This change adds full pathname support for external names that have been
resolved internally by the inclusion of additional ACPI tables (via the
iASL -e option). Without this change, the disassembler can emit multiple
externals for the same object, or it become confused when the Scope()
operator is used on an external object.
Linux kernel behaviour is not affected as the structure changes and the
new invocations are only used by compiler and disassembler which are
not shipped in the kernel currently.

After many years, different formatting for switch() has crept in.
This change makes every switch block identical. Chao Guan.

This is a cosmetic patch only. Comparison of the resulting binary showed
only line number differences.
This patch does not affect the generation of the Linux binary.
This patch decreases 170 lines of 20121018 divergence.diff.
This patch updates ACPICA codes surrounded by some disabled build options
so that the source code diff between Linux and ACPICA can be reduced.
Some of these build options may never be used in the kernel, so they may
be deleted entirely in future patches.

Fixes a problem where mutex errors can occur when running a _REG
method that is in the same scope as a method-defined operation
region or an operation region under a module-level IF block.
This is rare, so the problem has not been seen before.

Add limited support for executable AML code that exists outside
of any control method. This type of code has been illegal since
ACPI 2.0. The code must exist in an If/Else/While block. All AML
tables are supported, including tables that are dynamically loaded.
