phy: qcom-qmp-combo: shorten function prefixes

The driver function prefix has gotten unnecessarily long and hurts
readability.

Shorten "qcom_qmp_phy_" to "qmp_" (which likely stands for "Qualcomm
Multi PHY" or similar anyway).
