source files.
Finally remove the now redundant LICENSE.qla4xxx file.

This patch force driver to collect firmware dump using following command:-
"echo 2 > /sys/class/scsi_host/hostX/device/fw_dump"

Added support to capture dump (Minidump) which allows us to
catpure a snapshot of the firmware/hardware states at the time
of firmware failure
