6c17812d62 NET: mdio-octeon: Enable the hardware before using it.
In some cases the mdio bus is not enabled at the time of probing.
This prevents anything from working, so we will enable it before
trying to use it, and disable it when the driver is removed.

8626d3b432 phylib: Support phy module autoloading
We don't use the normal hotplug mechanism because it doesn't work. It will
load the module some time after the device appears, but that's not good
enough for us -- we need the driver loaded _immediately_ because otherwise
the NIC driver may just abort and then the phy 'device' goes away.

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

abf35df215 NET: Support clause 45 MDIO commands at the MDIO bus level
IEEE 802.3ae clause 45 specifies a somewhat modified MDIO protocol
for use by 10GIGE phys. The main change is a 21 bit address split into
a 5 bit device ID and a 16 bit register offset. The definition is designed
so that normal and extended devices can run on the same MDIO bus.

Extend mdio-bitbang to do the new protocol. At the MDIO bus level the
protocol is requested by or'ing MII_ADDR_C45 into the register offset.

Make phy_read/phy_write/etc pass a full 32 bit register offset.

This does not attempt to make the phy layer support C45 style PHYs, just
to provide the MDIO bus support.

Tested against a Broadcom 10GE phy with ID 0x206034, and several
Broadcom 10/100/1000 Phys in normal mode.

The oops pops up because TI DaVinci EMAC driver detaches PHY on
suspend and attaches it back on resume. Attaching makes phylib call
phy_start_machine() that initializes a workqueue. On the other hand,
PHY's resume routine will call phy_start_machine() again, and that
will cause the oops since we just destroyed the already scheduled
workqueue.

This patch fixes the issue by moving workqueue initialization to
phy_device_create().

p.s. We don't see this oops with ucc_geth and gianfar drivers because
they perform a fine-grained suspend, i.e. they just stop the PHYs
without detaching.

698244ace8 phy: SMSC device Energy Detect power-down mode
SMSC Ethernet Transceivers (LAN88710, LAN8710, LAN8720, LAN8187,
LAN8700, LAN83C185) provide a mechanism to conserve power when
the device is not connected to an active link partner
(Energy Detect Mode).
So this patch enables the Energy Detect power-down mode
for these Transceivers.

2f5cb43406 phylib: Properly reinitialize PHYs after hibernation
Since hibernation assumes power loss, we should fully reinitialize
PHYs (including platform fixups), as if PHYs were just attached.

This patch factors phy_init_hw() out of phy_attach_direct(), then
converts mdio_bus to dev_pm_ops and adds an appropriate restore()
callback.

Here is why.

On suspend:

- PM core starts suspending devices, ucc_geth_suspend gets called;

- ucc_geth calls phy_stop() on suspend. Note that phy_stop() is
  mostly asynchronous so it doesn't block ucc_geth's suspend routine,
  it just sets PHY_HALTED state and disables PHY's interrupts;

- Suddenly the state machine gets scheduled, it grabs the phydev->lock
  mutex and tries to process the PHY_HALTED state, so it calls
  phydev->adjust_link(phydev->attached_dev). In ucc_geth case
  adjust_link() calls msleep(), which reschedules the code flow back to
  PM core, which now finishes suspend and so we end up sleeping with
  phydev->lock mutex held.

On resume:

- PM core starts resuming devices (notice that nobody rescheduled
  the state machine yet, so the mutex is still held), the core calls
  ucc_geth's resume routine;

- ucc_geth_resume restarts the PHY with phy_stop()/phy_start()
  sequence, and the phy_*() calls are trying to grab the phydev->lock
  mutex. Here comes the deadlock.

This patch fixes the issue by stopping the state machine on suspend
and starting it again on resume.

25d967b72a NET: Add driver for Octeon MDIO buses.
The Octeon SOC has two types of Ethernet ports, each type with its own
driver.  However, the PHYs for all the ports are controlled by a
common MDIO bus.  Because the mdio driver is not associated with a
particular driver, but is instead a system level resource, we create s
stand-alone driver for it.

As for the driver, we put the register definitions in
arch/mips/include/asm/octeon where most of the other Octeon register
definitions live.  This is a platform driver with the platform device
for "mdio-octeon" being registered in the platform startup code.

f99b4a0284 net: fix mdio section mismatch warning
This fixes the following warning:

WARNING: drivers/net/phy/built-in.o(.devexit.text+0x70): Section mismatch in reference from the function .mdio_gpio_bus_destroy() to the function .devinit.text:.mdio_gpio_bus_deinit()
The function __devexit .mdio_gpio_bus_destroy() references
a function __devinit .mdio_gpio_bus_deinit().
This is often seen when error handling in the exit function
uses functionality in the init path.
The fix is often to remove the __devinit annotation of
.mdio_gpio_bus_deinit() so it may be used outside an init section.

32e5a8d651 tg3 / broadcom: Add code to disable rxc refclk
The 5785 does not use the RXC reference clock.  Turning it off is
desirable as it saves power.

By default, the 50610 enables the RXC reference clock and the 50610M
disables it.  Presumably this is one of the reasons why the hardware
architect chose one over the other.

Adding a "rx reference clock disable" flag is not the ideal way to
describe the option, as it would force the MAC using a 50610M to set
the flag.  Ideally we want the flags to represent opt-in behavior that
deviates from hardware defaults.  Furthermore, the lack of a
"disable" flag implies that the requester wants the rx reference clock
enabled, which doesn't necessarily follow.

By presenting the option as a passive statement (rx reference clock
unused) rather than a command, I hope to convey an opt-in option to
disable the rx reference clock that falls back to hardware defaults if
not set.  A secondary benefit of this is that it keeps the
intelligence about phy defaults in the broadcom module where it belongs
and allows the broadcom module more latitude should a bug arise.

Auto power-down (APD) support is a power-saving feature.  It should be
selectively enabled since it might expose MAC bugs.  This patch changes
the code to enable APD only if the PHY_BRCM_AUTO_PWRDWN_ENABLE flag is
set.  The tg3 driver was changed to set this bit.

63a14ce449 tg3 / broadcom: Add PHY_BRCM_CLEAR_RGMII_MODE flag
Broadcom 50610M parts changed the default definitions of the RGMII mode
shadow register.  The 5785 needs the RGMII mode selection bits [4:3]
cleared.

The default value of the remaining bits in this register are zero.
Rather than unnecessarily burn an extra bit in the dev_flags member in
an attempt to enumerate all possible combinations, this patch take a
more course grained approach and labels the option as "clear all bits".

42caa07404 phylib: fixes for PHY_RESUMING state changes
The PHY_HALTED state disables phydev->link, but the link will not be
updated upon entering PHY_RESUMING.  Add a call to phy_read_status() to
update the link before entering PHY_RUNNING.  If the link is not up at
this point, enter the PHY_NOLINK state instead.

Also, when transitioning from PHY_RESUMING to PHY_RUNNING, calls to
netif_carrier_on() and phydev->adjust_link() are missing.  Add the calls
similar to the other transitions to PHY_RUNNING.

drivers can use a pointer to the phy_device instead of trying to determine

4dea547fef phylib: rework to prepare for OF registration of PHYs
This patch makes changes in preparation for supporting open firmware
device tree descriptions of MDIO busses.  Changes include:
- Cleanup handling of phy_map[] entries; they are already NULLed when
  registering and so don't need to be re-cleared, and it is good practice
  to clear them out when unregistering.
- Split phy_device registration out into a new function so that the
  OF helpers can do two stage registration (separate allocation and
  registration steps).

Marvell 88E1121R Dual PHY device can be hardware-configured
to use shared interrupt pin for both PHY ports. For such
PHY configurations using shared PHY interrupt phy_interrupt()
handler will also schedule a work for PHY port which didn't
cause an interrupt.

This patch adds a possibility for PHY drivers to provide
did_interrupt() function which reports if the PHY (or a PHY
port in a multi-PHY device) generated an interrupt. This
function is called in phy_change() as phy_change() shouldn't
proceed if it is invoked for a PHY which didn't cause an
interrupt. So check for interrupt originator in phy_change()
to allow early-out.

a390d1f379 phylib: convert state_queue work to delayed_work
It closes a race in phy_stop_machine when reprogramming of phy_timer
(from phy_state_machine) happens between del_timer_sync and cancel_work_sync.

Without this change it could lead to crash if phy_device would be freed after
phy_stop_machine (timer would fire and schedule freed work).

3d1e4db2b0 phylib: Rework suspend/resume code to check netdev wakeup capability
In most cases (e.g. PCI drivers) MDIO and MAC controllers are
represented by the same device. But for SOC ethernets we have
separate devices. So, in SOC case, checking whether MDIO
controller may wakeup is not only makes little sense, but also
prevents us from doing per-netdevice wakeup management.

This patch reworks suspend/resume code so that now it checks
for net device's wakeup flags, not MDIO controller's ones.

Each netdevice should manage its wakeup flags, and phylib will
decide whether suspend an attached PHY or not.

    PHYID returns 0xffff and not 0xffffffff when not found and in some
    case(at91sam9263) 0x0. Maybe this patch could be useful.

phy_device.c treats PHY ID == 0x0 as bogus IDs, and that results in
gianfar driver failure to see the TBI PHYs. This code snippet triggers:

Although tbi-handle is specified in the device tree.

f162e97d77 phylib: Remove unnecessary "reset" fixups in genphy_setup_forced
genphy_setup_forced hasn't actually reset the PHY for a long time,
but a comment to that effect remained in the code, so code continued
to act as if it *had* reset the PHY, and called the necessary fixup
functions to respond to a PHY reset.  With no reset, those functions
are no longer needed, so we remove them.

0f0ca340e5 phy: power management support
This patch adds the power management support into the physical
abstraction layer.

Suspend and resume functions respectively turns on/off the bit 11
into the PHY Basic mode control register.
Generic PHY device starts supporting PM.

In order to support the wake-on LAN and avoid to put in power down
the PHY device, the MDIO is aware of what the Ethernet device wants to do.

Voluntary, no CONFIG_PM defines were added into the sources.
Also generic suspend/resume functions are exported to allow
other drivers use them (such as genphy_config_aneg etc.).

Within the phy_driver_register function, we need to remove the
memset. It overrides the device driver owner and it is not good.

11c6dd2c72 phylib: Add Vitesse VSC8221 SGMII PHY
PHY is mostly compatible with the existing VSC8244 PHY.  The init sequence
is different and the interrupt mask lacks some bits present in the VSC8244.

Rather than making a copy of the existing VSC234x config_intr function and
change one constant, I modify it to select the interrupt mask based on

de339c2aa7 phylib: Fix auto-negotiation restart avoidance
A previous patch, 51e2a3846e, made
genphy_config_aneg() not restart aneg by calling genphy_restart_aneg() if
the advertisement hadn't changed.

But, genphy_restart_aneg() doesn't just restart aneg, it may also *enable*
aneg or un-isolate the PHY from the MII (those functions are controlled by
the same register).  The code to avoid calling genphy_restart_aneg() didn't
consider this.

So, modify genphy_config_aneg() to also check if the PHY needs to have aneg
enabled or be un-isolated before deciding not to restart aneg.

This caused a problem with certain Davicom PHYs, as that driver isolates
the PHY (why?) before calling genphy_config_aneg() and expects the PHY to
be un-isolated by that function.

3e44017b58 phylib: fix premature freeing of struct mii_bus
Commit 46abc02175 ("phylib: give mdio
buses a device tree presence") added a call to device_unregister() in
a situation where the caller did not intend for the device to be
freed yet, but apart from just unregistering the device from the
system, device_unregister() does an additional put_device() that is
intended to free it.

The right function to use in this situation is device_del(), which
unregisters the device from the system like device_unregister() does,
but without dropping the reference count an additional time.

fd9abb3d97 SMSC LAN911x and LAN921x vendor driver
Attached is a driver for SMSC's LAN911x and LAN921x families of embedded
ethernet controllers.

There is an existing smc911x driver in the tree; this is intended to
replace it.  Dustin McIntire (the author of the smc911x driver) has
expressed his support for switching to this driver.

This driver contains workarounds for all known hardware issues, and has
been tested on all flavours of the chip on multiple architectures.

This driver now uses phylib, so this patch also adds support for the
device's internal phy

1. arch/powerpc/platforms/pasemi/gpio_mdio.c also needs to be
   converted over to mdiobus_{alloc,free}().

2. drivers/net/phy/fixed.c used to embed a struct mii_bus into its
   struct fixed_mdio_bus and then use container_of() to go from the
   former to the latter.  Since mii bus structures are no longer
   embedded, we need to do something like use the mii bus private
   pointer to go from mii_bus to fixed_mdio_bus instead.

51e2a3846e PHY: Avoid unnecessary aneg restarts
The PHY's aneg is configured and restarted whenever the link is brought up,
e.g. when DHCP is started after the kernel has booted.  This can take the
link down for several seconds while auto-negotiation is redone.

If the advertised features haven't changed, then it shouldn't be necessary
to bring down the link and start auto-negotiation over again.

genphy_config_advert() is enhanced to return 0 when the advertised features
haven't been changed and >0 when they have been.

genphy_config_aneg() then uses this information to not call
genphy_restart_aneg() if there has been no change.

Sometimes the specific interaction between the platform and the PHY
requires special handling.  For instance, to change where the PHY's
clock input is, or to add a delay to account for latency issues in the
data path.  We add a mechanism for registering a callback with the PHY
Lib to be called on matching PHYs when they are brought up, or reset.

57bb7e2228 phy/broadcom: add support for BCM5481 PHY
This patch adds support for BCM5481 PHY. Unfortunately it's hard to
get specifications for this PHY, so its special register 0x18 isn't
annotated properly (but we know it's used to set up the delays).

I've kept the magic numbers, so we'll not forget to fix it at the
first opportunity, and will name that register and its bits correctly.

Marvell PHY m88e1111 (not sure about other models, but think they too)
works in two modes: fiber and copper. In Marvell PHY driver (that we
have in current community kernels) code supported only copper mode,
and this is not configurable, bits for copper mode are simply written
in registers during PHY initialization.

This patch adds support for both modes.

35b5f6b1a8 PHYLIB: Locking fixes for PHY I/O potentially sleeping
PHY read/write functions can potentially sleep (e.g., a PHY accessed
via I2C).  The following changes were made to account for this:

    * Change spin locks to mutex locks
    * Add a BUG_ON() to phy_read() phy_write() to warn against
      calling them from an interrupt context.
    * Use work queue for PHY state machine handling since
      it can potentially sleep
    * Change phydev lock from spinlock to mutex

a79d8e93d3 phy/fixed.c: rework to not duplicate PHY layer functionality
With that patch fixed.c now fully emulates MDIO bus, thus no need
to duplicate PHY layer functionality. That, in turn, drastically
simplifies the code, and drops down line count.

As an additional bonus, now there is no need to register MDIO bus
for each PHY, all emulated PHYs placed on the platform fixed MDIO bus.
There is also no more need to pre-allocate PHYs via .config option,
this is all now handled dynamically.

Though actually there is nothing broken, it just device

6daf653103 PHYLIB: fix an interrupt loop potential when halting
Ensure the PHY_HALTED state is not entered with the IRQ asserted as it
could lead to an interrupt loop.

There is a small window in phy_stop(), where the state of the PHY machine
indicates it has been halted, but its interrupt output might still be
unmasked.  If an interrupt goes active right at this moment it will loop as
the phy_interrupt() handler exits immediately with IRQ_NONE if the halted
state is seen.  It is unsafe to extend the phydev spinlock to cover
phy_interrupt().  It is safe to swap the order of the actions though as all
the competing places to unmask the interrupt output of the PHY, which are
phy_change() and phy_timer() are already covered with the lock as is the
sequence in question.

0ac4952731 PHYLIB: IRQ event workqueue handling fixes
Keep track of disable_irq_nosync() invocations and call enable_irq() the
right number of times if work has been cancelled that would include them.

Now that the call to flush_work_keventd() (problematic because of
rtnl_mutex being held) has been replaced by cancel_work_sync() another
issue has arisen and been left unresolved.  As the MDIO bus cannot be
accessed from the interrupt context the PHY interrupt handler uses
disable_irq_nosync() to prevent from looping and schedules some work to be
done as a softirq, which, apart from handling the state change of the
originating PHY, is responsible for reenabling the interrupt.  Now if the
interrupt line is shared by another device and a call to the softirq
handler has been cancelled, that call to enable_irq() never happens and the
other device cannot use its interrupt anymore as its stuck disabled.

I decided to use a counter rather than a flag because there may be more
than one call to phy_change() cancelled in the queue -- a real one and a
fake one triggered by free_irq() if DEBUG_SHIRQ is used, if nothing else.
Therefore because of its nesting property enable_irq() has to be called the
right number of times to match the number disable_irq_nosync() was called
and restore the original state.  This DEBUG_SHIRQ feature is also the
reason why free_irq() has to be called before cancel_work_sync().

While at it I updated the comment about phy_stop_interrupts() being called
from `keventd' -- this is no longer relevant as the use of
cancel_work_sync() makes such an approach unnecessary.  OTOH a similar
comment referring to flush_scheduled_work() in phy_stop() still applies as
using cancel_work_sync() there would be dangerous.

Checked with checkpatch.pl and at the run time (with and without
DEBUG_SHIRQ).

9ff8c68b3c PHYLIB: Spinlock fixes for softirqs
Use spin_lock_bh()/spin_unlock_bh() for the phydev lock throughout as it
is used in phy_timer() that is called as a softirq and all the other
operations may happen in the user context.

There has been a change recently that did such a conversion for some of the
operations on the lock, but some have been left intact.  Many of them,
perhaps all, may be called in the user context and I was able to trigger
recursive spinlock acquisition indeed, so I think for the sake of long-term
maintenance it is best to convert them all, even if unnecessarily for one
or two -- better safe than sorry.

Perhaps one in phy_timer() could actually be skipped as only called as a
softirq -- I can send an update if that sounds like a good idea.

Checked with checkpatch.pl and at the runtime.
