90ad9e2d91 x86/io_apic: Reevaluate vector configuration on activate()
With the upcoming reservation/management scheme, early activation will
assign a special vector. The final activation at request_irq() assigns a
real vector, which needs to be updated in the ioapic.

Split out the reconfiguration code in set_affinity and use it for
reactivation.

3534be05e4 x86/ioapic: Mark legacy vectors at reallocation time
When the legacy PIC vectors are taken over by the IO APIC the current
vector assignement code is tricked to reuse the vector by allocating the
apic data in the early boot process. This can be avoided by marking the
allocation as legacy PIC take over. Preparatory patch for further cleanups.

ef9e56d894 x86/ioapic: Remove obsolete post hotplug update
With single CPU affinities the post SMP boot vector update is pointless as
it will just leave the affinities on the same vectors and the same CPUs.

Remove it.

c1d1ee9ac1 x86/apic: Get rid of apic->target_cpus
The target_cpus() callback of the apic struct is not really useful. Some
APICs return cpu_online_mask and others cpus_all_mask. The latter is bogus
as it does not take holes in the cpus_possible_mask into account.

Replace it with cpus_online_mask which makes the most sense and remove the
callback.

The usage sites will be removed in a later step anyway, so get rid of it
now to have incremental changes.

42e1cc2dc5 genirq/irqdomain: Propagate early activation
Propagate the early activation mode to the irqdomain activate()
callbacks. This is required for the upcoming reservation, late vector
assignment scheme, so that the early activation call can act accordingly.

7249164346 genirq/irqdomain: Update irq_domain_ops.activate() signature
The irq_domain_ops.activate() callback has no return value and no way to
tell the function that the activation is early.

The upcoming changes to support a reservation scheme which allows to assign
interrupt vectors on x86 only when the interrupt is actually requested
requires:

  - A return value, so activation can fail at request_irq() time
  
  - Information that the activate invocation is early, i.e. before
    request_irq().

ca7c6076ba x86/ioapic: Refactor the delay logic in timer_irq_works()
timer_irq_works() is used to detects the timer IRQs. It calls mdelay(10) to
delay ten ticks and check whether the timer IRQ work or not.

mdelay() depends on the loops_per_jiffy which is set up in
calibrate_delay(), but the delay calibration depends on a working timer
interrupt, which causes a chicken and egg problem.

The correct solution is to set up the interrupt mode and making sure that
the timer interrupt is delivered correctly before invoking calibrate_delay().
That means that mdelay() cannot be used in timer_irq_works(). 

Provide helper functions to make a rough delay estimate which is good enough
to prove that the timer interrupt is working. Either use TSC or a simple
delay loop and assume that 4GHz is the maximum CPU frequency to base the
delay calculation on.

adfaf18334 x86/ioapic: Print the IRTE's index field correctly when enabling INTR
When enabling interrupt remap, IOAPIC's RTE contains the interrupt_index
field of IRTE. This field is composed of the ->index and the ->index2 members
of 'struct IR_IO_APIC_route_entry' - but what we print out currently only
uses ->index.

Fix it.

e708e35ba6 x86/ioapic: Pass the correct data to unmask_ioapic_irq()
One of the rarely executed code pathes in check_timer() calls
unmask_ioapic_irq() passing irq_get_chip_data(0) as argument.

That's wrong as unmask_ioapic_irq() expects a pointer to the irq data of
interrupt 0. irq_get_chip_data(0) returns NULL, so the following
dereference in unmask_ioapic_irq() causes a kernel panic.

The issue went unnoticed in the first place because irq_get_chip_data()
returns a void pointer so the compiler cannot do a type check on the
argument. The code path was added for machines with broken configuration,
but it seems that those machines are either not running current kernels or
simply do not longer exist.

Hand in irq_get_irq_data(0) as argument which provides the correct data.

d966564fcd Revert "x86/ioapic: Restore IO-APIC irq_chip retrigger callback"
This reverts commit 020eb3daab.

Gabriel C reports that it causes his machine to not boot, and we haven't
tracked down the reason for it yet.  Since the bug it fixes has been
around for a longish time, we're better off reverting the fix for now.

Gabriel says:
 "It hangs early and freezes with a lot RCU warnings.

  I bisected it down to :

  Reverting this one fixes the problem for me..

  The box is a PRIMERGY TX200 S5 , 2 socket , 2 x E5520 CPU(s) installed"

aaaec6fc75 x86/irq: Make irq activate operations symmetric
The recent commit which prevents double activation of interrupts unearthed
interesting code in x86. The code (ab)uses irq_domain_activate_irq() to
reconfigure an already activated interrupt. That trips over the prevention
code now.

Fix it by deactivating the interrupt before activating the new configuration.

020eb3daab x86/ioapic: Restore IO-APIC irq_chip retrigger callback
commit d32932d02e removed the irq_retrigger callback from the IO-APIC
chip and did not add it to the new IO-APIC-IR irq chip.

Unfortunately the software resend fallback is not enabled on X86, so edge
interrupts which are received during the lazy disabled state of the
interrupt line are not retriggered and therefor lost.

Restore the callbacks.

a9b4f08770 x86/ioapic: Restore IO-APIC irq_chip retrigger callback
commit d32932d02e removed the irq_retrigger callback from the IO-APIC
chip and did not add it to the new IO-APIC-IR irq chip.

There is no harm because the interrupts are resent in software when the
retrigger callback is NULL, but it's less efficient. So restore them.

cff9ab2b29 x86/apic: Get rid of apic_version[] array
The array has a size of MAX_LOCAL_APIC, which can be as large as 32k, so it
can consume up to 128k.

The array has been there forever and was never used for anything useful
other than a version mismatch check which was introduced in 2009.

There is no reason to store the version in an array. The kernel is not
prepared to handle different APIC versions anyway, so the real important
part is to detect a version mismatch and warn about it, which can be done
with a single variable as well.

186f43608a x86/kernel: Audit and remove any unnecessary uses of module.h
Historically a lot of these existed because we did not have
a distinction between what was modular code and what was providing
support to modules via EXPORT_SYMBOL and friends.  That changed
when we forked out support for the latter into the export.h file.

This means we should be able to reduce the usage of module.h
in code that is obj-y Makefile or bool Kconfig.  The advantage
in doing so is that module.h itself sources about 15 other headers;
adding significantly to what we feed cpp, and it can obscure what
headers we are effectively using.

Since module.h was the source for init.h (for __init) and for
export.h (for EXPORT_SYMBOL) we consider each obj-y/bool instance
for the presence of either and replace as needed.  Build testing
revealed some implicit header usage that was fixed up accordingly.

Note that some bool/obj-y instances remain since module.h is
the header for some exception table entry stuff, and for things
like __init_or_module (code that is tossed when MODULES=n).

9d98bcec73 x86/ioapic: Fix incorrect pointers in ioapic_setup_resources()
On a 4-socket Brickland system, hot-removing one ioapic is fine.
Hot-removing the 2nd one causes panic in mp_unregister_ioapic()
while calling release_resource().

It is because the iomem_res pointer has already been released
when removing the first ioapic.

To explain the use of &res[num] here: res is assigned to ioapic_resources,
and later in ioapic_insert_resources() we do:

	struct resource *r = ioapic_resources;

        for_each_ioapic(i) {
                insert_resource(&iomem_resource, r);
                r++;
        }

Here 'r' is treated as an arry of 'struct resource', and the r++ ensures
that each element of the array is inserted separately. Thus we should call
release_resouce() on each element at &res[num].

Fix it by assigning the correct pointers to ioapics[i].iomem_res in
ioapic_setup_resources().

e23b257c29 x86/irq: Call chip->irq_set_affinity in proper context
setup_ioapic_dest() calls irqchip->irq_set_affinity() completely
unprotected. That's wrong in several aspects:

 - it opens a race window where irq_set_affinity() can be interrupted and the
   irq chip left in unconsistent state.

 - it triggers a lockdep splat when we fix the vector race for 4.3+ because
   vector lock is taken with interrupts enabled.

The proper calling convention is irq descriptor lock held and interrupts
disabled.

ababae4410 x86/ioapic: Prevent NULL pointer dereference in setup_ioapic_dest()
Commit 4857c91f0d changed the way how irq affinity is setup in
setup_ioapic_dest() from using the core helper function to
unconditionally calling the irq_set_affinity() callback of the
underlying irq chip.

That results in a NULL pointer dereference for the rare case where the
underlying irq chip is lapic_chip which has no irq_set_affinity()
callback. lapic_chip is occasionally used for the timer interrupt (irq
0).

The fix is simple: Check the availability of the callback instead of
calling it unconditionally.

The issue is easily reproducible with a simple instrumentation: if
mdelay(10) is put between mp_setup_entry() and mp_register_handler() calls
in mp_irqdomain_alloc() Hyper-V guest always fails to boot when re-routing
IRQ0. The issue seems to be caused by the fact that we don't disable
interrupts while doing IOPIC programming for legacy IRQs and IRQ0 actually
happens. 

Protect the setup sequence against concurrent interrupts.

The recent ioapic cleanups changed the affinity setting in
setup_ioapic_dest() from a direct write to the hardware to the delayed
affinity setup via irq_set_affinity().

That results in a warning from chained_irq_exit():

The reason is that irq_set_affinity() does not write directly to the
hardware. It marks the affinity setting as pending and executes it
from the next interrupt. The chained handler infrastructure does not
take the irq descriptor lock for performance reasons because such a
chained interrupt is not visible to any interfaces. So the delayed
affinity setting triggers the warning in irq_move_masked_irq().

Restore the old behaviour by calling the set_affinity function of the
ioapic chip in setup_ioapic_dest(). This is safe as none of the
interrupts can be on the fly at this point.

646c4b7549 x86/irq: Use the caller provided polarity setting in mp_check_pin_attr()
Commit d32932d02e ("x86/irq: Convert IOAPIC to use hierarchical
irqdomain interfaces") introduced a regression which causes
malfunction of interrupt lines.

The reason is that the conversion of mp_check_pin_attr() missed to
update the polarity selection of the interrupt pin with the caller
provided setting and instead uses a stale attribute value. That in
turn results in chosing the wrong interrupt flow handler.

Use the caller supplied setting to configure the pin correctly which
also choses the correct interrupt flow handler.

This restores the original behaviour and on the affected
machine/driver (Surface Pro 3, i2c controller) all IOAPIC IRQ
configuration are identical to v4.1.

and sfi. Have a global one in the io_apic code and be done with it.

335efdf57d x86, ioapic: Use proper defines for the entry fields
While looking at the printout issue, I stumbled more than once over
the various 0/1 assignments which are either commented in strange ways
or force to lookup the meaning.

Use proper constants and fix the misleading comments. While at it
remove pointless 0 assignments in native_disable_io_apic() which have
no value for understanding the code.

9a93d4736e x86/irq: Remove x86_io_apic_ops.write and x86_io_apic_ops.modify
x86_io_apic_ops.write is always set to native_io_apic_write(),
and nobody overrides it. So get rid of the indirection by changing
native_io_apic_write() as io_apic_write() and removing
x86_io_apic_ops.write.

Do the same for x86_io_apic_ops.modify and native_io_apic_modify().

irqdomain for it to manage CPU interrupt vectors. It's the base to
enable hierarchical irqdomains on x86 systems.

This also means stubbing it out when !CONFIG_X86_IO_APIC was at least
questionable.

Set the IRQCHIP_SKIP_SET_WAKE for IOAPIC IRQ chip objects so that
interrupts from them can work as wakeup interrupts for suspend-to-idle.

After this change, running enable_irq_wake() on one of the IRQs in
question will succeed and IRQD_WAKEUP_STATE will be set for it, so
all of the suspend-to-idle wakeup mechanics introduced previously
will work for it automatically.

Now IOAPIC driver dynamically allocates IRQ numbers for IOAPIC pins.
We need to keep IRQ assignment for PCI devices during runtime power
management, otherwise it may cause failure of device wakeups.

Commit 3eec595235 "x86, irq, PCI: Keep IRQ assignment for PCI
devices during suspend/hibernation" has fixed the issue for suspend/
hibernation, we also need the same fix for runtime device sleep too.

f395dcae7a x86: irq: Fix bug in setting IOAPIC pin attributes
Commit 15a3c7cc91 "x86, irq: Introduce two helper functions
to support irqdomain map operation" breaks LPSS ACPI enumerated
devices.

On startup, IOAPIC driver preallocates IRQ descriptors and programs
IOAPIC pins with default level and polarity attributes for all legacy
IRQs. Later legacy IRQ users may fail to set IOAPIC pin attributes
if the requested attributes conflicts with the default IOAPIC pin
attributes. So change mp_irqdomain_map() to allow the first legacy IRQ
user to reprogram IOAPIC pin with different attributes.

Introduce function mp_unmap_irq() to release IOAPIC IRQ when IRQ is not
used any more, which will typically called by pcibios_disabled_irq.

And function mp_irqdomain_unmap() is a common implementation of
irq_domain_ops.unmap for IOAPIC.

16ee7b3dcc x86, irq: Simplify the way to handle ISA IRQ
On startup, setup_IO_APIC_irqs() will program all IOAPIC pins for ISA
IRQs. Later when mp_map_pin_to_irq() is called, it just returns ISA IRQ
number without programming corresponding IOAPIC pin.

This patch consolidates the way to program IOAPIC pins for both ISA and
non-ISA IRQs into mp_map_pin_to_irq() as below:
1) For ISA IRQs, mp_irqs array is used to map IOAPIC pin to IRQ and
   mp_irqdomain_map() is used to actually program the pin.
2) For non-ISA IRQs, irqdomain is used to map IOAPIC pin to IRQ, and
   mp_irqdomain_map() is also used to actually program the pin.

15a3c7cc91 x86, irq: Introduce two helper functions to support irqdomain map operation
Currently there are multiple entries to program IOAPIC pins, such as
io_apic_setup_irq_pin_once(), io_apic_set_pci_routing() and
setup_IO_APIC_irq_extra() etc.

This patch introduces two functions to help consolidate the code to
program IOAPIC pins. Function mp_set_pin_attr() is used to optionally
set trigger, polarity and NUMA node property for an IOAPIC pin.
If mp_set_pin_attr() is not invoked for a pin, the default configuration
from BIOS will be used.

Function mp_irqdomain_map() is an common implementation of irqdomain map()
operation. It figures out attribures for pin and then actually programs
the IOAPIC pin. We hope this will be the only entrance for programming
IOAPIC pin.

And the flow will:
1) caller such as xxx_pci_irq_enable figures out pin attributes.
2) Invoke mp_set_pin_attr() to set attributes for a pin. If the pin has
   already bin programmed,  mp_set_pin_attr() will aslo detects attribute
   confictions.
3) Invoke mp_map_pin_to_irq()
3.1) If IRQ has already been assigned, return irq_find_mapping()
3.2) Else irq_create_mapping()
		->irq_domain_associate()
			->mp_irqdomain_map()
				->io_apic_setup_irq_pin()

So every pin will only programmed once by mp_irqdomain_map(), so we
could kill io_apic_setup_irq_pin_once(), io_apic_set_pci_routing() and
setup_IO_APIC_irq_extra() etc.

d7f3d47818 x86, irq: Introduce mechanisms to support dynamically allocate IRQ for IOAPIC
Currently x86 support identity mapping between GSI(IOAPIC pin) and IRQ
number, so continous IRQs at low end are statically allocated to IOAPICs
at boot time. This design causes trouble to support IOAPIC hotplug.

This patch implements basic mechanism to dynamically allocate IRQ on
demand for IOAPIC pins by using irqdomain framework.

It first adds several fields into struct ioapic to support irqdomain.
Then it implements an algorithm to dynamically allocate IRQ number
for IOAPIC pins on demand.

Currently it supports three types of irqdomain:
1) LEGACY: used to support IOAPIC hosting legacy IRQs and building
   identity mapping for legacy IRQs. A speical case, we dynamically
   allocate IRQ number for IOAPIC pin which has GSI number below
   nr_legacy_irqs() but isn't legacy IRQ. This is for backward
   compatibility and avoid regression.
2) STRICT: build identity mapping between GSI and IRQ nubmer.
3) DYNAMIC: dynamically allocate IRQ number for IOAPIC pin on demand.

Legacy(ISA) IRQs is not managed by irqdomain because there may be
multiple pins sharing the same IRQ number and current irqdomain only
supports 1:1 mapping between pins and IRQ.

Currently ACPI and ioapic both implement algorithms to map (ioapic, pin)
to IRQ number. So consolidate the common part into one place, which is
also preparing for irqdomain support.

It introduces mp_map_gsi_to_irq(), which will be used to allocate IRQ
number IOAPIC pins when irqdomain is enabled.

Also rename gsi_to_irq() to map_gsi_to_irq(), later we will introduce
unmap_gsi_to_irq() when enabling IOAPIC hotplug.
