Denys Vlasenko f2db9382c1 x86/asm/entry: Do mass removal of 'ARGOFFSET'
ARGOFFSET is zero now, removing it changes no code.

A few macros lost "offset" parameter, since it is always zero
now too.

No code changes - verified with objdump.
