy a contiguous region indexed
by cpu number on contiguous memory model or are grouped by node on
discontiguous memory model.

As allocation and initialization are done by the arch code, all that
setup_per_cpu_areas() needs to do is communicating the determined
layout to the percpu allocator.  This patch implements
setup_per_cpu_areas() for both contig and discontig memory models and
drops HAVE_LEGACY_PER_CPU_AREA.

Please note that for contig model, the allocation itself is modified
only to allocate for possible cpus instead of NR_CPUS.  As dynamic
percpu allocator can handle non-direct mapping, there's no reason to
allocate memory for cpus which aren't possible.

36886478f5 ia64: allocate percpu area for cpu0 like percpu areas for other cpus
cpu0 used special percpu area reserved by the linker, __cpu0_per_cpu,
which is set up early in boot by head.S.  However, this doesn't
guarantee that the area will be on the same node as cpu0 and the
percpu area for cpu0 ends up very far away from percpu areas for other
cpus which cause problems for congruent percpu allocator.

This patch makes percpu area initialization allocate percpu area for
cpu0 like any other cpus and copy it from __cpu0_per_cpu which now
resides in the __init area.  This means that for cpu0, percpu area is
first setup at __cpu0_per_cpu early by head.S and then moved to an
area in the linear mapping during memory initialization and it's not
allowed to take a pointer to percpu variables between head.S and
memory initialization.

12cda81777 ia64: initialize cpu maps early
All information necessary to initialize cpu possible and present maps
are available once early_acpi_boot_init() is complete.  Reorganize
setup_arch() and acpi init functions such that,

* CPU information is printed after LAPIC entries are parsed in
  early_acpi_boot_init().

* smp_build_cpu_map() is called by setup_arch() instead of acpi
  functions.

* smp_build_cpu_map() is called once all CPU related information is
  available before memory is initialized.

This is primarily to allow find_memory() to use cpu maps but is also a
general cleanup.  Please note that with this change, the somewhat
ad-hoc early_cpu_possible_map defined and used for NUMA configurations
is probably unnecessary.  Something to clean up another day.

2c86963b09 [IA64] implement ticket locks for Itanium
Back in January 2008 Nick Piggin implemented "ticket" spinlocks
for X86 (See commit 314cdbefd1).

IA64 implementation has a couple of differences because of the
available atomic operations ... e.g. we have no fetchadd2 instruction
that operates on a 16-bit quantity so we make ticket locks use
a 32-bit word for each of the current ticket and now-serving values.

Performance on uncontended locks is about 8% worse than the previous
implementation, but this seems a good trade for determinism in the
contended case. Performance impact on macro-level benchmarks is in
the noise.

6ae8635085 [IA64] Clean up linker script using standard macros.
Aside from using fewer output sections and moving some data around,
the main side effect of this change is changing the alignment of some
sections. In particular:

* cachline-aligned and read_mostly data are now aligned to
  SMP_CACHE_BYTES. (Previously, they were laid out consecutively after
  a PAGE_SIZE alignment)
* .init.ramfs is now page-aligned, per the INIT_RAM_FS
  macro. (Previously it had no explicit alignment).

f172468a14 [IA64] Use .ref.text, not .text.init for start_ap.
It seems that start_ap doesn't need to be in a special location in the
kernel, but it references some init code so it should be in .ref.text.

0cced40e7c [IA64] kdump: Short path to freeze CPUs
Setting monarch_cpu = -1 to let slaves frozen might not work, because
there might be slaves being late, not entered the rendezvous yet.
Such slaves might be caught in while (monarch_cpu == -1) loop.

Use kdump_in_progress instead of monarch_cpus to break INIT rendezvous
and let all slaves enter DIE_INIT_SLAVE_LEAVE smoothly.

And monarch no longer need to manage rendezvous if once kdump_in_progress
is set, catch the monarch in DIE_INIT_MONARCH_ENTER then.

1726b0883d [IA64] kdump: Mask INIT first in panic-kdump path
Summary:

  Asserting INIT might block kdump if the system is already going to
  start kdump via panic.

Description:

  INIT can interrupt anywhere in panic path, so it can interrupt in
  middle of kdump kicked by panic.  Therefore there is a race if kdump
  is kicked concurrently, via Panic and via INIT.

  INIT could fail to invoke kdump if the system is already going to
  start kdump via panic.  It could not restart kdump from INIT handler
  if some of cpus are already playing dead with INIT masked.  It also
  means that INIT could block kdump's progress if no monarch is entered
  in the INIT rendezvous.

  Panic+INIT is a rare, but possible situation since it can be assumed
  that the kernel or an internal agent decides to panic the unstable
  system while another external agent decides to send an INIT to the
  system at same time.

How to reproduce:

  Assert INIT just after panic, before all other cpus have frozen

Expected results:

  continue kdump invoked by panic, or restart kdump from INIT

Actual results:

  might be hang, crashdump not retrieved

Proposed Fix:

  This patch masks INIT first in panic path to take the initiative on
  kdump, and reuse atomic value kdump_in_progress to make sure there is
  only one initiator of kdump.  All INITs asserted later should be used
  only for freezing all other cpus.

  This mask will be removed soon by rfi in relocate_kernel.S, before jump
  into kdump kernel, after all cpus are frozen and no-op INIT handler is
  registered.  So if INIT was in the interval while it is masked, it will
  pend on the system and will received just after the rfi, and handled by
  the no-op handler.

  If there was a MCA event while psr.mc is 1, in theory the event will
  pend on the system and will received just after the rfi same as above.
  MCA handler is unregistered here at the time, so received MCA will not
  reach to OS_MCA and will result in warmboot by SAL.

  Note that codes in this masked interval are relatively simpler than
  that in MCA/INIT handler which also executed with the mask.  So it can
  be said that probability of error in this interval is supposed not so
  higher than that in MCA/INIT handler.

68cb14c7c4 [IA64] kdump: Don't return APs to SAL from kdump
Summary:

  Asserting INIT on cpu going to be offline will result in unexpected
  behavior.  It will be a real problem in kdump cases where INIT might
  be asserted to unstable APs going to be offline by returning to SAL.

Description:

  Since psr.mc is cleared when bits in psr are set to SAL_PSR_BITS_TO_SET
  in ia64_jump_to_sal(), there is a small window (~few msecs) that the
  cpu can receive INIT even if the cpu enter there via INIT handler.
  In this window we do restore of registers for SAL, so INIT asserted
  here will not work properly.

  It is hard to remove this window by masking INIT (i.e. setting psr.mc)
  because we have to unmask it later in OS, because we have to use branch
  instruction (br.ret, not rfi) to return SAL, due to OS_BOOT_RENDEZ to
  SAL return convention.

  I suppose this window will not be a real problem on cpu offline if we
  can educate people not to push INIT button during hotplug operation.
  However, only exception is a race in kdump and INIT.  Now kdump returns
  APs to SAL before processing dump, but the kernel might receive INIT at
  that point in time.  Such INIT might be asserted by kdump itself if an
  AP doesn't react IPI soon and kdump decided to use INIT to stop the AP.
  Or it might be asserted by operator or an external agent to start dump
  on the unstable system.

  Such panic+INIT or INIT+INIT cases should be rare, but it will be happy
  if we can retrieve crashdump even in such cases.

How to reproduce:

  panic+INIT or INIT+INIT, with kdump configured

Expected results:

  crashdump is retrieved anyway

Actual results:

  panic, hang etc. (unexpected)

Proposed fix

  To avoid the window on the way to SAL, this patch stops returning APs
  to SAL in case of kdump.  In other words, this patch makes APs spin
  in OS instead of spinning in SAL.

  (* Note: What impact would be there?  If a cpu is spinning in SAL,
   the cpu is in BOOT_RENDEZ loop, as same as offlined cpu.
   In theory if an INIT is asserted there, cpus in the BOOT_RENDEZ loop
   should not invoke OS_INIT on it.  So in either way, no matter where
   the cpu is spinning actually in, once cpu starts spin and act as
   "frozen," INIT on the cpu have no effects.
   From another point of view, all debug information on the cpu should
   have stored to memory before the cpu start to be frozen.  So no more
   action on the cpu is required.)

  I confirmed that the kdump sometime hangs by concurrent INITs (another
  INIT after an INIT), and it doesn't hang after applying this patch.

  should not be used from the cpu.  Even the resources for MCA handler
  are no exception.  So we cannot handle MCAs and its machine check
  errors during kernel transition, until new handler for new kernel is
  registered with new resources ready for handling the MCA.

How to reproduce:

  Assert MCA while kdump kernel is booting, before new MCA handler for
  kdump kernel is registered.

Expected(Desirable) results:

  No recovery, cancel kdump and reboot the system.

Actual results:

  MCA handler for previous kernel is invoked on the kdump kernel.
  => panic, hang etc. (unexpected)

Proposed fix:

  To avoid entering MCA handler from early stage of new kernel,
  unregister the entry point from SAL before leave from current
  kernel.  Then SAL will make all MCAs to warmboot safely, without
  invoking OS_MCA.

Summary:

  Asserting INIT on the beginning of kdump/kexec kernel will result
  in unexpected behavior because INIT handler for previous kernel is
  invoked on new kernel.

Description:

  In panic situation, we can receive INIT while kernel transition,
  i.e. from beginning of panic to bootstrap of kdump kernel.
  Since we initialize registers on leave from current kernel, no
  longer monarch/slave handlers of current kernel in virtual mode are
  called safely.  (In fact system goes hang as far as I confirmed)

How to Reproduce:

  Start kdump
    # echo c > /proc/sysrq-trigger
  Then assert INIT while kdump kernel is booting, before new INIT
  handler for kdump kernel is registered.

Expected(Desirable) result:

  kdump kernel boots without any problem, crashdump retrieved

Actual result:

  INIT handler for previous kernel is invoked on kdump kernel
  => panic, hang etc. (unexpected)

Proposed fix:

  We can unregister these init handlers from SAL before jumping into
  new kernel, however then the INIT will fallback to default behavior,
  result in warmboot by SAL (according to the SAL specification) and
  we cannot retrieve the crashdump.

  Therefore this patch introduces a NOP init handler and register it
  to SAL before leave from current kernel, to start kdump safely by
  preventing INITs from entering virtual mode and resulting in warmboot.

  On the other hand, in case of kexec that not for kdump, it also
  has same problem with INIT while kernel transition.
  This patch handles this case differently, because for kexec
  unregistering handlers will be preferred than registering NOP
  handler, since the situation "no handlers registered" is usual
  state for kernel's entry.

4295ab3488 [IA64] kdump: Mask MCA/INIT on frozen cpus
Summary:

  INIT asserted on kdump kernel invokes INIT handler not only on a
  cpu that running on the kdump kernel, but also BSP of the panicked
  kernel, because the (badly) frozen BSP can be thawed by INIT.

Description:

  The kdump_cpu_freeze() is called on cpus except one that initiates
  panic and/or kdump, to stop/offline the cpu (on ia64, it means we
  pass control of cpus to SAL, or put them in spinloop).  Note that
  CPU0(BSP) always go to spinloop, so if panic was happened on an AP,
  there are at least 2cpus (= the AP and BSP) which not back to SAL.

  On the spinning cpus, interrupts are disabled (rsm psr.i), but INIT
  is still interruptible because psr.mc for mask them is not set unless
  kdump_cpu_freeze() is not called from MCA/INIT context.

  Therefore, assume that a panic was happened on an AP, kdump was
  invoked, new INIT handlers for kdump kernel was registered and then
  an INIT is asserted.  From the viewpoint of SAL, there are 2 online
  cpus, so INIT will be delivered to both of them.  It likely means
  that not only the AP (= a cpu executing kdump) enters INIT handler
  which is newly registered, but also BSP (= another cpu spinning in
  panicked kernel) enters the same INIT handler.  Of course setting of
  registers in BSP are still old (for panicked kernel), so what happen
  with running handler with wrong setting will be extremely unexpected.
  I believe this is not desirable behavior.

How to Reproduce:

  Start kdump on one of APs (e.g. cpu1)
    # taskset 0x2 echo c > /proc/sysrq-trigger
  Then assert INIT after kdump kernel is booted, after new INIT handler
  for kdump kernel is registered.

Expected results:

  An INIT handler is invoked only on the AP.

Actual results:

  An INIT handler is invoked on the AP and BSP.

Sample of results:

  I got following console log by asserting INIT after prompt "root:/>".
  It seems that two monarchs appeared by one INIT, and one panicked at
  last.  And it also seems that the panicked one supposed there were
  4 online cpus and no one did rendezvous:

  To avoid this problem, this patch inserts ia64_set_psr_mc() to mask
  INIT on cpus going to be frozen.  This masking have no effect if the
  kdump_cpu_freeze() is called from INIT handler when kdump_on_init == 1,
  because psr.mc is already turned on to 1 before entering OS_INIT.
  I confirmed that weird log like above are disappeared after applying
  this patch.

ee18d64c1f KEYS: Add a keyctl to install a process's session keyring on its parent [try #6]
Add a keyctl to install a process's session keyring onto its parent.  This
replaces the parent's session keyring.  Because the COW credential code does
not permit one process to change another process's credentials directly, the
change is deferred until userspace next starts executing again.  Normally this
will be after a wait*() syscall.

To support this, three new security hooks have been provided:
cred_alloc_blank() to allocate unset security creds, cred_transfer() to fill in
the blank security creds and key_session_to_parent() - which asks the LSM if
the process may replace its parent's session keyring.

The replacement may only happen if the process has the same ownership details
as its parent, and the process has LINK permission on the session keyring, and
the session keyring is owned by the process, and the LSM permits it.

Note that this requires alteration to each architecture's notify_resume path.
This has been done for all arches barring blackfin, m68k* and xtensa, all of
which need assembly alteration to support TIF_NOTIFY_RESUME.  This allows the
replacement to be performed at the point the parent process resumes userspace
execution.

This allows the userspace AFS pioctl emulation to fully emulate newpag() and
the VIOCSETTOK and VIOCSETTOK2 pioctls, all of which require the ability to
alter the parent process's PAG membership.  However, since kAFS doesn't use
PAGs per se, but rather dumps the keys into the session keyring, the session
keyring of the parent must be replaced if, for example, VIOCSETTOK is passed
the newpag flag.

This can be tested with the following program:

ba6c548701 ia64: IOMMU passthrough mode shouldn't trigger swiotlb init
Since commit 19943b0e30 ('intel-iommu:
Unify hardware and software passthrough support'), hardware passthrough
mode will do the same as software passthrough mode was doing -- it'll
still use the IOMMU normally for devices which can't address all of
memory. This means that we don't need to bother with swiotlb.

dma_ops->dma_supported = iommu_dma_supported;

This dma_ops->dma_supported is first called in platform_dma_init() during kernel
boot. Then dma_ops->dma_supported will be called recursively in
iommu_dma_supported.

Kernel can not boot because kernel can not get out of iommu_dma_supported until
it runs out of stack memory.

023bf6f1b8 linker script: unify usage of discard definition
Discarded sections in different archs share some commonality but have
considerable differences.  This led to linker script for each arch
implementing its own /DISCARD/ definition, which makes maintaining
tedious and adding new entries error-prone.

This patch makes all linker scripts to move discard definitions to the
end of the linker script and use the common DISCARDS macro.  As ld
uses the first matching section definition, archs can include default
discarded sections by including them earlier in the linker script.

ia64 is notable because it first throws away some ia64 specific
subsections and then include the rest of the sections into the final
image, so those sections must be discarded before the inclusion.

defconfig compile tested for x86, x86-64, powerpc, powerpc64, ia64,
alpha, sparc, sparc64 and s390.  Michal Simek tested microblaze.

fa276f36f3 [IA64] address compiler warnings perfmon.c/salinfo.c
perfmon.c has a dubious cast directly from "int" to "void *". Add
an intermediate cast to "long" to keep gcc happy.

salinfo.c uses "down_trylock()" in a highly creative way (explained
in the comments in the file) ... but it does kick out this warning:

 arch/ia64/kernel/salinfo.c:195: warning: ignoring return value of 'down_trylock'

which people occasionally try to "fix" in ways that do not work. Use some
casts to keep gcc quiet.

b9bf3121af percpu: use DEFINE_PER_CPU_SHARED_ALIGNED()
There are a few places where ___cacheline_aligned* is used with
DEFINE_PER_CPU().  Use DEFINE_PER_CPU_SHARED_ALIGNED() instead.

DEFINE_PER_CPU_SHARED_ALIGNED() applies alignment only on SMPs.  While
all other converted places used _in_smp variant or only get compiled
for SMP, net/rds used unconditional ____cacheline_aligned.  I don't
see any reason these data structures should be aligned on UP and thus
converted together.

204fba4aa3 percpu: cleanup percpu array definitions
Currently, the following three different ways to define percpu arrays
are in use.

1. DEFINE_PER_CPU(elem_type[array_len], array_name);
2. DEFINE_PER_CPU(elem_type, array_name[array_len]);
3. DEFINE_PER_CPU(elem_type, array_name)[array_len];

Unify to #1 which correctly separates the roles of the two parameters
and thus allows more flexibility in the way percpu variables are
defined.

405d967dc7 linker script: throw away .discard section
x86 throws away .discard section but no other archs do.  Also,
.discard is not thrown away while linking modules.  Make every arch
and module linking throw it away.  This will be used to define dummy
variables for percpu declarations and definitions.

This patch is based on Ivan Kokshaysky's alpha percpu patch.

7b768f07dc ACPI: pdc init related memory leak with physical CPU hotplug
arch_acpi_processor_cleanup_pdc() in x86 and ia64 results in memory allocated
for _PDC objects that is never freed and will cause memory leak in case of
physical CPU remove and add. Patch fixes the memory leak by freeing the
objects soon after _PDC is evaluated.

e088a4ad7f [IA64] Convert ia64 to use int-ll64.h
It is generally agreed that it would be beneficial for u64 to be an
unsigned long long on all architectures.  ia64 (in common with several
other 64-bit architectures) currently uses unsigned long.  Migrating
piecemeal is too painful; this giant patch fixes all compilation warnings
and errors that come as a result of switching to use int-ll64.h.

Note that userspace will still see __u64 defined as unsigned long.  This
is important as it affects C++ name mangling.

[Updated by Tony Luck to change efi.h:efi_freemem_callback_t to use
 u64 for start/end rather than unsigned long]

8b0b1db013 remove put_cpu_no_resched()
put_cpu_no_resched() is an optimization of put_cpu() which unfortunately
can cause high latencies.

The nfs iostats code uses put_cpu_no_resched() in a code sequence where a
reschedule request caused by an interrupt between the get_cpu() and the
put_cpu_no_resched() can delay the reschedule for at least HZ.

The other users of put_cpu_no_resched() optimize correctly in interrupt
code, but there is no real harm in using the put_cpu() function which is
an alias for preempt_enable().  The extra check of the preemmpt count is
not as critical as the potential source of missing a reschedule.

Debugged in the preempt-rt tree and verified in mainline.

Impact: remove a high latency source

6484eb3e2a page allocator: do not check NUMA node ID when the caller knows the node is valid
Callers of alloc_pages_node() can optionally specify -1 as a node to mean
"allocate from the current node".  However, a number of the callers in
fast paths know for a fact their node is valid.  To avoid a comparison and
branch, this patch adds alloc_pages_exact_node() that only checks the nid
with VM_BUG_ON().  Callers that know their node is valid are then
converted.

32f8840064 KVM: use smp_send_reschedule in kvm_vcpu_kick
KVM uses a function call IPI to cause the exit of a guest running on a
physical cpu. For virtual interrupt notification there is no need to
wait on IPI receival, or to execute any function.

This is exactly what the reschedule IPI does, without the overhead
of function IPI. So use it instead of smp_call_function_single in
kvm_vcpu_kick.

Also change the "guest_mode" variable to a bit in vcpu->requests, and
use that to collapse multiple IPI's that would be issued between the
first one and zeroing of guest mode.

This allows kvm_vcpu_kick to called with interrupts disabled.

4ed0d3e6c6 Intel IOMMU Pass Through Support
The patch adds kernel parameter intel_iommu=pt to set up pass through
mode in context mapping entry. This disables DMAR in linux kernel; but
KVM still runs on VT-d and interrupt remapping still works.

In this mode, kernel uses swiotlb for DMA API functions but other VT-d
functionalities are enabled for KVM. KVM always uses multi level
translation page table in VT-d. By default, pass though mode is disabled
in kernel.

This is useful when people don't want to enable VT-d DMAR in kernel but
still want to use KVM and interrupt remapping for reasons like DMAR
performance concern or debug purpose.

edb91dc01a [IA64] smp_flush_tlb_mm() should only send IPI's to cpus in cpu_vm_mask
Having flush_tlb_mm->smp_flush_tlb_mm() send an IPI to every cpu
on the system is occasionally triggering spin_lock contention in
generic_smp_call_function_interrupt().

Follow x86 arch's lead and only sends IPIs to the cpus in mm->cpu_vm_mask.

Experiments with this change have shown significant improvement in this
contention issue.

99b7623380 proc 2/2: remove struct proc_dir_entry::owner
Setting ->owner as done currently (pde->owner = THIS_MODULE) is racy
as correctly noted at bug #12454. Someone can lookup entry with NULL
->owner, thus not pinning enything, and release it later resulting
in module refcount underflow.

We can keep ->owner and supply it at registration time like ->proc_fops
and ->data.

But this leaves ->owner as easy-manipulative field (just one C assignment)
and somebody will forget to unpin previous/pin current module when
switching ->owner. ->proc_fops is declared as "const" which should give
some thoughts.

->read_proc/->write_proc were just fixed to not require ->owner for
protection.

rmmod'ed directories will be empty and return "." and ".." -- no harm.
And directories with tricky enough readdir and lookup shouldn't be modular.
We definitely don't want such modular code.

Removing ->owner will also make PDE smaller.

So, let's nuke it.

Kudos to Jeff Layton for reminding about this, let's say, oversight.

53129c5c55 ia64/pv_ops: move down __kernel_syscall_via_epc.
Move down __kernel_syscall_via_epc to the end of the page.
We want to paravirtualize only __kernel_syscall_via_epc because
it includes privileged instructions. Its paravirtualization increases
its symbols size.

On the other hand, each paravirtualized gate must have e symbols of
same value and size to native's because the page is mapped to GATE_ADDR
and GATE_ADDR + PERCPU_PAGE_SIZE and vmlinux is linked to those symbols.
Later to have the same symbol size, we pads NOPs at the end of
__kernel_syscall_via_epc. Move it after other functions to keep
symbols of other functions have same values and sizes.

5dd3c9949a cpumask: prepare for iterators to only go to nr_cpu_ids/nr_cpumask_bits.: ia64
Impact: cleanup, futureproof

In fact, all cpumask ops will only be valid (in general) for bit
numbers < nr_cpu_ids.  So use that instead of NR_CPUS in various
places.

This is always safe: no cpu number can be >= nr_cpu_ids, and
nr_cpu_ids is initialized to NR_CPUS at boot.

19390c4d03 linker script: define __per_cpu_load on all SMP capable archs
Impact: __per_cpu_load available on all SMP capable archs

Percpu now requires three symbols to be defined - __per_cpu_load,
__per_cpu_start and __per_cpu_end.  There were three archs which
didn't have it.  Update them as follows.

* powerpc: can use generic PERCPU() macro.  Compile tested for
  powerpc32, compile/boot tested for powerpc64.

* ia64: can use generic PERCPU_VADDR() macro.  __phys_per_cpu_start is
  identical to __per_cpu_load.  Compile tested and symbol table looks
  identical after the change except for the additional __per_cpu_load.

* arm: added explicit __per_cpu_load definition.  Currently uses
  unified .init output section so can't use the generic macro.  Dunno
  whether the unified .init ouput section is required by arch
  peculiarity so I left it alone.  Please break it up and use PERCPU()
  if possible.
