b59f45d0b2 [IPSEC] xfrm: Abstract out encapsulation modes
This patch adds the structure xfrm_mode.  It is meant to represent
the operations carried out by transport/tunnel modes.

By doing this we allow additional encapsulation modes to be added
without clogging up the xfrm_input/xfrm_output paths.

Candidate modes include 4-to-6 tunnel mode, 6-to-4 tunnel mode, and
BEET modes.

546be2405b [IPSEC] xfrm: Undo afinfo lock proliferation
The number of locks used to manage afinfo structures can easily be reduced
down to one each for policy and state respectively.  This is based on the
observation that the write locks are only held by module insertion/removal
which are very rare events so there is no need to further differentiate
between the insertion of modules like ipv6 versus esp6.

The removal of the read locks in xfrm4_policy.c/xfrm6_policy.c might look
suspicious at first.  However, after you realise that nobody ever takes
the corresponding write lock you'll feel better :)

As far as I can gather it's an attempt to guard against the removal of
the corresponding modules.  Since neither module can be unloaded at all
we can leave it to whoever fixes up IPv6 unloading :)

9ead190bfd IB/uverbs: Don't serialize with ib_uverbs_idr_mutex
Currently, all userspace verbs operations that call into the kernel
are serialized by ib_uverbs_idr_mutex.  This can be a scalability
issue for some workloads, especially for devices driven by the ipath
driver, which needs to call into the kernel even for datapath
operations.

Fix this by adding reference counts to the userspace objects, and then
converting ib_uverbs_idr_mutex into a spinlock that only protects the
idrs long enough to take a reference on the object being looked up.
Because remove operations may fail, we have to do a slightly funky
two-step deletion, which is described in the comments at the top of
uverbs_cmd.c.

This also still leaves ib_uverbs_idr_lock as a single lock that is
possibly subject to contention.  However, the lock hold time will only
be a single idr operation, so multiple threads should still be able to
make progress, even if ib_uverbs_idr_lock is being ping-ponged.

Surprisingly, these changes even shrink the object code:

add/remove: 23/5 grow/shrink: 4/21 up/down: 633/-693 (-60)

75af908851 IB/ucm: Get rid of duplicate P_Key parameter
The P_Key is provided into a SIDR REQ in two places, once as a
parameter, and again in the path record.  Remove the P_Key as a
parameter and always use the one given in the path record.

This change has no practical effect on ABI functionality.

e51060f08a IB: IP address based RDMA connection manager
Kernel connection management agent over InfiniBand that connects based
on IP addresses.  The agent defines a generic RDMA connection
abstraction to support clients wanting to connect over different RDMA
devices.

The agent also handles RDMA device hotplug events on behalf of clients.

8d7feac3c7 [SCSI] remove RQ_SCSI_* flags
The RQ_SCSI_* flags are a vestiage of a long past history.  The EH code
still sets them but we never make use of that information.  The other
users is pluto.c which never had a chance to work but needs to be kept
compiling to keep Davem happy, so copy over the definition there.

We could probably get rid of RQ_ACTIVE/RQ_INACTIVE aswell with some
work, there's only two more or less bogus looking uses in ubd and scsi.

bc1c116974 [PATCH] elevator switching race
There's a race between shutting down one io scheduler and firing up the
next, in which a new io could enter and cause the io scheduler to be
invoked with bad or NULL data.

To fix this, we need to maintain the queue lock for a bit longer.
Unfortunately we cannot do that, since the elevator init requires to be
run without the lock held.  This isn't easily fixable, without also
changing the mempool API.  So split the initialization into two parts,
and alloc-init operation and an attach operation.  Then we can
preallocate the io scheduler and related structures, and run the attach
inside the lock after we detach the old one.

This patch has survived 30 minutes of 1 second io scheduler switching
with a very busy io load.

b1c231f5a5 [MIPS] Fix sparsemem support.
Move memory_present() in arch/mips/kernel/setup.c. When using sparsemem
extreme, this function does an allocate for bootmem. This would always
fail since init_bootmem hasn't been called yet.
    
Move memory_present after free_bootmem. This only marks actual memory
ranges as present instead of the entire address space.

7cb710c9a6 [MIPS] Fix non-linear memory mapping on MIPS
Fix the non-linear memory mapping done via remap_file_pages() -- it
didn't work on any MIPS CPU because the page offset clashing with
_PAGE_FILE and some other page protection bits which should have been left

6ebba0e2f5 [MIPS] Fix swap entry for MIPS32 36-bit physical address
With 64-bit physical address enabled, 'swapon' was causing kernel oops on
Alchemy CPUs (MIPS32) because of the swap entry type field corrupting the
_PAGE_FILE bit in 'pte_low' field. So, switch to storing the swap entry in
'pte_high' field using all its bits except _PAGE_GLOBAL and _PAGE_VALID which
gives 25 bits for the swap entry offset.

3219e52941 [SCSI] iscsi: fix writepsace race
We can race and misset the suspend bit if iscsi_write_space is
called then iscsi_send returns with a failure indicating
there is no space.

To handle this this patch returns a error upwards allowing xmitworker
to decide if we need to try and transmit again. For the no
write space case xmitworker will not retry, and instead
let iscsi_write_space queue it back up if needed (this relies
on the work queue code to properly requeue us if needed).

67a611149b [SCSI] iscsi: don't switch states when just cleaning up
If recovery failed or we are in recovery only overwrite the state
if we are going to terminate the session or if we logged back in.

STOP_CONN_SUSPEND and conn_cnt are not used. We only support
a single connection session ATM, so cleanup that code while
we are working around it.

6ae15df16e [PATCH] softmac: Fix handling of authentication failure
My router blew up earlier, but exhibited some interesting behaviour during
its dying moments. It was broadcasting beacons but wouldn't respond to
any authentication requests.

I noticed that softmac wasn't playing nice with this, as I couldn't make it try
to connect to other networks after it had timed out authenticating to my ill
router.

To resolve this, I modified the softmac event/notify API to pass the event
code to the callback, so that callbacks being notified from
IEEE80211SOFTMAC_EVENT_ANY masks can make some judgement. In this case, the
ieee80211softmac_assoc callback needs to make a decision based upon whether
the association passed or failed.

76ea4c7f4c [PATCH] softmac: complete shared key authentication
This patch finishes of the partially-complete shared key authentication
implementation in softmac.

The complication here is that we need to encrypt a management frame during
the authentication process. I don't think there are any other scenarios where
this would have to happen.

To get around this without causing too many headaches, we decided to just use
software encryption for this frame. The softmac config option now selects
IEEE80211_CRYPT_WEP so that we can ensure this available. This also involved
a modification to some otherwise unused ieee80211 API.

After removal of fixup_cpu_present_map() function Alpha ended up with an empty
cpu_present_map, so secondary CPUs on SMP systems are not being started.

Worse, on some platforms we route interrupts to secondary CPUs using
cpu_possible_map which is still populated properly.  As a result, these
interrupts go nowhere so the machines like DP264 aren't able to boot even with
a primary CPU.

Fixed basically by s/cpu_present_mask/cpu_present_map/.

Thanks to Ernst Herzberg for reporting the bug and testing the fix.

The current __ixp23xx_arch_is_coherent() check assumes that the
lower byte of IXP23XX_PRODUCT_ID is identical to the lower byte of
processor_id, but this is not the case, and because of this we were
incorrectly enabling coherency on A1 stepping CPUs.

Stepping A1 of the ixp2350, which has a PRODUCT_ID of 0x401, has '02'
in the lower byte of processor_id, while A2, with a PRODUCT_ID of
0x402, has '04' in the lower byte of processor_id.

So, to check for >= A2, we really need to check the lower byte of
processor_id against >= 4.

If we move a mapping from one virtual address to another,
and this changes the virtual color of the mapping to those
pages, we can see corrupt data due to D-cache aliasing.

Check for and deal with this by overriding the move_pte()
macro.  Set things up so that other platforms can cleanly
override the move_pte() macro too.

6e9538917c [MIPS] Fix marking buddy of pte global for MIPS32 w/36-bit physical address
In case of CONFIG_64BIT_PHYS_ADDR, set_pte() and pte_clear() functions
only set _PAGE_GLOBAL bit in the pte_low field of the buddy PTEs,
forgetting to propagate ito to pte_high. Thus, the both pages might not
really be made global for the CPU (since it AND's the G-bit of the
odd / even PTEs together to decide whether they're global or not). Thus,
if only a single page is allocated via vmalloc() or ioremap(), it's not
really global for CPU (and it must be, since this is kernel mapping),
and thus its ASID is compared against the current process' one -- so,
we'll get into trouble sooner or later...  Also, pte_none() will fail
on global pages because _PAGE_GLOBAL bit is set in both pte_low and
pte_high, and pte_val() will return u64 value consisting of those fields
concateneted.

6ae53cd496 [PATCH] x86_64: Fix stack/mmap randomization for compat tasks
ia32_setup_arg_pages would ignore the passed in random stack top
and use its own static value.

Now it uses the 8bit of randomness native i386 would use too.

This indirectly fixes mmap randomization for 32bit processes too,
which depends on the stack randomization.

Should also give slightly better virtual cache colouring and
possibly better performance with HyperThreading.

f1a28c0284 [MTD] NAND Expose the new raw mode function and status info to userspace
The raw read/write access to NAND (without ECC) has been changed in the
NAND rework. Expose the new way - setting the file mode via ioctl - to
userspace. Also allow to read out the ecc statistics information so userspace
tools can see that bitflips happened and whether errors where correctable
or not. Also expose the number of bad blocks for the partition, so nandwrite
can check if the data fits into the parition before writing to it.

8593fbc68b [MTD] Rework the out of band handling completely
Hopefully the last iteration on this!

The handling of out of band data on NAND was accompanied by tons of fruitless
discussions and halfarsed patches to make it work for a particular
problem. Sufficiently annoyed by I all those "I know it better" mails and the
resonable amount of discarded "it solves my problem" patches, I finally decided
to go for the big rework. After removing the _ecc variants of mtd read/write
functions the solution to satisfy the various requirements was to refactor the
read/write _oob functions in mtd.

The major change is that read/write_oob now takes a pointer to an operation
descriptor structure "struct mtd_oob_ops".instead of having a function with at
least seven arguments.

read/write_oob which should probably renamed to a more descriptive name, can do
the following tasks:

- read/write out of band data
- read/write data content and out of band data
- read/write raw data content and out of band data (ecc disabled)

struct mtd_oob_ops has a mode field, which determines the oob handling mode.

Aside of the MTD_OOB_RAW mode, which is intended to be especially for
diagnostic purposes and some internal functions e.g. bad block table creation,
the other two modes are for mtd clients:

MTD_OOB_PLACE puts/gets the given oob data exactly to/from the place which is
described by the ooboffs and ooblen fields of the mtd_oob_ops strcuture. It's
up to the caller to make sure that the byte positions are not used by the ECC
placement algorithms.

MTD_OOB_AUTO puts/gets the given oob data automaticaly to/from the places in
the out of band area which are described by the oobfree tuples in the ecclayout
data structre which is associated to the devicee.

The decision whether data plus oob or oob only handling is done depends on the
setting of the datbuf member of the data structure. When datbuf == NULL then
the internal read/write_oob functions are selected, otherwise the read/write
data routines are invoked.

Tested on a few platforms with all variants. Please be aware of possible
regressions for your particular device / application scenario

Disclaimer: Any whining will be ignored from those who just contributed "hot
air blurb" and never sat down to tackle the underlying problem of the mess in
the NAND driver grown over time and the big chunk of work to fix up the
existing users. The problem was not the holiness of the existing MTD
interfaces. The problems was the lack of time to go for the big overhaul. It's
easy to add more mess to the existing one, but it takes alot of effort to go
for a real solution.

Improvements and bugfixes are welcome!

The info structure for out of band data was copied into
the mtd structure. Make it a pointer and remove the ability
to set it from userspace. The position of ecc bytes is
defined by the hardware and should not be changed by software.

Add read/write function pointers to struct nand_ecc_ctrl to
prepare the modulaization of nand_read/write functions. The
current implementation handles every type of ecc mode
software/hardware and all kinds of strange ecc placement
schemes in one switch/if construct. Thats too complex to
maintain and too inflexible to expand. Modularization will
also shorten the code pathes of the read/write functions.

7a30601b3a [MTD] NAND Introduce NAND_NO_READRDY option
The nand driver has a superflous read ready / command
delay in the read functions. This was added to handle
chips which have an automatic read forward. Newer
chips do not have this functionality anymore. Add this
option to avoid the delay / I/O operation. Mark all
large page chips with the new option flag.

7abd3ef987 [MTD] Refactor NAND hwcontrol to cmd_ctrl
The hwcontrol function enforced a step by step state machine
for any kind of hardware chip access. Let the hardware driver
know which control bits are set and inform it about a change
of the control lines. Let the hardware driver write out the
command and address bytes directly. This gives a peformance
advantage for address bus controlled chips and simplifies the
quirks in the hardware drivers.

42f142371e [SPARC64]: Respect gfp_t argument to dma_alloc_coherent().
Using asm-generic/dma-mapping.h does not work because pushing
the call down to pci_alloc_coherent() causes the gfp_t argument
of dma_alloc_coherent() to be ignored.

Fix this by implementing things directly, and adding a gfp_t
argument we can use in the internal call down to the PCI DMA
implementation of pci_alloc_coherent().

This fixes massive memory corruption when using the sound driver
layer, which passes things like __GFP_COMP down into these
routines and (correctly) expects that to work.

5fa433942b [MTD] Introduce MTD_BIT_WRITEABLE
o Add a flag MTD_BIT_WRITEABLE for devices that allow single bits to be
  cleared.
o Replace MTD_PROGRAM_REGIONS with a cleared MTD_BIT_WRITEABLE flag for
  STMicro and Intel Sibley flashes with internal ECC.  Those flashes
  disallow clearing of single bits, unlike regular NOR flashes, so the
  new flag models their behaviour better.
o Remove MTD_ECC.  After the STMicro/Sibley merge, this flag is only set
  and never checked.

Two flags exist to decide whether a device is writeable or not.  None of
those two flags is checked for independently, so they are clearly redundant,
if not an invitation to bugs.  This patch removed both of them, replacing
them with a single new flag.

e984bb43f7 [PATCH] Align the node_mem_map endpoints to a MAX_ORDER boundary
Andy added code to buddy allocator which does not require the zone's
endpoints to be aligned to MAX_ORDER.  An issue is that the buddy allocator
requires the node_mem_map's endpoints to be MAX_ORDER aligned.  Otherwise
__page_find_buddy could compute a buddy not in node_mem_map for partial
MAX_ORDER regions at zone's endpoints.  page_is_buddy will detect that
these pages at endpoints are not PG_buddy (they were zeroed out by bootmem
allocator and not part of zone).  Of course the negative here is we could
waste a little memory but the positive is eliminating all the old checks
for zone boundary conditions.

SPARSEMEM won't encounter this issue because of MAX_ORDER size constraint
when SPARSEMEM is configured.  ia64 VIRTUAL_MEM_MAP doesn't need the logic
either because the holes and endpoints are handled differently.  This
leaves checking alloc_remap and other arches which privately allocate for
node_mem_map.

PXA270:

- Wrong timeout calculation in the setup function due to different
  peripheral clock rates in the PXAxxx family.

- Bad handling of SSSR_TFS interrupts in interrupt_transfer function.

- Added locking to interface between the pump_messages workqueue and the
  pump_transfers tasklet.

656cffc95f [SCSI] iscsi: fix command requeues during iscsi recovery
Do not flush queues then block session. This will cause commands
to needlessly swing around on us and remove goofy
recovery_failed field and replace with state value.

And do not start recovery from within the host reset function.
This causeis too many problems becuase open-iscsi was desinged to
call out to userspace then have userpscae decide if we should
go into recovery or kill the session.

There has been problems that for some paths that clock are not stopped
during new command programming and initiation. Result is issuing
of incorrect command to the card. Some other problems are cleaned too.

dd2d1c6f29 [SCTP]: Respect the real chunk length when walking parameters.
When performing bound checks during the parameter processing, we
want to use the real chunk and paramter lengths for bounds instead
of the rounded ones.  This prevents us from potentially walking of
the end if the chunk length was miscalculated.  We still use rounded
lengths when advancing the pointer. This was found during a
conformance test that changed the chunk length without modifying
parameters.

aef9ab4784 [JFFS2] Support new device nodes
Device node major/minor numbers are just stored in the payload of a single
data node. Just extend that to 4 bytes and use new_encode_dev() for it.

We only use the 4-byte format if we _need_ to, if !old_valid_dev(foo).
This preserves backwards compatibility with older code as much as
possible. If we do make devices with major or minor numbers above 255, and
then mount the file system with the old code, it'll just read the first
two bytes and get the numbers wrong. If it comes to garbage-collect it,
it'll then write back those wrong numbers. But that's about the best we
can expect.

ba9627b85f [JFFS2] Repack some on-medium structures. ARM is weirder than I thought.
We have to pack at least the jint16_t structure, because otherwise it'll
be four bytes in size. Thankfully, we can do that and _not_ pack the
actual node structures, and the compiler still doesn't emit stupid code.

We need to be able to have a "SPI bus 0" matching chip numbering; but
that number was wrongly used to flag dynamic allocation of a bus number.

This patch resolves that issue; now negative numbers trigger dynamic alloc.

It also updates the how-to-write-a-controller-driver overview to mention
this stuff.

4cff33f94f [PATCH] SPI: per-transfer overrides for wordsize and clocking
Some protocols (like one for some bitmap displays) require different clock
speed or word size settings for each transfer in an SPI message. This adds
those parameters to struct spi_transfer.  They are to be used when they are
nonzero; otherwise the defaults from spi_device are to be used.

The patch also adds a setup_transfer callback to spi_bitbang, uses it for
messages that use those overrides, and implements it so that the pure
bitbanging code can help resolve any questions about how it should work.

e6333fd4dd [PATCH] fix can_share_swap_page() when !CONFIG_SWAP
can_share_swap_page() is used to check if the page has the last reference.
This avoids allocating a new page for COW if it's the last page.

However, if CONFIG_SWAP is not set, can_share_swap_page() is defined as 0,
thus always causes a copy for the last COW page.  The below simple patch
fixes it.

The function symbol_put_addr() will deadlock the kernel.

symbol_put_addr() would acquire modlist_lock, then while holding the lock call
two functions kernel_text_address() and module_text_address() which also try
to acquire the same lock.  This deadlocks the kernel of course.

This patch changes symbol_put_addr() to not acquire the modlist_lock, it
doesn't need it since it never looks at the module list directly.  Also, it
now uses core_kernel_text() instead of kernel_text_address().  The latter has
an additional check for addr inside a module, but we don't need to do that
since we call module_text_address() (the same function kernel_text_address
uses) ourselves.

3e68fbb59b [JFFS2] Don't pack on-medium structures, because GCC emits crappy code
If we use __attribute__((packed)), GCC will _also_ assume that the
structures aren't sensibly aligned, and it'll emit code to cope with
that instead of straight word load/save. This can be _very_ suboptimal
on architectures like ARM.

Ideally, we want an attribute which just tells GCC not to do any
padding, without the alignment side-effects. In the absense of that,
we'll just drop the 'packed' attribute and hope that everything stays as
it was (which to be fair is fairly much what we expect). And add some
paranoia checks in the initialisation code, which should be optimised
away completely in the normal case.

aa98d7cf59 [JFFS2][XATTR] XATTR support on JFFS2 (version. 5)
This attached patches provide xattr support including POSIX-ACL and
SELinux support on JFFS2 (version.5).

There are some significant differences from previous version posted
at last December.
The biggest change is addition of EBS(Erase Block Summary) support.
Currently, both kernel and usermode utility (sumtool) can recognize
xattr nodes which have JFFS2_NODETYPE_XATTR/_XREF nodetype.

In addition, some bugs are fixed.
- A potential race condition was fixed.
- Unexpected fail when updating a xattr by same name/value pair was fixed.
- A bug when removing xattr name/value pair was fixed.

The fundamental structures (such as using two new nodetypes and exclusion
mechanism by rwsem) are unchanged. But most of implementation were reviewed
and updated if necessary.
Espacially, we had to change several internal implementations related to
load_xattr_datum() to avoid a potential race condition.

bd89efc532 [NEIGH]: Fix IP-over-ATM and ARP interaction.
The classical IP over ATM code maintains its own IPv4 <-> <ATM stuff>
ARP table, using the standard neighbour-table code. The
neigh_table_init function adds this neighbour table to a linked list
of all neighbor tables which is used by the functions neigh_delete()
neigh_add() and neightbl_set(), all called by the netlink code.

Once the ATM neighbour table is added to the list, there are two
tables with family == AF_INET there, and ARP entries sent via netlink
go into the first table with matching family. This is indeterminate
and often wrong.

To see the bug, on a kernel with CLIP enabled, create a standard IPv4
ARP entry by pinging an unused address on a local subnet. Then attempt
to complete that entry by doing

ip neigh replace <ip address> lladdr <some mac address> nud reachable

Looking at the ARP tables by using 

ip neigh show

will reveal two ARP entries for the same address. One of these can be
found in /proc/net/arp, and the other in /proc/net/atm/arp.

This patch adds a new function, neigh_table_init_no_netlink() which
does everything the neigh_table_init() does, except add the table to
the netlink all-arp-tables chain. In addition neigh_table_init() has a
check that all tables on the chain have a distinct address family.
The init call in clip.c is changed to call
neigh_table_init_no_netlink().

Since ATM ARP tables are rather more complicated than can currently be
handled by the available rtattrs in the netlink protocol, no
functionality is lost by this patch, and non-ATM ARP manipulation via
netlink is rescued. A more complete solution would involve a rtattr
for ATM ARP entries and some way for the netlink code to give
neigh_add and friends more information than just address family with
which to find the correct ARP table.

[ I've changed the assertion checking in neigh_table_init() to not
  use BUG_ON() while holding neigh_tbl_lock.  Instead we remember that
  we found an existing tbl with the same family, and after dropping
  the lock we'll give a diagnostic kernel log message and a stack dump.

One Block of the NAND Flash Array memory is reserved as
a One-Time Programmable Block memory area.
Also, 1st Block of NAND Flash Array can be used as OTP.

The OTP block can be read, programmed and locked using the same
operations as any other NAND Flash Array memory block.
OTP block cannot be erased.

OTP block is fully-guaranteed to be a valid block.

The last step of netdevice registration was being done by a delayed
call, but because it was delayed, it was impossible to return any error
code if the class_device registration failed.

Side effects:
 * one state in registration process is unnecessary.
 * register_netdevice can sleep inside class_device registration/hotplug
 * code in netdev_run_todo only does unregistration so it is simpler.

debugged by Ming and Rohan:

The problem Ming and Rohan debugged was that during a normal session
login, open-iscsi is not incrementing the exp_statsn counter. It was
stuck at zero. From the RFC, it looks like if the login response PDU has
a successful status then we should be incrementing that value. Also from
the RFC, it looks like if when we drop a connection then reconnect, we
should be using the exp_statsn from the old connection in the next
relogin attempt.

e4de00215c powerpc/32: Define an is_kernel_addr() to fix ARCH=ppc compilation
My commit 6bfd93c32a broke the ARCH=ppc
compilation by using the is_kernel_addr() macro in asm/uaccess.h.
This fixes it by defining a suitable is_kernel_addr() for ARCH=ppc.

ac71d12c99 [PATCH] x86_64: Avoid EBDA area in early boot allocator
Based on analysis&patch from Robert Hentosch

Observed on a Dell PE6850 with 16GB

The problem occurs very early on, when the kernel allocates space for the
temporary memory map called bootmap. The bootmap overlaps the EBDA region.
EBDA region is not historically reserved in the e820 mapping. When the
bootmap is freed it marks the EBDA region as usable.

If you notice in setup.c there is already code to work around the EBDA
in reserve_ebda_region(), this check however occurs after the bootmap
is allocated and doesn't prevent the bootmap from using this range.

spotting some mistakes.

e0c1e9bf81 [PATCH] x86_64: avoid IRQ0 ioapic pin collision
The patch addresses a problem with ACPI SCI interrupt entry, which gets
re-used, and the IRQ is assigned to another unrelated device.  The patch
corrects the code such that SCI IRQ is skipped and duplicate entry is
avoided.  Second issue came up with VIA chipset, the problem was caused by
original patch assigning IRQs starting 16 and up.  The VIA chipset uses
4-bit IRQ register for internal interrupt routing, and therefore cannot
handle IRQ numbers assigned to its devices.  The patch corrects this
problem by allowing PCI IRQs below 16.
