When GDB writes a breakpoint into address area of inferior process the
kernel needs to invalidate the modified memory in the inferior which
is done by calling flush_cache_page which in turns calls
r4k_flush_cache_page and local_r4k_flush_cache_page for VSMP or SMTC
kernel via r4k_on_each_cpu().
As the VSMP and SMTC SMP kernels for 34K are running on a single shared
caches it is possible to get away without interprocessor function calls.
This optimization is implemented in r4k_on_each_cpu, so
local_r4k_flush_cache_page is only ever called on the local CPU.
This is where the following code in local_r4k_flush_cache_page() strikes:

On VSMP and SMTC had a function of cpu_context() for each CPU(TC).
So in case another CPU than the CPU executing local_r4k_cache_flush_page
has not accessed the mm but one of the other CPUs has there may be data
to be flushed in the cache yet local_r4k_cache_flush_page will falsely
return leaving the I-cache inconsistent for the breakpoint.
While the issue was discovered with GDB it also exists in
local_r4k_flush_cache_range() and local_r4k_flush_cache().
Fixed by introducing a new function has_valid_asid which on MT kernels
returns true if a mm is active on any processor in the system.
This is relativly expensive since for memory acccesses in that loop
cache misses have to be assumed but it seems the most viable solution
for 2.6.23 and older -stable kernels.

Contrary to the belief of some, the R3000 and related processors did have
caches, both a data and an instruction cache. Here is an implementation
of r3k_flush_cache_page(), which is the processor-specific back-end for
flush_cache_range(), done according to the spec in
Documentation/cachetlb.txt.
While at it, remove an unused local function: get_phys_page(), do some
trivial formatting fixes and modernise debugging facilities.

Mmap with MAP_FIXED was not validating the addr and len parameters. This
leads to the failure of GCC's gcc.c-torture/execute/loop-2[fg].c testcases
when using the o32 ABI on a 64 bit kernel.
These testcases try to mmap 65536 bytes at 0x7fff8000 and then access all
the memory. In 2.6.18 and 2.6.23.1 (and likely other versions as well)
the kernel maps the requested memory, but since half of it is above
0x80000000 a SIGBUS is generated when it is accessed.
This patch moves the len validation above the MAP_FIXED processing so that
it is always validated. It also adds validation to the addr parameter for
MAP_FIXED mappings.

They break the timer interrupt initialization and only seem to be a kludge
for initialization happening in the wrong order. Further testing done by
Thiemo confirms the suspicion that the other invocations also seem to have
useless.

Convert jmr3927_clock_event_device to more generic
txx9tmr_clock_event_device which supports one-shot mode. The
txx9tmr_clock_event_device can be used for TX49 too if the cp0 timer
interrupt was not available.
Convert jmr3927_hpt_read to txx9_clocksource driver which does not
depends jiffies anymore. The txx9_clocksource itself can be used for
TX49, but normally TX49 uses higher precision clocksource_mips.

Modify the SMTC initialization code to allow boot-time specification not
only of how many VPEs and TCs to use, but also how many TCs out of the
allowed pool are to be bound to VPE 0. The new boot option is "vpe0tcs=N",
where N is an integer. Using it in combination with the existing options
allows arbitrary assignments across the 2 VPEs of a 34K. e.g. "maxtcs=3
vpe0tcs=1" forces VPE0 to have 1 TC, while VPE1 has 2, and "maxtcs=4
vpe0tcs=3" forces VPE0 to have 3 TCs, while VPE1 gets 1. If no vpe0tcs
option is specified, the traditional algorithm of evenly dividing TCs
between available VPEs, with the odd "slop" going to VPE0, is retained.
The reason for doing this is to allow a finer balancing of TCs which can
handle I/O interrupts on Malta (those on VPE 0) and those which cannot.

The BCM148 has 4 cores but there are also just 4 generic timers available
so use the ZBbus cycle counter instead of it. In addition the ZBbus
counter also offers a much higher resolution and 64-bit counting so I'm
considering a later complete conversion to it once I figure out if all
members of the Sibyte SOC family support it - the docs seem to agree but
the headers files seem to disagree ...

One of the easiest things to isolate is the pid printed in kernel log.
There was a patch, that made this for arch-independent code, this one makes
so for arch/xxx files.
It took some time to cross-compile it, but hopefully these are all the
printks in arch code.

is_init() is an ambiguous name for the pid==1 check. Split it into
is_global_init() and is_container_init().
A cgroup init has it's tsk->pid == 1.
A global init also has it's tsk->pid == 1 and it's active pid namespace
is the init_pid_ns. But rather than check the active pid namespace,
compare the task structure with 'init_pid_ns.child_reaper', which is
initialized during boot to the /sbin/init process and never changes.

The set of functions process_session, task_session, process_group and
task_pgrp is confusing, as the names can be mixed with each other when looking
at the code for a long time.
The proposals are to
* equip the functions that return the integer with _nr suffix to
represent that fact,
* and to make all functions work with task (not process) by making
the common prefix of the same name.
For monotony the routines signal_session() and set_signal_session() are
replaced with task_session_nr() and set_task_session(), especially since they
are only used with the explicit task->signal dereference.

cycles_per_jiffy was only ever getting assigned and the function pointer
not being called anymore and mips_timer_ack had gotten similarly stale. I
leave the remaining assignments unfixed as a lighthouse pointing platform
maintainers to what needs a rewrite. These changes make null_timer_ack()
unreferenced, so delete that too.

Some processors offer the option of using the interrupt on which
normally the count / compare interrupt would be signaled as a normal
interupt pin. Previously this required some ugly hackery for each
system which is much easier done by a quick and simple probe.

Userland, including the C library and the dynamic linker, is keen to use
the SYNC instruction, even for "generic" MIPS I binaries these days.
Which makes it less than useful on MIPS I processors.
This change adds the emulation, but as our do_ri() infrastructure was not
really prepared to take yet another instruction, I have rewritten it and
its callees slightly as follows.
Now there is only a single place a possible signal is thrown from. The
place is at the end of do_ri(). The instruction word is fetched in
do_ri() and passed down to handlers. The handlers are called in sequence
and return a result that lets the caller decide upon further processing.
If the result is positive, then the handler has picked the instruction,
but a signal should be thrown and the result is the signal number. If the
result is zero, then the handler has successfully simulated the
instruction. If the result is negative, then the handler did not handle
the instruction; to make it more obvious the calls do not follow the usual
0/-Exxx result convention they now return -1 instead of -EFAULT.
The calculation of the return EPC is now at the beginning. The reason is
it is easier to handle it there as emulation callees may modify a register
and an instruction may be located in delay slot of a branch whose result
depends on the register. It has to be undone if a signal is to be raised,
but it is not a problem as this is the slow-path case, and both actions
are done in single places now rather than the former being scattered
through emulation handlers.
The part of do_cpu() being covered follows the changes to do_ri().

Remove the old-fashioned lk201 driver under drivers/tc/ that used to be
used by the old dz.c and zs.c drivers, which is now orphan code referred to
from nowhere and does not build anymore. A modern replacement is available
as drivers/input/keyboard/lkkbd.c.
There are no plans to do anything about this piece of code and it does not
fit anywhere anymore, so it is not just a matter of maintenance or the lack
of. There are still some bits that might be added to the new lkkbd.c
driver based on the old code, and the embedded hardware documentation which
is otherwise quite hard to get hold of might be useful to keep too. Both
of these can be done separately though. RIP.

dma_cache_(wback|inv|wback_inv) were the earliest attempt on a generalized
cache managment API for I/O purposes. Originally it was basically the raw
MIPS low level cache API exported to the entire world. The API has
suffered from a lack of documentation, was not very widely used unlike it's
more modern brothers and can easily be replaced by dma_cache_sync. So
remove it rsp. turn the surviving bits back into an arch private API, as
discussed on linux-arch.

For some time /proc/sys/kernel/core_pattern has been able to set its output
destination as a pipe, allowing a user space helper to receive and
intellegently process a core. This infrastructure however has some
shortcommings which can be enhanced. Specifically:
1) The coredump code in the kernel should ignore RLIMIT_CORE limitation
when core_pattern is a pipe, since file system resources are not being
consumed in this case, unless the user application wishes to save the core,
at which point the app is restricted by usual file system limits and
restrictions.
2) The core_pattern code should be able to parse and pass options to the
user space helper as an argv array. The real core limit of the uid of the
crashing proces should also be passable to the user space helper (since it
is overridden to zero when called).
3) Some miscellaneous bugs need to be cleaned up (specifically the
recognition of a recursive core dump, should the user mode helper itself
crash. Also, the core dump code in the kernel should not wait for the user
mode helper to exit, since the same context is responsible for writing to
the pipe, and a read of the pipe by the user mode helper will result in a
deadlock.
This patch:
Remove the check of RLIMIT_CORE if core_pattern is a pipe. In the event that
core_pattern is a pipe, the entire core will be fed to the user mode helper.

Switch single-linked binfmt formats list to usual list_head's. This leads
to one-liners in register_binfmt() and unregister_binfmt(). The downside
is one pointer more in struct linux_binfmt. This is not a problem, since
the set of registered binfmts on typical box is very small -- (ELF +
something distro enabled for you).
Test-booted, played with executable .txt files, modprobe/rmmod binfmt_misc.

Extremly low values are of dubious usefulness anyway but in this case
they actually were killing Qemu which simply wasn't able to complete

Turns out b868868ae0 wasn't quite right.
When called for a page that isn't marked dirty it would artificially
create an alias instead of doing the obvious thing and access the page
via KSEG0.
The same issue also exists in copy_to_user_page and copy_from_user_page
which was causing the machine to die under rare circumstances for example
when running ps if the BUG_ON() assertion added by the earlier fix was
getting triggered.

MIPSsim probably doesn't have any sort of environment, but writing
a zero in it kills even the compiled in command line. This prevents
booting via NFS root.

The rtc-ds1742 platform driver name doesn't match its module name,
which might prevents it from properly hotplugging. There is only two
in-tree user of its driver, which are fixed by this patch too.

We have had complaints where a threaded application is left in a bad state
after one of it's threads is killed when we hit a VM: out_of_memory
condition.
Killing just one of the process threads can leave the application in a bad
state, whereas killing the entire process group would allow for the
application to restart, or be otherwise handled, and makes it very obvious
that something has gone wrong.
This change allows the entire process group to be taken down, rather
than just the one thread.

pass in additional flags to gcc.
This patch replace use of AFLAGS with KBUILD_AFLAGS all over
the tree.

The variable CFLAGS is a wellknown variable and the usage by
kbuild may result in unexpected behaviour.
On top of that several people over time has asked for a way to
pass in additional flags to gcc.
This patch replace use of CFLAGS with KBUILD_CFLAGS all over the
tree and enabling one to use:
make CFLAGS=...
to specify additional gcc commandline options.
One usecase is when trying to find gcc bugs but other
use cases has been requested too.
Patch was tested on following architectures:
alpha, arm, i386, x86_64, mips, sparc, sparc64, ia64, m68k
Test was simple to do a defconfig build, apply the patch and check
that nothing got rebuild.

and my new least favourite crap, the "mod_devicetable" support in the
files include/linux/mod_devicetable.h and scripts/mod/file2alias.c.
(The latter files seem to be explicitly _designed_ to get conflicts when
different subsystems work with them - that have an absolutely horrid
lack of subsystem separation!)

Loading ELF binaries based on the section table is totally wrong. This
still leaves the other fat bug of referencing symbols in an executable
unfixed, so people better don't run strip on their binaries ...
As added bonus the new loader is also 23 lines shorter.

affect since blasting away cachelines may or may not do the right
thing wrt. cache coherency.

-msym32 and previously the strategy to tell the compiler to generate
64-bit code but the assembler to put it into 32-bit ELF was initially
a hack to get around the lack of proper 64-bit binutils support and
later turned into a neat optimization with significant code size
savings. But it's really just an optimization so there is nothing
wrong with just dropping the option (and whatever else goes along with
it, I forgot all the nasty details) on the floor if due to a vintage
compiler it can't be suported.

The kernel currently only supports broadcasting of the timer interrupt
from a single timer, not multicasting into two multicast groups of
processors. So the implemented mechanism for SMTC works by broadcasting
the cp0 compare interrupt on VPE 0 and ignoring it on any additional VPEs.

This replaces the MIPS-specific to_tm function with the generic
rtc_time_to_tm function. The big difference between the two functions is
that rtc_time_to_tm uses epoch 70 while to_tm uses 1970, so the result of
rtc_time_to_tm needs to be fixed up.

It may not be perfect yet but the SB1 code is badly borken and has
horrible performance issues.
Downside: This seriously breaks support for pass 1 parts of the BCM1250
where indexed cacheops don't work quite reliable but I seem to be the
last one on the planet with a pass 1 part anyway.

This patch renames it for 3 reasons:
- "CONFIG" pattern is used by Kconfig. Now this macro is
no more defined by Kconfig but by Kbuild itself make this
clear by translating "CONFIG" into "KBUILD".
- "ELF32" word is improper because it is irrelevant to ELF
format and it makes confusion with CONFIG_BOOT_ELF32. So
translate it with SYM32.
- Add "64BIT" part to make clear that this macro implies a
64 bits kernel.

We do not rely on user anymore to setup this config correctly.
Instead we make our choice depending on the load address.
If we want to force Kbuild to use ELF64 format whatever
the load address we can still do:
$ make BUILD_ELF32=no

This patch removes '-mno-explicit-relocs' usage when
CONFIG_BUILD_ELF64 is set since this option was only required
with the old hack to truncate addresses at the assembly level
where "-mabi=64 -Wa,-mabi=32" was used.
This should yield a small code size improvement for inline
assembly, where the R constraint is used.

This patch fixes a wrong ifdef in the board setup code, leading to the GPIO
pin not being pulled high, and thus the USB switch not being powered at all.
This finishes the rename of CONFIG_USB_OHCI to CONFIG_USB_OHCI_HCD, which
started in 2005 (before 2.6.12-rc2), then probably because things were
working anyway for most people got forgotten.
[Ralf: Paolo's original patch didn't fix the module case, Florian's patch
only fixed MTX1 etc. so this is a combined patch plus some cleanups.]
