The double indirect bio leads to somewhat suboptimal code generation.
Instead return the (original or split) bio, and make sure the
request_queue arguments to the lower level helpers is passed after the
bio to avoid constant reshuffling of the argument passing registers.
Also give it and the helpers used to implement it more descriptive names.

The last traces of the IDE driver went away in commit b7fb14d3ac
("ide: remove the legacy ide driver") but it left behind some traces
of old documentation.
As luck would have it Randy and I would submit similar changes within
a week of each other to address this. As Randy's commit is in the doc
tree already - this delta is just the stuff my removal contained that
was not in Randy's IDE doc removal.

Remove the magic autofree semantics and require the callers to explicitly
call bio_init to initialize the bio.
This allows bio_free to catch accidental bio_put calls on bio_init()ed
bios as well.

Now that each scsi_request is backed by a scsi_cmnd, there is no need to
indirect the CDB storage. Change all submitters of SCSI passthrough
requests to store the CDB information directly in the scsi_cmnd, and while
doing so allocate the full 32 bytes that cover all Linux supported SCSI
hosts instead of requiring dynamic allocation for > 16 byte CDBs. On
64-bit systems this does not change the size of the scsi_cmnd at all, while
on 32-bit systems it slightly increases it for now, but that increase will
be made up by the removal of the remaining scsi_request fields.

Pass the block_device that we plan to use this bio for and the
operation to bio_reset to optimize the assigment. A NULL block_device
can be passed, both for the passthrough case on a raw request_queue and
to temporarily avoid refactoring some nasty code.

There is no need to create kobject children of the pktcdvd device just
to display a subdirectory name. Instead, use a named attribute group
which removes the extra kobjects and also fixes the userspace race where
the device is created yet tools like libudev can not see the attributes
as they think the subdirectories are some other sort of device.

The bdi congestion framework isn't widely used and should be
deprecated.
pktdvd makes use of it to track congestion, but this can be done
entirely internally to pktdvd, so it doesn't need to use the framework.
So introduce a "congested" flag. When waiting for bio_queue_size to
drop, set this flag and a var_waitqueue() to wait for it. When
bio_queue_size does drop and this flag is set, clear the flag and call
wake_up_var().
We don't use a wait_var_event macro for the waiting as we need to set
the flag and drop the spinlock before calling schedule() and while that
is possible with __wait_var_event(), result is not easy to read.

All modern drivers can support extra partitions using the extended
dev_t. In fact except for the ioctl method drivers never even see
partitions in normal operation.
So remove the GENHD_FL_EXT_DEVT and allow extra partitions for all
block devices that do support partitions, and require those that
do not support partitions to explicit disallow them using
GENHD_FL_NO_PART.

Add a new helper that calls blk_get_request and initializes the
scsi_request to avoid the indirect call through ->.initialize_rq_fn.
Note that this makes the pktcdvd driver depend on the SCSI core, but
given that only SCSI devices support SCSI passthrough requests that
is not a functional change.

Replace the blk_poll interface that requires the caller to keep a queue
and cookie from the submissions with polling based on the bio.
Polling for the bio itself leads to a few advantages:
- the cookie construction can made entirely private in blk-mq.c
- the caller does not need to remember the request_queue and cookie
separately and thus sidesteps their lifetime issues
- keeping the device and the cookie inside the bio allows to trivially
support polling BIOs remapping by stacking drivers
- a lot of code to propagate the cookie back up the submission path can
be removed entirely.

Replace the gendisk pointer in struct bio with a pointer to the newly
improved struct block device. From that the gendisk can be trivially
accessed with an extra indirection, but it also allows to directly

The make_request_fn is a little weird in that it sits directly in
struct request_queue instead of an operation vector. Replace it with
a block_device_operations method called submit_bio (which describes much
better what it does). Also remove the request_queue argument to it, as
the queue can be derived pretty trivially from the bio.

Current make_request based drivers use either blk_alloc_queue_node or
blk_alloc_queue to allocate a queue, and then set up the make_request_fn
function pointer and a few parameters using the blk_queue_make_request
helper. Simplify this by passing the make_request pointer to
blk_alloc_queue, and while at it merge the _node variant into the main
helper by always passing a node_id, and remove the superfluous gfp_mask
parameter. A lower-level __blk_alloc_queue is kept for the blk-mq case.

This is the only ioctl command that does not have a proper
compat handler. Making the normal implementation do the
right thing is actually very simply, so just do that by
using an in_compat_syscall() check to avoid the special
case in the pkcdvd driver.

There is only one implementation of this ioctl, so move the handling out
of the common block layer code into the place where it's actually needed.
It also gets called indirectly through pktcdvd, which needs to be aware
of this change.
As I noticed, the old implementation of the compat handler failed to
convert the structure on the way out, so the updated fields never got
written back to user space. This is either not important, or it has
never worked and should be fixed now.

The support for the compat ioctl did not actually do what it was
supposed to do because of a typo, instead it broke native support for
CDROM_LAST_WRITTEN and CDROM_SEND_PACKET on all architectures with
CONFIG_COMPAT enabled.

pkt_ioctl() implements the generic SCSI_IOCTL_SEND_COMMAND
and some cdrom ioctls by forwarding to the underlying block
device. For compat_ioctl handling, this always takes a
roundtrip through fs/compat_ioctl.c that we should try
to avoid, at least for the compatible commands.
CDROM_SEND_PACKET is an exception here, it requires special
translation in compat_blkdev_driver_ioctl().
CDROM_LAST_WRITTEN has no compat handling at the moment.

The async_events field, intended to be used for drivers that support
asynchronous notifications about disk events (aka media change events),
isn't currently used by any driver, and apparently that has been that
way for a long time (if not forever). Remove it.
