sound fixes for 3.12-rc5

All stable fixes except for a trivial headset mic fixup:
 the removal of bogus frame checks in snd-usb-usx2y driver that have
 regressed in the recent kernel versions, the HD-audio HDMI channel
 map fix, and a few HD-audio device-specific fixes.
